Abstract:Abstract African Americans have a significantly higher risk of developing chronic kidney disease, especially focal segmental glomerulosclerosis -, than European Americans. Two coding variants (G1 and G2) in the APOL1 gene play a major role in this disparity. While 13% of African Americans carry the high-risk recessive genotypes, only a fraction of these individuals develops FSGS or kidney failure, indicating the involvement of additional disease modifiers. Here, we show that the presence of the APOL1 p.N264K missense variant, when co-inherited with the G2 APOL1 risk allele, substantially reduces the penetrance of the G1G2 and G2G2 high-risk genotypes by rendering these genotypes low-risk. These results align with prior functional evidence showing that the p.N264K variant reduces the toxicity of the APOL1 high-risk alleles. These findings have important implications for our understanding of the mechanisms of APOL1 -associated nephropathy, as well as for the clinical management of individuals with high-risk genotypes that include the G2 allele.

Abstract:Background: Among patients in the United States with chronic kidney disease, black patients are at increased risk for end-stage renal disease, as compared with white patients. Methods: In two studies, we examined the effects of variants in the gene encoding apolipoprotein L1 (APOL1) on the progression of chronic kidney disease. In the African American Study of Kidney Disease and Hypertension (AASK), we evaluated 693 black patients with chronic kidney disease attributed to hypertension. In the Chronic Renal Insufficiency Cohort (CRIC) study, we evaluated 2955 white patients and black patients with chronic kidney disease (46% of whom had diabetes) according to whether they had 2 copies of high-risk APOL1 variants (APOL1 high-risk group) or 0 or 1 copy (APOL1 low-risk group). In the AASK study, the primary outcome was a composite of end-stage renal disease or a doubling of the serum creatinine level. In the CRIC study, the primary outcomes were the slope in the estimated glomerular filtration rate (eGFR) and the composite of end-stage renal disease or a reduction of 50% in the eGFR from baseline. Results: In the AASK study, the primary outcome occurred in 58.1% of the patients in the APOL1 high-risk group and in 36.6% of those in the APOL1 low-risk group (hazard ratio in the high-risk group, 1.88; P<0.001). There was no interaction between APOL1 status and trial interventions or the presence of baseline proteinuria. In the CRIC study, black patients in the APOL1 high-risk group had a more rapid decline in the eGFR and a higher risk of the composite renal outcome than did white patients, among those with diabetes and those without diabetes (P<0.001 for all comparisons). Conclusions: Renal risk variants in APOL1 were associated with the higher rates of end-stage renal disease and progression of chronic kidney disease that were observed in black patients as compared with white patients, regardless of diabetes status. Abstract:BACKGROUND. Two coding alleles within the APOL1 gene, G1 and G2, found almost exclusively in individuals genetically similar to West African populations, contribute substantially to the pathogenesis of chronic kidney disease (CKD). The APOL gene cluster on chromosome 22 contains a total of 6 APOL genes that have arisen as a result of gene duplication. METHODS. Using a genome-first approach in the Penn Medicine BioBank, we identified 62 protein-altering variants in the 6 APOL genes with a minor allele frequency of >0.1% in a population of participants genetically similar to African reference populations and performed population-specific phenome-wide association studies. RESULTS. We identified rs1108978, a stop-gain variant in APOL3 (p.Q58*) , to be significantly associated with increased CKD risk, even after conditioning on APOL1 G1/G2 carrier status. These findings were replicated in the Veterans Affairs Million Veteran Program and the All of Us Research Program. APOL3 p.Q58* was also significantly associated with a number of quantitative traits linked to CKD, including decreased kidney volume. This truncating variant contributed the most risk for CKD in patients monoallelic for APOL1 G1/G2, suggesting an epistatic interaction and a potential protective effect of wild-type APOL3 against APOL1 -induced kidney disease. CONCLUSION. This study demonstrates the utility of targeting population-specific variants in a genome-first approach, even in the context of well-studied gene-disease relationships. FUNDING. Abstract:BACKGROUND AND IMPORTANCE: Balloon-assisted coil placement is an important technique for coil embolization of broad-neck aneurysms. With this technique, we can prevent coil migration into a parent artery during a procedure. Complications of intraprocedural coil migration have been reported in the literature. However, delayed coil migration is extremely rare. We present a case of delayed coil migration after balloon-assisted coil embolization and describe our management of this complication. CLINICAL PRESENTATION: A 59-year-old man presented with hypertension and a tension headache. Clinical evaluation incidentally discovered an unruptured broad-neck aneurysm at the left internal carotid bifurcation. Endovascular embolization of the aneurysm was performed with a balloon-assisted technique. The patient had a transient ischemic attack, and a skull radiograph showed coil migration 3 months after the procedure. We performed an operation to remove the coils and to clip the aneurysm with superficial temporal artery and middle cerebral artery bypass. The patient was discharged without neurological deficit. CONCLUSION: This is a rare case in which delayed coil migration into the parent artery occurred after balloon-assisted coil embolization, highlighting the importance of surgical management of delayed coil migration.

Abstract:Background: Approximately 13% of black individuals carry 2 copies of the apolipoprotein L1 ( APOL1 ) risk alleles G1 or G2, which are associated with 1.5- to 2.5-fold increased risk of chronic kidney disease. There have been conflicting reports as to whether an association exists between APOL1 risk alleles and cardiovascular disease (CVD) that is independent of the effects of APOL1 on kidney disease. We sought to test the association of APOL1 G1/G2 alleles with coronary artery disease, peripheral artery disease, and stroke among black individuals in the Million Veteran Program. Methods: We performed a time-to-event analysis of retrospective electronic health record data using Cox proportional hazard and competing-risks Fine and Gray subdistribution hazard models. The primary exposure was APOL1 risk allele status. The primary outcome was incident coronary artery disease among individuals without chronic kidney disease during the 12.5-year follow-up period. We separately analyzed the cross-sectional association of APOL1 risk allele status with lipid traits and 115 cardiovascular diseases using phenome-wide association. Results: Among 30 903 black Million Veteran Program participants, 3941 (13%) carried the 2 APOL1 risk allele high-risk genotype. Individuals with normal kidney function at baseline with 2 risk alleles had slightly higher risk of developing coronary artery disease compared with those with no risk alleles (hazard ratio, 1.11 [95% CI, 1.01–1.21]; P =0.039). Similarly, modest associations were identified with incident stroke (hazard ratio, 1.20 [95% CI, 1.05–1.36; P =0.007) and peripheral artery disease (hazard ratio, 1.15 [95% CI, 1.01–1.29l; P =0.031). When both cardiovascular and renal outcomes were modeled, APOL1 was strongly associated with incident renal disease, whereas no significant association with the CVD end points could be detected. Cardiovascular phenome-wide association analyses did not identify additional significant associations with CVD subsets. Conclusions: APOL1 risk variants display a modest association with CVD, and this association is likely mediated by the known APOL1 association with chronic kidney disease.

Abstract:Importance: Coronavirus disease 2019 (COVID-19) confers significant risk of acute kidney injury (AKI). Patients with COVID-19 with AKI have high mortality rates. Objective: Individuals with African ancestry with 2 copies of apolipoprotein L1 (APOL1) variants G1 or G2 (high-risk group) have significantly increased rates of kidney disease. We tested the hypothesis that the APOL1 high-risk group is associated with a higher-risk of COVID-19-associated AKI and death. Design, setting, and participants: This retrospective cohort study included 990 participants with African ancestry enrolled in the Million Veteran Program who were hospitalized with COVID-19 between March 2020 and January 2021 with available genetic information. Exposures: The primary exposure was having 2 APOL1 risk variants (RV) (APOL1 high-risk group), compared with having 1 or 0 risk variants (APOL1 low-risk group). Main outcomes and measures: The primary outcome was AKI. The secondary outcomes were stages of AKI severity and death. Multivariable logistic regression analyses adjusted for preexisting comorbidities, medications, and inpatient AKI risk factors; 10 principal components of ancestry were performed to study these associations. We performed a subgroup analysis in individuals with normal kidney function prior to hospitalization (estimated glomerular filtration rate ≥60 mL/min/1.73 m2). Results: Of the 990 participants with African ancestry, 905 (91.4%) were male with a median (IQR) age of 68 (60-73) years. Overall, 392 (39.6%) patients developed AKI, 141 (14%) developed stages 2 or 3 AKI, 28 (3%) required dialysis, and 122 (12.3%) died. One hundred twenty-five (12.6%) of the participants were in the APOL1 high-risk group. Patients categorized as APOL1 high-risk group had significantly higher odds of AKI (adjusted odds ratio [OR], 1.95; 95% CI, 1.27-3.02; P = .002), higher AKI severity stages (OR, 2.03; 95% CI, 1.37-2.99; P < .001), and death (OR, 2.15; 95% CI, 1.22-3.72; P = .007). The association with AKI persisted in the subgroup with normal kidney function (OR, 1.93; 95% CI, 1.15-3.26; P = .01). Data analysis was conducted between February 2021 and April 2021. Conclusions and relevance: In this cohort study of veterans with African ancestry hospitalized with COVID-19 infection, APOL1 kidney risk variants were associated with higher odds of AKI, AKI severity, and death, even among individuals with prior normal kidney function.

Abstract:The disproportionate risk for idiopathic proteinuric podocytopathies in Black people is explained, in part, by the presence of two risk alleles (G1 or G2) in the APOL1 gene. The pathogenic mechanisms responsible for this genetic association remain incompletely understood. We analyzed glomerular RNASeq transcriptomes from patients with idiopathic nephrotic syndrome of which 72 had inferred African ancestry (AA) and 152 did not (noAA). Using gene coexpression networks we found a significant association between APOL1 risk allele number and the coexpression metamodule 2 (MM2), even after adjustment for eGFR and proteinuria at biopsy. Unadjusted Kaplan-Meier curves showed that unlike noAA, AA with the highest tertile of MM2 gene activation scores were less likely to achieve complete remission (p≤0.014). Characteristic direction (ChDir) identified a signature of 1481 genes, which separated patients with APOL1 risk alleles from those homozygous for reference APOL1. Only in AA, the tertile with the highest activation scores of these 1481 genes was less likely to achieve complete remission (p≤0.022) and showed a trend to faster progression to the composite event of kidney failure or loss of 40% eGFR (p≤0.099). The MM2 and ChDir genes significantly overlapped and were both enriched for Epithelial Mesenchymal Transition and inflammation terms. Finally, MM2 significantly overlapped with a parietal epithelial cell (PEC)-identity gene signature but not with a podocyte identity signature. Podocytes expressing variant APOL1s may generate inflammatory signals that activate PECs by paracrine mechanisms contributing to APOL1 nephropathy.

Abstract:Two coding variants of apolipoprotein L1 ( APOL1 ), called G1 and G2, explain much of the excess risk of kidney disease in African Americans. While various cytotoxic phenotypes have been reported in experimental models, the proximal mechanism by which G1 and G2 cause kidney disease is poorly understood. Here, we leveraged 3 experimental models and a recently reported small molecule blocker of APOL1 protein, VX-147, to identify the upstream mechanism of G1-induced cytotoxicity. In HEK293 cells, we demonstrated that G1-mediated Na + import/K + efflux triggered activation of GPCR/IP3–mediated calcium release from the ER, impaired mitochondrial ATP production, and impaired translation, which were all reversed by VX-147. In human urine-derived podocyte-like epithelial cells (HUPECs), we demonstrated that G1 caused cytotoxicity that was again reversible by VX-147. Finally, in podocytes isolated from APOL1 G1 transgenic mice, we showed that IFN-γ–mediated induction of G1 caused K + efflux, activation of GPCR/IP3 signaling, and inhibition of translation, podocyte injury, and proteinuria, all reversed by VX-147. Together, these results establish APOL1-mediated Na + /K + transport as the proximal driver of APOL1-mediated kidney disease.

Abstract:Summary Background APOL1 variants G1 and G2 are common in populations with recent African ancestry. They are associated with protection from African sleeping sickness, however homozygosity or compound heterozygosity for these variants is associated with chronic kidney disease (CKD) and related conditions. What is not clear is the extent of associations with non-kidney-related disorders, and whether there are clusters of diseases associated with individual APOL1 genotypes. Methods Using a cohort of 7462 UK Biobank participants with recent African ancestry, we conducted a phenome-wide association study investigating associations between individual APOL1 genotypes and conditions identified by the International Classification of Disease phenotypes. Findings We identified 27 potential associations between individual APOL1 genotypes and a diverse range of conditions. G1/G2 compound heterozygotes were specifically associated with 26 of these conditions (all deleteriously), with an over-representation of infectious diseases (including hospitalisation and death resulting from COVID-19). The analysis also exposed complexities in the relationship between APOL1 and CKD that are not evident when risk variants are grouped together: G1 homozygosity, G2 homozygosity, and G1/G2 compound heterozygosity were each shown to be associated with distinct CKD phenotypes. The multi-locus nature of the G1/G2 genotype means that its associations would go undetected in a standard genome-wide association study. Interpretation Our findings have implications for understanding health risks and better-targeted detection, intervention, and therapeutic strategies, particularly in populations where APOL1 G1 and G2 are common such as in sub-Saharan Africa and its diaspora. Abstract:Background Some but not all African-Americans (AA) who carry APOL1 nephropathy risk variants ( APOL1 ) develop kidney failure (end-stage kidney disease, ESKD). To identify genetic modifiers, we assessed gene–gene interactions in a large prospective cohort of the REasons for Geographic and Racial Differences in Stroke (REGARDS) study. Methods Genotypes from 8,074 AA participants were obtained from Illumina Infinium Multi-Ethnic AMR/AFR Extended BeadChip. We compared 388 incident ESKD cases with 7,686 non-ESKD controls, using a two-locus interaction approach. Logistic regression was used to examine the effect of APOL1 risk status (using recessive and additive models), single nucleotide polymorphism (SNP), and APOL 1 * SNP interaction on incident ESKD, adjusting for age, sex, and ancestry. APOL1 * SNP interactions that met the threshold of 1.0 × 10 −5 were replicated in the Genetics of Hypertension Associated Treatment (GenHAT) study (626 ESKD cases and 6,165 controls). In a sensitivity analysis, models were additionally adjusted for diabetes status. We conducted additional replication in the BioVU study. Results Two APOL1 risk alleles prevalence (recessive model) was similar in the REGARDS and GenHAT studies. Only one APOL1 –SNP interaction, for rs7067944 on chromosome 10, ~10 KB from the PCAT5 gene met the genome-wide statistical threshold ( P interaction = 3.4 × 10 −8 ), but this interaction was not replicated in the GenHAT study. Among other relevant top findings (with P interaction < 1.0 × 10 −5 ), a variant ( rs2181251 ) near SMOC2 on chromosome six interacted with APOL1 risk status (additive) on ESKD outcomes (REGARDS study, P interaction =5.3 × 10 −6 ) but the association was not replicated (GenHAT study, P interaction = 0.07, BioVU study, P interaction = 0.53). The association with the locus near SMOC2 persisted further in stratified analyses. Among those who inherited ≥1 alternate allele of rs2181251, APOL1 was associated with an increased risk of incident ESKD (OR [95%CI] = 2.27[1.53, 3.37]) but APOL1 was not associated with ESKD in the absence of the alternate allele (OR [95%CI] = 1.34[0.96, 1.85]) in the REGARDS study. The associations were consistent after adjusting for diabetes. Conclusion In a large genome-wide association study of AAs, a locus SMOC2 exhibited a significant interaction with the APOL1 locus. SMOC2 contributes to the progression of fibrosis after kidney injury and the interaction with APOL1 variants may contribute to an explanation for why only some APOLI high-risk individuals develop ESKD.

Abstract:The G1 and G2 APOL1 risk alleles are important risk factors for chronic kidney disease (CKD) among Black Americans; however, the genetic risk of CKD associated with APOL1 variants among individuals living in West Africa — the ancestral origin of most Black Americans — has not been thoroughly investigated. New findings from the H3Africa Kidney Disease Research Network and presented at ASN Kidney Week 2024 show that APOL1 risk variants are also important risk factors for the development and progression of CKD among individuals living in Ghana and Nigeria. This case–control study included 8,355 participants, 5,578 of whom had CKD. Forty-three per cent of all participants carried one APOL1 risk allele and 29.7% carried two. Individuals with two APOL1 risk alleles had a 25% higher chance of CKD than individuals with one risk allele or no risk alleles (adjusted OR 1.25; 95% CI 1.11–1.40) and an 84% higher chance of focal segmental glomerulosclerosis (FSGS; adjusted OR, 1.84; 95% CI 1.30–2.61). However, these risks were also increased among individuals with one APOL1 risk allele (18% higher chance of CKD; adjusted OR 1.18; 95% CI 1.04–1.33 and a 61% higher chance of FSGS; adjusted OR 1.61; 95% CI 1.04–2.48). Among individuals with APOL1 risk alleles, the adjusted odds ratio for CKD also increased according to CKD stage, suggesting that APOL1 risk variants are also a risk factor for disease progression.

Abstract:Significance People of recent African ancestry develop chronic kidney disease and end stage kidney failure at rates five times that of European-Americans. Two coding variants in the apolipoprotein-L1 (APOL1) gene account for nearly all this excess risk. The mechanisms by which APOL1 variants cause kidney failure are not understood. Recent evidence suggests that APOL1 transports cations, including K + , across lipid bilayers. Here, we show that tetracycline-induced expression of APOL1 kidney risk variants in T-REx-293 cells causes significant net efflux of intracellular K + , which, in turn, activates the stress-activated protein kinases (SAPKs) p38 MAPK and JNK, ultimately resulting in cytotoxicity. We propose that APOL1 nephropathy may be mediated by APOL1 risk variant-induced loss of intracellular K + and aberrant activation of SAPK signaling.

Abstract:Abstract Background and Aims Two polymorphisms of APOL1 gene, G1 and G2, exclusively found among patients of African ancestry, have been associated with chronic kidney disease (CKD) and end-stage kidney disease (ESKD)1. These high-risk polymorphisms have also been associated with collapsing glomerulopathy in various diseases. We aimed to evaluate the impact of APOL1 G1 and G2 polymorphisms on the clinical and pathological course LN which is already known to be more severe among patients of African ancestry2. Method We included patients from 6 hospitals in Paris and Marseille in France, between January 2017 and March 2020, with biopsy-proven LN, African ancestry and age >18 years at the time of inclusion. We excluded those with HIV infection. The data were retrospectively collected at LN diagnosis, 1 year after diagnosis and at last follow-up. APOL1 genotyping was performed and we divided patients in 2 groups: the high-risk genotype (HRG) group with 2 risk alleles and the low risk genotype (LRG) group with 1 or 0 risk allele. All patients signed a consent form for the genetic analysis and protocol approval was obtained from the ethic committee CERAPHP (Comité d'Ethique de la Recherche AP-HP Centre), registration number 00011928. Results Ninety-nine patients were included in the study, 13 in the HRG group and 86 in the LRG group. The median duration between LN diagnosis and inclusion in the study was 9.6 years [4.9-16.9]. At LN diagnosis, clinical and biological characteristics were similar except for kidney function that was lower in the HRG group compared to the LRG group with a median serum creatinine of 131μmol/L [69-687] versus 66μmol/L [52-133] (p = 0.0085). Patients in the HRG group were more likely to have a serum creatinine above 200 μmol/L compared to the LRG group (45.5% versus 10.5%, p = 0.0096, OR 7.1[1.8-28.6]), and required acute haemodialysis more frequently (30.8% versus 1.3% respectively, p = 0.0012, OR 34.7[3.5-345.1]). Collapsing glomerulopathy was more frequent in the HRG group (45.5% of patients, versus 4.5%, OR 17.5[3.3-91.9], p = 0.001). No significant difference was observed in the proportion of kidney response at 12 months, however, patients in the HRG group were more likely to develop CKD (33.3% versus 4.9%, OR 9.6[2.0-46.1]). Median follow-up from LN diagnosis to the end of study, ESKD or last follow-up was 7.9 years [3.4-13.3] and was similar between the 2 groups. At last follow-up, median eGFR was 22mL/min/1.73m2 [10-98] vs 99mL/min/1.73m2 [55-118] respectively, p = 0.0189. Survival without ESKD was poorer in the HRG group than in the LRG group with a hazard ratio (HR) of 5.0[1.6-15.8], p = 0.006, even after adjusting with the kidney response at 12 months (adjusted HR 6.24[1.8-21.6], p = 0.004) (Figure 1). Conclusion APOL1 genotype affects LN prognosis, resulting in a worse kidney function at diagnosis, development of collapsing glomerulopathy and higher risk of subsequent ESKD. Kidney survival remained significantly lower in the HRG group, after adjustment for kidney response at 12 months, suggesting that the prognosis of LN is heavily driven by APOL1 genotype regardless of the kidney response. APOL1 inhibitors are currently being developed, and could be used in the next future, for HRG patients with primary or secondary forms of collapsing glomerulopathy.

Abstract:Introduction Among individuals with high-risk APOL1 genotypes, the lifetime risk of developing kidney failure is ∼15%, indicating that other genetic variants or non-genetic modifiers likely contribute substantially to an individual patient's risk of progressive kidney disease. Here we estimate the prevalence and distribution of Mendelian kidney diseases among patients with high-risk APOL1 genotypes undergoing commercial genetic testing in the United States. Methods We analyzed clinical exome sequencing data from 15,181 individuals undergoing commercial genetic testing for Mendelian kidney disease in the United States from 2020-2021. We identified patients with high-risk APOL1 genotypes by the presence of G1/G1, G1/G2, or G2/G2 alleles. Patients carrying single risk APOL1 alleles were identified as G1/G0, G2/G0; the remainder of patients were G0/G0. We estimated the prevalence and distribution of Mendelian kidney disease stratified by APOL1 genotype and genetically predicted ancestry. Results Of 15,181 patients, 3119 had genetic testing results consistent with a molecular diagnosis of Mendelian kidney disease (20.5%). 1035 (6.8%) had high-risk APOL1 genotypes. Among patients with recent genomic African ancestry, the prevalence of Mendelian kidney diseases was lower in those with high-risk APOL1 genotypes (9.6%; n=91/944) compared to single risk APOL1 allele carriers (13.6%; n=198/1453) and those with G0/G0 APOL1 genotypes (16.6%; n=213/1281). Among patients with Mendelian kidney disease and recent genomic African ancestry, we observed differences in the prevalence of pathogenic/likely pathogenic variants in PKD1 (19.8% in high-risk v. 30.2% in low-risk genotypes), and COL4A4 (24.2% in high-risk v. 10.5% in low-risk genotypes). Conclusion In this selected population of patients undergoing clinical genetic testing, we found evidence of Mendelian kidney disease in ∼10% patients with high-risk APOL1 genotypes.

Abstract:In a study published in this edition of Sexual Health, Wilson et al. using mathematical modelling, suggest that individuals with primary HIV infection and those who are HIV infected but undiagnosed, contribute disproportionately to new HIV infections among men who have sex with men. Based on these findings, they make the case for increased HIV testing to increase rates of HIV diagnosis. The diagnosis of HIV has clear benefits for HIV infected individuals as well as the population. For the individual, early diagnosis provides the opportunity for appropriate patient management and initiation of antiretroviral therapy at an optimal point in time, which can now offer the expectation of a near normal life span. In contrast, those who are diagnosed late have much poorer health outcomes at significant health service cost. It is a disheartening reflection of current testing strategies that a substantial proportion of people newly diagnosed with HIV still present at an advanced stage in their infection. In the UK, 31% of people newly diagnosed with HIV first present for testing when their CD4 count is less than 200. At the population level, early detection of HIV contributes to improved HIV control by enabling partner notification and evidence suggests that individuals diagnosed with HIV usually alter their sexual behaviour so as to reduce the risk of further HIV transmission. The benefits from increased HIV detection would be greatest in populations where the proportion of undiagnosed individuals is highest. In 2003, there were an estimated 1million persons infected with HIV in the USA with one-quarter of these persons unaware of their status. In the UK it is estimated that of the 77 400 people with HIV, 28% of them are unaware of their infection. These data highlight a pressing need for new approaches to reduce barriers to testing. So how might testing for HIV be substantially increased? One way is through the use of rapid testing for HIV using either a finger prick blood sample or a sample of oral fluid obtained by a mouth swab with results available at the point of testing within minutes. The use of rapid HIV testing confers a number of potential benefits which may reduce barriers to testing. Rapid testing removes the typical 1–2 week delay between test and result with conventional HIV blood tests. This is important as it is not uncommon for clients to express anxiety about this waiting period and this may actually deter individuals from testing. Recent studies suggest that clients favour rapid HIV testing over conventional testing, citing the immediacy of results as the main factor behind this preference. 10 The provision of a result at the time of testing has the added advantage of preventing loss of patients to follow up. In the USA, 31% of people who tested HIV positive in 2000 did not return for their test result. However, the implementation of rapid testing in New York State in 2003 meant that 100% of those tested received their result and led to a 36% increase in testing over the previous year resulting in increased detection of previously undiagnosed HIV. A further advantage of rapid HIV tests is that they are ideally suited to non-clinical or community settings, providing access to HIV testing to individuals who might not otherwise access clinical services for testing. Oral fluid sampling may confer added benefits to users over finger prick blood testing as it may be perceived as less invasive and is less painful. Given that oral fluid is not considered potentially infectious unless it contains blood, the disposal of samples is also much easier. Many settings have been proposed as outreach sites including bars, clubs and saunas, further education and sports settings. However, we must recognise that individuals need a choice of venues for testing to include both community and clinic-based settings, as people vary greatly in their preferences. One sauna based UK study of men who have sex with men reported that some men would not feel comfortable testing for HIV in a club as they feared the consequences of receiving a positive result in that setting. Rapid testing appears popular with health care professionals. Moreover, after training and experience with rapid testing, counsellors without previous laboratory training have shown proficiency in rapid testing and express a preference for rapid testing over conventional testing. A number of rapid HIV tests have been approved for use by the US Food and Drug Administration (US FDA). Some of these have only been approved for use within the laboratory setting, whereas others can be administered outside laboratories by persons without formal laboratory training. Abstract:Homozygous Apolipoprotein L1 (APOL1) variants G1 and G2 cause APOL1-mediated kidney disease, purportedly acting as surface cation channels in podocytes. APOL1-G0 exhibits various single nucleotide polymorphisms, most commonly haplotype E150K, M228I and R255K ("KIK"; the Reference Sequence is "EMR"), whereas variants G1 and G2 are mostly found in a single "African" haplotype background ("EIK"). Several labs reported cytotoxicity with risk variants G1 and G2 in KIK or EIK background haplotypes, but used HEK-293 cells and did not verify equal surface expression. To see if haplotype matters in a more relevant cell type, we induced APOL1-G0, G1 and G2 EIK, KIK and EMR at comparable surface levels in immortalized podocytes. G1 and G2 risk variants (but not G0) caused dose-dependent podocyte death within 48h only in their native African EIK haplotype and correlated with K + conductance (thallium FLIPR). We ruled out differences in localization and trafficking, except for possibly greater surface clustering of cytotoxic haplotypes. APOL1 surface expression was required, since Brefeldin A rescued cytotoxicity; and cytoplasmic isoforms vB3 and vC were not cytotoxic. Thus, APOL1-EIK risk variants kill podocytes in a dose and haplotype-dependent manner (as in HEK-293 cells), whereas unlike in HEK-293 cells the KIK risk variants did not.

Effects of Testing and Disclosing Ancestry-Specific Genetic Risk for Kidney Failure on Patients and Health Care Professionals: A Randomized Clinical Trial

Abstract:Importance: Risk variants in the apolipoprotein L1 (APOL1 [OMIM 603743]) gene on chromosome 22 are common in individuals of West African ancestry and confer increased risk of kidney failure for people with African ancestry and hypertension. Whether disclosing APOL1 genetic testing results to patients of African ancestry and their clinicians affects blood pressure, kidney disease screening, or patient behaviors is unknown. Objective: To determine the effects of testing and disclosing APOL1 genetic results to patients of African ancestry with hypertension and their clinicians. Design, setting, and participants: This pragmatic randomized clinical trial randomly assigned 2050 adults of African ancestry with hypertension and without existing chronic kidney disease in 2 US health care systems from November 1, 2014, through November 28, 2016; the final date of follow-up was January 16, 2018. Patients were randomly assigned to undergo immediate (intervention) or delayed (waiting list control group) APOL1 testing in a 7:1 ratio. Statistical analysis was performed from May 1, 2018, to July 31, 2020. Interventions: Patients randomly assigned to the intervention group received APOL1 genetic testing results from trained staff; their clinicians received results through clinical decision support in electronic health records. Waiting list control patients received the results after their 12-month follow-up visit. Main outcomes and measures: Coprimary outcomes were the change in 3-month systolic blood pressure and 12-month urine kidney disease screening comparing intervention patients with high-risk APOL1 genotypes and those with low-risk APOL1 genotypes. Secondary outcomes compared these outcomes between intervention group patients with high-risk APOL1 genotypes and controls. Exploratory analyses included psychobehavioral factors. Results: Among 2050 randomly assigned patients (1360 women [66%]; mean [SD] age, 53 [10] years), the baseline mean (SD) systolic blood pressure was significantly higher in patients with high-risk APOL1 genotypes vs those with low-risk APOL1 genotypes and controls (137 [21] vs 134 [19] vs 133 [19] mm Hg; P = .003 for high-risk vs low-risk APOL1 genotypes; P = .001 for high-risk APOL1 genotypes vs controls). At 3 months, the mean (SD) change in systolic blood pressure was significantly greater in patients with high-risk APOL1 genotypes vs those with low-risk APOL1 genotypes (6 [18] vs 3 [18] mm Hg; P = .004) and controls (6 [18] vs 3 [19] mm Hg; P = .01). At 12 months, there was a 12% increase in urine kidney disease testing among patients with high-risk APOL1 genotypes (from 39 of 234 [17%] to 68 of 234 [29%]) vs a 6% increase among those with low-risk APOL1 genotypes (from 278 of 1561 [18%] to 377 of 1561 [24%]; P = .10) and a 7% increase among controls (from 33 of 255 [13%] to 50 of 255 [20%]; P = .01). In response to testing, patients with high-risk APOL1 genotypes reported more changes in lifestyle (a subjective measure that included better dietary and exercise habits; 129 of 218 [59%] vs 547 of 1468 [37%]; P < .001) and increased blood pressure medication use (21 of 218 [10%] vs 68 of 1468 [5%]; P = .005) vs those with low-risk APOL1 genotypes; 1631 of 1686 (97%) declared they would get tested again. Conclusions and relevance: In this randomized clinical trial, disclosing APOL1 genetic testing results to patients of African ancestry with hypertension and their clinicians was associated with a greater reduction in systolic blood pressure, increased kidney disease screening, and positive self-reported behavior changes in those with high-risk genotypes.
