Abstract:Parkinson’s disease (PD) is a neurodegenerative disorder that severely affects the motor system of patients. Early PD detection will greatly improve the quality of lives. However, existing automatic PD detection systems either rely on customized sensors or require users to perform special activities, using machine learning models whose prediction process is not understandable by medical professionals. In this article, we develop a non-disruptive PD detection system on smartphones based on interpretable prediction models. We design an application named PDGes to passively collect touchscreen and Inertial Measurement Unit data of users’ tapping and swiping actions on smartphones. Meaningful features that reflect finger dexterity , tremor , stiffness , and hand movement are extracted to build the prediction model. To better comprehend the decisions made by the model, we conduct a systematic analysis of feature importance to help validate the conformity of the model with clinical PD diagnosis. We collected data from 108 volunteers to evaluate the performance of PDGes . The experiment results show that PDGes achieves a detection accuracy of more than 94.5% on different smartphones.

Abstract:Abstract Parkinson’s Disease (PD) is the second most common neurodegenerative disorder, affecting more than 1% of the population above 60 years old with both motor and non-motor symptoms of escalating severity as it progresses. Since it cannot be cured, treatment options focus on the improvement of PD symptoms. In fact, evidence suggests that early PD intervention has the potential to slow down symptom progression and improve the general quality of life in the long term. However, the initial motor symptoms are usually very subtle and, as a result, patients seek medical assistance only when their condition has substantially deteriorated; thus, missing the opportunity for an improved clinical outcome. This situation highlights the need for accessible tools that can screen for early motor PD symptoms and alert individuals to act accordingly. Here we show that PD and its motor symptoms can unobtrusively be detected from the combination of accelerometer and touchscreen typing data that are passively captured during natural user-smartphone interaction. To this end, we introduce a deep learning framework that analyses such data to simultaneously predict tremor, fine-motor impairment and PD. In a validation dataset from 22 clinically-assessed subjects (8 Healthy Controls (HC)/14 PD patients with a total data contribution of 18.305 accelerometer and 2.922 typing sessions), the proposed approach achieved 0.86/0.93 sensitivity/specificity for the binary classification task of HC versus PD. Additional validation on data from 157 subjects (131 HC/26 PD with a total contribution of 76.528 accelerometer and 18.069 typing sessions) with self-reported health status (HC or PD), resulted in area under curve of 0.87, with sensitivity/specificity of 0.92/0.69 and 0.60/0.92 at the operating points of highest sensitivity or specificity, respectively. Our findings suggest that the proposed method can be used as a stepping stone towards the development of an accessible PD screening tool that will passively monitor the subject-smartphone interaction for signs of PD and which could be used to reduce the critical gap between disease onset and start of treatment.

Abstract:A paradigm using chronic social stress and multiple measures of the reproductive system were used to assess changes with ageing in the dynamics of endogenous steroid interactions. The 22- to 24-month-old male rats lived for 8 weeks in one of four types of colony, in groups of the same sex or groups of mixed sex including familiar or unfamiliar old males. Measures of endocrinology (circulating steroid levels), behaviour (exploration and sociosexual responses), physiology (body and organ weights and epididymal sperm count) and histology (adrenal and ventral prostate glands) served as markers of activation of the hypothalamic-pituitary-adrenal (HPA) or hypothalamic-pituitary-testicular (HPT) axes. Old males living under stable conditions as familiar same-sex colonies served as the comparison group. Results indicated clear chronic activation of the HPA axis in the unfamiliar all-male colonies and of the HPT axis in the familiar males from mixed-sex colonies, whereas both steroidal axes were stimulated in colonies of unfamiliar males and females. Findings from aged males under chronic stress suggested that reproductive dysfunction may be limited to situations in which activation of the HPA axis occurs without concurrent stimulation of the HPT axis. Data on steroidal interactions from mixed-sex groups suggested that (1) chronic excitation of the HPA failed to suppress function in the reproductive system of the old males, (2) their stress responses were little affected by chronic HPT activation and (3) there was no evidence for stress-induced pathology, even in the vulnerable prostate gland.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract:Parkinson's Disease (PD) is a slowly evolving neurological disease that affects about $text{1}%$ of the population above 60 years old, causing symptoms that are subtle at first, but whose intensity increases as the disease progresses. Automated detection of these symptoms could offer clues as to the early onset of the disease, thus improving the expected clinical outcomes of the patients via appropriately targeted interventions. This potential has led many researchers to develop methods that use widely available sensors to measure and quantify the presence of PD symptoms such as tremor, rigidity and braykinesia. However, most of these approaches operate under controlled settings, such as in lab or at home, thus limiting their applicability under free-living conditions. In this work, we present a method for automatically identifying tremorous episodes related to PD, based on IMU signals captured via a smartphone device. We propose a Multiple-Instance Learning approach, wherein a subject is represented as an unordered bag of accelerometer signal segments and a single, expert-provided, tremor annotation. Our method combines deep feature learning with a learnable pooling stage that is able to identify key instances within the subject bag, while still being trainable end-to-end. We validate our algorithm on a newly introduced dataset of 45 subjects, containing accelerometer signals collected entirely in-the-wild. The good classification performance obtained in the conducted experiments suggests that the proposed method can efficiently navigate the noisy environment of in-the-wild recordings.

A Machine Learning Approach for Early Identification of Prodromal Parkinson's Disease

Abstract:Parkinson's disease (PD) affects approximately 6 million people worldwide. Data analysis of early PD symptoms using machine learning (ML) models may provide an inexpensive, non-invasive, and simple method for the remote diagnosis of early PD. The aim of this project was to analyze voice, computer keystrokes, spiral drawings, and gait data involving PD patients and controls available in public databases using ML models and identify early PD characteristics that are more pronounced than others. An ML model was developed using Random Forest to analyze existing clinical data for PD patients, prodromal PD patients with REM (rapid eye movement) sleep behavior disorder (RBD) symptoms, and non-PD healthy controls. We reviewed and collected data from the UCI (University of California Irvine) Machine Learning Repository, PPMI (Parkinson's Progression Markers Initiative), and Kaggle databases. ML analysis was carried out on voice samples in PD and RBD patients, computer keystroke data, spiral drawings, and gait datasets. The ML prediction model developed may be helpful in improving risk prediction for PD, enabling early intervention and resource prioritization. The ML study suggests that voice analysis is the most robust test, followed by computer keystroke data, spiral drawings, and gait analysis, in that order. Voice is affected even in RBD patients, revealing that it is a sensitive and early measure of prodromal PD. The low accuracy of the analysis indicates that several PD-positive samples may remain undetected and unclassified. Combining all four features, that is, voice analysis, computer keystroke data, spiral drawings, and gait analysis, may improve the overall accuracy.

Abstract:Parkinson's disease is a neurodegenerative disease that can affect a person's movement, speech, dexterity, and cognition. Clinicians primarily diagnose Parkinson's disease by performing a clinical assessment of symptoms. However, misdiagnoses are common. One factor that contributes to misdiagnoses is that the symptoms of Parkinson's disease may not be prominent at the time the clinical assessment is performed. Here, we present a machine-learning approach towards distinguishing between people with and without Parkinson's disease using long-term data from smartphone-based walking, voice, tapping and memory tests. We demonstrate that our attentive deep-learning models achieve significant improvements in predictive performance over strong baselines (area under the receiver operating characteristic curve = 0.85) in data from a cohort of 1853 participants. We also show that our models identify meaningful features in the input data. Our results confirm that smartphone data collected over extended periods of time could in the future potentially be used as a digital biomarker for the diagnosis of Parkinson's disease.

Abstract:Parkinson's disease (PD) is the second most prevalent dementia in the world. Wearable technology has been useful in the computer-aided diagnosis and long-term monitoring of PD in recent years. The fundamental issue remains how to assess the severity of PD using wearable devices in an efficient and accurate manner. However, in the real-world free-living environment, there are two difficult issues, poor annotation and class imbalance, both of which could potentially impede the automatic assessment of PD. To address these challenges, we propose a novel framework for assessing the severity of PD patient's in a free-living environment. Specifically, we use clustering methods to learn latent categories from the same activities, while latent Dirichlet allocation (LDA) topic models are utilized to capture latent features from multiple activities. Then, to mitigate the impact of data imbalance, we augment bag-level data while retaining key instance prototypes. To comprehensively demonstrate the efficacy of our proposed framework, we collected a dataset containing wearable-sensor signals from 83 individuals in real-life free-living conditions. The experimental results show that our framework achieves an astounding 73.48% accuracy in the fine-grained (normal, mild, moderate, severe) classification of PD severity based on hand movements. Overall, this study contributes to more accurate PD self-diagnosis in the wild, allowing doctors to provide remote drug intervention guidance.

Abstract:In the quantification of symptoms of Parkinson's disease (PD), healthcare professional assessments, patient reported outcomes (PRO), and medical device grade wearables are currently used. Recently, also commercially available smartphones and wearable devices have been actively researched in the detection of PD symptoms. The continuous, longitudinal, and automated detection of motor and especially non-motor symptoms with these devices is still a challenge that requires more research. The data collected from everyday life can be noisy and frequently contains artefacts, and novel detection methods and algorithms are therefore needed. 42 PD patients and 23 control subjects were monitored with Garmin Vivosmart 4 wearable device and asked to fill a symptom and medication diary with a mobile application, at home, for about four weeks. Subsequent analyses are based on continuous accelerometer data from the device. Accelerometer data from the Levodopa Response Study (MJFFd) were reanalyzed, with symptoms quantified with linear spectral models trained on expert evaluations present in the data. Variational autoencoders (VAE) were trained on both our study accelerometer data and on MJFFd to detect movement states (e.g., walking, standing). A total of 7590 self-reported symptoms were recorded during the study. 88.9% (32/36) of PD patients, 80.0% (4/5) of DBS PD patients and 95.5% (21/22) of control subjects reported that using the wearable device was very easy or easy. Recording a symptom at the time of the event was assessed as very easy or easy by 70.1% (29/41) of subjects with PD. Aggregated spectrograms of the collected accelerometer data show relative attenuation of low (<5Hz) frequencies in patients. Similar spectral patterns also separate symptom periods from immediately adjacent non-symptomatic periods. Discriminative power of linear models to separate symptoms from adjacent periods is weak, but aggregates show partial separability of patients vs. controls. The analysis reveals differential symptom detectability across movement tasks, motivating the third part of the study. VAEs trained on either dataset produced embedding from which movement states in MJFFd could be predicted. A VAE model was able to detect the movement states. Thus, a pre-detection of these states with a VAE from accelerometer data with good S/N ratio, and subsequent quantification of PD symptoms is a feasible strategy. The usability of the data collection method is important to enable the collection of self-reported symptom data by PD patients. Abstract:Parkinsons Disease is a neurological disorder and prevalent in elderly people. Traditional ways to diagnose the disease rely on in-person subjective clinical evaluations on the quality of a set of activity tests. The high-resolution longitudinal activity data collected by smartphone applications nowadays make it possible to conduct remote and convenient health assessment. However, out-of-lab tests often suffer from poor quality controls as well as irregularly collected observations, leading to noisy test results. To address these issues, we propose a novel time-series based approach to predicting Parkinson's Disease with raw activity test data collected by smartphones in the wild. The proposed method first synchronizes discrete activity tests into multimodal features at unified time points. Next, it distills and enriches local and global representations from noisy data across modalities and temporal observations by two attention modules. With the proposed mechanisms, our model is capable of handling noisy observations and at the same time extracting refined temporal features for improved prediction performance. Quantitative and qualitative results on a large public dataset demonstrate the effectiveness of the proposed approach.

Abstract:Parkinson's disease (PD) is a prevalent neurodegenerative disorder known for its impact on motor neurons, causing symptoms like tremors, stiffness, and gait difficulties. This study explores the potential of vocal feature alterations in PD patients as a means of early disease prediction. This research aims to predict the onset of Parkinson's disease. Utilizing a variety of advanced machine-learning algorithms, including XGBoost, LightGBM, Bagging, AdaBoost, and Support Vector Machine, among others, the study evaluates the predictive performance of these models using metrics such as accuracy, area under the curve (AUC), sensitivity, and specificity. The findings of this comprehensive analysis highlight LightGBM as the most effective model, achieving an impressive accuracy rate of 96% alongside a matching AUC of 96%. LightGBM exhibited a remarkable sensitivity of 100% and specificity of 94.43%, surpassing other machine learning algorithms in accuracy and AUC scores. Given the complexities of Parkinson's disease and its challenges in early diagnosis, this study underscores the significance of leveraging vocal biomarkers coupled with advanced machine-learning techniques for precise and timely PD detection.

Leveraging wearable-systems to find accurate biomarkers for diagnosis has become the need of the hour, especially for a neurodegenerative condition like Parkinson's. This work aims at focusing on early-occurring, common symptoms, such as motor and gait related parameters to arrive at a quantitative analysis on the feasibility of an economical and a robust wearable device. A subset of the Parkinson's Progression Markers Initiative (PPMI), PPMI Gait dataset has been utilised for feature-selection after a thorough analysis with various Machine Learning algorithms. Abstract:Parkinson’s disease (PD) is the second most common neurodegenerative disorder and the heterogeneity of early PD leads to interrater and intrarater variability in observation-based clinical assessment. Thus, objective monitoring of PD-induced motor abnormalities has attracted significant attention to manage disease progression. Here, we proposed a hierarchical architecture to reliably detect abnormal characteristics and comprehensively quantify the multisymptom severity in patients with PD. A novel wearable device was designed to measure motor features in 15 PD patients and 15 age-matched healthy subjects, while performing five types of motor tasks. The abnormality classes of multimodal measurements were recognized by hidden Markov models (HMMs) in the first layer of the proposed architecture, aiming at motivating the evaluation of specific motor manifestations. Subsequently, in the second layer, three single-symptom models differentiated PD motor characteristics from normal motion patterns and quantified the severity of cardinal PD symptoms in parallel. In order to further analyze the disease status, the multilevel severity quantification was fused in the third layer, where machine learning algorithms were adopted to develop a multisymptom severity score. The experimental results demonstrated that the quantification of three cardinal symptoms was highly accurate to distinguish PD patients from healthy controls. Furthermore, strong correlations were observed between the Unified PD Rating Scale (UPDRS) scores and the predicted subscores for tremor ${(R = 0.75,\;P = 1.40e - 3)}$ , bradykinesia ${(R = 0.71,\;P = 2.80e - 3)}$ , and coordination impairments ${(R = 0.69,\;P = 4.20e - 3)}$ , and the correlation coefficient can be enhanced to ${0.88}\,\,{(P = 1.26e - 5)}$ based on the fusion schemes. In conclusion, the proposed assessment architecture holds great promise to push forward the in-home monitoring of clinical manifestations, thus enabling the self-assessment of disease progression.

Abstract:The Federated States of Micronesia and the Republics of Palau and the Marshall Islands signed a series of treaties known as the Compacts of Free Association (COFA) with the United States (U.S.). While the islands became independent nations, certain rights and responsibilities were assigned to the U.S. However since the signing of the treaties, U.S. federal and Hawai‘i state policies have reduced health care coverage for COFA migrants living in the U.S. and its territories. This commentary reports the ongoing efforts of the Micronesian community and its allies to rectify a legacy of unjust policies. We outline the need for sound policies that support appropriate health care to all members of society, and highlight the community’s strength and ability to mobilize for political action. If health is a human right, providing adequate, equitable health care to all members of society is not up for debate.
Accessible, At-Home Detection of Parkinson's Disease via Multi-task Video Analysis

Abstract:Limited access to neurological care leads to missed diagnoses of Parkinson's disease (PD), leaving many individuals unidentified and untreated. We trained a novel neural network-based fusion architecture to detect Parkinson's disease (PD) by analyzing features extracted from webcam recordings of three tasks: finger tapping, facial expression (smiling), and speech (uttering a sentence containing all letters of the alphabet). Additionally, the model incorporated Monte Carlo Dropout to improve prediction accuracy by considering uncertainties. The study participants (n = 845, 272 with PD) were randomly split into three sets: 60% for training, 20% for model selection (hyper-parameter tuning), and 20% for final performance evaluation. The dataset consists of 1102 sessions, each session containing videos of all three tasks. Our proposed model achieved significantly better accuracy, area under the ROC curve (AUROC), and sensitivity at non-inferior specificity compared to any single-task model. Withholding uncertain predictions further boosted the performance, achieving 88.0% (95% CI: 87.7% - 88.4%) accuracy, 93.0% (92.8% - 93.2%) AUROC, 79.3% (78.4% - 80.2%) sensitivity, and 92.6% (92.3% - 92.8%) specificity, at the expense of not being able to predict for 2.3% (2.0% - 2.6%) data. Further analysis suggests that the trained model does not exhibit any detectable bias across sex and ethnic subgroups and is most effective for individuals aged between 50 and 80. This accessible, low-cost approach requiring only an internet-enabled device with a webcam and microphone paves the way for convenient PD screening at home, particularly in regions with limited access to clinical specialists.

Abstract:One of the most catastrophic neurological disorders worldwide is Parkinson's Disease. Along with it, the treatment is complicated and abundantly expensive. The only effective action to control the progression is diagnosing it in the early stage. However, this is challenging because early detection necessitates a large and complex clinical study. This experimental work used Machine Learning techniques to automate the early detection of Parkinson's Disease from clinical characteristics, voice features and motor examination. In this study, we develop ML models utilizing a public dataset of 130 individuals, 30 of whom are untreated Parkinson's Disease patients, 50 of whom are Rapid Eye Movement Sleep Behaviour Disorder patients who are at a greater risk of contracting Parkinson's Disease, and 50 of whom are Healthy Controls. We use MinMax Scaler to rescale the data points, Local Outlier Factor to remove outliers, and SMOTE to balance existing class frequency. Afterwards, apply a number of Machine Learning techniques. We implement the approaches in such a way that data leaking and overfitting are not possible. Finally, obtained 100% accuracy in classifying PD and RBD patients, as well as 92% accuracy in classifying PD and HC individuals.

Abstract:Parkinson's Disease (PD) is a prevalent neurological condition characterized by motor and cognitive impairments, typically manifesting around the age of 50 and presenting symptoms such as gait difficulties and speech impairments. Although a cure remains elusive, symptom management through medication is possible. Timely detection is pivotal for effective disease management. In this study, we leverage Machine Learning (ML) and Deep Learning (DL) techniques, specifically K-Nearest Neighbor (KNN) and Feed-forward Neural Network (FNN) models, to differentiate between individuals with PD and healthy individuals based on voice signal characteristics. Our dataset, sourced from the University of California at Irvine (UCI), comprises 195 voice recordings collected from 31 patients. To optimize model performance, we employ various strategies including Synthetic Minority Over-sampling Technique (SMOTE) for addressing class imbalance, Feature Selection to identify the most relevant features, and hyperparameter tuning using RandomizedSearchCV. Our experimentation reveals that the FNN and KSVM models, trained on an 80–20 split of the dataset for training and testing respectively, yield the most promising results. The FNN model achieves an impressive overall accuracy of 99.11%, with 98.78% recall, 99.96% precision, and a 99.23% f1-score. Similarly, the KSVM model demonstrates strong performance with an overall accuracy of 95.89%, recall of 96.88%, precision of 98.71%, and an f1-score of 97.62%. Overall, our study showcases the efficacy of ML and DL techniques in accurately identifying PD from voice signals, underscoring the potential for these approaches to contribute significantly to early diagnosis and intervention strategies for Parkinson's Disease.

Abstract:Introduction: Accurately and objectively quantifying the clinical features of Parkinson's disease (PD) is crucial for assisting in diagnosis and guiding the formulation of treatment plans. Therefore, based on the data on multi-site motor features, this study aimed to develop an interpretable machine learning (ML) model for classifying the "OFF" and "ON" status of patients with PD, as well as to explore the motor features that are most associated with changes in clinical symptoms. Methods: We employed a support vector machine with a recursive feature elimination (SVM-RFE) algorithm to select promising motion features. Subsequently, 12 ML models were constructed based on these features, and we identified the model with the best classification performance. Then, we used the SHapley Additive exPlanations (SHAP) and the Local Interpretable Model agnostic Explanations (LIME) methods to explain the model and rank the importance of those motor features. Results: A total of 96 patients were finally included in this study. The naive Bayes (NB) model had the highest classification performance (AUC = 0.956; sensitivity = 0.8947, 95% CI 0.6686–0.9870; accuracy = 0.8421, 95% CI 0.6875–0.9398). Based on the NB model, we analyzed the importance of eight motor features toward the classification results using the SHAP algorithm. The Gait: range of motion (RoM) Shank left (L) (degrees) [Mean] might be the most important motor feature for all classification horizons. Conclusion: The symptoms of PD could be objectively quantified. By utilizing suitable motor features to construct ML models, it became possible to intelligently identify whether patients with PD were in the "ON" or "OFF" status. The variations in these motor features were significantly correlated with improvement rates in patients' quality of life. In the future, they might act as objective digital biomarkers to elucidate the changes in symptoms observed in patients with PD and might be used to assist in the diagnosis and treatment of patients with PD.

Abstract:Parkinson's Disease (PD) is the second most common age-related neurological disorder that leads to a range of motor and cognitive symptoms. A PD diagnosis is difficult since its symptoms are quite similar to those of other disorders, such as normal aging and essential tremor. When people reach 50, visible symptoms such as difficulties walking and communicating begin to emerge. Even though there is no cure for PD, certain medications can relieve some of the symptoms. Patients can maintain their lifestyles by controlling the complications caused by the disease. At this point, it is essential to detect this disease and prevent it from progressing. The diagnosis of the disease has been the subject of much research. In our project, we aim to detect PD using different types of Machine Learning (ML), and Deep Learning (DL) models such as Support Vector Machine (SVM), Random Forest (RF), Decision Tree (DT), K-Nearest Neighbor (KNN), and Multi-Layer Perceptron (MLP) to differentiate between healthy and PD patients by voice signal features. The dataset taken from the University of California at Irvine (UCI) machine learning repository consisted of 195 voice recordings of examinations carried out on 31 patients. Moreover, our models were trained using different techniques such as Synthetic Minority Over-sampling Technique (SMOTE), Feature Selection, and hyperparameter tuning (GridSearchCV) to enhance their performance. At the end, we found that MLP and SVM with a ratio of 70:30 train/test split using GridSearchCV with SMOTE gave the best results for our project. MLP performed with an overall accuracy of 98.31%, an overall recall of 98%, an overall precision of 100%, and f1-score of 99%. In addition, SVM performed with an overall accuracy of 95%, an overall recall of 96%, an overall precision of 98%, and f1-score of 97%. The experimental results of this research imply that the proposed method can be used to reliably predict PD and can be easily incorporated into healthcare for diagnosis purposes.

Abstract:Prevalence estimates of Parkinson's disease (PD)—the fastest-growing neurodegenerative disease—are generally underestimated due to issues surrounding diagnostic accuracy, symptomatic undiagnosed cases, suboptimal prodromal monitoring, and limited screening access. Remotely monitored wearable devices and sensors provide precise, objective, and frequent measures of motor and non-motor symptoms. Here, we used consumer-grade wearable device and sensor data from the WATCH-PD study to develop a PD screening tool aimed at eliminating the gap between patient symptoms and diagnosis. Early-stage PD patients (n = 82) and age-matched comparison participants (n = 50) completed a multidomain assessment battery during a one-year longitudinal multicenter study. Using disease- and behavior-relevant feature engineering and multivariate machine learning modeling of early-stage PD status, we developed a highly accurate (92.3%), sensitive (90.0%), and specific (100%) random forest classification model (AUC = 0.92) that performed well across environmental and platform contexts. These findings provide robust support for further exploration of consumer-grade wearable devices and sensors for global population-wide PD screening and surveillance.

Abstract:Parkinson's disease (PD), a severe and progressive neurological illness, affects millions of individuals worldwide. For effective treatment and management of PD, an accurate and early diagnosis is crucial. This study presents a deep learning-based model for the diagnosis of PD using resting state electroencephalogram (EEG) signal. The objective of the study is to develop an automated model that can extract complex hidden nonlinear features from EEG and demonstrate its generalizability on unseen data. The model is designed using a hybrid model, consists of convolutional neural network (CNN), bidirectional gated recurrent unit (Bi-GRU), and attention mechanism. The proposed method is evaluated on three public datasets (Uc San Diego Dataset, PRED-CT, and University of Iowa (UI) dataset), with one dataset used for training and the other two for evaluation. The results show that the proposed model can accurately diagnose PD with high performance on both the training and hold-out datasets. The model also performs well even when some part of the input information is missing. The results of this work have significant implications for patient treatment and for ongoing investigations into the early detection of Parkinson's disease. The suggested model holds promise as a non-invasive and reliable technique for PD early detection utilizing resting state EEG.
