Abstract:When to initiate treatment on patients is an important problem in many medical studies such as AIDS and cancer. In this article, we formulate the treatment initiation time problem for time-to-event data and propose an optimal individualized regime that determines the best treatment initiation time for individual patients based on their characteristics. Different from existing optimal treatment regimes where treatments are undertaken at a pre-specified time, here new challenges arise from the complicated missing mechanisms in treatment initiation time data and the continuous treatment rule in terms of initiation time. To tackle these challenges, we propose to use restricted mean residual lifetime as a value function to evaluate the performance of different treatment initiation regimes, and develop a nonparametric estimator for the value function, which is consistent even when treatment initiation times are not completely observable and their distribution is unknown. We also establish the asymptotic properties of the resulting estimator in the decision rule and its associated value function estimator. In particular, the asymptotic distribution of the estimated value function is nonstandard, which follows a weighted chi-squared distribution. The finite-sample performance of the proposed method is evaluated by simulation studies and is further illustrated with an application to a breast cancer data.This article is protected by copyright. On Estimation of Optimal Treatment Regimes For Maximizing t-Year Survival Probability

Abstract:A treatment regime is a deterministic function that dictates personalized treatment based on patients' individual prognostic information. There is a fast-growing interest in finding optimal treatment regimes to maximize expected long-term clinical outcomes of patients for complex diseases, such as cancer and AIDS. For many clinical studies with survival time as a primary endpoint, a main goal is to maximize patients' survival probabilities given treatments. In this article, we first propose two nonparametric estimators for survival function of patients following a given treatment regime. Then, we derive the estimation of the optimal treatment regime based on a value-based searching algorithm within a set of treatment regimes indexed by parameters. The asymptotic properties of the proposed estimators for survival probabilities under derived optimal treatment regimes are established under suitable regularity conditions. Simulations are conducted to evaluate the numerical performance of the proposed estimators under various scenarios. An application to an AIDS clinical trial data is also given to illustrate the methods.

Abstract:Individualized treatment rules (ITRs) recommend treatment according to patient characteristics. There is a growing interest in developing novel and efficient statistical methods in constructing ITRs. We propose an improved doubly robust estimator of the optimal ITRs. The proposed estimator is based on a direct optimization of an augmented inverse-probability weighted estimator of the expected clinical outcome over a class of ITRs. The method enjoys two key properties. First, it is doubly robust, meaning that the proposed estimator is consistent when either the propensity score or the outcome model is correct. Second, it achieves the smallest variance among the class of doubly robust estimators when the propensity score model is correctly specified, regardless of the specification of the outcome model. Simulation studies show that the estimated ITRs obtained from our method yield better results than those obtained from current popular methods. Abstract:Comparative effectiveness research often involves evaluating the differences in the risks of an event of interest between two or more treatments using observational data. Often, the post-treatment outcome of interest is whether the event happens within a pre-specified time window, which leads to a binary outcome. One source of bias for estimating the causal treatment effect is the presence of confounders, which are usually controlled using propensity score-based methods. An additional source of bias is right-censoring, which occurs when the information on the outcome of interest is not completely available due to dropout, study termination, or treatment switch before the event of interest. We propose an inverse probability weighted regression-based estimator that can simultaneously handle both confounding and right-censoring, calling the method CIPWR, with the letter C highlighting the censoring component. CIPWR estimates the average treatment effects by averaging the predicted outcomes obtained from a logistic regression model that is fitted using a weighted score function. The CIPWR estimator has a double robustness property such that estimation consistency can be achieved when either the model for the outcome or the models for both treatment and censoring are correctly specified. We establish the asymptotic properties of the CIPWR estimator for conducting inference, and compare its finite sample performance with that of several alternatives through simulation studies. The methods under comparison are applied to a cohort of prostate cancer patients from an insurance claims database for comparing the adverse effects of four candidate drugs for advanced stage prostate cancer.

Abstract:To promote precision medicine, individualized treatment regimes (ITRs) are crucial for optimizing the expected clinical outcome based on patient-specific characteristics. However, existing ITR research has primarily focused on scenarios with categorical treatment options and a single outcome. In reality, clinicians often encounter scenarios with continuous treatment options and multiple, potentially competing outcomes, such as medicine efficacy and unavoidable toxicity. To balance these outcomes, a proper weight is necessary, which should be learned in a data-driven manner that considers both patient preference and clinician expertise. In this paper, we present a novel algorithm for developing individualized treatment regimes (ITRs) that incorporate continuous treatment options and multiple outcomes, utilizing observational data. Our approach assumes that clinicians are optimizing individualized patient utilities with sub-optimal treatment decisions that are at least better than random assignment. Treatment assignment is assumed to directly depend on the true underlying utility of the treatment rather than patient characteristics. The proposed method simultaneously estimates the weighting of composite outcomes and the decision-making process, allowing for construction of individualized treatment regimes with continuous doses. The proposed estimators can be used for inference and variable selection, facilitating the identification of informative treatment assignments and preference-associated variables. We evaluate the finite sample performance of our proposed method via simulation studies and apply it to a real data application of radiation oncology analysis.

Abstract:Individualized treatment rules (ITR) can improve health outcomes by recognizing that patients may respond differently to treatment and assigning therapy with the most desirable predicted outcome for each individual. Flexible and efficient prediction models are desired as a basis for such ITRs to handle potentially complex interactions between patient factors and treatment. Modern Bayesian semiparametric and nonparametric regression models provide an attractive avenue in this regard as these allow natural posterior uncertainty quantification of patient specific treatment decisions as well as the population wide value of the prediction-based ITR. In addition, via the use of such models, inference is also available for the value of the Optimal ITR. We propose such an approach and implement it using Bayesian Additive Regression Trees (BART) as this model has been shown to perform well in fitting nonparametric regression functions to continuous and binary responses, even with many covariates. It is also computationally efficient for use in practice. With BART we investigate a treatment strategy which utilizes individualized predictions of patient outcomes from BART models. Posterior distributions of patient outcomes under each treatment are used to assign the treatment that maximizes the expected posterior utility. We also describe how to approximate such a treatment policy with a clinically interpretable ITR, and quantify its expected outcome. The proposed method performs very well in extensive simulation studies in comparison with several existing methods. We illustrate the usage of the proposed method to identify an individualized choice of conditioning regimen for patients undergoing hematopoietic cell transplantation and quantify the value of this method of choice in relation to the Optimal ITR as well as non-individualized treatment strategies.

Abstract:In survival contexts, substantial literature exists on estimating optimal treatment regimes, where treatments are assigned based on personal characteristics to maximize the survival probability. These methods assume that a set of covariates is sufficient to deconfound the treatment-outcome relationship. However, this assumption can be limited in observational studies or randomized trials in which non-adherence occurs. Therefore, we propose a novel approach to estimating optimal treatment regimes when certain confounders are unobservable and a binary instrumental variable is available. Specifically, via a binary instrumental variable, we propose a semiparametric estimator for optimal treatment regimes by maximizing a Kaplan–Meier-like estimator of the survival function. Furthermore, to increase resistance to model misspecification, we construct novel doubly robust estimators. Since the estimators of the survival function are jagged, we incorporate kernel smoothing methods to improve performance. Under appropriate regularity conditions, the asymptotic properties are rigorously established. Moreover, the finite sample performance is evaluated through simulation studies. Finally, we illustrate our method using data from the National Cancer Institute's prostate, lung, colorectal, and ovarian cancer screening trial.

Abstract:Individualized treatment recommendation (ITR) is an important analytic framework for precision medicine. The goal is to assign proper treatments to patients based on their individual characteristics. From the machine learning perspective, the solution to an ITR problem can be formulated as a weighted classification problem to maximize the average benefit that patients receive from the recommended treatments. Several methods have been proposed for ITR in both binary and multicategory treatment setups. In practice, one may prefer a more flexible recommendation with multiple treatment options. This motivates us to develop methods to obtain a set of near-optimal individualized treatment recommendations alternative to each other, called alternative individualized treatment recommendations (A-ITR). We propose two methods to estimate the optimal A-ITR within the outcome weighted learning (OWL) framework. We show the consistency of these methods and obtain an upper bound for the risk between the theoretically optimal recommendation and the estimated one. We also conduct simulation studies, and apply our methods to a real data set for Type 2 diabetic patients with injectable antidiabetic treatments. These numerical studies have shown the usefulness of the proposed A-ITR framework. Constructing optimal treatment length strategies to maximize quality-adjusted lifetimes

Abstract:Real-world clinical decision making is a complex process that involves balancing the risks and benefits of treatments. Quality-adjusted lifetime is a composite outcome that combines patient quantity and quality of life, making it an attractive outcome in clinical research. We propose methods for constructing optimal treatment length strategies to maximize this outcome. Existing methods for estimating optimal treatment strategies for survival outcomes cannot be applied to a quality-adjusted lifetime due to induced informative censoring. We propose a weighted estimating equation that adjusts for both confounding and informative censoring. We also propose a nonparametric estimator of the mean counterfactual quality-adjusted lifetime survival curve under a given treatment length strategy, where the weights are estimated using an undersmoothed sieve-based estimator. We show that the estimator is asymptotically linear and provide a data-dependent undersmoothing criterion. We apply our method to obtain the optimal time for percutaneous endoscopic gastrostomy insertion in patients with amyotrophic lateral sclerosis.

Abstract:SUMMARY Micro-randomized trials are commonly conducted for optimizing mobile health interventions such as push notifications for behavior change. In analyzing such trials, causal excursion effects are often of primary interest, and their estimation typically involves inverse probability weighting (IPW). However, in a micro-randomized trial, additional treatments can often occur during the time window over which an outcome is defined, and this can greatly inflate the variance of the causal effect estimator because IPW would involve a product of numerous weights. To reduce variance and improve estimation efficiency, we propose two new estimators using a modified version of IPW, which we call “per-decision IPW.” The second estimator further improves efficiency using the projection idea from the semiparametric efficiency theory. These estimators are applicable when the outcome is binary and can be expressed as the maximum of a series of sub-outcomes defined over sub-intervals of time. We establish the estimators’ consistency and asymptotic normality. Through simulation studies and real data applications, we demonstrate substantial efficiency improvement of the proposed estimator over existing estimators. The new estimators can be used to improve the precision of primary and secondary analyses for micro-randomized trials with binary outcomes.

Abstract:Precision medicine is an emerging scientific topic for disease treatment and prevention that takes into account individual patient characteristics. It is an important direction for clinical research, and many statistical methods have been proposed recently. One of the primary goals of precision medicine is to obtain an optimal individual treatment rule (ITR), which can help make decisions on treatment selection according to each patient's specific characteristics. Recently, outcome weighted learning (OWL) has been proposed to estimate such an optimal ITR in a binary treatment setting by maximizing the expected clinical outcome. However, for ordinal treatment settings, such as individualized dose finding, it is unclear how to use OWL. In this article, we propose a new technique for estimating ITR with ordinal treatments. In particular, we propose a data duplication technique with a piecewise convex loss function. We establish Fisher consistency for the resulting estimated ITR under certain conditions, and obtain the convergence and risk bound properties. Simulated examples and an application to a dataset from a type 2 diabetes mellitus observational study demonstrate the highly competitive performance of the proposed method compared to existing alternatives.

Abstract:In this article, we develop nonparametric inference methods for comparing survival data across two samples, which are beneficial for clinical trials of novel cancer therapies where long-term survival is a critical outcome. These therapies, including immunotherapies or other advanced treatments, aim to establish durable effects. They often exhibit distinct survival patterns such as crossing or delayed separation and potentially leveling-off at the tails of survival curves, clearly violating the proportional hazards assumption and rendering the hazard ratio inappropriate for measuring treatment effects. The proposed methodology utilizes the mixture cure framework to separately analyze the cure rates of long-term survivors and the survival functions of susceptible individuals. We evaluate a nonparametric estimator for the susceptible survival function in the one-sample setting. Under sufficient follow-up, it is expressed as a location-scale-shift variant of the Kaplan-Meier (KM) estimator. It retains several desirable features of the KM estimator, including inverse-probability-censoring weighting, product-limit estimation, self-consistency, and nonparametric efficiency. In scenarios of insufficient follow-up, it can easily be adapted by incorporating a suitable cure rate estimator. In the two-sample setting, besides using the difference in cure rates to measure the long-term effect, we propose a graphical estimand to compare the relative treatment effects on susceptible subgroups. This process, inspired by Kendall's tau, compares the order of survival times among susceptible individuals. The proposed methods' large-sample properties are derived for further inference, and the finite-sample properties are examined through extensive simulation studies. The proposed methodology is applied to analyze the digitized data from the CheckMate 067 immunotherapy clinical trial.

Abstract:In many important applications of precision medicine, the outcome of interest is time to an event (e.g., death, relapse of disease) and the primary goal is to identify the optimal individualized decision rule (IDR) to prolong survival time. Existing work in this area have been mostly focused on estimating the optimal IDR to maximize the restricted mean survival time in the population. We propose a new robust framework for estimating an optimal static or dynamic IDR with time-to-event outcomes based on an easy-to-interpret quantile criterion. The new method does not need to specify an outcome regression model and is robust for heavy-tailed distribution. The estimation problem corresponds to a nonregular M-estimation problem with both finite and infinite-dimensional nuisance parameters. Employing advanced empirical process techniques, we establish the statistical theory of the estimated parameter indexing the optimal IDR. Furthermore, we prove a novel result that the proposed approach can consistently estimate the optimal value function under mild conditions even when the optimal IDR is nonunique, which happens in the challenging setting of exceptional laws. We also propose a smoothed resampling procedure for inference. The proposed methods are implemented in the R-package QTOCen. We demonstrate the performance of the proposed new methods via extensive Monte Carlo studies and a real data application. Supplementary materials for this article are available online.

Abstract:An optimal dynamic treatment regime (DTR) consists of a sequence of decision rules in maximizing long-term benefits, which is applicable for chronic diseases such as HIV infection or cancer. In this article, we develop a novel angle-based approach to search the optimal DTR under a multicategory treatment framework for survival data. The proposed method targets to maximize the conditional survival function of patients following a DTR. In contrast to most existing approaches which are designed to maximize the expected survival time under a binary treatment framework, the proposed method solves the multicategory treatment problem given multiple stages for censored data. Specifically, the proposed method obtains the optimal DTR via integrating estimations of decision rules at multiple stages into a single multicategory classification algorithm without imposing additional constraints, which is also more computationally efficient and robust. In theory, we establish Fisher consistency and provide the risk bound for the proposed estimator under regularity conditions. Our numerical studies show that the proposed method outperforms competing methods in terms of maximizing the conditional survival probability. Abstract:We propose a reinforcement learning method for estimating an optimal dynamic treatment regime for survival outcomes with dependent censoring. The estimator allows the failure time to be conditionally independent of censoring and dependent on the treatment decision times, supports a flexible number of treatment arms and treatment stages, and can maximize either the mean survival time or the survival probability at a certain time point. The estimator is constructed using generalized random survival forests and can have polynomial rates of convergence. Simulations and data analysis results suggest that the new estimator brings higher expected outcomes than existing methods in various settings. An R package dtrSurv is available on CRAN.

Abstract:Marginal structural models (MSMs) with inverse probability weighting offer an approach to estimating causal effects of treatment sequences on repeated outcome measures in the presence of time-varying confounding and dependent censoring. However, when weights are estimated by maximum likelihood, inverse probability weighted estimators (IPWEs) can be inefficient and unstable in practice. We propose a joint calibration approach for inverse probability of treatment and censoring weights to improve the efficiency and robustness of the IPWEs for MSMs with time-varying treatments of arbitrary (i.e., binary and non-binary) distributions. Specifically, novel calibration restrictions are derived by explicitly eliminating covariate associations with both the treatment assignment process and the censoring process after weighting the current sample (i.e., to optimise covariate balance in finite samples). A convex minimization procedure is developed to implement the calibration. Simulations show that IPWEs with calibrated weights perform better than IPWEs with weights from maximum likelihood. We apply our method to a natural history study of HIV for estimating the cumulative effect of highly active antiretroviral therapy on CD4 cell counts over time.

Abstract:This paper introduces a novel approach to estimating censored quantile regression using inverse probability of censoring weighted (IPCW) methodology, specifically tailored for data sets featuring partially interval‐censored data. Such data sets, often encountered in HIV/AIDS and cancer biomedical research, may include doubly censored (DC) and partly interval‐censored (PIC) endpoints. DC responses involve either left‐censoring or right‐censoring alongside some exact failure time observations, while PIC responses are subject to interval‐censoring. Despite the existence of complex estimating techniques for interval‐censored quantile regression, we propose a simple and intuitive IPCW‐based method, easily implementable by assigning suitable inverse‐probability weights to subjects with exact failure time observations. The resulting estimator exhibits asymptotic properties, such as uniform consistency and weak convergence, and we explore an augmented‐IPCW (AIPCW) approach to enhance efficiency. In addition, our method can be adapted for multivariate partially interval‐censored data. Simulation studies demonstrate the new procedure's strong finite‐sample performance. We illustrate the practical application of our approach through an analysis of progression‐free survival endpoints in a phase III clinical trial focusing on metastatic colorectal cancer.

Identification and estimation of treatment effects on long-term outcomes in clinical trials with external observational data

Abstract:In biomedical studies, estimating drug effects on chronic diseases requires a long follow-up period, which is difficult to meet in randomized clinical trials (RCTs). The use of a short-term surrogate to replace the long-term outcome for assessing the drug effect relies on stringent assumptions that empirical studies often fail to satisfy. Motivated by a kidney disease study, we investigate the drug effects on long-term outcomes by combining an RCT without observation of long-term outcome and an observational study in which the long-term outcome is observed but unmeasured confounding may exist. Under a mean exchangeability assumption weaker than the previous literature, we identify the average treatment effects in the RCT and derive the associated efficient influence function and semiparametric efficiency bound. Furthermore, we propose a locally efficient doubly robust estimator and an inverse probability weighted (IPW) estimator. The former attains the semiparametric efficiency bound if all the working models are correctly specified. The latter has a simpler form and requires much fewer model specifications. The IPW estimator using estimated propensity scores is more efficient than that using true propensity scores and achieves the semiparametric efficient bound in the case of discrete covariates and surrogates with finite support. Both estimators are shown to be consistent and asymptotically normally distributed. Extensive simulations are conducted to evaluate the finite-sample performance of the proposed estimators. We apply the proposed methods to estimate the efficacy of oral hydroxychloroquine on renal failure in a real-world data analysis.

Learning Robust Treatment Rules for Censored Data

Abstract:There is a fast-growing literature on estimating optimal treatment rules directly by maximizing the expected outcome. In biomedical studies and operations applications, censored survival outcome is frequently observed, in which case the restricted mean survival time and survival probability are of great interest. In this paper, we propose two robust criteria for learning optimal treatment rules with censored survival outcomes; the former one targets at an optimal treatment rule maximizing the restricted mean survival time, where the restriction is specified by a given quantile such as median; the latter one targets at an optimal treatment rule maximizing buffered survival probabilities, where the predetermined threshold is adjusted to account the restricted mean survival time. We provide theoretical justifications for the proposed optimal treatment rules and develop a sampling-based difference-of-convex algorithm for learning them. In simulation studies, our estimators show improved performance compared to existing methods. We also demonstrate the proposed method using AIDS clinical trial data.

Abstract:Estimating individualized treatment rules is a central task for personalized medicine. [zhao2012estimating] and [zhang2012robust] proposed outcome weighted learning to estimate individualized treatment rules directly through maximizing the expected outcome without modeling the response directly. In this paper, we extend the outcome weighted learning to right censored survival data without requiring either an inverse probability of censoring weighting or a semiparametric modeling of the censoring and failure times as done in [zhao2015doubly]. To accomplish this, we take advantage of the tree based approach proposed in [zhu2012recursively] to nonparametrically impute the survival time in two different ways. The first approach replaces the reward of each individual by the expected survival time, while in the second approach only the censored observations are imputed by their conditional expected failure times. We establish consistency and convergence rates for both estimators. In simulation studies, our estimators demonstrate improved performance compared to existing methods. We also illustrate the proposed method on a phase III clinical trial of non-small cell lung cancer.
