Health belief model constructs including perceived benefits, barriers, severity, susceptibility, self-efficacy and social support predicted 33.5% of the observed variance of self-care behaviors. Perceived susceptibility and self-efficacy had positive effect on self-care behavior; whereas perceived barrier's has negative effect. Abstract:Background Type 2 diabetes complications are responsible for 2% of hospital emergency visits. Self-management practices are one of the most essential approaches to control type 2 diabetes. The goal of this study was to use an ecological approach to investigate the predictors of self-management behaviors in diabetes patients referred to the emergency department in Ardabil in 2020. Methods In this cross-sectional study, 273 individuals with type 2 diabetes who were sent to the emergency department of Imam Khomeini Educational and Medical Center in Ardabil were included using the available sample method. Demographic information questionnaires, including the Diabetes Distress Screening Tool (DDS2), General Diabetes Knowledge (DKT2), Diabetes Empowerment Questionnaire (DES-SF), Patient Health Questionnaire 9 (PHQ 9), Beliefs to Treatment Effectiveness Scale (BTES), Diabetes Self-Efficacy Scale (DSES), Chronic Illness Resources Survey (CIRS), Situational Effects Questionnaire, and Diabetes self-management support (DSMS), were all used to collect data. The independent t -test, one-way ANOVA, Pearson correlation coefficient, and multiple regression were used to analyze the data. Results The results of the study showed that in the framework of ecological approach, predictors of self-management behaviors at the four levels are as follows: at the individual level—gender ( p = 0.025), education ( p = 0.002), duration of diabetes ( p = 0.38), having a glucometer at home ( p < 0.001), diabetes empowerment ( p < 0.001), personal support ( p = 0.002), and self-efficacy ( p = 0.047); at the interpersonal level—the main health support ( p < 0.001), membership in social networks ( p < 0.005), family/friends support ( p < 0.001), and neighborhoods support ( p < 0.001); at the group and organizational level—organizational support ( p = 0.013); at the community and policy level—the impact of mass media in health ( p < 0.001) and situational influence ( p < 0.001). Conclusion The impact of non-individual levels, such as the environment, on a person's decision to manage diabetes is crucial. Diabetes management necessitates a significant amount of effort, which involves maintaining the health of diabetes patients and the community while also minimizing emergency department workload.

Abstract:Background: Self-care practices in diabetes patients are crucial to keep the illness under control and prevent complications. Effective management of diabetes will be a difficult task without an adequate understanding of the current level of practice related to diabetes self-care. Therefore, this study aims to assess the factors affecting the self-care behaviors of patients with type II diabetes and the role of demographic variables in Iran. Materials and methods: The present study is a descriptive-analytical study that was designed and conducted in 2022. This study population was type II diabetic patients at the Diabetes Research Center of Ayatollah Taleghani Hospital in Kermanshah who were selected by available sampling method of 280 samples. Data collection tools included demographic characteristics questionnaire and standard self-care questionnaire for the Glasgow and Toobert diabetic patients. Data were analyzed using SPSS-23 software, independent t-tests, one-way analysis of variance, and stepwise regression. Results: The mean of self-care scores was 2.07 ± 2.08 and indicated an unfavorable condition. The highest mean scores of self-care were in the field of regular use of medications, and the lowest scores were in the field of blood-glucose testing. The mean scores of self-care were significantly different based on marital status, occupation status, residence, type of treatment, duration of diabetes, years, smoking status (P < 0.05). Regression results showed that the variables predictors; type of treatment, smoking status, the complication of diabetes, education level status, place of residence explained 515% of the variance of self-care. Conclusions: The study showed that the extent to which individuals adhere to type II diabetes recommended management is substantially low. Improving awareness of patients and the community at large is imperative, especially on the diet, exercise, blood-glucose testing, foot care, and no smoking.

Abstract:Diabetes self-care activities are essential for achieving optimal glycemic control. However, little investigation has been conducted in Ethiopia to evaluate the relationship between the rate glycemic controland self-care activities among patients with type 2 diabetes mellitus (T2DM). Therefore, this study was conducted to assess self -care activities and their association with glycemic control among patients with T2DM in Northwest Ethiopia general hospitals. This multicenter cross-sectional study was conducted in Northwest Ethiopia general hospitals diabetic clinics. Diabetes self-care activities were measured using the Amharic version of the Summary of Diabetes Self-Care Activities (SDSCA-Amharic). Glycated hemoglobin (HbA1c) were used to assess the rate of glycemic control. A linear regression model was used to identify predictors of self-care activities and glycemic control. P-value of < 0.05 at 95% confidence interval (CI) was considerd as statistically significant. Of 413 participants included in the final analysis, two-thirds (66.3%) had poor glycemic control, with a mean HbA1c of 7.94% (SD = 1.75). Blood glucose testing was the most important self-care activity domain for predicting better glycemic control [β=-0.36, 95% CI (-0.48, -0.24); P = 0.0001] followed by diet [β=-0.29, 95% CI (-0.39, -0.083); P = 0.0001], foot-care [β=-0.28, 95% CI (-0.3, -0.061); P = 0.003], and physical activity [β=-0.27, 95% CI (-0.29, -0.056); P = 0.004], respectively. Moreover, unable to read and write [β = 0.72, 95% CI (0.57, 3.8); P = 0.037], overweight [β = 0.32, 95% CI (0.011, 0.62); P = 0.042], obesity [β = 0.67, 95% CI (0.39, 0.94); P = 0.0001], and low level of medication adherence [β = 0.7, 95% CI (0.39, 1.1); P = 0.0001] were significant predictors of poor glycemic control. Previous diabetes education [β=-0.88, 95% CI (-1.2, -0.57); P=0.0001] was a significant predictor of good glycemic control. The prevalence of poor glycemic control and poor self-care activities were high among patients with T2DM. Self-care activities were independent predictors of glycemic control among patients with T2DM. Therefore, management interventions for patients with T2DM should focus on improving self-care activities and other predictor variables.

Abstract:Background: Diabetes is a global health issue and the most prevalent chronic metabolic disease, which requires lifelong self-care behaviors. Thus, the present study was conducted with the aim of predicting the factors of self-care behaviors based on social cognitive theory in diabetic patients referred to comprehensive health service centers in Fasa. Methods: This cross-sectional research comprised 106 type 2 diabetes patients who were referred to Fasa service providers to determine the optimal solution. Outcomes were assessed using an existing demographic questionnaire, the diabetic self-care questionnaire, and a unique questionnaire to assess social cognitive theory, the validity and reliability of which were estimated. SPSS21 software with a Chi license was used for data analysis, and Spearman correlations, independent t-tests, and one-way ANOVA were performed. Results: The age range of patients was between 33 and 67 years. There was a significant relationship between gender, education, and type of treatment with self-care. According to the findings, the variables of self-efficacy ( r = 0.200), self-efficacy of overcoming obstacles ( r = 0.285), environmental factors ( r = 0.334), observational learning ability ( r = 0.148), situational perception ( r = 0.297), emotional adaptation ( r = 0.051), outcome assessment ( r = 0.114) and outcome expectation ( r = 0.082) had a positive and significant correlation with self-care behaviors. Also, the self-efficacy variable (Beta coefficient = 0.340) had the highest predictive value. Conclusion: In designing and implementing educational interventions for self-care of diabetic patients, the theory of social cognition can be used as a framework to promote and maintain public health in patients.

This study aimed to examine the influence of demographic characteristics and perceived social support on diabetes self-management behaviors in individuals with T2DM. Methods: An analytical cross-sectional design was employed to collect data from 300 T2DM patients using validated questionnaires from April to May 2023. Self-care behaviors were assessed using a Summary of Diabetes Self-Care Activities-Arabic (SDSCA-Arabic), while perceived social support was measured using a Diabetes Care Profile (DCP) using an Arabic Social Support subscale. Descriptive statistics, correlation analysis, and regression analysis were performed to examine the relationships and predictors of self-care behaviors. Results: Patients exhibited varying levels of self-care behaviors, with exercise and blood sugar testing being the most frequently reported activities. Additionally, the findings revealed that health education on self-examining regarding the feet, exercising, comorbidities, perceived social support, medical insurance, and gender were significant predictors of diabetes self-management behaviors. These factors collectively accounted for 21.2% of the variance in self-management behavior levels. Specifically, health education on self-examining regarding the feet (β = − 0.286, p = 0.001) and exercising (β = − 0.175, p = 0.001) showed a negative association, while perceived social support (β = 0.235, p = 0.001) demonstrated a positive association with self-management behaviors. Conclusion: The study highlights the importance of comprehensive health education programs that address multiple aspects of diabetes management and the role of social support in fostering better self-management behaviors. It also emphasizes the influence of demographic factors on self-management, with individuals with higher comorbidities exhibiting better self-management behaviors and those with medical insurance showing poorer self-management behaviors. These findings have implications for nursing practice, suggesting the need for tailored interventions, effective education, and support networks to enhance self-management behaviors in T2DM patients. This study aimed to explore the factors related to SMB among patients with type 2 diabetes and how these factors interacted with each other. Patients and Methods: Patients diagnosed with type 2 diabetes were recruited from 18 community healthcare stations (CHSs) from six community healthcare centers (CHCs) in Beijing, China from April to May in 2017. Motivation, competence, autonomy support, social support, self-management skills, adherence to self-monitoring of blood glucose (SMBG) and haemoglobin A1c (HbA1c) measurement were tested by questionnaire. Correlation analysis and path analysis were performed so as to identify the factors associated with patients' SMB. Results: A total of 532 participants completed this study. Participants who have good compliance to SMBG got higher scores in social support (F = 7.68, p = 0.01), competence (F = 10.47, p = 0.01), and skills (F = 12.34, p < 0.01). Higher competence (β = 0.03, P < 0.001), higher social support (β = 0.01, P < 0.001), better skills(β = 0.01, P < 0.001) directly led to better adherence to SMBG. Social support had a positive effect on autonomy support (β = 0.69, P < 0.001), motivation (β = 0.45, P < 0.001), competence (β = 0.28, P < 0.001), skills (β = 0.14, P < 0.001), which was also indirectly linked to better adherence to SMBG. Better self-management skills directly led to better adherence to HbA1c measurement (β = 0.03, P < 0.001). Social support had a positive effect on autonomy support (β = 0.69, P < 0.001), motivation (β = 0.45, P < 0.001), competence (β = 0.28, P < 0.001), skills (β = 0.14, P < 0.001), which was also indirectly linked to better adherence to HbA1c measurement. Conclusion: Self-determination theory and social support theory were practical in explaining SMB in Chinese population. Competence, motivation and social support played an important role in diabetic self-management. Paying attention to the promotion of individual's intrinsic motivation and self-efficacy may be able to help patients maintain self-management behavior in the long-term routine. According to a national research conducted in China in 2010, the estimated prevalence of diabetes and pre-diabetes in adults was 11.6% and 50.1%. 1 In diabetes treatment, self-management of diabetes is an effective way to minimize the risk of developing complications and improve their quality of life. Self-management behaviors (SMB) includes taking medications regularly, following a healthy diet, being physical active, foot care, smoking cessation and self-monitoring of blood glucose (SMBG) and hemoglobin A1c (HbA1c). 2 According to previous researches, SMBG and HbA1c measurement are crucial SMB and could effectively assist patients in controlling blood glucose levels. 3,4 For example, Bartolo et al found that increasing the number of SMBG tests from 1 to 2 per day was associated with HbA1c reduction in young adult patients. 5 Besides, previous research showed that SMBG could reduce the incidence and progression of micro-vascular disease in both type 1 (T1D) and 2 diabetes (T2D). 6 Patients with regular SMBG also reported a better quality of life. 7,8 Furthermore, persons with type 2 diabetes and type 1 diabetes also require regular measurement of HbA1c. 9 Therefore, SMBG and HbA1c measurement play a significant role in glycemic control and routine care for diabetic patients. Abstract:Background. Self-efficacy in diabetes management can empower patients and result in their more active participation in the treatment process. In addition, a patient’s behavior is influenced by their knowledge of and attitude toward their illness. Therefore, the goal of the present study was to examine the association of knowledge of diabetes, attitude toward diabetes, and health literacy with diabetes management self-efficacy in patients with type 2 diabetes. Methods. This descriptive-correlational study was conducted in 2019. The sample included 115 patients with diabetes (59 women and 56 men) attending diabetes centers of Sanandaj, Kurdistan, Iran, who were selected using a convenience sampling method. The Brief Diabetes Knowledge Test (DKT), the Diabetes Attitude Scale (DAS), the Health Literacy for Iranian Adults (HELIA) scale, and the Diabetes Management Self-Efficacy Scale (DMSES) were used to gather data. The data was analyzed using descriptive statistics, Pearson correlation coefficient, and stepwise regression analysis. All the analyses were perfumed using SPSS, version 16. The significance level was set at 0.05 for all the tests. Results. The mean age of participants was 46.61 ± 13.73 years. Health literacy (r = 0.585) and attitude (r = 0.396) were significantly correlated with self-efficacy (P = 0.001). According to the results of stepwise regression analysis, 34.3% and 36.6% of the variance of self-efficacy was explained by health literacy alone and health literacy together with attitude, respectively. One standard deviation change in health literacy and attitude was associated with 0.51 and 0.71 standard deviation change in self-efficacy, respectively. Conclusion. Health literacy and attitude toward diabetes are positively associated with self-efficacy in diabetes management, and improving these variables can improve self-efficacy in patients with type 2 diabetes.

Abstract:BackgroundSelf-efficacy is one of the factors involved in successful self-care of diabetic patients. The aim of this study was to evaluate general self-efficacy and diabetes management self-efficacy and to determine their association with glycemic control in diabetic individuals, referred to the diabetes clinic of Aq Qala city, North of Iran.MethodsIn this cross-sectional study, 251 type 2 diabetes mellitus patients were enrolled using census method. Data collection tools consisted of Sherer General Self-Efficacy Scale (SGSES) and Diabetes Management Self-Efficacy Scale (DMSES) with minor demographic adjustments and hemoglobin A1C test. Data were analyzed using descriptive statistics and analytical techniques include independent t-test, Spearman correlation coefficient and linear regression were applied for further data analysis.ResultsThe mean and standard deviation age of subjects was 56.17 ± 10.45 years. The mean level of HbA1C of studied subject was 8.35 ± 2.02%. There was a negative correlation between age and general self-efficacy and diabetes self-efficacy while, there was a positive correlation between general self-efficacy and diabetes self-efficacy (P < 0.001). Results of the regression analysis showed that duration of the disease was the only variable which had a significant effect on the level of hemoglobin A1C (P < 0.001), so that for each year of having the disease, the level of hemoglobin A1C increased by 0.084% (CI 95% = 0.048–0.121).ConclusionsGeneral self-efficacy and diabetes self-efficacy does not affect glycemic control in diabetic individuals. The duration of the disease is the only affecting variable on glycemic control by its worsening in diabetic individuals. Interventions are recommended to help glycemic control in individuals who are having this disease for longer periods. Moreover, further studies on the affecting factors on poor glycemic control of diabetic patients as well as the role of time variable, are recommended.

Abstract:Background: The essential strategies to avoid diabetes complications are lifelong health education and self-care. This study determines the impacts of an educational intervention based on a health belief model. Methods: This quasi-experimental study was conducted on patients with type 2 diabetes using convenience sampling. They were randomly assigned to experimental (n=55) and control groups (n=55). A standard questionnaire was used to collect the data. The healthy behaviors were measured again after 3 and 6 months, and the data analysis was performed using the SPSS software, version 20. Results: The mean value of perceived susceptibility boosted in the intervention group from 18.94 before the training to 22.90 and 22.3 after 3 and 6 months of the intervention, respectively. The mean score perceived severity increased in the intervention group to 31.30 and 31.42 after 3 and 6 months of the intervention, respectively. Meanwhile, the mean score of perceived benefits increased in the intervention group from 10.06 before the training to 11.61 and 13.32 after 3 and 6 months of the intervention, respectively. The mean score of perceived barriers decreased in the intervention group from 19.50 before the training to 13.35 and 12.21 after 3 and 6 months of the intervention, respectively. The mean score of perceived self-efficacy boosted in the intervention group from 24.16 before the training to 29.44 and 29.48 after 3 and 6 months of the intervention, respectively. The mean value of cues to action boosted in the intervention group from 20.15 before the training to 22.2 and 23.2 after 3 and 6 months of the intervention, respectively. Conclusion: The results demonstrated the importance of designing educational programs based on the health belief model constructs over 3 and 6 months in adopting healthy behaviors by subjects with type 2 diabetes.
Predictors of Adherence Toward Specific Domains of Diabetic Self-Care Among Type-2 Diabetes Patients

Abstract:Background: Adequate knowledge, awareness, and adherence to diabetic self-care practices are vital tools to protect patients from risks of disease complications, developing comorbidity and mortality. Therefore, this study aimed to assess specific domains of diabetic self-care practice and associated factors among patients with type-2diabetes in Hawassa University Comprehensive Specialized Hospital, Sidama regional state. Materials and methods: A hospital-based cross-sectional study design was conducted on 217 patients with type 2 diabetes from January 01 to April 30, 2020. A structured questionnaire and the Summary of Diabetes Self-Care Activities (SDSCA) tool were used to collect relevant data through interviewer administration. Statistical analysis was done using SPSS version 23. Results: A total of 207 patients with type-2 diabetes were participated in the study with a 95% response rate. Overall 47.8% (95%CI: 41.2-55) of patients adhered to diabetic self-care practice. Concerning the specific domain of self-care practice, 54.6%, 39.1%, 28%, and 65.2% of patients adhered to a healthy diet, physical exercise, self-monitoring blood glucose (SMBG), and diabetic foot care practices, respectively. Besides, all patients received at least 80% of the prescribed doses and frequency of anti-diabetic agents and 60.4% had good glycemic control. Receipt of advice from treating physicians and having no familial history of diabetes were significantly associated with adherence toward eating a healthy diet, diabetic foot care, and SMBG. While male sex was associated with adherence toward healthy diet management. Moreover, having glucometer, age, male sex, diabetes duration ⩾ 5 years, and anti-diabetic treatment modality were associated with adherence toward SMBG. Conclusion: This study indicates 52.2%, 72%, and 60.1% of diabetes patients did not adhere to diabetic self-care, SMBG, and physical exercise, respectively. Improving awareness and regular diabetic education is imperative to scale up patients’ adherence toward diabetic self-care practice.

Abstract:Franck and Hetrz in 1914 were the first to demonstrate that free electrons can be decelerated by mercury atoms in discerete energy quanta. In 1930 Latyscheff and Leipunksy have demonstrated the inverse effect namely, free electrons can be accelerated by energy stored in the mercury atoms (collision of the second kind). It was only in 1958 that Schawlow and Townes have used multiple collisions between photons and excited atoms to amplify radiation (MASER and LASER). In 1995 Schachter suggested to use excited atoms for coherently accelerate particles. The results of a proof-of-principle experiment (2006) demonstrating the PASER scheme are reported here. Performed at the BNL-ATF, the essence of the experiment is to inject a 45 MeV density modulated electrons' beam, bunched by its interaction with a high- power CO2 laser pulse within a wiggler, into CO2 excited molecules cell. The electrons experienced 0.15% relative change in their kinetic energy, in less than 40 cm long interaction region. The experimental results indicate that a fraction of these electrons have gained 200 keV each, implying that such an electron has undergone two-million collisions of the second kind. Hence, this is the first experimental demonstration of coherent collisions of the second kind.

Abstract:Background: Diabetes is a highly prevalent metabolic disorder in the world. Complications of diabetes mellitus can have an extreme effect on the quality of life in terms of physical and mental health, as well as social and environmental well-being. Objectives: The current study aimed to measure the quality of life, its determinants, and self-care behaviors in patients with type 2 diabetes. Methods: A descriptive cross-sectional study was conducted in the diabetes clinic of Imam Reza hospital in Mashhad on 140 patients with type 2 diabetes from October to December 2019. The Summary of Diabetes Self-care Activities (SDSCA) questionnaire and the World Health Organization Quality of Life-Brief (WHOQOL-BREF-26 items) were used for data collection. Data analysis was carried out with the Mann-Whitney test and ANCOVA. Results: The mean age of study participants was 58.41 ± 8.91 years, and the majority of them (67.14%) were female. The overall self-care score was 43.32 ± 10.93 in males and 39.93 ± 9.94 in females. The mean scores of the dimensions of quality of life were 61.29 ± 15.66 for physical health, 60.62 ± 13.70 for mental health, 68.67 ± 11.63 for social health, and 61.54 ± 14.88 for environmental health. Among self-care behaviors, physical activity (P = 0.006) was a stronger predictor of quality of life. Demographic characteristics (except for the duration of disease, family history, and age) showed significant correlations with the overall aspects of quality of life. Conclusions: The quality of life of people with diabetes was correlated with some demographic variables. However, the duration of disease, family history, and age did not have any effect on the quality of life. We found that the self-care behavior of physical activity was a significant predictor of quality of life in adults with diabetes. Therefore, it is important to implement programs to improve self-care behaviors.

Abstract:BackgroundSelf-efficacy has been shown to be positively correlated with self-care behaviour and glycaemic control among patients with type 2 diabetes mellitus. However, such evidence is lacking in the Malaysian primary care setting. The objectives of this study were to i) determine the levels of self-efficacy, self-care behaviour and glycaemic control among patients with type 2 diabetes mellitus in the Malaysian primary care setting ii) determine the relationship between self-efficacy, self-care behaviour and glycaemic control iii) determine the factors associated with glycaemic control.MethodsThis was a cross-sectional study involving patients with type 2 diabetes mellitus from two public primary care clinics in Malaysia. Self-efficacy and self-care behaviour levels were measured using previously translated and validated DMSES and SDSCA questionnaires in Malay versions, respectively. Glycaemic control was measured using HbA1c.ResultsA total of 340 patients with type 2 diabetes mellitus were recruited. The total mean (±SD) of self-efficacy and self-care behaviour scores were 7.33 (±2.25) and 3.76 (±1.87), respectively. A positive relationship was found between self-efficacy and self-care behaviour (r 0.538, P < 0.001). Higher self-efficacy score was shown to be correlated with lower HbA1c (r − 0.41, P < 0.001). Multiple linear regression analysis demonstrated that higher self-efficacy scores (b − 0.398; 95% CI: -0.024, − 0.014; P < 0.001), shorter duration of diabetes (b 0.177; 95% CI: 0.002, 0.007; P < 0.001) and smaller waist circumference (b 0.135; 95% CI: 0.006, 0.035; P = 0.006), were significantly associated with good glycaemic control.ConclusionThis study demonstrated that higher self-efficacy was correlated with improved self-care behaviour and better glycaemic control. Findings of this study suggest the importance of including routine use of self-efficacy measures in the management of type 2 diabetes mellitus in primary care.

Effectiveness of Self-Management Behavior Intervention on Type 2 Diabetes Based on Self-Determination Theory

Abstract:OBJECTIVE:To provide autonomy support from three dimensions based on self-determination theory (SDT), i.e. professional support, peer support, family support, and to investigate whether this intervention can improve diabetes self-management behavior and glycemic control of diabetic patients, and to analyze the influencing factors of the effect.METHODS:Using convenient sampling method, three communities were selected respectively in Beijing. Each community selected health service stations with similar conditions as different intervention groups. The diabetic patients managed by the station who were eligible for inclusion were recruited into this intervention group. The community stations were divided into three groups. The routine intervention group only issued knowledge manuals and conducted health management according to the requirements of basic public health services. Peer support groups were divided into small groups and carried out doctor-led group activities. Based on doctor-led peer support activities, the doctors and peers were trained to provide autonomy support based on self-determination theory, and their family members were trained in the form of manuals to provide autonomy support, forming a concerted support of the three dimensions. Activity processes and materials were also designed based on SDT. The intervention duration was 3 months, and the main evaluation indexes were HbA1c and patients' self-management behaviors, skills, knowledge, and self-efficacy scores.RESULTS:Before and after the intervention, the HbA1c of routine intervention group were 7.40%±1.37%, 7.30%±1.18%. The HbA1c of peer support group before and after the intervention were 7.33%±1.15% and 7.13%±1.27%. The HbA1c of autonomy support group before and after the intervention were 7.42%±1.22% and 6.78%±0.80%. Before and after the intervention, the self-management score in routine intervention group was 10.54±2.28 and 10.80±2.15, the score in peer support group was 11.09±1.89 and 11.40±1.78, the score in autonomy support group was 10.34±1.99 and 11.10±1.65, respectively. The HbA1c and self-management score increased higher in autonomy support group than in the other two groups. After intervention, the control rate in autonomy support group was higher than in the other two groups. According to the multi-factor analysis, the value of HbA1c after intervention was positively related to the baseline HbA1c, and negatively related to self-management behavior. The value in autonomy support group was higher than in routine intervention group. Baseline self-management behavior, self-efficacy, knowledge, skill, family support, autonomy support, peer support and age were positively correlated with the change of behavior.CONCLUSION:self-management behavior intervention based on self-determination theory can effectively promote self-management behavior and glycemic control of diabetic patients, and the effect is better than single peer support activities.
Diabetes self-management and its related factors among Chinese young adults with type 2 diabetes mellitus

Abstract:Aim: To explore the status quo of self-management among young adults with type 2 diabetes mellitus (T2DM) and the determinants of self-management under the guidance of social cognitive theory. Design: A cross-sectional study. Methods: In total, 227 young adults (18-44 years old) with T2DM at two hospitals in Beijing completed the questionnaires. The Summary of Diabetes Self-care Activities (SDSCA) was used, along with additional questionnaires about diabetes self-efficacy, attitude, diabetes distress, diabetes knowledge, coping styles and social support. Univariate analysis and multiple linear regression were used to explore the related factors of self-management among the young patients. Results: The scores for the SDSCA in diet, exercise, blood-glucose testing, foot care and medication taking were (4.16 ± 1.51), (3.46 ± 2.50), (2.28 ± 2.24), (1.08 ± 1.84) and (6.09 ± 1.88), respectively. Stepwise multiple linear regression showed that fasting blood-glucose value was significantly associated with self-management behaviours of diet, exercise, blood-glucose testing and medication taking. Self-efficacy was significantly associated with self-management behaviours of diet, exercise and foot care. Diabetes distress, diabetes-related social activity, confrontation, education, duration of T2DM, treatment modalities and diabetes knowledge were associated with the one or two dimensions of SDSCA in the young adults with T2DM.

Abstract:Abstract Background: This study is aimed at identifying the factors associated with the self-management practices of type 2 diabetes among the middle-aged population of rural area of Pakistan.Methods: The sample of participants was purposively recruited from the medical clinic of rural area of Pakistan. The participants (n=200) recruited were asked to complete the questionnaire following their informed consents. The study questionnaire consisted of three main sections: socio-demographics, the Urdu-version of Summary of Diabetes Self-care Activities (U-SDSCA), and the extended version of U-SDSCA. In addition, glycosylated hemoglobin (HbA1c) level results were obtained from the medical clinic. The study explored the factors associated with the self-management practices of type 2 diabetes among the middle-aged population of rural area of Pakistan.Results: The current study has demonstrated that self-management activities level among the patients of type 2 diabetes in the rural area of Pakistan was not achieved in majority of patients, and that glycemic control was unsatisfactory. The medications sub-scale mean (Mean: 6.17 ± 1.18 SD) was the highest among all the U-SDSCA instrument sub-scales. The results of blood glucose monitoring subscale revealed that longer duration of diabetes ≥8 years) was associated significantly with poor glycemic control (Diff= -.299; p = .120).The multivariate regression analysis revealed that the participants’ characteristics accounted for 21% of the variability in the total self-management score (R2=0.211). In addition, women were more inclined to undertake appropriate diabetes self-management activities (β .302; p = .001). Conclusion: This study has identified the factors associated with the self-management activities among the patients with type 2 diabetes in rural area of Pakistan. In order to overcome the influence of these factors, healthcare professionals should pay special attention to the patients of middle-aged population of rural area of Pakistan and provide them educational programme that emphasizes lifestyle modification with importance of adherence to treatment and encourage them to make diabetes self-management activities as part of their day to day life.

Abstract:Context: Patients with type 2 diabetes (T2D) need to commit to permanent discipline to minimize the risks of disease. On the other hand, the approach to disease self-management has shifted from a solely biological and doctor-patient-oriented approach to a more comprehensive model with a cooperative approach that takes advantage of social factors such as the community and family. Therefore, it is necessary to comprehensively extract social factors affecting consumer discipline from scientific sources. Evidence Acquisition: This narrative review identified social factors T2D self-management. Relevant studies were retrieved from reputable databases including PubMed, Google Scholar, Iran Doc, Emerald, and ScienceDirect through a web search (with no temporal limitations). Out of the 18,082 retrieved articles, 68 studies resonating with research goals were selected and underwent qualitative analysis. Results: Four categories of social factors were identified, including family, diabetes educators, peers, and diabetes online communities. Family member behaviors were classified into three groups: Diabetes self-management facilitators, obstacles, and neutral behaviors. Studies on diabetes educators accentuated their essential capabilities. Also, two face-to-face peer support models were identified. Ultimately, diabetes online communities were investigated as an emerging context capable of identifying and covering the needs of diabetic patients and their families. Conclusions: Social factors, including family, educators, peers, and online communities, play a crucial role in T2D self-management. By recognizing these factors, healthcare professionals can design interventions that ultimately result in better outcomes for patients with T2D.
Mediating Effect of Self-Efficacy on Self-Management Ability and Self-Management Behavior in Patients with Type 2 Diabetes Mellitus

Abstract:OBJECTIVE:To investigate the mechanism of self-efficacy between self-management ability and self-management behavior and its differences among patients with different disease courses through mediation tests. METHODS:In the study, 489 patients with type 2 diabetes who attended the endocrinology departments of four hospitals in Shanxi Province and Inner Mongolia Autonomous Region from July to September 2022 were enrolled as the study population. They were investigated by General Information Questionnaire, Diabetes Self-Management Scale, Chinese version of Diabetes Empowerment Simplified Scale, and Diabetes Self-Efficacy Scale. Mediation analyses were performed using the linear regression model, Sobel test, and Bootstrap test in the software Stata version 15.0 and divided the patients into different disease course groups for subgroup analysis according to whether the disease course was > 5 years. RESULTS:In this study, the score of self-management behavior in the patients with type 2 diabetes was 6.16±1.41, the score of self-management ability was 3.99±0.74, and the score of self-efficacy was 7.05±1.90. The results of the study showed that self-efficacy was positively correlated with self-management ability (r=0.33) as well as self-management behavior (r=0.47) in the patients with type 2 diabetes (P < 0.01). The mediating effect of self-efficacy accounted for 38.28% of the total effect of self-management ability on self-management behaviors and was higher in the behaviors of blood glucose monitoring (43.45%) and diet control (52.63%). The mediating effect of self-efficacy accounted for approximately 40.99% of the total effect for the patients with disease course ≤ 5 years, while for the patients with disease course > 5 years, the mediating effect accounted for 39.20% of the total effect. CONCLUSION:Self-efficacy enhanced the effect of self-management ability on the behavior of the patients with type 2 diabetes, and this positive effect was more significant for the patients with shorter disease course. Targeted health education should be carried out to enhance patients' self-efficacy and self-management ability according to their disease characteristics, to stimulate their inner action, to promote the development of their self-management behaviors, and to form a more stable and long-term mechanism for disease management.
Self-care behaviors, medication adherence status, and associated factors among elderly individuals with type 2 diabetes

Abstract:Elderly individuals face an increased likelihood of developing chronic diseases such as diabetes. Self-care practices and medication adherence play crucial roles in preventing complications and adverse effects of this condition. Therefore, this study aimed to determine self-care behaviors, medication adherence status, and related factors among elderly patients with type 2 diabetes. This descriptive-analytical study was conducted on 374 elderly patients with type 2 diabetes who visited educational healthcare centers in Qazvin, Iran, during 2023 (March–September). Sampling was performed using the convenience method. Data collection instruments included a demographic characteristics checklist, the summary of diabetes self-care activities questionnaire, and the Morisky medication adherence scale. Data analysis was conducted using SPSS-22 software, employing the Kolmogorov–Smirnov test, mean, standard deviation, univariate and multivariate regression analyses. The significance level was set at p ≤ 0.05. The mean age of participants was 67.56 ± 5.93 years. In the self-care questionnaire, the highest score pertained to adherence to the diet recommended by the treating physician (3.16 ± 1.87). In contrast, the lowest scores were related to the frequency of checking inside shoes (0.17 ± 0.93) and foot examination (0.31 ± 1.07), respectively. Furthermore, results in self-care behaviors indicated that with increasing education levels, self-monitoring of blood glucose (SMBG) significantly decreased ( P = 0.048). This variable was considerably higher in rural residents than in urban dwellers ( P = 0.016). Additionally, the frequency of blood glucose measurements was significantly higher in urban residents than in rural inhabitants ( p = 0.006). Based on the results, the mean score for medication adherence among patients was 5.53 ± 1.65. Based on our findings, the level of self-care in physical activity, SMBG, and foot care among the elderly is below average. Furthermore, medication adherence in these patients is poor. We expect that managers and policymakers take steps to reduce complications and improve these two variables by developing educational programs on self-care and emphasizing the importance of treatment adherence for these patients.
