Abstract:Rationale: Structure and function of the microvasculature provides critical information about disease state, can be used to identify local regions of pathology, and has been shown to be an indicator of response to therapy. Improved methods of assessing the microvasculature with non-invasive imaging modalities such as ultrasound will have an impact in biomedical theranostics. Ultrasound localization microscopy (ULM) is a new technology which allows processing of ultrasound data for visualization of microvasculature at a resolution better than allowed by acoustic diffraction with traditional ultrasound systems. Previous application of this modality in brain imaging has required the use of invasive procedures, such as a craniotomy, skull-thinning, or scalp removal, all of which are not feasible for the purpose of longitudinal studies. Methods: The impact of ultrasound localization microscopy is expanded using a 1024 channel matrix array ultrasonic transducer, four synchronized programmable ultrasound systems with customized high-performance hardware and software, and high-performance GPUs for processing. The potential of the imaging hardware and processing approaches are demonstrated in-vivo. Results: Our unique implementation allows asynchronous acquisition and data transfer for uninterrupted data collection at an ultra-high fixed frame rate. Using these methods, the vasculature was imaged using 100,000 volumes continuously at a volume acquisition rate of 500 volumes per second. With ULM, we achieved a resolution of 31 µm, which is a resolution improvement on conventional ultrasound imaging by nearly a factor of ten, in 3-D. This was accomplished while imaging through the intact skull with no scalp removal, which demonstrates the utility of this method for longitudinal studies. Conclusions: The results demonstrate new capabilities to rapidly image and analyze complex vascular networks in 3-D volume space for structural and functional imaging in disease assessment, targeted therapeutic delivery, monitoring response to therapy, and other theranostic applications.

Abstract:Angiogenesis has been known as a hallmark of solid tumor cancers for decades, yet ultrasound has been limited in its ability to detect the microvascular changes associated with malignancy. Here, we demonstrate the potential of 'ultrasound localization microscopy' applied volumetrically in combination with quantitative analysis of microvascular morphology, as an approach to overcome this limitation. This pilot study demonstrates our ability to image complex microvascular patterns associated with tumor angiogenesis in-vivo at a resolution of tens of microns - substantially better than the diffraction limit of traditional clinical ultrasound, yet using an 8 MHz clinical ultrasound probe. Furthermore, it is observed that data from healthy and tumor-bearing tissue exhibit significant differences in microvascular pattern and density. Results suggests that with continued development of these novel technologies, ultrasound has the potential to detect biomarkers of cancer based on the microvascular 'fingerprint' of malignant angiogenesis rather than through imaging of blood flow dynamics or the tumor mass itself.

Abstract:Angiogenesis, the formation of new vessels, is one of the key mechanisms in tumor development and an appealing target for therapy. Non-invasive, high-resolution, high sensitivity, quantitative 3D imaging techniques are required to correctly depict tumor heterogeneous vasculature over time. Ultrafast Doppler was recently introduced and provides an unprecedented combination of resolution, penetration depth and sensitivity without requiring any contrast agents. The technique was further extended to 3D with Ultrafast Doppler Tomography (UFD-T). In this work, UFD-T was applied to the monitoring of tumor angiogenesis in vivo providing structural and functional information at different stages of development. UFD-T volume renderings showed that our murine model's vasculature stems from pre-existing vessels and sprouts to perfuse the whole volume as the tumor grows until a critical size is reached. Then, as the network becomes insufficient, the tumor core is no longer irrigated because the vasculature is mainly concentrated in the periphery. In addition to spatial distribution and growth patterns, UFD-T allowed a quantitative analysis of vessel size and length, revealing that the diameter-distribution of vessels remained relatively constant throughout tumor growth. The network is dominated by small vessels at all stages of tumor development with more than 74% of the vessels less than 200 $\mu$m in diameter. This study also showed that cumulative vessel length is more closely related to tumor radius than volume, indicating that the vascularization becomes insufficient when a critical mass is reached. UFD-T was also compared with dynamic contrast-enhanced ultrasound (DCE-US) and shown to provide complementary information regarding the link between structure and perfusion. In conclusion, UFD-T is capable of an in vivo quantitative assessment of the development of tumor vasculature (vessels with blood speed >1mm/s (sensitivity limit) assessed with a resolution limit of 80 $\mu$m) in 3D. The technique has very interesting potential as a tool for treatment monitoring, response assessment and treatment planning for optimal drug efficiency.

Abstract:Neovascularization together with aberrant vasculature is a hallmark of glioblastomas. Understanding tumor vasculature can have a pivotal role in planning therapeutics approaches. The objective of this study was to investigate morphological, vasculature and blood flow changes induced by aggressive tumor growth, using in vivo imaging - magnetic resonance imaging (MRI) and functional ultrasound (fUS) imaging in a mouse model of orthotopic glioma. Female NMRI nude mice were xenografted orthotopically with U-87 MG, a human glioblastoma, cells (5 × 104) at the age of 8 weeks. Four weeks after implantation the mice we anesthetized with isoflurane and scanned with 11.7 T small animal MRI (Bruker) for tumor volumetry. Thereafter, the mice were imaged with Iconeus One imaging system (Iconeus, Paris, France) for structural information of brain vasculature and relative cerebral blood volume (rCBV) changes. High resolution vascular imaging was performed after intravenous injection of microbubble contrast agent (SonoVue, sulphur hexafluoride microbubbles). The average tumor size was 58.8 ± 27.6 mm3 (mean ± SD, n=7) four weeks after implantation. Aberrant vasculature could be visualized with structural fUS imaging as and the necrotic core of the tumor could be observed. A reduced rCBF was observed in ipsilateral hemisphere on the relative power doppler signal timeseries as a lower amplitude and faster decay compared to contralateral side. As a summary, in vivo fUS imaging is a noninvasive tool to visualize vascular changes induced by aggressive brain tumor growth which are not detectable with conventional imaging methods (CT, MRI, PET, SPECT) and can provide a novel readout for efficacy studies. Abstract:Cancer treatment response has traditionally been monitored by measuring tumor volume without considering tumor microvascular heterogeneity (TMH), which is known to profoundly influence treatment outcomes. As TMH presents as distinct vascular subtypes with different functions and structures, we employed super-resolution ultrasound imaging (SRUS) to resolve various tumor vascular subgroups. The feasibility of characterizing tumor vascular subtypes in evaluating anti-angiogenic treatment was also validated. Compared with traditional vascular parameters, the evaluation of multiple vascular subtypes provides a new approach to investigating the complex vascular dynamics induced by antiangiogenic drugs. It is reasonable to believe that tumor microvascular categorization allows the identification of at-risk vascular subgroups with unfavorable functional properties. In summary, this preliminary study of SRUS in the non-invasive characterization of microvascular subtypes sheds light on personalized cancer management.
Ultrasound Super-Resolution Imaging Enables Multiparameter Analyses of Tumor Angiogenesis During Growth

Abstract:Angiogenesis plays a crucial role in the process of tumor genesis. However, histological study limits the ability to repeatedly and invasively probe dynamic changes in tumor microvessels and functions over prolonged periods for patients. Ultrasound localization microscopy (ULM) has achieved noninvasive and non-radiation microvascular imaging with superior spatial resolution (8 µm). Here, we present 3D ULM imaging of microvasculature. The functional activity of blood vessels and vascular structure in different regions of the same tumor were quantified and compared during tumor growth. In the periphery and core areas of the tumor, we identified a rise vessel density at the beginning. During tumor growth, a slight but not significant increase in flow velocity is observed in both areas, and microvasculature in the periphery area consistently exhibits a more chaotic vascular network relative to the vessels in the core area. We emphasize the non-invasive application of ULM to monitor vascular dynamics and morphometric changes over wide fields during tumor growth. This unprecedented exploration of ultrasound super-resolution imaging strategy has notable potential for ultrahigh resolution assessment of treatment effect.

Abstract:Super-resolution ultrasound imaging (SR-US) is a new technique that breaks the diffraction limit and allows visualization of microvascular structures down to tens of microns. The objective of this presentation is to briefly introduce SR-US imaging and use in various small animal models. Engineering challenges and ongoing opportunities will also be introduced. SR-US is predominantly a software advance that can be integrated with both clinical and preclinical US imaging platforms. After injection of an intravascular microbubble (MB) contrast agent, a time-series of contrast-enhanced ultrasound (CEUS) images are acquired. Spatiotemporal filtering of the CEUS images helps remove the tissue clutter signal and segmentation of the MB signal. Precise localization of the center of each agent is then determined. This process is repeated for each CEUS image in the time series. The resultant SR-US images depict fine microvascular detail typically unresolved using traditional CEUS imaging methods. During this presentation, we will explore use of methods to help improve the MB detection and localization process in space and time like with the use of motion correction and machine learning algorithms. We will also present a series of preclinical examples highlighting the potential of this new technology in cancer research. Results demonstrate that SR-US imaging depicts microvascular structures not visible using traditional CEUS methods. The utility of this new diagnostic tool will play more of a role in theranostics over the next decade.

Abstract:Ultrasound microvessel imaging (UMI), when applied with ultrafast planewave acquisitions, has demonstrated superior blood signal sensitivity in comparison to conventional Doppler imaging. Here we propose a high spatial resolution and ultra-sensitive UMI that is based on conventional line-by-line high-frequency ultrasound imagers and singular value decomposition (SVD) clutter filtering for the visualization and quantification of tumor microvasculature and perfusion. The technique was applied to a chicken embryo tumor model of renal cell carcinoma that was treated with two FDA-approved anti-angiogenic agents at clinically relevant dosages. We demonstrate the feasibility of 3D evaluation with UMI to achieve highly sensitive detection of microvasculature using conventional line-by-line ultrasound imaging on a preclinical and commercially available high-frequency ultrasound device without software or hardware modifications. Quantitative parameters (vascularization index and fractional moving blood volume) derived from UMI images provide significantly improved evaluation of anti-angiogenic therapy response as compared with conventional power Doppler imaging, using histological analysis and immunohistochemistry as the reference standard. This proof-of-concept study demonstrates that high-frequency UMI is a low-cost, contrast-agent-free, easily applicable, accessible, and quantitative imaging tool for tumor characterization, which may be very useful for preclinical evaluation and longitudinal monitoring of anti-cancer treatment.

Doppler Slicing for Ultrasound Super-Resolution Without Contrast Agents

Abstract:Abstract Much of the information needed for diagnosis and treatment monitoring of diseases like cancer and cardiovascular disease is found at scales below the resolution limit of classic ultrasound imaging. Recently introduced vascular super-localization methods provide more than a ten-fold improvement in spatial resolution by precisely estimating the positions of microbubble contrast agents. However, most vascular ultrasound scans are currently performed without contrast agents due to the associated cost, training, and post-scan monitoring. Here we show that super-resolution ultrasound imaging of dense vascular structures can be achieved using the natural contrast of flowing blood cells. Instead of relying on separable targets, we used Fourier-based decomposition to separate signals arising from the different scales of vascular structures while removing speckle noise using multi-ensemble processing. This approach enabled the use of compressed sensing for super-resolution imaging of the underlying vascular structures, improving resolution by a factor of four. Reconstruction of ultrafast mouse brain scans revealed details that could not be resolved in regular Doppler images, agreeing closely with bubble-based super-localization microscopy of the same fields of view. By combining multi-ensemble Doppler acquisitions with narrowband Fourier decomposition and computational super-resolution imaging, this approach opens new opportunities for affordable and scalable super-resolution ultrasound imaging.

Abstract:Abstract Intravital 2P-microscopy enables the longitudinal study of brain tumor biology in superficial mouse cortex layers. Intravital microscopy of the white matter, an important route of glioblastoma invasion and recurrence, has not been feasible, due to low signal-to-noise ratios and insufficient spatiotemporal resolution. Here, we present an intravital microscopy and artificial intelligence-based analysis workflow (Deep3P) that enables longitudinal deep imaging of glioblastoma up to a depth of 1.2 mm. We find that perivascular invasion is the preferred invasion route into the corpus callosum and uncover two vascular mechanisms of glioblastoma migration in the white matter. Furthermore, we observe morphological changes after white matter infiltration, a potential basis of an imaging biomarker during early glioblastoma colonization. Taken together, Deep3P allows for a non-invasive intravital investigation of brain tumor biology and its tumor microenvironment at subcortical depths explored, opening up opportunities for studying the neuroscience of brain tumors and other model systems.

Abstract:Accurate grading of gliomas is critical to guide therapy and predict prognosis. The presence of microvascular proliferation is a hallmark feature of high grade gliomas which traditionally requires targeted surgical biopsy of representative tissue. Superb microvascular imaging (SMI) is a novel high resolution Doppler ultrasound technique which can uniquely define the microvascular architecture of whole tumours. We examined both qualitative and quantitative vascular features of gliomas captured with SMI, analysing flow signal density, vessel number, branching points, curvature, vessel angle deviation, fractal dimension, and entropy. Results indicate that high-grade gliomas exhibit significantly greater vascular complexity and disorganisation, with increased fractal dimension and entropy, correlating with known histopathological markers of aggressive angiogenesis. The integrated ROC model achieved high accuracy (AUC = 0.95), highlighting SMI's potential as a non-invasive diagnostic and prognostic tool. While further validation with larger datasets is required, this study opens avenues for SMI in glioma management, supporting intraoperative decision-making and informing future prognosis.

Abstract:OBJECTIVE:Evaluation of tumor microvascular morphology is of great significance in tumor diagnosis, therapeutic effect prediction, and surgical planning. Recently, two-dimensional ultrasound localization microscopy (2DULM) has demonstrated its superiority in the field of microvascular imaging. However, it suffers from planar dependence and is unintuitive. We propose a novel three-dimensional ultrasound localization microscopy (3DULM) to avoid these limitations. METHODS:We investigated 3DULM based on a 2D array for tumor microvascular imaging. After intravenous injection of contrast agents, all elements of the 2D array transmit and receive signals to ensure a high and stable frame rate. Microbubble signal extraction, filtering, positioning, tracking, and other processing were used to obtain a 3D vascular map, flow velocity, and flow direction. To verify the effectiveness of 3DULM, it was validated on double helix tubes and rabbit VX2 tumors. Cisplatin was used to verify the ability of 3DULM to detect microvascular changes during tumor treatment. RESULTS:In vitro, the sizes measured by 3DULM at 3 mm and 13 mm were 178 μ m and 182 μ m , respectively. In the rabbit tumors, we acquired 9000 volumes to reveal vessels about 30 μ m in diameter, which surpasses the diffraction limit of ultrasound in traditional ultrasound imaging, and the results matched with micro-angiography. In addition, there were significant changes in vascular density and curvature between the treatment and control groups. CONCLUSIONS:The effectiveness of 3DULM was verified in vitro and in vivo. Hence, 3DULM may have potential applications in tumor diagnosis, tumor treatment evaluation, surgical protocol guidance, and cardiovascular disease. CLINICAL RELEVANCE STATEMENT:3D ultrasound localization microscopy is highly sensitive to microvascular changes; thus, it has clinical potential for tumor diagnosis and treatment evaluation. Noninvasive three-dimensional optoacoustic localization microangiography of deep tissues

Abstract:Structural microvascular alterations and dysfunction serve as key disease indicators of cancer, diabetes, ischemic stroke, neurodegenerative disorders and many other conditions. In vivo visualization of the microvasculature has traditionally been restricted to millimeter scale depths accessible with optical microscopy. Optoacoustic imaging has enabled breaking through the barrier imposed by light diffusion to map functional hemodynamic parameters in deep-seated vessels, but diffraction and dispersion of ultrasound waves in heterogeneous living tissues prevents reaching capillary resolution. Herein, we demonstrate three-dimensional microangiography of deep mouse brain beyond the acoustic diffraction limit (<20$\mu$m resolution) through the intact scalp and skull via optoacoustic localization of sparsely-distributed highly absorbing microparticles. This was enabled by devising 5$\mu$m sized extremely absorbing dichloromethane microdroplets exhibiting four orders of magnitude higher optical absorption than red blood cells at near-infrared wavelengths, thus facilitating in vivo compatibility and single particle sensitivity in the presence of highly absorbing blood background. Accurate mapping of the blood flow velocity within microvascular structures was also facilitated by the high 3D frame rate of the optoacoustic tomography system. We further show that the detected optoacoustic signal intensities from the localized particles may serve for estimating the light fluence distribution within optically heterogeneous tissues, a long-standing quantification challenge in biomedical optics. Given the intrinsic sensitivity of optoacoustics to various functional, metabolic and molecular events in living tissues, this new approach paves the way for non-invasive deep-tissue microscopic observations with unrivaled resolution, contrast and speed.
In vivo imaging in a murine model of glioblastoma

Abstract:Objective: To use in vivo imaging methods in mice to quantify intracranial glioma growth, to correlate images and histopathological findings, to explore tumor marker specificity, to assess effects on cortical function, and to monitor effects of chemotherapy. Methods: Mice with DBT glioma cell tumors implanted intracranially were imaged serially with a 4.7-T small-animal magnetic resonance imaging (MRI) scanner. MRI tumor volumes were measured and correlated with postmortem histological findings. Different nonspecific and specific positron emission tomography radiopharmaceuticals, [18F]2-fluoro-2-deoxy-d-glucose, [18F]3'-deoxy-3'-fluorothymidine, or [11C]RHM-I, a sigma2-receptor ligand, were visualized with microPET (CTI-Concorde MicroSystems LLC, Knoxville, TN). Intrinsic optical signals were imaged serially during contralateral whisker stimulation to study the impact of tumor growth on cortical function. Other groups of mice were imaged serially with MRI after one or two doses of the antimitotic N,N'-bis(2-chloroethyl)-N-nitrosourea (BCNU). Results: MRI and histological tumor volumes were highly correlated (r2 = 0.85). Significant binding of [11C]RHM-I was observed in growing tumors. Over time, tumors reduced and displaced (P # 0.001) whisker-activated intrinsic optical signals but did not change intrinsic optical signals in the contralateral hemisphere. Tumor growth was delayed 7 days after a single dose of BCNU and 18 days after two doses of BCNU. Mean tumor volume 15 days after DBT implantation was significantly smaller for treated mice (1- and 2-dose BCNU) compared with controls (P = 0.0026). Conclusion: Mouse MRI, positron emission tomography, and optical imaging provide quantitative and qualitative in vivo assessments of intracranial tumors that correlate directly with tumor histological findings. Short Acquisition Time Super-Resolution Ultrasound Microvessel Imaging via Microbubble Separation

Abstract:Abstract Super-resolution ultrasound localization microscopy (ULM), based on localization and tracking of individual microbubbles (MBs), offers unprecedented microvascular imaging resolution at clinically relevant penetration depths. However, ULM is currently limited by the requirement of dilute MB concentrations to ensure spatially sparse MB events for accurate localization and tracking. The corresponding long imaging acquisition times (tens of seconds or several minutes) to accumulate sufficient isolated MB events for full reconstruction of microvasculature preclude the clinical translation of the technique. To break this fundamental tradeoff between acquisition time and MB concentration, in this paper we propose to separate spatially overlapping MB events into sub-populations, each with sparser MB concentration, based on spatiotemporal differences in the flow dynamics (flow speeds and directions). MB localization and tracking are performed for each sub-population separately, permitting more robust ULM imaging of high-concentration MB injections. The superiority of the proposed MB separation technique over conventional ULM processing is demonstrated in flow channel phantom data, and in the chorioallantoic membrane of chicken embryos with optical imaging as an in vivo reference standard. Substantial improvement of ULM is further demonstrated on a chicken embryo tumor xenograft model and a chicken brain, showing both morphological and functional microvasculature details at super-resolution within a short acquisition time (several seconds). The proposed technique allows more robust MB localization and tracking at relatively high MB concentrations, alleviating the need for dilute MB injections, and thereby shortening the acquisition time of ULM imaging and showing great potential for clinical translation.

Abstract:Fluorescent molecular tomographic (FMT) imaging can noninvasively monitor molecular function in living animals using specific fluorescent probes. However, macroscopic imaging methods such as FMT generally exhibit low anatomical details. To overcome this, we report a quantitative technique to image both structure and function by combining FMT and magnetic resonance (MR) imaging. We show that FMT-MR imaging can produce three-dimensional, multimodal images of living mouse brains allowing for serial monitoring of tumor morphology and protease activity. Combined FMT-MR tumor imaging provides a unique in vivo diagnostic parameter, protease activity concentration (PAC), which reflects histological changes in tumors and is significantly altered by systemic chemotherapy. Alterations in this diagnostic parameter are detectable early after chemotherapy and correlate with subsequent tumor growth, predicting tumor response to chemotherapy. Our results reveal that combined FMT-MR imaging of fluorescent molecular probes could be valuable for brain tumor drug development and other neurological and somatic imaging applications.

Abstract:Photoacoustic (PA) imaging can map the physiological conditions of tissues and track the biodistribution of contrast agents. Ultrasound localization microscopy (ULM) with microbubbles provides deep-tissue super-resolution blood vessel images and blood velocity maps. The integration of these techniques offers a potential tool for oncological applications, such as visualizing tumor hypoxia and vasculature. Additionally, because PA imaging and ULM can share the same ultrasound detector and data-acquisition system, PA images can seamlessly fuse with ULM images without requiring post co-registration processing. However, the acquisition time of ULM is considerably longer (several minutes per frame) compared to PA imaging (10 milliseconds to 100 milliseconds per frame), limiting the simultaneous acquisition of the two imaging modalities. To address this issue, we developed a deep learning model based on U-Net to accelerate the frame rate of ULM using information from sparse microbubble locations and power Doppler images. By enabling fast ULM acquisition, we developed an interleaved PA/fast ULM imaging technique to simultaneously capture photoacoustic and ULM images. Our experiments demonstrated up to a 16 times improvement in acquisition speed with U-Net, utilizing in vivo mouse brain and skin tumor data. The hybrid imaging sequence achieved super-resolution vascular and tissue oxygenation imaging with less than 2 seconds of acquisition time per frame in vivo. The implementation of this rapid dual imaging scheme enables the longitudinal monitoring of 3D tumor microvasculature and hypoxia conditions using a common linear ultrasound transducer. We monitored tumor growth of epidermoid carcinoma (A431) in a mouse model from Day 7 to Day 17 of tumor implantation, and the results reveal a significant decrease in tumor vasculature density and oxygen saturation as the tumor grew larger over time. The results indicate a positive relationship between oxygen saturation and vessel density. Our findings showcase the potential of this innovative technique for advancing deep tissue, high-resolution, multi-biological parameters imaging in oncological research. Abstract:Functional ultrasound imaging (fUS) and ultrasound localization microscopy (ULM) are advanced ultrasound imaging modalities for assessing both functional and anatomical characteristics of the brain. However, the application of these techniques at a whole-brain scale has been limited by technological challenges. While conventional linear acoustic probes provide a narrow 2D field of view and matrix probes lack sufficient sensitivity for 3D transcranial fUS, multilinear probes have been developed to combine high sensitivity to blood flow with fast 3D acquisitions. In this study, we present a novel approach the combined implementation of transcranial whole-brain fUS and ULM in mice using a motorized multilinear probe. This technique provides high-resolution, non-invasive imaging of neurovascular dynamics across the entire brain. Our findings reveal a significant correlation between absolute cerebral blood volume (ΔCBV) increases and microbubble velocity, indicating vessel-level dependency of the evoked response. However, the lack of correlation with relative CBV (rCBV) suggests that fUS cannot distinguish functional responses alterations across different arterial vascular compartments. This methodology holds promise for advancing our understanding of neurovascular coupling and could be applied in brain disease diagnostics and therapeutic monitoring.

Abstract:As a sign of significant infiltration, the high inhomogeneity in the radial distribution of tumor peripheral microvessels can guide the identification of malignant tumors. However, clinical histology inhibits our ability to detect dynamic changes in tumor microvessels. Here, based on super-resolution ultrasound imaging (SRUS) techniques, we aim to achieve the non-invasive assessment of radial heterogeneity of tumor peripheral microvessels by calculating the standard deviations of these vascular parameters across 4 sectors along the radial direction in the tumor periphery. The observation of the LS174T tumor model established in the right flank region of the BLAB/c nude mouse proved that radial heterogeneity analysis of tumor peripheral microvessels can offer a better characterization of margin infiltration degree.

Abstract:Abstract Non-invasive detection of microvascular alterations in deep tissues in vivo provides critical information for clinical diagnosis and evaluation of a broad-spectrum of pathologies. Recently, the emergence of super-resolution ultrasound localization microscopy (ULM) offers new possibilities for clinical imaging of microvasculature at capillary level. Currently, the clinical utility of ULM on clinical ultrasound scanners is hindered by the technical limitations, such as long data acquisition time, high microbubble (MB) concentration, and compromised tracking performance associated with low imaging frame-rate. Here we present a robust in-human ULM on a high frame-rate (HFR) clinical ultrasound scanner to achieve super-resolution microvessel imaging using a short acquisition time (<10 s). Ultrasound MB data were acquired from different human tissues, including a healthy liver and a diseased liver with acute-on-chronic liver failure, a kidney, a pancreatic tumor, and a breast mass using an HFR clinical scanner. By leveraging the HFR and advanced processing techniques including sub-pixel motion registration, MB signal separation, and Kalman filter-based tracking, MBs can be robustly localized and tracked for ULM under the circumstances of relatively high MB concentration associated with standard clinical MB administration and limited data acquisition time in humans. Subtle morphological and hemodynamic information in microvasculature were shown based on data acquired with single breath-hold and free-hand scanning. Compared with contrast-enhanced power Doppler generated based on the same MB dataset, ULM showed a 5.7-fold resolution improvement in a vessel based on a linear transducer, and provided a wide-range blood flow speed measurement that is Doppler angle-independent. Microvasculatures with complex hemodynamics can be well-differentiated at super-resolution in both normal and pathological tissues. This preliminary study implemented the ultrafast in-human ULM in various human tissues based on a clinical scanner that supports HFR imaging, indicating the potentials of the technique for various clinical applications. However, rigorous validation of the technique in imaging human microvasculature (especially for those tiny vessel structure), preferably with a gold standard, is still required.
