Abstract:Complex functional issues arising from temporomandibular joint (TMJ) ankylosis are associated with the abnormal mandibular growth secondary to condylar-glenoid fossa and its surrounding structures. These include severe limited mouth opening, micrognathia, mandibular asymmetry and obstructive sleep apnoea (OSA) which necessitate effective treatment to allow optimum functional rehabilitation. This article aims to present a comprehensive systematic review on the surgical strategy for patients presented with triad of TMJ ankylosis, micrognathia and OSA via a systematic literature search via PubMed, Google Scholar and Scopus using PRISMA guidelines. The outcomes of interest were difference in maximum mouth opening, incidence of reankylosis, amount of mandibular advancement, posterior airway space, pre- and post-operative apnoea/hypopnea index and arterial oxygen saturation as well as changes in other cephalometric or polysomnographic variables. Thirty four studies involving 360 patients were included in this review. Surgical intervention reported includes distraction osteogenesis (DO), TMJ ankylosis release and mandibular advancement, simultaneous arthroplasty and DO, pre-athroplasty DO, as well as post-arthroplasty DO. Most studies reported functional post-intervention mouth opening, with re-ankylosis reported in 4 studies. Mandibular advancement achieved was between 6mm to 34 mm. All studies reported improvement in various polysomnographic variables measured. In conclusion, the systematic review was conducted based on low level of literature evidence. Even though various surgical strategies in TMJ ankylosis have been reported, effective case-specific management of TMJ ankylosis with micrognathia and OSA requires comprehensive assessment and careful consideration of surgical options that promote mandibular advancement and airway improvement.

Abstract:Temporomandibular joint (TMJ) ankylosis is generally characterised by a complex aetiology, with several contributing causes, including infections, autoimmune diseases, trauma, and congenital anomalies. This case report describes a three-year-old female suffering from traumatic temporomandibular ankylosis with retrognathia, severe mouth-opening restriction, and obstructive sleep apnea (OSA). The present case highlights the difficulties with TMJ ankylosis, especially when access to healthcare is sought out late and delayed diagnosis is prevalent. Mandibular distraction osteogenesis and awake fiberoptic intubation were used in the surgical and anaesthetic management of this case, with the otorhinolaryngology team on standby to perform a tracheostomy if required, highlighting the necessity of a multidisciplinary approach in such cases. Patients with TMJ ankylosis have significant life-altering changes, including psychological stress, chewing difficulty, speech difficulties, facial distortion, and speech impediment. When OSA progresses, it also presents more health risks. For the purpose of treating TMJ ankylosis, avoiding serious problems, and enhancing patient well-being, prompt diagnosis and therapy are crucial. In order to optimise patient results, this case study highlights the need for knowledge and research in the treatment of TMJ ankylosis as well as the requirement of medical professionals working together in a synergistic way.

Abstract:Temporomandibular joint (TMJ) ankylosis is a form of TMJ condition that causes mouth opening limitation, ranging from partial reduction to total immobilization of the jaw. Bony and fibrous ankylosis is most commonly caused by trauma, although it can also happen as a result of surgery, local or systemic infections, or systemic diseases. Childhood TMJ produces facial deformities, which increase with growth and have a major detrimental impact on the patient's psychological development. Each patient with TMJ ankylosis must have a history, physical examination, and radiographic examination in order to determine a definitive diagnosis, severity, involvement of surrounding tissues, and, ultimately, treatment planning. Technical challenges and a high recurrence rate make treating TMJ ankylosis challenging. Intubating a young child with TMJ ankylosis is a difficult job, which is exacerbated by limited mouth opening. This case report describes a five-year-old boy who reported an inability to open his mouth, diagnosed as TMJ ankylosis, and managed in the absence of an appropriately sized tracheostomy tube.

Abstract:Temporo-mandibular joint (TMJ) ankylosis is a common cause of acquired mandibular deformity in children and adults. It causes reduced mouth opening and limitation of functional movements resulting in mandibular growth impairment leading to mandibular retrognathism and facial asymmetry. The treatment of TMJ ankylosis is challenging, not only due to the complexities involved and the risk of relapse but also because it requires a high degree of patient cooperation. The treatment may be performed in 1 or 2 phases consisting of the initial release of ankylosis with or without condylar reconstruction, followed by a correction of mandibular hypoplasia and of facial asymmetry by orthognathic surgery. Distraction osteogenesis has been proposed to treat cases with severe deformity due to its inherent advantages of generating new bone and soft tissue. This case report describes the staged treatment of a patient with unilateral TMJ ankylosis. The patient presented with significant facial deformity due to mandibular retrognathism and facial asymmetry as a consequence of impaired growth. The treatment objectives included releasing ankylosis to establish mouth opening, addressing the dentofacial deformity and achieving a normal occlusion. The patient was treated with a combined surgical-orthodontic approach including distraction osteogenesis. The case was treated with a rigid external distractor and CBCT generated facial models were used to plan and execute adjunctive surgeries. The staged treatment approach resulted in a significant improvement of facial aesthetics.

Abstract:Obstructive sleep apnea (OSA) is a respiratory disorder that has a high prevalence in patients with craniofacial, neurocognitive, and neuromuscular disorders. Currently, the treatments for this population are diverse and depend on the individual conditions of the patient and the severity of the case. However, there are no multidisciplinary dental treatment guidelines. The aim of the present study was to determine the multidisciplinary dental treatment alternatives in patients with craniofacial, neurocognitive, and neuromuscular disorders with a diagnosis of OSA through evidence-based medicine. A systematic review of the literature has been performed by searching scientific articles in the PubMed, Cochrane, Ovid, ScienceDirect and Scopus databases, through controlled and uncontrolled language. Articles were classified according to the level of evidence and grades of recommendation through the Scottish Intercollegiate Guidelines Network. A total of 19,439 references were identified, of which 15 articles met the predetermined requirements to be included in the investigation. The articles included for this systematic review showed that mandibular distraction osteogenesis and adenotonsilectomy are the first-choice therapies for craniofacial and neurocognitive disorders. However, for neuromuscular disorders, the findings reported were not enough to provide information about surgical or nonsurgical alternatives. Despite the reported high frequency of OSA in those children with craniofacial, neurocognitive, and neuromuscular disorders, the evidence on the surgical and nonsurgical therapeutic success for OSA in these patients is scarce. Abstract:Introduction: Sleep-disordered breathing could affect 10% of an orthodontic population. The integration of obstructive sleep apnea syndrome (OSAS) diagnosis could influence the choice of orthodontic techniques or their implementation, with the aim of improving ventilatory function. Material and method: The author summarizes the clinical studies using dentofacial orthopedics, alone or in combination with other interventions, in pediatric OSAS or the repercussions of orthodontic interventions on upper airways. Results: For the same orthodontic anomaly, in particular, transverse maxillary deficiency, the temporality and the modality of treatment could be modified by a diagnosis of OSAS. It could be recommended to propose early orthopedic maxillary expansion, seeking to potentiate its skeletal effect, to reduce the severity of OSAS. Class II orthopedic devices have shown interesting results but the evidence value of the studies is not yet sufficient to recommend them widely and as an early treatment. Extractions of permanent teeth do not significantly reduce the upper airway. Discussion: OSAS in children and adolescents includes several endotypes and phenotypes for which orthodontics may or may not be indicated. It is not recommended to orthodontically treat an apneic patient with no significant malocclusion, for the sole purpose of having an effect on the respiratory tract. Conclusion: The orthodontic therapeutic decision is likely to be modified by a diagnosis of sleep-disordered breathing underlining the interest in systematic screening.

Abstract:Obstructive sleep apnea syndrome (OSAS) is a frequent, albeit underdiagnosed problem in children. If left untreated, OSAS may lead to substantial morbidities affecting multiple target organs and systems. The immediate consequences of OSAS in children include behavioral disturbance and learning deficits, pulmonary hypertension, as well as compromised somatic growth. However, if not treated promptly and early in the course of the disease, OSAS may also impose long-term adverse effects on neurocognitive and cardiovascular function, thereby providing a strong rationale for effective treatment of this condition. This review provides a detailed description of the current treatment modalities for pediatric OSAS, and uncovers the potential limitations of the available data on these issues. Furthermore, we postulate that OSAS will persist relatively often after tonsillectomy and adenoidectomy, and that critical studies need to be conducted to identify such patients and refine the clinical management algorithm for pediatric OSAS.

Abstract:Failing to address Obstructive Sleep Apnea (OSA) in Down Syndrome (DS) patients can have serious consequences, leading to increased mortality. Moreover, a notable portion of individuals find it challenging to tolerate Continuous Positive Airway Pressure (CPAP). Therefore, this study aims to share our experiences in treating adult DS patients with moderate to severe OSA who don't tolerate CPAP using various surgical approaches. A retrospective analytical study including 20 DS with moderate to severe/very severe OSA who had no tolerance to Continuous Positive Airway Pressure (CPAP) was conducted. Regarding the individual skeletal characteristics of each patient various orthognathic surgery techniques were performed. Two in-hospital polysomnographies (PSG) were undertaken for every patient; one before the surgical procedure, while another was performed between 6 and 60 months of the follow-up post-surgery period. Polysomnographic variables were analyzed in this study such as AHI, ODI, and T90, which all showed statistically significant improvement after surgery with a p value in the Wilcoxon test <0,01. These results have remained stable over time, with no recurrences of OSA observed over the five years of follow-up. Thus, orthognathic surgery may be the only viable option for individuals with DS who are unable to tolerate CPAP.
Maxillary Expansion and Mandibular Setback Surgery With and Without Mandibular Anterior Segment Osteotomy to Correct Mandibular Prognathism With Obstructive Sleep Apnea

Abstract:Mandibular prognathism is usually treated with mandibular setback surgery. However, this approach reduces the pharyngeal airway space, and can aggravate obstructive phenomena in patients with obstructive sleep apnea (OSA). While maxillary expansion is known to lead to an increase in the pharyngeal airway volume (PAS), its effect on the PAS in mandibular setback surgery has not yet been reported. The authors report a surgical approach with maxillary expansion in 2 patients with mandibular prognathism that was accompanied by OSA: maxillary midsagittal expansion with minimum maxillary advancement and minor mandibular setback without mandibular anterior segmental osteotomy (ASO) or major mandibular setback with mandibular ASO. Preoperative and postoperative computed tomography and polysomnography indicated that OSA was improved and pharyngeal airway space was increased or sustained, and the prognathic profile could be corrected to an acceptable facial esthetic profile. In summary, maxillary transversal expansion and mandibular setback with or without mandibular ASO can be successfully applied to treat mandibular prognathism with OSA.

Abstract:Abstract Introduction Treatment of persistent obstructive sleep apnea (OSA) in medically complex pediatric patients can be challenging due to concurrent developmental delays contributing to poor adherence with positive airway pressure (PAP) therapy. There is limited literature describing a multidisciplinary approach to treating persistent OSA in medically complex pediatric patients utilizing a pulmonologist, otolaryngologist, sleep medicine provider, psychologist, nutritionist, and nurse coordinator. Objectives: 1: Describe the multidisciplinary approach to treating persistent OSA in children at our institution; 2: Review concurrent patient diagnoses and interventions recommended; 3: Report resulting decreased patient costs and increased access to care. Methods We developed a multidisciplinary OSA (MOSA) clinic for patients with recalcitrant OSA in November 2022 in a pediatric tertiary care setting. Patients with persistent OSA after adenotonsillectomy who failed PAP therapy were seen for treatment optimization. This included consideration for drug induced sleep endoscopy (DISE) and/or Hypoglossal Nerve Stimulator (HGNS), as well as an evaluation by sleep psychology, pulmonary medicine, and a sleep provider to improve PAP adherence. Data was collected and reviewed via retrospective chart review. Results Twelve clinic sessions have been completed and 46 pediatric patients have been evaluated (n=46) with an average age of 10.4 years (±5 years). 58% were referred for nonadherence to PAP therapy. The most prevalent concurrent diagnosis was developmental delay (n=36, 78%). Other diagnoses included Trisomy 21 (n=18, 46%), other genetic disorders (n=8, 17%), and autism (n=6, 13%). 61% (n=46) had >95th%ile BMI-for-age resulting in nutrition consultation. Surgical intervention was recommended for 41% (n=19) while 43% (n=20) were scheduled for ongoing desensitization to PAP therapy. Combining specialties reduced average patient travel by 220 miles and gas costs of $35.44. Wait times to see the otolaryngologist decreased from 9 months to 3 months. Due to high demand, a second clinic location will launch in February 2024. Conclusion Our MOSA clinic has provided significant value in treating complex pediatric patients with persistent OSA. This review identifies patients with comorbidities that may be good candidates for a multidisciplinary sleep clinic, the potential improvement in access to care, and cost savings for patients and families requiring complex care related to persistent OSA. Facial Deformity in an Operated Case of Bilateral Temporomandibular Joint Ankylosis

Abstract:Temporomandibular joint (TMJ) ankylosis results in malocclusion, poor feeding, difficulty in maintaining oral hygiene, and facial esthetic deformity. The basic surgical objectives in the treatment of TMJ ankylosis are to establish joint movement, prevent relapse, and achieve normal growth and development. Here, we present an operated case ofsurgical correction of mandibular hypoplasia; however, the patient came back after three years due to unsatisfactory results and underwent bilateral coronoidectomy and gap arthroplasty. Bones were osteotomized at the LeFort I level and the maxillary segment was down-fractured and mobilized to bring into occlusion with the mandible. In the present case, the lower pharyngeal airway changed from 5 mm pre-treatment to 10 mm post-treatment, and the facial angle was changed from 73 to 84 post-treatment. Assessment of the pharyngeal airway is done with a high suspicion of obstructive sleep apnea and facial deformity is mandatory in the management of TMJ ankylosis.

Abstract:<span>To suggest an updated interdisciplinary treatment protocol for pediatric obstructive sleep apnea (POSA) based on the integration of craniofacial growth modification into medical and surgical sleep practice.PubMed, Scopus, and Cochrane library were searched up to February 2020 using keywords. Among 184 articles, 80 studies were finally included. An integrated treatment protocol for POSA encompassing craniofacial skeletal management as well as medical and surgical care was attempted.A differential diagnostic workflow for identifying the phenotype of POSA was suggested, and a phenotype-based treatment protocol for POSA was proposed. Despite the lack of high level of evidence, timely skeletal growth modification in three dimensions using craniofacial growth potential could be valuable treatment for upper airway development in POSA patients with craniofacial phenotypic cause.A novel precision treatment protocol will advance clinicians to determine the primary option or to apply the combined strategy for POSA patients.</span>

Abstract:Temporomandibular joint (TMJ) ankylosis is a debilitating state that involves fusion of glenoid fossa and condyle of mandible. This can lead to restriction in mouth opening and mandibular movements. Subsequently, patient develops difficulty in chewing, speech, breathing as well as poor dental hygiene. All of these have detrimental effects on patient’s wellbeing and their quality of life. TMJ ankylosis can also have secondary effects on facial growth like asymmetry of face, retro or micrognathia, malocclusion, anterior or posterior open bite and overcrowding of teeth specially if it presents at early age. Main event that leads to TMJ ankylosis is trauma but this can occur as a secondary insult to surgery, local and systemic infections or diseases like chronic otitis media, tuberculosis, juvenile rheumatoid arthritis and septic arthritis. The management of TMJ ankylosis poses a significant challenge because of technical difficulties and a high incidence of recurrence. It is mainly managed by surgery with different options like gap and interpositional arthroplasty, reconstruction arthroplasty or distraction osteogenesis and aggressive post op physiotherapy. Here, we present a case five years old girl child with post traumatic TMJ ankylosis who was managed with excision of ankylotic mass and interpositional arthroplasty by using bilateral temporo-parietal fascial flap. She was followed for a 1 year and had adequate mouth opening and no recurrence.
Distraction osteogenesis in correction of micrognathia accompanying obstructive sleep apnea syndrome.

Abstract:To evaluate the effect of distraction osteogenesis in correction of micrognathia accompanying obstructive sleep apnea syndrome, a total of 28 patients with different severities of obstructive sleep apnea syndrome underwent mandibular distraction osteogenesis. A total of 51 distraction devices were placed for bilateral distraction in 23 patients and for unilateral distraction in five patients. The mean age of patients was 21.2 years (range, 3 to 60 years). Eleven patients had micrognathia accompanying obstructive sleep apnea syndrome secondary to bilateral temporomandibular joint ankylosis, and 10 patients had micrognathia accompanying obstructive sleep apnea syndrome secondary to unilateral temporomandibular joint ankylosis. Three patients had developmental micrognathia accompanying obstructive sleep apnea syndrome. The other four patients had micrognathia and concomitant obstructive sleep apnea syndrome induced by trauma, infection, or tumor resection. Each patient had been evaluated preoperatively and postoperatively with cephalometry and polysomnography. Mandible advancement ranged from 9 to 30 mm (average, 20.4 mm) and was successfully achieved after distraction. Fine new])one formed in the distraction gap when the distraction devices were removed 3 to 4 months after distraction was completed. No infection or other complications occurred in any patients. Complete curative effects were achieved in nine severe, six moderate, and eight mild obstructive sleep apnea syndrome patients after distraction, and the other five patients had been improved to the mild level. After distraction was completed, the posterior airway space was increased on average from 4.6 mm to 12.5 mm and the sella-nasion-point B angle was increased on average from 66 degrees to 75 degrees on cephalometric studies. The polysomnographic examination showed that the apnea hypopnea index was lowered on average from 58.0 to 3.15, and the lowest oxygen saturation was increased on average from 77 percent to 90.3 percent after distraction was completed. The follow-up period was 3 to 61 months (average, 18.1 months). The curative effect was stable and no relapse occurred. Therefore, the authors conclude that mandibular distraction osteogenesis is an effective method for correcting micrognathia accompanying obstructive sleep apnea syndrome. Compared with other current routine surgical procedures, it has many advantages, such as low risk, simple manipulation, high curative rate, low relapse rate, and stable result. It is presently the most effective method for the treatment of this difficult and complicated disorder.
Treatment of OSA beyond adenotonsillectomy

Abstract:Pediatric obstructive sleep apnea (OSA) is a common syndrome in children. While an adenotonsillectomy is effective for most, it may not be the treatment of choice for all, and possible alternative treatment options are briefly discussed herein. In obese children, in whom OSA is more prevalent, and has high rates of persistence after surgery, weight loss interventions may be recommended. Anti-inflammatory therapies, such as nasal steroids and montelukast, reduce the size of the adenoids and tonsils and are considered part of the stepwise treatment approach. Functional orthodontic therapies may improve OSA as well as orthopedic interventions in children in whom orthodontic indications exist. Positive airway pressure and high flow nasal cannula, stent the airway open and alleviate the obstruction, but adherence may be challenging. Tongue surgery, as well as several additional interventions are further discussed. In conclusion, multiple therapeutic options are available for pediatric OSA. Further studies are required to better identify the children most likely to benefit from these therapies.
Failure of NIMV and operator factor: when making things work, sometimes it is hard

Abstract:Despite its well-known advantages in acute respiratory failure (reduces intubation rate and mortality), non invasive mechanical ventilation (NIMV) have failure rates of 5%–60%, and mortality rates have been found to be higher in patients who are intubated after failure of NIV (1). Predictors for NIMV failure were defined to avoid delay in intubation which can cause clinical deterioration and increase morbidity and mortality (2). We read with great interest the article written by Bhatti et al. (3) entitled ‘Operator dependent factors implicated in failure of non-invasive positive pressure ventilation for respiratory failure’ accepted to Clinical Respiratory Journal. The study evaluated retrospectively 111 patients with NIMV failure and investigated operator related factors that may have contributed to failure of NIMV and clinical outcomes. They found three most common reasons for failure as inappropriate indication (33%), progression of underlying disease (30%) and lack of titration (23%). This study definitely requires attention, since it gives valuable information to clinicians to not only prevent failure but also provide protocoled approach to initiation and maintenance of NIMV. However, we think that there are some issues to be commented on. First, the study includes chart review of patients who were initiated and failed NIMV at different places like emergency room, on medical/surgical floors, progressive care units or intensive care units. This means that different operators with different devices and different level of education and training on NIMV. Schettino et al. evaluated 441 patients with NIMV treatment in their prospective one center study. They found entubation and mortality rate in patients only treated in the emergency room as 22.6%, 7.5%, in the intensive care unit as 49.4%, 28.4% and in the ward as 27.3%, 14.9%, respectively. The authors related these diversions with different type and severity of comorbidities. We think that these situations are also valid for the study of Bhatti et al. it is crucial to report failure rates of these different places defined in the study. Also it is necessary to give information about distribution of defined operator based factors among these places. We believe that there may be differences other than operator based factors like disease severity, type of NIMV devices, equipment, and education level of medical stuff and nurse/patient ratio in these different areas. Second, although it was noted in limitations because of the retrospective nature of the study, it does not include variables such as leak; amount and management of secretions that have been proved to be influential in some studies that have evaluated the success or failure of NIMV (4). In addition data of type of respiratory failure whether hypoxic or hypercapnic was not given. It was reported that NIMV success rate of hypoxemic respiratory failure as 50% while in hypercapnic respiratory failure it is %70 (5). We believe that it would be valuable information to give ratio of operator based factors of NIMV failure among hypoxic and hypercapnic respiratory failure. This study gives valuable and useful information about operator based factor for NIMV failure for patients with respiratory failure. Due to multiple factors that affect NIMV success, having heterogeneous places and patients make study hard to give accurate information about only operator based predictors. We consider large randomized clinical based prospective studies for operator based factors of NIMV failure.
