Abstract:Ventriculoperitoneal (VP) shunt placement is the most frequently used treatment for hydrocephalus. This procedure is not always free of complications, and patients may need additional surgeries to overcome these complications. We are presenting the case of a seven-month-old baby who underwent myelomeningocele repair and VP shunt placement 13 days ago and now presents with inguinal swelling extending into the scrotum. The radiological workup revealed that the peritoneal end of the VP shunt had migrated to the scrotum, causing hydrocele. The shunt was relocated to the abdomen after a right herniotomy and sac reduction. He was discharged on the second postoperative day without any complications, and the further recovery was good at three months. Scrotal migration of a VP shunt is a rare complication and can be avoided by careful early assessment of inguinal hernia or patent processus vaginalis and its surgical repair.

Abstract:Introduction and importance: Colonic ventriculoperitoneal shunt (VPS) migration with trans-anal protrusion remains uncommon. Patients may be asymptomatic, and diagnosis may only be made on visualization of the prolapsed catheter from the anus. This unique case of early post-operative trans-anal shunt protrusion highlight the possibility of this rare complication specially when shunt revision accompanies bowel surgery. Case presentation: The authors present a case of early postoperative colonic shunt migration in a thirteen-year-old female with who underwent Malone Antegrade Continence Enema (MACE) with concomitant revision of the distal part of the peritoneal catheter. She presented two weeks post operatively with shunt catheter protruding from the anus. This was noticed by her carer and she was asymptomatic on her presentation. Clinical discussion: Delayed post-operative shunt related bowel perforation and trans-anal shunt protrusion is an uncommon complication after ventriculoperitoneal shunting. Most cases present months after surgery and majority are asymptomatic on presentation. The exact pathophysiology is not established, and mechanisms have been proposed. Early post -operative trans-anal shunt protrusion is rare and suggests inadvertent occult bowel injury especially when shunt placement or revision accompanies extensive bowel surgery. Conclusion: The authors recommended shunt imaging within the first two to three weeks after shunt revision in patient who undergo concomitant bowel surgery with risk of inadvertent bowel injury to identify early colonic migration and avoid its potentially fatal sequelae.

Abstract:The ventriculoperitoneal (VP) shunt is a routine procedure for cerebrospinal fluid (CSF) diversion, and is associated with many complications. A delayed hemorrhage after the VP shunt surgery, however, is quite rare. In this study, we report a case involving late-onset hemorrhage. The 67-year-old male patient with a history of head trauma and brain surgery underwent a VP shunt placement for hydrocephalus. The surgery course was uneventful and no bleeding was revealed in the first computed tomographic (CT) scan after the procedure. However, a massive intraparenchymal and intraventricular hemorrhage occurred 8h following adjustment of the valve system on the 8th day after surgery.Erosion of the vasculature by catheter cannulation and a sudden reduction of CSF pressure after downregulation of the valve could be one of the possible causes of the intracerebral hemorrhage (ICH).

Abstract:Bowel perforation is an unusual complication of ventriculoperitoneal shunting. This article describes a case of bowel perforation associated with a ventriculoperitoneal shunt inserted in an 8-month-old male infant for meningocoele and hydrocephalus. Ten months after insertion of the shunt the infant presented with the shunting tube protruding through the anus. There were no signs of meningitis or peritonitis. At laparotomy the tube was seen to enter the transverse colon and was encapsulated by the greater omentum. The tube was cut and the distal end removed via the anus. The transverse colon was repaired. The catheter continued to function effectively and the patient remained asymptomatic. The literature on this rare complication is reviewed and the therapeutic options are discussed.
Early diagnosis to avoid invasive treatment in cardiac migration of a ventriculoperitoneal catheter: a qualitative systematic review and Weibull analysis of case reports

Abstract:Objective: The migration of distal catheter after ventriculoperitoneal shunt placement is a rare but significant complication. Especially in a case of cardiac migration, open-heart surgery or catheter intervention may be required. The authors encountered a case of cardiac migration that fortunately could be treated by withdrawal. A systematic review of cardiac migration was performed to clarify when and how migration was diagnosed and why invasive treatments were required. Based on the collected cases, a Weibull analysis of the latency until diagnosis was performed to examine whether cardiac migration is caused by an initial factor and to compare the result with the other migration sites such as gastrointestinal tract or urinary tract. Methods: A qualitative systematic review was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. A database of case reports was created by searching PubMed and Scopus with the keywords "ventriculoperitoneal AND shunt AND migration" (last search date: April 2022). Whether the occurrence rate of migration is constant over time was examined by fitting a Weibull distribution. Results: A total of 339 articles of all migration sites were identified. Among them, 36 articles reporting 38 cases of cardiac migration were considered eligible. A total of 39 cases including the authors' case were reviewed qualitatively. When classifying the cases by their latency to diagnosis, the rates of pulmonary thrombosis and of cardiac adhesion were higher in the delayed group (≥ 1 year) than in the early group (≤ 1 month). The rate of open chest surgery was higher in the delayed, intermediate, and early groups, in that order. In the Weibull analysis, the shape parameter (β) was less than 1, indicating that the occurrence rate of cardiac migration was initially high, followed by a decline. The finding supports the hypothesis that migration results from an intraoperative vascular injury. Note that these findings are subject to bias given that they are derived from case reports. Conclusions: In light of the previous reports, the latency until diagnosis of cardiac migration was associated with the rate of thrombosis and adhesion, which resulted in escalation to invasive treatment. Early diagnosis will prevent invasive treatment because most cases are caused by initial factors, as the Weibull analysis showed.

Abstract:Ventriculoatrial (VA) shunt is one of the most commonly used solutions for hydrocephalus. In recent years, the number of VA shunt has decreased worldwide, given the perceived technical challenges and the potentially serious complications associated with VA shunt. However, VA shunt remains as a viable treatment option for hydrocephalus in selected patients. Novel placement strategies and monitoring methods have been developed to reduce complications following VA shunt. In this article, we reported that seven consecutive cases who received a VA shunt. VA shunts were applied in seven hydrocephalic patients who experienced previous ventriculoperitoneal (VP) shunt failures or had contraindications to abdominal catheter placement. The insertion of VA shunt catheters was guided with the aid of intraoperative electromagnetic neuronavigation and electrocardiographic technique. There were three female and four male patients with a mean age of 46 years (range 22–68 years) received VA shunts under the guidance of electromagnetic neuronavigation and electrocardiographic method intraoperatively. In all cases, postoperative cranial CT scans and chest radiography demonstrated appropriate positioning of the catheter tips. And no postoperative complications occurred during the follow-up period of 3–26 months. VA shunts are potential favorable alternatives for patients who cannot tolerate VP shunts. It is noteworthy that VA is not free of complications. Intraoperative application of electromagnetic neuronavigation and electrocardiographic technique contributes to safe and optimal catheter placement of VA shunts.

Case report: Extravascular catheter migration in a child: A rare complication of the totally implantable venous access device

Abstract:Background: Totally implantable venous access devices (TIVADs) are widely used to gain intermittent central venous access, such as in patients who need long-term chemotherapy, total parenteral nutrition, and long-term antibiotic treatment. At present, there are many complications associated with the use of these devices. Complete extravascular migration of TIVADs via the internal jugular vein is a very rare and potentially serious condition, especially in children. Case presentation: A 1-year-old girl needed palliative chemotherapy because of hepatoblastoma complicated by inferior vena cava thrombosis. A TIVAD was implanted through the right internal jugular vein with a routine heparin flushing tube. On the second day after the operation, a pale bloody liquid was drawn out from the device and the chest X-ray was checked to confirm that the position of the catheter was normal. On the third day after the operation, however, the patient's right respiratory sound was weakened on physical examination and auscultation. Fluoroscopy showed that the tip of the catheter was located in the right thoracic cavity, and there was a large amount of effusion in the right thoracic cavity. The pleural effusion was removed, the TIVAD was replaced again, and the child was discharged 2 days later. Conclusions: Following TIVAD implantation, if abnormalities are found, in addition to chest X-ray, saline flush and echocardiography should be performed to determine the position of the catheter and rule out extravascular migration of the catheter to avoid irreparable consequences.

Abstract:Cerebrospinal fluid (CSF) hydrothorax is a rare complication of a ventriculoperitoneal shunt (VPS), and even rarer in the absence of shunt migration into the thoracic compartment. Because of the limited volume within the thoracic cavity, hydrothorax in infants can rapidly cause severe respiratory distress. The case of an infant with recurrent CSF hydrothorax despite a well-positioned VPS is presented. A ventriculoatrial shunt was successfully performed as the definitive treatment. The absence of ascites or predisposing factors for decreased peritoneal absorption and a false-negative β(2)-transferrin CSF marker are some of the factors that can add to the diagnostic challenge in these patients. This is a potentially life-threatening condition in infants, which should be closely considered in patients with VPS who develop hydrothorax, although diagnosis is not always straight forward. The etiology and pathophysiology of this very rare disease continue to be elusive, and treatment with ventriculoatrial shunt provides good results.

Abstract:Abstract In most experimental designs, the inhibitory effect of water-soluble dietary fibers on lipid absorption is evaluated by the decrease in plasma lipid concentration or the increase in fecal lipid output. The aim of this study was to evaluate the acute effect of a water-soluble polysaccharide, polydextrose, on lipid transport to the mesenteric lymph using lymph-fistula rats. The mesenteric lymph duct of rats was cannulated, and an infusion tube was introduced into the duodenum. After recovery, a lipid emulsion containing radioactive triolein and cholesteryl oleate was infused into the duodenum for 8 hr. The tested group was infused with the lipid emulsion containing 5% or 10% polydextrose as dietary fiber. Samples from the lymph-fistula were collected, and the luminal contents and mucosa were collected at the end of infusion. Lymph flow in the mesenteric lymph decreased in the polydextrose group after the infusion. The amounts of both triglyceride and cholesterol remaining in the lumen were greater In the polydextrose group, due to decreased transport of lipid into the lymph. These effects were dose dependent in the 5% and 10% polydextrose groups. The results of this study indicate that polydextrose retarded the transport of triolein and cholesterol into the mesenteric lymph.

Abstract:Ventriculoatrial shunts are the alternative treatments when it is impossible to use ventriculoperitoneal shunts. Limited indication for ventriculoatrial shunt is due to the possibility of very serious complications inherent with this procedure. We present a case report of a young patient who suffered from disconnection of an atrial catheter from the valve after an accidental blow to his neck. The atrial catheter was dislocated to the heart and pulmonary artery and it was extracted through the femoral vein in the groin area using an endovascular technique. The procedure went without complications. A new atrial catheter was introduced under ultrasonic guidance during surgical revision.

Abstract:The ventriculoperitoneal shunt placement procedure is common throughout the world, with a prevalence of approximately 30,000 procedures per year in the United States (5). However, the rate of complications is still important and must be remembered, with a percentage of 11 to 25% after the first year of implantation. Abdominal complications are described in 5–47% of the cases (6), and the abdominal pseudocyst is one of them. We present the case of a patient colonized by multidrug-resistant bacteria, who underwent exteriorization surgery of a ventriculoperitoneal shunt. Intraoperatively, a blue coloration was observed in the catheter, a phenomenon previously described in urinary infections (1)(2) but not before in pathologies of the central nervous system. Our objective is to present an atypical case of dysfunction of the ventriculoperitoneal shunt, whose diagnosis couldbe evidenced through the coloration of the catheter exteriorized in surgery.

Abstract:An LTE MIMO antenna array, which is adaptive to the effects of the user's body, is presented for mobile terminals. The bands of 750-960 MHz and 1700-2700 MHz are covered with a good efficiency in free space. Three kinds of user effects are studied, namely, “SAM head and PDA hand,” “PDA hand,” and “dual hands.” The array is formed by selecting the best two elements out of four, with the two ports and grounding points of the other two elements open. The user effects on MIMO channel capacity are reduced through the adaptive selection of two optimal antenna elements. The total efficiency, envelope correlation coefficient, and multiplexing efficiency are presented for the three kinds of user effects. In the lower band, the decreased correlation due to the optimal selection improves the multiplexing efficiency. The underlying physical mechanisms and some general rules are discussed. The specific absorption rate (SAR) in the “SAM head and PDA hand” case is studied. Experiments for the three kinds of user effects are also carried out.

Abstract:Objective Evaluated our experience in the management of arachnoid cysts in children applying cysto-peritoneal (C-P) shunt in ten years. Methods Between January 1997 and December 2007,358 pediatric patients with arachnoid cyst underwent C-P shunt.Different shunt tubes were selected according to the neuroimaging. Microscopically, a small hole were opened, and tube were placed into the cysts about 2～3 cm. Result The effective power of C-P shunting was about 97.2%, and about 1.7% was obstructed, 1.1% infected. Conclusions C-P shunting is a simple,safe,effective method to treat the intracranial arachnoid cysts in pediatric populations.

Abstract:1 The possible mechanisms of action of the inhibitory effect of gomisin C on the respiratory burst of rat neutrophils in vitro was investigated. 2 The peptide formyl‐Met‐Leu‐Phe (FMLP) induced superoxide anion (O2−) formation and O2 consumption, which was inhibited by gomisin C in a concentration‐dependent manner (IC50 21.5 ± 4.2 μg ml−1 for O2− formation). Gomisin C also suppressed O2− formation and O2 consumption at low concentrations of phorbol myristate acetate (PMA) with an IC50 value of 26.9 ± 2.1 μg ml−1 for O2− formation. However, gomisin C did not affect the responses induced by a high concentration of PMA. 3 Gomisin C had no effect on O2− generation and uric acid formation in the xanthine‐xanthine oxidase system, and failed to alter O2− generation during dihydroxyfumaric acid (DHF) autoxidation, indicating that it does not scavenge superoxide. 4 Like trifluoperazine (TFP), gomisin C attenuated the activity of PMA‐activated neutrophil particulate NADPH oxidase in a concentration‐dependent manner. 5 Gomisin C reduced the elevations of cytosolic free Ca2+ in neutrophils stimulated by FMLP in the presence or absence of EDTA. Cyclopiazonic acid (CPA) induced the release of Ca2+ from intracellular stores and this was also reduced by gomisin C. However, the Ca2+ influx pathway activated by CPA was not affected by gomisin C. 6 The cellular cyclic AMP level was markedly increased by forskolin, but not by gomisin C. Moreover, the inositol phosphate levels in FMLP‐activated neutrophils were not affected by gomisin C. 7 These results show that the inhibitory action of gomisin C on the respiratory burst is not mediated by changes in cellular cyclic AMP or in inositol phosphates, or by scavenging O2− released from neutrophils, but may be mediated partly by the suppression of NADPH oxidase and partly by the decrease of cytosolic Ca2+ released from an agonist‐sensitive intracellular store.

Abstract:BACKGROUND Physicians treating patients for chronic pain have limited means of determining whether a person is taking their medications as prescribed and are not taking extra medication. Complicating patient treatment regimens is the fact that pain physicians' prescribing practices may come under scrutiny by the Drug Enforcement Agency and other licensing agencies. If questioned, doctors can be hard-pressed to substantiate that their particular practices meet the established standard of care. It would be helpful to establish that their patients adhere to medications when compared with other practices. Previous studies show that urinary excretion data transformed by mathematical models can produce a reliable range of expected values for pain medications and may be useful to help resolve the aforementioned issues. PURPOSE OF THE STUDY To provide comparative urinary excretion information data on three commonly prescribed opioid medications (morphine, hydrocodone, and oxycodone). METHODS This retrospective study involved quantitative analysis of300, 000 urine specimens for three test drugs using previously described methods. The results were analyzed as percent frequency distributions and logarithmic functions. RESULTS The authors established a creatinine-normalized range of urinary concentration values for each drug. Results for two practices were compared with the meta-findings, providing quantitative evidence of overall standard of care prescription practice by that physician. CONCLUSIONS Expected urinary drug excretion values for morphine, hydrocodone, and oxycodone can potentially benefit pain physicians by showing that they are within the expected standard of care, helping to establish patient compliance, and identifying patients whose metabolism of these drugs may put them at risk.
