Abstract:Metabolic syndrome (MS) is likely to be associated with non‐alcoholic fatty liver disease (NAFLD). The prevalence of NAFLD in visceral fat type MS (V‐type MS) is known to be higher than in subcutaneous fat type MS (S‐type MS) in men with MS, and a larger subcutaneous fat area is reported to be not associated with NAFLD in women. We elucidated differences between V‐type S‐type MS in Japanese women with MS.

Abstract:Extended suicide, a specific type of homicide-suicide event, has severe social consequences yet remains lacking systematic research. This retrospective study investigated 51 cases of extended suicide involving mental disorders in central China with aim of better understanding risk factors for such events and guiding prevention strategies. Over an 8-year period from 2015 to 2022, cases were collected from forensic institutions, and demographic characteristics, case details, and psychiatric data were recorded. The 51 incidents involved 51 perpetrators and 79 victims, with more female perpetrators (58.8%) and more female victims (54.4%). The average age of the perpetrators was 36.1, and most were married (88.2%). Almost all of the victims were family members of the perpetrator, like the most numerous children (64.6%), followed by spouses (24.1%). The most common homicide mode of death was mechanical asphyxia (38.0%), followed by sharp devices (36.7%) and drug poisoning (16.5%). Depressive disorders (76.5%) were the most common diagnosis of mental disorder for perpetrators. The study analyzed the unique characteristics of extended suicide to enrich such data. These findings help strengthen the screening and identification of potential perpetrators and victims to prevent such cases from occurring.

Abstract:OMEGA - Journal of Death and Dying, Ahead of Print. This study compares the features of homicide-suicide in Russia and Pakistan. To understand this, we conducted a content analysis of news reports about homicide-suicide published in Pakistani and Russian newspapers between March 2020 and May 2023. We identified 35 and 104 homicide-suicide cases in Pakistani and Russian media, respectively. Men were three to five times more likely to commit homicide-suicides than women (3.16: 1 in Russia; 4.83: 1 in Pakistan). Accounting for over 65.73% of all homicide-suicides, spousal homicide-suicides and filicide-suicide were the most common homicide-suicides in both regions. Filicide-suicides were more often done by Russian women and extra-family homicide-suicides by Pakistani women. Reasons for homicide-suicides in Pakistani women were divorce or separation and in Russian women – mental disorders. With the difference of homicide in the victims, Pakistani victims were aged 15–30 years, while Russian were 31–45 years and 46 years and older. There were more similarities in homicide-suicides than differences.

Abstract:Background Homicide followed by suicide is rare, devastating and perpetrated worldwide. It is commonly assumed that the perpetrator had a mental disorder, raising concomitant questions about prevention. Though events have been reported, there has been no previous systematic review of the mental health of perpetrators. Aims Our aims were twofold. First, to identify whether there are recognisable subgroups of homicide–suicides in published literature and, secondly, to investigate the relationship between perpetrator mental state and aspects of the incident. Methods We conducted a systematic review of published literature on studies of homicide followed within 24 h by suicide or serious suicide attempt that included measures of perpetrator mental state. Results Sixty studies were identified, most from North America or Europe. Methodologically, studies were too heterogeneous for meta‐analysis. They fell into three main groups: family, mass shooter, and terrorist with an additional small mixed group. There was evidence of mental illness in a minority of perpetrators; its absence in the remainder was only partially evidenced. There was no clear association between any specific mental illness and homicide–suicide type, although depression was most cited. Social role disjunction, motive, substance misuse and relevant risk or threat behaviours were themes identified across all groups. Pre‐established ideology was relevant in the mass shooter and terrorism groups. Prior trauma history was notable in the terrorist group. Conclusion Research data were necessarily collected post‐incident and in most cases without a standardised approach, so findings must be interpreted cautiously. Nevertheless, they suggest at least some preventive role for mental health professionals. Those presenting to services with depression, suicidal ideation, relationship difficulties and actual, or perceived, changes in social position or role would merit detailed, supportive assessment over time.

Abstract:Background and Objective: Our understanding of homicide-suicide (H-S), a rare yet tragic event, is sparse. While the phenomenon has been studied in the West for many years, only limited literature is available from Asia and none to our knowledge from Pakistan. There is evidence of complexity of the interaction between cultural, societal, and psychological mechanisms underlying this phenomenon; therefore, research findings from the Western countries cannot be directly applied in non-Western societies. Our objective was to study homicide-suicides in Pakistan, describe the characteristics of offenders and victims, determine the types of H-S, and examine possible motives and any events prior to the offense. Methods: In the absence of any official data on homicide-suicides, we used newspaper surveillance approach of four most widely circulated Pakistani newspapers (one Urdu and three English Daily) for two years (1st January 2019 to 31st December 2020). Each case was categorized using the modified Marzuk et al., Tardiff, and Hirsch's classification of homicide-suicides. Descriptive statistics were used to analyze the data that was then compared with published literature. Results: There were 114 H-S incidents with 198 victims of homicide during the study period, reported in the newspapers. Familial H-S particularly filicide-suicide were predominant, followed by spousal/ consortial H-S. Forty-one (36%) H-S involved multiple victims. The perpetrators across all categories of H-S were predominantly male (67%); the victims were predominantly women and children. Firearms were used in most incidents. The primary motive for the majority of H-S cases was familial, financial, and social stressors. Conclusions: The study highlights several unique patterns (predominance of familicide, multiple victims including high proportion of children) and a series of vulnerabilities (incidents related mostly to familial/ financial and social stressors) that overlap each other and ultimately lead to this tragic end. There is need to increase our understanding and develop effective evidence-based prevention strategies for H-S in Pakistan. An empirical study of characteristics and types of homicide-suicides in Hong Kong, 1989-2005

Abstract:Background: Clinical classification of types of homicide-suicide (HS) was proposed, but no information on empirical-based classification and prevalence of different types of HS was available. This paper aimed to empirically classify HS events into different clusters and to discuss specific evidence-based prevention initiatives. Method: Data of HS offenders from Coroner's Court were analyzed through a two-step cluster analysis. Number of clusters and appropriate allocations of cases were obtained. External background variables were tested through post hoc tests to explore the differences among clusters. Results: Two hundred and thirty-one people died in 98 episodes in the study period (1989-2005). The majority of HS offenders were male (n=68, 68.7%) and aged 30-49 (n=62, 62.6%). Domestic killing was the major type of HS in which over 60% of the homicide motivation was related to spousal conflicts or altruistic reasons. Spouses (n=46, 46.5%) and children (n=47, 47.5%) were predominantly the victims. The common killing methods included chopping with weapons (n=33, 33.3%) and charcoal burning (n=22, 22.2%). Six clusters of HS were derived from the cluster analysis and were further reduced to four major classes. Four major classes were dispute, conflicts in a relationship, altruistic, and mental illness. These classes could be differentiated by methods of homicide and suicide, gender of perpetrator, relationship with victim, and indebtedness. Conclusions: Financial problem, dispute and domestic violence are significant precipitants of HS in Hong Kong. Those people associated with the precipitating factors should be the targets for intervention and prevention.

Abstract:Homicide-suicide is a tragic event characterized by multiple fatalities within the intimate relationships, familial bonds, and community networks. Our aim was to explore the extent of coverage of homicide-suicide cases in the newspaper reports delivered in Pakistan during the COVID-19 lockdown (March–August 2020). We conducted a quantitative content analysis of news- reports obtained from eight Pakistani newspapers published in English and Urdu languages and daily reviewed by the authors. A total of 50 news-reports have been selected and analyzed, documenting 107 deaths due to homicide-suicide incidents. The majority of these cases involved male perpetrators within family settings, such as parent-child and spousal/married couples, often attributed to factors such us familial discordance and poverty. Negligibly, reasons for homicide-suicides included mental health issues of the perpetrators. Gunshot wounds emerged as the most prevalent method of homicide-suicide. Given the familial backgrounds in most of cases, it is likely that they occurred within the domestic environments. These findings may suggest the need of specific family interventions across the country in order to prevent family discordances or cope with relevant issues within the families, as both may contribute to homicidal and suicidal incidents, either independently or concurrently.

Abstract:Background: This is a systematic review of suicide and homicide-suicide events involving aircraft. In aeromedical literature and in the media, these very different events are both described as pilot suicide, but in psychiatry they are considered separate events with distinct risk factors. Methods: Medical databases, internet search engines, and aviation safety databases were searched in a systematic way to obtain relevant cases. Relevant articles were searched for additional references. Results: There were 65 cases of pilot suicide and 6 cases of passengers who jumped from aircraft found. There were also 18 cases of homicide-suicide found involving 732 deaths. Pilots perpetrated 13 homicide-suicide events. Compared to non-aviation samples, a large percentage of pilot suicides in this study were homicide-suicides (17%). Discussion: Homicide-suicide events occur extremely rarely. However, their impact in terms of the proportion of deaths is significant when compared to deaths from accidents. There is evidence of clustering where pilot suicides occur after by media reports of suicide or homicide-suicide. Five of six homicide-suicide events by pilots of commercial airliners occurred after they were left alone in the cockpit. This, along with a sixth incident in which active intervention by a Japan Air crew saved 147 lives, suggests that having two flight members in the cockpit is potentially protective. No single factor was associated with the risk for suicide or homicide-suicide. Factors associated with both events included legal and financial crises, occupational conflict, mental illness, and relationship stressors. Drugs and/or alcohol played a role in almost half of suicides, but not in homicide-suicides.

Abstract:Purpose Suicide is a leading cause of death in prison and the suicide rates are several times higher in the prison population than in the general population in most countries. Of the studies that have investigated risk factors for suicide in prison, few have controlled for possible confounding factors. The aim of this study is to identify risk factors of suicide among people in French prisons, over a four-year period. Methods All incarcerations that occurred in France during 2017–2020 were eligible. Sociodemographic, criminal and prison characteristics were collected for each incarceration from data of the National Prison Service. Survival univariate and multivariate analyses were performed with a Cox regression model. Results 358,522 incarcerations were included, of which 449 ended in suicide during the follow-up. The median length of prison stay was 5.1 months. The median age at prison entry was 30 years and 95.8% of incarcerations involved men. The overall suicide rate was 173 [157–189] per 100,000 person-years. Factors associated with suicide in the multivariate model ( p < 0.05) were the early stage of incarceration and in particular the first week (HR = 7.6 [5.4–10.8]), violent offences and in particular homicide (HR = 3.0 [2.1–4.2]), French (HR = 1.7 [1.2–2.4]) and other European nationalities (HR = 2.1 [1.4–3.3]), age above 40 (HR = 2.0 [1.5–2.6]), pre-trial incarceration (HR = 1.8 [1.4–2.3]), being separated (HR = 1.6 [1.1–2.3] and having a high school diploma (HR = 1.4 [1.0–1.8]). Conclusions Factors associated with suicide in prison are complex and involve individuals' criminal history as well as conditions of incarceration. These characteristics may be relevant to focus suicide prevention efforts.

Abstract:As many as one-quarter of uroxicides [Johnson and Hotton (2003) Homicide Studies 7:58–84] and filicides [Wilczynski (1997) Child Homicide, London: Greenwich Medical Media] end in the suicide of the offender. As in homicide- or suicide-only events, homicide–suicide (HS) can arise from a variety of circumstances, ranging from relationship breakdown, to financial and health problems. However, to date our understanding of this disheartening phenomenon remains limited. As an HS event comprises both a homicidal and a suicidal component, the question of whether HS is primarily an extended suicide dominated by weariness or an alternate form of homicide overwhelmed by anger is a contentious topic. This article argues that there are at least two types of HS, with one type being dominated by hostility against the victim, whilst in the other hostility is not apparent. Based on HS events that occurred between 1989 and 2003 in Hong Kong, significant differences are found between these two types of HS in terms of offender and victim characteristics, antecedent, offender–victim relationships, situational factors and modes of killing. This is the first HS typological classification built on the hostility debate that has been validated with empirical data.

Abstract:Serious mental illness is associated with a higher risk of violent interpersonal behavior. Characteristic traits denoting a psychopatho-logical picture such as schizophrenia, depression, a borderline, or narcissistic, antisocial, or paranoid personality disorder, are frequently observed in the personality of those who have committed one or more murders. For example, mass murderers are affected by a delusional syndrome with a persecution fixation, that develops in a subject with a paranoid, anacastic personality disorder. An abnormal mental frame-work develops as a consequence of continual experiences interpreted as traumatic. We present a complex case regarding a double murder whose vic-tims were shot by a 50-year-old neighbor, who underwent a psychiatric evaluation for the assessment of criminal responsibility. In the light of the historical reconstruction of his relation sensitive delusional mindset that triggered the final reaction, his life history, the "key event" and the "triggering environment" (constant conflictual and dysfunctional rela-tions with others) led to the violence. The correct understanding of the reported case demands investigation not only of the clinical, diagnostic and psychodiagnostic aspects, but also of the perpetrator-victim/s, their relational context and the situation where the crime evolves, in order to be able to reconstruct the event in a meaningful manner.

Abstract:An active sensor system using parallel processing circuits is proposed and its characteristics are discussed. From the viewpoint of a model of sensory processing mechanism of active touch, the system uses information of efferent copy and internal model of actuator in order to generate active motions from the local pattern information sensed by local pattern sensors such as tactile sensors. In this paper, an experimental system and its basic experimental results are also shown. The experimental system is a sensor system for active perception of the shape of twodimensional objects by tracing the edge of the objects. The system realizes both high speed processing of the local pattern and real time control of the actuator.

Abstract:The purpose of this study was to examine the prevalence and characteristics (victims' profiles, circumstances surrounding the incidents, and methods employed) of complex and complicated suicides over a 12-year period in the broader area of Athens, Greece. A retrospective analysis of 5,568 autopsy cases performed at the Department of Forensic Medicine and Toxicology of the National and Kapodistrian University of Athens from January 1, 2011, to December 31, 2022, was carried out. Out of a total sample of 5,568 autopsies, 360 suicide cases were identified, among which 14 (3.9%) were classified as complex suicides, and one case (0.3%) was identified as complicated suicide. Among the victims, 78.6% were males. The age range of the victims varied between 25 and 82 years old. The most prevalent method of complex suicide was the use of sharp objects followed by jumping from a height (42.8%). The next most common combination of methods was poisoning (21.4%) along with hanging. Prior suicidal attempts and suicide note were mentioned in 16.7% and 8.3% of the cases respectively. Overall, a total of 9 different methods were used in the above 14 cases. Only half (50.0%) of the victims had an established psychiatric diagnosis. In determining the cause of death in cases of a complex or complicated suicides, it is of utmost importance for the forensic pathologist to gather and analyze all available information provided by the police, the victim's relatives along with a thorough investigation of the scene, a detailed autopsy and a toxicological analysis.

Abstract:Combined homicide-suicide (H-S) is a phenomenon described as an offender committing a homicidal act followed by their suicide. Current literature on H-S is dominated by a focus on men and their particular set of motivations and actions, primarily because females constitute only a small fraction of the cases of completed H-S. This review begins by analyzing this data and integrating females within two subclassifications of H-S: the psychopathology of the actor (or perpetrator), and the actor's relationship to her homicide victims. Within the relational subcategory of H-S, females are: (1) underrepresented when victims are their spouses or intimate partners (consortial H-S); (2) more prevalent when victims are their own children (filial H-S); and (3) with rare exceptions, not represented in extrafamilial, adversarial and pseudo-commando H-S perpetrators. This review includes female bombers in this gender comparison.

Abstract:Introduction The objective of this study was to compare jealousy as a correlate of intimate partner homicide‐suicide cases to homicide‐only cases using data from the National Violent Death Reporting System (NVDRS), a state‐based surveillance system maintained by the Centers for Disease Control and Prevention. Jealousy is categorized as jealous feelings or distress over a current or former intimate partner's relationship or suspected relationship with another person. Methods NVDRS data from a 5‐year period (2016–2020) was used to estimate frequencies and identify significant differences in jealousy and other sociodemographic, mental health, relationship, and incident‐related correlates of intimate partner homicide‐suicide compared to homicide‐only cases. Cases were included if they involved an intimate partner single homicide or a single homicide followed by suicide. Results The study sample included 5335 cases (intimate partner homicide‐suicide n = 1402; homicide‐only n = 3933). A significantly higher percentage of intimate partner homicide‐suicide cases reported jealousy preceding the event (9%) compared to homicide‐only cases (6%). Compared with homicide‐only cases, homicide‐suicide cases had 3.5 greater odds of recording jealousy as a precipitating event. Conclusion Findings suggest that intimate partner homicide‐suicide cases are distinct from homicide‐only cases both in terms of individual‐ and incident‐level and situational factors, including the presence of jealousy.

Gender differences in homicides. A comparative analysis of 106 fatalities in forensic autopsy data

Abstract:While most homicides worldwide are committed against men, women and girls are disproportionately affected by domestic violence and its fatal consequences. The Istanbul Convention mandates the prevention of gender-based violence, particularly against women. This study analyzes the characteristics of male and female victims of homicides to provide a comprehensive understanding of the different situations and ways in which men and women are killed. Files of 106 forensic autopsies of homicide victims aged 14 years and older (51.9% males, 48.1% females) from 2012 to 2019 were reviewed. Demographic data, previous history with a focus on the perpetrator-victim relationship, substance influence, type of violence and law enforcement data were recorded. A gender-separated statistical analysis was carried out. Male victims were killed most often outside their own home (63.6%) by acquaintances or friends (54.5%). They were frequently under the influence of substances (56.4%). The perpetrators suffered from addictive diseases in 49.1%, and 52.7% had prior convictions. In both groups, stab/cut injuries were the most common causes of death, but stab/cut and gunshot injuries occurred more frequently in male victims. Female victims were killed most often in their own homes (76.5%) by intimate partners (62.7%). Compared to male victims, violent asphyxiation and blunt force trauma were more common causes of death. Furthermore, there was a higher frequency of multiple injuries (33.3%). The prevalence of a guilty verdict of the perpetrators was nearly equal in both groups. Gender-specific aspects should be considered when investigating homicides and establishing prevention and intervention strategies for interpersonal violence. Protective concepts are easier to establish in the public sphere than in private homes, which is why homicides against women require special attention in society, law enforcement, and legislation. Significant gender differences were found in the circumstances and the perpetrator-victim relationships of homicides. Gender-specific aspects should be taken into account when investigating homicides and developing prevention and intervention strategies. Abstract:This article describes suicide-related penal legislation in contemporary Europe, and analyzes and relates the results to cultural attitudes towards suicide and to national suicide rates. Data were obtained from 42 legal entities. Of these, 34 have penal regulations which--according to definition--chiefly and directly deal with suicide. There are three main types of act: aiding suicide, abetting suicide, and driving to suicide. The laws vary considerably with regard to which acts are sanctioned, how severely they are punished, and whether any special circumstances such as the motive, the result, or the object can make the crime more serious. Various ideologies have inspired legislation: religions, the euthanasia movement, and suicide prevention have all left their mark. There are some cases in which neighboring legal systems have clearly influenced laws on the topic. However, the process seems mostly to have been a national affair, resulting in surprisingly large discrepancies between European legal systems. The laws seem to reflect public opinions: countries which punish the crimes harder have significantly less permissive cultural attitudes towards suicide. Likewise, suicide rates were significantly higher in countries with a narrow scope of criminalization and milder punishments for suicide-related crimes. The cultural and normative elements of society are connected with its suicide mortality.

Abstract:Abstract Complex suicide is defined as death by the intentional use of more than 1 method to end one's life. Complex suicides can fit into the planned or premeditated category, where 2 or more methods are used simultaneously or in close succession, or unplanned, the use of 2 or more methods, in which the first method attempted fails (often because it is too unpleasant or painful), and another method is employed subsequently that is successful. A retrospective review of 3 years of suicide fatalities examined by the Washoe County Regional Medical Examiner's Office in Reno, Nevada, was conducted. A total of 725 suicide cases dated from April 1, 2020, to April 1, 2023, were reviewed, including autopsy, toxicology, and scene investigation reports. Among the 725 suicide cases reviewed, 19 cases (2.62%) of complex suicide were identified. Of those cases, 17 employed 2 methods, and 2 cases employed 3 methods. No cases were identified with >3 methods. In the 19 complex suicides that were identified, gender, age, marital status, military service, sexual orientation, religion, race, occupation, incident location, suicide note, suicidal ideation history, past suicide attempt, psychiatric disorders, and motive for suicide were examined in addition to methods used.
