Abstract:Background:Depression and anxiety are the most common psychiatric comorbidities in patients with epilepsy (PWE). However, they are often unrecognized and consequently untreated.Objective:The study was conducted to evaluate the prevalence and risk factors of anxiety and depression among Chinese adult PWE.Design:Cross-sectional study.Methods:Adult PWE were recruited from 13 tertiary epilepsy centers from February to September 2022. Generalized Anxiety Disorder-7 and Neurological Disorders Depression Inventory for Epilepsy were applied to evaluate anxiety and depression, respectively. Both univariate and multivariate logistic regression analyses models were performed to explore the risk factors of anxiety and depression.Results:A total of 1326 PWE were enrolled in this study. The prevalence of anxiety and depression was 31.45% and 27.30%, respectively. Being female [odds ratio (OR) = 1.467, 95% CI: 1.134-1.899; p = 0.004], focal and focal to bilateral tonic-clonic seizures (TCSZ) (OR = 1.409, 95% CI: 1.021-1.939; p = 0.036), and seizure occurrence in the last 3 months (OR = 1.445, 95% CI: 1.026-2.044; p = 0.036) were the risk factors for anxiety. Focal and focal to bilateral TCSZ (OR = 1.531, 95% CI: 1.094-2.138; p = 0.013) and seizure occurrence in the last 3 months (OR = 1.644, 95% CI: 1.130-2.411; p = 0.010) were the risk factors for depression. In addition, for every 1-year increment of age, the odds of developing depression were decreased by 3.8% (p = 4.12e-5). Nevertheless, up to 70% of PWE did not receive any treatment for comorbidity.Conclusion:There were approximately 30% of PWE screened positive for anxiety or depression. Both focal and focal to bilateral TCSZ and seizure occurrence in the last 3 months were estimated as risk factors for anxiety and depression. However, the current status of treatment was not optimal.

Abstract:Background: Anxiety and depression remain underdiagnosed in routine clinical practice in up to two thirds of epilepsy patients despite significant impact on medical and psychosocial outcome. Barriers to adequate mental health care for epilepsy and/or psychogenic non-epileptic seizures (PNES) include a lack of integrated mental health specialists and standardized procedures. This naturalistic study outlines the procedures and outcome of a recently established psychotherapeutic service. Methods: Routine screening included the Neurological Disorders Depression Inventory for Epilepsy (NDDI-E, cut-off value > 13) and Generalized Anxiety Disorder scale (GAD-7, cut-off value > 5). Positively (above cut-off in at least one questionnaire) screened patients were seen for a standardized interview for mental health disorders and the development of a personalized treatment plan. PNES patients were seen irrespective of their screening score. Resources were provided to support self-help and access to psychotherapy. Patients were contacted 1 month after discharge to evaluate adherence to therapeutic recommendations. Results: 120 patients were screened. Overall, 56 of 77 positively screened patients (77%) were found to have a psychiatric diagnosis through standardized interview. More epilepsy patients with an anxiety disorder had previously been undiagnosed compared to those with a depressive episode (63% vs. 30%); 24 epilepsy patients (62%) with a psychiatric comorbidity and 10 PNES patients (59%) were not receiving any mental health care. At follow-up, 16/17 (94%) epilepsy patients and 7/7 PNES patients without prior psychiatric treatment were adhering to therapeutic recommendations. Conclusion: Integrating mental health specialists and establishing standardized screening and follow-up procedures improve adherence to mental health care recommendations in epilepsy and PNES patients.

A comprehensive digital mental health screening tool for people with epilepsy: A feasibility study in UK outpatients

Abstract:Background: Mental health symptoms are common in people with epilepsy, impacting medication adherence, quality of life, and mortality. Early detection and timely interventions for mental health symptoms will be crucial for improved outcomes but the absence of standardized screening procedures and time constraints hinder regular assessment and management. Purpose: To evaluate feasibility, acceptability and, value of a digital tool for identifying mental health symptoms in adult and paediatric epilepsy outpatients using electronic Patient-Reported Outcome Measures (ePROMs). Methods and materials: The study used an established local platform (IMPARTS - Integrating Mental and Physical Healthcare: Research Training and Services) to develop an online tool using e-PROMS for a comprehensive mental health screen (psychiatric symptoms, neurodevelopmental traits, and psychosocial/behavioural risk factors) of people with epilepsy. Prior to attending the outpatient clinical epilepsy services at King's College Hospital, participants were invited to complete the online screening tool through an SMS appointment link. Results: Out of 1081 epilepsy patients (955 adults, 126 paediatric), 38.2% of adults and 51.6% of carers of paediatric patients accessed the ePROMs, with modest completion rates of 15% and 14%, respectively. Adults reported mild to significant anxiety (37.4%), minor to major depression symptoms (29.2%), and occasionally psychotic symptoms (11.1%). Adults with self-reported psychiatric symptoms reported significantly higher number of seizures, seizure burden, insomnia, autistic and ADHD traits and lower quality of life and perceived social support. Only 21% of those reporting psychiatric symptoms were receiving any form of mental health support. A large proportion of paediatric patients presented emotional/behavioural difficulties (32%), high impulsivity (38.8%), low self-esteem (27.7%), sleep difficulties (50%), comorbid neurodevelopmental syndromes (27.7%). Both groups reported good level of perceived social support. Conclusion: Our epilepsy adapted IMPARTS e-PROMS allowed remote screening for mental health symptoms, neurodevelopmental and resilience factors. Integrating these tools into electronic patient records might enhance early identification and facilitate referral to appropriate care pathways.

Abstract:Objective: We aimed to evaluate the assessment and management of epilepsy with anxiety and depression, and their clinical practice based on a survey. Methods: A cross-sectional survey of epilepsy health professionals was undertaken in Zhejiang Province using the modified International League Against Epilepsy (ILAE) Psychology Task Force questionnaire. We recorded the characteristics of participants and the practice of screening, referral, and treatment for depression and anxiety disorders. A total of 146 participants joined in the survey, of which 76.0% were neurologists, and 69 participants were the member of the Zhejiang Association Against Epilepsy (ZAAE). Results: This survey revealed that almost all participants (87.7%) agreed that screening for depression and anxiety in patients with epilepsy (PWEs) was very important; however, the frequency of screening was very low (41.1% of participants screened less than 10% of patients, and 34.2% participants screened between 10% and 30% of patients). A higher frequency of screening was reported in the member group and compared with that in the non-member group (P = 0.025). The main barrier to screening was the lack of time during clinic visits: 81.5% participants included screening questions as part of their clinical review. When anxiety/depression was diagnosed, the next step should be to refer patients to a psychiatrist (78.1%). No standardized procedures and lack of mental health specialists trained to assess and/or manage PWEs, were the main barriers to follow-up assessment and management. Lack of appropriately trained mental health specialists was also the main barrier to psychological treatment for depression and anxiety. Conclusion: This survey highlighted that epilepsy healthcare professionals in Zhejiang province agreed on the importance of screening for psychiatric comorbidities in PWEs; however, the screening and management were actually insufficient. Validity Study of Patient Health Questionnaire-9 Items for Internet Screening in Depression among Chinese University Students

Abstract:BackgroundInternet intervention in Chinese university students would be a possible approach to overcome the gap between high rate of depression and high rates of underdiagnosis and undertreatment. As a popular measure of screening, the feasibility and user satisfaction of Patient Health Questionnaire-9 items for online program were tested.MethodsThe subjects were enrolled based on an email list from the students' office of a Chinese university, and 300 undergraduate students were randomly invited of which 230 (76.7%) students were willing to participate in the study and completed the first test. After 2weeks, a subsample of 150 (65.2%) subjects were randomly chosen to retake the test for the test-retest reliability. And 81 (35.2%) among the 230 subjects were randomly selected to undergo the Mini International Neuropsychiatric Interview within 48hours. Among 150 subjects, 120 (52.2%) completed client satisfaction questionnaire about this online screening program.Results(1) The Cronbach alpha was 0.80 and the test-retest reliability was 0.78. (2) The optimal cutoff score of 10 revealed a sensitivity of 0.74 and specificity of 0.85, with an area under the curve of 0.897 (95% confidence interval, 0.823-0.970). (3) The mean duration of administration was 3.5minutes. (4) Satisfaction with the online screening program was highly appreciated.ConclusionsThe results indicated potential value of the online screening program for further Internet-administrated programs of depression among Chinese university students.
Clinical Risk Factors for Depressive Symptoms in Patients with Epilepsy

Abstract:Background and aimsTo investigate the relationships between demographic data, seizure-related factors, anti-epileptic drugs (AEDs) taking, and depressive symptoms in patients with epilepsy (PWE), determining the major clinical risk factors of depression.MethodsPatients with epilepsy who visited our epilepsy clinic from 2010 to 2012 were included. The clinical data were collected, and Hamilton Depression Rating Scale (HAMD), National Hospital Seizure Severity Scale (NHS3) and Pittsburgh Sleep Quality Index (PSQI) were evaluated.ResultsA total of 116 PWE were recruited. They were divided into three groups. Age, duration of epilepsy, percentages of patients with partial seizures, history of status epilepticus (SE), using topiramate (TPM) or clonazepam (CZP), and using greater than or equal to 2 types of AEDs were all significantly higher in patients with moderate depressive symptoms than patients without depression. HAMD scores were positively correlated with age, duration of epilepsy, and the number of AEDs taking, respectively. PSQI scores were positively correlated with HAMD scores in patients with depressive symptoms. Age greater than 35years, females, having partial seizures, history of SE, and using TPM were independent predictors of depressive symptoms in PWE by regression analysis.ConclusionsAge greater than 35years, females, having partial seizures, history of SE, and using TPM might become risk factors for depressive symptoms in PWE.

Abstract:Introduction: The overall combined prevalence of anxiety and depression in patients with epilepsy has been estimated at 20.2 and 22.9%, respectively, and is considered more severe in drug-refractory epilepsy. Patients admitted to epilepsy monitoring units constitute a particular group. Also, patients with psychogenic non-epileptic seizures can reach more than 20% of all admissions. This study aims to characterize these symptoms in a large cohort of patients admitted for evaluation in a tertiary epilepsy center. Materials and Methods: The study was conducted among 493 consecutive patients (age: 38.78 ± 12.7, 57% females) admitted for long-term video EEG from January 2013 to February 2021. Demographic, clinical, and mood disorder patients' data were collected. Anxiety and depression symptoms were assessed through the Hospital Anxiety Depression Scale (HADS-A and HADS-D), the State Trait Anxiety Inventory (STAI), and Beck Depression Inventory (BDI-II). Quality of life was determined using the QOLIE-10. Patients were divided into three groups: patients with epilepsy ( n = 395), psychogenic non-epileptic seizures (PNES) ( n = 56), and combined ( n = 33). A univariate and multivariate regression analysis was performed for variables associated with quality of life. Results: Of 493 patients, 45.0% had structural etiology, and considering epilepsy classification, 43.6% were of temporal lobe origin. In addition, 32.45% of patients had a previous psychiatric history, 49.9% of patients had depressive symptoms in BDI, and 30.9% according to HADS-D; 56.42 and 52.63% of patients presented pathological anxiety scores in STAI-T and STAI-S, respectively; and 44.78% according to HADS-A. PNES and combined groups revealed a higher incidence of pathologic BDI scores (64.29 and 78.79%, p < 0.001) as well as pathologic HADS-A scores ( p = 0.001). Anxiety and depression pathologic results are more prevalent in females, HADS-A (females = 50.7%, males = 36.8%; p = 0.0027) and BDI > 13 (females = 56.6%, males = 41.0%; p = 0.0006). QOLIE-10 showed that 71% of the patients had their quality of life affected with significantly higher scores in the combined group than in the epilepsy and PNES groups ( p = 0.0015). Conclusions: Subjective anxiety, depression, and reduced quality of life are highly prevalent in patients with refractory epilepsy. These symptoms are more evident when PNES are associated with epilepsy and more severe among female patients. Most of the cases were not previously diagnosed. These factors should be considered in everyday clinical practice, and specific approaches might be adapted depending on the patient's profile.

Effect of Endotracheal Tube Cuff Shape on Postoperative Sore Throat After Endotracheal Intubation

Abstract:BACKGROUND: Although minor, a sore throat after endotracheal intubation can adversely affect patient satisfaction and postoperative function. We compared the effects of 2 endotracheal tube cuff shapes on postoperative sore throat. METHODS: One hundred ninety-one adult patients were included in the study. After induction of anesthesia, patients were randomized to endotracheal intubation with a conventional cylindrical-shaped cuff (Group C, n = 95) or a tapered-shaped cuff (Group T, n = 96). The number of intubation attempts, time to achieve endotracheal intubation, and duration of intubation were recorded. Postoperative sore throat and hoarseness were assessed at 1, 6, and 24 hours after surgery. A 0- to 100-mm visual analog scale was used to evaluate sore throat severity. The primary outcome of this study was the overall cumulative incidence of postoperative sore throat in the 24-hour evaluation period in the 2 groups. RESULTS: The overall incidence of postoperative sore throat was lower in Group T than in Group C (32% vs 54%; relative risk = 0.60, 95% confidence interval: 0.43–0.85; P= .003). At 6 hours after surgery, the incidence and severity of postoperative sore throat were lower in Group T compared with Group C (Bonferroni-corrected P< .05). Postoperative hoarseness also occurred less frequently in Group T compared with Group C (19% vs 37%; P= .006). Group T had lower incidence of hoarseness at 1 and 6 hours after surgery than Group C (Bonferroni-corrected P< .05), but the incidence of hoarseness at 24 hours after surgery did not differ between groups. CONCLUSIONS: Intubation using an endotracheal tube with a tapered cuff reduced the incidence and severity of postoperative sore throat and the incidence of hoarseness after surgery when compared with an endotracheal tube with a cylindrical cuff.
The usefulness of NDDI‐E and QOLIE‐10 scales for the screening of major depressive disorders in patients with epilepsy in video‐EEG units

Abstract:Objective Mood disorders in patients with epilepsy are common, with depression being the most prevalent. However, this comorbidity is often underdiagnosed. The systematic use of scales such as NDDI‐E and QOLIE‐10 in prolonged video‐EEG monitoring units could be a useful tool for the detection of this comorbidity. Methods Descriptive cross‐sectional study of a series of patients with epilepsy evaluated in a prolonged video‐EEG monitoring unit. Results Three hundred forty‐nine patients were included. The mean age was 49.1 years, and 49.3% were female. 66.2% had focal epilepsy. 20.4% had pharmacoresistant epilepsy. 38.7% of patients had NDDI‐E > 13. 43% of patients with focal epilepsy had NDDI‐E > 13 versus 21.8% of patients with idiopathic generalized epilepsy (p = .015). Patients with focal temporal epilepsy had the highest rate of NDDI‐E > 13 (48.5%). Significant association was found between patient‐perceived mood and NDDI‐E score (p 13, 37.6% had reported feeling "very good" or "good" in mood. Likewise, in the group that had reported feeling "very good" or "good" 21.6% had NDDI‐E > 13. In 50.5% of patients with NDDI‐E > 13 some kind of therapeutic intervention aimed at this comorbidity was performed. Abstract:Ghrelin is a 28 aminoacids peptide secreted from the stomach that stimulates the release of growth hormone (GH) from the anterior pituitary cells and is the strongest orexigenic hormone discovered so far. Ghrelin seems to be involved in the pathogenesis of obesity, anorexia nervosa and cachexia. Furthermore, low levels of ghrelin are negatively correlated with the degree of insulin resistance, blood pressure and the prevalence of type 2 diabetes. The role of ghrelin in the energy homeostasis and carbohydrate metabolism is discussed.

The impact of social factors, especially psychological worries on anxiety and depression in patients with epilepsy

Abstract:BACKGROUND: Social factors are believed to affect mental health in patients with epilepsy (PWE). However, there is still a lack of sufficient manifest proof, given the difficulty of exposing PWE to relatively consistent natural social environments with a low or high level of social interaction to study their significant role.METHODS: This single-center, longitudinal study was conducted via online questionnaires during the coronavirus disease 2019. PWE were recruited from downtown Wuhan and surrounding areas. The Patient Health Questionnaire-9 and Generalized Anxiety Disorder-7 were used to assess psychological status.RESULTS: We analyzed 588 questionnaires completed by 294 PWE who participated in the dual survey. Under lockdown and reopening, the prevalence of anxiety was 13.6%/22.5%, and the prevalence of depression was 19.4%/34.0%. Raising children and seizure-related characteristics, including uncontrolled seizures, seizure exacerbation, seizure frequency ≥ 2/m, and changes in drug regimen, were risk factors in the first and second surveys. A high education level (OR = 1.946, 95% CI = 1.191-3.182), low life satisfaction (OR = 1.940, 95% CI = 1.007-3.737), worry about unanticipated seizures (OR = 2.147, 95% CI = 1.049-4.309), and worry about purchasing medication outside (OR = 2.063, 95% CI = 1.060-4.016) were risk factors for higher scores after reopening. Worry about unanticipated seizures (OR = 3.012, 95% CI = 1.302-6.965) and in-person medical consultation (OR = 2.319, 95% CI = 1.262-4.261) were related to newly diagnosed patients with psychological disorder after reopening.CONCLUSIONS: We identified an association between social variables and epileptic psychiatric comorbidities.

Abstract:The co-occurrence of depressive and anxiety symptoms in a variety of neurological conditions are widely common and disabling. Furthermore, it could persist for a long time and interfere in the development of neurological disorders, and affect the prognosis of neurological disorders.1 However, despite their high incidence and jeopardy, depressive and anxiety symptoms remain under-recognised and undertreated.2 Most previous studies focused on depressive and anxiety symptoms in specific neurological diseases such as stroke, Parkinson’s disease, epilepsy and multiple sclerosis. However, the prevalence of depressive and anxiety symptoms in neurological diseases were reported differently, and few studies reported the situation in Asia. Furthermore, a number of outpatients are newly referred to general neurology clinics for different symptoms such as headache, dizziness, cerebrovascular diseases and insomnia. Hence, the prevalence of depressive and anxiety symptoms in these new patients at the neurology outpatient clinic remains unclear, and the symptoms which predict the experience of depressive and anxiety symptoms are still ambiguous. Abstract:OBJECTIVE: To validate the Chinese version of the Generalized Anxiety Disorder-7 (GAD-7) in Chinese people with epilepsy (PWE).METHODS: A consecutive cohort of PWE from the West China Hospital was recruited. The GAD-7 was easily understood and quickly completed by all participants. Fifty patients (23.5%) had GAD according to the MINI criteria. Cronbach's α coefficient for the GAD-7 was 0.888. ROC analysis showed an area under the curve of 0.974 (95% CI=0.956-0.993). At a cut-off score of >6, the GAD-7 achieved the largest Youden index of 0.854 with a sensitivity of 94%, a specificity of 91.4%, a positive predictive value of 77% and a negative predictive value of 98%.SIGNIFICANCE: The Chinese version of the GAD-7 is a valuable tool for screening for GAD in Chinese PWE.

Comparison of mental health screening tools for detecting antenatal depression and anxiety disorders in South African women

Abstract:BACKGROUND: Antenatal depression and anxiety disorders are highly prevalent in low and middle-income countries. Screening of pregnant women in primary care antenatal settings provides an opportunity for entry to care, but data are needed on the performance of different screening tools. We compared five widely-used questionnaires in a sample of pregnant women in urban South Africa.METHOD: Pregnant women attending a primary care antenatal clinic were administered five tools by trained research assistants: the Edinburgh Postnatal Depression Scale (EPDS), the Patient Health Questionnaire (PHQ-9), the Kessler Psychological Distress scale (K10) and a shortened 6-item version (K6), the Whooley questions and the two-item Generalised Anxiety Disorder scale (GAD-2). Following this, a registered mental health counsellor administered the MINI Plus, a structured clinical diagnostic interview. The Area Under the Curve (AUC) from Receiver Operator Characteristic curve analysis was used to summarise screening test performance and Cronbach's α used to assess internal consistency.RESULTS: Of 376 participants, 32% were diagnosed with either MDE and/or anxiety disorders. All five questionnaires demonstrated moderate to high performance (AUC = 0.78-0.85). The EPDS was the best performing instrument for detecting MDE and the K10 and K6 for anxiety disorder. For MDE and/or anxiety disorders, the EPDS had the highest AUC (0.83). Of the short instruments, the K10 (AUC = 0.85) and the K6 (AUC = 0.85) performed the best, with the K6 showing good balance between sensitivity (74%) and specificity (85%) and a good positive predictive value (70%). The Whooley questions (AUC = 0.81) were the best performing ultra-short instrument. Internal consistency ranged from good to acceptable (α = 0.89-0.71). However, the PPV of the questionnaires compared with the diagnostic interview, ranged from 54% to 71% at the optimal cut-off scores.CONCLUSIONS: Universal screening for case identification of antenatal depression and anxiety disorders in low-resource settings can be conducted with a number of commonly used screening instruments. Short and ultra-short screening instruments such as the K6 and the Whooley questions may be feasible and acceptable for use in these settings.

Abstract:Objective Epilepsy is one of the most prevalent chronic neurological diseases, presenting a high frequency of psychiatric disorders (PD). This study sought to evaluate the clinical and sociodemographic profile of patients with epilepsy (PWE) attended at a regional reference psychiatric emergency unit. Methods A retrospective, cross-sectional, and descriptive observational study was conducted utilizing a patient record database of individuals with epilepsy who were attended in a regional reference psychiatric emergency unit between January 2018 and August 2022. Results Out of the 31,800 psychiatric emergency visits, 260 (0.8%) were of patients with epilepsy (ICD-10: G40). The majority were males (63.5%) with a mean age of 42.11±15.39 years, single marital status (154; 59.25%) and elementary education (75; 28.9%). Most of them (203; 78.1%) presented at least one psychiatric comorbidity at the emergency visit, but 109 (41.9%) were not receiving any psychiatric follow-up. A total of 106 patients (40.8%) had experienced at least one past psychiatric hospitalization. Conclusions The occurrence of PD is highly prevalent in PWE, and probably with a high frequency of psychiatric emergencies occurring among them. Further studies are needed to assess such patient profile, particularly involving data from psychiatric emergency units.

Abstract:Background and Objectives Mood, anxiety disorders, and suicidality are more frequent in people with epilepsy than in the general population. Yet, their prevalence and the types of mood and anxiety disorders associated with suicidality at the time of the epilepsy diagnosis are not established. We sought to answer these questions in patients with newly diagnosed focal epilepsy and to assess their association with suicidal ideation and attempts. Methods The data were derived from the Human Epilepsy Project study. A total of 347 consecutive adults aged 18–60 years with newly diagnosed focal epilepsy were enrolled within 4 months of starting treatment. The types of mood and anxiety disorders were identified with the Mini International Neuropsychiatric Interview, whereas suicidal ideation (lifetime, current, active, and passive) and suicidal attempts (lifetime and current) were established with the Columbia Suicidality Severity Rating Scale (CSSRS). Statistical analyses included the t test, χ 2 statistics, and logistic regression analyses. Results A total of 151 (43.5%) patients had a psychiatric diagnosis; 134 (38.6%) met the criteria for a mood and/or anxiety disorder, and 75 (21.6%) reported suicidal ideation with or without attempts. Mood (23.6%) and anxiety (27.4%) disorders had comparable prevalence rates, whereas both disorders occurred together in 43 patients (12.4%). Major depressive disorders (MDDs) had a slightly higher prevalence than bipolar disorders (BPDs) (9.5% vs 6.9%, respectively). Explanatory variables of suicidality included MDD, BPD, panic disorders, and agoraphobia, with BPD and panic disorders being the strongest variables, particularly for active suicidal ideation and suicidal attempts. Discussion In patients with newly diagnosed focal epilepsy, the prevalence of mood, anxiety disorders, and suicidality is higher than in the general population and comparable to those of patients with established epilepsy. Their recognition at the time of the initial epilepsy evaluation is of the essence.

Abstract:Background Major depressive disorder is highly prevalent among persons with epilepsy (PWEs). Between 30% and 50% of PWEs suffer from depression. Many factors contribute to this prevalence, including the psychosocial impact of the diagnosis, restrictions on driving and certain types of work, and adverse effects associated with antiseizure medications. Without proper treatment, depressed PWEs have increased risks for suicide, strained relationships, lowered seizure control, and impairment in functioning. Our objective was to use the existing literature and insights from our experience in treating depression and anxiety in PWEs within an academic mood disorders center. We aimed to provide practical guidance for health care professionals who treat depression in this population. Methods Persons with epilepsy and depression were identified by their treating psychiatrists. Their electronic health records were reviewed and compiled for this report, with a total of 12 included in this review. Records were reviewed regarding antiseizure medications, psychotropic medications, light therapy, psychotherapy, other interventions, and treatment response. Results Based on our review of literature, as well as review of cases of individuals with epilepsy and comorbid psychiatric conditions, we recommend a step-wise evidence-based approach of optimizing psychiatric medication doses, augmenting with additional medication and/or implementing nonpharmacological interventions such as light therapy and psychotherapy. Conclusions In PWEs, improvement in depression, other psychiatric symptoms, and function are the goals of drug and nondrug interventions. Depression care has the potential to significantly improve the quality of life of PWEs and reduce both morbidity and mortality.

Abstract:Objectives: Epilepsy is a common chronic neurological disorder in pediatrics. Depression is an often underdetected comorbidity in childhood epilepsy. This study aimed to adapt the Neurological Disorders Depression Inventory-Epilepsy for Youth (NDDI-E-Y) to the Indonesian language and population, as well as to validate the Indonesian version of NDDI-E-Y (NDDI-E-Y[ID]). Methods: This three-stage study comprised instrument translation, cultural verification, and content validity testing (first stage), pilot testing (second stage), followed by concurrent validity and reliability testing (third stage) of the NDDI-E-Y(ID). Validation was done against the Centre for Epidemiologic Studies Depression Scale - Revised (CESD-R). Content validity, assessed by six experts, was quantified using the content validity index for items (I-CVI) and scale (S-CVI). Participants were adolescents aged 12-17 years diagnosed with any type of epilepsy who completed both instruments. Concurrent validity was evaluated using Spearman's correlation and reliability was measured using Cronbach's alpha. Results: The first stage produced a culturally appropriate NDDI-E-Y(ID). Thirty healthy adolescents and 10 adolescents with epilepsy participated in the second stage. In the third stage, another group of 30 adolescents with epilepsy took part. We obtained I-CVI and S-CVI values averaging 1. The NDDI-E-Y(ID) showed a positive and significant correlation with CESD-R (Spearman's rho = 0.671, p < 0.001). A Cronbach's alpha of 0.928 reflected a high internal consistency. Significance: Based on the results, the NDDI-E-Y(ID) was found to be a valid and reliable screening instrument for detecting depression in youth with epilepsy. Plain language summary: Depression is an under-recognized problem in youth with epilepsy. Currently available depression screening tools are in English, making it less suitable for detection purposes in Indonesia. This study developed and validated the Indonesian version of the NDDI-E-Y, a depression screening tool for youth with epilepsy.
