London SE228DF Dear S1r, Professor Baum's discussion paper (July Journal, p 504) has important ethical and economic implications for clinical diagnosis and management far beyond the problems posed by his excellent consideration of clinical trials in breast cancer. • 'Conceptual rationalism' creates the need for facilities for many 'long-stay' patients by morbidly affecting the health care of old people. This is simply because those not so expert in their management still use, in wards and committees, the disparaging diagnostic labels of 'just old age', 'senility', 'social problem' or more generally 'geriatric', while they as blatantly and continually deny adequate expert facilities for the elderly within the district general hospital. Moreover, all of this occurs despite the knowledge gained from, and the evidence produced in favour of, active health care programmes for the elderly by generations of geriatricians. The creation of a 'long-stay', 'bed-blocking' role for many elderly and aged patients is becoming nay, has becomea national scandal and is the direct result of inadequate facilities, indeed, often none at all, for the clinical care of the elderly and teaching in far too many district general hospitals (particularly Teaching Health Districts). As Professor Baum has pointed out, the end result of 'conceptual rationalism' is a self-fulfilling prophecy which in this case means that 'long-stay' for the elderly can all too easily and inevitably lead to the stage when nothing can be done about it. The next phase of this unnecessary situation is when, as now, these 'long-stay' problems are promptly proffered by non-geriatricians as bones of contention for the graveyard of a non-existing clinical undertaker after their geriatrician colleagues' potential to help has been frustrated. This frustration is the direct result of a continuing 'no go' policy on effective decision-making that is again particularly evident in the Teaching Health Districts, where an obviously weighted and inadequate committee structure has a vested interest in high-tech,nology medicine. Professor Baum's paper is a timely stimulus to our consideration of what scientific empiricism has produced in clinical medicine today. The zenith of high-technological medical practice appears to be merely the ability to transplant some livers successfully into chronic alcoholics and some hearts into those who have smoked too much; neither of these achievements is, in my opinion, ethical nor economic. Meanwhile, the inadequacy of consultant 'conceptual rationalism' about the elderly not only denies more effective care to a large and increasing number of elderly patients, but also denies more effective training to those who inevitably will have t~ deal with still more clinical problems 'produced by the elderly tomorrow, next month, next year, and into the next decade. The essential reorganization now required within the National Health Service must link the real health care requirements of the community to those who are prepared to tackle the job properly; this would make both ethical and economic sense. Furthermore, it would be encouraged if clinical 'conceptual rationalism' was buried by the scientific spade of medical audit. Yours faithfully

Abstract:A simple technique for obtaining high‐quality echocardiograms of the left ventricle during upright bicycle exercise is described. The use of this method has resulted in left ventricular dimension data that are consistent with the results of studies in conscious, previously instrumented dogs during running. Careful subject selection is important, however, as only 1 of 5 normal subjects was found suitable for these studies. Therefore, we believe that upright exercise echocardiography is feasible in selected subjects and provides useful physiologic information about left ventricular performance during exercise.
Gallstone Ileus as an Infrequent Cause of Bowel Obstruction: A Review of Small Cohort

Abstract:Introduction Gallstone ileus is an uncommon cause of small bowel obstruction; it is a rare complication of calculus chronic cholecystitis which leads to cholecystoenteric fistula and impaction of gallstone in the gastrointestinal tract leading to mechanical bowel obstruction. Our aim is to report the natural history and management of this rare condition in a teaching hospital. Materials and methods It is a retrospective study, where 10 years of data related to the management of intestinal obstruction secondary to gallstone ileus was collected. The cohort included 10 patients, whose demographic data, clinical findings, and management outcomes were evaluated. Results Majority of patients were female (90%, n=9) with a median of 83 years (range 61-96) although 90% of the population were above 70 years. Presenting complaints were mostly pain and vomiting. The onset of symptoms was between two and seven days. The site of obstruction was mostly the ileum (n=9) with the exception of one case in the sigmoid proximal to a benign stricture, and the size of the stone ranged from 2.5 to 4 cm. Moreover, most of the patients had a previous history of gallstone (n=7) with one post-cholecystectomy status. The laboratory investigations in 50% of patients had deranged liver function test (LFT) and acute kidney injury (AKI), and 60% had raised inflammatory markers, namely, white blood cells (WBC) and C-reactive protein (CRP). Intervention as enterolithotomy was the preferred approach (n=8 (two laparoscopic, six open surgery)), and two patients were managed conservatively. The mean postoperative length of stay was 10 days in the open approach and five days in the laparoscopic approach, respectively. Conclusions Elderly female patients are more prone to have gallstone ileus particularly with a past medical history of gallstones, and the preferred management option is enterolithotomy which could be open or laparoscopic depending on the expertise of the surgeon.

Abstract:Gallstone ileus is a rare disease and accounts for 1%-4% of all cases of mechanical intestinal obstruction. It usually occurs in the elderly with a female predominance and may result in a high mortality rate. Its diagnosis is difficult and early diagnosis could reduce the mortality. Surgery remains the mainstay of treatment. We report two cases of gallstone ileus. The first was a 78-year old woman who had a 2-d history of vomiting and epigastralgia. Plain abdominal film suggested small bowel obstruction clinically attributed to adhesions. Later on, gallstone ileus was diagnosed by abdominal computed tomography (CT) based on the presence of pneumobilia, bowel obstruction, and an ectopic stone within the jejunum. She underwent emergent laparotomy with a one-stage procedure of enterolithotomy, cholecystectomy and fistula repair. The second case was a 76-year old man with a 1-wk history of epigastralgia. Plain abdominal film showed two round calcified stones in the right upper quadrant. Fistulography confirmed the presence of a cholecystoduodenal fistula and gallstone ileus was also diagnosed by abdominal CT. We attempted to remove the stones endoscopically, but failed leading to an emergent laparotomy and the same one-stage procedure as for the first case. The postoperative courses of the two cases were uneventful. Inspired by these 2 cases we reviewed the literature on the cause, diagnosis and treatment of gallstone ileus.

Abstract:Introduction and importance: Biliary ileus is a rare yet significant cause of mechanical intestinal obstruction, which occurs when a gallstone enters the gastrointestinal tract through a bilioenteric fistula, leading to intestinal blockage. This condition primarily affects elderly patients and is associated with high morbidity and mortality if not diagnosed and treated promptly. Case presentation: We present the case of a 94-year-old female with a history of hypertension and chronic venous insufficiency. The patient was admitted with severe abdominal pain, nausea, and vomiting, with clinical findings suggestive of intestinal obstruction. Computed tomography revealed Rigler's triad, confirming the diagnosis of biliary ileus. An exploratory laparotomy was performed, identifying three gallstones in the small intestine. The patient underwent enterotomy for stone extraction and had a favorable immediate postoperative outcome. Clinical discussion: Biliary ileus presents a diagnostic challenge due to its nonspecific symptoms. While Rigler's triad (pneumobilia, intestinal obstruction, and ectopic gallstone) is diagnostic, it is not always apparent in imaging. Surgical intervention remains the standard of care for resolving the obstruction, though appropriate preoperative management and timely surgery are crucial for improving outcomes. Conclusion: This case emphasizes the importance of considering biliary ileus in the differential diagnosis of intestinal obstruction, particularly in elderly patients. Early surgical intervention is essential to prevent severe complications. Evidence based medicine ranking: Level IV.

Abstract:Gallstone ileus is a rare condition constituting 1%-4% of cases of small bowel obstruction (SBO). It primarily affects women with a history of gallstones. Mechanical bowel obstruction is a feared complication. Patients typically present with abdominal pain, jaundice, nausea, and vomiting. Diagnosis is often confirmed by a computed tomography (CT) abdomen, although serial abdominal X-rays may also be useful. We present a case of a patient presenting with SBO and hyperbilirubinemia, without evident biliary pathology on imaging but with a surgical diagnosis of SBO secondary to a gallstone. This case highlights the diagnostic and treatment challenges in such patients.

Abstract:The analysis of sleep quality and its impact on diurnal sleepiness is a crucial aspect of human health. Modern life in large cities compels people to spend less hours sleeping than they need. A wearable actigraphy that records accelerometry, light and body temperature was used to register the sleep habits of 54 healthy subjects for 14 days. From them, 28 participants resided in a town that could be regarded as a rural environment while the other subjects lived in the largest metropole of South America. The subjects filled three questionnaires to assess sleep quality (Pittsburgh Sleep Quality Index — PSQI), sleepiness (Epworth Sleepiness Scale — ESS) and chronotype (Morningness-Eveningness Questionnaire — MEQ-HO). The subjects were divided in urban and rural groups and several parameters obtained from actigraphy were compared between groups. Overall, the metropolitan group had worse sleep parameters. The total sleep time in metropolitan group was lower than the rural population, as well as the sleep quality parameters such as sleep efficiency. A possible explanation for these differences could be the social and work characteristics imposed in the metropolitan environment, forcing subjects to stay awake longer and away of natural light exposure. Sleep disorders are associated with several health and brain function problems. Therefore, it is imperative to establish changes in daily routine to promote a better sleep quality and prevent sleep disorders.

Abstract:The use of the carbon dioxide laser to perform surgical procedures in the oral cavity has been described the last few years. This study involved the clinical audit of 83 patients who had undergone laser surgery in the oral cavity during 1985 to 1990 in Birmingham, England. The diagnosis of the 83 lesions treated were benign, premalignant or malignant. The most common sites of the lesions were the floor of the mouth/ventral surface of the tongue and other parts of the tongue, followed by upper alveolus and/or palate, angle of the mouth and/or cheeks, lower alveolus, and lips. The results showed that the use of the carbon dioxide laser in treating oral soft-tissue pathology presented advantages over conventional techniques. Although trismus, numbness and early signs of infection were observed after surgery, local discomfort and pain were the most common complaints after laser surgery. The carbon dioxide laser does not offer any enhanced cure-rate for oral pathology, but rather it is a precise means of removing soft tissue lesions in selected patients with little upset afterwards.

Abstract:Gallstone ileus is an uncommon but potentially life-threatening complication of gallstone disease, characterized by the obstruction of the gastrointestinal tract by a gallstone, typically at the ileocecal valve. This condition predominantly affects elderly patients and carries a high risk of morbidity and mortality due to delayed diagnosis and the complexity of associated comorbidities. We report the case of a 60-year-old woman with a history of hypertension and cholelithiasis who presented with a four-day history of intermittent epigastric pain, nausea, vomiting, and an inability to pass stool or flatus. Initial imaging studies, including ultrasonography and computed tomography, revealed a biliary-enteric fistula with a large obstructing gallstone at the ileocecal valve. Despite conservative management with intravenous fluids, nasogastric tube suction, and antibiotics, the patient's symptoms persisted, necessitating surgical intervention. A midline laparotomy was performed, during which the gallstone was successfully removed via enterotomy. The patient recovered without complications and was discharged in stable condition. The complexity of management, particularly in elderly patients with multiple comorbidities, necessitates careful consideration between the one-stage and two-stage surgical approaches. In this case, the decision to perform an enterotomy without immediate cholecystectomy reflects a two-stage strategy, aimed at minimizing operative risk while addressing the immediate obstruction. This approach underscores the need for individualized management plans, where the choice between one-stage and two-stage surgery is guided by the patient's overall clinical status.
Subsite preferences of pepstatin-insensitive carboxyl proteinases from bacteria.

Abstract:Pseudomonas sp. 101 carboxyl proteinase (PCP) and Xanthomonas sp. T-22 carboxyl proteinase (XCP), the first and second unique carboxyl proteinases from prokaryotes to be isolated and characterized, are not inhibited by the classical carboxyl proteinase inhibitor pepstatin. In this study, we elucidated their subsite preferences by using a series of synthetic chromogenic substrates, Lys-Pro-Ile(P3)-Glu(P2)-Phe*Nph-Arg(P2')-Leu(P3') (Nph is p-nitrophenylalanine, Phe*Nph is the cleavage site) with systematic substitutions at the P3, P2, P2', and P3' positions. Among 45 substrates tested, the best substrate for PCP had a Leu replacement at the P2 position (kcat = 27.2 s-1, Km = 4.22 microM, kcat/Km = 6.43 microM-1.s-1), and that for XCP had an Ala replacement at the P3 position (kcat = 79.4 s-1, Km = 6.05 microM, kcat/Km = 13.1 microM-1. s-1). PCP and XCP preferred such charged amino acid residues as Glu, Asp, Arg, or Lys at the P2' position. This suggested that the S2' subsites of PCP and XCP are occupied by hydrophilic residues, similar to that of pepstatin-insensitive carboxyl proteinase from Bacillus coagulans J-4 [Shibata et al. (1998) J. Biochem. 124, 642-647]. In contrast, the S2' subsite of pepstatin-sensitive carboxyl proteinases (aspartic proteinases) is hydrophobic in nature. Thus, the hydophilic nature of the S2' subsite appears to be a distinguishing feature of pepstatin-insensitive carboxyl proteinases.

Abstract:[structure: see text] Eight new compounds, MPC1001 and MPC1001B-H, were isolated from the fungus Cladorrhinum sp. KY4922. Multiple NMR experiments and CD data revealed MPC1001 to be an O-methyl derivative of emestrin, a 15-membered antifungal antibiotic containing a unique epidithiodioxopiperazine skeleton. Other compounds were elucidated to be structurally related novel analogues. MPC1001 and the analogues exerted potent antiproliferative activities against a human tumor cell line.
Enhancement of Target Contrast in Polarimetric Imagery Using Image Fusion

Abstract:Abstract : We have examined the use of two statistical/numerical image fusion techniques for polarimetric imagery that each allow for an objective analysis of the resultant data products. The first technique is principal component analysis (PCA), a standard multivariate image processing method used to redistribute the information within the data set and possibly reduce the dimensions of the data set. The second method fuses conventional thermal imagery with a novel data product, the enhanced degree-of-linear polarization (DOLP). We performed receiver operating characteristic (ROC) curve analysis to quantify the resultant contrast between the manmade objects in the scene and the natural backgrounds. We found that the PCA data products inconsistently conveyed the desired contrast information, and that ROC curve analysis is not well suited to quantify the contrast in the PCA imagery. On the other hand, fusing the conventional thermal and the enhanced DOLP images provided a robust data product that effectively combines the information provided by the sensor.
Effects of general and local anaesthesia on innate and cell-mediated immunity in dogs.

Abstract:Received: Revised: Accepted: 2014–06–19 2014–07–08 2014–07–08 The dynamics of innate and specific immunity was investigated in dogs under halothane general anaesthesia and lumbosacral epidural anaesthesia using lidocaine. Phagocytic number (PN), phagocytic index (PI), and nitro blue tetrazolium (NBT) test were used to determine phagocytic properties of neutrophils during both anaesthesia regimens. Specific immunity was assessed by changes in lymphocyte number and distribution of their main subsets. The principal stress hormones were also measured in the blood and their relation with the factors of immune system was evaluated. The results showed an elevation in PN, PI and a reduction in NBT value at 120 min of anaesthesia in both groups, which returned gradually to base values. The changes were, however more pronounced in epidural group. Total lymphocyte number decreased at the cost of B–lymphocytes at 120 min and 24 hours in both groups. On the contrary, CD5+ and CD8+ cells increased in both anaesthesia types at 120 min. General and local anaesthesia produced similar increase in adrenaline, cortisol, and blood glucose. In conclusion, 2 hours of halothane and epidural anaesthesia may stimulate innate immunity and lead to increased number of cells with cytotoxic properties. Abstract:Gallstone ileus is a rare complication of cholelithiasis which represent 1-3% of the causes of intestinal obstruction in the general population and is more commonly seen in females. Gallstone ileus does not present with unique symptoms, making the diagnosis difficult. Symptoms are often non-specific with intermittent intestinal obstruction. The management is surgical, but there is no consensus as to which of the different surgical techniques is the procedure of choice. We report an interesting case of a 78-year-old male brought to casualty with abdominal pain following a history of fall. Managed as a case of blunt trauma abdomen initially. Surprisingly investigations showed features of small bowel obstruction, a hyperdense structure in the distal jejunum with dilated proximal jejunal loops suggestive of gallstone ileus. The patient had undergone exploratory laparotomy with enterotomy and removal of gallstones without any postoperative complications. Intraoperatively cholecystoduodenal fistula was identified and the patient was electively planned for cholecystectomy and fistula repair.

Abstract:Introduction: Gallstone ileus is a life-threatening surgical emergency where characteristic imaging can be diagnostic. Jejunum is the one of the rare sites of gallstone impaction. Materials and methods: We hereby emphasize the role of multidetector computed tomography (MDCT) by describing a case of jejunal gallstone ileus with cholecystoduodenal fistula in a 59-year-old lady who presented with symptoms and signs of proximal small bowel obstruction. Conclusion: MDCT of the abdomen established the diagnosis, and the patient managed surgically.

Abstract:We compare dynamic mean-field and dynamic cavity methods to describe the stationary states of dilute kinetic Ising models. We compute dynamic mean-field theory by expanding in interaction strength to third order, and we compare to the exact dynamic mean-field theory for fully asymmetric networks. We show that in diluted networks, the dynamic cavity method generally predicts magnetizations of individual spins better than both first-order ("naive") and second-order ("TAP") dynamic mean-field theory.
Loss of FAT Atypical Cadherin 4 Expression Is Associated with High Pathologic T Stage in Radically Resected Gastric Cancer

Abstract:Purpose Recent studies have revealed recurrent alterations in the cell adhesion gene FAT4, a candidate tumor suppressor gene, in cancer. FAT atypical cadherin 4 (FAT4) is a transmembrane receptor involved in the Hippo signaling pathway, which is involved in the control of organ size. Here, we investigated the loss of FAT4 expression and its association with clinicopathological risk factors in gastric cancer. Materials and Methods We assessed the expression of FAT4 by using immunohistochemistry on three tissue microarrays containing samples from 136 gastric cancer cases, radically resected in the Soonchunhyang University Cheonan Hospital between July 2006 and June 2008. Cytoplasmic immunoexpression of FAT4 was semi-quantitatively scored using the H-score system. An H-score of ≥10 was considered positive for FAT4 expression. Results Variable cytoplasmic expressions of FAT4 were observed in gastric cancers, with 33 cases (24.3%) showing loss of expression (H-score <10). Loss of FAT4 expression was associated with an increased rate of perineural invasion (H-score <10 vs. ≥10, 36.4% vs. 16.5%, P=0.015), high pathologic T stage (P=0.015), high tumor-node-metastasis stage (P=0.017), and reduced disease-free survival time (H-score <10 vs. ≥10, mean survival 62.7±7.3 months vs. 79.1±3.1 months, P=0.025). However, no association was found between the loss of FAT4 expression and tumor size, gross type, histologic subtype, Lauren classification, lymphovascular invasion, or overall survival. Conclusions Loss of FAT4 expression appears to be associated with invasiveness in gastric cancer.
Gallstone ileus 30 years after cholecystectomy and hepaticojejunostomy

Abstract:Gallstone ileus occurs when the small or large intestine is obstructed by a gallstone and is a rare occurrence in a post-cholecystectomy patient. Non-specific clinical symptoms and inconsistent imaging results often lead to a delay in diagnosis. Complex anatomy, such as a Roux-en-Y biliary-enteric anastomosis, can increase the risk of stone formation and further confound a potential case of gallstone ileus. Here, we present a rare case of gallstone ileus at the anastomosis site of a Roux-en-Y hepaticojejunostomy done 30 years prior for a common bile duct injury during a cholecystectomy. The possibility of negative CT findings, pattern of presentation on imaging as intussusception, and potential pathomechanism of gallstone formation in post-cholecystectomy patients are discussed. Through this case and review of similar cases, we emphasise the need for further study of post-cholecystectomy gallstone ileus and the importance of clinical suspicion during diagnosis.
