Abstract:Background: Firefighting is among the most dangerous professions and requires exceptional physical fitness and focus while working. Patient-reported outcomes are a commonly used method to evaluate subjective health information and may be utilized by fire departments to identify the health status of firefighters and provide insight to promote their health and wellness. Objective: This study is a novel analysis of firefighters' self-reported health to potentially identify musculoskeletal dysfunction, assist in therapeutic intervention, and improve overall health and wellness. Methods: Firefighters were evaluated using seven different self-reported health surveys to assess various physical capabilities and quality of life. The questionnaires were delivered via online format and administered once to provide a snapshot of a suburban Oklahoma fire department. Results: Using the Disablement in the Physically Active Scale, 14 of the 35 firefighters answered "slight, moderate, or severe" for the pain and motion variables. Only two of the firefighters indicated no stiffness or soreness after activity on the Nirschl Phase Rating Scale. The firefighters mean rating for "energy/fatigue" via the RAND-36 was 54.14 out of 100. Conclusion: Firefighters frequently report pain, impaired motion, and soreness, indicating areas in which health and wellness interventions may be helpful. The incorporation of periodic health surveys into firefighter health and wellness programming can highlight the presence of concerns, as well as intervention effectiveness by subjective health status reporting. By combining the health surveys with aerobic and core strength exercises, fire departments may be able to monitor and improve firefighter health.

Abstract:The purpose of this study was to utilize community-based participatory research (CBPR) methods to identify behaviors that may increase the exposure rates of firefighters to carcinogens and other occupational hazards. Key informant interviews and focus groups were conducted as part of a larger study that included exposure assessments at fire stations, in fire engines, and in personal vehicles. A purposive sample of five fire chiefs and leaders of the firefighter association was chosen, and these individuals were selected to participate in interviews. Unstructured interviews explored issues related to firefighter exposures and occupational hazards. Three focus groups were conducted over a three-month period. Following the focus groups, survey questions emphasizing the following three aspects of firefighting were developed: exposure to airborne smoke contaminants during fire suppression, accumulated exposure on turnout gear after fire suppression, and exposure to airborne contaminants at the fire departments. The use of community-based participatory research can be very beneficial, especially when conducting research with a group that may be hard to reach or have misgivings regarding researchers. By utilizing this approach in the current study, researchers were able to partner with a community that may be hard to reach and gain buy-in from community leadership.

An updated analysis of safety climate and downstream outcomes in two convenience samples of U.S. fire departments (FOCUS 1.0 and 2.0 survey waves)

Abstract:Background: The Fire service Organizational Culture of Safety (FOCUS) survey is an assessment tool comprised of psychometrically validated metrics of safety climate, safety behavior, and downstream outcomes (organizational and injury) that are specific to the U.S. fire and rescue service. Methods: This analysis consists of a descriptive summary of two independent survey waves (FOCUS 1.0 and 2.0). The fire departments included in these survey waves were from convenience sampling (n1.0 = 275; n2.0 = 170). In addition to department level characteristics, we examined individual level characteristics for firefighters and EMS providers in participating departments (n1.0 = 22,719; n2.0 = 16,882). We conducted regression analyses to examine the associations between safety climate and safety behaviors, organizational outcomes, and safety outcomes. All analyses were stratified by organization type (career, volunteer). Results: Our analysis indicated that a majority of respondents were males (90.7%FOCUS 1.0; 90.4%FOCUS 2.0), non-officers (68.4%FOCUS 1.0; 66.4%FOCUS 2.0), and non-Hispanic Whites (70.8%FOCUS 1.0; 69.5%FOCUS 2.0). For both samples there was a higher prevalence of injuries among individuals in career departments (nFOCUS 1.0 = 3778 [17.5%]; nFOCUS 2.0 = 3072 [18.7%]) than volunteer departments (nFOCUS 1.0 = 103 [8.8%]; nFOCUS 2.0 = 34 [7.4%]). We observed an approximate 10-point difference between the mean scores of Management Commitment to Safety for career and volunteer departments in both samples. We observed associations for two organizational outcomes, Safety Behavior and Job Satisfaction, with Management Commitment to Safety and Supervisor Support for Safety overall and when stratified by organization type. We observed a decrease in the odds of injuries associated with a one-unit increase in Management Commitment to Safety (OR1.0 overall: 0.98, 95% CI 0.97-0.99; OR2.0 volunteer: 0.90, 95% CI 0.85-0.95) and Supervisor Support for Safety (OR1.0 overall: 0.95, 95% CI 0.93-0.97; OR1.0 career: 0.95, 95% CI 0.92-0.98). Conclusions: From our current study, and a prior analysis of a geographically stratified random sample of U.S. fire departments, we identified that from all the organizational outcomes, job satisfaction was most consistently associated with FOCUS safety climate. Further, firefighters in our samples consistently rated Supervisor Support for Safety higher than Management Commitment to Safety. Future interventions should support fire departments in improving their departmental Management Commitment to Safety and maintaining their Supervisor for Safety.

Abstract:Objective: This study aimed to examine the COVID-19 pandemic's impact on fire service safety culture, behavior and morale, levers of well-being, and well-being outcomes. Methods: Two samples (Stress and Violence against fire-based EMS Responders [SAVER], consisting of 3 metropolitan departments, and Fire service Organizational Culture of Safety [FOCUS], a geographically stratified random sample of 17 departments) were assessed monthly from May to October 2020. Fire department-specific and pooled scores were calculated. Linear regression was used to model trends. Results: We observed concerningly low and decreasing scores on management commitment to safety, leadership communication, supervisor sensegiving, and decision-making. We observed increasing and concerning scores for burnout, intent to leave the profession, and percentage at high risk for anxiety and depression. Conclusions: Our findings suggest that organizational attributes remained generally stable but low during the pandemic and impacted well-being outcomes, job satisfaction, and engagement. Improving safety culture can address the mental health burden of this work.
Risk management of emergency service vehicle crashes in the United States fire service: process, outputs, and recommendations

Abstract:Background: Emergency service vehicle crashes (ESVCs) are a leading cause of death in the United States fire service. Risk management (RM) is a proactive process for identifying occupational risks and reducing hazards and unwanted events through an iterative process of scoping hazards, risk assessment, and implementing controls. We describe the process, outputs, and lessons learned from the application of a proactive RM process to reduce ESVCs in US fire departments. Methods: Three fire departments representative of urban, suburban, and rural geographies, participated in a facilitated RM process delivered through focus groups and stakeholder discussion. Crash reports from department databases were reviewed to characterize the context, circumstances, hazards and risks of ESVCs. Identified risks were ranked using a risk matrix that considered risk likelihood and severity. Department-specific control measures were selected based on group consensus. Interviews, and focus groups were used to assess acceptability and utility of the RM process and perceived facilitators and barriers of implementation. Results: Three to six RM meetings were conducted at each fire department. There were 7.4 crashes per 100 personnel in the urban department and 10.5 per 100 personnel in the suburban department; the rural department experienced zero crashes. All departments identified emergency response, backing, on scene struck by, driver distraction, vehicle/road visibility, and driver training as high or medium concerns. Additional high priority risks varied by department; the urban department prioritized turning and rear ending crashes; the suburban firefighters prioritized inclement weather/road environment and low visibility related crashes; and the rural volunteer fire department prioritized exiting station, vehicle failure, and inclement weather/road environment related incidents. Selected controls included new policies and standard operating procedures to reduce emergency response, cameras to enhance driver visibility while backing, and increased training frequency and enhanced training. The RM process was generally acceptable to department participants and considered useful. All departments reported that the focused and systematic analysis of crashes was particularly helpful. Implementation of controls was a commonly cited challenge. Conclusions: Proactive RM of ESVCs in three US fire departments was positively received and supported the establishment of interventions tailored to each department's needs and priorities.

Abstract:One of the practical contributions of epidemiology studies is to inform risk assessment and management to protect public health. However, there is a perception among some that environmental and occupational epidemiology is falling short of satisfying the needs of risk assessors. The specific reasons for this are not clearly understood. To help identify the points of dissatisfaction and possible areas for mutual learning, we conducted a survey of risk assessors, seeking their opinions of epidemiology research. We present a few quantitative measures and a thematic analysis of responses to open-ended questions. Survey results suggest that some risk assessors (with some adamant exceptions) believe that epidemiology has great potential to contribute to risk assessment but can be deficient in many ways. For example, respondents identified the lack of full disclosure of methods, deficiencies in exposure assessment, the absence of comprehensive uncertainty analyses, and the failure to investigate or explore thresholds of effects as some of the common shortcomings. These could be straightforward to address. Respondents also brought up a wide collection of more complicated and subtle concerns that could lead to further improvement of useful results. We identify areas where mutually-educating interdisciplinary dialogue seems particularly promising. Epidemiology research is expensive, and risk management decisions even more so; therefore, it is desirable for the risk assessment and epidemiologic communities to work toward making epidemiologic research more useful for informing decisions.

Abstract:Abstract Introduction Burn injuries among people experiencing homelessness (PEH) are a major and escalating public health problem affecting a population facing issues of socioeconomic disparities, racism, comorbidities (e.g., disabilities, substance use disorders), and inequitable housing policies. Given the intensifying issue of burn injuries among PEH, it is crucial to Identify key drivers of fire risk in this already marginalized population to inform injury prevention and control initiatives. Methods Key informant interviews (KII) with PEH who experienced burn injuries were conducted to identify fire risks and opportunities to mitigate hazards. Transcripts were coded using deductive and inductive strategies grounded in harm reduction theory. For the deductive component, an established conceptual model was applied to group factors into the following categories: weather conditions, neighborhood characteristics, group behavior, individual behavior, dwelling characteristics, and fire sources and containment. For the inductive component, emerging themes were identified and coded. Results Participants from 13 KII had varied etiologies of their burn injuries (e.g., sleeping by fire, assault by fire, tent fire, car heater contact burn, propane tank explosion). The most common fire sources identified were propane, hand sanitizer, and other alcohol-based liquids. Unsafely contained open flames and improper propane tank storage were frequently expressed hazards. Participants described the interplay of environmental, social, and behavioral factors that need to be accounted for when addressing fire hazards (Figure 1). For example, participants highlighted the relationship between fire risks and acts of violence (e.g., theft of fuel sources, assault, forced relocation), as well as the multitude of considerations that impact fuel choice (e.g., accessibility, cost, smoke exposure, burn duration, flame control, weather conditions). Furthermore, participants described other specific opportunities for prevention within the intersectional context of the environments and risks they experience. Conclusions A variety of fire hazards were identified and amenable to harm reduction (e.g., safe stove dissemination, education on safe storage, availability of warming shelters). To be effective, implementation of fire prevention and control strategies must be evaluated for their effectiveness in complex environments (e.g., exposure to severe weather, unsafe and potentially violent living conditions, substance use, social marginalization, and poor access to injury prevention media). Applicability of Research to Practice Understanding environmental and socioeconomic risks and their interplay with fire hazards is foundational for collaborative injury prevention and control initiatives to be implemented by PEH, homelessness service providers, public health officials, and burn centers.

Perceptions of Urban Community Resilience: Beyond Disaster Recovery in the Face of Climate Change

Abstract:Resilience of human systems has increasingly become a popular topic of research. The aim of this article is to present a juxtaposition of public officials' and residents' perceptions of community resilience along the three-class typology of resilience (basic, adaptive, and transformative) using Dayton, OH as a case study. A two-pronged data collection approach was designed to recruit public officials and residents. This approach was structured using the Community Capitals Framework. A multi-chain referral sampling process (and subsequent snowball sampling) was initiated subsequently. The data were gathered through semi-structured interviews with 75 participants. The interviews were analyzed using a three-tiered deductive structural coding approach. The findings highlight the similarities and differences in resilience perceptions between public officials and community members along the three-class typology of resilience that could inform creative policy initiatives. The factors that might undergird residents' and public officials' perceptions of resilience are discussed. Based on these perceptions, the importance of social capital, communication infrastructure, and addressing chronic stressors are discussed as important strategies to build community resilience, in addition to focusing on essential community infrastructure systems (such as roads, energy, water, sewer, and gas systems).

Abstract:Firefighters are trained to respond to a wide range of emergencies, and over the course of responding to emergencies, are exposed to a variety of potentially traumatic events. Repeated exposure to traumatic events may adversely impact firefighters' mental health. Combined with stigma surrounding help-seeking, lack of systematic resources and referral programs, and other barriers to care, firefighters do not routinely receive a continuum of trauma informed, assessment and treatment services from trained professionals. The purpose of this paper is to describe an evidence-informed approach to a reciprocal, collaborative model among social work and public health practice, policy, and research; and to offer a framework for researchers, clinicians, and policy advocates to more effectively achieve goals of preventing and treating trauma exposure in firefighters. This paper provides a comprehensive literature review, as well as describes a case study of a successful public health intervention for mitigating exposure to trauma among a population of firefighters. The significant contribution of mental health issues to the burden of disease among our firefighters is a matter of considerable public health concern. Effective approaches to mitigating behavioral health needs must bridge both mental health disciplines and public health to minimally, include access to evidence-based assessment and treatment, peer support capacity building, widely available mental health education, and leadership development to promote culture change. When considering a model for services with firefighters, reciprocal collaboration among researchers, health care professionals, first responder organizations, families, and other relevant community stakeholders is essential for success.

Integrating Pixel- and Polygon-Based Approaches to Wildfire Risk Assessment: Application to a High-Value Watershed on the Pike and San Isabel National Forests, Colorado, USA

Abstract:We develop a novel risk assessment approach that integrates complementary, yet distinct, spatial modeling approaches currently used in wildfire risk assessment. Motivation for this work stems largely from limitations of existing stochastic wildfire simulation systems, which can generate pixel-based outputs of fire behavior as well as polygon-based outputs of simulated final fire perimeters, but due to storage and processing limitations do not retain spatially resolved information on intensity within a given fire perimeter. Our approach surmounts this limitation by merging pixel- and polygon-based modeling results to portray a fuller picture of potential wildfire impacts to highly valued resources and assets (HVRAs). The approach is premised on using fire perimeters to calculate fire-level impacts while explicitly capturing spatial variation of wildfire intensity and HVRA susceptibility within the perimeter. Relative to earlier work that generated statistical expectations of risk, this new approach can better account for the range of possible fire-level or season-level outcomes, providing far more comprehensive information on wildfire risk. To illustrate the utility of this new approach, we focus on a municipal watershed on the Pike and San Isabel National Forests in Colorado, USA. We demonstrate a variety of useful modeling outputs, including exceedance probability charts, conditional distributions of watershed area burned and watershed impacts, and transmission of risk to the watershed based on ignition location. These types of results can provide more information than is otherwise available using existing assessment frameworks, with significant implications for decision support in pre-fire planning, fuel treatment design, and wildfire incident response.

Bridging the Gap From Rural Trauma to Rural Healthcare: Fire Department Education Sessions

Abstract:Introduction: Many rural communities within the United States rely upon volunteer firefighters and EMS personnel in emergency response scenarios. However, it has been found that "medical direction in rural EMS was identified as a major issue for a majority of states" within America. With the consideration that some firemen can arrive on the scene ~15 minutes before an ambulance, a gap exists in time and skills where knowledge of basic emergency response techniques could equip firefighters to assist EMS personnel in life-sensitive scenarios. Methods: Our study took place in Parkston, South Dakota with a volunteer crew of ~ 30 firefighters and EMS personnel. Outside of their pre-scheduled bi-weekly meetings, our study provided further training for firefighters with various emergency response skills. Three nights of training were provided with pre-and post-surveys administered for each training station, analyzing confidence levels on a five-point system. Mean pre- and post-training confidence levels were obtained. Standard deviation, standard error, 95% confidence interval, and percent increase in mean confidence levels where calculated. Grouped bar graphs were plotted and analyzed for statistical significance. Results: With the exception of two stations in night 2, all remaining training stations in nights 1, 2, and 3 demonstrated a statistically significant increase in mean confidence levels for each skill being taught. Conclusions: Based on the data obtained from this study, a statistically significant increase in mean confidence levels per training station demonstrates value in continued delivery of rural training sessions to firefighter personnel. As firefighter and EMS numbers across the United States continue to decline, timing is becoming increasingly more important in emergency response scenarios within our country's rural communities. Creating confident firefighters that are trained in skills beyond basic first aid and CPR may provide immense value for the continued evolution of rural emergency healthcare.

Abstract:Objective: Public preparedness for natural hazard events is low. With worsening severe weather events due to climate change, public health policy and practices must evolve to more effectively engage communities. This study's findings identify and suggest new strategic public health policies to shift the practice of all‐hazards preparedness into routine, everyday life.Methods: Semi‐structured interviews, focus groups and Thematic Analysis were used to investigate the interactions between participant groups: emergency responders and animal owners.Results: Three policies designed to improve human safety and well‐being are proposed and discussed. These are (i) a new system of workplace leave, (ii) an innovative regime of financial incentives for fire‐ready properties, and (iii) review of the use of firebreaks on farms and rural blocks.Conclusion: Policies proposed in this research aim to proactively narrow the awareness‐preparedness gap and build adaptive capacity to minimise risk to human health in all‐hazards contexts. Further research could evaluate the efficacy of trialled public policy.Implications for public health: These new policies seek to contribute to establishing and maintaining a culture of preparedness as a routine aspect of everyday life, and thus promote and protect public health in the short, medium and long terms.

Abstract:A water main break that contaminated the Boston area's water distribution system prompted a four‐day “boil water” order. To understand risk communication during this incident, 600 randomly sampled residents were mailed questionnaires, yielding 110 valid responses. This article describes how perceptions of different social stakeholders influenced whether respondents complied with the Protective Action Recommendation—PAR (i.e., drank boiled water), took alternative protective actions (i.e., drank bottled water or/and self‐chlorinated water), or ignored the threat (i.e., continued to drink untreated tap water). Respondents perceived technical authorities (i.e., water utility, public health, and emergency management) to be higher on three social influence attributes (hazard expertize, trustworthiness, and protection responsibility) than public (i.e., news media, elected officials) and private (i.e., self/family, peers, and personal physicians) intermediate sources. Furthermore, respondents were most likely to comply with the PAR if they perceived authorities and public intermediates to be high on all three attributes and if they had larger households and lower income. Contrarily, they were more likely to take alternative actions if they were younger and had higher levels of income, risk perception, and emergency preparedness. These results underscore the need for technical authorities to develop credibility with their potential audiences before a crisis occurs. Abstract:In recent years, altered forest conditions, climate change, and the increasing numbers of homes built in fire prone areas has meant that wildfires are affecting more people. An important part of minimizing the potential negative impacts of wildfire is engaging homeowners in mitigating the fire hazard on their land. It is therefore important to understand what makes homeowners more or less willing to take action. The research presented here comes from a study that interviewed a total of 198 homeowners in six communities in the western United States about the activities they had undertaken to mitigate their fire risk, the factors that contributed to their decisions, and their future intentions. The current paper reports on findings from the first half of the longitudinal study, after 3 years we will return to interview the current homeowner on the same properties to assess maintenance actions and facilitating and limiting factors. Overall we found a body of individuals who understand the fire risk, are taking numerous mitigation actions, and think that these actions have reduced their risk. These homeowners typically did not expect the government to do it for them: they wanted information about what to do and, in some cases, assistance with the work, but saw taking care of their property primarily as their responsibility. Responses also show that key information sources and motivating factors vary by location and that it is not inherently necessary to have relationships between community members to create defensible space.

Abstract:In this paper, we make the case for more social science research into fire incidents and fire-related risk behaviour. Unlike other vulnerabilities, such as crime, illness or risk-associated activities such as smoking, or accident avoidance, remarkably little research has focused on this area. This is perhaps surprising given the propensity for fire, its emotional, social and economic impacts, and evidence that fires and fire victims are not equally distributed across socio-demographic or geographical domains. In making our case, we outline: recent numbers and trends in incidents in the UK, focusing on domestic incidents and recent policy developments affecting fire and rescue services. Next, we review the social-science based literature on fire incidents, suggesting that while this offers useful insight, much more needs to be done to develop a rigorous evidence base. While we would not want to dismiss or downplay existing social science contributions, our contention is that a considerable number of opportunities exist for further work in this area. Consequently, we propose a number of ways in which popular ideas about risk theory can be applied to a domestic fire context and raise a number of questions that social scientists are well positioned to contribute to an interdisciplinary understanding of domestic fire incidents and associated risks.

Abstract:In 2016, the USDA Forest Service, the largest wildfire management organization in the world, initiated the risk management assistance (RMA) program to improve the quality of strategic decision-making on its largest and most complex wildfire events. RMA was designed to facilitate a more formal risk management process, including the use of the best available science and emerging research tools, evaluation of alternative strategies, consideration of the likelihood of achieving objectives, and analysis of tradeoffs across a diverse range of incident objectives. RMA engaged personnel from a range of disciplines within the wildfire management system to co-produce actionable science that met the needs of the highly complex incident decision-making environment while aiming to align with best practices in risk assessment, structured decision-making, and technology transfer. Over the four years that RMA has been in practice, the content, structure, and method of information delivery have evolved. Furthermore, the RMA program’s application domain has expanded from merely large incident support to incorporate pre-event assessment and training, post-fire review, organizational change, and system improvement. In this article, we describe the history of the RMA program to date, provide some details and references to the tools delivered, and provide several illustrative examples of RMA in action. We conclude with a discussion of past and ongoing program adaptations and of how this can inform ongoing change efforts and offer thoughts on future directions.

Abstract:Purpose The purpose of this paper is to articulate the results of a comprehensive literature review and grassroots outreach with first responder organizations to present an operationalized framework for organizations to utilize as a blueprint in developing customized behavioral health access program (BHAP) programs. Design/methodology/approach Historically, authorities having jurisdiction (AHJ)over fire service organizations have primarily offered behavioral health interventions through Employee Assistance Programs (EAPs) or commercial insurance carriers. These programs are necessary but may prove insufficient to meet the scope and needs of trauma-exposed firefighters and the firefighters' families. Findings A BHAP is a comprehensive and operationalized plan which clearly specifies the mental health services fire department members and families need, where those services are available within their communities and levels and standards of care that are expected in the provision of these services. Originality/value The BHAP is becoming a world standard of behavioral health care for first responders. While some fire service agencies are beginning to create BHAP guides, developing and implementing a BHAP can be time consuming and overwhelming, particularly for departments with limited internal and external resources. While the results of this review focus on BHAP within the fire service, this framework is applicable across all first responder professions.
Exploring practices, challenges, and priorities for human health and ecological risk assessments in Indigenous communities in Canada

Integrated Environmental Assessment and Management

Abstract:Indigenous peoples in Canada are disproportionately exposed to environmental contaminants and may face elevated health risks related to their unique cultural, spiritual, and economic relationships with the land, including the use of traditional food systems. However, to date, institutionalized approaches to assess risks to human and ecological health from contaminants have not been well developed or implemented with Indigenous community contexts in mind. There is regulatory interest in developing new approach methods for risk assessment, and thus an opportunity to increase their relevance to Indigenous communities in which they will be ultimately applied. Therefore, we conducted an anonymous mixed‐methods survey of those involved with risk assessment in Indigenous communities in Canada to: (1) understand risk assessment practice in Indigenous communities, (2) explore challenges with conventional assessment methods and compare these across sectors, and (3) gather perspectives on the development of new approaches. In all, 38 completed survey responses were received (14% response rate). Respondents were from Indigenous community environment and health offices (21% of respondents), Indigenous governments (8%), federal and provincial governments (21%), and academia (45%). Risk communication was seen as the most challenging part of risk assessment (71% responded "difficult"), and nearly all respondents agreed that time (86%), cost (76%), and resource availability (86%) were "moderate" to "serious" problems. Few respondents (16%) had heard of "new approach methods" for risk assessment, and 76% of respondents (and 100% of community‐based respondents) agreed on the need to develop improved risk assessment approaches. To modernize risk assessment, respondents recommended advancing cumulative risk assessment methods, improving risk communication, and promoting Indigenous leadership and Traditional Knowledge in assessment activities. Abstract:Community initiatives can shape health behaviors, such as physical activity and dietary habits, across a population and help reduce the risk of developing chronic disease. To achieve this goal and impact health outcomes, Pasadena Vibrant Community aimed to engage communities in an ongoing dialogue about the importance of healthy behaviors, implement and advance community-based strategies to promote health, and improve diet and physical activity behaviors. The initiative was centered around a collaboration between a backbone organization, steering committee, and 7 collaborating organizations funded to implement multicomponent, evidence-based programs.. The common agenda was detailed in a community action plan, which included 19 interventions targeting healthy eating and active living among adults and youth in Pasadena, Texas.
