Abstract:Background: Infants born at extremely preterm gestational ages are typically admitted to the neonatal intensive care unit (NICU) after initial resuscitation. The subsequent hospital course can be highly variable, and despite counseling aided by available risk calculators, there are significant challenges with shared decision-making regarding life support and transition to end-of-life care. Improving predictive models can help providers and families navigate these unique challenges. Objective: Machine learning methods have previously demonstrated added predictive value for determining intensive care unit outcomes, and their use allows consideration of a greater number of factors that potentially influence newborn outcomes, such as maternal characteristics. Machine learning–based models were analyzed for their ability to predict the survival of extremely preterm neonates at initial admission. Methods: Maternal and newborn information was extracted from the health records of infants born between 23 and 29 weeks of gestation in the Medical Information Mart for Intensive Care III (MIMIC-III) critical care database. Applicable machine learning models predicting survival during the initial NICU admission were developed and compared. The same type of model was also examined using only features that would be available prepartum for the purpose of survival prediction prior to an anticipated preterm birth. Features most correlated with the predicted outcome were determined when possible for each model. Results: Of included patients, 37 of 459 (8.1%) expired. The resulting random forest model showed higher predictive performance than the frequently used Score for Neonatal Acute Physiology With Perinatal Extension II (SNAPPE-II) NICU model when considering extremely preterm infants of very low birth weight. Several other machine learning models were found to have good performance but did not show a statistically significant difference from previously available models in this study. Feature importance varied by model, and those of greater importance included gestational age; birth weight; initial oxygenation level; elements of the APGAR (appearance, pulse, grimace, activity, and respiration) score; and amount of blood pressure support. Important prepartum features also included maternal age, steroid administration, and the presence of pregnancy complications. Conclusions: Machine learning methods have the potential to provide robust prediction of survival in the context of extremely preterm births and allow for consideration of additional factors such as maternal clinical and socioeconomic information. Evaluation of larger, more diverse data sets may provide additional clarity on comparative performance. Trial Registration:

Abstract:Objective: There is a scarcity in tools to predict postpartum depression (PPD). We propose a machine learning framework for PPD risk prediction using data extracted from electronic health records (EHRs). Methods: Two EHR datasets containing data on 15,197 women from 2015 to 2018 at a single site, and 53,972 women from 2004 to 2017 at multiple sites were used as development and validation sets, respectively, to construct the PPD risk prediction model. The primary outcome was a diagnosis of PPD within 1 year following childbirth. A framework of data extraction, processing, and machine learning was implemented to select a minimal list of features from the EHR datasets to ensure model performance and to enable future point-of-care risk prediction. Results: The best-performing model uses from clinical features related to mental health history, medical co morbidity, obstetric complications, medication prescription orders, and patient demographic characteristics. The model performances as measured by area under the receiver operating characteristic curve (AUC) are 0.937 (95% CI 0.912 - 0.962) and 0.886 (95% CI 0.879-0.893) in the development and validation datasets, respectively. The model performances were consistent when tested using data ending at multiple time periods during pregnancy and at childbirth. Limitations: The prevalence of PPD in the study data represented a treatment prevalence and is likely lower than the illness prevalence. Conclusions: EHRs and machine learning offer the ability to identify women at risk for PPD early in their pregnancy. This may facilitate scalable and timely prevention and intervention, reducing negative outcomes and the associated burden.

Abstract:Multiunit activity of paraventricular nucleus and fornix was studied during activation and inhibition of the pituitary--adrenocortical system in chronic experiments on rabbits. Immobilization causing blood corticosteroid levels to rise was followed by the excitation of the neurons in paraventricular nucleus and by inhibition in the fornix. Administration of cortisol (100 micrograms/kg before the immobilization) causing inhibition of pituitary--adrenocortical system was followed by the inhibition of neurons in paraventricular nucleus and fornix. The activation of the paraventricular nucleus seems to be responsible for the activation of the pituitary--adrenocortical system whereas its inhibition induced the inhibition of this system.
Prediction Analysis of Preterm Neonates Mortality using Machine Learning Algorithms via Python Programming

Abstract:Prediction analysis of preterm neonate mortality is necessary and significant for benchmarking and evaluating healthcare services in Hospitals and other medical centers. Application of artificial intelligence and machine learning models, which is a hot topic in medicine/healthcare and engineering, may improve physicians skill to predict the preterm neonatal deaths. The main purpose of this research article is to introduce a preterm neonatal mortality risk prediction by means of machine learning/ML predictive models to survive infants using supervised ML models if possible. Moreover, this paper presents some effective parameters and features which affect to survive the infants directly. It means, the obtained model has an accuracy of about 91.5% to predict the status of infant after delivery. After recognizing the critical status for an infant, physicians and other healthcare personnel can help to infant for possible surviving using special medical NICU cares. It has been tried to get some suitable models with high accuracy and comparing the results. In a word, a survival prediction analysis of preterm neonate mortality has been carried out using machine learning methods via Python programming (possible surviving infants after delivery in the hospital).

Abstract:Splenosis represents the autotransplantation of splenic tissue after splenic trauma or surgery. Disruption of the splenic capsule causes fragments of splenic tissue to be seeded mainly throughout the peritoneal cavity, where they are characterized by diffusely scattered bluish implants. Extraperitoneal locations are very rare and mainly include the thoracic cavity after thoracoabdominal trauma with simultaneous splenic rupture and diaphragmatic laceration. We retrospectively identified all patients in the pathology registry with the diagnosis of splenosis between December 1974 and July 2003 at our urban teaching hospital. Data collected included presenting signs and symptoms, history, imaging studies, treatment, pathology, and outcome. Five cases of splenosis were identified and described. Location of the splenosis was intraperitoneal in two and intrahepatic, intrathoracic, and subcutaneous in one each. In these cases, there was an average interval of 29 years between splenic injury and diagnosis, and most were found incidentally. One of the cases was managed entirely laparoscopically and another thoracoscopically.

Abstract:Background Machine learning could be used for prognosis/diagnosis of maternal and neonates' diseases by analyzing the data sets and profiles obtained from a pregnant mother. Purpose We aimed to develop a prediction model based on machine learning algorithms to determine important maternal characteristics and neonates' anthropometric profiles as the predictors of neonates' health status. Methods This study was conducted among 1280 pregnant women referred to healthcare centers to receive antenatal care. We evaluated several machine learning methods, including support vector machine (SVM), Ensemble, K-Nearest Neighbor (KNN), Naïve Bayes (NB), and Decision tree classifiers, to predict newborn health state. Results The minimum redundancy-maximum relevance (MRMR) algorithm revealed that variables, including head circumference of neonates, pregnancy intention, and drug consumption history during pregnancy, were top-scored features for classifying normal and unhealthy infants. Among the different classification methods, the SVM classifier had the best performance. The average values of accuracy, precision, recall, F1-score, and area under the receiver operating characteristic curve (AUC) in the test group were 75%, 75%, 76%, 76%, and 65%, respectively, for SVM model. Conclusion Machine learning methods can efficiently forecast the neonate's health status among pregnant women. This study proposed a new approach toward the integration of maternal data and neonate profiles to facilitate the prediction of neonates' health status.

Abstract:Rkp1/Cpc2, a fission yeast RACK1 homolog, interacts with Pck2, a PKC homolog, and is involved in the regulation of pck2-mediated signaling process. The N-terminal region of split pleckstrin homology domain (nPH) in human PLC-gamma1 bound to Rkp1/Cpc2 concomitantly with Pck2. nPH inhibited kinase activity of GST-Pck2 purified from Schizosaccharomyces pombe in vitro. The lethality induced by pck2(+) overexpression was suppressed by coexpression of either rkp1(+) or nPH domain. This result suggests that Rkp1/Cpc2 interacts with PH domain-containing protein and regulates the Pck2-mediated signaling process in S. pombe.

Abstract:Introduction To develop and validate a support tool for healthcare providers, enabling them to make precise and critical decisions regarding intensive care unit (ICU) admissions for high‐risk pregnant women, thus enhancing maternal outcomes. Methods This retrospective study involves secondary data analysis of information gathered from 9550 pregnant women, who had severe maternal morbidity (any unexpected complication during labor and delivery that leads to substantial short‐term or long‐term health issues for the mother), collected between 2009 and 2010 from the Brazilian Network for Surveillance of Severe Maternal Morbidity, encompassing 27 obstetric reference centers in Brazil. Machine‐learning models, including decision trees, Random Forest, Gradient Boosting Machine (GBM), and Extreme Gradient Boosting (XGBoost), were employed to create a risk prediction tool for ICU admission. Subsequently, sensitivity analysis was conducted to compare the accuracy, predictive power, sensitivity, and specificity of these models, with differences analyzed using the Wilcoxon test. Results The XGBoost algorithm demonstrated superior efficiency, achieving an accuracy rate of 85%, sensitivity of 42%, specificity of 97%, and an area under the receiver operating characteristic curve of 86.7%. Notably, the estimated prevalence of ICU utilization by the model (11.6%) differed from the prevalence of ICU use from the study (21.52%). Conclusion The developed risk engine yielded positive results, emphasizing the need to optimize intensive care bed utilization and objectively identify high‐risk pregnant women requiring these services. This approach promises to enhance the effective and efficient management of pregnant women, particularly in resource‐constrained regions worldwide. By streamlining ICU admissions for high‐risk cases, healthcare providers can better allocate critical resources, ultimately contributing to improved maternal health outcomes.

Abstract:Machine learning technologies and translation of artificial intelligence tools to enhance the patient experience are changing obstetric and maternity care. An increasing number of predictive tools have been developed with data sourced from electronic health records, diagnostic imaging and digital devices. In this review, we explore the latest tools of machine learning, the algorithms to establish prediction models and the challenges to assess fetal well-being, predict and diagnose obstetric diseases such as gestational diabetes, pre-eclampsia, preterm birth and fetal growth restriction. We discuss the rapid growth of machine learning approaches and intelligent tools for automated diagnostic imaging of fetal anomalies and to asses fetoplacental and cervix function using ultrasound and magnetic resonance imaging. In prenatal diagnosis, we discuss intelligent tools for magnetic resonance imaging sequencing of the fetus, placenta and cervix to reduce the risk of preterm birth. Finally, the use of machine learning to improve safety standards in intrapartum care and early detection of complications will be discussed. The demand for technologies to enhance diagnosis and treatment in obstetrics and maternity should improve frameworks for patient safety and enhance clinical practice.
Prediction of Intrauterine Growth Restriction and Preeclampsia Using Machine Learning-Based Algorithms: A Prospective Study

Abstract:(1) Background: Prenatal care providers face a continuous challenge in screening for intrauterine growth restriction (IUGR) and preeclampsia (PE). In this study, we aimed to assess and compare the predictive accuracy of four machine learning algorithms in predicting the occurrence of PE, IUGR, and their associations in a group of singleton pregnancies; (2) Methods: This observational prospective study included 210 singleton pregnancies that underwent first trimester screenings at our institution. We computed the predictive performance of four machine learning-based methods, namely decision tree (DT), naïve Bayes (NB), support vector machine (SVM), and random forest (RF), by incorporating clinical and paraclinical data; (3) Results: The RF algorithm showed superior performance for the prediction of PE (accuracy: 96.3%), IUGR (accuracy: 95.9%), and its subtypes (early onset IUGR, accuracy: 96.2%, and late-onset IUGR, accuracy: 95.2%), as well as their association (accuracy: 95.1%). Both SVM and NB similarly predicted IUGR (accuracy: 95.3%), while SVM outperformed NB (accuracy: 95.8 vs. 94.7%) in predicting PE; (4) Conclusions: The integration of machine learning-based algorithms in the first-trimester screening of PE and IUGR could improve the overall detection rate of these disorders, but this hypothesis should be confirmed in larger cohorts of pregnant patients from various geographical areas.

Abstract:Introduction: Preeclampsia, one of the leading causes of maternal and fetal morbidity and mortality, demands accurate predictive models for the lack of effective treatment. Predictive models based on machine learning algorithms demonstrate promising potential, while there is a controversial discussion about whether machine learning methods should be recommended preferably, compared to traditional statistical models. Methods: We employed both logistic regression and six machine learning methods as binary predictive models for a dataset containing 733 women diagnosed with preeclampsia. Participants were grouped by four different pregnancy outcomes. After the imputation of missing values, statistical description and comparison were conducted preliminarily to explore the characteristics of documented 73 variables. Sequentially, correlation analysis and feature selection were performed as preprocessing steps to filter contributing variables for developing models. The models were evaluated by multiple criteria. Results: We first figured out that the influential variables screened by preprocessing steps did not overlap with those determined by statistical differences. Secondly, the most accurate imputation method is K-Nearest Neighbor, and the imputation process did not affect the performance of the developed models much. Finally, the performance of models was investigated. The random forest classifier, multi-layer perceptron, and support vector machine demonstrated better discriminative power for prediction evaluated by the area under the receiver operating characteristic curve, while the decision tree classifier, random forest, and logistic regression yielded better calibration ability verified, as by the calibration curve. Conclusion: Machine learning algorithms can accomplish prediction modeling and demonstrate superior discrimination, while Logistic Regression can be calibrated well. Statistical analysis and machine learning are two scientific domains sharing similar themes. The predictive abilities of such developed models vary according to the characteristics of datasets, which still need larger sample sizes and more influential predictors to accumulate evidence.

Developing machine learning models for prediction of mortality in the medical intensive care unit

Abstract:Background and objective: Alert of patient deterioration is essential for prompt medical intervention in the Medical Intensive Care Unit (MICU). Logistic Regression (LR) has been used for the development of most conventional severity-of-illness scoring systems to anticipate the risk of mortality in the MICU. Machine Learning (ML) models such as probabilistic graphical models and Extreme Gradient Boosting (XGB) have demonstrated improved prediction accuracy in patient outcomes compared to LR. The aim was to compare three ML models to the SAPS, SAPS II, SAPS III, SOFA, serial SOFA, LODS, and OASIS for prediction of MICU mortality. Methods: A Bayesian Network (BN), Naïve Bayes network (NB), and a XGB model were developed. 9893 adult MICU-stays from the MIMIC-III database were studied. The primary outcome was MICU mortality prediction and the secondary outcome was 1-year mortality prediction. Data analyzed consisted on routine physiological measurements collected during 5 hours in the MICU, demographic and diagnoses/procedure features. The performance was evaluated by accuracy statistics, discrimination and calibration measures. Limitations of the study were discussed. Results: The AUROC for MICU mortality prediction was 0.919 for XGB, 0.905 for BN, and 0.864 for NB, while the conventional systems displayed much lower values with the serial SOFA having the best value (0.814). The Diagnostic Odds Ratio was ≤7.099 for all the conventional systems, reaching values of 30.115 for XGB and 22.648 for BN. The XGB achieved a sensitivity of 0.831 and specificity of 0.86 assuring an acceptable precision (0.528), whose values were much lower for the conventional systems. The Brier score was better for the ML models, except for the NB (0.119), with 0.072 for XGB and 0.081 for BN. Conclusions: The XGB and BN substantially outperformed the conventional systems for discrimination, calibration and the accuracy statistics assessed. The NB showed inferior performance to the XGB and BN but improved the discrimination and all accuracy statistics of the conventional systems except for an inferior calibration and 1-year mortality discrimination. The XGB showed the best performance among all models. These ML models have the potential to improve the monitoring of MICU patients, which must be evaluated in future studies.
Machine Learning Models for Prediction of Maternal Hemorrhage and Transfusion: Model Development Study

Abstract:Background: Current postpartum hemorrhage (PPH) risk stratification is based on traditional statistical models or expert opinion. Machine learning could optimize PPH prediction by allowing for more complex modeling. Objective: We sought to improve PPH prediction and compare machine learning and traditional statistical methods. Methods: We developed models using the Consortium for Safe Labor data set (2002-2008) from 12 US hospitals. The primary outcome was a transfusion of blood products or PPH (estimated blood loss of ≥1000 mL). The secondary outcome was a transfusion of any blood product. Fifty antepartum and intrapartum characteristics and hospital characteristics were included. Logistic regression, support vector machines, multilayer perceptron, random forest, and gradient boosting (GB) were used to generate prediction models. The area under the receiver operating characteristic curve (ROC-AUC) and area under the precision/recall curve (PR-AUC) were used to compare performance. Results: Among 228,438 births, 5760 (3.1%) women had a postpartum hemorrhage, 5170 (2.8%) had a transfusion, and 10,344 (5.6%) met the criteria for the transfusion-PPH composite. Models predicting the transfusion-PPH composite using antepartum and intrapartum features had the best positive predictive values, with the GB machine learning model performing best overall (ROC-AUC=0.833, 95% CI 0.828-0.838; PR-AUC=0.210, 95% CI 0.201-0.220). The most predictive features in the GB model predicting the transfusion-PPH composite were the mode of delivery, oxytocin incremental dose for labor (mU/minute), intrapartum tocolytic use, presence of anesthesia nurse, and hospital type. Conclusions: Machine learning offers higher discriminability than logistic regression in predicting PPH. The Consortium for Safe Labor data set may not be optimal for analyzing risk due to strong subgroup effects, which decreases accuracy and limits generalizability.

Abstract:Background Globally, pre-eclampsia (PE) is a leading cause of maternal and perinatal morbidity and mortality. PE prediction using routinely collected data has the advantage of being widely applicable, particularly in low-resource settings. Early intervention for high-risk women might reduce PE incidence and related complications. We aimed to replicate our machine learning (ML) published work predicting another maternal condition (gestational diabetes) to (1) predict PE using routine health data, (2) identify the optimal ML model, and (3) compare it with logistic regression approach. Methods Data were from a large health service network with 48,250 singleton pregnancies between January 2016 and June 2021. Supervised ML models were employed. Maternal clinical and medical characteristics were the feature variables (predictors), and a 70/30 data split was used for training and testing the model. Predictive performance was assessed using area under the curve (AUC) and calibration plots. Shapley value analysis assessed the contribution of feature variables. Results The random forest approach provided excellent discrimination with an AUC of 0.84 (95% CI: 0.82–0.86) and highest prediction accuracy (0.79); however, the calibration curve (slope of 1.21, 95% CI 1.13–1.30) was acceptable only for a threshold of 0.3 or less. The next best approach was extreme gradient boosting, which provided an AUC of 0.77 (95% CI: 0.76–0.79) and well-calibrated (slope of 0.93, 95% CI 0.85–1.01). Logistic regression provided good discrimination performance with an AUC of 0.75 (95% CI: 0.74–0.76) and perfect calibration. Nulliparous, pre-pregnancy body mass index, previous pregnancy with prior PE, maternal age, family history of hypertension, and pre-existing hypertension and diabetes were the top-ranked features in Shapley value analysis. Conclusion Two ML models created the highest-performing prediction using routinely collected data to identify women at high risk of PE, with acceptable discrimination. However, to confirm this result and also examine model generalisability, external validation studies are needed in other settings, utilising standardised prognostic factors.

Abstract:Background: The COVID-19 pandemic has affected people of all ages and backgrounds, but pregnant women are a particularly vulnerable group. They are at increased risk of severe illness and death from COVID-19, yet there is limited and conflicting data about the factors that contribute to mortality in this population. In this study, we sought to use machine learning (ML) to predict the possible factors associated with mortality in pregnant women with COVID-19. Methods: We collected data from a large cohort of pregnant women who had been diagnosed with COVID-19 at Hung Vuong Hospital. We included a range of demographic, clinical, and laboratory variables in our analysis, such as age, gestational age, comorbidities, symptoms, and laboratory test results. Our goal was to identify the critical factors that could be used to predict mortality in this vulnerable population. We applied several ML models and analyzed the results to determine the most useful predictors. Results: We analyzed a cohort of 550 pregnant women diagnosed with COVID-19, comprising 525 survivors and 25 deaths. Using ML algorithms, we identified key clinical and patient factors that correlated with mortality risk, such as infection severity, pulse rate, breathing patterns, SpO2 levels, rapid diagnostic tests, internal pathology, breathing difficulties, and changes in consciousness. Additionally, several medical test factors were found to be significant, including white blood cells, neutrophils, platelets, C-reactive protein (CRP), procalcitonin, and chest X-ray abnormalities. Our analysis revealed that the Gaussian Naive Bayes (NB) model was the most accurate, with an AUC score of 0.79, demonstrating its potential as a valuable tool for identifying high-risk patients. Conclusions: Our results suggest that the available data can capture the relationship between COVID-19 symptoms and mortality in pregnant women. More studies are needed to improve the accuracy and generalizability of models for predicting mortality risk in this population.

Abstract:Background Early and accurate identification of sepsis patients with high risk of in-hospital death can help physicians in intensive care units (ICUs) make optimal clinical decisions. This study aimed to develop machine learning-based tools to predict the risk of hospital death of patients with sepsis in ICUs. Methods The source database used for model development and validation is the medical information mart for intensive care (MIMIC) III. We identified adult sepsis patients using the new sepsis definition Sepsis-3. A total of 86 predictor variables consisting of demographics, laboratory tests and comorbidities were used. We employed the least absolute shrinkage and selection operator (LASSO), random forest (RF), gradient boosting machine (GBM) and the traditional logistic regression (LR) method to develop prediction models. In addition, the prediction performance of the four developed models was evaluated and compared with that of an existent scoring tool – simplified acute physiology score (SAPS) II – using five different performance measures: the area under the receiver operating characteristic curve (AUROC), Brier score, sensitivity, specificity and calibration plot. Results The records of 16,688 sepsis patients in MIMIC III were used for model training and test. Amongst them, 2949 (17.7%) patients had in-hospital death. The average AUROCs of the LASSO, RF, GBM, LR and SAPS II models were 0.829, 0.829, 0.845, 0.833 and 0.77, respectively. The Brier scores of the LASSO, RF, GBM, LR and SAPS II models were 0.108, 0.109, 0.104, 0.107 and 0.146, respectively. The calibration plots showed that the GBM, LASSO and LR models had good calibration; the RF model underestimated high-risk patients; and SAPS II had the poorest calibration. Conclusion The machine learning-based models developed in this study had good prediction performance. Amongst them, the GBM model showed the best performance in predicting the risk of in-hospital death. It has the potential to assist physicians in the ICU to perform appropriate clinical interventions for critically ill sepsis patients and thus may help improve the prognoses of sepsis patients in the ICU.
Early prediction of need for invasive mechanical ventilation in the neonatal intensive care unit using artificial intelligence and electronic health records: a clinical study

Abstract:Background: Respiratory support is crucial for newborns with underdeveloped lung. The clinical outcomes of patients depend on the clinician's ability to recognize the status underlying the presented symptoms and signs. With the increasing number of high-risk infants, artificial intelligence (AI) should be considered as a tool for personalized neonatal care. Continuous monitoring of vital signs is essential in cardiorespiratory care. In this study, we developed deep learning (DL) prediction models for rapid and accurate detection of mechanical ventilation requirements in neonates using electronic health records (EHR). Methods: We utilized data from the neonatal intensive care unit in a single center, collected between March 3, 2012, and March 4, 2022, including 1,394 patient records used for model development, consisting of 505 and 889 patients with and without invasive mechanical ventilation (IMV) support, respectively. The proposed model architecture includes feature embedding using feature-wise fully connected (FC) layers, followed by three bidirectional long short-term memory (LSTM) layers. Results: A mean gestational age (GA) was 36.61 ± 3.25 weeks, and the mean birth weight was 2,734.01 ± 784.98 g. The IMV group had lower GA, birth weight, and longer hospitalization duration than the non-IMV group (P < 0.05). Our proposed model, tested on a dataset from March 4, 2019, to March 4, 2022. The mean AUROC of our proposed model for IMV support prediction performance demonstrated 0.861 (95%CI, 0.853-0.869). It is superior to conventional approaches, such as newborn early warning score systems (NEWS), Random Forest, and eXtreme gradient boosting (XGBoost) with 0.611 (95%CI, 0.600-0.622), 0.837 (95%CI, 0.828-0.845), and 0.0.831 (95%CI, 0.821-0.845), respectively. The highest AUPRC value is shown in the proposed model at 0.327 (95%CI, 0.308-0.347). The proposed model performed more accurate predictions as gestational age decreased. Additionally, the model exhibited the lowest alarm rate while maintaining the same sensitivity level. Conclusion: Deep learning approaches can help accurately standardize the prediction of invasive mechanical ventilation for neonatal patients and facilitate advanced neonatal care. The results of predictive, recall, and alarm performances of the proposed model outperformed the other models.

Abstract:Prenatal prediction of postnatal outcomes in newborns with congenital diaphragmatic hernia (CDH) remains challenging, especially for mortality and neonatal persistent pulmonary hypertension (PPHN). Despite the increasing utilization of advanced artificial intelligence (AI) technologies in the neonatal field, this study is pioneering in exploring AI methodologies in the context of CDH. It represents an initial attempt to implement a Machine Learning (ML) system to predict postnatal mortality and PPHN severity, using prenatal and early postnatal data as input variables. We enrolled 50 patients with isolated left-sided CDH from singleton pregnancies and retrospectively collected clinical and imaging variables from fetal ultrasound (US) and shape features extracted from magnetic resonance imaging (MRI), combined with gestational age and birth weight. A supervised ML model for predicting mortality and PPHN severity was developed, achieving good accuracy (88% for mortality prediction and 82% for PPHN) and sensitivity (95% for mortality and 85% for PPHN). The area under the curve (AUC) of the ROC curve was 0.88 for mortality and 0.82 for PPHN predictions. Our results may lead to novel AI applications in the neonatal field, focusing on predicting postnatal outcomes based on prenatal data, ultimately improving prognostic assessments and intervention strategies for such a complex disease.

Abstract:Maternal mortality is a major public health concern worldwide. It is the number of preventable deaths that occur each year due to pregnancy and childbirth. The research investigates how machine learning may be used to minimize maternal mortality. Historical data on maternal health is used to develop predictive models, early detection systems, and resource allocation techniques. Machine learning helps to identify risk factors, monitor vital signs, and improve access to care. This allows for targeted interventions and better healthcare delivery. The challenges of data accessibility and model interpretation are addressed, highlighting the ethical and equitable applications of machine learning in maternal healthcare. This study emphasizes the potential of machine learning to reduce maternal mortality rates and the pressing need for its incorporation into healthcare systems worldwide.
