Abstract:Background and Aim: Acute graft-versus-host disease (aGVHD) is a life-threatening complication after allogeneic hematopoietic stem cell transplantation (allo-HSCT), particularly if treatment is refractory. Human mesenchymal stromal cells (MSCs) have demonstrated safety and efficacy in steroid refractory acute graft versus host disease, due to their well-described immunosuppressive properties, however, clinical trials have resulted in variable success and an optimal source of MSC has yet to be defined. Based on the importance of maternal-fetal interface immune tolerance, the umbilical cord may provide a superior tissue source of MSC to mediate immunomodulation in aGVHD (SR-aGVHD). This study aimed to investigate the safety and efficacy of MSCs delivered as third-line salvage therapy for SR-aGVHD who had received at least two-lines of steroid-containing immunosuppressive therapy. Method: This phase1b/2, multicenter, open-label clinical trial of a third-party, off-the-shelf preparation of human umbilical cord derived mesenchymal stromal cells (hUC-MSCs) (ChiCTR2200061603) enrolled grades II to IV steroid-refractory aGVHD patients who had been treated with at least two-lines of steroid-containing immunosuppressive therapy. This trial consist of 2 parts：a phase Ib dose escalation part using a standard 3+3 design, which planed biweekly i.v. infusions of hUC-MSCs, at 0.5× 10 6 per kg, 1.0× 10 6per kg and 2.0× 10 6 per kg for 3 weeks, to determine the maximum tolerated dose (MTD) and recommended phase 2 dose (RP2D). Phase II of this work is an expansion cohort study of hUC-MSCs at RP2D 1.0×10 6 per kg with 15 patients. The primary endpoint of the study was safety and RP2D. The key secondary endpoints were efficacy of the overall response rate (ORR) at Day 28 and Day 56 defined per the 2014 NIH consensus criteria. Results: From June 23, 2022, through June 12, 2023, a total of 25 patients were enrolled with the median follow up period of 106 (10,210) days. Of the included 25 cases 11 patients (44.0%) were grade III/IV and 14 patients (56.0%) were grade II aGVHD at enrollment. Median patient age was 37.0 (range:24.5-47.0) years, 44.0% male. hUC-MSCs was well tolerated without dose-limiting toxicities in aforementioned three dosages. The reported adverse events (AEs) potentially associated with hUC-MSCs were hypofibrinogenemia (12.0%), upper respiratory tract infection (4.0%), leukopenia (4.0%) and anemia (8.0%) within the median follow-up period of 106 (10, 210) days. Grade 3 or higher AEs were reported in 6 patients (6/25, 24%) with 11 events, all of them were supposed uncorrelated with hUC-MSC. Three patients unfinished the administration:1 was due to withdrawn ICF; 1 was SAE and 1 was due to participate another trial. Up to date of June 12, 2023, twenty of the initial included 25 patients were still in group. And the RP2D of this trial was identified as 1.0× 10 6 per kg. At Day 28, 20 patients (80.0%, 95% CI:59.3%-93.2%) had an overall response, including 10 (40.0%) with complete responses. And at day 56, only one patient loss response, while another non-response patient at day 28 in the dose of 1.0× 10 6 per kg achieved late responses at day 56 ( Table 1). Responses were observed across skin (19/19, 100%), gastrointestinal tract (7/11,72.7%), and liver (2/2, 100.0%) at day 28. Durable overall response at day 56 was 76% (19/25), Figure 1. The median follow-up time after the initiation of hUC-MSC treatment was 106 (10,210) days with the cutoff date of 180 days after recruitment. The probability of 180-day overall survival (OS) and non-relapse mortality was 88.0% (95% CI, 68.8-97.5%) and 8.0% (95% CI, 1.0-26.0%), respectively. The 180-day cumulative incidence of relapse of primary hematologic disease was 8.0% (95%CI,1.0-26.0%). The percentage of total T lymphocyte, neutrophil, cytokine IL-10, IP-10 were statistically decreased in CR group, while CD8+central memory T cell was significantly increased compared to non-CR groups 24h or 96h after hUC-MSC administration (P<0.05）. Conclusion: Collectively, our results support hUC-MSCs as an effective salvage treatment for SR-aGVHD with safe profile. Abstract:Acute graft-versus-host disease (GVHD) is a frequent and potentially life-threatening complication following allogeneic hematopoietic cell transplantation (HCT). Mesenchymal stromal cells (MSCs), rare precursors found in all body tissues, possess immunosuppressive properties and can inhibit alloreactivity both in vitro and in vivo. Two decades ago, we introduced bone marrow-derived (BM) MSCs as a novel therapy for acute GVHD. While some patients responded to BM-MSCs, the response was not universal. Commercially available BM-MSCs are now used for acute GVHD treatment in Canada, Japan, and New Zealand. The fetus is protected from the mother's immune system by the placenta, and our research found that placenta-derived decidua stromal cells (DSCs) offer a stronger immunosuppressive effect than other sources of stromal cells. Safety studies in rabbits, rats, mice, and humans have shown negligible or no side effects from BM-MSCs or DSCs. In a phase I/II trial for severe acute GVHD, we treated 21 patients (median age, 49 years; range 1.6-72 years) with severe biopsy-proven gastrointestinal acute GVHD. The median cell dose of DSCs was 1.2 x10 6 (range 0.9-2.9) cells/kg body weight, with a median of 2 (range 1-6) infusions given one week apart. DSCs cell viability was 93% (range, 69-100%), and the median cell passage number was 4 (range, 2-4). All patients responded, with a complete response of acute GVHD in 11 patients and partial response in 10 and one-year survival of 81%. Randomized trials are needed to prove the superiority of DSCs compared to Ruxolitinib and/or other novel immunosuppressive therapies.

Abstract:Abstract Severe acute graft-versus-host disease (GVHD) is a life-threatening complication after allogeneic hematopoietic stem cell transplantation (HSCT). The placenta protects the fetus from the mother's immune system. We evaluated placenta-derived decidua stromal cells (DSCs), which differ from bone marrow mesenchymal stromal cells (BM-MSCs), as a treatment for severe acute GVHD. DSCs were obtained from term placentas. The DSCs were given to 38 patients with severe acute GVHD; 25 were steroid refractory (SR). DSCs were thawed and infused in buffer supplemented with either 10% AB plasma (group 1, n = 17), or 5% albumin (group 2, n = 21). The viability of cells was higher when thawed in albumin rather than AB plasma (p < .001). Group 1 received a higher cell dose (p < .001), cells of lower passage number (p < .001), and fewer infusions (p = .002) than group 2. The GVHD response (no/partial/complete) was 7/5/5 in group 1 and 0/10/11 in group 2 (p = .01). One-year survival in the two groups was 47% (95% confidence interval [CI] 23–68) and 76% (95% CI 51–89), respectively (p = .016). For the SR patients, 1-year survival was 73% (95% CI 37–90) in SR group 2 (n = 11), which was better than 31% (95% CI 11–54) in SR group 1 (n = 13; p = .02), 20% (95% CI 5–42) in BM-MSC treated (n = 15; p = .0015), and 3% (95% CI 0–14) in historic controls (n = 32; p < .001). DSCs are a promising new treatment for severe acute GVHD. Prospective randomized trials are needed for evaluation of efficacy. (Clinical trial NCT-02172937.)

Abstract:Acute graft-versus-host disease (GVHD) is a major life-threatening complication after allogeneic hematopoietic cell transplantation (allo-HCT). Traditional standard prophylaxis for acute GVHD has included a calcineurin-inhibitor plus an antimetabolite, while treatment has relied mainly on corticosteroids followed by multiple non-standard second line options. In the last decade this basic framework has been reshaped by approval of ATG products, the emergence of post-transplant cyclophosphamide and recent pivotal trials studying abatacept and vedolizumab for GVHD prophylaxis while ruxolitinib was approved for steroid refractory acute GVHD treatment. Due to this progress, routine acute GVHD prophylaxis and treatment practices are starting to shift and results of ongoing trials are eagerly awaited. Here, we review recent developments in acute GVHD prevention and therapy along with ongoing and future planned clinical trials in this space, outlining what future goals should be and the limitations of current clinical trial designs and endpoints.

Abstract:Acute and chronic graft versus host disease (GVHD) remain the major barriers to successful hematopoietic cell transplantation. The induction of GVHD may be divided into three phases: (i) recipient conditioning; (ii) donor T-cell activation; and (iii) effector cells mediating GVHD. This review examines GVHD prevention and treatment using this conceptual model as framework.The various pharmacological agents discussed impact on different phases of the GVHD cascade. For example, keratinocyte growth factor and interleukin (IL)-11 are cytokines that may be useful in disrupting phase I of the GVHD cascade by blocking gastrointestinal tract damage, and lowering serum levels of lipopolysaccharide and tumour necrosis factor (TNF)-α. Cyclosporin, tacrolimus (FK-506) and sirolimus (rapamycin) are some of the main agents that disrupt phase II (donor T-cell activation). Mycophenolate mofetil and tresperimus probably act on this phase as well. Other novel drugs that affect phase II are tolerance-induction agents such as CTLA-4 and anti-CD40-ligand monoclonal antibodies, and preliminary results using CTLA-4 monoclonal antibody in GVHD prevention are encouraging. Examples of agents that disrupt phase III are the IL-2 receptor antagonist daclizumab and the anti-TNFα monoclonal antibody infliximab. These anti-cytokine antibodies have shown promising results in early studies.The most effective approach to GVHD prevention will probably be a combination regimen where the three phases of the GVHD cascade are disrupted. Once GVHD has occurred, all three phases of the cascade are activated. Developments of combination therapy for treatment of both acute and chronic GVHD are likely to yield better results than monotherapy The numerous new treatment modalities presented should improve the outlook for patients with acute and chronic GVHD.

Dupuytren's Disease in the Hispanic Population: A 10-Year Retrospective Review

Abstract:Background: Dupuytren's disease is a common benign fibroproliferative disorder of the hand. Epidemiologic studies have reported significant variation in disease prevalence among races, focusing primarily on those of northern European descent. In contrast, Dupuytren's disease in the Hispanic population has received little attention. Thus, in this study, the authors aimed to determine the prevalence and operative rate of Dupuytren's disease in the Hispanic, black, white, Asian, Native American, and other races and to characterize the disease presentation in Hispanics who required surgical treatment. Methods: A retrospective review was conducted to identify the racial distribution of Dupuytren's disease patients seen at Bellevue Hospital between July of 2000 and August of 2010. In Hispanic patients requiring surgical treatment for their disease, data were collected on the following parameters: age, sex, ethnicity, hand dominance, hand affected, and digits operated on. Epidemiologic factors including smoking, alcoholism, diabetes mellitus, hypercholesterolemia, epilepsy, and hypertension were also evaluated. Results: Dupuytren's disease prevalence was found to be 533 per 100,000 in Hispanics. Of these patients, 1.8 percent required surgical treatment, and this group was characterized by the following comorbidities: smoking (57.1 percent), hypertension (57.1 percent), alcoholism (52.4 percent), diabetes mellitus (47.6 percent), and hypercholesterolemia (19.0 percent). Conclusions: The authors' results indicate that Dupuytren's disease is more prevalent in the Hispanic population than previously reported. Although the epidemiologic factors identified in Hispanics with Dupuytren's disease are similar to those in other races investigated, there are important differences with respect to clinical presentation and surgical treatment.

Abstract:Abstract Acute Graft versus Host Disease (aGvHD) is a common immunological complication occurring in patients undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT). Moreover, aGvHD is associated with a higher risk of infections and metabolic complications, affecting non-relapse mortality. Progress in transplantation has changed the prophylactic and therapeutic strategies of aGvHD and improved patient outcomes. The standard first-line therapy remains steroids, with a response rate of about 50%. The Janus Kinase 2 (JAK2) inhibitor, ruxolitinib, is an effective second-line therapy. The management of patients who developed a disease that is refractory to steroids and ruxolitinib, especially in the severe gastrointestinal forms of aGvHD, is not validated and remains an unmet medical need. In the article, we present the current clinical practice, as well as the latest advances targeting pathophysiological pathways of GvHD and gut microbiota, which may be a potential future of aGvHD therapy.

Abstract:Acute and chronic graft-vs.-host disease (aGvHD and cGvHD) are major complications after allogeneic hematopoietic cell transplantation (HCT) leading to substantial morbidity and mortality. This retrospective single-center study analyzes incidence, therapy, and outcome of GvHD in n = 721 patients ≥18 years having received allogeneic HCT 2004–2013 with a special focus on steroid refractory GvHD. Acute (n = 355/49.2%) and chronic (n = 269/37.3%) GvHD were mainly treated by steroids in first-line therapy. The proportion of steroid refractory aGvHD and cGvHD was 35.7% and 31.4%, respectively. As there is no standard therapy for steroid refractory GvHD, a range of different agents was used. In aGvHD, the overall response rate (ORR) of steroid refractory GvHD to second-line treatment was 27.4%. Mycophenolate mofetil (MMF) and mTOR inhibitors led to superior response rates (ORR 50.0% and 53.3%, respectively). In steroid refractory cGvHD therapy, ORR was 44.4%. Use of calcineurin inhibitors (CNI; n = 11/45.5%), MMF (n = 18/50.0%), mTOR inhibitors (n = 10/60.0%), and extracorporeal photophoresis (ECP; n = 16/56.3%) showed ORR above average. Targeted therapies lead to responses in 7.7% (n = 13). This data may help to improve the design of future prospective clinical studies in GvHD.

Abstract:Graft-versus-host disease (GVHD) is a serious and deadly complication of patients, who undergo hematopoietic stem cell transplantation (HSCT). Despite prophylactic treatment with immunosuppressive agents, 20-80% of recipients develop acute GVHD after HSCT. And the incidence rates of chronic GVHD range from 6 to 80%. Standard therapeutic strategies are still lacking, although considerable advances have been gained in knowing of the predisposing factors, pathology, and diagnosis of GVHD. Targeting immune cells, such as regulatory T cells, as well as tolerogenic dendritic cells or mesenchymal stromal cells (MSCs) display considerable benefit in the relief of GVHD through the deletion of alloactivated T cells. Monoclonal antibodies targeting cytokines or signaling molecules have been demonstrated to be beneficial for the prevention of GVHD. However, these remain to be verified in clinical therapy. It is also important and necessary to consider adopting individualized treatment based on GVHD subtypes, pathological mechanisms involved and stages. In the future, it is hoped that the identification of novel therapeutic targets and systematic research strategies may yield novel safe and effective approaches in clinic to improve outcomes of GVHD further. In this article, we reviewed the current advances in targeted immunotherapy for the prevention of GVHD.

Abstract:Graft-versus-host disease (GVHD) is a significant cause of morbidity and mortality after allogeneic hematopoietic stem cell transplantation (HSCT). For many years, corticosteroids have been the mainstay treatment for GVHD, but cases of steroid-refractory GVHD and the severe adverse effects of high-dose corticosteroids have increased the need for preventative and therapeutic strategies for GVHD. Due to the nature of alloreactive T cells, GVHD is inherently linked to the graft-versus-leukemia (GVL) effect, the therapeutic driving force behind stem cell transplantation. A considerable clinical challenge is to preserve GVL while suppressing GVHD. The field of GVHD research has greatly expanded over the past decades, including advancements in T cell modulation and depletion, antibody therapies, chemotherapeutics, cellular therapies, and Janus kinase inhibition. In this review, we discuss current approaches and advances in the prophylaxis and treatment of GVHD with a focus on new emerging advancements in Janus kinase inhibitor therapy.

Abstract:Abstract Acute graft-versus-host disease (aGVHD) is a common complication of allogeneic hematopoietic stem cell transplantation (alloHCT) and is a major cause of morbidity and mortality. Systemic steroid therapy is the first-line treatment for aGVHD, although about half of patients will become refractory to treatment. As the number of patients undergoing alloHCT increases, developing safe and effective treatments for aGVHD will become increasingly important, especially for those whose disease becomes refractory to systemic steroid therapy. Abstract:INTRODUCTION: Allogeneic hematopoietic stem cell transplantation (alloHSCT) offers potential curative treatment for a wide range of malignant and nonmalignant hematological disorders. However, its success may be limited by post-transplant acute graft-versus-host disease (aGVHD), a systemic syndrome in which donor's immune cells attack healthy tissues in the immunocompromised host. aGVHD is one of the main causes of morbidity and mortality after alloHSCT. Despite standard GVHD prophylaxis regimens, aGVHD still develops in approximately 40-60% of alloHSCT recipients.AREAS COVERED: In this review, after a brief summary of current knowledge on the pathogenesis of aGVHD, the authors review the current combination of a calcineurin inhibitor with an antimetabolite with or without added anti-thymocyte globulin (ATG) and emerging strategies for GVHD prevention.EXPERT OPINION: A new understanding of the involvement of cytokines, intracellular signaling pathways, epigenetics and immunoregulatory cells in GVHD pathogenesis will lead to new standards for aGVHD prophylaxis allowing better prevention of severe aGVHD without affecting graft-versus-tumor effects.

Abstract:The Oncology Grand Rounds series is designed to place original reports published in the Journal into clinical context. A case presentation is followed by a description of diagnostic and management challenges, a review of the relevant literature, and a summary of the authors' suggested management approaches. The goal of this series is to help readers better understand how to apply the results of key studies, including those published in Journal of Clinical Oncology, to patients seen in their own clinical practice.Chronic graft-versus-host disease (cGVHD) is an immune-mediated disease that ensues after allogeneic hematopoietic cell transplantation often affecting multiple organs, decreases quality of life, and necessitates immunosuppressive therapy. The current standard first-line cGVHD therapy is glucocorticoids (GCs) which have major side effects. So far, all attempts to improve response to first-line GC therapy of cGVHD by adding other agents, such as mycophenolate mofetil, or cyclosporine A have not improved response. The data reported in the trial by Miklos et al indicate that ibrutinib should not be added to GC in first-line cGVHD therapy. The findings instruct future studies that aim at the identification of patients with cGVHD who may benefit from a GC-ibrutinib combination using biomarkers or clinical parameters. Currently, ibrutinib, ruxolitinib, and belumosudil are the only drugs that are approved by the Food and Drug Administration for steroid-refractory cGVHD. In light of the current standard management of cGVHD, the trial results indicate that ibrutinib will remain reserved for second-line therapy, which may be due to its mechanisms of action. The recommendation on the basis of this trial is that GC monotherapy remains the standard therapy for cGVHD.

Abstract:Acute graft-versus-host disease (GVHD) is a significant cause of morbidity and mortality following allogeneic hematopoietic cell transplantation (HCT). This analysis of 168 patients (mean age, 54.8 years) from a multicenter, retrospective chart review describes the clinical course, treatment patterns, hospitalizations, and clinical outcomes of patients aged ≥12 years who developed grades II-IV acute GVHD after their first allogeneic HCT (January 1, 2014, to June 30, 2016) and were refractory to or dependent on corticosteroids. Between diagnosis and maximum grade (median, 6.0 days), 53.6% of patients had new organ involvement, particularly lower gastrointestinal tract acute GVHD, or an increase in acute GVHD grade. Eighty-nine patients (53.0%) received additional systemic GVHD therapy (after systemic corticosteroids) within a median of 21.0 days. Hospital readmission(s) was required for 56.5% of patients within 100 days post-HCT (mean inpatient length of readmission stay, 49.5 days); 24.4% had ≥2 readmissions within 100 days post-HCT. From the date of acute GVHD diagnosis, 70.2% of patients died at a median (interquartile range) of 117.5 (49-258) days. In summary, steroid-refractory and steroid-dependent acute GVHD is associated with a rapidly worsening clinical course that leads to high readmission and mortality rates, emphasizing the need for effective and tolerable therapies.

Abstract:Whereas aGVHD has strong inflammatory components, cGVHD displays autoimmune and fibrotic features; incidence and risk factors are similar but not identical; indeed, the aGVHD is the main risk factor for cGVHD. Calcineurin Inhibitors (CNI) with either Methotrexate (MTX) or Mycophenolate (MMF) still represent the standard prophylaxis in HLA-matched allogeneic stem cell transplantation (HSCT); other strategies focused on ATG, Post-Transplant Cyclophosphamide (PTCy), Abatacept and graft manipulation. Despite the high rate, first-line treatment for aGVHD is represented by corticosteroids, and Ruxolitinib is the standard second-line therapy; investigational approaches include Microbiota transplant and the infusion of Mesenchymal stem cells. GVHD is a pleiotropic disease involving any anatomical district; also, Ruxolitinib represents the standard for steroid-refractory cGVHD in this setting. Extracorporeal Photopheresis (ECP) is still an option used for steroid refractoriness or to achieve a steroid-sparing. For Ruxolitinib-refractory cGVHD, Belumosudil and Axatilimab represent the most promising agents. Bronchiolitis obliterans syndrome (BOS) still represents a challenge; among the compounds targeting non-immune effectors, Alvelestat, a Neutrophil elastase inhibitor, seems promising in BOS. Finally, in both aGVHD and cGVHD, the association of biological markers with specific disease manifestations could help refine risk stratification and the availability of reliable biomarkers for specific treatments.

Abstract:Allogeneic hematopoietic stem cell transplantation (allo-HSCT) has been widely used to treat hematological malignancies and genetic diseases. Graft-versus-host disease (GVHD) induced by donor immune system is the most common complication, contributing to severe morbidity and mortality after allo-HSCT. Currently, in terms of the prevention and treatment of GVHD, the major first-line therapeutic drugs are corticosteroids. However, most patients with systemic corticosteroid treatment are prone to steroid-refractory and poor prognosis. The use of several immune cells including Tregs, Bregs and mesenchymal stromal cells (MSCs) as an alternative on prevention or therapy of GVHD has been demonstrated to be beneficial. However, there are still many defects to a certain degree. Based on immune cells, it is promising to develop new and better approaches to improve GVHD. In this article, we will review the current advance of immune cells (Tregs, Bregs, MSCs) with negative regulation in the treatment of GVHD and present emerging strategies for the prevention and treatment of GVHD by other immune regulatory cells and chimeric antigen receptor (CAR) Tregs. In addition, these new therapeutic options need to be further evaluated in well-designed prospective multicenter trials to determine the optimal treatment for GVHD patients and improve their prognosis.
