Abstract:It seems that the routine circumcision of infant boys is joining the ranks of the intractable. In a recent review of 50 years of paediatric ethics in Australia, Gillam notes that the ethical debate on infant male circumcision ‘...has seemingly gone round in circles rather than moving forward...’. One only need Google ‘circumcision’ to see that it is now keeping company with issues like abortion and euthanasia by virtue of its thorniness and ability to polarise. At one end are the ‘Intactivists’, who maintain that the benefits of circumcision do not outweigh the risks and that a child’s right to bodily integrity should be respected regardless of the beliefs of their parents. At the other end are those who believe circumcision is akin to a surgical ‘vaccine’ and that parents should be compelled to have their infant sons circumcised. While it is true that the spectrum of opinion on both the scientific and moral questions related to circumcision is broad, it is also true that there is significant common ground held, particularly in the recommendations of major paediatric medical bodies in the westernworld. For example, the AmericanAcademy of Paediatrics states that themedical benefits of circumcision outweigh the risks, a notably different assessment to that of the Royal Australasian College of Physicians (RACP) and similar bodies in the United Kingdom, Europe and Canada. However, despite their differing interpretations of the evidence, none of the aforementioned bodies believe the evidence for benefit is strong enough to insist on routine circumcision, nor do any think the evidence for harm is compelling enough to impose a complete ban. The situation is similar with the ethical concerns. Some key arguments for circumcision falling within the zone of parental discretion are that there may be significant socio-cultural benefits to being circumcised and that parents are best placed to assess these. Additionally, some argue that the harms associated with circumcision are not great enough to warrant over-riding parental autonomy, with parallel examples such as the permissibility of ear piercing often being given. The main argument for circumcision not being within the zone of parental discretion is that children have rights to bodily integrity, regardless of the beliefs of their parents. Many children’s rights advocates agree that this should also apply to practices like ear piercing in infants. Perhaps the most compelling parallel example against circumcision being in the zone of parental discretion is the absolute prohibition, in western countries, of all forms of female genital cutting, including nonfunction-limiting ritual nicks. Proponents for a ban on all forms of circumcision argue that there is nomorally significant difference between non-function-limiting female genital cutting and male circumcision, and it is therefore ethically inconsistent to allow one and not the other. The ethical positions of paediatric medical bodies reflect this spectrum of opinion, but, similar to their positions on the scientific benefits of circumcision, their recommendations for practice overlap considerably. For example, the Royal Dutch Medical Association considers infant circumcision a violation of a child’s right to physical integrity. However, they do not go so far as to recommend it be made illegal, but rather emphasise the importance of informed consent of parents and an ongoing dialoguewith religious and cultural groups for whom circumcision is an important ritual. At the other end of the spectrum, the American Academy of Paediatrics are very comfortable with parents taking their own cultural and religious beliefs into account when deciding whether or not to circumcise their sons but still emphasise the importance of a robust consent process. In a nutshell, with significantly varying degrees of comfort, most major paediatric bodies agree that the decision to circumcise lies within the zone of parental discretion. While the Intactivistsmayfind this unsatisfactory,when ethics is practiced outside of an ivory tower, pragmatism is essential. It is highly unlikely that circumcision will be made illegal in Australia in the foreseeable future.While this does not mean that campaigns at legislative and policy level should cease, it does mean that minimising the harms associated with circumcision is an urgent priority. Another commonality in the recommendations given by paediatric groups is that of the conditions under which circumcision should be performed. The majority of paediatric medical bodiesmake recommendations in linewith those of the RACP: that parents are given accurate and unbiased information with which to make the decision, and that the procedure be performed by an appropriately trained practitioner, with adequate pain management. The RACP define an appropriately trained practitioner as a person who can both perform the procedure and deal with any complications arising from it. They identify dorsal penile nerve block (DPNB) as the anaesthetic of choice in neonatal circumcision. The harm associated with inadequate pain relief in neonates is well established, as is the inadequacy of topical anaesthetic cream alone in this situation. Given there are no other Australian guidelines for this procedure, it is reasonable to consider the RACP recommendations as the accepted standard of practice. Approximately 10–20% of newborn boys in Australia are circumcised. The majority of circumcisions in Australia are performed by general practitioners (GPs) in their rooms. For example, Medicare data for the year 2013–2014 show that 16827 circumcisions were performed nationally on males under 6 months of age and 7300 on males of all other ages. Abstract:Objective To develop a predictive model for thiamine responsive disorders (TRDs) among infants and young children hospitalized with signs or symptoms suggestive of thiamine deficiency disorders (TDDs) based on response to therapeutic thiamine in a high-risk setting. Study design Children aged 21 days to <18 months hospitalized with signs or symptoms suggestive of TDD in northern Lao People's Democratic Republic were treated with parenteral thiamine (100 mg daily) for ≥3 days in addition to routine care. Physical examinations and recovery assessments were conducted frequently for 72 hours after thiamine was initiated. Individual case reports were independently reviewed by three pediatricians who assigned a TRD status (TRD or non-TRD), which served as the dependent variable in logistic regression models to identify predictors of TRD. Model performance was quantified by empirical area under the receiver operating characteristic curve. Results A total of 449 children (median [Q1, Q3] 2.9 [1.7, 5.7] months old; 70.3% exclusively/predominantly breastfed) were enrolled; 60.8% had a TRD. Among 52 candidate variables, those most predictive of TRD were exclusive/predominant breastfeeding, hoarse voice/loss of voice, cyanosis, no eye contact, and no diarrhea in the previous 2 weeks. The area under the receiver operating characteristic curve (95% CI) was 0.82 (0.78, 0.86). Conclusions In this study, the majority of children with signs or symptoms of TDD responded favorably to thiamine. While five specific features were predictive of TRD, the high prevalence of TRD suggests that thiamine should be administered to all infants and children presenting with any signs or symptoms consistent with TDD in similar high-risk settings. The usefulness of the predictive model in other contexts warrants further exploration and refinement. Trial registration Clinicaltrials.gov NCT03626337 .

Anaemia among lactating and non-lactating women in low-income and middle-income countries: a comparative cross-sectional study

Abstract:Objective This study aimed to assess the prevalence and determinants of anaemia among lactating and non-lactating women in low-income and middle-income countries (LMICs). Design Comparative cross-sectional study. Setting LMICs. Participants Reproductive-age women. Primary outcome Anaemia. Methods Data for the study were drawn from the recent 46 LMICs Demographic and Health Surveys (DHS). A total of 185 330 lactating and 827 501 non-lactating women (both are non-pregnant) who gave birth in the last 5 years preceding the survey were included. STATA V.16 was used to clean, code and analyse the data. Multilevel multivariable logistic regression was employed to identify factors associated with anaemia. In the adjusted model, the adjusted OR with 95% CI and a p value <0.05 was reported to indicate statistical association. Result The prevalence of anaemia among lactating and non-lactating women was found at 50.95% (95% CI 50.72, 51.17) and 49.33% (95% CI 49.23%, 49.44%), respectively. Maternal age, mother's educational status, wealth index, family size, media exposure, residence, pregnancy termination, source of drinking water and contraceptive usage were significantly associated determinants of anaemia in both lactating and non-lactating women. Additionally, the type of toilet facility, antenatal care visit, postnatal care visit, iron supplementation and place of delivery were factors significantly associated with anaemia in lactating women. Besides, smoking was significantly associated with anaemia in non-lactating women. Conclusions and recommendations The prevalence of anaemia was higher in lactating women compared with non-lactating. Almost half of the lactating and non-lactating women were anaemic. Both individual-level and community-level factors were significantly associated with anaemia. Governments, non-governmental organisations, healthcare professionals and other stakeholders are recommended to primarily focus on disadvantageous communities where their knowledge, purchasing power, access to healthcare facilities, access to clean drinking water and clean toilet facilities are minimal.

Abstract:Introduction The WHO recommends daily oral iron supplementation for 12 weeks in women and adolescents where anaemia prevalence is greater than 40%. However, if iron deficiency is not a major cause of anaemia, then, at best, untargeted iron supplementation is a waste of resources; at worst, it could cause harm. Further, different forms of iron with varying bioavailability may present greater risks of harm. Methods and analysis A 12-week three-arm, double-blind, randomised controlled supplementation trial was conducted in Cambodia to determine if there is potential harm associated with untargeted iron supplementation. We will recruit and randomise 480 non-pregnant women (ages 18–45 years) to receive one of three interventions: 60 mg elemental iron as ferrous sulfate (the standard, commonly used form), 18 mg ferrous bisglycinate (a highly bioavailable iron amino acid chelate) or placebo. We will measure ferritin concentrations (to evaluate non-inferiority between the two forms of iron), as well as markers of potential harm in blood and stool (faecal calprotectin, gut pathogen abundance and DNA damage) at baseline and 12 weeks. Mixed-effects generalised linear models will be used to assess the effect of iron on ferritin concentration and markers of potential harm at 12 weeks. Ethics and dissemination Ethical approval was obtained from the University of British Columbia Clinical Research Ethics Board (H18-02610), the Children's and Women's Health Centre of British Columbia Research Ethics Board (H18-02610) and the National Ethics Committee for Health Research in Cambodia (273-NECHR). Findings will be published in peer-reviewed journals, presented to stakeholders and policymakers globally and shared within participants’ communities. The amelioration of myopia using biofeedback of accommodation: a review

Abstract:The purpose of this review is to give an account of the issues raised by reports that myopia can be treated by operant conditioning or biological feedback. Devices are available that provide auditory feedback of accommodative state such that variation in tone frequency provides a means by which the patient can monitor his or her ability to induce changes in accommodation response, the task thus being to change the pitch of the tone in a direction corresponding lo a more distant far point. The aim of a training programme is to reinforce and establish control over the accommodative response by a process akin lo operant conditioning and subsequently to transfer the skill to normal visual environments such that a manifest reduction in myopia becomes apparent. Although there are reports that the technique can induce a reduction in myopia by up to 3 D the findings are of limited value owing to the tack of objective data, for example pre‐ and post‐training cycloplegic refraction. The pre‐ and post‐training measures of subjective letter acuity used by many studies are, with repeated trials, particularly vulnerable to individual differences in the ability to learn how to discriminate and interpret blurred images. The review concludes that more comprehensive clinical trials arc needed before accommodation biofeedback can qualify as an established method of clinical treatment of myopia. The trials should be designed to encompass the following issues: the characteristics of a feasible physiological model linking accommodation and myopia development; the rationale with regard to patient selection: the technical performance of the optometer employed; the characteristics of the control group used; the criteria for assessment of myopic change: the transfer of training to performance in normal visual environments; the economic viability of the programme of training and equipment: and the skill, training and knowledge of the clinician implementing the training programme.
The Prevalence of Anemia Among Vietnamese Children: A Systematic Review and Meta-Analysis

Abstract:Asia Pacific Journal of Public Health, Ahead of Print. While anemia constitutes a prevalent health issue among children in Vietnam, comprehensive aggregate data remain scarce. Consequently, the objective of this meta-analysis is to establish the pooled prevalence of anemia in the pediatric population of Vietnam. A comprehensive systematic search of existing prevalence studies was conducted up until June 2022, encompassing databases such as PubMed, Scopus, and the Cochrane Library, as well as reference list analysis. Pooled prevalence estimates, along with 95% confidence intervals (CIs), were calculated using a random-effects model. Publication bias was assessed using the Egger regression test and funnel plot inspection. Subgroup analyses were performed to identify potential variations in anemia prevalence across study groups. The pooled prevalence estimate for anemia among children under 10-year-olds amounted to 35.5% (95% CI = [23.9, 49.1]). Notably, gender-specific analyses unveiled distinct prevalence rates, with 37.8% among males and 33.4% among females. Furthermore, variations in anemia prevalence among Vietnamese children were evident across different demographic strata, encompassing gender, age groups, and rural or poor rural residency. The calculated pooled prevalence of anemia among children in Vietnam revealed a significant burden. Addressing this issue necessitates targeted interventions within specific regions and the implementation of government policies aligned with global targets for the eradication of childhood anemia.

Abstract:In Myanmar, 60% of the population consists of mothers and children, and they are the groups most vulnerable to anemia. The objectives of this study are to determine (1) the anemia prevalence among lactating women and (2) the risk factors associated with anemia. Convenience sampling was used to select three villages in two different regions (Kachin and Shan) in Myanmar. Hemoglobin and anthropometric indicators were measured for 733 lactating women. Logistic regression analyses were used to determine factors associated with anemia. The anemia prevalence rate was 60.3% in lactating women, with 20.3% of lactating women having severe anemia. Factors of malnutrition (P = 0.026), self-reported symptoms of night blindness or poor dark adaptation (P < 0.001), lack of primary education experience (P < 0.001), low family annual capita income (< 800 MMK; P < 0.001), drinking spring or river water (P < 0.001), and drinking unboiled water (P = 0.016) were associated with anemia. To promote health in lactating women, a comprehensive intervention is needed in these regions.

Abstract:Abstract Background During pregnancy, a mother’s nutritional needs increase to meet the added nutrient demands for fetal growth and development. An enhanced understanding of adequate nutrition and sufficient weight gain during pregnancy can guide development of policies and strategies for maternal nutrition care, actions that will ultimately promote better pregnancy outcomes. In a sample of pregnant women in Vietnam, this study characterized maternal nutrition status and gestational weight gain at a mid-pregnancy baseline, then examined the association of these variables with specific birth outcomes. Methods The study used baseline data from a randomized, controlled trial that compared pregnant Vietnamese women who received a nutritional intervention group with those who received only standard dietary counseling (control group). At baseline (26–29 weeks gestation), mothers’ dietary reports were collected, and intake of 10 macro- and micronutrients was estimated; data for baseline gestational weight gain was collected for all pregnant women enrolled into the study ( n = 228). This analysis also used weights, lengths, and head circumferences at birth for infants of mothers in the control group. Results At baseline, 95% of the pregnant women had concurrent inadequacies for more than five nutrients, and nearly half had concurrent inadequacies for more than ten nutrients. Almost two-thirds of the pregnant women did not meet recommendations for gestational weight gain. We found a significant, inverse association between the number of nutrient inadequacies and gestational weight gain (overall p ≤ 0.045). After adjusting for potential confounders, gestational weight gain was positively associated with birth weight, length at birth, birth weight-for-age z -score and length-for-age z -score (all p ≤ 0.006). Conclusions Our findings raise concern over the high proportion of pregnant women in Vietnam who have multiple concurrent nutrient inadequacies and who fall short of meeting recommended gestational weight gain standards. To ensure better birth outcomes in this population, policies and strategies to improve the status of maternal nutrition are greatly needed. Prevalence of Anaemia, Iron Deficiency, and Iron Deficiency Anaemia in Women of Reproductive Age and Children under 5 Years of Age in South Africa (1997–2021): A Systematic Review

Abstract:Using a systematic review method, the prevalence of anaemia, iron deficiency (ID), and iron deficiency anaemia (IDA) in women of reproductive age (WRA) and children under 5 years of age was obtained to inform priorities in health planning and policy in South Africa. We searched electronic databases for articles published between 1997 and 2021. A total of 713 articles were identified, of which 14 articles comprising 9649 WRA and 4085 children were included. Since most of the included studies were of low quality, we did not pool data in a meta-analysis due to heterogeneity (I2 > 75%). In WRA, anaemia prevalence ranged from 22.0% to 44.0%; ID from 7.7% and 19.0%; and IDA from 10.5% to 9.7%. The prevalence of anaemia in pregnancy was 29.0% to 42.7%; and 60.6% to 71.3% in HIV-infected pregnant women. Three national surveys reported anaemia in children at 28.9%, 10.7%, and 61.3%, respectively. Overall, among the children under 5 years old, anaemia was more prevalent in 1-year-olds (52.0%) compared to the other age groups. Between 2005 and 2012, ID increased by 3.8% and IDA decreased by 83.2% in children. Anaemia in WRA and children under 5 years in South Africa was a moderate public health concern. Therefore, interventions addressing anaemia should be intensified, and policies on iron supplementation and food fortification need to be revised and aligned to the WHO multiple micronutrient supplementation recommendations.

Abstract:Anaemia in pregnant women is a major public health concern. A number of multilevel factors have been attributed as contributors to anaemia in pregnancy. The purpose of this study was to examine the multilevel factors predicting anaemia among pregnant women in Ghana. Data for this study were obtained from the 2019 Ghana Malaria Indicator Survey (GMIS) conducted between September 25 and November 24, 2019 in all regions in Ghana. The weighted sample comprised 353 pregnant women aged 15-49 years. Data were analysed with SPSS version 25 using descriptive statistics, Pearson's chi-square test and binary logistic regression modelling. In this study, the outcome variable was anaemia status among pregnant women, while the predictor variables included individual, household, community, and health system level factors. The overall prevalence of anaemia among pregnant women was 28.7%. Of these, 14.5% had mild anaemia, and 13.2% and 1.1% had moderate and severe anaemia, respectively. The results indicate that a higher probability of anaemia in pregnancy is likely to be found among pregnant women less than 35 years (15-24 years, aOR = 3.31; C.I: 1.13-9.73) (25-34 years, aOR = 2.49; C.I:1.06-5.84). A higher likelihood of anaemia was found among pregnant women who did not take SP drug (aOR = 3.70; C.I:1.20-11.43) and also among household heads aged 30-39 years (aOR = 4.51; C.I:1.09-18.71). However, a lower odd of being anaemic was found among pregnant women who had attained secondary or higher education (aOR = 0.19; C.I:0.05-0.76), women in the richest households (aOR = 0.02; C.I:0.00-0.42) and those accessing unimproved drinking water (aOR = 0.37; C.I:0.14-0.95). Furthermore, pregnant women with health insurance coverage had lower probability (aOR = 0.24; C.I: 0.06-0.94) of being anaemic. The results highlight the importance of varying factors at different levels in understanding the prevalence of anaemia among pregnant women. Understanding these factors will play a major contributor to developing strategies and programmes aimed at addressing anaemia among pregnant women.

Abstract:Objective: This study assessed nutritional status among Thai children using anthropometry, dietary intakes, and micronutrient status. Design: Cross-sectional survey with multi-stage cluster sampling. Body weight and height were measured in all children. Dietary intakes were assessed using 24-hour dietary recall. Biochemical assessment was performed in one-third of the children. Setting: The study was conducted in Thailand's four geographical regions and Bangkok. Participants: 3478 Thai children aged 0.5-12.9 years. Results: Stunting showed a downward trend by age group and was most prevalent among infants and toddlers. Overweight and obesity showed a significant upward trend by age group, location, and sex, and was highest among children aged 7-12.9 years. Risks of inadequate micronutrient intakes (calcium, iron, zinc, vitamins A, C, and D) were high (53.2-93.6%). Prevalence of zinc and mild vitamin A deficiencies were low; vitamin D and B12 deficiencies were nil. Vitamin D insufficiency was significantly higher in the urban area and among girls aged 7-12.9 years. Anemia was very high in infants and toddlers (56.6 and 35.2%), but showed a significant downward trend by age group. There was an overall high prevalence of iron deficiency without anemia (25%) versus iron deficiency anemia (4.2%) among children aged 4-12.9 years old. Conclusions: The high prevalence of stunting and anemia among children aged 1-3.9 years and overweight and obesity among children aged 7-12.9 years requires continued attention. While prevalence of biochemical micronutrient deficiencies was not high (except for iron), high prevalence of dietary inadequacies for several micronutrients warrants further in-depth investigations.

Abstract:These studies indicate that this new steroid is more effective in the treatment of psoriasis than are other steroids. The maintenance dose for retaining remission in disseminated lupus is much less than with other steroids. Nevertheless, the authors emphasize the use of all steroids only for short-term therapy except in certain serious diseases. Triamcinolone produces untoward side-effects as do other steroids. Its use topically is effective.
Prevalence and determinants of anaemia among pregnant women in a high malaria transmission setting: a cross-sectional study in rural Burkina Faso

Abstract:Introduction: anemia, the commonest nutritional deficiency disorder among pregnant women in sub-Saharan Africa, is associated with severe peripartum complications. Its regular monitoring is necessary to timely inform clinical and preventive decision-making. The aim of this study was to assess the prevalence and determinants of anemia among pregnant women in rural areas of Burkina Faso. Methods: between August 2019 and March 2020, a cross-sectional study was conducted to collect maternal sociodemographic, gynaeco-obstetric, and medical characteristics by face-to-face interview or by review of antenatal care books. In addition, maternal malaria was diagnosed by standard microscopy and the hemoglobin levels (Hb) measured by spectrophotometry. The proportion of anaemia (Hb<11.0 g/dL), moderate (7.0<Hb<9.9 g/dL) and severe (Hb<7.0 g/dL) anaemia were determined. The maternal factors associated with anaemia were identified using regression models with likelihood ratio tests. A p-value < 0.05 was considered statistically significant. Results: of 594 pregnant women assessed, the mean hemoglobin level (± standard deviation) was 10.7 (±0.1) g/dL, and the prevalence of anemia was 54.4% (323/594). The proportion of moderate, and severe anemia among pregnant women was 49.2% (95% CI: 45.1%-53.2%), and 5.2% (95% CI: 3.7%-7.3%) respectively. Multivariate analysis showed that the young maternal age (<20 years old) (adjusted OR (aOR): 1.5, 95% CI: 1.1-2.3) and the presence of malaria (aOR: 2.0, 95% CI: 1.3-3.2) were independently associated with the presence of maternal anemia. Conclusion: anemia remains common in the study setting and interventions to strengthen malaria prevention in pregnancy, particularly among young adolescent pregnant women, are required to prevent maternal anemia.

Abstract:Background: Anaemia, especially iron deficiency anemia, is an important cause of morbidity and mortality in African women and children. Aim: To assess the intake of nutrients related to iron and anaemia status among mothers in smallholder agrarian communities in Northern Ghana where anaemia is known to be endemic. Setting: Tolon Kumbumgu district and Tamale Metropolis in Ghana. Methods: A cross-sectional study was conducted among 161 mothers with children 6-59 months. Questionnaires on socio-demographics, household food security and production and food frequencies, and three 24-hour recalls were administered during structured interviews, and BMI was assessed. Dietary intakes were analysed with the Ghana Nutrient Database® (version 6.02). Nutrient intake was evaluated using the estimated average requirements and iron intakes using the probability method. Results: Most mothers (91.9%) had low literacy and were subsistence farmers. The staple diet was homemade unrefined, unfortified maize meal, homemade unfortified oil (shea butter), and seasonal green leafy vegetables (mostly amaranth), butternut, tomatoes, onions and legumes. Inadequate intakes of vitamin A (in 9.9%), folate (in 46.6%) and vitamin B12 (in 98.8%) were observed, in combination with high fibre (47.8 ± 19.0 g/day) intakes and high tea consumption. If 10% iron bio-availability was assumed, 33.1% were estimated to have inadequate iron intake; if 5% iron bio-availability was assumed, 80.8% were estimated to have inadequate iron intakes. Conclusion: In these low socio-economic agrarian communities, mothers of infants are living on home produce and rarely consumed foods (fortified salt, cooking oil and wheat flour) from the national food fortification programmes intended to address anaemia and other micronutrient deficiencies.
