Abstract:Background: High-sensitivity Troponins (hs-cTnT and hs-cTnI) are established biomarkers to identify patients with an acute myocardial infarction. However, data comparing the capacity of these two subtypes in predicting recurrent cardiovascular disease (CVD) events in a population with stable coronary heart disease (CHD) after adjustment for several other modern biomarkers are lacking. Methods: We measured both troponins at baseline in 1068 CHD patients, followed them for 13years, assessed a combined CVD endpoint, and adjusted for multiple traditional and novel risk factors. Results: Both troponins correlated significantly with age, low and high BMI, male gender, statin therapy, and emerging biomarkers (e.g. cystatin C, NT-proBNP, GDF-15, hsCRP or galectin 3). During follow-up of 13years, 267 fatal and non-fatal CVD events occurred. Top quartiles of both troponin concentrations were significantly associated with CVD events compared to the bottom quartile after adjustment for age, gender and established CVD risk factors (hs-cTnT: hazard ratio (HR) 2.54 (95% CI, 1.60-4.03), p for trend: <0.0001; hs-cTnI: HR 2.20 (95% CI, 1.44-3.36), p for trend: <0.0002 and 0.0003). However, after adjustment for other emerging biomarkers, the associations were no longer statistically significant (hs-cTnT: HR 1.63 (95% CI, 0.97-2.73), p for trend: 0.17; hs-cTnI: HR 1.61 (95% CI, 1.00-2.60), p for trend: 0.067). Conclusion: Both troponins are reliable biomarkers of recurrent cardiovascular events, especially if other novel, important markers such as NT-proBNP, GDF-15 and galectin 3 are not available. Nevertheless, a further workup is still needed to explain the complex interaction of biomarkers indicating vascular and myocardial function.
Analysis of composite time-to-event endpoints in cardiovascular outcome trials

Abstract:Composite time-to-event endpoints are commonly used in cardiovascular outcome trials. For example, the IMPROVE-IT trial comparing ezetimibe+simvastatin to placebo+simvastatin in 18,144 patients with acute coronary syndrome used a primary composite endpoint with five component outcomes: (1) cardiovascular death, (2) non-fatal stroke, (3) non-fatal myocardial infarction, (4) coronary revascularization ≥30 days after randomization, and (5) unstable angina requiring hospitalization. In such settings, the traditional analysis compares treatments using the observed time to the occurrence of the first (i.e. earliest) component outcome for each patient. This approach ignores information for subsequent outcome(s), possibly leading to reduced power to demonstrate the benefit of the test versus the control treatment. We use real data examples and simulations to contrast the traditional approach with several alternative approaches that use data for all the intra-patient component outcomes, not just the first.

Abstract:In clinical trials for chronic heart failure (CHF), time to a composite of first hospitalization for worsening heart failure or death is a widely accepted primary efficacy measure. Motivated by lower event rates in recent CHF trials, there are proposals to use endpoints accounting for recurrent heart failure hospitalization (HFH) events (e.g., recurrent HFH events plus death as an additional event) as the primary endpoint to better quantify disease burden and to improve trial efficiency. However, analysis and interpretation of recurrent event endpoints may be complicated by the terminal event death. In a previous simulation study it was shown that the Type I error rate was well-controlled for the analysis of the composite recurrent event endpoint using the negative binomial (NB) model and the Lin-Wei-Yang-Ying (LWYY) model. As these results were based on limited scenarios, we extended the previous simulation study and identified situations where the Type I error rate is no longer controlled for the analysis of the composite recurrent event endpoint using the LWYY model, despite a neutral effect on HFH and a detrimental effect on death of the experimental treatment. Therefore, depending on clinical settings, the LWYY model should be applied and interpreted with more caution.

Nonparametric tests of treatment effect based on combined endpoints for mortality and recurrent events

Abstract:Terminal events are commonly combined with other outcomes to improve the power for detecting treatment effects in clinical studies. This manuscript explores novel ways to combine information on terminal and recurrent events in constructing two-sample tests. Existing approaches follow either a time-to-first event analysis approach or a recurrent event modeling approach. Nonparametric recurrent event analyses are often restricted by independence assumptions on gap times between events. Although time-to-first event analyses are not subject to this restriction, they discard information that occurs beyond the initial event and are much less powerful for detecting treatment differences. We develop two new approaches for determining treatment effects, motivated by less restrictive assumptions of time-to-first event analyses that combine information from multiple follow-up intervals. The first testing procedure pools (correlated) short-term τ -restricted outcomes from prespecified intervals starting at times t(k), k = 1, . . . , b, and compares estimated τ -restricted mean survival across treatment groups from this combined dataset. The second procedure calculates conditional τ-restricted means from those at risk at times t(k), k = 1, . . . , b, and compares the area under a function of these by treatment. Variances calculations, taking into account correlation of short-term outcomes within individuals, linearize random components of the test statistics following Woodruff (1971. A simple method for approximating the variance of a complicated estimate. Journal of the American Statistical Association 66, 411-414) and more recently Williams (1995. Simulations compare the finite sample performance of our tests to the robust proportional rates model proposed by Lin and others (2000. Early and late recurrent cardiovascular events among high-risk patients with an acute coronary syndrome: Meta-analysis of phase III studies and implications on trial design

Abstract:Background: Despite low-density lipoprotein cholesterol-lowering therapies and other standard-of-care therapy, there remains a substantial residual atherosclerotic risk among patients with an acute coronary syndrome (ACS). This study aims to estimate the risk of early and late recurrent major adverse cardiovascular events (MACE) and address its implications on trial design. Methods: A literature search was performed to collect phase III interventional trials on high-risk ACS patients. Pooled event rates at 90 and 360 days were estimated by fitting random-effects models using the DerSimonian-Laird method. Under the assumption of a total sample size of 10,000 and 1:1 allocation at a one-sided alpha of 0.025 using the log-rank test, the relationship between power and relative risk reduction (RRR) or absolute risk reduction (ARR) was explored for early versus late MACE endpoint. Results: Seven trials representing 82,727 recent ACS patients were analyzed. The pooled rates of recurrent MACE were 4.1% and 8.3% at 90 and 360 days. Approximately 49% of events occurred within the first 90 days. Based on the estimated risks at 90 and 360 days, to attain 90% statistical power, a lower magnitude of RRR is required for late MACE than early MACE (22% vs. 30%), whereas a lower magnitude of ARR is required for early MACE than late MACE (1.2% vs. 1.8%). Conclusion: The initial 90-day window after ACS represents a vulnerable period for recurrent events. From a trial design perspective, determining a clinically important benefit by RRR versus ARR may influence the decision between early and late MACE as the study endpoint.

Abstract:Although increasingly used as a data resource for assembling cohorts, electronic health records (EHRs) pose many analytic challenges. In particular, a patient's health status influences when and what data are recorded, generating sampling bias in the collected data. In this article, we consider recurrent event analysis using EHR data. Conventional regression methods for event risk analysis usually require the values of covariates to be observed throughout the follow-up period. In EHR databases, time-dependent covariates are intermittently measured during clinical visits, and the timing of these visits is informative in the sense that it depends on the disease course. Simple methods, such as the last-observation-carried-forward approach, can lead to biased estimation. On the other hand, complex joint models require additional assumptions on the covariate process and cannot be easily extended to handle multiple longitudinal predictors. By incorporating sampling weights derived from estimating the observation time process, we develop a novel estimation procedure based on inverse-rate-weighting and kernel-smoothing for the semiparametric proportional rate model of recurrent events. The proposed methods do not require model specifications for the covariate processes and can easily handle multiple time-dependent covariates. Abstract:Clinical trials in acute care settings, particularly those involving small populations or high-mortality contexts, present unique challenges in design and analysis. This review explores novel statistical approaches and methodological considerations for such trials, with a focus on cardiovascular therapies. We discuss the concept of "small" sample sizes and their limitations and cover various analytical frameworks, including frequentist and Bayesian approaches, emphasizing their implications for result interpretation and reproducibility. We examine endpoints such as "days alive and free" (DAF*), which combines mortality and morbidity measures, the Win Ratio for hierarchical endpoints, and ordinal scales that capture detailed patient outcomes. These methods potentially increase statistical power and provide more clinically relevant measures compared to traditional binary outcomes; an extensive use of simulations is used to clarify this point. The use of longitudinal ordinal models is presented as a promising method to capture complex patient trajectories over time, offering insights into treatment effects at various disease stages. We also address the potential of adaptive platform trials for rare conditions, allowing for more efficient use of limited patient populations. This overview aims to guide researchers and clinicians in selecting optimal trial designs and analytical strategies, ultimately improving the quality, efficiency, and interpretability of evidence in acute care cardiology.

The design and analysis of randomized trials with recurrent events

Abstract:This paper describes a method for planning the duration of a randomized parallel group study in which the response of interest is a potentially recurrent event. At the design stage we assume patients accrue at a constant rate, we model events via a homogeneous Poisson process, and we utilize an independent exponential censoring mechanism to reflect loss to follow-up. We derive the appropriate study duration to ensure satisfaction of power requirements for the effect size of interest under a Poisson regression model. An application to a kidney transplant study illustrates the potential savings of the Poisson-based design relative to a design based on the time to the first event. Revised design criteria are also derived to accommodate overdispersed Poisson count data. We examine the frequency properties of two non-parametric tests recently proposed by Lawless and Nadeau for trials based on the above design criteria. In simulation studies involving homogeneous and non-homogeneous Poisson processes they performed well with respect to their type I error rate and power. Results from supplementary simulation studies indicate that these tests are also robust to extra-Poisson variation and to clustering in the event times, making these tests attractive in their generality. We illustrate both tests by application to data from a completed kidney transplant study.
A win ratio approach to the re-analysis of Multiple Risk Factor Intervention Trial

Abstract:Background: Composite outcomes, which combine multiple types of clinical events into a single outcome, are common in clinical trials. The usual analysis considers the time to first occurrence of any event in the composite. The major criticisms of such an approach are (1) this implicitly treats the outcomes as if they were of equal importance, but they often vary in terms of clinical relevance and severity, (2) study participants often experience more than one type of event, and (3) often less severe events occur before more severe ones, but the usual analysis disregards any information beyond that first event. Methods: A novel approach, referred to as the win ratio, which addresses the aforementioned criticisms of composite outcomes, is illustrated with a re-analysis of data on fatal and non-fatal cardiovascular disease time-to-event outcomes reported for the Multiple Risk Factor Intervention Trial. In this trial, 12,866 participants were randomized to a special intervention group ( n = 6428) or a usual care ( n = 6438) group. Non-fatal outcomes were ranked by risk of cardiovascular disease death up to 20 years after trial. In one approach, participants in the special intervention and usual care groups were first matched on coronary heart disease risk at baseline and time of enrollment. Each matched pair was categorized as a winner or loser depending on which one experienced a cardiovascular disease death first. If neither died of cardiovascular disease causes, they were evaluated on the most severe non-fatal outcome. This process continued for all the non-fatal outcomes. A second win ratio statistic, obtained from Cox partial likelihood, was also estimated. This statistic provides a valid estimate of the win ratio using multiple events if the marginal and conditional survivor functions of each outcome satisfy proportional hazards. Loss ratio statistics (inverse of win ratios) are compared to hazard ratios from the usual first event analysis. A larger 11-event composite was also considered. Results: For the 7-event cardiovascular disease composite, the previously reported first event analysis based on 581 events in the special intervention group and 652 events in the usual care group yielded a hazard ratio (95% confidence interval) of 0.89 (0.79–0.99), compared to 0.86 (0.77–0.97) and 0.91 (0.81–1.02) for the severity ranked estimates. Results for the 11-event composite also confirmed the findings of the first event analysis. Conclusion: The win ratio analysis was able to leverage information collected past the first experienced event and rank events by severity. The results were similar to and confirmed previously reported traditional first event analysis. The win ratio statistic is a useful adjunct to the traditional first event analysis for trials with composite outcomes.

Comparing Analytical Methods for Composite Endpoints in Clinical Trials: Insights from the Vericiguat Global Study in Subjects with Heart Failure with Reduced Ejection Fraction (VICTORIA) Trial

Abstract:Background and aims: In the VICTORIA trial of participants with heart failure (HF) and reduced ejection fraction, vericiguat (V) reduced the primary composite outcome [time to first HF hospitalization (HFH) or cardiovascular death (CVD)] (897 events) compared to placebo (P) (972 events) (hazard ratio, 0.90; 95% confidence interval [CI], 0.82-0.98; p=0.02). In this prespecified secondary analysis, we applied the weighted composite endpoint (WCE) and the win ratio (WR) methods to provide complementary assessments of treatment effect. Methods and results: The WCE method estimated the mean HFH-adjusted survival based on prespecified weights from a Delphi panel of the VICTORIA executive committee and national leaders: mild (weight per event: 0.39), moderate (0.5) or severe (0.67) HFH, and CVD (1.0). The unmatched WR was estimated for the descending hierarchy of CVD, then recurrent HFH. The WCE used all 3412 primary clinical events: 875(V:416/P:459) severe HFH, 1614(767/847) moderate HFH and 68(38/30) mild HFH, 855(414/441) CVD. Improved HFH-adjusted survival occurred with vericiguat [mean 78.2% vs. 75.6%; difference (95% CI): 2.4% (1.7%-3.2%); p<0.0001]. Based on the comparison of 6,375,624 pairs, the WR of 1.13 (95% CI, 1.03-1.24, p=0.01) also indicated improved clinical outcomes with vericiguat. Conclusions: The results of the WCE and WR methods were consistent with the primary analysis of the time to first HFH or CVD. Whereas both WCE and WR assessed recurrent events, the WCE allowed inclusion of all recurrent events, insights on the severity of HFH events, and an absolute measure of the participant-treatment experience. This approach complements conventional assessment, better informing consumers of new therapeutics and future trial designs.
Use of the Win Ratio for Analysis of Stroke Trials: Description, Illustration, and Planned Use in the Second European Carotid Surgery Trial (ECST-2)

Abstract:Stroke, Ahead of Print. Randomized trials in stroke often focus on outcomes beyond a single clinical event. Trials of stroke prevention commonly use composite outcomes that include multiple components (eg, death, stroke, or myocardial infarction). A major limitation is that all events count equally but may differ markedly in terms of clinical severity. Trials in acute stroke often use ordinal outcomes or scale scores. Limitations include the requirement for statistical assumptions and the difficulty of handling the competing risk of death. We introduce the win ratio as an alternative method. It works by placing components of a composite into a hierarchy, whereby clinically more important outcomes take priority over less important ones. We illustrate how it works using data from 2 major stroke trials: the ICSS (International Carotid Stenting Study, a trial in stroke prevention) and the MR CLEAN (Multicenter Randomized Clinical Trial of Endovascular Treatment for Acute Ischemic Stroke in the Netherlands). Potential benefits of the win ratio approach include the possibility to (1) emphasize the clinically most important outcomes, (2) combine components of different outcome types into a composite (eg, a mixture of time-to-event, continuous, and categorical), and (3) naturally handle the competing risk of death in analyses of quantitative outcomes. The win ratio will be used in the upcoming analysis of the ECST-2 (Second European Carotid Surgery Trial), which has a hierarchical primary outcome of (1) time to perioperative death, fatal stroke, or fatal myocardial infarction (most important); (2) time to nonfatal stroke; (3) time to nonfatal myocardial infarction (excluding silent infarcts); and (4) new silent cerebral infarct on brain imaging (least important). The win ratio provides a useful clinically relevant method for analyzing trial outcomes. It has some advantages over conventional methods, and we recommend its wider application in future stroke trials.

Abstract:STUDY OBJECTIVE: The purpose of this paper is to give an overview and comparison of different easily applicable statistical techniques to analyse recurrent event data.SETTING: These techniques include naive techniques and longitudinal techniques such as Cox regression for recurrent events, generalised estimating equations (GEE), and random coefficient analysis. The different techniques are illustrated with a dataset from a randomised controlled trial regarding the treatment of lateral epicondylitis.MAIN RESULTS: The use of different statistical techniques leads to different results and different conclusions regarding the effectiveness of the different intervention strategies.CONCLUSIONS: If you are interested in a particular short term or long term result, simple naive techniques are appropriate. However, if the development of a particular outcome is of interest, statistical techniques that consider the recurrent events and additionally corrects for the dependency of the observations are necessary.

Abstract:During patient follow-up in a randomized trial, some deaths may occur. Where death (or noncardiovascular death) is not part of an outcome of interest it is termed a competing risk. Conventional analyses (eg, Cox proportional hazards model) handle death similarly to other censored follow-up. Patients still alive are unrealistically assumed to be representative of those who died. The Fine and Gray model has been used to handle competing risks, but is often used inappropriately and can be misleading. We propose an alternative multiple imputation approach that plausibly accounts for the fact that patients who die tend also to be at high risk for the (unobserved) outcome of interest. This provides a logical framework for exploring the impact of a competing risk, recognizing that there is no unique solution. We illustrate these issues in 3 cardiovascular trials and in simulation studies. We conclude with practical recommendations for handling competing risks in future trials.

Abstract:Suppression of disability progression is an important goal in the treatment of multiple sclerosis (MS). Randomized clinical trials in MS frequently use the time to the first confirmed disability progression (CDP) on the ordinal Expanded Disability Status Scale (EDSS) as an endpoint. However, especially in progressive forms of MS, patients may experience repeated CDP events over time. We investigate first how recurrent disability progression events can be defined, and consider then recurrent event analyses that could increase power and improve clinical interpretation of results. Data on disease activity and results from two simulation studies which compare analyses of the time to the first event with recurrent event analyses (including negative binomial, Andersen-Gill and Lin-Wei-Ying-Yang models) are presented to demonstrate challenges in defining disability progression and to identify analysis methods suitable for treatment comparisons in MS trials with disability progression endpoints. This article is based on the Master Thesis by Alexandra B\=uhler at the University of Ulm which was written under the supervision of Jan Beyersmann, Marcel Wolbers, Fabian Model and Qing Wang.
Advanced models for predicting event occurrence in event-driven clinical trials accounting for patient dropout, cure and ongoing recruitment

Abstract:We consider event-driven clinical trials, where the analysis is performed once a pre-determined number of clinical events has been reached. For example, these events could be progression in oncology or a stroke in cardiovascular trials. At the interim stage, one of the main tasks is predicting the number of events over time and the time to reach specific milestones, where we need to account for events that may occur not only in patients already recruited and are followed-up but also in patients yet to be recruited. Therefore, in such trials we need to model patient recruitment and event counts together. In the paper we develop a new analytic approach which accounts for the opportunity of patients to be cured, as well as for them to dropout and be lost to follow-up. Recruitment is modelled using a Poisson-gamma model developed in previous publications. When considering the occurrence of events, we assume that the time to the main event and the time to dropout are independent random variables, and we have developed a few advanced models with cure using exponential, Weibull and log-normal distributions. This technique is supported by well developed, tested and documented software. The results are illustrated using simulation and a real dataset with reference to the developed software.

In a longitudinal randomized study where multiple time-to-event outcomes are collected, the overall treatment effect may be quantified by a composite endpoint defined as the time to the first occurrence of any of the selected events including death. The reverse counting process (RCP) was recently proposed to extend the restricted mean survival time (RMST) approach with an advantage of utilizing observations of events beyond the "first-occurrence" endpoint. However, the interpretation may be questionable because RCP treats all events equally without considering their different associations with the overall survival. In this work, we propose a novel approach, the weighted reverse counting process (WRCP), to construct a weighted composite endpoint to evaluate the overall treatment effect. A multi-state transition model is used to model the association between events, and an adaptive weighting algorithm is developed to determine the weight for individual endpoints based on the association between the nonfatal endpoints and death using the trial data. Simulation studies are presented to compare the performance of WRCP with RCP, log-rank test and RMST approach. The results show that WRCP is a powerful and robust method to detect the overall treatment effect while controlling the clinically false positive rate well across different simulation scenarios.

Abstract:The win ratio method for analysing a composite clinical hierarchy of outcomes is growing in popularity especially in cardiovascular trials. This article gives a perspective on its use so far and the issues derived from that experience. Specifically, it focuses on the limitations of a conventional composite outcome; how does the win ratio work, what does it mean, and how to display its findings; guidance on choosing an appropriate clinical hierarchy of outcomes including clinical events, quantitative outcomes, and other options; the additional value of the win difference as a measure of absolute benefit: extension to stratified win ratio, subgroup analysis, matched win ratio, and covariate adjustment; determining trial size for a win ratio outcome; specific insights such as adaptive designs, use of repeat events, and use of margins and time averages for quantitative outcomes; a critique of potential misuses; availability of statistical software; and a statistical appendix on the methodological details. Throughout, each principle is illustrated by examples from specific cardiology trials. The article concludes with a set of recommendations for future use of the win ratio.

Analysis of Residual Risk and Recurrent Event Trends Following Acute Coronary Syndrome: A Cohort Study

Abstract:Background: A comprehensive real-world analysis of residual risk factors for recurrent major adverse cardiovascular events (MACE) following hospital admission for acute coronary syndrome (ACS) is lacking. The objectives of this study were: 1) to describe population trends for outcomes, risk factors, and medication prescribing patterns post-ACS and 2) to identify factors associated with recurrent MACE. Methods: A retrospective cohort study of 4,884 post-ACS patients admitted at a large integrated healthcare system between 2015-2021 was performed to investigate the relationship between recurrent MACE (ACS, cerebrovascular events, all-cause mortality, and unplanned revascularization), modifiable risk factor trends, and medical therapy prescribing patterns. Patients were separated into 2 cohorts based upon whether they experienced recurrent MACE following the initial hospitalization. Data were obtained via programmatic extraction from the electronic health record. Descriptive statistics were performed. Generalized linear models were used to assess risk factor trends and pairwise comparisons were performed between time points. Results: Median length of follow-up after ACS was 31.2 months. Recurrent MACE occurred in 28% of patients. Despite 95.9% of all patients receiving prescriptions for high-intensity statins, >40% did not achieve LDL-C goal of <70 mg/dL, and only 11.6% and 2.6% of all patients were prescribed ezetimibe or proprotein convertase subtilisin kexin type 9 inhibiting monoclonal antibodies, respectively. Although >30.0% of patients had triglycerides ≥150 mg/dL at all time points, ≤6% were prescribed any non-statin triglyceride lowering therapy and 0.6% were prescribed icosapent ethyl. Persistent hypertriglyceridemia (≥150 mg/dL) was associated with recurrent MACE at 6-, 12-, and 24-months post-ACS (p<0.05), and the relative risk ranged between 1.20-1.35 at those timepoints. Conclusions: This study demonstrates the need for more comprehensive post-ACS care to address residual cardiometabolic risk factors and suboptimal prescribing patterns for indicated therapies. Targeted strategies are needed to address hypertriglyceridemia for cardiovascular risk reduction.

Abstract:OBJECTIVES: Systematic understanding is lacking regarding how current trials handle repeated measure data and the extent to which appropriate statistical methods are used for such data set. This study investigated the current practice of analyzing the repeated measure data among randomized controlled trials (RCTs).STUDY DESIGN AND SETTING: We searched the Core Clinical Journals indexed in PubMed for RCTs published in 2019 and included a continuous primary outcome with repeated measures. We randomly sampled RCTs from the eligible trials. Team of methods trained investigators screened studies for eligibility and collected data using the pilot-tested, standardized questionnaires. We thoroughly documented statistical analyses of the continuous primary outcome with repeated measures and particularly recorded how statistically advanced methods were used to handle these repeated measures.RESULTS: In total, 200 trials were included. Of these trials, the mean number of repeated measures for the continuous primary outcome was 5.46 (SD = 3.4); 58 (29.0%) trials did not specify the time point of primary outcome in the method; 113 (56.5%) trials did not use statistically advanced methods for handling repeated measure data in the primary analyses. Among187 trials included the baseline values, 88 (47.1%) trials did not adjust for outcome value at baseline. Among 87 trials using statistically advanced methods, 49 (56.3%) did not specify correlation structure for model.CONCLUSIONS: The statistical analyses of repeatedly measured continuous outcomes in RCTs need substantial improvements. Careful planning of the primary outcome and the use of statistically advanced methods for analyzing data are warranted.

Abstract:Background and aims: Utilising real-world data, we quantified the burden of cardiovascular risk factors and long-term residual risk of atherothrombotic events among routine care cohorts with coronary (CAD) or peripheral (PAD) artery disease or ischemic stroke (IS) on guideline-recommended antiplatelet monotherapy (APMT). Methods: Retrospective cohort study using data (2010-2020) from the United Kingdom Clinical Practice Research Datalink (CPRD) and Hospital Episode Statistics, including adults with CAD, PAD or IS who were first prescribed APMT (CAD/IS: aspirin; PAD: clopidogrel). Primary outcomes (recurrent events): major adverse cardiovascular events (MACE) for CAD/PAD/IS cohorts, major adverse limb events (MALE) for PAD. Results: 266,478 CAD, 13,162 PAD, and 14,788 IS patients were included (mean age: 71 years; women 37.7%-47.5 %). Risk factor burden was high and attainment of recommended goals was low. There were 73,691, 3,121 and 7,137 MACE among CAD, PAD and IS patients, respectively (median follow-up: 89.9, 42.4 and 75.9 months, respectively), and 4,767 MALE among PAD patients. MACE incidence rate per 1000 person-years was higher in IS (268.7; 95%CI 265.3-272.0) than CAD (92.9; 95%CI 92.5-93.4) or PAD cohorts (97.2; 95%CI 94.6-99.8). MALE incidence rate was 195.9 (95%CI 192.2-199.6) per 1000 person-years. IS patients presented a lower rate of hospitalisations and longer time-to-first hospitalisation, but once hospitalised, they had a longer length-of-stay. PAD patients had the highest hospitalisation rate. Conclusions: Among a contemporary cohort with cardiovascular disease on APMT, long-term residual atherothrombotic risk remains high despite being on APMT. Greater attention to risk factor control and use of appropriate evidence-based therapy is required to reduce residual risk among this very high-risk population.
