Abstract:Abstract Background and Aims Early diagnosing Chronic Kidney Disease (CKD) is of major importance in daily practice, and the automatic reporting of the estimated glomerular filtration rate (eGFR) for all plasma creatinine (Pcr) determinations is mandatory. The CKD-EPI 2009 formula [1] is considered the most accurate creatinine-based GFR estimating formula in adults in Europe and is currently utilized by laboratories. However, CKD-EPI equation is inadequate in young adults < 25 years and CKD-EPI equation is debated in the elderly over 75 years, with the BIS1 equation [2] showing better performance in this age group. The creatinine-based European Kidney Function Consortium (EKFC) equation [3] provides continuous GFR estimation from 2 years to adulthood and the elderly, presenting itself as a potential alternative to both the CKD-EPI and BIS1 equations. Our objective was to assess the impact of using the EKFC equation (and/or BIS1 in the elderly) compared to CKD-EPI in daily practice in identifying patients with a GFR < 60 or < 30 ml/min/1.73 m2, which represents the threshold for referring to a nephrologist and adjusting drug dosage, respectively. Method We retrieved data from the laboratory database, including gender, age, and the lowest measurement of Pcr for all patients over 18 years who underwent Pcr determination in November 2023. eGFR was calculated for all patients using the CKD-EPI and the EKFC equation. For patients over 70 years, eGFR was also estimated with the BIS1 equation. Pcr was assessed using an enzymatic method traceable to IDMS (Architect c®, Abbott Diagnostics). Results were expressed as the mean bias between EKFC and BIS1, and CKD-EPI (considered as the reference) according to age groups, and as the number of patients misclassified as < 60 or < 30 ml/min/1.73 m2. Results 10 066 patients (50.5% females, mean age 56.0 ± 21.2 years [18.0-106.7]) underwent Pcr determination (mean 84.7 ± 72.3 μmol/L) with 1 706 (16.9%) and 405 (4.0%) having a GFR < 60 or < 30 ml/min/1.73 m2, respectively. EKFC and BIS underestimated CKD-EPI eGFR in all groups, but especially in the groups of young adults and the elderly. However, the number of misclassified patients is small, except in the elderly when CKD-EPI is considered less accurate. Conclusion The use of EKFC in laboratories to estimate eGFR offers continuous GFR estimation from 2 years to adulthood and elderly and could be an alternative to both CKD-EPI and BIS1. The automatic reporting of eGFR with EKFC by laboratories in Europe could allow for an earlier diagnosis of CKD and initiation of nephroprotection in young adults, in addition to providing better nephrological care for the elderly.

Abstract:The European Kidney Function Consortium (EKFC) developed two novel equations in 2023 for estimating glomerular filtration rate (GFR): one sex-free cystatin C-based equation (EKFC Cys ) and one creatinine-cystatin C combined equation (EKFC Cr-Cys ). This study compared their performance with the previous creatinine-based EKFC equation (EKFC Cr ) and commonly used Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) and Berlin Initiative Study (BIS) equations in Chinese adults. A total of 2,438 Chinese adults (mean age=53.04 years) who underwent the 99mTc-DTPA renal dynamic imaging for reference GFR (rGFR) were included. Diagnostic value was evaluated using correlation coefficients, sensitivity, specificity, and area under the receiver operating characteristic curve (ROC AUC ). Performance was assessed in terms of bias, precision (interquartile range of the median difference [IQR]), accuracy (percentage of estimates ±30 % of rGFR [P30], and root-mean-square error [RMSE]) across age, sex, and rGFR subgroups. Gender differences in bias and P30 were also analyzed. Average rGFR was 73.37 mL/min/1.73 m 2 . EKFC equations showed stronger correlations and larger AUCs compared to the parallel CKD-EPI equations, with EKFC Cr-Cys demonstrating the greatest improvement (R=0.771, ROC AUC =0.913). Concerning bias, precision, and accuracy, EKFC equations consistently outperformed CKD-EPI equations. EKFC Cr-Cys and EKFC Cr performed acceptably well in the entire population and were equivalent to BIS equations in the elderly. All equations, including EKFC Cys , showed similar P30 accuracy across sexes. EKFC equations provided a reasonable alternative for estimating GFR in the Chinese adult population. While EKFC Cys did not outperform EKFC Cr , EKFC Cr-Cys improved the accuracy of single-marker equations.

Comparisons between the 2012 New CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) Equations and Other Four Approved Equations

Abstract:OBJECTIVE: The Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) reported two equations in 2012: one based on cystatin C concentration (CKD-EPI2012cys) and the other using both serum creatinine and cystatin C concentrations (CKD-EPI2012Scr-cys). We compared the adaptability of new formulae with other four equations.METHODS: Participants (n = 788; median age, 54 [range, 19-94] years) were recruited from the First Affiliated Hospital of Nanjing Medical University. The reference glomerular filtration rate (rGFR) was measured by a (99)mTc-DTPA renal dynamic imaging method, and the estimated glomerular filtration rate (eGFR) was calculated separately by the Chinese adapted Modification of Diet in Renal Disease equation (C-MDRD), MacIsaac, Ma, serum creatinine-based CKD-EPI equation (CKD-EPI2009Scr), CKD-EPI2012cys and CKD-EPI2012Scr-cys equations. We compared the performance of six equations with rGFR.RESULTS: Median rGFR was 76.35 (interquartile range, 59.03-92.50) mL/min/1.73 m(2). Compared with CKD-EPI2009Scr, CKD-EPI2012Scr-cys formula had better diagnostic value with larger area under the receiver operating characteristic curve (ROC(AUC), 0.879, p = 0.006), especially in young participants (ROC(AUC), 0.883, p = 0.005). CKD-EPI2012cys equation did not perform better than other available equations. Accuracy (the proportion of eGFR within 30% of rGFR [P30]) of the CKD-EPI2012Scr-cys equation (77.03%) was inferior only to MacIsaac equation (80.2%) in the entire participants, but performed best in young participants with normal or mildly-injured GFR. Neither of the two new CKD-EPI equations achieved any ideal P30 in the elderly participants with moderately-severely injured GFR. Linear regression analysis demonstrated a consistent result. In this study, CKD-EPI2012Scr-cys had a relatively better diagnosis consistency of GFR stage between the eGFR and rGFR in the whole cohort.CONCLUSION: CKD-EPI2012Scr-cys appeared less biased and more accurate in overall participants. Neither of the new CKD-EPI equations achieved ideal accuracy in senior participants with moderately-severely injured GFR. A large-scale study with more subjects and cooperating centers to develop new formulae for the elderly is assumed to be necessary.

Abstract:Background: Recent debate on the race correction factor in creatinine-based estimated glomerular filtration rate (eGFR) has led to the development of a new race-independent equation (Chronic Kidney Disease Epidemiology Collaboration, CKD-EPI_2021). Previously, some institutions have already modified the early version of the CKD-EPI or Modification of Diet in Renal Disease (MDRD) equations by removing the race factors (CKD-EPI_2009_non-Black (NB), MDRD_NB) for Black populations although this approach remains controversial. Methods: In this study, the CKD-EPI_2009_NB, MDRD_NB, and European Kidney Function Consortium (EKFC) equations were compared directly with the CKD-EPI_2021 equation in eGFR calculation, chronic kidney disease (CKD) diagnosis, and staging in a local population. Results: These 3 previous methods underestimated eGFR compared to CKD-EPI_2021 for eGFR < 90 mL/min/1.73 m2 but overestimated eGFR at the high end (>120 mL/min/1.73 m2). Around the CKD diagnosis cutoff (60 mL/min/1.73 m2), both MDRD_NB and EFKC equations resulted in an increase in CKD cases compared to CKD-EPI_2021. CKD-EPI_2009_NB demonstrated a similar trend although the difference was not statistically significant. In a population with low eGFR (<60 mL/min/1.73 m2), the EKFC equation showed a CKD staging pattern significantly different from that by CKD-EPI_2021, but all 3 previous methods resulted in a similar number of end-stage renal failure cases. In general, the EKFC equation demonstrated a weaker agreement in eGFR calculation and concordance in classification with the CKD-EPI_2021 equation than MDRD_NB and CKD-EPI_2009_NB. Conclusions: Our study provides a direct visual comparison to demonstrate the potential clinical impact between 3 previously used race-independent methods and the CKD-EPI_2021 equation and aids the communication with healthcare providers during the implementation of this new equation.

Abstract:Heart chamber and pulmonary dilution curves were used to diagnose intracardiac and extracardiac shunts in 46 patients. Pulmonary dilution curves, as measured by C2∕C1 ratios, could separate normal patients from those with intracardiac or extracardiac shunts, but patients with right-to-left and left-to-right shunts had similar C2∕C1 values. Heart chamber dilution curves not only diagnosed left-to-right, right-to-left, and bidirectional intracardiac shunts but located the shunt at the atrial or ventricular level. Among 33 patients with shunts, the condition was diagnosed by heart curves in 19 and by pulmonary curves in 25.
Collaboration equation for estimating glomerular fi ltration rate in the Chinese population

Abstract:Background. Previous studies have indicated that the performance of glomerular filtration rate (GFR) estimation equations vary according to the races of the target population. The Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation has not been validated in the Chinese population including patients with chronic kidney disease (CKD) and healthy controls. Methods. A total of 977 adult persons (682 patients with CKD and 295 healthy volunteers) from nine renal institutes of university hospitals located in nine geographic regions of China were enrolled in the study. A diagnostic test study comparing the CKD-EPI two-level and four-level race equation, the Modification of Diet in Renal Disease (MDRD) Study equation and the modified MDRD equation for Chinese (the Chinese equation). The Tcdiethylenetriamine pentaacetic acid dual plasma clearance was used as a reference method for measuring GFR. Results. The mean age of participants was 48.3 ± 16.0 years and 479 (49.0%) were male. The CKD-EPI two-level race equation and the Chinese equation performed better than the MDRD Study equation and CKD-EPI four-level race equation, with less bias (median difference between estimated GFR and reference GFR, 0.2 and 0.3 versus −2.4 and 3.0 mL/ min/1.73 m), improved precision (interquartile range of the difference, 20.5 and 20.8 versus 23.4 and 20.5 mL/min/1.73 m) and greater accuracy (percentage of estimated GFR within 30% of reference GFR, 73.4 and 73.0% versus 69.8 and 70.1%). Conclusions. The CKD-EPI two-level race equation and the Chinese equation performed similarly in the Chinese population, and both performed better than the MDRD Study equation and the CKD-EPI four-level race equation.

Abstract:The EFLM recommends not to implement the race-free Chronic Kidney Disease Epidemiology Consortium (CKD-EPI) equation in European laboratories and to keep the 2009 version of the CKD-EPI equation, without applying a race correction factor. This recommendation is completely in line with a recent Editorial published by the European Renal Association who has also proposed to change to a novel equation only when it has considerably better performance, trying to reach global consensus before implementing such a new glomerular filtration rate (GFR) estimation equation. In Europe, this equation could be for instance the new European Kidney Function Consortium (EKFC) equation, which is population-specific, developed from European cohorts and accurate from infants to the older old. Beyond serum creatinine, the estimating equations based on cystatin C will probably gain in popularity, especially because cystatin C seems independent of race. Finally, we must keep in mind that all GFR equations remain an estimation of GFR, especially rough at the individual level. Measuring GFR with a reference method, such as iohexol clearance, remains indicated in specific patients and/or specific situations, and here also, the role of the clinical laboratories is central and should still evolve positively in the future.

Abstract:An equation from the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) provides more accurate estimates of the glomerular filtration rate (eGFR) than that from the modification of diet in renal disease (MDRD) Study, although both include a two-level variable for race (Black and White and other). Since creatinine generation differs among ethnic groups, it is possible that a multilevel ethnic variable would allow more accurate estimates across all groups. To evaluate this, we developed an equation to calculate eGFR that includes a four-level race variable (Black, Asian, Native American and Hispanic, and White and other) using a database of 8254 patients pooled from 10 studies. This equation was then validated in 4014 patients using 17 additional studies from the United States and Europe (validation database), and in 1022 patients from China (675), Japan (248), and South Africa (99). Coefficients for the Black, Asian, and Native American and Hispanic groups resulted in 15, 5, and 1% higher levels of eGFR, respectively, compared with the White and other group. In the validation database, the two-level race equation had minimal bias in Black, Native American and Hispanic, and White and other cohorts. The four-level ethnicity equation significantly improved bias in Asians of the validation data set and in Chinese. Both equations had a large bias in Japanese and South African patients. Thus, heterogeneity in performance among the ethnic and geographic groups precludes use of the four-level race equation. The CKD-EPI two-level race equation can be used in the United States and Europe across a wide range of ethnicity. © 2011 International Society of Nephrology.
Impact of switching from the 2009 to the 2021 CKD-EPI Equation for eGFR

Abstract:Abstract Recent national and international guidelines have recommended implementing the 2021 CKD-EPI equation for estimating glomerular filtration rate (eGFR). Relative to the 2009 CKD-EPI equation, the 2021 equation eliminates race-based modifiers in the calculation of eGFR based on a remodeling of the original CKD-EPI dataset. Further, updated guidelines now recommend integrating Cystatin C as a confirmatory test in all patients with an eGFR between 45-59 mL/min/1.73m2. Here we assess the impact of switching from the 2009 CKD-EPI creatinine-based equation to the 2021 CKD-EPI creatinine-based equation on both non-black and black populations and assess the feasibility of implementing a reflex for Cystatin C on all patients with an eGFR between 45-59 mL/min/1.73m2. The laboratory information system was queried retrospectively from 11/01/2021 to 12/31/2021 for age, sex, self-identified ethnicity, and serum creatinine concentrations from basic/comprehensive metabolic panels and renal panels. Slope and linearity of the reported eGFR with the 2009 and 2021 equations were compared using Deming regression and Fisher’s exact test was used to assess the proportion of eGFR results <30 ml/min/1.73m2 and between 45-59 ml/min/1.73m2. Of the 64,657 serum creatinine values, 37,589 were from self-identifying non-black patients and 27,068 were from self-identifying black patients. Deming regression revealed a slope of 0.997 (95% CI; 0.997-0.998) and intercept of 2.69 (2.65-2.74) for the non-black population and a slope of 0.863 (0.862-0.864) and intercept of 2.451 (2.39-2.51) for the black population. The number of non-black individuals with an eGFR <30 mL/min/1.73m2 decreased 5622 to 5060 (p<0.001) and black patients increased from 4018 to 4387 (p<0.001). The number of non-black individuals with an eGFR in the range of 45-59 ml/min/1.73m2 increased marginally from 4,410 (11.7%) to 4,525(12.0%, p=0.19) and the number of black individuals increased from 2,501 (9.2%) to 3,299 (12.2%, p<0.001). If a Cystatin C reflex test was performed on all patients with an eGFR between 45-59 mL/min/1.73m2, 2325 assays would have been performed on out-patients and 5499 on in-patients. These results, in the context of recently updated guidelines imply that the changing from the 2009 CKD-EPI equation to the 2021 CKD-EPI equation is likely to increase the proportion of black individuals diagnosed with moderate to severe and severe loss of kidney function more so than the non-black populations. Implementation of a Cystatin C reflex on all patients with an eGFR between 45-59 mL/min/1.73m2 is likely to lead to considerable overuse and should be limited to out-patients and physician ordered tests.

Abstract:Aim: Our previous study demonstrated that the cystatin C-based chronic kidney disease (CKD)-EPI equation and combined by serum creatinine (CKD-EPIscr-cys) had better capability to accurately evaluate glomerular filtration rate in the CKD participants. Considering that the accuracy of estimated glomerular filtration rate (eGFR) remains less ideally, it is essential to modify the equation by including the Chinese eGFR racial factor in order to improve its performance. Methods: Two prospective cohorts were enrolled in 2 medical centers. New equations were developed in 529 participants and validated in 313 participants. Reference glomerular filtration rate (rGFR) was taken by 99mTc-DTPA renal dynamic imaging method (Gates method). The primary outcomes of this study were bias, precision (interquartile range of difference [IQR]), and accuracy (the proportion of eGFR within 30% of rGFR [P30] and root mean square error [RMSE]) of eGFR versus rGFR. Results: In a development data set, Chinese coefficients for CKD-EPIscr (C-CKD-EPIscr), CKD-EPIcys (C-CKD-EPIcys), and CKD-EPIscr-cys (C-CKD-EPIscr-cys) were 0.871, 0.879, and 0.891, respectively. In a validation data set, C-CKD-EPIcys was the most accurate with highest P30 value (62.3%), relative lowest IQR (15.45), and RMSE (0.80) among 6 equations, though the bias of C-CKD-EPIcys was not better than CKD-EPIcys. C-CKD-EPIscr and C-CKD-EPIscr-cys equations were improved in bias (p < 0.001), precision, and accuracy (p = 0.004 and <0.001 for P30) compared with CKD-EPIscr and CKD-EPIscr-cys. Conclusion: C-CKD-EPIcys was the most accurate with the highest P30 value, relative lowest IQR, and RMSE among 6 equations. C-CKD-EPIscr and C-CKD-EPIscr-cys equations were improved in bias, precision, and accuracy. Other external validation of these equations is needed.

Abstract:Five equations are commonly used by biopharma companies to estimate glomerular filtration rate (GFR) or creatinine clearance (CrCl) based on regulatory and stakeholder recommendations. Changes observed in GFR and/or CrCl can be challenging to interpret as participants transition between age strata, prompting changes in equations. This manuscript explores how GFR, CrCl, and the resulting Kidney Disease Improving Global Outcomes (KDIGO) categories of kidney function can change when different equations are used. We analyzed descriptively real‐world data (RWD) from 48 health care organizations in the United States to identify patients with data (age, sex, height, weight, serum biomarkers such as creatinine [Scr] and/or cystatin C [Scys]) sufficient to estimate GFR and/or CrCl. Five equations (Bedside Schwartz, Modified Schwartz, Cockcroft‐Gault, 2021 Chronic Kidney Disease‐Epidemiology Collaboration [CKD‐EPI] Scr, and 2021 CKD‐EPI Scr/Scys) were used to estimate GFR and/or CrCl and assign individuals to KDIGO categories (G1–G5). We identified 32,728 patients, including 378 infants (<2 years), 1191 younger children (2 to <6 years), 1891 older children (6 to <12 years), 3174 adolescents (12 to <18 years), 16,367 younger adults (18 to <65 years), and 9727 older adults (65+ years) with sufficient information to estimate kidney function. For patients greater than or equal to 2 years, estimated GFR and/or CrCl and KDIGO categories based on the 2021 CKD‐EPI Scr/Scys equation appeared similar to those based on currently recommended equations. RWD suggests the 2021 CKD‐EPI Scr/Scys equation could potentially be used to estimate GFR in individuals aged 2 years and older; more research is needed to confirm these findings.

Abstract:Abstract Introduction: The guidelines recommend estimating the glomerular filtration rate using serum creatinine-based equations as a predictor of kidney disease, preferably adjusted for local population groups. Methods: Cross-sectional study that evaluated the performance of four equations used for estimating GFR compared to endogenous creatinine clearance (ClCr) in 1,281 participants. Modification of Diet equations in Renal Disease Study Group (MDRD), Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI), CKD-EPI with adjustment for local population (CKD-EPI local) and Full Age Spectrum (FAS) in comparison with endogenous creatinine clearance (ClCr). We used the Quantile Regression to calculate the median bias, interquartile range (IQR), Bland-Altman agreement analysis and 30% margin of error (P30). Results: The mean age of participants was 52.5 ± 16.5 years with 466 women (38%), median ClCr[IQR] of 92.0 [58.0; 122.0] mL/min/1.73 m2, with 320 (25%) participants presenting ClCr < 60 mL/min/1.73 m2. The performance of the local CKD-EPI and FAS equations were superior to MDRD and CKD-EPI in relation to variability (0.92 [0.89; 0.94]) and P30 (90.5% [88.7; 92, 0]). In the group with ClCr < 60 mL/min/1.73 m2, the local CKD-EPI and FAS equations showed less variability than the CKD-EPI and MDRD (0.90 [0.86; 0.98] and 1.05 [0.97; 1.09] vs. 0.63 [0.61; 0.68] and 0.65 [0.62; 0.70], P < 0.01) and best P30 (85.5) % [81.0; 90.0], 88.0% [84.0; 92.0] vs. 52.0% (46.0; 58.0) and 53.0% [47.0; 58 .5], P < 0.01). Conclusion: Local CKD-EPI and FAS equations performed better than CKD-EPI and MDRD when compared to ClCr.

Abstract:Abstract Background and hypothesis The estimation of glomerular filtration rate (GFR) is one main tool to detect renal disease. The most used biomarker remains serum creatinine and the European Kidney Function Consortium (EKFCcrea) equation is the most validated in Europe. More recently, cystatin C, has been proposed. We studied the performances of the EKFC equations in a large cohort of subjects according to their diabetic status. Methods Four cohorts from the EKFC dataset were retrospectively considered in which the diabetic status was available. GFR was measured by plasma clearances (mGFR) (iohexol or 51Cr-EDTA). The performance of the equations was assessed by calculating bias, precision (IQR) and P30 (percentage of eGFR-values within ± 30% of mGFR). Results In the whole population (n = 6 158), median [IQR] age was 61 [47;72] years, with 45.8% women. Mean mGFR was 60 [39;82] mL/min/1.73m2. Compared to non-diabetic individuals (n = 5 124), diabetic patients (n = 1 034) were older, more frequently male, heavier, and had lower mGFR. The performance of the EKFCcys equation was similar to EKFCcrea, but the EKFCcrea+cys had better P30 than the single-biomarker equations. P30 values were substantially lower in diabetic patients than in non-diabetic but, according to a matched analysis, this is mainly explained by the difference in GFR levels between the two populations, not by diabetic status. Conclusion We showed that equation combining creatinine and cystatin C present a better performance. If accuracy of equations seems better in non-diabetic than in diabetic individuals, it is more due to differences in GFR levels than to the diabetic status.

Abstract:Background: Equations based on serum creatinine (SCr) have been extensively applied to estimate glomerular filtration rate (GFR), but their performance is debatable. In 2021, the European Kidney Function Consortium (EKFC) published one novel SCr-based formula, which combined the feature of Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) and full age spectrum (FAS) equations, but its potential applications remain unknown. We seek to assess the appropriateness of the three equations in Chinese adults. Methods: A total of 3,692 participants (median age, 54 years) were included. Reference GFR (rGFR) was measured by the 99mTc-DTPA renal dynamic imaging method. Estimated GFR (eGFR) was calculated by the CKD-EPI, FAS, and EKFC equations. Correlation coefficients and Bland-Altman analysis were adopted to evaluate their validity. The performance was assessed in subgroups according to age, sex, rGFR, and SCr, considering the bias, accuracy, and precision. Results: The average rGFR was 74.2 mL/min/1.73 m2. eGFR by EKFC showed a relatively stronger correlation with rGFR (R = 0.749) and a larger area under the receiver operating characteristic curve (0.902). EKFC was significantly less biased and exhibited the highest P30 in the entire population (bias = 3.61, P30 = 73.3%). It also performed well in all analyzed subgroups, especially in participants with normal or slightly impaired renal function (rGFR≥60 mL/min/1.73 m2), and low SCr. Conclusions: Compared to the other two SCr-based formulas, EKFC performed better in the Chinese. Thus, it might serve as a good alternative, until a more suitable formula is developed for the Chinese population.
Validation of the EKFC equation for glomerular filtration rate estimation and comparison with the Asian-modified CKD-EPI equation in Chinese chronic kidney disease patients in an external study

Abstract:The aim of this study is to determine whether new European Kidney Function Consortium (EKFC) equation is more applicable than Asian-modified CKD-EPI equation in clinical practice, having a higher accuracy in estimating GFR in our external CKD population. We calculated estimated GFR EKFC and GFR CKD-EPI independently using the EKFC and Asian-modified CKD-EPI formulas, respectively. The clinical diagnostic performance of the two equations was assessed and compared by median bias, precision, accuracy ( P 30 ) and so on, using 99m Tc-DTPA dual plasma sample clearance method as a reference method for GFR measurement (mGFR). The equation that met the following targets was superior: (1) median bias within ± 3 mL/min/1.73 m 2 ; (2) P 30 > 75%; and (3) better precision and 95% limits of agreement in Bland–Altman analysis. Totally, 160 CKD patients were recruited in our external cohort. GFR EKFC was highly related to mGFR, with a regression equation of GFR EKFC =mGFR × 0.87 + 5.27. Compared with the Asian-modified CKD-EPI equation, EKFC equation demonstrated a wider median bias (–1.64 vs. 0.84 mL/min/1.73 m 2 , p < 0.01) that was within 3 mL/min/1.73 m 2 and not clinically meaningful. Furthermore, the precision (12.69 vs. 12.72 mL/min/1.73 m 2 , p = 0.42), 95% limits of agreement in Bland–Altman analysis (42.4 vs. 44.4 mL/min/1.73 m 2 ) and incorrect reclassification index of the two target equations were almost identical. Although, EKFC equation had a slightly better P 30 (80.0% vs. 74.4%, p = 0.01). The overall performance of EKFC equation is acceptable. There is no clinically meaningful difference in the performance of the Asian-modified CKD-EPI and EKFC equations within the limits imposed by the small sample size.

Abstract:Introduction: In 2021, CKD-EPI equation without the race coefficient (2021 CKD-EPI equation) was recommended for use in clinical practice by international guidelines. The performance of the 2021 equation in people with diabetes is unknown. This study aims to assess and compare the performance of the 2021 CKD-EPI equation to the 2009 CKD-EPI equation in individuals with type 1 and/or type 2 diabetes. Methods: Datapoints (n=16,182) were collected from people with type 1 and type 2 diabetes from six international cohorts (Diabetes Control and Complication Trial [DCCT, USA], Colorado [USA], Germany, Korea, Spain, and Australia), with mGFR assessed using exogenous filtration markers. The intra-class correlation coefficient between mGFR and eGFR for 2009 CKD-EPI equation and 2021 CKD-EPI equation was estimated using (a) a one-step intra-individual participant data meta-analysis approach, and (b) a four-level mixed-effect linear model which included studies and individual participants as first and second level random effects, method of measurement as third level fixed effect and time as fourth level random effect. Results: Overall performance of the 2009 CKD-EPI equation was excellent (ICC: 0.96, 95%CI: 0.88, 0.99) and 2021 CKD-EPI equations (ICC: 0.87, 95%CI: 0.77, 0.93) was good. However, subgroup analyses based on clinically relevant GFR ranges (<30, 30-59, 60-89 ml/min/1.73m2) revealed poor agreement between mGFR and eGFR measurements for the 2009 CKD-EPI equation (ICC: 0.41, 0.43, 0.40 respectively) and excellent agreement for GFR ≥90 ml/min/1.73m2 (ICC: 0.92). The 2021 CKD-EPI equation revealed poor agreement across GFR ranges <30, 30-59 and 60-89 (ICC: 0.38, 0.41, 0.39 respectively) and moderate agreement for GFR ≥90 ml/min/1.73m2 (ICC: 0.55). Conclusion: These findings suggest that the 2009 CKD-EPI equation still performs better than the 2021 CKD-EPI equation without the race co-efficient, especially in higher GFR ranges for people with diabetes. Abstract:•In this work, we evaluated the performance of Scr- and CysC-based equations for GFR estimation in Chinese renal transplant recipients. To the best of our knowledge, this is the first evaluation of the GFR estimation equations in renal transplant recipients with a large population.•Among all the equations included in our study, the Scr-based JSN-CKDI equation had the best estimation; the genetic and environmental similarities between Chinese and Japanese people may contribute to the current result.•Our results were similar with several previous studies. These results supported the use of CysC-based GFR equations in renal transplant recipients.•We observed that equations involving females generally had larger bias than those in males. Moreover, these differences were significant in the JSN-CKDI, Larsson, Rule, Hoek, and Filler equations. This finding may be attributed to the absence or inadequate gender calibration in these equations.•In our research, similar results were observed that nearly all the equations have increased net biases when the GFR value is >60 mL/min/1.73 m2, which indicates that GFR equations are not suited for renal transplant recipients whose GFR values are at high level.

Abstract:BackgroundThe aim of this study was to identify the optimal equation that accurately estimates the glomerular filtration rate (GFR) and the chronic kidney disease (CKD) stage in the Chinese population.MethodsA total of 1296 Chinese patients aged 18–65 years old were enrolled in this study. The estimated GFRs (eGFRs) calculated separately by three Diet in Renal Disease (MDRD) equations and three Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equations were compared with the reference GFR (rGFR) measured by the 99Tcm-DTPA renal dynamic imaging method.ResultsBy Bland-Altman analysis, eGFRcys and eGFRscr_cys performed similarly, showing the tightest limits of agreement among the six equations. They also achieved the first and second highest 30% and 50% accuracies. Using a combination of the serum creatinine and cystatin C levels (eGFRscr_cys) could improve the bias (−0.3 for eGFRscr_cys) of the equation and achieve the highest diagnostic accuracy for renal insufficiency (AUC60, 0.953; P < 0.05, except for eGFR_MDRD). All equations predicted stage 3 CKD with moderate accuracy (49.7–51.4%) and stage 5 CKD with good accuracy (90.2–96.4%). For stage 1 CKD, eGFRcys showed a higher percentage of misclassification than the other equations. All equations seemed to perform poorly at predicting stage 2 and 4 CKD, as compared to the other CKD stages. eGFRscr_cys was the best-performing equation in terms of accurate classification of the CKD stage based on the overall performance (kappa value, 0.423).ConclusionFor a Chinese population, the CKD-EPIscr_cys equation seems more suitable for estimating the GFR than the other equations. Each equation had its own advantages in predicting different CKD stages.

Abstract:ABSTRACT Background Novel creatinine-based equations have recently been proposed but their predictive performance for cardiovascular outcomes in participants at high cardiovascular risk in comparison to the established CKD-EPI 2009 equation is unknown. Method In 9361 participants from the United States included in the randomized controlled SPRINT trial, we calculated baseline estimated glomerular filtration rate (eGFR) using the CKD-EPI 2009, CKD-EPI 2021, and EKFC equations and compared their predictive value of cardiovascular events. The statistical metric used is the net reclassification improvement (NRI) presented separately for those with and those without events. Results During a mean follow-up of 3.1 ± 0.9 years, the primary endpoint occurred in 559 participants (6.0%). When using the CKD-EPI 2009, the CKD-EPI 2021, and the EKFC equations, the prevalence of CKD (eGFR <60 ml/min/1.73 m2 or >60 ml/min/1.73 m2 with an ACR ≥30 mg/g) was 37% vs. 35.3% (P = 0.02) vs. 46.4% (P < 0.001), respectively. The corresponding mean eGFR was 72.5 ± 20.1 ml/min/1.73 m2 vs. 73.2 ± 19.4 ml/min/1.73 m2 (P < 0.001) vs. 64.6 ± 17.4 ml/min/1.73 m2 (P < 0.001). Neither reclassification according to the CKD-EPI 2021 equation [CKD-EPI 2021 vs. CKD-EPI 2009: NRIevents: −9.5% (95% confidence interval (CI) −13.0% to −5.9%); NRInonevents: 4.8% (95% CI 3.9% to 5.7%)], nor reclassification according to the EKFC equation allowed better prediction of cardiovascular events compared to the CKD-EPI 2009 equation (EKFC vs. CKD-EPI 2009: NRIevents: 31.2% (95% CI 27.5% to 35.0%); NRInonevents: −31.1% (95% CI −32.1% to −30.1%)). Conclusion Substituting the CKD-EPI 2009 with the CKD-EPI 2021 or the EKFC equation for calculation of eGFR in participants with high cardiovascular risk without diabetes changed the prevalence of CKD but was not associated with improved risk prediction of cardiovascular events for both those with and without the event.
