Abstract:Objective To present the modified surgical technique of lateral lumbar interbody fusion and investigate its approach related complications.Methods Fifty-eight patients treated with novel surgical technique of lateral lumbar interbody fusion(LLIF)from June 2016 to January 2017 were studied retrospectively.There were 24 males and 34 females,aged from 45 to 82 years old(averaged at 66.1±12.1).The diagnosis was as following:degenerative spinal scoliosis in 24 cases,degenerative lum-bar spinal stenosis in 16,lumbar spinal spondylolisthesis in 14,spinal infection and spinal trauma in 2 respectively.The modified surgical technique included operation under direct visualization,the"safety"transpsoas approach and the adjustable microretrac-tor.The patient was placed in a lateral decubitus position,and a transverse or oblique skin incision was planned using fluoroscopy to target the center of the desired disc spaces.The 3 abdominal layers were split longitudinally along the muscle fiber.The psoas major was split longitudinally along the muscle fiber according to the"safety"working window.The adjustable microretractor was used as working corridor to carry out interbody fusion. Oswestry disability index (ODI) was used to assess the clinical outcome. The surgical time,estimated blood loss and approach related complications(pain/numbness at anterior thigh,weakness of psoas muscle and quadriceps)were recorded.Results All patients were followed up from 6-12months,the mean follow-up time(9.1± 2.3)months.A total of 132 levels were performed with novel surgical technique of LLIF,one level in 19 cases,2 levels in 10,3 lev-els in 23 and 4 levels in 6 cases.The mean surgical time was(57.2±13.3)min for 1 level,(94.5±31.3)min for 2 levels,(129.8± 42.1)min for 3 levels and(208.3±22.7)min for 4 levels.The estimated blood loss during surgery was less than 10ml every level. Preoperative ODI was 61.8%±20.1%,it was 22.5%±18.3%(t=7.572,P=0.000)at the last time follow-up.The incidence rate of ap-proach related complications was 12.1%(7/58),with anterior thigh pain in 6 cases(10.3%),numbness 5 cases(8.6%)and psoas major weakness 3 cases(5.2%).No quadriceps weakness,vascular injury,sympathetic nerve injury,visceral injury and ureteral in-jury was found in these series.The incidence rate of complications increased significantly in patients underwent three or more lev-els interbody fusion(χ2=4.453,P=0.035).Conclusion The modified surgical technique reduces the approach related complica-tions of traditional lateral lumbar interbody fusion through the operation under the direct visualization, the"safety"transpsoas approach and the adjustable microretractor.

Abstract:Background: The aim of this study was to compare surgical trauma and radiographic and clinical outcomes of stand-alone and instrumented lateral lumbar interbody fusion (LLIF) in the treatment of single-level low-grade lumbar spondylolisthesis. Methods: Ninety-five patients with single-level low-grade lumbar spondylolisthesis, who underwent stand-alone LLIF (stand-alone group, [n = 54]) or LLIF plus percutaneous posterior fixation (instrumented group, [n = 41]) were enrolled in this study. Operative time, intraoperative blood loss, serum C-reactive protein (CRP) and creatine kinase (CK) levels, the length of postoperative bed rest time, and hospital stay were compared between the 2 groups. Disc height, the percent of slip, segment lordosis, lumbar lordosis, the visual analog scale score, the Oswestry Disability Index and complications were also compared. Results: Operative and bed rest time were shorter, intraoperative blood loss was less, and postoperative CRP and CK levels were lower in the stand-alone group. During follow-up, 6 patients in stand-alone group underwent posterior fixation due to cage subsidence. Although satisfactory radiographic results were achieved in both groups, the maintenance of increased disc heights and segment lordosis was inferior in the stand-alone group at the final follow-up. Greater improvement in postoperative VAS scores and ODI were observed in the stand-alone group, although the rates of cage subsidence and revision were higher. Conclusion: Stand-alone LLIF was superior to instrumented LLIF in terms of tissue trauma for the treatment of single-level low-grade lumbar spondylolisthesis. However, stand-alone LLIF was inferior in the maintenance of disc height and segment lordosis, and the occurrence of cage subsidence and revision. (C) 2020 Elsevier Ltd. All rights reserved.
Lateral lumbar interbody fusion for severe lumbar spinal stenosis: a randomized controlled trial with 1-year follow-up

Abstract:Objective:To investigate the clinical outcomes of minimally invasive lateral lumbar interbody fusion (LLIF) and the necessity to perform LLIF plus posterior direct decompression in the treatment of severe degenerative lumbar spinal stenosis (DLSS).Methods:In this prospective randomized, controlled trial, we assigned 71 patients, who were 50 to 80 years old, and diagnosed with severe DLSS (Schizas Classification grade C on magnetic resonance imaging), in a 1∶1 ratio to undergo either one-stage LLIF plus posterior internal fixation (treatment group) or CLIF plus posterior internal fixation with laminectomy (control group). Demographic and perioperative data were collected and compared. The clinical outcome measures included Oswestry Disability Index (ODI), Zurich Claudication Questionnaire (ZCQ) score as well as visual analogue scale (VAS). Patients were followed up for at least 1 year.Results:The treatment group included 36 patients with 46 surgical levels, while the control group included 35 patients with 46 surgical levels. The baseline demographic data of the 2 groups were equivalent in preoperative central canal areas, spinal canal anteroposterior diameter, disc height, ODI, ZCQ score for symptom severity and physical function, as well as VAS scores for back and leg pain. The mean operative time, blood loss, drainage volume and hospital stay of the treatment group are significantly less than the control group (157.2±29.1 min vs. 180.6±26.8 min, 75.6±39.1 ml vs. 108.6±43.3 ml, 136.9±73.9 ml vs. 220.5±121.3 ml, 5.3±1.1 d vs. 6.6±2.3 d). There were 2 cases with dura tear and 1 case with wound infection in control group. Thus, the surgical trauma and complications of the control group were more than the treatment group. At 1-year follow-up, the mean ODI score of treatment group improved from 42.24%±10.70% preoperatively to 18.21%±11.49%, the mean ZCQ symptom severity from 2.89±0.38 to 1.61±0.41, the mean ZCQ physical function from 2.31±0.45 to 1.50±0.37, the mean VAS for back from 5.56±1.19 to 1.97±1.13 and the mean VAS for leg from 4.44±1.81 to 0.94±1.26. At 1-year follow-up, the mean ODI score of the control group improved from 43.65%±14.93% preoperatively to 17.36%±12.15%, the mean ZCQ symptom severity from 2.92±0.52 to 1.65±0.39, the mean ZCQ physical function from 2.37±0.52 to 1.55±0.39, the mean VAS for back from 5.63±1.40 to 2.34±1.47, and the mean VAS for leg from 4.37±2.14 to 0.83±1.20. The ZCQ satisfactory score of both groups were not significant different (1.25±0.45 vs. 1.26±0.43, t=0.07, P=0.944). The mean improvement rate of both groups for ODI, ZCQ symptom severity, ZCQ physical function, VAS back and VAS leg at 1-year follow-up were not significant different (55.43%±27.74% vs. 58.36%±25.06%, 43.07%±17.22% vs. 42.66%±12.95%, 32.25%±23.65% vs. 31.71%±23.24%, 62.65%±21.25% vs. 58.37%±22.44%, 78.94%±26.41% vs. 85.45%±20.53%). One adjacent segment disease was found in each group at 1 year follow-up. Conclusion:CLIF+ posterior internal fixation in the treatment of Schizas Grade C DLSS has satisfactory clinical outcome at 1-year follow-up. Laminectomy increases surgical trauma, but does not significantly improve the clinical outcome at 1-year follow-up.
Oblique Lateral Interbody Fusion Combined With Lateral Plate Fixation for the Treatment of Lumbar Degenerative Diseases: a Preliminary Clinical Study

Abstract:Abstract Background The oblique lateral interbody fusion (OLIF) is a minimally invasive indirect decompression technique for the treatment of lumar spine disease. The OLIF has usually combined with supplemental posterior pedicle screw fixation for decreasing the perioperative complications. The purpose of this study was to evaluate clinical efficacy and complications of oblique lateral interbody fusion (OLIF) combined with lateral plate instrumentation for the treatment of lumbar degenerative diseases.Methods From May 2020 to August 2020, the clinical data of 20 patients who underwent OLIF combined with lateral plate instrumentation were retrospectively analyzed. The operation time, blood loss, and the complications were recorded. Also, the radiological parameters, Visual Analog Scale (VAS) score and Oswestry Disability Index (ODI) were evaluated before and after surgery.Results The average operation time, blood loss, and length of hospital stay were 75.41±11.53 min, 39.57±9.22 ml, and 7.22±1.85 days, respectively. The VAS and ODI had both significantly improved after surgery (7.23±1.26 VS 2.15±0.87; 60.27±7.91 VS 21.80±6.32, P < 0.001). The postoperative disk height (DH) was 13.02±8.83 mm, which is much higher than before (P < 0.001); The postoperative foraminal height (FH) improved significantly (16.18±3.49 VS 21.54±2.12 mm, P < 0.01), and the cross-sectional area (CSA) had improved from 88.95±14.79 mm2 to 126.53±8.83 mm2 (P < 0.001). The radiological fusion rate was 95% at the last follow-up, while cage subsidence was found in one case. No major complications, such as ureteral injury, vascular injury and vertebral body fracture occurred.Conclusions OLIF combined with lateral plate fixation can avoid the lumbar posterior surgery, minimize the operation time, blood loss and the posterior ligament complex disruption. It can realize one-stage intervertebral fusion and instrumentation through a single small incision.

Abstract:Background The oblique lateral interbody fusion (OLIF) is a minimally invasive indirect decompression technique for the treatment of degenerative spinal disease. OLIF with posterior pedicle screws fixation frequently is performed, whereas it requires much more surgery time and blood loss. The purpose of this study was to compare the oblique lateral interbody fusion (OLIF) combined with lateral plate (LP) vs. OLIF combined with posterior pedicle screw (PS) fixation for the treatment of lumbar degenerative diseases.Methods The clinical data of 53 patients with lumbar degenerative diseases who underwent OLIF from January 2020 to September 2020 were retrospectively analyzed，24 in OLIF combined with lateral plate (OLIF+LP) group and 29 in OLIF combined with pedicle screw (OLIF+PS) group. All patients completed a minimum 1-year follow-up. The duration of operation, blood loss, fusion rate and complications were recorded. The visual analog scale (VAS) score, Oswestry Disability Index (ODI), disc height (DH), foraminal height (FH) and cross-sectional area (CSA) were also evaluated.Results The operation time was 75.41±11.53 min in the OLIF+LP group, which shorter than that in OLIF+PS group(127.05±5.62min, P<0.01). Also, the blood loss was significantly less in the OLIF+LP group (39.55±5.32 ml ) than in the OLIF+PS group (89.81±9.62, P<0.01). The VAS and ODI scores both significantly reduced after operation in OLIF+LP group. There was no difference either in VAS or ODI scores by 1 year after surgery between two groups (P>0.01). The DH, FH, and CSA parameters were all improved significantly after operation in both groups, however, there was no significant difference at the any follow-up point between the two groups. The total complication rate was 13.21% (7/53) in this study, and there was no siginificant difference between the two groups. The fusion rate was 91.67% in the OLIF+LP group and 93.10% in the OLIF+PS group (P =0.69).Conclusions OLIF+LP fixation seems to be a valuable surgical option for single-segmental lumbar degenerative disease, it can achieve much better clinical outcomes than OLIF+PS group.
Minimally Invasive Anterior Lumbar Interbody Fusion for Adjacent Segment Disease after Posterior Lumbar Fusion

Abstract:Posterior lumbar fusion is a popular technique for managing lumbar spine degenerative diseases including disc hernia, canal stenosis, scoliosis, spondylosis and degenerative spondylolisthesis. One of the commonest complications is adjacent segment disease (ASD). About 4.7% patients who have undergone lumbar fusion need revision surgery for ASD1. There are various revision procedures; they include anterior and posterior approaches. Due to the previous posterior procedure, the risks of a posterior revision approach are obviously high and include dural tear injury, nerve root injury, and secondary damage to posterior structures. Thus, a retroperitoneal approach to performing minimally invasive anterior lumbar interbody fusion (ALIF) would avoid all the risks mentioned above and might therefore be a better treatment option2, 3. A 45-year-old female had undergone posterior lumbar interbody fusion at the L3–4 to L4–5 level and posterolateral fusion of the L5S1 level 6 months previously for degenerative disease of the lumbar spine in a local hospital with an initially satisfactory outcome. However, five months after this surgery, she had developed progressive low back pain with numbness and tingling throughout her right leg. Careful physical examination showed tenderness on the right side of the L5 spinous process. She had a positive Lasègue sign and straight leg raising test in the right leg. There was also decreased sensation over the lateral aspect of the leg, lateral malleolus and lateral aspect of the foot. Plantar flexion was weaker in her right foot than in her left. The ankle jerk was reduced on the right side. Routine laboratory investigations were normal. X-ray films revealed that L3–S1 had been fixed with pedicle screws and there were cages in the L3–4 and L4–5 interspaces. Osteolysis was noted around the pedicle screws in the S1 vertebral body. MRI revealed disc herniation on the right side at the L5S1 level. Therefore, the preoperative diagnosis was disc herniation of L5S1 and ASD (L5S1). ALIF with an integrated titanium plate and a poly(ether-ether-ketone) (PEEK) cage (SynFix-LR, Synthes, Solothurn, Switzerland) was planned. To avoid vessel injury and identify an appropriate operating area, computed tomography angiography (CTA) of the abdominal aorta was performed preoperatively. After induction of general anesthesia, the patient was positioned supine on a table with a break at the level of the lumbar spine to exaggerate lumbosacral lordosis. A Pfannenstiel incision was made between the level of the umbilicus and symphysis pubis. The left side of the rectus sheath was dissected parallel to the midline and the left rectus abdominis was retracted laterally, after which the retroperitoneal space was entered by retracting all the periosteum to the right4. After the L5S1 level had been exposed and fluoroscopically verified, a SynFrame Retractor System (Synthes) was used to create a stable surgical field5. Before performing discectomy, the median sacral vessels were carefully ligatured or electrocoagulated. Then an adequate discectomy was performed6 and the nerve root was carefully decompressed. The adjacent vertebral endplates were scratched appropriately. After a trial cage had been tested, an appropriately sized cage with allograft was inserted into the intervertebral space. The cage was anchored by four diverging angle-stable locking screws into the adjacent vertebrae and its position was fluoroscopically confirmed. The operation time was 110 min with an estimated blood loss of 120 mL. The duration of hospitalization was 7 days. With the improved PEEK cage integrated with a titanium locking plate and four angle-stable screws (SynFix-LR, Synthes), stand-alone ALIF can meet the biomechanical needs of lumbar fusion7, 8. The improved cage is biomechanically superior to cages with separate plates9 and has a zero profile construct, which reduces the rates of vessel and visceral organ injury. The SynFrame Retractor System provides a more stable operative field and allows a smaller skin incision, reducing the risk of vessel and organ injury during the procedure5. A preoperative CTA of the abdominal aorta and careful handling of median sacral vessels increase the safety of this procedure. This technique facilitates more direct discectomy and more complete nerve decompression and involves a larger cage which results in higher fusion rates10. As a revision for ASD after posterior lumbar fusion, ALIF offers direct anterior column support without disturbing the posterior scars around the dural sac and nerve roots. Comparative study of three surgical procedures of lumbar interbody fusion for adult low-grade isthmic spondylolisthesis

Abstract:Objective To compare the clinical and radiographic outcomes of 3 kinds of lumbar interbody fusion (ALIF,PLIF,TLIF) for adult low-grade isthmic spondylolisthesis at L4.5 and L5S1 levels.Methods Data of seventy patients with lowgrade isthmic spondylolisthesis who had undergone different lumbar interbody fusion (LIF) procedures according to the assessment of the clinical manifestation,neurological symptoms,plane of the common iliac vessels and the width of "vascular window" by three-dimensional computed tomography angiography between May 2004 and May 2014 were retrospectively reviewed.21 patients had undergone anterior LIF (ALIF)(13 males and 8 females with an average of 43.29±8.15 years old (range,29-53 years old);L4,5 in ten,L5S1 in 11;Meyerding grade Ⅰ 10 cases,grade Ⅱ 11 cases);25 patients had undergone posterior LIF (PLIF) (12 males and 13 females,with an average of 44.04±12.71 years old (range,25-64 years old);L4.5 in 12,L5S1 in 13;Meyerding grade Ⅰ 15 cases,grade Ⅱ 10 cases) and 24 patients had undergone transforaminal LIF (TLIF) (12 males and 12 females with an average of 45.00± 9.36 years old (range,34-62 years old);L4,5 in 12,L5S1 in 12;Meyerding grade Ⅰ 11 cases,grade Ⅱ 13 cases).Results All patients were followed up for 24-37 months (average,25.6±8.7 months).The visual analogue scale (VAS) in all groups were decreased from 7.05±0.87 (ALIF),6.60±1.39 (PLIF),6.75±1.11 (TLIF) pre-operation to 0.90±0.70,0.96±0.68,1.04±0.62 24 months after operation.Oswestry disability index (ODI) scores were decreased from 78.53％±6.25％,79.80％±6.55％,79.92％±8.10％ preoperation to 17.14％±3.01％,21.32％±4.40％,22.46％±3.87％.Perioperative data including operation time,blood loss,postoperative blood drainage,length of stay showed no significant difference between ALIF group and TLIF group,but showing significant difference comparing with PLIF group.The imageological results showed that ALIF procedure increased whole lumbar lordosis (WL) and disc height (DH),showing no significant difference comparing with PLIF group,but showing significant difference comparing with TLIF group.The ALIF procedure increased segmental lordosis angle (SL),showing significant difference comparing with PLIF group and TLIF group.The PLIF procedure decreased spondylolisthesis index (SI),showing significant difference comparing with A/TLIF group,but showing no significant difference between ALIF group and TLIF group.There was no significant difference showed comparing the L1 axis S1 distance (LASD) and sacral slope (SS),in three groups.The graft fusion was achieved in all patients after 6-8 months postoperatively without instrument failure or displacement.Conclusion Three surgical procedures of lumbar interbody fusion for adult low-grade isthmic spondylolisthesis showed silimar clinical efficacy.The ALIF procedure is superior in its capacity to restore WL,DH,and SL,and the P/TLIF procedure are superior in correcting the vertebral slippage and spinal cord decompression.

Abstract:Objective To evaluate the early clinical outcomes and complications of oblique lateral interbody fusion (OLIF) in the treatment of degenerative lumbar diseases.Methods All of 83 patients,29 males and 54 females with ages from 32 to 83 (average 60.8± 13.7 y),underwent OLIF with or without posterior pedicle screw-rod instrumentations from October 2014 to February 2017.The index diagnosis was discogenic back pain in 17 cases,spondylolisthesis in 23,lumbar spinal canal stenosis in 25,and degenerative lumbar spinal kyphoscoliosis in 18 cases.The distribution of operative level was 5 at L1,2,13 at L2,3,38 at L3,4,and 69 at L4,5.The mean number of fusion level for each case was 1.5 segments.The operative duration,blood loss during operation,intra-operative and post-operative complications,the length of post-operative hospital stay were recorded.Clinical outcomes were evaluated using visual analogue scale (VAS) and Oswestry disability index (ODI).All patients were followed up for at least 3 months.Lumbar X-ray and CT scans were taken and the clinical outcomes were re-assessed during follow-up.Results Fifty-one in the 83 patients underwent supplementary posterior pedicle screw-rod instrumentation with OLIF procedures.The operation lasted for 43-295 min,with a mean duration of (153 ± 72) min.Mean operation time for each OLIF segment was 43± 12 min.Blood loss during the operation was 30-800 ml,with a mean of 125±74 ml.Mean blood loss for each OLIF segment was 27±13 min.Average length of stay was 5.6 ± 3.2 d,ranging from 3-15 d.The VAS for back pain and leg pain and ODI scores were decreased apparently for each patient.The total incidence of complications was 22.9％ (19/83),including 6.0％ (5/83) of intra-operative complications (4 cases of cage subsidence,1 case of segmental artery injury) and 16.9％ (14/83) of post-operative ones.The latter consisted of ipsilateral hip flexor weakness in 6,ipsilateral anterolateral thigh pain in 2,ipsilateral lateral thigh numbness in 1,contralateral pain in flexion of hip in 1,ipsilateral sympathetic chain injury in 2,and pain in area of iliac bone donor site in 2.All symptoms were released or disappeared during follow-up.Conclusion OLIF as a novel minimally invasive technique can act as a safe and effective treatment for degenerative lumbar diseases,which can also reduce approach-related complications.
Does Lateral Lumbar Interbody Fusion Decrease the Grading of Lenke-Silva Classification and Determine the Optimal Fusion Level in Severe Adult Degenerative Scoliosis?

Abstract:OBJECTIVE: To testify whether lateral lumbar interbody fusion (LLIF) decreases the Lenke-Silva classification grading and determines the optimal fusion level in adult degenerative scoliosis (ADS) of global imbalance.METHODS: Thirty-seven patients with ADS of level V and VI based on Lenke-Silva classification were included. After the first-stage LLIF, patients received reassessment and were divided into group A (the Lenke-Silva classification grading changed) and group B (the grading was unchanged). Posterior fixation was performed according to the reassessment. The demographic, operative, radiographic, and clinical data were compared between the 2 groups.RESULTS: Twenty-five patients of level V and 12 patients of level VI were included, with a mean follow-up of 29.6 months. After first-stage LLIF, the Lenke-Silva classification grading changed in 22 patients (group A), with the remaining 15 patients unchanged (group B). There were significant differences in preoperative Lenke-Silva classification grading, use of anterior column realignment and hyperlordotic cage and high-grade cage subsidence between the 2 groups. The posterior fusion levels in the second surgery were less and the rate of fusion to thoracic spine region was lower in group A. The visual analog scale and Oswestry Disability Index were significantly improved and restorations of coronal and sagittal balance were found at the latest follow-up in both groups.CONCLUSIONS: LLIF decreased the Lenke-Silva classification grading and determined the optimal fusion level in patients with severe ADS. Change of Lenke-Silva classification may be associated with preoperative Lenke-Silva classification grading, use of anterior column realignment, and hyperlordotic cage.

Abstract:This study compares the safety and efficacy of posterolateral lumbar fusion (PLF) and transforaminal lumbar interbody fusion (TLIF) in the treatment of degenerative lumbar scoliosis (DLS). Forty DLS patients with Cobb angles of 20–60 degrees were randomized into either the PLF or TLIF treatment group, and were followed up for 2–5years. Operating time, intraoperative blood loss, clinical outcomes, complications and imaging were compared between the two groups. There were significant differences between the PLF and TLIF treatment groups in operative time (187.8±63.5minutes and 253.2±57.6minutes, respectively; p=0.002) and intraoperative blood loss (1166.7±554.1mL and 1673.7±922.4mL, respectively; p=0.048). The occurrence rates of early complications in the two groups were 11.1% and 26.3%. The recovery rates of the lumbar lordotic angle and spinal sagittal balance were significantly different (36.7% versus 62.5% and 44.8% versus 64.1%, respectively). In various domains of the Scoliosis Research Society-22 (SRS-22) questionnaire, the scores for pain and satisfaction with the treatment showed significant differences between PLF and TLIF group (p=0.033 and p=0.006, for pain and satisfaction respectively), and the TLIF group showed better outcomes than the PLF group. There were no significant differences in the recovery rates in the Cobb angle and the spinal coronal balance, function, self-image, or mental health scores. Although TLIF increases the surgical trauma and occurrence of complications, it helps to improve lumbar lordosis and sagittal balance and shows better clinical outcomes. For patients without significant loss of lumbar lordosis and with good spinal sagittal balance preoperatively, PLF is still an option.

Abstract:Objective To evaluate the clinical outcomes of one-level oblique lateral interbody fusion (OLIF) for the treatment of degenerative lumbar diseases.Methods A total of 36 patients,11 males and 25 females with ages from 32 to 75 (average 55.2 ± 13.6 years),underwent one-level OLIF with or without posterior pedicle screw-rod instrumentations from October 2014 to May 2016.The index diagnosis was discogenic back pain in 15 cases,and spondylolisthesis with or without segmental instability in 21 cases.The distribution of operative level was 30 cases at L4/5 and 6 cases at L3/4.The changes of serum level of creatinine kinase was measured 1 and 3 days post-operatively,and compared to that of the pre-operative one.Clinical outcomes were evaluated using visual analogue scale (VAS) and Oswestry disability questionnaire (ODI) scores.Lumbar X-ray was taken and the clinical outcomes were re-assessed.Results 25 in the 36 patients underwent supplementary posterior pedicle screw-rod instrumentation with OLIF procedures.The operation lasted for 46-175 min,with a mean duration of (112±43) min.Blood loss during the operation was 30-180 ml,with a mean of (63 ± 35) ml.Average length of stay was (4.5±2.1) d,ranging from 3-7 d.Serum level of creatinine kinase was (63.5±16.8) IU/L before operation,(416.8±152.1) IU/L 1 day after operation,(112.4±37.5) IU/L 3 days after operation.The VAS and ODI score decreased from (8.3±2.3) pre-operatively to (1.6±0.4) 6 months post-operatively,and from (69.2±17.5) to (10.7±2.7),respectively.The total incidence of complications was 25％,including 8.3％ of intra-operative complications (3 cases of cage subsidence) and 16.7 ％ of post-operative ones.The latter consisted of ipsilateral hip flexor weakness in 1,ipsilateral anterior thigh pain in 1,contralateral pain in flexion of hip in 1,ipsilateral sympathetic chain injury in 1,and pain in area of iliac bone donor site in 2.All symptoms were released or disappeared during follow-up.Conclusion OLIF as a novel technique for spinal surgical management showed excellent short-term outcomes.Clinical results with long-term follow-up are required.
Oblique Lumbar Interbody Fusion with Stand-Alone Cages for the Treatment of Degenerative Lumbar Spondylolisthesis: A Retrospective Study with 1-Year Follow-Up

Abstract:Patients with degenerative lumbar spondylolisthesis (DLS) often suffer from years of low back pain (LBP) due to instability of the lumbar spine and the reduction of disc height. Since January 2016, we have performed oblique lateral interbody fusion (OLIF) on 154 patients. Among these, 56 patients who suffered from DLS underwent OLIF with stand-alone cages. Forty-two patients with a follow-up time that exceeded 1-year were enrolled for this study. The forty-two patients were followed up for at least one year. Operation segments ranged from L3-4 to L4-5. All the patients were with 1-level fusion. The mean postoperative ventral-disc height and dorsal-disc height increased significantly compared with preoperative (P<0.05). A significant postoperative increase was also observed in the mean operative segmental lordotic angle and the whole lumbar lordotic angle (P<0.05). Compared with preoperative, the postoperative VAS significantly decreased with no significant increase in the VAS in the last follow-up. The LBP was significantly relieved. The mean postoperative VAS of LBP decreased significantly compared with the preoperative ((1.6 ± 0.8) vs. (7.8 ± 0.8)). Postoperative complications included psoas major abscess and intervertebral space infection (1/56). Except for one patient whose cage subsided during the last follow-up, the other patients had good cage position. The one whose cage collapsed complained no symptoms including LBP. OLIF with stand-alone cages should be considered as a safe and effective option which can effectively alleviate LBP for the treatment of DLS.
Low profile posterior lumbar-sacral interbody fusion for lumbosacral degenerative diseases: a technical note

Abstract:Background: The purpose of this study was to report our surgical experience in patients with lumbosacral degenerative diseases who underwent posterior decompression and interbody fusion fixed with cortical bone trajectory screw and sacral alar screw, which is known as low-profile posterior lumbosacral interbody fusion (LP-PLSIF). Methods: Patients with lumbosacral degenerative disease who underwent LP-PLSIF and traditional PLSIF (control group) internally fixed with pedicle screws were included retrospectively. Patients' demographic data, operative parameters, and perioperative complications were recorded and analyzed. Results: A total of 18 patients were enrolled in this study, which included 9 patients (5 male and 4 female) who underwent LP-PLSIF, and 9 patients (4 male and 5 female) who underwent traditional PLSIF. There wasn't a significant difference in the average age between the two groups, 56.78 ± 10.92 years in the LP-PLSIF group and 60.22 ± 8.21 years in the PLSIF group (p = 0.460). The bone mineral density (BMD) of the two groups of patients were -2.00 ± 0.26 T and -2.13 ± 0.19 T, respectively (P = 0.239). The mean postoperative follow-up time was 12.7 months (range, 12-14 months). The mean operation time was 142.78 ± 11.21 min and 156.11 ± 13.41 min in the LP-PLSIF group and PLSIF group respectively (P < 0.05). The average blood loss was 137.78 ± 37.09 ml in the LP-PLSIF group, and 150.00 ± 27.84 ml in the PLSIF group (P = 0.441). The average postoperative drainage was 85.56 ± 37.45 ml and 122.22 ± 22.24 ml in the LP-PLSIF group and control group respectively (P < 0.05). Patients in the LP-PLSIF group had shorter incision length compared with the control group, 61.44 ± 10.56 mm vs. 74.56 ± 10.22 mm (P < 0.05). The average length of hospitalization of 11.33 ± 2.92 days in the LP-PLSIF group, and 11.11 ± 1.62 days in the PLSIF group (p = 0.844). All patients had significant improvement in VAS pain score, ODI, and JOA evaluation. However, patients in the LP-PLSIF group had better improvement in terms of VAS back pain and ODI in the short term after the operation. There were no neurological complications or wound infection. The fusion rate at the last follow-up was 100% (9 of 9) in the LP-PLSIF group, and 88.89% (8 of 9) in the control group based on CT scans. 1 patient in the control group had asymptomatic sacral pedicle screw loosening. Conclusions: LP-PLSIF is a safe and effective surgical technique for patients with lumbosacral degenerative disease, which has the potential strength of less invasive and better clinical improvement.
[Preliminary clinical results of endoscopic assisted anterior lumbar interbody fusion]

Abstract:Objective: To summarize the experiences of endoscopic assisted anterior lumbar interbody fusion (ALIF) with self-locking cage via retroperitoneal approach through small incision lateral to abdominal rectus. Methods: There were 47 cases of endoscopic assisted ALIF from April 2010 to April 2012 were reviewed retrospectively, including 28 male and 19 female patients whose age ranged from 39 to 65 years, averaged of (48 ± 11) years. Of 47 cases, 17 cases were founded instability in lumbar spine, 16 cases of discogenetic low back pain, 7 cases of degenerative spondylolithesis, and 7 cases of recurrent disc herniation. The level involved included 7 cases at L3-4 level, 33 cases at L4-5 level, 7 cases at L5-S1. X-ray and/or 3D CT scan were done in each patient at post-operative day 4, 1 month, 3 months, and 6 months. The height and angle of disc space, angle of lumbar lordosis (LL), pelvic incidence (PI), pelvic tilt (PT), and sacral slope (SS) were recorded and measured, as well as length of incision, operation time, blood loss, complications and ratio of bone graft fusion. The clinical outcomes were evaluated by Oswestry disability index (ODI) and visual analogue scale (VAS). Results: Average length of incision was (6.2 ± 0.8)cm (5.3-7.8 cm) , average blood loss was (81 ± 20)ml (50-150 ml) , mean operation time was (96 ± 10)minutes (65-122 minutes) . There were significant improvement of ODI(48% ± 10% vs. 14% ± 5%), the VAS of back pain(6.3 ± 1.1 vs.0.7 ± 0.7) and VAS of leg pain(3.4 ± 2.1 vs.0.6 ± 0.6) during preoperative and postoperation last follow-up (t = 20.78, 25.92 and 8.74, P = 0.000). The disc height of operative segments were all recovered significantly (F = 18.971, P = 0.000) postoperatively, from preoperative (10.3 ± 2.4)mm to postoperative (12.6 ± 2.1)mm. For complications, 5 cases had peritoneum tear, 19 cases had bone donor site pain, 14 cases had abdominal distension and 5 cases had lower limb paraesthesia, which were all treated with supportive managements. Bony intervertebral fusion was obtained in all cases in 6 months postoperatively. Conclusions: The advantages of endoscopic assisted ALIF with self-locking cage via retroperitoneal approach are less damage, rapid recovery, and less blood loss. Comparing with traditional ALIF procedure, the view of operative field is also improved.

Abstract:Objective Adjacent-segment disease (ASD) is common in patients undergone previous lumbar fusion. A typical revision treatment from posterior approach requires management of postoperative scar tissue and previously implanted instrumentation. An oblique lumbar interbody fusion (OLIF) approach allows surgeon to reduce the potential risk of posterior approach. This study aimed to analyze the clinical and radiographic efficacy of stand-alone OLIF for the treatment of lumbar adjacent-segment disease. Methods A total of 13 consecutive patients who underwent stand-alone OLIF for the treatment of adjacent-segment disease from December 2016 to January 2019 were reviewed. Visual analog scale (VAS) of back pain and leg pain and the Oswestry Disability Index (ODI) before surgery and at last postoperative clinic visits were obtained. Radiography, CT and MRI before and at last follow-up after surgery was evaluated in all patients. Results During the study period, 13 cases were successfully treated with stand-alone OLIF. The mean follow-up was 17.7 ± 8.3 months. The back pain VAS improved from 6.2 ± 1.0 to 2.0 ± 1.1 ( P < 0.01), and the leg pain VAS improved from 7.0 ± 1.9 to 1.0 ± 0.9 ( P < 0.01). ODI improved from 28.0 ± 7.5 to 10.8 ± 4.0 ( P < 0.01). The disc height (DH) increased from 9 ± 2 to 12 ± 2 mm ( P < 0.01), the cross-sectional area (CSA) of spinal canal increased from 85 ± 26 to 132 ± 24 mm 2 ( P < 0.01), the foraminal height increased from 17 ± 2 to 21 ± 3 mm ( P < 0.01) and the CSA of foramen increased from 95 ± 25 to 155 ± 36 mm 2 ( P < 0.01). Cage subsidence was observed in 2 cases. Conclusions Stand-alone OLIF provides a safe and effective alternative way to treat ASD.

Abstract:The anaerobic technology application for domestic sewage treatment in Pernambuco State (Brazil) is relatively recent. Some UASB reactors of less than 250 m3 were built in the Recife Metropolitan Region (RMR) in the 1990s. Mangueira (18,000 inhabitants) was the first neighborhood where the municipality built a plant with a bigger UASB reactor of 810 m3. It was intended to evaluate the performance and verify if such technology would be feasible. The objective would be the possible application of UASB reactors to the RMR, according to the new sewage master-plan under elaboration that would benefit about 3 million inhabitants. The monitoring of the Mangueira UASB reactor over 30 months showed that satisfactory results were obtained. Three distinct operational phases occurred, in which efficiency varied from 60% up to 90% based on COD removal. The results were very dependent on the operation and maintenance, either in the plant or in the sewage collection. Significant amount of inert solids was measured inside the reactor. Despite the operational problems, the UASB was shown to be very robust and stable. Under high fluctuation of influent concentrations (150-750 mg COD/L) during the period, resulting in applied organic loading rate of 0.5 to 2.5 kg COD/m3.d, the average values of COD removal efficiency did not change significantly. An active biomass with specific methanogenic activity varying from 0.18 to 0.25 g COD/g VSS.d was measured at the end of the period.
Effects of Cage Implantation Depth on Sagittal Parameters and Functional Outcomes in Posterior Lumbar Interbody Fusion for the Treatment of L4‐L5 Lumbar Degenerative Spondylolisthesis

Abstract:This is a retrospective cohort study, the purpose of this study was to compare the radiographic and clinical functional improvement of L4‐L5 lumbar degenerative spondylolisthesis (LDS) patients with anterior cage implantation and posterior cage implantation. This study reviewed 128 patients with L4‐L5 LDS between January 2016 and August 2019. The lumbar lordosis (LL), segmental lordosis (SL), sacral slope (SS), pelvic incidence (PI), pelvic tilt (PT) and slope degree (SD) was measured for radiographic outcomes. We used the visual analog scale (VAS) and the oswestry disability index (ODI) score to assess functional outcomes. The LL, SL, PT, SS, SD and PI‐LL after operation showed great improvements, relative to the corresponding preoperative values in both groups. Compared to Group P, Group A exhibited far enhanced SL, LL outcomes and showed depressed PI‐LL outcomes at the final follow‐up. Meanwhile, the SL in Group A 1 months after operation were also higher than in Group P. We found that VAS and ODI at the final follow‐up in Group A were significantly lower than those in Group P. For patients with LDS, the anteriorly placed cage may provide better improvement of SL after PLIF surgery. Meanwhile, the anteriorly placed cage may achieve better sagittal balance and functional outcomes in the long run. All patients underwent an open PLIF surgery that included intravertebral decompression, implantation of pedicle screws and cage. We grouped according to the position of the center of the cage relative to the L5 vertebral endplate. Patients with the center of the cage located at the anterior 1/2 of the upper end plate of the L5 vertebral body were divided into Anterior group, and located at the posterior 1/2 of the upper end plate of the L5 vertebral body were divided into Posterior group. The lumbar lordosis (LL), segmental lordosis (SL), sacral slope (SS), pelvic incidence (PI), pelvic tilt (PT) and slope degree (SD) was measured for radiographic outcomes. We used the visual analog scale (VAS) and the oswestry disability index (ODI) score to assess functional outcomes. Paired t‐test was used to compare imaging and bedside data before and after surgery between the two groups, and independent sample t‐test, χ2 test and Fisher exact test were used to compare the data between the two groups. Result The mean follow‐up of Anterior group was 44.13 ± 9.23 months, and Posterior group was 45.62 ± 10.29 months (P > 0.05). Abstract:Lateral lumbar interbody fusion (LLIF) is a minimally invasive surgical approach used to treat a variety of degenerative and deformity conditions of the lumbar spine such as advanced degenerative disease, degenerative scoliosis, foraminal and central stenosis. It has emerged as an alternative to the traditional posterior and anterior lumbar approaches with some potential benefits such as lower blood loss and shorter hospital stay. In this article, we provide our single institutional surgical experience including main indications and contraindications, a step-by-step surgical technique description, a detailed preoperative imaging assessment with a focus on magnetic resonance imaging (MRI) psoas anatomy, operative room (OR) setup and patient positioning. A descriptive surgical technical note of the following steps is provided: positioning and fluoroscopic confirmation, incision and intraoperative level confirmation, discectomy and endplate preparation, implant size selection and insertion and final fluoroscopic control, hemostasis check and wound closure along with an instructional surgical video with tips and pearls, postoperative patient care recommendations, common approach-related complications, along with our historical clinical institutional group experience. Finally, we summarize our research experience in this surgical approach with a focus on LLIF as a standalone procedure. Based on our experience, LLIF can be considered an effective surgical technique to treat degenerative lumbar spine conditions. Proper patient selection is mandatory to achieve good outcomes. Our institutional experience shows higher fusion rates with good clinical outcomes and a relatively low rate of complications.

The incidence of postoperative chronic low back pain (improvement of NRS less than 50%) in the two groups was also recorded. Results: Significant restoration of spino-pelvic sagittal balance was observed in the PLIF group after surgery. Both groups achieved significant improvements in NRS and ODI postoperatively. Significant differences in postoperative lumbar lordosis and pelvic tilt were found between the PLF and PLIF groups. Significant difference in the incidence of postoperative chronic low back pain was also detected between the two groups. Conclusion: PLF and PLIF can improve the clinical outcomes of patients with degenerative spondylolisthesis. PLIF can achieve better restoration of spino-pelvic sagittal balance parameters and less incidence of postoperative chronic low back pain than PLF.
