Abstract:Imaging-based screening is an important public health focus and a fundamental part of Diagnostic Radiology. Hence, radiologists should be familiar with the concepts that drive imaging-based screening practice including goals, risks, biases and clinical trials. This review article discusses an array of imaging-based screening exams including the key epidemiology and evidence that drive screening guidelines for abdominal aortic aneurysm, breast cancer, carotid artery disease, colorectal cancer, coronary artery disease, lung cancer, osteoporosis, and thyroid cancer. We will provide an overview on societal interests in screening, screening-related inequities, and opportunities to address them. Emerging evidence for opportunistic screening and the role of AI in imaging-based screening will be explored. In-depth knowledge and formalized training in imaging-based screening strengthens radiologists as clinician scientists and has the potential to broaden our public health leadership opportunities. SUMMARY SENTENCE: An overview of key screening concepts, the evidence that drives today's imaging-based screening practices, and the need for radiologist leadership in screening policies and evidence development.
Signal subspace approach for speech enhancement in nonstationary noises

Abstract:In this paper, we propose a speech enhancement algorithm that is based on signal subspace approach (SSA) combined with RL noise estimation for nonstationary noises. A signal/noise Karhunen-Loeve transform (SNK) is used to implement signal subspace decomposition for noisy speech signal. RL noise estimation is used for adaptive estimates over time for nonstationary noises. Since the noise covariance matrix for SSA can be recursively updated by RL noise estimation, it results that the proposed algorithm can efficiently suppress the noise while reducing speech distortion. Objective experimental measurements indicated that the proposed algorithm was preferred over other SSA methods when tested with sentences corrupted by nonstationary noises.

Abstract:Low- and middle-income countries are significantly impacted by the global scarcity of medical imaging services. Medical imaging is an essential component for diagnosis and guided treatment, which is needed to meet the current challenges of increasing chronic diseases and preparedness for acute-care response. We present some key themes essential for improving global health equity, which were discussed at the 2023 RAD-AID Conference on International Radiology and Global Health. They include (1) capacity building, (2) artificial intelligence, (3) community-based patient navigation, (4) organizational design for multidisciplinary global health strategy, (5) implementation science, and (6) innovation. Although not exhaustive, these themes should be considered influential as we guide and expand global health radiology programs in low- and middle-income countries in the coming years.

Abstract:Imaging is central to the diagnosis, staging, treatment planning, and monitoring of prostate cancer (PCa). Unequal access to new imaging techniques may directly contribute to gaps in PCa treatment and outcome. Thus, identifying disparities in PCa diagnosis and treatment are centrla to informing strategies to promote equitable cancer care. This review examines the existing evidence regarding clinical and sociodemographic factors associated with disparities in imaging utilization and treatment for PCa. Major areas of disparities identified include healthcare and research access. Sociodemographic disparities are present in screening and diagnosis; Black patients are consistently less likely to receive both prostate multiparametric MRI and timely molecular imaging used to evaluate for biochemical recurrence. Regional variation in appropriate and inappropriate diagnostic imaging also contributes to corresponding differences in outcomes, especially between urban and rural settings. Delays in PCa imaging and diagnosis also delay definitive treatment or placement on active surveillance, with prominent differences by race and measures of social advantage Recognition of these disparities in PCa imaging and treatment can reinforce actions to improve equitable access to patients affected by PCa. Identifying modifiable steps in the PCa diagnosis, staging, and treatment workflow may inform interventions to bridge gaps in cancer outcome.

Abstract:Breast cancer survival has increased significantly over the last few decades due to more effective strategies for prevention and risk modification, advancements in imaging detection, screening, and multimodal treatment algorithms. However, many have observed disparities in benefits derived from such improvements across populations and demographic groups. This review summarizes published works that contextualize modern disparities in breast cancer prevention, diagnosis, and treatment and presents potential strategies for reducing disparities. We conducted searches for studies that directly investigated and/or reported disparities in breast cancer prevention, detection, or treatment. Demographic factors, social determinants of health, and inequitable healthcare delivery may impede the ability of individuals and communities to employ risk-mitigating behaviors and prevention strategies. The disparate access to quality screening and timely diagnosis experienced by various groups poses significant hurdles to optimal care and survival. Finally, barriers to access and inequitable healthcare delivery patterns reinforce inequitable application of standards of care. Cumulatively, these disparities underlie notable differences in the incidence, severity, and survival of breast cancers. Efforts toward mitigation will require collaborative approaches and partnerships between communities, governments, and healthcare organizations, which must be considered equal stakeholders in the fight for equity in breast cancer care and outcomes.

Abstract:When referring patients to radiology, it is important that the most appropriate test is chosen to avoid inappropriate imaging that may lead to delayed diagnosis, unnecessary radiation dose, worse patient outcome, and poor patient experience. The current radiology appropriateness guidance standard at our institution is via access to a standalone web-based clinical decision support tool (CDST). A point-of-care (POC) CDST that incorporates guidance directly into the physician workflow was implemented within a subset of head and neck cancer specialist referrers. The purpose of this audit was to evaluate the imaging pathway, pre- and post-implementation to assess changes in referral behavior.

Abstract:Introduction: This scoping review aims to highlight key social determinants of health associated with breast cancer screening behavior in United States women aged ≥40 years old, identify public and private databases with SDOH data at city, state, and national levels, and share lessons learned from United States based observational studies in addressing SDOH in underserved women influencing breast cancer screening behaviors. Methods: The Arksey and O'Malley York methodology was used as guidance for this review: (1) identifying research questions; (2) searching for relevant studies; (3) selecting studies relevant to the research questions; (4) charting the data; and (5) collating, summarizing, and reporting results. Results: The 72 included studies were published between 2013 and 2023. Among the various SDOH identified, those related to socioeconomic status ( n = 96) exhibited the highest frequency. The Health Care Access and Quality category was reported in the highest number of studies ( n = 44; 61%), showing its statistical significance in relation to access to mammography. Insurance status was the most reported sub-categorical factor of Health Care Access and Quality. Discussion: Results may inform future evidence-based interventions aiming to address the underlying factors contributing to low screening rates for breast cancer in the United States.

Abstract:Background: Worldwide there has been a shift in the ideology within healthcare systems that focuses on 'people' rather than the 'patients'. A patient should not only be perceived by their condition but rather viewed holistically as a person. Within a South African (SA) context, the constitution has been amended to state that all South African citizens have the right to healthcare that is caring, free from harm, and effective. Hence, it can be rationalized that patient-centered care (PCC) is a necessary and vital approach in South African healthcare as well. A study conducted by Hyde and Hardy in the United Kingdom (UK) explored measures of PCC from radiography patients, radiographers, radiography managers, radiography educators, and radiography students' perspectives. This study was duplicated to define informed measures of PCC from a South African perspective. Therefore, this study aimed to gain perspectives on PCC measures in diagnostic radiography within a sample of the South African community. This paper focuses on the perspectives of the clinical radiographer and patient respondents. Method: This study was conducted using a quantitative approach with the use of an online survey. The Qualtrics software was utilized to design the survey. The survey consisted of three PCC themes; use of technology, comfort and emotional support, and control over the environment. Recruitment of the subgroups was via advertisement in social media, email networks, and word of mouth. Results: There were 28 radiographer responses and 14 patient responses. The data were analyzed using SPSS software version 28. Patient care while explaining the use of technology in radiography, received positive responses from both respondent groups. Patient care measures that explored comfort and emotional support received varied responses from patients and radiographers. Important PCC concepts, such as asking the patient about their care needs and health problems, were lacking. Patients indicated that radiographers do not include their family members in the discussion of the care needs even though family involvement is a PCC element. Conclusion: The perceptions of PCC measures differed between the two groups which indicated that there are still some gaps in clinical practice. Patient involvement in their care and individualized care should be prioritized. There is a need for family involvement during radiography procedures to be better highlighted within clinical practice. It is also recommended that more importance be placed on PCC during undergraduate training. While service delivery is important, radiographers must not disregard their PCC responsibilities towards the patient.

Abstract:In recent years, there has been a growing emphasis on understanding the relationship between individual-level social determinants of health, which include social risks and social needs, within the realm of cancer care delivery. Social risks include food insecurity, housing instability, and transportation barriers, leading to adverse health outcomes. These social risks become social needs when patients identify a desire for assistance in addressing them. As the number of cancer survivors increases and disparities in the burden of cancer persist, there is a critical need to focus efforts on ensuring all survivors experience high-quality and equitable care across the cancer continuum, from diagnosis to end-of-life care. To inform research activities and intervention development, we aim to summarize and describe cancer care delivery research focused on three social risk factors: food insecurity, housing insecurity, and transportation barriers. To achieve our research objectives, we conducted a scoping review using Covidence software to examine published research assessing and addressing housing insecurity, food insecurity, and transportation barriers among individuals diagnosed with cancer. Six databases were searched: PubMed, Scopus, Cochrane Library, CINAHL Plus, EMBASE, and PsycINFO. Inclusion criteria for this review included original research in English published between 2010 and 2023 that focused on cancer patients and survivors in the United States and included social risk factors and cancer care delivery data. Data selection and screening were performed in two phases: (1) title and abstract and (2) full text, which required two independent reviewers. Discrepancies and disagreements between the two reviewers for inclusion of studies were resolved through weekly discussions at team meetings. A data abstraction tool that contains nine general variable categories was developed. We identified 10,032 studies, of which 76 were duplicates, and 9,368 were irrelevant at the title and abstract levels. We are currently finishing the full-text screening for the 563 studies deemed potentially eligible for inclusion at the title abstract level. To date, 164 of the 335 studies screened have met our eligibility criteria. We will extract data from all eligible studies once the full-text screening is completed. Across eligible studies, transportation barriers (91.5%) were the most common social risk factor of focus, followed by studies focusing on food insecurity (6.7%) and housing insecurity (3.7%). Our scoping review findings will provide us with a wide spectrum of knowledge regarding the most prominent relationships between social risk factors and cancer care delivery, gaps in current work, and existing interventions targeting these efforts. The results of this work will enhance our understanding of how social-structural disadvantages influence cancer outcomes and inform the development of concerted efforts aimed at addressing the social factors contributing to the unequal burden of cancer among survivors. Abstract:Introduction: Breast cancer is the leading cause of cancer-related deaths among women, with rural communities having the highest rates of breast cancer mortality. Despite this burden, many women begin mammography later than recommended and the mammography screening rate remains low, with a previously reported declining trend, particularly among rural women compared to urban women. This study seeks to identify barriers to breast cancer screening and examine strategies to overcome these barriers in rural Missouri. Methods: In partnership with the Missouri Cancer Consortium and the Missouri Comprehensive Cancer Control Programs, we conducted key informant interviews with mammography health providers at health centers in each of ten counties with the highest breast cancer mortality and lowest screening mammography rates in Missouri. Semi-structured interviews included questions about seeking and delivering breast cancer screening as well as provider experiences with patient navigation, mobile mammography, and the CDC's National Breast and Cervical Cancer Early Detection Program (NBCCEDP). Data were analyzed using a deductive-inductive thematic approach in NVivo 14. Results: We conducted 14 interviews with health providers across 12 health sites. The majority of these sites used patient navigation (67%), mobile mammography (80%), and the CDC NBCCEDP (67%) as strategies to improve access to breast healthcare. Health providers frequently mentioned "no-shows" as a major challenge to providing breast healthcare, along with limited transportation, cost, fear, and competing work priorities as the major barriers faced by patients in seeking breast healthcare. Patient navigation and mobile mammography were beneficial in bringing breast healthcare closer to the community, while the CDC NBCCEDP covered screening and follow-up costs. The main facilitating factors to implementing these strategies were having dedicated staff and funding for patient navigation, and existing partnerships with other health institutions for mobile mammography. However, health providers identified several implementing challenges for the strategies. This included limited staff capacity and difficulty contacting or following up patients to schedule appointments for patient navigation and mobile mammography. For the CDC NBCCEDP, the challenges included difficulty identifying eligible patients and extensive paperwork in the enrollment process. Providers recommended increasing staff capacity to support patient navigation, using digital technology in identifying and enrolling women for the CDC NBCCEDP, and integrating mobile mammography with the CDC NBCCEDP to improve implementation efficiency. Discussion: The integration of multiple component strategies within breast healthcare may address the multi-level barriers women face in accessing breast cancer screening. This study contributes to the identification of organizational supports and informs development of policies needed to enhance the adoption and implementation of strategies that support the effective delivery of breast healthcare. Abstract:1615 Background: Despite advances in personalized breast cancer risk assessment and screening, populations at highest risk for cancer-related mortality are often excluded. At the IU Simon Comprehensive Cancer Center, a high-risk screening program (HRSP) identifies and serves women at increased risk of breast cancer. Risk assessment using Tyrer Cuzick v.8 and Gail models is performed in mammography; women at increased risk are navigated to a clinic appointment to discuss individualized screening and prevention services including MRI, chemoprevention, and/or genetic testing. We hypothesize gaps in the demographic and socioeconomic (SES) characteristics of women seen for a HRSP appointment, compared to women receiving a cancer diagnosis at the same mammography sites. Methods: Retrospective review was performed of women who met criteria for the HRSP (age ≤65 and Gail 5 yr risk ≥3% or TC v.8 10-yr risk ≥8%, or recommended by the radiologist) and subsequently accepted, scheduled, and attended an appointment from 6/2020 to 6/2022 (n=233). This population was compared to women who were diagnosed with breast cancer at the same clinical sites over the same dates, without history of being evaluated in the HRSP. Age was restricted to ≤65 to match the HRSP population (n=685). Zip codes were used to calculate area-based SES by median household income and percent poverty. CDC social vulnerability index (SVI) was evaluated using the U.S. Census Bureau and American Community Survey (0=least vulnerable, 1=most vulnerable). Comparisons across groups by age, race, insurance status, area SES and SVI were made using the Wilcoxon or Chi-square test. Results: Cancer cases were significantly older than those attending the HRSP (median age 54 vs 49 p < 0.001). Black patients represented 19.4% of participants diagnosed with cancer, versus 10.8% of participants attending the HRSP (p=0.0032). There was also a disparity in insurance coverage, with 83.8% of women seen in the HRSP being privately insured, versus 75.6% of women diagnosed with cancer (p=0.011). There was no statistical difference in SVI (0.64 vs 0.66 p=0.11) or area poverty rate (11.2 vs 12.0 p=0.26) between the groups; cancer cases came from areas with lower household income (69,611 vs 73,165 p=0.04). Conclusions: Women seen in the HRSP were more likely to be younger, White, privately insured, and from an area with higher household income, compared to women diagnosed with cancer in the same health system at the same mammography sites. This highlights disparities in access to individualized breast cancer interception such as chemoprevention, high-risk screening, and genetic testing, which may further existing disparities in breast cancer-related mortality based on race and SES. Further studies will investigate barriers and motivators to personalized breast cancer prevention in high-risk groups, and utilize community partnerships to develop equitable interventions.

Abstract:Breast cancer is a significant public health concern, and addressing disparities in breast cancer screening is crucial for improving early detection and reducing mortality rates. This review article examines efforts to bridge the gap between awareness and action in reducing disparities in breast cancer screening. A systematic approach was employed to gather relevant literature using various databases. The selected studies encompassed a range of interventions, including policy changes, community-based programs, culturally competent interventions, technological advancements, and patient navigation. The review highlights the importance of policies and legislation in improving access to screening services and the impact of community-based initiatives in addressing disparities. Culturally competent interventions, tailored messaging, and language support were found to be effective in improving screening rates among diverse populations. Technological advancements, such as telemedicine and mobile health applications, were identified as promising approaches to increase access to screening services. Patient navigation programs effectively addressed barriers to screening and improved screening rates. The review also discusses evaluating efforts, limitations, and the need for continuous improvement. Future directions and recommendations include addressing gaps in the existing literature, proposing research directions, and providing recommendations for policymakers, healthcare providers, and researchers. By implementing these recommendations and working collaboratively, we can strive for equitable access to breast cancer screening for all populations, ultimately leading to improved outcomes and reduced disparities.

Abstract:PURPOSE: Despite well-established preventive screening guidelines for breast cancer, screening rates do not meet targets in both the United States and Canada. Although access to preventive care is an important factor toward participation, breast cancer screening rates in Canada vary despite a universal health care system. The objective of this study is to understand features within the Canadian population that potentiate screening disparities through a systematic review of the literature.METHODS: A search of MEDLINE and Embase was performed to identify relevant studies published from 2005 onward. Titles and abstracts were screened, followed by full-text screening. Inclusion criteria were defined as studies reporting on disparities in image-based screening for breast cancer.RESULTS: Three hundred twenty-four studies were retrieved, from which 29 studies were selected on the basis of the predetermined inclusion criteria. Population groups identified at risk for low image-based screening participation included those of low socioeconomic status, individuals with comorbidities, new immigrants and refugees, those in remote geographic locations, individuals with intellectual or developmental disabilities, and ethnocultural minorities. Barriers to image-based screening can be improved by targeting measures specific to these at-risk groups at the individual, organization, and policy levels.CONCLUSIONS: Multiple at-risk population groups exist for preventive cancer screening within a universal health care system. By understanding specific characteristics within these vulnerable populations, effective intervention strategies can be established to improve breast cancer preventive care.

Abstract:Objective To understand how breast and cervical cancer screening are influenced by communicating with a healthcare provider, patient activation, and social determinants of health. Method Data were from the National Cancer Institute's Health Information National Trends Survey, focusing on women with no history of cancer at least 21 years old ( N = 1,466) to study cervical cancer screening and those at least 40 years old ( N = 1,114) to study breast cancer screening. Variables included patient-centered communication, electronic healthcare communication, patient activation, race/ethnicity, education, health insurance status, Appalachian residence, and financial insecurity. Results Electronic communication predicted both cancer screens, but especially for White women. Patient-centered communication influenced cervical cancer screening only for women with insurance. It only influenced mammography for those without insurance. Patient activation did not influence either cancer screen behavior. Conclusion These data demonstrate more nuance is needed to parse potential effects of advocated-for healthcare behaviors. Use of publicly available datasets from can be informative but are limited methodologically. Practice Implications Healthcare providers and systems should promote use of patient portals and other electronic means of interaction outside regular clinical visits for all patients. However, attention needs to be paid to the unequal benefits they afford to patients.

Abstract:Rationale and objectives Underrepresentation of minorities is a worsening issue in the field of radiology. Early educational interventions are a promising approach to mitigating this disparity. We present an approach for a radiology department to increase community outreach via establishment of an educational program for local public high school students and building a mentorship pipeline for radiology education. Materials and Methods The department of radiology committee for Inclusion Diversity and Equity (IDE), in collaboration with the Office of Outreach, Education and Research (OER), invited yearly cohorts of 25 and 24 public high school students in 2022 and 2023, respectively, to an on-site educational event featuring rotating small group hands-on workshops in a multi-stage format. The event inspired students to consider various careers in radiology and their corresponding academic pathways after high school. Post-workshop surveys featuring Likert scale and open-ended questions were administered to collect student reflections and feedback. Analysis was conducted to assess student understanding, interest in radiological careers, and opportunities for future event improvements. Longitudinal mentorship was established between students and point-persons to provide continued career guidance. Results For two consecutive cohort years, the program received high scores on clarity of presentations and increased student awareness of opportunities within radiology. Standout positive elements included interactive sessions, hands-on activities, and the discovery of radiology as a collaborative field. Of the initial student group, one student went on to enroll in a radiography program. To date, five participants have returned for shadowing experiences, three of whom are currently enrolled in science undergraduate programs, including one pre-medical student. Conclusion We present an accessible and effective approach for a radiology department to collaboratively increase community outreach and improve minority representation through early educational programming and establishing a longitudinal pipeline mentorship program for public high school students.

Abstract:Background Diversity, equity, and inclusion (DEI) are crucial for building a just and equitable healthcare system. However, academic radiology continues to face significant underrepresentation of women and underrepresented minorities (URMs). Purpose This study examines demographic disparities in academic radiology and presents strategies for improving DEI. Materials and Methods I analyzed demographic data from various sources and reviewed successful DEI initiatives. Results My analysis revealed a significant underrepresentation of URMs and women at all stages of the radiology training pipeline, from medical school to faculty positions. I identified several challenges contributing to this underrepresentation, including implicit bias, financial barriers, and lack of mentorship. However, I also presented successful case studies of initiatives, such as the Radiology Leadership Institute and the Association of University Radiologists Mentorship Program, that are making progress in diversifying the field. Conclusions Achieving DEI in academic radiology requires a multi-pronged approach that includes early outreach, financial aid, mentorship, inclusive recruitment practices, and a focus on creating a welcoming environment. By continuously evaluating and adapting initiatives, the radiology community can create a more equitable and inclusive future for all individuals.

Abstract:There remains a need to synthesize linkages between social determinants of health (SDOH) and cancer screening to reduce persistent inequities contributing to the US cancer burden. The authors conducted a systematic review of US‐based breast, cervical, colorectal, and lung cancer screening intervention studies to summarize how SDOH have been considered in interventions and relationships between SDOH and screening. Five databases were searched for peer‐reviewed research articles published in English between 2010 and 2021. The Covidence software platform was used to screen articles and extract data using a standardized template. Data items included study and intervention characteristics, SDOH intervention components and measures, and screening outcomes. The findings were summarized using descriptive statistics and narratives. The review included 144 studies among diverse population groups. SDOH interventions increased screening rates overall by a median of 8.4 percentage points (interquartile interval, 1.8–18.8 percentage points). The objective of most interventions was to increase community demand (90.3%) and access (84.0%) to screening. SDOH interventions related to health care access and quality were most prevalent (227 unique intervention components). Other SDOH, including educational, social/community, environmental, and economic factors, were less common (90, 52, 21, and zero intervention components, respectively). Studies that included analyses of health policy, access to care, and lower costs yielded the largest proportions of favorable associations with screening outcomes. SDOH were predominantly measured at the individual level. This review describes how SDOH have been considered in the design and evaluation of cancer screening interventions and effect sizes for SDOH interventions. Findings may guide future intervention and implementation research aiming to reduce US screening inequities.
