Abstract:Objective To study the anatomical characteristic of nanophthalmos with ultrasonography and its clinical significance. Methods Forty-four nanophthalmic eyes (22 patients) and 200 normal eyes (100 persons) were included. A series of ocular parameters, including ocular axial length, angle between trabeculum and iris,thickness of lens and sclera.and vitreous cavity length were measured with ultrasound A-scan and ultrasound biomicroscopy. The refractive status was examined with dioptometry. Results The ocular axial length[(15. 95±0. 76)mm] and vitreous cavity length [(8. 48±0. 90)mm] were shorter, the lens [(4. 9±0. 53)mm] and sclera [(0. 917±0. 119)mm] were thicker in nanophthalmic eyes than those of normal eyes( P <0.01). The relative position of lens (0.312±0.09) was more forward, and the angle between trabeculum and iris (11. 41 ±7. 63 ) was narrower in nanophthalmic eyes, compared with normal eyes( P <0. 01). The hyperopia of nanophthalmic eyes was negatively correlated with the ocular axial length ( r = -0.806) and vitreous cavity length ( r = - 0. 663) ( P <0. 01). Conclusions Nanophthalmos was characterized by short ocular axial length, thick lens and sclera.and narrow anterior chamber angle. The thickness of sclera and the angle between trabeculum and iris measured with ultrasound biomicroscopy could provide a solid indication of the treatment to the complication of angle closure glaucoma.

Abstract:Axenfeld-Rieger syndrome (ARS) is a rare congenital disease that is primarily characterized by ocular anterior segment anomalies but is also associated with craniofacial, dental, cardiac, and neurologic abnormalities. Over half of cases are linked with autosomal dominant mutations in either FOXC1 or PITX2, which reflects the molecular role of these genes in regulating neural crest cell contributions to the eye, face, and heart. Within the eye, ARS is classically defined as the combination of posterior embryotoxon with iris bridging strands (Axenfeld anomaly) and iris hypoplasia causing corectopia and pseudopolycoria (Rieger anomaly). Glaucoma due to iridogoniodysgenesis is the main source of morbidity and is typically diagnosed during infancy or childhood in over half of affected individuals. Angle bypass surgery, such as glaucoma drainage devices and trabeculectomies, is often needed to obtain intraocular pressure control. A multi-disciplinary approach including glaucoma specialists and pediatric ophthalmologists produces optimal outcomes as vision is dependent on many factors including glaucoma, refractive error, amblyopia and strabismus. Further, since ophthalmologists often make the diagnosis, it is important to refer patients with ARS to other specialists including dentistry, cardiology, and neurology.

Abstract:CONTEXT Since anorectocele is usually associated with mucosa prolapse and/or rectal intussusceptions, it was developed a stapled surgical technique using one circular stapler. OBJECTIVE To report the results of Transanal Repair of Rectocele and Rectal Mucosectomy with one Circular Stapler (TRREMS procedure) in the treatment of anorectocele with mucosa prolapse in a prospective multicenter trial. METHODS It was conducted by 14 surgeons and included 75 female patients, mean aged 49.6 years, with symptoms of obstructed defecation due to grade 2 (26.7%) and grade 3 (73.3%) anorectocele associated with mucosa prolapse and/or rectal intussusception (52.0%) and an average validated Wexner constipation score of 16. All patients were evaluated by a proctological examination, cinedefecography, anal manometry and colonic transit time. The TRREMS procedure consists of the manual removal of the rectocele wall with circumferential rectal mucosectomy performed with a circular stapler. The mean follow-up time was 21 months. RESULTS All patients presented obstructed defecation and they persisted with symptoms despite conservative treatment. The mean operative time was 42 minutes. In 13 (17.3%) patients, bleeding from the stapled line required hemostatic suture. Stapling was incomplete in 2 (2.6%). Forty-nine patients (65.3%) required 1 hospitalization day, the remainder (34.7%) 2 days. Postoperatively, 3 (4.0%) patients complained of persistent rectal pain and 7 (9.3%) developed stricture on the stapled suture subsequently treated by stricturectomy under anesthesia (n = 1), endoscopic stricturectomy with hot biopsy forceps (n = 3) and digital dilatation (n = 3). Postoperative cinedefecography showed residual grade I anorectoceles in 8 (10.6%). The mean Wexner constipation score decreased significantly from 16 to 4 (0-4: n = 68) (6: n = 6) (7: n = 1) (P<0.0001). CONCLUSION Current trial results suggest that TRREMS procedure is a safe and effective technique for the treatment of anorectocele associated with mucosa prolapse. The stapling technique is low-cost as requires the use of a single circular stapler.
The Correlation of Anterior Segment Structures in Primary Congenital Glaucoma by Ultrasound Biomicroscopy with Disease Severity and Surgical Outcomes.

Abstract:PURPOSE:To evaluate the anterior segment structures using ultrasound biomicroscopy (UBM) in primary congenital glaucoma (PCG) and explore their correlation with disease severity and surgical outcomes. METHODS:Clinical information of PCG patients who underwent UBM prior to their first glaucoma surgeries from September 2014 to March 2021 were reviewed. The study included 214 UBM images of 154 PCG eyes and 60 fellow unaffected eyes. Anterior segment characteristics were analyzed. UBM parameters, including the iris thickness (IT) at variant distances from the pupil edge and iris root, anterior chamber depth (ACD), and pupil diameter (PD), were compared between two groups and their relationship with clinical factors and surgical outcomes were analyzed in PCG eyes. RESULTS:PCG eyes had unclear scleral spur, thin iris, wide anterior chamber angle, deep anterior chamber, rarefied ciliary body, elongated ciliary processes, and abnormal anterior iris insertion. ITs were thinner, ACD was deeper, and PD was larger in PCG eyes than fellow unaffected eyes (all P < 0.001). In PCG eyes, thinner ITs correlated with bilateral involvement and earlier age at presentation, and larger PD correlated with earlier age at presentation (P = 0.030) and higher intraocular pressure (P < 0.001). Thinner IT2 (P = 0.046) and larger PD (P = 0.049) were identified as risk factors for surgical failure. CONCLUSION:UBM is a powerful technique to exam anterior segment structures in PCG. The anatomical features are associated with disease severity and surgical outcomes, providing essential clinical insights.
Cell proliferation and differentiation in the fetal and early postnatal mouse thymus.

Abstract:The relationships between cell proliferation and cell differentiation during thymus ontogeny were studied by labeling DNA-synthesizing thymocytes with bromodeoxyuridine and staining with antibodies against CD4, CD8, J11d, phagocytic glycoprotein 1, TCR V beta 8 chain, Thy-1, and IL-2R surface proteins. The development of the thymus was discontinuous, with two well defined growth periods from 13 days to 18 days of fetal life and from 3 days to 6 days after birth, and more progressive growth from day 8 to 2 wk. Cell proliferation started on fetal day 12, 1 day after the arrival of hemopoietic stem cells in the third branchial pouch. These cells were phagocytic glycoprotein 1-positive but IL-2R and Thy-1 negative. Thus, cell proliferation preceded IL-2R expression. Until day 15, CD4-8- thymocytes expanded without differentiation. Then CD4-8+ and CD4+8+ cells appeared; this induction was proliferation dependent and occurred on cells which had already lost IL-2R, but just after maximum expression of this receptor. During several days, the thymus remained of constant size (around 10(7) cells) and behaved like the steady state thymus. On day 3 after birth, expansion started again and was correlated with an increase in CD4-8- proliferation index and IL-2R expression. At the same time, the thymic subset capable of expansion without differentiation was again, transiently, detectable. These results suggest that the inflow of precursor cells into the thymus is permanent but transiently increased at several times during ontogeny. Moreover, the behavior of fetal CD4-8- cells does not appear radically different from that of adult precursors, but the actual difference resides in the variation of the relative proportion of CD4-8- cells at different maturation stages, as revealed by striking variations of IL-2R expression by cycling cells.
Axenfeld-Rieger syndrome: a novel histopathologic finding associated with corneal abnormalities

Abstract:Background: Axenfeld-Rieger syndrome (ARS) is a rare kind of anterior segment dysgenesis (ASD). The most common ocular features of ARS are posterior embryotoxon and iris hypoplasia, while some patients may manifest as corneal opacity and edema. However, the current understanding of how ARS affects the cornea is still incomplete. This study reports a novel histopathological finding of ARS, complicating corneal abnormalities, including congenital corneal opacity and irreversible endothelial decompensation. Methods: This retrospective study included 6 eyes of 3 ARS patients, 5 of which underwent keratoplasty for irreversible endothelial decompensation from May 2016 to January 2019. No eye had a history of surgery. We reviewed the data of epidemiology, clinical manifestations and histopathologic examinations. Results: Five eyes developed irreversible endothelial decompensation, among which 4 were born with corneal opacity. One eye exhibited transparent cornea but showed a continuous loss of endothelial cells in the absence of surgery and elevated intraocular pressure thereafter. Anterior segment optical coherence tomography photographs showed that anterior synechia existed in the area with corneal opacities, where we found the interlayer splitting of the Descemet membrane inserted by hypoplastic iris and a basement membrane-like structure under a light microscope. Conclusion: Anterior synechia might be associated with corneal abnormalities in ARS patients. The novel histopathologic finding revealed the internal relation between anterior segment dysgenesis and would help explore the inner mechanism of corneal abnormalities in ARS.

Abstract:Background Axenfeld-Rieger syndrome (ARS) is a congenital disease with a series of developmental abnormalities, and no case of ARS with cataract and small cornea has been reported in previous studies. In the present report, we aimed to describe the diagnosis and phacoemulsification of an ARS patient with small cornea. Case presentation A 58-year-old Han Chinese male patient who was referred to Eye Center of the Second Affiliated Hospital of Zhejiang University Medical College was diagnosed with ARS. Systemic and ophthalmic examination and genetic testing were performed. The slit-lamp microscopic examination of anterior segment showed obvious nuclear cataract, iris lesions, and the abnormal cornea of both eyes with small transversal and longitudinal diameters. ARS with bilateral complicated cataract and small cornea was diagnosed. Microincision-phacoemulsification in combination with intraocular lens implantation was performed on his left eye. After successful surgery of his left eye, the best-corrected visual acuity (BCVA) was obviously improved from 2 to 0.5 (LogMAR). A transient elevation of intraocular pressure (IOP) was controlled with medication. Conclusions Through genetic testing, a known pathogenic mutation NM_153427.2:c.272G > A was detected on the PITX2 gene; and an unknown mutation NM_001453.2:c.1063C > T was detected on FOXC1 gene. For the ARS patient with complicated cataract, the visual acuity was increased by phacoemulsificasion in combination with microincision.
Observation of relationship between morphology of ciliary body and glaucoma with ultrasound biomicroscopy

Abstract:Objective To observe the relationship between morphology of ciliary body and glaucoma with ultrasound biomicroscopy (UBM). Methods Eighty shallow-anterior chamber patients and 80 normal-anterior chamber patients were enrolled as shallow-anterior chamber group and control group,respectively. All patients were greater than 40 years old. The shallow-anterior chamber group included 28 patients of acute angle-closure glaucoma (AACG subgroup),26 patients of chronic angle-closure glaucoma (CACG subgroup),and 26 patients of primary angle-closure suspects (PACS subgroup). Both eyes of all patients were checked with UBM,and position of each eye on sagittal and coronal section of superior,nasal,inferior and temporal were recorded. Then ciliary process height (CPH),ciliary process thickness (CPT),ciliary process interval (CPI),ciliary process number (CPN),A angle and α angle were measured. Furthermore,the difference between shallow-anterior chamber group and control group,the difference between AACG,CACG and PACS subgroup were compared,as well as the difference between PACS subgroup and control group,and the correlation between A angle and α angle was analyzed. Results Compared with control group,CPH and CPT of shallow-anterior chamber group increased,while CPI,CPN,A angle and α angle decreased (P<0.05). Except CPH and CPN between CACG and PACS subgroup,there were significant differences of parameters between subgroup of AACG,CACG and PACS (P<0.05). CPH of PACS subgroup increased than those of control group,while CPI,CPN,A angle and α angle decreased (P<0.05). There was positive correlation between A angle and α angle. Conclusion Morphologic parameters of ciliary process may be sensitive indexes which can early screen primary angle-closure glaucoma.

Abstract:Background: Axenfeld-Rieger syndrome (ARS) is a rare cause of congenital glaucoma and may result in loss of vision. ARS is mostly autosomal dominant in nature characterized by developmental abnormalities in the angle of anterior chamber and iris of the eye, also associated with structural abnormalities in the body. Aim: To study and observe the demographics and clinical findings in a very rare ocular disease known as ARS. Methods: Case records of symptomatic patients attending Ophthalmology outpatient department and diagnosed to have ocular hypertension or glaucoma in 3 years from March 2017 to March 2020 were evaluated to search for cases diagnosed with ARS. Records of all patients diagnosed with ARS were then analysed for demographic and clinical characterization as well as management and success of therapy. Results: Eight out of ten patients with positive clinical signs were symptomatic and had glaucoma. One of these patients had limbal stem cell deficiency and another had vernal keratoconjunctivitis. Conclusion: Clinical characterization of ARS is important for making a definitive diagnosis and determining prognosis.

Abstract:Objective To study the difference between acute angle-closure glaucoma and chronicangle-closure glaucoma in anatomical structure. Methods anatomical structures including the anterior cham-ber depth(ACD), lens thickness(LT) and the axial length(AL),were measured by A-mode ultrasonic scanning in33 eyes of 33 patients with AACG, 36 eyes of 36 patients with CACG and 36 eyes of 36 normal controls. Therelative location of lens(RLL) and LT/AL ratio were also calculated. All mean values were analyzed with t-test.Results There were significant differences between the AACG eyes and the CACG eyes in all items exceptRLL. Compared with normal eyes, both AACG eyes and the CACG eyes had significant differences in all items.Conclusions There were significant differences between the AACG eyes and the CACG eyes in anatomicalstructure. AACG eyes had a shallower ACD, thicker LT, shorter AL. It indicates that there is different mecha-nism between AACG and CACG.

Abstract:Axenfeld-Rieger syndrome (ARS) is characterized by posterior embryotoxon, Axenfeld anomaly (adherent iris strands to Schwalbe's line), and Rieger anomaly (iris hypoplasia with corectopia or pseudopolycoria). There are a few case reports of optic nerve abnormalities associated with 6p25 deletion syndrome, which is a multigenic region that contains the FOXC1 gene. We present 4 patients with ARS, including 1 with a FOXC1 nonsense mutation, who also have prominent congenital optic nerve abnormalities.

Abstract:To investigate the mechanism of primary angle closure glaucoma (PACG) especially the role of the changes of ciliary body using ultrasound biomicroscope (UBM) combined with general A scan.99 eyes with PACG, including 50 eyes with acute and 49 eyes with chronic PACG, were evaluated and 58 normal eyes were in the control group.Compared with the normal eyes the PACG eyes had their anatomic features. They had a smaller corneal diameter, shorter axial length, shallower anterior chamber, thicker lens, more relative anterior location of lens, swelling of ciliary process and anterior rotation of ciliary body. All these differences were significant statistically (P < 0.05). Compared with the chronic PACG eyes the acute PACG eyes had a shallower anterior chamber and a more anteriorly situated lens position. The differences were significant statistically (P < 0.01). Compared with the normal eyes the acute PACG eyes had a shorter ciliary body - lens distance and the difference was significant (P < 0.05); the chronic PACG eyes also had a shorter ciliary body-lens distance, but the difference was not significant (P > 0.05).The PACG eyes have their anatomic features. Among these factors the pupillary block caused by the anterior displacement of lens and the narrow angle induced consequently are the common mechanism. The swelling ciliary process, the positional changes of the ciliary body and age-related thicker lens are possibly the initial causes causing the forward movement of lens, and the pupillary block plays a more important role in the pathogenesis of acute PACG.
Ultrasound biomicroscopic differences between fellow eyes of acute angle-closure glaucoma and chronic angle-closure glaucoma

Abstract: To compare the structural differences of the anterior segment between fellow eyes of acute angle-closure glaucoma (AACG) and the eyes of chronic angle-closure glaucoma (CACG) with milder glaucomatous damage. In this case-control study, patients with AACG (41 eyes) and CACG (46 eyes) without prior treatment in the glaucoma clinic of Peking University People's Hospital from September 2016 to October 2018 were enrolled. Ultrasound biomicroscopy was performed under dark condition. Parameters were measured on images including lens vault (LV), anterior chamber depth (ACD), iris thickness (IT750 and IT2000), angle-opening distance (AOD500 and AOD750), and trabecular iris angle (TIA500 and TIA750). The independent -test was used to compare the continuous variables of the fellow eyes of AACG patients and the eyes of CACG patients with milder glaucomatous damage. After adjusting for age and gender, univariate and multivariate logistic regression analyses were performed to explore the most important parameters that may distinguish AACG from CACG. The mean age of AACG patients was (65±10) years old and the age of CACG patients was (67±12) years old (>0.05). The ACD [(1.79±0.25) mm . (1.99±0.34) mm], IT750 [(0.39±0.07) mm . (0.43±0.05) mm], AOD500 [(0.12±0.06) mm . (0.15±0.07) mm], TIA500 (10.91°±5.23° . 13.93°±6.33°), and TIA750 (9.33°±5.02° . 13.93°±6.82°) were less and the LV [(0.99±0.44) mm . (0.72±0.30) mm] was greater in the fellow eyes of AACG as compared to the eyes of CACG with milder glaucomatous damage (all <0.05). In the forward multivariate logistic regression analysis, every 1-degree decrease in TIA750 (odds ratio=0.872, 95%: 0.794 to 0.958, <0.01) and every 1-mm increase in LV (odds ratio=14.138, 95%: 2.348 to 85.130, <0.01) were significantly associated with AACG. Compared with the eyes of CACG with milder glaucomatous damage, fellow eyes of AACG have thinner peripheral iris thickness, narrower angle width, shallower ACD, and greater LV. LV and TIA750 may play important roles in distinguishing eyes predisposed to AACG or CACG. .
The study of relationship between primary iridociliary cysts and anterior chamber angle by ultrasound biomicroscopy

Abstract:Objective To observe the morphology of primary iridociliary cysts,and to analyze the relationship between the cysts and adjacent anterior chamber angle and the influence and degree of multiple cysts to anterior chamber angle on the whole circle by ultrasound biomicroscopy(UBM).Methods Fifty patients(13～40 years)and sixty patients(41～70 years)without primary angle-closure and history of ophthalmologic operation and ocular trauma were randomly selected.The former was adolescent group,while the latter was middle-aged and old age group.Each eye was examined by UBM on the whole circle,and the differences of the morphology of the primary iridociliary cysts between two groups was analyzed.Then the relationship between morphology of the cysts and adjacent anterior chamber angle was compared.When multiple cysts leaded to constriction and closure of two or more than two position of anterior chamber angle,the ratio between the extent of the whole constrictive or closed anterior chamber angle and the circumference of each eye was estimated.Results The incidence of primary iridociliary cysts was 31.8%,the rate of multiple cysts in this group was 62.9%.To adjacent anterior chamber angle,the cyst that was bigger or closer to ciliary groove would have more possibility to lead to constrictive and occlusive adjacent anterior chamber angle(P≤0.05).The incidence of the cysts in the adolescent group was 40.0% while in the middle-aged and old age group that was 25.0%,(P<0.05).The mean diameter of two groups were(0.51±0.54)mm and(0.31±0.17)mm,(P< 0.05).In addition,the rate of multiple cysts was 75.0% in adolescent group while that was 46.6% in the middle-aged and old age group,(P<0.05).In this study,multiple constrictive and occlusive anterior chamber which caused by multiple cysts were all found in adolescent group,and the incidence was 12.5% in adolescent group.Conclusion Compared to middle-aged and old age group,the cysts in the adolescent group have more possibility to lead to multiple constriction and closure of anterior chamber angle.Primary iridociliary cysts may be one of the main causes of secondary angle-constriction and angle-closure in adolescent.

Abstract:Purpose: To analyze primary congenital glaucoma (PCG) anterior chamber and angle anomalies over 360° as possible biomarkers of severity and prognosis. Methods: A cross-sectional observational study was conducted analyzing anterior segment anomalies of PCG patients over 4 years of age who underwent trabeculectomy combined with trabeculotomy and age-matched controls using anterior segment optical coherence tomography (ASOCT), CASIA-2. Anterior iridotrabecular adhesions or anterior iris insertion was identified and quantified from the scleral spur using the iridotrabecular contact (ITC) index parameter as a surrogate. Results: There was a variable but significantly increased anterior iridotrabecular adhesion on ITC index, ITC area, corneal volume, anterior chamber volume, iris volume, anterior chamber depth, and small/absent trabecular meshwork in PCG eyes compared to control eyes. In PCG eyes, anterior iridotrabecular adhesion had a positive correlation with pre-operative central corneal thickness (CCT) (r = 0.53, P = 0.02), review iris thickness (r = 0.4, P = 0.04), and ITC area (r = 0.85, P < 0.001). Review iris thickness had a negative correlation with pre-operative vertical cup-disc ratio (r = -0.51, P = 0.008). Iris hypoplasia with fewer or absent folds, collarette, pupillary ruff, and pupillary ruff to collarette distance was significantly different from controls. Conclusion: ASOCT in PCG eyes has shown that they have variable anterior iridotrabecular tissue adhesions, anomalous tissue/membranes in the angle, and iris hypoplasia correlating with pre-operative cup-disc ratio. These features could be used as gonioscopic and clinical biomarkers to assess the severity and prognosis of the disease. The presence of abnormal iris morphology and iridotrabecular tissue anomalies in PCG suggests that it is more than just isolated trabeculodysgenesis and is probably best considered as part of the anterior segment dysgenesis spectrum.

Abstract:PURPOSE:To compare the features of postiridotomy ultrasound biomicroscopy (UBM) in Chinese patients with acute primary angle-closure (aPAC) and with chronic primary angle-closure (cPAC) glaucoma.PATIENTS AND METHODS:Consecutive cases were classified into acute and chronic groups. The acute group included 77 patients with unilateral aPAC. The chronic group included 57 patients with unilateral advanced cPAC glaucoma. In both the groups, the patients' fellow eye underwent a laser iridotomy and was studied. The main outcome measures included qualitative UBM parameters such as a plateau iris, anterior iris insertion, and an anteriorly rotated ciliary process and quantitative UBM parameters such as central anterior chamber depth (ACD), basal iris thickness (IT500), and scleral ciliary process angle (SCPA).RESULTS:For the qualitative parameters, more eyes in the chronic group had a plateau iris (P=0.046), an anterior iris insertion (P=0.222), and an anteriorly rotated ciliary process (P=0.090) than those in the acute group. For the quantitative parameters, the eyes in the chronic group had a deeper ACD (P<0.001), thicker IT500 (P<0.001), and smaller SCPA (P<0.001) than those in the acute group.CONCLUSIONS:The UBM features of Chinese patients with cPAC include a more plateaued iris, a thicker basal iris, and a smaller SCPA, whereas patients with aPAC may have a shallower ACD. For Chinese patients, a nonpupillary block component may play a more important role in asymptomatic cPAC than in aPAC.

Abstract:Purpose: To investigate the frequency of appositional angle closure and related anatomic characteristics in fellow eyes of Chinese subjects with acute primary angle closure (APAC) after laser peripheral iridotomy (LPI).Design: Cross-sectional study.Participants: Consecutive subjects with APAC presenting from April 2006 to September 2006 at the Glaucoma Service of Peking University Eye Center, Peking University Third Hospital.Methods: Under dark conditions, fellow eyes were divided into 2 groups based on the detection of appositional angle closure by ultrasound biomicroscopy (UBM). For all subjects, UBM parameters were measured to evaluate the different anatomic features between the 2 groups. Darkroom provocative tests (DRPTs) and gonioscopy were performed to investigate the relationship between angle closure and intraocular pressure.Main Outcome Measures: Central anterior chamber depth (ACD); angle opening distance(500) (AOD(500)); angle recess area(750) (ARA(750)); trabecular-iris angle (T-I angle); trabecular-ciliary process distance (TCPD); peripheral iris thickness (IT1); iris-zonule distance (IZD); and the position of the iris insertion.Results: Thirty-four post-LPI fellow eyes of 34 patients with APAC (8 men, 26 women; mean +/- standard deviation age 66.3 +/- 7.2 years, range 54-83) were included. Peripheral anterior synechiae (PAS) were not observed in any subject. Appositional angle closure was observed in at least 1 quadrant in 13 (38.2%) of the 34 patients. Compared with eyes without appositional closure, eyes with appositional closure showed significantly lower AOD(500), ARA(750), and T-I angle in 4 quadrants; shorter TCPD in the inferior and temporal quadrants; and thicker IT1 in the superior and nasal quadrants (P<0.05). The DRPT results were positive in 3 (11.1%) of 27 eyes with appositional closure in 0 to 2 quadrants and in 3 (75.0%) of 4 eyes with appositional closure in 3 to 4 quadrants (P = 0.0164).Conclusions: Under dark conditions, more than one third of fellow eyes of APAC showed appositional angle closure after LPI. The anatomic findings indicate a narrower angle, a more anterior position of the ciliary body, and a thicker peripheral iris in fellow eyes of APAC after LPI may be associated with an increased risk for progressive angle closure.Financial Disclosure(s): The author(s) have no proprietary or commercial interest in any materials discussed in this article. Differences Between Fellow Eyes of Acute and Chronic Primary Angle Closure (glaucoma): an Ultrasound Biomicroscopy Quantitative Study

Abstract:Purpose To compare various biometric parameters between fellow eyes of acute primary angle closure (glaucoma) [APAC(G)] and fellow eyes of chronic primary angle closure (glaucoma) [CPAC(G)]. Methods Ultrasound biomicroscopy examinations were performed on 47 patients with unilateral APAC(G) and 41 patients with asymmetric CPAC(G) before laser peripheral iridotomy and pilocarpine treatment. Anterior chamber depth and width (ACD and ACW), lens vault (LV), iris curvature (IC), iris root distance (IRD), trabecular-ciliary process distance (TCPD), iris-ciliary process distance (ICPD), trabecular-ciliary angle (TCA), and other biometric parameters were compared between fellow eyes of APAC(G) and fellow eyes of CAPC(G). Results Compared with fellow eyes of CPAC(G), fellow eyes of APAC(G) had smaller ACD (P < 0.001), ACW (P = 0.007), TCPD (P = 0.016), ICPD (P = 0.008), and TCA (P = 0.006), as well as larger LV (P = 0.002), IC (P = 0.012), and IRD (P = 0.003). On multivariate logistic regression analyses, a 0.1 mm decrease in ACD (odds ratio [OR]: 0.705, 95% CI: 0.564 +/- 0.880, P = 0.002), ICPD (OR: 0.557, 95% CI: 0.335 +/- 0.925, P = 0.024), and a 0.1 mm increase in IRD (OR: 2.707, 95% CI: 1.025 +/- 7.149, P = 0.045), was significantly associated with occurrence of acute angle closures. Conclusions Fellow eyes of APAC(G) had smaller anterior segment dimensions, higher LV, more posterior iris insertion, greater IC, and more anteriorly rotated ciliary body compared with fellow eyes of CPAC(G). ACD, ICPD, and IRD were the three most important parameters that distinguish eyes predisposed to APAC(G) or CPAC(G).

Abstract:Axenfeld–Rieger Syndrome (ARS) is a rare disease with a wide spectrum of ocular and systemic manifestations. The genetic spectrum of Chinese patients with ARS and genotype-phenotype correlations have yet to be described. To explore the molecular and clinical features in Chinese patients, fifty-five patients clinically diagnosed with ARS from independent families were recruited. Complete ophthalmic examinations and next generation sequencing of anterior segment dysgenesis associated genes were performed in all patients, and segregation in available relatives was verified using Sanger sequencing. 18 FOXC1 variants, 13 PITX2 variants, and two gross deletions spanning FOXC1 were detected in 35 out of 55 (63.6%) patients. 12 FOXC1 variants, 9 PITX2 variants, and two gross deletions were novel. There was a wide range of variability and severity in ocular and systemic manifestations displayed in our patients. Patients with FOXC1 variants were diagnosed at a younger age and had a lower prevalence of systemic manifestations than patients harboring PITX2 variants and those without variants. To our best knowledge, this is the largest study of Chinese patients with ARS to date. Our findings expand the genetic spectrum of ARS and reveal genotype-phenotype correlations in Chinese patients with ARS. Genetic and clinical heterogeneity were present in our patients. Awareness of the extensive characterization may aid in the clinical management and genetic counseling of patients with this rare disease.

Abstract:PURPOSE To determine the prevalence of appositional angle closure (AAC) after laser peripheral iridotomy (LPI) in the eyes of Chinese patients with primary angle closure (PAC) and primary angle closure glaucoma (PACG) and to evaluate its pathogenesis by investigating anatomic characteristics. METHODS This was a cross-sectional observational study. PAC and PACG subjects were consecutively enrolled after LPI. Ultrasound biomicroscopy (UBM) images, obtained in darkness, of each quadrant without peripheral anterior synechia (PAS) under gonioscopy were qualitatively assessed. Darkroom provocative test (DRPT) was also performed. RESULTS A total of 134 eyes of 134 patients were enrolled. AAC was observed in ≥ 1 quadrant of UBM image in 85 subjects (63.4%). There were 116 randomly selected quadrants without PAS for 134 patients (86.6%). AAC existed in 38 quadrants (32.8%). Among these, plateau iris accounted for 44.7%, anteriorly inserted iris for 13.2%, thick iris for 13.2%, and anteriorly inserted iris combined with thick iris for 18.4% of the total. One hundred fifteen patients underwent DRPT and its positive rate of eyes with AAC ≥ 2 quadrants (37.5% [12 of 32 patients]) was significantly higher than those ≤ 1 quadrant (16.9% [14 of 83 patients]; P = 0.018). However, no significant differences were found between eyes with nonsynechia plateau iris ≥ 2 quadrants (36.4% [4 of 11 patients]) and those ≤ 1 quadrant (21.2% [22 of 104 patients]; P = 0.266). CONCLUSIONS Approximately two-thirds of PAC and PACG eyes of Chinese patients after LPI had AAC. Plateau iris accounted for less than 50% of AAC. Other factors such as a thick peripheral iris and an anteriorly inserted iris also contributed to AAC. DRPT results suggested AAC might have more functional meaning than plateau iris.
