Abstract:Objective To demonstrate the MRI manifestations of congenital cystic adenomatoid malformation(CCAM)and to evaluate the diagnostic value of MRI.Methods Thoracic axial,sagittal and coronal plane scanning were performed with SSFSE in 9 fetuses diagnosed or suspected of CCAM by ultrasound(US)within 1—2 days after US examination.The diagnosis was confirmed by postnatal autopsy or follow-up.Results In nine fetuses,seven cases of CCAM were diagnosed with MRI and confirmed by autopsy,one case was congenital pulmonary sequestration,one was normal on MRI and two weeks late US.In seven cases of CCAM(5 males and 2 females),four cases were in the right side,three in the left.Four cases of macrocystic type CCAM showed single or multiple cystic long T_2 signal in the unilateral lung,three cases of microcystic type CCAM presented long T_2 signal without cyst.Vessels with flow void phenomenon were found in 2 cases of CCAM and 1 case of pulmonary sequestration.Conclusion MRI can clearly show the location,range and contour of CCAM.The vessel originated from the aorta is suggestive of pulmonary sequestration.

Abstract:OBJECTIVE:To investigate the value of MR imaging in the diagnosis of benign sacrococcygeal teratomas of the infants.METHODS:MR imaging of benign sacrococcygeal teratomas in 6 cases proved by surgery and pathology was retrospectively reviewed. In all patients, a fast imaging sequence, fast spin echo sequence was employed, together with short time inversion recovery sequence and contrast enhancement scanning by intravenous injection of Gd-DTPA.RESULTS:There were 6 benign sacrococcygeal teratomas, which were heterogeneous masses and arose from the distal sacrococcygeal region in the pelvis. The MR imaging appearances of the benign sacrococcygeal teratomas were characteristic, T1- and T2-weighted images demonstrated a large mass containing round, well-defined areas of varying signal intensity representing its cystic, solid, and sometimes fat, calcification within the lesions.CONCLUSION:MR imaging provides definitive information of benign sacrococcygeal teratomas and clearly shows both extra-and intra-pelvic components, and even better anatomic details, which facilitates the surgical planning adequately.
Magnetic Resonance Imaging for Pelvic Tumors During Pregnancy: Manifestations and Clinical Application

Abstract:Objective To investigate the manifestations and clinical application of magnetic resonance imaging (MRI) in the diagnosis of pelvic tumors during pregnancy.Methods Clinical data and MRI manifestations of 161 patients attending Women's Hospital,School of Medicine,Zhejiang University with pelvic tumors during pregnancy between March 2013 and July 2016 were retrospectively analyzed.MRI manifestations and clinical treatments of different tumors in pregnancy were summarized.All data were analyzed by descriptive statistics.Results (1) The gestational ages at initial diagnosis of the 161 patients ranged between 8-39 weeks and the average gestational age was (29±-4) weeks.Of all patients,147 (91.3％) had benign tumor,while the other 14(8.7％) were diagnosed as malignanices.The sensitivity,specificity and accuracy of MRI in the diagnosis of malignancy during pregnancy were 92.8％ (13/14),97.3％ (143/147) and 96.7％ (156/161).Uterine neoplasm,ovarian tumor,intestinal tumor and pelvic peripheral nerve schwannoma respectively accounted for 67.1％ (108/161),29.8％ (48/161),1.9％ (3/161) and 1.2％ (2/161) of all cases.(2) Among the 108 gravidas with uterine neoplasm,104 cases of leiomyoma continued to term pregnancy,among which 71 underwent myomectomy after vaginal delivery and 33 had myomectomy during cesarean section;the other four cases of cervical cancer terminated their pregnancy.(3) Among the 48 gravidas with ovarian tumor,38 were diagnosed as having benign tumor by MRI and continued to term pregnancy;five were diagnosed with tumor pedicle torsion and treated with emergency operation because of acute abdominal disease;two patients who were both in their second trimester (16 and 20 gestational weeks) were diagnosed with ovarian cancer complicated with peritoneal metastasis by MRI and decided to terminate pregnancy;another two patients with uncertain preoperative diagnosis of ovarian cancer were followed up closely to term and the tumor was removed during cesarean section;one patient with ovarian malignant tumor,which was diagnosed by MRI at the 32nd gestational week,received cesarean section and cytoreductive surgery.(4) In the three gravidas with intestinal tumor,one was complicated with sigmoid colon cancer and underwent cesarean section,radical resection of sigmoid colon tumor,colostomy and enterodialysis at 28+3 weeks of gestation;one with appendiceal mucinous adenocarcinoma was monitored closely until full-term pregnancy and received right hemicolectomy during cesarean section;the third case with malignant stromal tumor of the small intestine underwent cesarean section at 30+5 weeks of gestation as well as the completer resection of tumor.(5) One gravida with pelvic peripheral nerve schwannomas was diagnosed with pelvic schwannoma and underwent cesarean section and tumor extirpation at term.The other one underwent tumor extirpation prior to delivery due to the larger size of tumor and had a vaginal delivery at term.Conclusions MRI can provide more precise information for prenatal diagnosis of pelvic tumors during pregnancy.

Abstract:Objective:To compare the differences of clinical manifestations and prognoses of neonatal sacrococcygeal teratomas between prenatal diagnosis and postnatal diagnosis, to analyze the influencing factors of adverse clinical manifestations and to summarize the therapeutic experience.Methods:Clinical data were collected for operated children with neonatal sacrococcygeal treatomas from August 2013 to August 2019. Retrospective analysis was performed for comparing the differences of gender, delivery mode, gestational age, birth weight, level of alpha fetoprotein (AFP), imaging findings, pathological features, Altman's classification, age of admission, tumor size, hospital stay, complications, recurrence and mortality between prenatal and postnatal diagnoses. The relevant factors of adverse manifestations were evaluated by Logistic analysis.Results:Among them, 71.43%(15/21) of prenatal diagnoses and 100%(20/20) of postnatal diagnoses were full-term births. And 85.71%(18/21) of prenatal diagnoses and 40%(8/20) of postnatal diagnoses were delivered by cesarean section. The average age of admission was (5.67±14.18) days for prenatal diagnoses and (35.80±42.53) days for postnatal diagnoses. Tumor size was (9.46±3.19) cm for prenatal diagnoses and (6.90±3.18) cm for postnatal diagnoses. And hospital stay was (19.57±8.19) days for prenatal diagnoses and (15.10±6.89) days for postnatal diagnoses. Two data sets showed a statistically significant difference ( P=0.002, P=0.032, P<0.001, P=0.006, P=0.018). No significant differences existed in gender, birth weight, AFP level, imaging findings, pathological features or Altman's classification. During a median follow-up period of 24 months, there was no death and one child replaced (prenatal diagnoses). A total of 14 children had adverse clinical manifestations. The diagnostic time was correlated with poor prognosis by Logistic analysis ( OR=0.029, 95% CI 0.001-0.747, P=0.033). No significant differences existed in complications, recurrence or mortality ( P>0.05) . Conclusions:Neonates with prenatally diagnosed neonatal sacrococcygeal teratomas have a higher rate of premature caesarean delivery, younger age, larger tumour and longer hospital stay. And the risk of adverse manifestations is higher in the group of prenatal diagnosis. However, two groups both have a favorable prognosis.
Evaluation and Tracing of Prenatally Diagnosed Abdominal Masses

Abstract:Objective To assess the accuracy of prenatal diagnosis in abdominal masses, natural development of the mass and the status of parent's choice for termination. Methods Clinical data of prenatally ultrasonographic diagnosed abdominal masses in recent 3 years were reviewed, including gestation age, mass size and location, mass features (cystic, mixed, solid), prenatal diagnosis, prenatal follow-up, the parents' choice for termination, postnatal diagnosis and management and follow-up.Results A total of 59 fetal abdominal masses were found. They accounted for 2. 2 ％ among the prenatally diagnosed malformation and tumors(59/2723)in same period. Of these cases, the youngest was 20 weeks and the oldest 39 weeks. Mean gestation age is 31.3 weeks. Confirmed diagnosis from autopsy, postnatal operation and image examination was gained. Only 3 cases were not match to the prenatal diagnosis. The total accordance is 91.9％. The masses presented regression before or after birth were observed in 18 cases (18/59, 3 confirmed and 15 not confirmed). Ninteen parents(19/59) refused the physician's advice and chose termination. Twenty-one (21/59) were termination without autopsy and lost follow-up. Fifty tumors diagnosed via operation or postnatal image examination were established. Conclusions Fetal abdominal mass can be found by sonography. Most prenatal diagnosis is match to postnatal result. The natural history of fetal mass is favorable. Some parents may choose termination when they know the fetal with suspected tumor in China.

Abstract:Fetal intracranial teratoma presents a rare and devastating diagnosis. Typically, this condition is first detected during routine prenatal ultrasounds, appearing as an irregular heterogeneous lesion. Further insights are gained through fetal magnetic resonance imaging (MRI), better characterizing the anomaly. The combination of these modalities provides detail-oriented high resolution MRI images, while follow-up ultrasounds capture dynamic growth changes, serving as a cost-effective and easily accessible adjunct. This fast-growing tumor leads to macrocephaly and ventriculomegaly, causing severe distortion of the brain parenchyma. Early detection is crucial for effective fetal management and preventing maternal complications. Unfortunately, treatment options are limited due to the tumor's aggressive nature, typically resulting in fetal demise shortly after birth. Here, we present the sonographic and MRI findings of a congenital intracranial teratoma, reaching massive proportions and replacing the entire cerebral hemisphere.
Pregnancy and Long‐Term Postnatal Outcomes of Congenital Sacrococcygeal Teratoma: A Single Institution's 18‐Year Experience

Abstract:Objective The objective of this study is to evaluate outcomes of fetal sacrococcygeal teratoma (SCT) from an academic tertiary center. Study Design This is a retrospective study evaluating pregnancy and postnatal outcomes of fetal SCT management at a single institution between 2006 and 2023. Results are reported as median (range). Results Fourteen patients with fetal SCT were studied. Hydrops fetalis occurred in 2 (14.3%) cases. Pregnancy course included expectant management in 12 (85.7%) and palliative preterm induction in 1 patient (7.1%). Indications for delivering timing included fetal deterioration in 7 patients (50%), preeclampsia in 3 (21.4%), elective induction of delivery in 3 (21.4%), and preterm labor in 1 (7.1%). Delivery GA was 36.4 weeks (26.4–40.1 weeks), with 11 (78.5%) cesareans and 3 (21.4%) vaginal deliveries. There were 13 (92.9%) live births and 11 (78.6%) long‐term survivors. Postnatal resection pathology demonstrated 7 (53.8%) mature, 3 (23.1%) immature, and 3 (23.1%) malignant SCTs. Of the 11 long‐term survivors, 3 (27.3%) had teratoma recurrences, including 2 (18.2%) with metastatic disease requiring chemotherapy. Notable long‐term complications involved gastrointestinal (n = 7, 63.6%), genitourinary (n = 4, 36.4%), and musculoskeletal (n = 2, 18.2%) systems. Conclusion SCT confers significant morbidity to both the pregnant patient and neonate. Multidisciplinary prenatal and postnatal care is needed to comprehensively manage this complex condition.

Abstract:Congenital tumors are rare and, owing to this rarity, there is limited information on many of them. A total of 839 fetal and postnatal MRI studies performed in the first 3 months of life were retrospectively reviewed. They were performed with the use of 1.5 T scanners. Seventy-six tumors were diagnosed based on fetal MRI between 20 and 37 gestational weeks, and 27 were found after birth, from 1 day of age to 3 months of life. Teratomas were the most common tumors in our dataset, mainly in the sacrococcygeal region (SCT), followed by cardiac rhabdomyomas and subependymal giant cell astrocytomas (SEGA) associated with TSC, and neuroblastomas. The group of less common tumors consisted of infantile fibrosarcomas, malignant rhabdoid tumors, mesoblastic nephromas and Wilms tumor, craniopharyngiomas, brain stem gliomas, desmoplastic infantile astrocytoma, choroid plexus carcinoma, glioblastoma, hemangiopericytoma, rhabdomyosarcoma, melanoma, mesenchymal hamartomas of the chest wall and the liver, and juvenile xanthogranuloma, with special consideration of blue rubber bleb nevus syndrome. MRI plays a significant role in further and better characterization of congenital tumors, leading to a correct diagnosis in many cases, which is crucial for pregnancy and neonatal management and psychological preparation of the parents. No diagnosis is impossible and can be absolutely excluded.
Prenatal diagnosis in estimating the prognosis of sacrococcygeal teratoma

Abstract:: Sacrococcygeal teratoma is a rare congenital malformation, the prognosis depends on factors affecting foetal development. The diagnosis is based on ultrasound examination, especially the evaluation of the detailed morphology of the foetus in the 20th week of pregnancy. Therefore, it is crucial to keep looking for ultrasound markers that would prenatally determine the most accurate prognosis for the foetus. Now, we rely on a small number of studies with a predominance of case reports. We offer a literature review of the essential information concerning sacrococcygeal teratoma diagnostics, therapy, and complications of sacrococcygeal teratomas in connection with prenatal diagnosis. It turns out that in cases with a favourable prognosis according to prenatal ultrasound examination and adequate surgical treatment after childbirth, the prognosis of this congenital malformation is excellent.
Perinatal Outcomes of Intrauterine Interventions for Fetal Sacrococcygeal Teratoma Based on Different Surgical Techniques—A Systematic Review

Abstract:Background: This study aims to evaluate the outcomes of fetal sacrococcygeal teratoma (SCT) submitted to prenatal interventions. Methods: We performed a systematic literature review of fetal SCT patients and compared the outcomes between open fetal surgery and percutaneous intervention. In addition, we also compared the results of SCT fetuses who did not undergo any surgical intervention (NI). Results: We identified 16 cases of open fetal surgery (OS), 48 cases of percutaneous fetal intervention (PI), and 93 NI patients. The survival rate was 56.2% in OS, 45.8% in PI (p = 0.568), and 71.0% in NI patients. The gestational age at delivery was earlier in cases where there was no survival compared to cases where the fetuses did survive across all evaluated cohorts (OS: p = 0.033, PI: p < 0.001, NI: p < 0.001). The gestational weeks at delivery in OS and PI fetuses were more similar; however, OS tended to be performed later on in pregnancy, and the affected fetuses had more severe presented findings. In our evaluation, we determined that the presence of fetal hydrops and cardiac failure had no significant impact on survival in SCT cases. In NI patients, polyhydramnios was much higher in fetuses who did not survive compared to their surviving cohorts (p < 0.001). Conclusions: In conclusion, gestational age at delivery can affect the short-term prognosis of fetuses affected with sacrococcygeal teratomas. Regardless of the mode of delivery or the necessity for intervention during the fetal period, monitoring for complications, including polyhydramnios, can prevent premature delivery.

Abstract:Congenital facio-cervical masses can be a developmental anomaly of cystic, solid, or vascular origin, and have an inseparable relationship with adverse prognosis. This retrospective cross-sectional study aimed at determining on the prenatal diagnosis of congenital facio-cervical masses, its management and outcome in a large tertiary referral center. We collected information on prenatal clinical data, pregnancy outcomes, survival information, and final diagnosis. Out of 130 cases of facio-cervical masses, a total of 119 cases of lymphatic malformations (LMs), 2 cases of teratoma, 2 cases of thyroglossal duct cyst, 4 cases of hemangioma, 1 case of congenital epulis, and 2 cases of dermoid cyst were reviewed. The accuracy of prenatal ultrasound was 93.85% (122/130). Observations of diameters using prenatal ultrasound revealed that the bigger the initial diameter is, the bigger the relative change during pregnancy. Magnetic resonance imaging (MRI) revealed that 2 cases of masses were associated with airway compression. In conclusion, ultrasound has a high overall diagnostic accuracy of fetal face and neck deformities. Prenatal US can enhance the management of ambulatory monitoring and classification. Furthermore, MRI provided a detailed assessment of fetal congenital malformations, as well as visualization of the trachea, presenting a multi-dimensional anatomical relationship.

Abstract:To the Editor: In the recent interesting article by Sakamoto et al,1 the authors found that rats undergoing exercise training showed lower degree of left ventricular (LV) hypertrophy, higher expression and activity of heat shock transcription factor 1 (HSF1), and preserved LV systolic function in comparison with rats exposed to sustained pressure overload by surgical constriction of the transverse aorta (TAC). They also observed that transgenic mice expressing constitutively active HSF1 showed less evident hypertrophic response and better systolic function than their wild-type littermates 5 weeks after TAC. Conversely, HSF1-deficient mice showed similar degree of LV hypertrophy but worse systolic function in comparison with their wild-type littermates after 4 weeks of exercise or 1 week after TAC. These data support the intriguing hypothesis that HSF1 upregulation in cardiac myocytes may play a key role in preventing LV systolic impairment in the exercise-induced hypertrophy model, and in determining the adaptive or maladaptive nature of the hypertrophic response. Intracranial hemorrhage and additional anomalies detected on prenatal magnetic resonance imaging: a large, retrospective study in two tertiary medical institutions

Abstract:Abstract Purpose To clarify the prenatal magnetic resonance (MR) imaging characteristics of foetal intracranial haemorrhages (ICHs) in a large cohort and correlate them with birth outcomes. Methods We retrospectively reviewed MR images of foetuses with ICH on screening ultrasound (US) on picture archiving communication system (PACS) servers within a nearly five-year period from two medical tertiary centres. The indications, main abnormal findings and coexistent anomalies were recorded by two experienced radiologists with census readings. Results We recruited 81 cases (average gestational week, 28.0 ± 5.0 weeks) with prenatal MR imaging, including 71 singleton pregnancies and 10 monochromic twin pregnancies. Predominant coexistent anomalies were ventriculomegaly (35.8%), holoprosencephaly or porencephaly (13.6%) and enlarged posterior fossa/ or posterior fossa cyst (7%) in the lesion-based evaluation. The number of haemorrhagic lesions and the occurrence of the detected complications did not show a correlation with the size of the haematoma. The mass effect of ICH was more commonly observed in foetuses with a large for gestational age (GA) than in foetuses with a small for GA. Conclusion Prenatal MR imaging could better show ICH morphology and associated abnormal findings. As a complementary tool of US, MR imaging could help with prenatal counselling and treatment selection after birth.
Advances in the Prognostic Predication of Fetal Sacrococcygeal Teratoma

Abstract:Sacrococcygeal teratoma (SCT) is a common fetal and neonatal tumor in live births with an incidence of 1/40,000 to 1/35,000.And it has a male-to-female ratio of 1∶3 to 1∶4.The prognosis of SCT is generally fair after birth.However,the mortality of fetal SCT was reported at 16％to 63 ％ in the literature.Thus its prenatal diagnosis and prognostic indicators have become a hot focus of current researches.Here recent advances in its prognostic predication are reviewed.Various indicators such as tumor cystic proportion,tumor-fetal ratio (TFR),solid tumor volume divided by head volume (STV∶ HV),solid tumor volume index (STVI),tumor growth and cardiovascular profile score (CVPS) are discussed.In clinical practices,tumor cystic proportion and TFR are recommended.Or either STV∶ HV or STVI may be used.Tumor growth should be always monitored.CVPS may be monitored be to assess the extent of fetal heart failure to determine the timing of intervention.If high output heart failure occurs,fetal surgery should be performed in the first 28 weeks of pregnancy and therapeutic delivery should be considered after 28 weeks of pregnancy.Currently the prognosis of SCT fetus is still in an exploratory stage.Due to imperfect prenatal policies,more in-depth researches are warranted.
Prenatal Ultrasonic Features of a Mediastinal Teratoma: A Case Report and Literature Review

Abstract:Fetal mediastinal teratomas represent only 10% of congenital teratomas in children and 2.6% of all mediastinal masses in children. Teratomas have multifactorial etiology, such as chromosomal abnormalities. Fetal mediastinal teratomas are rare. Mediastinal teratomas can cause hydrops fetalis, fetal demise, and neonatal respiratory distress; therefore, accurate perinatal management and interventions are very important. We describe a case of fetal mediastinal teratoma wherein the cystic fluid in the fetal tumor was aspirated and confirmed by surgical pathology after birth at the authors' center. The teratoma in this case was characterized by a large single cystic mass with clear borders in the anterosuperior mediastinum, which grew rapidly and was closely related to the thymus. The infant was healthy at birth, and the tumor was surgically removed the age of 1 year. The postoperative course was uneventful, and the patient was in good health 6 years postoperatively. This case and literature review suggests that ultrasound examination can accurately diagnose fetal mediastinal teratomas, which is beneficial to provide an accurate basis for fetal prenatal intervention and treatment. Additionally, an important ultrasound feature of a fetal unicystic mediastinal teratoma is a saddle‐shaped mass with clear boundaries, which provided an accurate reference for the diagnosis of a fetal cystic mediastinal teratoma by prenatal ultrasonography.

Abstract:Fetal tumors represent an infrequent pathology when compared to congenital malformations, although their true incidence may be underestimated. A variety of benign and malignant neoplasms may occur anywhere in the neural axis. Imaging plays an important role in the fetal tumors diagnosis and evaluation of their resultant complications. Discovery of a fetal mass on obstetric ultrasound necessitates further evaluation with prenatal MRI. New MR sequences and new applications of existing techniques have been successfully implemented in prenatal imaging. A detailed assessment may be performed using a variety of MR. Fetal tumors may be histologically benign or malignant, but their prognosis generally remains poor, especially for intracranial lesions. Unfavorable tumor location or heightened metabolic demands on a developing fetus may result in severe complications and a fatal outcome, even in cases of benign lesions. Nowadays, prenatal treatment focuses mainly on alleviation of secondary complications caused by the tumors. In this paper we review congenital tumors of the brain, face and neck encountered in prenatal life, and discuss diagnostic clues for appropriate diagnosis.

Clinicopathological Characteristics and Treatment Outcomes of Pregnancy Complicated by Malignant Ovarian Germ Cell Tumors.

Abstract:PURPOSE:This study aimed to analyze the clinicopathological features, treatment, and feto-maternal outcomes of pregnancy complicated by malignant ovarian germ cell tumors (MOGCTs), to increase the awareness on this condition.PATIENTS AND METHODS:We retrospectively reviewed the medical records of patients diagnosed with MOGCTs during pregnancy, who were treated and followed-up at Peking Union Medical College Hospital from January 2000 to December 2017. The demographic characteristics, pathological features, treatment and prognosis were analyzed.RESULTS:The histological subtypes varied in 14 patients (dysgerminoma, n=1; immature teratoma, n=4; yolk sac tumor, n=6; and mixed germ cell tumors, n=3). Ten (71.4%) patients, including three who opted for conservative therapy until childbirth, one who only received salvage chemotherapy during pregnancy, and six who underwent cystectomy or unilateral salpingo-oophorectomy during pregnancy, desired fetal preservation. After undergoing surgery, four patients chose surveillance instead of timely adjuvant chemotherapy. Eight patients delivered their babies, and the preterm delivery rate was 50.0%. One newborn died of premature birth. The median follow-up period was 44 (range: 13 to 86) months. During the current study period, 12 patients had survived and did not report any diseases. However, two died due to disease progression.CONCLUSION:Pregnant women with MOGCTs had favorable outcomes. However, when a malignant tumor is suspected, surgery cannot be avoided. Thus, instead of timely postoperative adjuvant chemotherapy, close surveillance may be an acceptable alternative for pregnant women with low-risk MOGCTs.
Sacrococcygeal Teratoma in One Twin: a Case Report and Literature Review

Abstract:Background Sacrococcygeal teratoma is one of the most common congenital tumors in newborns and infancy. The incidence is 1 per 20,000–40,000 live births. Ultrasonography is an optimal method for prenatal screening and diagnosis of fetal sacrococcygeal teratoma. MRI can be used to assist in the diagnosis. However, sacrococcygeal teratoma in the twin pregnancy is rare. Case presentation We reported a case of one twin with sacrococcygeal teratoma in dichorionic-diamniotic twin pregnancy.One twin with sacrococcygeal teratoma was diagnosed at the second trimester by ultrasonic examination and another twin was normal. A regular and careful antenatal care was conducted by the multidisciplinary team. The parents refused to perform the fetal MRI and examine the chromosome of both twin.At 37 + 1 of gestation, planned cesarean section was performed. The healthy male co-twin (twin A) weighed 2880 g.The male twin with SCT (twin B) weighed 2900 g, complying with 6 × 3 × 3 cm cystic and solid mass in sacrococcygeal region. At four days of age twin B underwent excisional surgery of the sacrococcygeal teratoma and coccyx and discharged 7 days after surgery. The mother and both babies were followed up and are all in good health until now. Conclusion(s) Sacrococcygeal teratoma in twin pregnancy is rare. Early antenatal diagnosis is important. Once the sacrococcygeal teratoma is diagnosed, clinicians should be aware of the associated maternal and fetal complications. Expecting parents should be counseled by the multidisciplinary team about the management and prognosis of the STC twin and co-twin. Prompt surgical excision of the sacrococcygeal teratoma after birth should be suggested.
Diagnosis and management of a sacrococcygeal teratoma at a tertiary hospital in northern Tanzania: A case report

Abstract:Introduction and importance: Sacrococcygeal teratoma is a rare extragonadal germ cell tumor occurring at the lower end of the back. It is congenital, and can be diagnosed in utero or just after birth. These tumors are sporadic in nature, more common in females than in males. They can be solid, cystic or a mixture of the two in consistency. Authors present this case to share their experience because these congenital tumors are rare and a lapse in appropriate management can be devastating because of chances of recurrence and the possibility of a malignant nature of the disease. Case presentation: Authors in this case present a baby that was referred to their center 5 days post vaginal delivery with a huge mass on the sacrococcygeal region. MRI revealed sacrococcygeal teratoma type II. Wide local excision was done. Tumor margins were microscopically negative. The patient recovered well and was discharged. The patient did not require adjuvant treatment. Clinical discussion: Routine obstetric ultrasound can diagnose sacrococcygeal teratoma with 100 % sensitivity. Fetuses with tumors <5 cm in size can be delivered vaginally. Tumors larger than that should be delivered at term through cesarean section. High risk sacrococcygeal teratomas can be delivered at 28 weeks of gestation by cesarean section followed by ex-utero intrapartum therapy. MRI is useful to assess the intrapelvic extent of the tumor and to plan management. Surgical intervention should be done after delivery. Conclusion: Intrauterine diagnosis is not an indication for termination of pregnancy. Surgical resection with negative margins provides a cure. Histological examination of the tumor is mandatory. For malignant sacrococcygeal teratomas, adjuvant chemotherapy is necessary for positive tumor margin after surgical resection. Positive margins may require chemotherapy. Neoadjuvant chemotherapy may be used for tumor debulking prior to surgery if the tumor infiltrates neighboring structures or is metastatic.
