Abstract:BACKGROUND CONTEXT: Suspected as a cause of back pain, Modic changes (MCs) have received increasing attention in spine research and care. Yet, epidemiologic knowledge of MCs based on the general population, which may provide an important clinical reference, is limited.PURPOSE: To investigate the prevalence and distribution patterns of MCs in the lumbosacral spine and their associations with age in a large population-based sample of men.STUDY DESIGN: An epidemiologic investigation of lumbar magnetic resonance images (MRIs).PATIENT SAMPLE: This study was based on the Twin Spine Study database, comprising a sample of male twins shown to be largely representative of the base Finnish population. Lumbar spine MRIs (1.5 Tesla Magnetom; Siemens AG, Erlangen, Germany) of 561 subjects (mean age, 49.8 years; range, 35-70 years) were included in the present study.METHODS: For each spine, all 11 end plates (L1-S1) in the lumbar region were evaluated using both T1- and T2-weighted images to identify MCs, which were classified into Type 1, 2, 3, and mixed types. Furthermore, the number and location of MCs were recorded, as well as the antero-posterior (AP) and transverse sizes, to explore the prevalence and distribution pattern of MCs in the lumbar region and associations with age.RESULTS: Modic changes were identified in 55.6% (312) of individuals and 13.5% (830) of end plates studied. Among these MCs, 64.2% (533) were Type 2, 16.0% (133) were Type 1, 18.1% (150) were Mixed Type 1/2, and the remaining 1.6% (13) were noted as Type 3 or Mixed Type 2/3. Modic changes were more common in the lower (74.5%) than in the upper lumbar region (25.5%), and 77.9% (642) of MCs presented in pairs at opposing end plates of a disc. Moreover, the specific type of MCs on opposing end plates was usually concordant. The presence of MCs in the lumbar region was associated with age (odds ratio=1.05-1.08 for each additional year of age, depending on type of MCs, p<.001). In addition, greater age was associated with a greater number of end plates affected and MCs of larger size (p<.001).CONCLUSIONS: Modic changes are common MRI findings in the lumbar spines of middle-aged white men, with Type 2 MCs predominating. Mainly present in the lower lumbar region, MCs tend to affect both end plates adjacent to a disc simultaneously, and they commonly involve the entire AP diameter of the vertebral end plate. The presence and size of MCs are clearly related to age, suggesting that aging or associated factors may play an essential role in the pathogenesis of MCs. (C) 2012 Elsevier Inc. All rights reserved.
Lifestyle and Lifetime Occupational Exposures May Not Play a Role in the Pathogenesis of Modic Changes on the Lumbar Spine MR Images

Abstract:BACKGROUND CONTEXT: Modic changes (MCs) have long been suspected as a pathologic cause of back pain. Although much attention has been focused on clinical perspectives, the etiology of MCs remains unclear. Although some studies have reported that sex, body mass index (BMI), cigarette smoking, and physical loading may associate with MCs, the observed associations are inconsistent among studies. PURPOSE: To investigate associations between MCs and lifestyle and lifetime occupational exposures using a general population sample. STUDY DESIGN: Cross-sectional study. PATIENT SAMPLE: The study was an extension of the Hangzhou Lumbar Spine Study, a population-based study of mainland Chinese focusing on lumbar degenerative changes. A total of 644 randomly selected subjects from a typical community in Hangzhou, Eastern China participated. OUTCOME MEASURES: The presence and type of MCs in the lumbosacral spine were evaluated on sagittal magnetic resonance images. Demographics, lifestyle factors, and occupational exposures were measured using a structured interview. METHODS: Univariate and multivariate logistic regressions were used to examine the associations of MCs with various environmental exposures. RESULTS: Among the 644 subjects (52.6 +/- 13.9 years; range 20-88 years) included in this study, 44.7% had MCs. In univariate regression analyses, the presence of MCs was associated with greater age, higher BMI, greater cigarette smoking, regular exercise, and absence of daily vehicle vibration. Modic changes were not univariately associated with sex or alcohol consumption. In addition, all occupational loading measurements were associated with the occurrence of MCs in univariate analyses, except work time spent in vehicles and work-related back injuries. However, in multivariate regression analyses, no statistically significant associations between the occurrence of MCs and lifestyle or lifetime occupational exposures were observed after adjusting for age, sex, and BMI. CONCLUSIONS:: Age is an important determinant of MCs, with BMI and sex also playing a role. Lifestyle and occupational factors appear to have minor effects, if any, on the pathogenesis of MCs in the lumbar spine. (C) 2019 Elsevier Inc. All rights reserved.
Analyzing the Influence of Modic Changes on Patients with Lower Back Pain Undergoing Conservative Treatment.

Abstract:Objective. This study aimed to investigate if the presence of Modic changes (MCs) was correlated with lower back pain (LBP) and LBP-related disability in patients who underwent nonsurgical treatment. Methods. In this study, 129 patients who experienced consecutive LBP and underwent lumbar spine magnetic resonance imaging in our institute were divided into three groups according to the presence or type of MCs. The Oswestry Disability Index (ODI) and visual analog scale (VAS) were used to assess the outcomes of the treatment. Results. Based on the achieved results, there was no significant difference between three groups before treatment (P>0.05). Three months after undergoing nonsurgical treatment, the rates of improved ODI and VAS scores were statistically significantly different (P=0.014,0.023). After an additional 3months of treatment, in patients with Modic type I changes, the symptoms significantly improved in comparison with those 3months prior (P=0.037,0.026), while that improvement did not occur in patients with Modic type II changes (P>0.05). Conclusions. The existence of MCs affects the outcomes of nonsurgical treatment in patients with LBP. However, symptoms can be improved after an additional round of treatment for Modic type I changes, while this is not confirmed for Modic type II changes.
Association Between Modic Changes and Endplate Sclerosis: Evidence from a Clinical Radiology Study and a Rabbit Model

Abstract:Purpose: To analyse the presence of endplate sclerosis in patients with various types of Modic changes (MCs) and to confirm the results using a rabbit model. Methods: Participants in the clinical study included 1023 consecutive inpatients with lumbar degenerative disease who attended the Department of Orthopaedics between August 2011 and August 2015. All patients underwent computed tomography (CT) and magnetic resonance imaging of the lumbar spine. In those patients with MCs, endplate sclerosis was evaluated from sagittally reconstructed CT images. In addition to the clinical study, MCs type I, II and III were initiated using a previously developed rabbit model of MCs. Specimens of MCs type I, II and III and normal endplates were harvested, bone mineral density and bone volume/tissue volume of "treated" vertebrae were evaluated using mCT and osteogenic protein expressions of run-related transcription factor 2 and osteocalcin were assessed using immunohistochemical staining. Measurements were compared between vertebrae with normal endplates and those with different types of MCs. Results: Of 1023 patients, 214 (20.9%) had MCs in one or more endplates; these changes affected 1044 (10.2%) of 10230 endplates. Type I, II and III MCs were seen in 164 (1.6%), 838(8.2%) and 40 (0.4%) endplates, respectively. Of 1044 endplates with MCs, 274 (26.2%) showed evidence of sclerosis on CT images including: 26/164 endplates (15.8%) with type I MCs, 208/838 (24.8%) with type II and 40/40 (100%) with type III. HU (CT value) ratios for sclerotic and nonsclerotic endplates with MCs were 2.0 +/- 0.3 and 1.1 +/- 0.1, respectively. In the animal study, the bone mineral density, bone volume/tissue volume and expression of runt-related transcription factor 2 and osteocalcin of endplates with type I and II MCs were higher than those of normal endplates and lower than those of endplates with type III MCs. Conclusion: Sclerosis can occur in endplates with any type of MCs. However, the clinical and animal study suggests that sclerosis is greatest in endplates showing type III MCs. The translational potential of this article: The study showed that sclerosis can occur in endplates with MCs type I, II and III. In patients with endplate sclerosis on plain radiographs or CT scans, the endplate can still represent an inflammatory process associated with chronic lower back pain. (C) 2018 The Authors. Abstract:Study Design: This is a cross-sectional study. Objective: To determine the prevalence and distribution patterns of Modic changes (MCs) in the lumbar spine and their associations with disk degeneration in mainland Chinese using a sample of general population. Summary of Background Data: Previous studies reported that the prevalence of MCs in Hong Kong Chinese was much lower than in other populations. Moreover, their associations with disk degeneration need further study. Materials and Methods: The study sample consisted of 442 subjects (53.614.9y; range, 20-88y) randomly selected from a typical Chinese community. Lumbar spines were imaged using a 3.0T magnetic resonance scanner. Eleven endplates (L1-S1) in the lumbar spine were evaluated for the presence of MCs, type, location, and size to determine MCs prevalence and distribution patterns. Disk degeneration was graded using a Pfirrmann scale. Results: MCs were present in 209 (47.3%) subjects and 593 (12.2%) endplates. Among these endplates, 84.1% (499) were type II, 9.1% (54) were type I, and 6.4% (38) were mixed MCs. Approximately 2/3 MCs were present in the lower lumbar spine and 44.9% of MCs were at the L5/S1 disk level. Most MCs (73.9%) involved both endplates of a disk. Greater age [odds ratio (OR) = 2.44 for each 10-year increase, P < 0.001] and body mass index (OR = 1.07, P = 0.016) were associated with the presence of MCs, as was adjacent disk degeneration (OR = 6.00, P < 0.001), controlling for age and other covariates. Greater age, body mass index, and adjacent disk degeneration were also associated with greater MCs size. Conclusions: MCs are common in mainland Chinese, with type II predominating. MCs mainly present in the lower lumbar region and tend to occur in pairs. MCs were strongly associated with age and disk degeneration, suggesting MCs may be aging-related degenerative findings that parallel disk degeneration.

Abstract:Back pain is common and costly. Although lumbar disc degeneration has long been regarded as a major contributor to back pain, how disc degeneration leads to back pain remains unclear. Recent studies observed microglia activation in the spinal cord after disc degeneration, suggesting activated microglia may be involved in discogenic back pain. To determine whether microglia activation participates in disc degeneration-induced back pain, we used a modified disc puncture-induced degeneration-related back pain mouse model to examine the changes in spinal microglia and investigate the potential link between microglia activation and discogenic back pain. In this study, 46 CX3CR1(GFP/+) male mice were used in experimental and sham groups. A modified posterolateral retroperitoneal approach was used to expose the L3/L4 disc to induce the needle puncture in the experimental group. Behavioral tests, including grip force and physical function, were used to measure back pain at pre- and postsurgery. The L3 dorsal root ganglions and lumbar spinal cord were obtained at postoperative weeks 1 to 4 followed by immunofluorescence with different antibodies. Micrographs were obtained by confocal microscopy, and morphometric measurements of microglia were analyzed using Imaris. The punctured disc underwent progressive degeneration and mice with disc degeneration showed impaired grip force and physical function. Compared to the control mice, the number of microglia in the lumbar spinal cord was significantly increased in the disc-punctured animals. Moreover, accumulated microglia exhibited larger soma size and lesser ramification in the disc-injured mice. Immunofluorescence demonstrated colony-stimulating factor 1, a cytokine that promotes microglia repopulation, was significantly increased in L3 dorsal root ganglions, whereas its receptor colony-stimulating factor 1 receptor was upregulated on microglia in the disc-injured mice. In summary, lumbar disc puncture caused progressive disc degeneration which induced microglia activation and back pain in mice. Increased colony-stimulating factor 1/colony-stimulating factor 1 receptor signaling is involved in the disc degeneration-induced microglia activation and back pain.
Mrgprb2-mediated Mast Cell Activation Exacerbates Modic Changes by Regulating Immune Niches

Abstract:Modic changes are radiographic features associated with microfracture, low-virulence organism infection and chronic inflammation with inflammatory cell infiltration in the vertebral endplate region. Mast cells, as innate immune cells similar to macrophages, are present in painful degenerated intervertebral discs. However, the involvement and mechanisms of mast cells in the development of Modic changes remain unclear. Herein, we found increased mast cell infiltration in samples from patients with Modic changes and in mouse models of Modic changes. To clarify the role of mast cells in the progression of Modic changes, we used mast cell-deficient (KITW-SH/W-SH) mice to construct a model of Modic changes and found that the severity of Modic changes in KITW-SH/W-SH mice was significantly lower than that in WT mice. These findings were further supported by the use of a mast cell-specific activator (compound 48/80) and a stabilizer (cromolyn). Furthermore, we found that mast cells were not activated via the classic IgE pathway in the Modic change models and that Mrgprb2 is the specific receptor for mast cell activation reported in recent studies. Then, we utilized Mrgprb2 knockout mice to demonstrate that Mrgprb2 knockout inhibited mast cell activation and thus reduced the degree of Modic changes. Transcriptomic sequencing revealed aberrant PI3K-AKT and MAPK pathway activation in the Mrgprb2-deficient mast cells. Additionally, Mrgpbrb2-activated mast cells regulate immune niches by recruiting macrophages, promoting M1 polarization and reducing M2 polarization, thereby promoting the progression of Modic changes. These findings suggest that mast cells may serve as a novel therapeutic target for addressing Modic changes. Degenerative disorders of the lower back are common and can greatly affect life quality. Abstract:Background: Modic changes (inflammatory-like changes visible on magnetic resonance imaging [MRI] scans of a vertebral end plate) are common and are associated with low back pain, but their origin is unclear. To our knowledge, there have been no previous in vivo animal models of Modic changes. We hypothesized that Modic changes may be related to Propionibacterium acnes. Methods: Ten New Zealand White rabbits were injected percutaneously with 1 mL of P. acnes (1.6 × 107 colony forming units/mL) into the subchondral bone superior to the L4-L5 and L5-L6 discs; 10 other control rabbits received sham injections at L4-L5 and 1 mL of normal saline solution (vehicle) at L5-L6. The subchondral bone superior to L3-L4 discs was untreated (blank). Development of Modic changes was investigated with MRI studies before the operation and at 2 weeks and 1, 2, 3, and 6 months postoperatively. Following sacrifice of the rabbits, histological analysis and microcomputed tomography (micro-CT) were performed, and blood samples were analyzed. Cytokine expression in end-plate tissues was quantified using real-time polymerase chain reaction (PCR). Results: The group that received P. acnes showed significantly increased T1-weighted signal intensity at 6 months (mean and standard deviation, 3.43 ± 0.41 [range, 2.42 to 4.44] compared with 2.43 ± 0.66 [range, 1.98 to 2.87] before the injection) and higher T2-weighted signal intensity at 6 months. Positive culture results were obtained from 9 of 20 samples injected with P. acnes. Specimens with positive cultures had a higher prevalence of Modic changes (4 of 9 samples positive for P. acnes compared with 3 of 11 samples negative for P. acnes). Real-time PCR showed significantly increased expression of tumor necrosis factor-α, interleukin-1β, and interferon-γ following injection of P. acnes, but no changes were seen on histological analysis, micro-CT, or blood analysis. Conclusions: P. acnes can survive within the end-plate region and can initiate mild inflammatory-like responses from host cells, leading to signal intensity changes in MRI scans, which potentially resemble Modic changes. Clinical Relevance: Disc degeneration and low back pain are associated with Modic changes. Our results indicate that Modic changes can be associated with P. acnes in the conjunction area of the disc and subchondral bone. These results may be useful for understanding the underlying mechanisms of Modic changes and related pain.

Abstract:Objective This research aimed to evaluate the influence of Modic changes (MCs) on disc degeneration at the same and adjacent cephalad levels in the cervical spine. Methods This research retrospectively reviewed 1036 patients with neck pain, upper limb pain, or numbness who were treated at our out-patient clinic and underwent cervical MRI and cervical anteroposterior/lateral radiography from Jan, 2016 to Jan, 2021. MCs and disc degeneration parameters at same and nearby cephalad levels of MCs were evaluated. Discs were divided into the MCs, adjacent, and control groups, and the association between MCs and disc degeneration at the same and adjacent cephalad levels was investigated. Results Of the 1036 patients whose MRI scans were reviewed, 986 met the inclusion criteria (503 women and 483 men; average age, 62.8 years; scope of 35–79 years). The prevalence of MCs in the cervical spine was 13.0% (128/986). Type I, II, III changes were observed in 38 (29.69%), 82 (64.06%), and 8 (6.25%) patients, respectively. MCs were most frequently identified at the C5–6 (59/986; 5.98%) and C6–7 (38/986; 3.85%) levels. Disc with MCs showed worse outcomes with regard to disc degeneration grade, anterior osteophyte formation than the adjacent and control groups ( p < 0.05), whereas they were more severe in the adjacent group compared to normal group. Conclusion Our findings indicate that MCs increased disc degeneration at the same and nearby cephalad levels in cervical spine, and the severity of degeneration at the same segment was more serious than that at the cephalad level.

Abstract:Background Context: There are few comparisons of Modic changes (MCs) in the lumbar and cervical spine.Purpose: Compare the prevalence of MCs in the lumbar and cervical spine, and determine how MC prevalence depends on spinal pain, age, disc degeneration, spinal level, and the presence or absence of kyphosis.Study Design: Retrospective clinical survey.Materials and Methods: Magnetic resonance images (MRIs) were compared from five patient groups: 1. 1223 patients with low-back pain/radiculopathy only; 2. 1023 patients with neck pain/radiculopathy only; 3. 497 patients with concurrent low-back and neck symptoms; 4. 304 asymptomatic subjects with lumbar MRIs; and 5. 120 asymptomatic subjects with cervical MRIs.Results: The prevalence of MCs was higher in those with spinal pain than in those without, both in the lumbar spine (21.0% vs 10.5%) and cervical spine (8.8% vs 3.3%). Type II MCs were most common and Type III were least common in all groups. The prevalence of lumbar MCs in people with back pain was little affected by the presence of concurrent neck pain, and the same was true for the prevalence of cervical MCs in people with neck pain with or without concurrent back pain. When symptomatic patients were reclassified into two groups (back pain, neck pain), the prevalence of lumbar MCs in people with back pain was greater than that of cervical MCs in people with neck pain. The prevalence of lumbar and cervical MCs increased with age, disc degeneration, (descending) spinal level, and increased kyphosis.Conclusions: There is a significantly higher prevalence of MCs in patients with back and neck pain. The reported association with increased kyphosis (flat back) is novel.

Abstract:Abstract Background Back pain often arises from degenerative changes in lumbar intervertebral discs and their adjacent endplates. A painful endplate is not easy to identify in patients, but could possibly be revealed by inflammatory-like ‘Modic’ changes and by a ‘vacuum phenomenon’ within the disc. We hypothesize that Modic changes and a VP often co-exist in those lumbar levels most closely associated with back pain Methods We scanned 1023 consecutive inpatients of the Department of Orthopaedics from 2015 August to 2018 August, all patients suffered from lumbar degenerative diseases, whether the patients had back pain or not were evaluated, and the prevalence and location of vacuum phenomenon(VP) and Modic changes were compared at each spinal level. Results 5115 discs were studied from 1023 patients. The number of discs showed to have a VP was 430 using CT, of the 430 discs with a CT-diagnosed VP, 116 were L4-5, and 171 were L5-S1. 522 of the 5115 discs exhibited Modic changes, with prevalence of type I, type II and type III Modic changes being 1.6%, 8.2% and 0.4% respectively. Modic changes usually occurred adjacent to L4-5 discs or L5-S1 discs. The prevalence of back pain was higher in the VP group than no-VP group, VP were significantly associated with Modic changes Type II at L4/5 and at L5/S1. Conclusion VP are closely associated with back pain and Modic changes in the lower lumbar spine. Further investigations may be warranted when radiographs or CT identify a VP in a lumbar disc.

Abstract:Modic changes are bone marrow and endplate lesions visible in magnetic resonance imaging (MRI). They are regarded as a part of degenerative disc disease and associated with low back pain. And severe disc degeneration was occurred more in the patients with Modic changes. But there is still no study to analyze the relationship between Modic changes and intervertebral disc degeneration.We hypothesize that Modic changes are the possible causes and promotion of lumbar intervertebral disc degeneration. And there are three possible mechanisms for this hypothesis: a structural cause: Modic changes make cartilaginous material easier in extruded disc herniations, to destroy the structure of intervertebral disc and inhibit the absorption of the disc. A biomechanical cause: Modic changes alter the mechanical loading distribution on disc, to initiate a series of disc disruption and inhibit the self-recovery of the disc. A nutritional cause: Modic changes destroy the vascular architecture in vertebral endplate and block the most important metabolism pathway between vertebrae and disc.Perspectives: (1) Find out procedures to cure Modic changes may be an important breakthrough for disc degenerative disease. Abstract:Spinal cord injury (SCI) in dogs is commonly attributed to intervertebral disc extrusion (IVDE). Over the last years substantial progress was made in the elucidation of factors contributing to the pathogenesis of this common canine disease. A detailed understanding of the underlying histopathological and molecular alterations in the lesioned spinal cord represents a prerequisite to translate knowledge on the time course of secondary injury processes into the clinical setting. This review summarizes the current state of knowledge of the underlying pathology of canine IVDE-related SCI. Pathological alterations in the spinal cord of dogs affected by IVDE-related SCI include early and persisting axonal damage and glial responses, dominated by phagocytic microglia/macrophages. These processes are paralleled by a pro-inflammatory microenvironment with dysregulation of cytokines and matrix metalloproteinases within the spinal cord. These data mirror findings from a clinical and therapeutic perspective and can be used to identify biomarkers that are able to more precisely predict the clinical outcome. The pathogenesis of progressive myelomalacia, a devastating complication of SCI in dogs, is not understood in detail so far; however, a fulminant and exaggerating secondary injury response with massive reactive oxygen species formation seems to be involved in this unique neuropathological entity. There are substantial gaps in the knowledge of pathological changes in IVDE with respect to more advanced and chronic lesions and the potential involvement of demyelination. Moreover, the role of microglia/macrophage polarization in IVDE-related SCI still remains to be investigated. A close collaboration of clinical neurologists and veterinary pathologists will help to facilitate an integrative approach to a more detailed understanding of the molecular pathogenesis of canine IVDE and thus to identify therapeutic targets.

Abstract:Background Modic changes are the MRI signal changes of degenerative lumbar vertebral endplate and which lead to or accelerate intervertebral disc degeneration. NLRP3, caspase-1, and interleukin-1β (IL-1β) play a pivotal role in the pathogenesis of many inflammatory diseases, such as osteoarthritis. However, the roles of IL-1β and its activators caspase-1 and NLRP3 are unclear in the degenerative endplate. Questions/purposes We asked: (1) What are the degenerative changes of the histologic features and chondrogenic markers’ gene expressions between the cartilaginous endplates of patients with Modic changes and trauma (control)? (2) How does the NLRP3/caspase-1/IL-1β axis in the cartilaginous endplates of patients with Modic changes compare with control (trauma) specimens? Methods Surgical specimens of cartilaginous endplates were divided into Modic changes (n = 56) and the trauma control (n = 16) groups. Hematoxylin and eosin and safranin O staining of cartilaginous endplate tissues were done to evaluate the extracellular matrix. Reverse transcription-polymerase chain reaction was performed on these tissues to investigate mRNA expression of type II collagen (Col II), SOX-9, matrix metalloproteinase-3, and a disintegrin like and metalloproteinase thrombospondin type I motifs-5. NLRP3, caspase-1, and IL-1β were evaluated by reverse transcription-polymerase chain reaction and immunohistochemistry. Results Hematoxylin and eosin and safranin O staining showed the extracellular matrix degraded in the cartilaginous endplates of patients with Modic changes but not in the control cartilaginous endplates. Chondrogenic Col II (p = 0.024) and SOX9 (p = 0.053) were downregulated in the Modic changes group compared with the control group. In contrast to the control group, the transcriptional levels of NLRP3 (p < 0.001), caspase-1 (p = 0.054), and IL-1β (p = 0.001) were all upregulated in the Modic changes group. Conclusions The expression of NLRP3, caspase-1, and IL-1β was upregulated in the patients with low back pain and Modic changes on MRI compared with patients with vertebral burst fracture without degenerative changes on MRI. The data suggest the NLRP3/caspase-1/IL-1β axis may be implicated in lumbar cartilaginous endplate degeneration. Clinical Relevance The NLRP3/caspase-1/IL-1β axis is active in cartilaginous endplates of patients with Modic changes and inflammatory cascades can exacerbate the cartilaginous endplate degeneration which may act as a trigger for intervertebral disc degeneration and low back pain. If these findings can be confirmed by others, we hope that new and effective therapy could be developed directed against this target.
Modic Changes in Ecuadorian Mestizo Patients: Epidemiology, Clinical Significance, and Role in Chronic Low Back Pain

Abstract:Modic changes (MC) are bone marrow lesions seen within a vertebral body on MRI, possibly associated with low back pain (LBP). Though the causes and mechanisms responsible for the formation of MC are still poorly understood, progress is being made in linking his spinal phenotype with disc degeneration and LBP. This paper analyzes the epidemiology, clinical signs, lesions type, and treatment of vertebral discopathy associated with MC in Ecuadorian mestizo patients, comparing MC type I-II changes versus MC type III differences. We performed an epidemiological, observational, cross-sectional study with two cohorts of Mestizo patients collected at "Hospital de los Valles" in Quito, Ecuador, between January 2017 and December 2020; 288 patients diagnosed with degenerative lumbar disc disease plus MC was taken who underwent surgery; 144 with MC type I-II (cohort 1) and 144 with MC type III changes (cohort 2). Cohort 1 was characterized by 68.8% of men with a mean age of 45 years who perform minimal or moderate exercise in 82% of cases. They showed only one level lesion in 88.9% of patients with a pain intensity of 7 or more on the visual analog scale, with three or more months of evolution, in 78.5% of cases of degenerative etiology, mainly between the L5-S1 lesion of the left side. Cohort 2 was 53.5% of women with a mean age of 62. In 81.4% of cases, they perform minimal or moderate exercise. They showed two-level lesions in 45.8% of patients with a pain intensity of 7 or more on the visual analog scale, with three or more months of evolution, in 97.9% of cases of degenerative etiology, mostly between L4-L5 lesions of the left side. In both groups, most patients showed a protruded and lateral hernia. There is a greater predisposition to require surgery for lumbar disc herniation in young men and older women. In addition, surgery at an older age has a higher risk of complications, especially infection.

Abstract:Objective To evaluate the distribution of Modic changes of lumbar endplate in patients suffering low back pain or radiculopathy and its related factors. Methods 1287 patients suffering from low back pain without tumor,infection and degenerative scoliosis were examined by MRI scan from July 2012 to June 2013. According to the criteria of Modic changes,the features of distribution of Modic changes of lumbar endplate on age,segment and type of intervertebral disc degenemtion were analyzed retrospectively. Results Among 6435 intervertebral disc of the 1287 patients. 450(34. 97% )cases and 675(10. 49% )intervertebral discs were invo1ved with Modic changes. Among them,103(8. 0% )cases and 116(1. 80% )discs were type Ⅰ;313(24. 32% )cases and 519(8. 01% )discs were typeⅡ;34(2. 64% )cases and 40(0. 62% )discs were type Ⅲ. There were 182 male cases(28. 09% ),268 female cases(41. 94% ),the difference was statistically significant from men and women(P ﹤ 0. 05). There were 56 cases(14. 32% )less than 40 years old,394 cases(43. 97% )greater than or equal to 40 years old. The difference was statistically significant(P ﹤ 0. 05). There were 86(2. 29% )normal disc and 589(21. 97% )de-generative disc,the difference was statistically significant(P ﹤ 0. 05). There were 285(7. 38% )lumbar,390(15. 15% )lower lumbar. The difference was statistically significant(P ﹤ 0. 05). Conclusion Endplate Modic changes happen more in women patients with low back pain,overe 40 years old and multiple degeneration. And it correlates with the degree of disc degenera-tion.

Abstract:Diabetic microangiopathy is a degenerative process specific to diabetes. Metabolic theory holds that the metabolic defect predisposes to vascular abnormalities, but other theory suggests that microangiopathy and metabolic disorders are of different origin. Metabolic and ultrastructural modifications in kidney and retina are reviewed. Human and animal studies suggest a specific organ response to fortuitous association of several factors leading to microvascular disease.

Abstract:Intervertebral disc herniation (IVDH) is one of the most common causes of spinal cord injury (SCI) in dogs. As a result of acute SCI, a complex inflammatory response occurs in the spinal cord. Th17 cells (Th17) produce pro-inflammatory cytokines, while regulatory T cells (Treg) have opposite effects producing anti-inflammatory cytokines. Therefore, the aim of this study was to determine whether Th17- and Treg cells are involved in the pathogenesis of SCI or whether cellular changes occur due to coexisting inflammatory diseases. We hypothesized that chronic alterations in the Th17/Treg ratio are associated with a worse outcome after SCI.

Abstract:BACKGROUND AND OBJECTIVE: The intervertebral disc degeneration changes and paraspinal muscles changes are believed to be risk factors for lumbar degenerative spondylolisthesis (LDS). But there is limited quantitative information about this progression. This study is to reveal their changes the in the progression of LDS. METHODS: Data were gathered from 149 middle-aged degenerative spondylolisthesis patients and same amount of age-and sex-matched control group with both lumber spine MRI and X-ray. Narrowed disc space were measured in percent as anterior inferior disc height (DHIA)/anterior superior disc height (DHSA), inferior disc height (DHI)/superior disc height (DHS), and posterior inferior disc height (DHIP)/posterior superior disc height (DHSP). Signal intensity ratio of multifidus muscle (RM) and erector spinae (RES) to psoas muscle, muscle atrophy ratio of lean CSA (LCSA) to gross CSA (GCSA) of paraspinal muscles were calculated. RESULTS: In the case group the most common slipped vertebra was L4 (75.84%). Disc height (DHIA/DHSA, DHI/DHS) and multifidus muscle atrophy ratio (M-LCSA/M-GCSA) tended to be lower than those in the control group, whereas the disc degeneration degree and T2 signal intensity ratio (RM, RES) of the paraspinal muscles and erector spinae muscle atrophy ratio were higher than control group. The difference between the two groups was statistically significant (P < 0.05). Using multivariate logistic regression analysis, it was confirmed that ES-LCSA/ES-GCSA, especially RM are independent predisposing factors to lumbar spondylolisthesis (OR > 1, P < 0.05) while DHIA/DHSA, M-LCSA/M-GCSA are independent protective factors (OR < 1, P < 0.05). CONCLUSIONS: Decreased anterior disc height and multifidus muscle atrophy are found in the LDS patients and thy could be the cause of LDS. The presence of erector spinae hypertrophy could be a compensatory mechanism to compensate for the instability.
