Abstract:Abstract Introduction Altered pain perception was found in OSA. However, the relationship remains unclear, possibly due to differential effects on pain of apnea-related sleep disturbance and hypoxemia, or other factors. This study examines the relationship between retrospective pain reports and PSG variables, sex, age, BMI, depression and insomnia questionnaires in patients referred for OSA evaluation. Methods From all in-lab patients between 08/2017 and 04/2021, 420 (259 women) were selected without significant cardiac, pulmonary, neurological, endocrine or psychiatric conditions; 136 had chronic pain conditions (CPC, e.g., fibromyalgia, arthritis, sciatica, musculoskeletal pain and similar). Patients filled out the Center for Epidemiologic Studies Depression Scale-Revised (CESDR), Insomnia Severity Index (ISI), Epworth Sleepiness Scale (ESS), Chronic Pain Grade Scale measuring pain intensity (CPGS-PI) and functional disability (CPGS-FD) in the preceding 6 months. CPGS-PI and CPGS-FD were outcomes in hierarchical backward regression models. Blocks of explanatory variables were: (1) age, sex, BMI; (2) AHI, SpO2-nadir, time below SpO2-90%, respiratory-related arousal index, (3) total sleep time, sleep latency, sleep efficiency, #awakenings, (4) CESDR, ISI, ESS, (5) OSA(AHI≥5), CPC presence, and sex-OSA-CPC interactions. Results Higher CPGS-PI was unrelated to the presence of OSA (AHI≥5, n=248), but was predicted by higher BMI (p=0.008), higher CESD (p< 0.001), higher ISI (p=0.006), CPC presence (p< 0.001), and CPC-by-sex interaction (p=0.005). Higher CPGS-FD was unrelated to OSA, but was predicted by higher BMI (p< 0.001), higher CESD (p< 0.001), higher ISI (p< 0.011), CPC presence (p< 0.001), and interactions CPC-by-OSA (p=0.017), CPC-by-OSA-by-sex (p=0.002). Analyzing the interaction terms, in the absence of CPCs OSA was unrelated to CPGS-FD, although women reported higher CPGS-FD (M=2.5±3.1) than men (M=1.4±2.3). Among patients with CPC, OSA had a differential effect on pain: women with OSA reported higher CPGS-FD (M=5.3±3.5) than women without OSA (M=4.2±3.5), while men with OSA reported lower CPGS-FD (M=2.7±3.1) than men without OSA (M=4.2±4.0). Conclusion Demographic, mood and insomnia variables affect pain experience in OSA patients. Pain experience in men and women with CPC appears to be differentially affected by OSA. Abstract:Objective:To evaluate the effect of continuous positive airway pressure (CPAP) on symptoms and the quality of life of patients with mild obstructive sleep apnea hypopnea syndrome (OSAHS) complicated with somnolence.Methods:Seventy-four patients with mild OSAHS were recruited.Epworth sleepiness scale (ESS) was used to evaluate the degree of somnolence.Patients with ESS scores more than ten were designated as somnolence,and CPAP was conducted.Patients with ESS scores less than ten were designated as non-somnolence,and health education was conducted.Quebec sleep questionnaire (QSQ) was adopted to evaluate the quality of life.All patients were followed up for eight weeks.Results:After CPAP treatment,the ESS scores of mild OSAHS patients significantly decreased (P＜0.01),and the QSQ scores significantly increased (P＜0.01),indicating that CPAP treatment can improve the somnolence and quality of life in patients with mild OSAHS.Conclusions:Patients with mild OSAHS have different degrees of somnolence,which leads to a decline in quality of life.CPAP can improve somnolence and quality of life in patients with mild OSAHS.

Abstract:Abstract Introduction Obstructive Sleep Apnea (OSA) is a prevalent sleep disorder associated with poor health outcomes, including sleepiness, anxiety, and depression. This study aimed to further explore this relationship by looking at outcomes after treatment of OSA with continuous positive airway pressure (CPAP) therapy, using the Patient Health Questionnaire (PHQ-9), Generalized Anxiety Disorder 7 (GAD-7), and Epworth Sleepiness Scale (ESS) questionnaires respectively. Methods An observational longitudinal study in which the PHQ-9, GAD7, and ESS questionnaires were administered to adult patients diagnosed with OSA during the initial consultation and after three months of CPAP use. All patients had a diagnosis of OSA by a sleep study. Data collected included demographics, PSG data, and questionnaire results. Patients without follow-up or incomplete data were not included. Results One hundred patients were initially recruited. Thirty-five patients have completed data after three months. The mean±SD age was 50.9±15.4, BMI 36.5±8.9, 54% male and 46% female. Women were older 61.1±14 than men 46.5±14.9, p< 0.001. The percent of CPAP adherence was 68.7±36.7, and hours used per day were 5.5±1.8. The PHQ-9 initially was 10.4±5.6 decreased to 5.32±5.3 p< 0.001. GAD-7 initially 6.4±6.2 decreased to 4.29±4.8 p< 0.001, ESS initially 11.1±5.5 decreased to 5.8±4.3 p< 0.001. Neither age, BMI, or AHI correlated with compliance or hours used. The strongest negative correlation was between post-treatment GAD-7 and compliance (r= -0.45), followed by a mild negative correlation between PHQ-9 and compliance (r = -0.345). A mild positive correlation existed between the initial ESS and the AHI (r = 0.288). There were no statistical differences in questionnaire results between men and women. Conclusion The study demonstrates that daytime symptoms of sleepiness, depression, and anxiety improved in a cohort of patients with OSA and CPAP. The post-CPAP GAD-7 was moderately and negatively correlated with CPAP compliance percent use, while PHQ-9 was mildly negatively correlated. Initial ESS was mildly correlated with AHI, and although it improved, it was not correlated with compliance. Support (if any)

Development of Miliary Tuberculosis under Infliximab in a Patient with Spondyloarthritis and Suspected Crohn’s Disease

Abstract:Abstract Tuberculosis (TB) infection is a major concern in patients with chronic autoimmune conditions under immunosuppressive therapy. Gastrointestinal tuberculosis can be misdiagnosed as Crohn’s disease with detrimental consequences for the patient. We report on a 40-year old ethnic Turkish patient with HLA-B27 positive spondyloarthritis who developed gastrointestinal symptoms under immunosuppressive treatment with infliximab. Crohn’s disease was diagnosed at a primary care hospital and immunosuppressive treatment was escalated. Initial diagnostic tests for tuberculosis were negative. When the clinical condition deteriorated, the patient was transferred to our intensive care unit for further diagnosis and treatment. Tuberculosis was suspected due to clinical presentation and radiological signs and anti-tuberculous treatment was initiated. After the onset of treatment, first microbiological results confirmed the diagnosis of miliary TB with Mycobacterium bovis. As an infection route we assume primary gastrointestinal infection with M. bovis during the patient’s annual holidays in Turkey with a rapid development of miliary TB under infliximab and escalated immunosuppressive therapy. This case report demonstrates the difficulties in differentiating intestinal TB from other granulomatous conditions such as Crohn’s disease. The diagnostic tools for gastrointestinal tuberculosis are discussed in detail regarding their sensitivity, specificity as well as positive and negative predictive values.
Effect on Quality of Life of Continuous Positive Airway Pressure in Patients with Obstructive Sleep Apnea Syndrome: A Meta-analysis

Abstract:Continuous positive airway pressure (CPAP) is the standard treatment for obstructive sleep apnea syndrome (OSAS). However, the impact of CPAP on quality of life (QOL) is controversial. The aim of this study was to systematically review and determine whether CPAP improves QOL in patients with OSAS. We performed a comprehensive literature search to identify studies published between 1966 and 2007 comparing values of CPAP with control. Weighted mean difference (WMD) was used to analyze the data. The pooled WMD was calculated by using a fixed or random-effect model. The outcomes for 1,256 patients from 16 studies, of whom 656 patients underwent CPAP and 600 were controls, were included. CPAP led to significant improvements in the Nottingham health profile part 2 (WMD=1.657; 95% CI=3.005, -0.308; p=0.016), but there was no difference in other general QOL scores. Patients undergoing CPAP scored better in physical function (WMD=3.457; 95% CI=0.144, 6.771; p=0.041), body pain (WMD=4.017; 95% CI= -0.008, 8.042; p=0.05), energy vitality (WMD=6.984; 95% CI = 0.557, 13.411; p=0.033) and physical component summary (PCS) (WMD=2.040; 95% CI=0.045, 4.035; p=0.045) using the SF-36 tool. This meta-analysis shows that CPAP does not improve general QOL scores but does improve physical domains and vitality. Study design and QOL questionnaire tools are important to capture and evaluate information efficiently. However, generic QOL instruments may not be adequate in detecting important changes in quality of life in patients with OSAS.
Effect of Depressive Symptoms on the Adherence to Continuous Positive Airway Pressure in Obstructive Sleep Apnea Patients Using a Path Analysis-based Approach

Abstract:Background The treatment adherence is closely related to the effect of continuous positive airway pressure (CPAP) therapy in patients with obstructive sleep apnea (OSA) , which is also strongly influenced by patients' psychosocial characteristics. However, the influence of depression symptoms on the adherence to CPAP needs to be further explored. Objective To evaluate the relationship of depression symptoms with adherence to CPAP, and the pathwaybetween the associated factors in OSA patients. Methods A total of 177 patients who were diagnosed with OSA and treated with CPAP in the Respiratory and Sleep Medicine Center, Peking University People's Hospital from May 2019 to July 2021 were selected. The general information, disease severity and adherence to CPAP were evaluated. The prevalence of depression symptoms was measured by the 10-item Center for Epidemiologic Studies Depression Scale (CES-D-10) . The severity of insomnia was assessed by the Insomnia Severity Index (ISI) . The influence of daytime sleepiness on activities of daily living was measured by the 10-item Functional Outcomes of Sleep Questionnaire (FOSQ-10) . The 12-item Short Form Health Survey (SF-12) was used to evaluate the quality of life. The relationships among variables were determined by path analysis. Results The prevalence of depressive symptoms in OSA patients was relatively high (19.7%, 35/177) in our study. Depression symptoms were associated with increased insomnia prevalence (r=0.65, P<0.05) , and decreased levels of activities of daily living (r=-0.51, P<0.05) and quality of life (rPCS=-0.27, P<0.05; rMCS=-0.72, P<0.05) . In addition, depressive symptoms had no relationship on the adherence to CPAP (r=0.09, P>0.05) , but could increase the adherence to CPAP by decreasing the activities of daily living (β=0.078, P<0.01) . Conclusion Depressive symptoms were significantly associated with insomnia, decreased quality of life and activities of daily living in OSA patients, and could increase patients' adherence to CPAP through decreasing the activities of daily living.

Abstract:OBJECTIVES:We identified the primary symptoms leading to Chinese patients presenting at hospital with suspected obstructive sleep apnea (OSA) and studied the prevalence and characteristics of OSA in confirmed cases. METHODS:We collected data on 350 consecutive patients (302 males and 43±11 years old) with suspected OSA who underwent overnight polysomnography (PSG). RESULTS:Among all patients, rankings of primary symptoms that led to the patients presenting at hospital for PSG were observed apnea (33%), snoring alone (29%), choking/gasping (13%), daytime sleepiness (5%) and other (20%). For severe OSA, prevalence rate was 61%, apnea hypopnea index (AHI) was 64±18, age was 44±10 years old, body mass index (BMI) was 28±3.5 kg/m(2), and hypertension rate was 28%. CONCLUSIONS:Self-awareness of symptoms led a majority of the patients to present at hospital in China. Compared to currently available case series studies, our results suggest that OSA patients in East Asian countries are characterized by higher prevalence and more severe apnea, younger age, poorer sleep quality, but less obesity and less comorbidity with hypertension, relative to countries in North America, South America and Europe.

Abstract:Obstructive sleep apnea is a chronic illness with increasing prevalence. In addition to associated cardiovascular comorbidities, obstructive sleep apnea syndrome has been linked to poor quality of life, occupational accidents, and motor vehicle crashes secondary to excessive daytime sleepiness. Although continuous positive airway pressure is the gold standard for sleep apnea treatment, its effects on quality of life are not well defined. In the current study we investigated the effects of treatment on quality of life using the data from the Apnea Positive Pressure Long-term Efficacy Study (APPLES), a randomized controlled trial of continuous positive airway pressure (CPAP) versus sham CPAP. The Calgary Sleep Apnea Quality of Life Index (SAQLI) was used to assess quality of life. Overall we found no significant improvement in quality of life among sleep apnea patients after CPAP treatment. However, after stratifying by OSA severity, it was found that long-term improvement in quality of life might occur with the use of CPAP in people with severe and possibly moderate sleep apnea, and no demonstrable improvement in quality of life was noted among participants with mild obstructive sleep apnea.

Abstract:Objective To investigate the compliance and its influence factors of applying continuous positive airway pressure(CPAP) in obstructive sleep apnea hypopnea syndrome(OSAHS) patients.Methods 91 severe OSAHS patients(apnea hyponea index,AHI≥40),whose mean age was (53±11) year,79% was men,body mass index was (27.8±3.6) kg/m2,AHI was (50.9±8.2)/h,CPAP level was (11.6±2.0) cm H2O,were divided into 2 groups according to the use or nonuse of CPAP.The possible influence factors of the use of CPAP (such as age,body mass index,AHI,percentage arterial oxygen saturation,education background,average household income,complications,sleepiness and ventilator heated humidifier) were studied.Results 47% patients gave up the CPAP therapy.The two groups exhibited significant difference (P<0.05) in education background above senior middle school,average household income≥$2000,complications and sleepiness.The Logistic regression analysis showed the CPAP compliance was positively correlated with sleepiness,average household income and complications[OR=12.9(95% CI 2.209-75.338),3.134(1.048-5.673),2.438(1.031-1.190)respectively,all P<0.05].Conclusions Patients with higher average household income,sleepiness or complications tends to rely more on CPAP.

Abstract:Background Obstructive sleep apnea-hypopnea syndrome (OSAHS) is common in people with hypertension and cardiac rhythm disorder. The objectives of this study were to determine the effect of short-term continuous positive airway pressure (CPAP) on blood pressure (BP) and cardiac rhythm in Chinese patients with moderate to severe OSAHS. Patients and methods Eligible patients in the two hospitals were consecutively enrolled into the prospective study. Ambulatory BP monitoring and Holter monitoring were both performed for 24h in 214 patients who previously encountered a full night polysomnography. Ambulatory BP was measured again in the follow-up of 59 patients with OSAHS who underwent home CPAP for 30 days, whereas Holter was repeated within 2-3 days after institution of CPAP therapy in 15 patients with OSAHS. Results Fifty-one patients with OSAHS with hypertension who used CPAP for at least 4h/night received 30 days of CPAP treatment. Added CPAP on usual antihypertension treatment showed that systolic BP, diastolic BP, and mean arterial BP were significantly reduced at night (5.08, 3.05, and 3.73 mmHg, respectively), in the morning (6.31, 4.83, and 5.32 mmHg, respectively), and during the whole a day (3.09, 2.60, and 2.76 mmHg, respectively). There were no significant changes in daytime BP values but did reduce daytime BP by 2.09, 2.37, and 2.28 mmHg, respectively. In addition, CPAP therapy resulted in abolition of most sinus pauses and atrioventricular block in the 15 patients with OSAHS having coexisting pathologically rhythm disturbances, whereas the effect on other types of arrhythmia was not effective enough. Conclusion Short-term CPAP reduced BP modestly in patients with OSAHS with hypertension, especially in the morning and at night-time. Copyright (C) 2018 Wolters Kluwer Health, Inc. Pain Problems for Patients with Mild and Moderate Chronic Obstructive Pulmonary Disease - A Community-Based Study in Shanghai

Abstract:BACKGROUND:Chronic obstructive pulmonary disease (COPD) is a great public health burden worldwide. Few studies have focused on pain problems in patients with mild and moderate COPD in Chinese community settings.METHODS:A cross-sectional study of 283 patients with mild and moderate COPD was conducted in six communities that were randomly sampled in Pudong New Area of Shanghai, China, in 2016. A face-to-face interview was conducted to collect data on personal characteristics and health conditions. The short form McGill Pain Questionnaire and the COPD assessment test (CAT) were applied to evaluate pain problems and health status, respectively.RESULTS:Among 283 subjects, more than one third (37%) had pain problems indicated by the present pain intensity (PPI) scale. COPD patients aged <65 years with exacerbation in the past 12 months or a CAT score of ≥10 had a significantly higher score in affective dimension. Female sex, COPD severity, and length of disease were significantly related to higher scores of the sensory dimension. Those with moderate COPD or a CAT score of ≥10 had significantly higher scores of visual analog scale than those with mild COPD or a CAT score <10. Patients with moderate COPD had a higher rank of PPI than those with mild COPD.CONCLUSION:Pain was common in patients with mild and moderate COPD in the community settings of Shanghai, China. Severity of COPD and CAT score were significantly related to the prevalence of pain. Intervention measures should be developed to improve pain problems for COPD patients.
Analysis of the improvement of quality of life in patients with obstructive sleep apnea hypopnea syndromes associated hypertension with continuous positive airway pressure

Abstract:Objective: This paper aims at analyzing how continuous positive airway pressure (CPAP) could improve the quality of life of patients with obstructive sleep apnea hypopnea syndromes (OSAHS) associated hypertension with continuous positive airway pressure. Methods: 49 patients with moderate or severe OSAHS associated hypertension were selected and were treated by CPAP. Then 5 measures including 24-hour ambulatory blood pressure monitoring, arterial blood gas analysis, and polysomnogram (PSG), the Epworth sleepiness scale (ESS) and the Berlin sleep assessment were carefully observed before and after the CPAP treatment. Results: After the treatment, 49 patients with CPAP were found that blood pressure control is better than before (P ＜ 0.05). Also, the sleep apnea hypopnea index (AHI) was lower than before, and the difference was statistically significant (P ＜ 0.05). The min Sa O2 was higher than that before (P ＜ 0.05). Besides, the Pa O2 was higher than before and the Pa CO2 was lower than before, and the differences of both were statistically significant (P ＜ 0.05). The Epworth somnolence scale (ESS) score and Berlin sleep quality assessment score were lower than that before treatment, and the difference was also statistically significant (P ＜ 0.05). Conclu-sion: CPAP could significantly improve the quality of life of patients with OSAHS associated hypertension.

However, as readers, we identified some areas that warrant further discussion and improvement. Firstly, the categorization of sleep disorders in the study is somewhat broad. The authors combined all types of sleep disorders into a single group without distinguishing between specific conditions such as obstructive sleep apnea (OSA), insomnia, or circadian rhythm disorders, which may have distinct impacts on CLBP. It is worth noting that the NHANES dataset contains specific questionnaires related to OSA, 2,3 making it feasible to analyze this subtype separately. Given the varying pathophysiological mechanisms underlying different sleep disorders, distinguishing these categories could provide a more nuanced understanding of their individual effects on chronic pain. Secondly, while the article's approach of dividing participants into four groups based on the presence or absence of depressive symptoms and sleep disorders is a valid method to analyze their combined effects, it may overlook a potential mediation effect. Specifically, depression could act as a mediator between sleep disorders and the outcome variable, CLBP. In this case, sleep disorders might exacerbate depressive symptoms, which in turn could influence the onset and severity of chronic low back pain. 4 Future research should explore whether depression serves as a mediating factor between sleep disorders and CLBP, as understanding this potential mediation pathway could provide more targeted intervention strategies and deepen our comprehension of the complex relationship between these variables. In our clinical practice as otolaryngologists, we often encounter patients with sleep-disordered breathing who also experience chronic pain and psychological challenges. The findings of this study highlight the critical need to consider the complex interplay between sleep quality, pain, and mental health. By focusing on improving sleep quality—particularly through the treatment of common conditions like obstructive sleep apnea (OSA)—there is potential to not only alleviate pain symptoms but also reduce the prevalence of depressive symptoms. Association Between Sleep Quality and Pain Intensity in Mild Patients with COPD: A Community Study

Abstract:Purpose: Poor sleep quality and pain were common and had been proved as an important influenced factor of quality of life for patients with COPD. The association of sleep quality with pain has been observed in other population but remains unclear in mild patients with COPD from a community setting. Methods: A cross-sectional study was conducted to include eligible mild patients with COPD in Pudong New District of Shanghai. A structured questionnaire was used to collect general and clinical information for the patients. The Chinese version of Pittsburgh Sleep Quality Index (PSQI) and the short form of McGill Pain Questionnaire (SF-MPQ) was used to assess sleep quality and intensity of pain. Logistic regression was performed to test the association between sleeping quality and pain intensity. Results: Two hundred and sixty-four patients with COPD, with an average age of 64 years (SD 5.78 years), were enrolled, and of 52% were women. Seventy-one (26.9%) participants reported at least one exacerbation during the past year. About 28.2% of the patients were classified as having poor sleep quality. Sleep quality was significantly associated with PRI score (adjusted odds ratio (ORad)=2.16, 95% CI: 1.16-4.00) and PPI rank (ORad=1.90, 95% CI: 1.08-3.34). People with daytime disturbance were more likely to have pain (ORad =2.03, 95% CI: 1.18-3.50). Conclusion: Poor sleep quality was common in mild patients with COPD in community and was associated with higher pain intensity. Pain may involve an impairment of sleep quality.

Abstract:OBJECTIVE To investigate the effects of obstructive sleep apnea hypopnea syndrome on the quality of life of the patients. METHODS The quality of life was assessed with the Medical Outcomes Study SF-36 Health Survey in 145 patients with OSAHS and 17 snorers without OSAHS. Excessive Daytime Sleepiness was assessed with Epworth Sleepiness Scale. Pearson linear correlation and stepwise multiple regression analysis were used to study the association between each domain score of SF-36 and patients information,PSG parameters,EDS scores. RESULTS Three domains of SF-36 correlated negatively with Body Mass Index. Four domains correlated negatively with EDS. Three domains and total score correlated significantly with the lowest oxygen saturation. Two domains correlated negatively with the longest time of apnea. AHI and the lowest oxygen saturation have a mild side effect on the quality of life. BMI and EDS score have significantly side effect on the quality of the life. CONCLUSION Comparing to the mild OSAHS patients and snorers without OSAHS,the severe OSAHS patients have worse quality of life. EDS score and BMI have a significantly side effect on the severe OSAHS patients. OSAHS has a side effect on the quality of life of the patients.

Abstract:PurposeOur aim was to determine the pathway underlying the effects of continuous positive airway pressure (CPAP) adherence on intimate relationship with bed partner in men with obstructive sleep apnea (OSA). We hypothesized that CPAP with good adherence affected the intimate relationship with bed partner directly and indirectly, and it was mediated through daytime sleepiness and activity level in men with OSA.MethodsData were obtained from an education program for enhancing CPAP adherence. Men who were newly diagnosed of OSA and CPAP therapy naïve were recruited in a tertiary teaching hospital.ResultsSelf-reported quality of life [Functional Outcomes of Sleep Questionnaire], daytime sleepiness [Epworth Sleepiness Scale (ESS)], and negative emotion symptoms [depression, anxiety, stress scale] were assessed before and after CPAP treatment at 1-year assessment. Seventy-three men were included in the data analysis, with a mean ± SD age of 52 ± 10 years, body mass index of 29.0 ± 5.2 kg/m2, ESS of 9.5 ± 5.6, and median [interquartile range(IR)] apnea and hypopnea index of 31 (21, 56) events/h. The median (IR) CPAP daily usage was 4.3(0, 6.1) h/day. From the path analysis, CPAP therapy was shown to improve intimate relationship directly (ß = 0.185) and indirectly (ß = 0.050) by reducing daytime sleepiness and increasing activity level. However, negative emotion symptoms were not the mediators between CPAP adherence and the intimate relationship.ConclusionsCPAP therapy with good adherence is related directly and indirectly to a better intimate relationship with bed partner in men with OSA. It was possibly attributed to reduced daytime sleepiness and increased activity level.

Abstract:Obstructive sleep apneahypopnea syndrome (OSAHS) refers to the apnea and hypoventilation induced by the repeated collapse and obstruction of the upper airway during sleep.OSAHS causes frequent oxygen desaturation and is accompanied by symptoms such as snoring,sleep structural disturbance,and daytime sleepiness.Positive airway pressure therapy is the first-line treatment for OSAHS,continuous positive airway pressure (CPAP) is most commonly used,however,some patients find it difficult to tolerate to CPAP therapy for varied reasons.Current study shows that poor adherence (30％-60％) is a significant limiting factor in treating OSAHS,which reduces the overall effectiveness of the treatment and leaves many OSAHS patients at heightened risk for cardiovascular and co-morbid conditions,as well as impaired quality of life.It is imperative to explore the factors of poor compliance of CPAP,and to promote CPAP adherence in order to improve health and functional outcomes in all patients with OSAHS.
Effects of short-term continuous positive airway pressure treatment on arterial stiffness in patients with obstructive sleep apnea hypopnea syndrome]

Abstract:To investigate the effect of short-term continuous positive airway pressure (CPAP) treatment on the arterial stiffness in patients with obstructive sleep apnea hypopnea syndrome (OSAHS).Cardio ankle vascular index (CAVI) and blood pressure (BP) were measured before and after sleep in 60 non-hypertensive patients with OSAHS and gender and 60 age-matched healthy controls. CPAP was administrated in 22 of the 60 OSAHS patients. And on the first and third days of the CPAP treatment CAVI and BP were measured in the morning, i. e., after sleep.In the morning, the CAVI of the OSAHS patients was 8.0 +/- 1.2 m/s, significantly higher than that before sleep (7.3 +/- 1.0, P = 0.000), the diastolic BP (DBP) was (86 +/- 12) mm Hg, significantly higher than that before sleep (83 +/- 13 mmHg, P = 0.001), and the mean BP (MBP) was (101 +/- 12) mm Hg, significantly higher than that before sleep (98 +/- 14, P = 0.00116). However, there were no significant differences in these parameters among the controls The systolic BP (SBP) of the OSAHS patients did not changed significantly after sleep, however, there was a tendency to decrease in the controls [(123 +/- 14) vs (121 +/- 13) mm Hg, P = 0.074). After the first night treatment, the CAVI, SBP, DBP, and mean BP of the 22 severe OSAHS patients decreased significantly (all P < 0.05), and after three nights treatment, only the CAVI showed further significant decrease (P < 0.05).Sleep induces increase of artery stiffness in OSAHS patients, but not in the normal controls. Short-term CPAP may decrease CAVI without affecting the blood pressures. Early atherosclerosis in the patients with OSAHS may be reversed by CPAP therapy.
