Abstract:Adult age estimation is one of the most challenging problems in forensic science and physical anthropology. In this study, we aimed to develop and evaluate machine learning (ML) methods based on the modified Gustafson's criteria for dental age estimation. In this retrospective study, a total of 851 orthopantomograms were collected from patients aged 15 to 40 years old. The secondary dentin formation (SE), periodontal recession (PE), and attrition (AT) of four mandibular premolars were analyzed according to the modified Gustafson's criteria. Ten ML models were generated and compared for age estimation. The partial least squares regressor outperformed other models in males with a mean absolute error (MAE) of 4.151 years. The support vector regressor (MAE = 3.806 years) showed good performance in females. The accuracy of ML models is better than the single-tooth model provided in the previous studies (MAE = 4.747 years in males and MAE = 4.957 years in females). The Shapley additive explanations method was used to reveal the importance of the 12 features in ML models and found that AT and PE are the most influential in age estimation. The findings suggest that the modified Gustafson method can be effectively employed for adult age estimation in the southwest Chinese population. Furthermore, this study highlights the potential of machine learning models to assist experts in achieving accurate and interpretable age estimation.

Abstract:Objectives: To investigate the application value of combining the Demirjian's method with machine learning algorithms for dental age estimation in northern Chinese Han children and adolescents. Methods: Oral panoramic images of 10 256 Han individuals aged 5 to 24 years in northern China were collected. The development of eight permanent teeth in the left mandibular was classified into different stages using the Demirjian's method. Various machine learning algorithms, including support vector regression (SVR), gradient boosting regression (GBR), linear regression (LR), random forest regression (RFR), and decision tree regression (DTR) were employed. Age estimation models were constructed based on total, female, and male samples respectively using these algorithms. The fitting performance of different machine learning algorithms in these three groups was evaluated. Results: SVR demonstrated superior estimation efficiency among all machine learning models in both total and female samples, while GBR showed the best performance in male samples. The mean absolute error (MAE) of the optimal age estimation model was 1.246 3, 1.281 8 and 1.153 8 years in the total, female and male samples, respectively. The optimal age estimation model exhibited varying levels of accuracy across different age ranges, which provided relatively accurate age estimations in individuals under 18 years old. Conclusions: The machine learning model developed in this study exhibits good age estimation efficiency in northern Chinese Han children and adolescents. However, its performance is not ideal when applied to adult population. To improve the accuracy in age estimation, the other variables can be considered.

Abstract:Background Machine learning (ML) algorithms play a key role in estimating dental age. In this study, three ML models were used for dental age estimation, based on different preprocessing methods. Aim The seven mandibular teeth on the digital panorama were measured and evaluated according to the Cameriere and the Demirjian method, respectively. Correlation data were used for decision tree (DT), Bayesian ridge regression (BRR), k-nearest neighbors (KNN) models for dental age estimation. An accuracy comparison was made among different methods. Subjects and methods We analyzed 748 orthopantomographs (392 males and 356 females) from eastern China between the age of 5 and 13 years in this retrospective study. Three models, DT, BRR, and KNN, were used to estimate the dental age. The data in ML is obtained according to the Cameriere method and the Demirjian method. Coefficient of determination (R 2 ), mean error (ME), root mean square error (RMSE), mean square error (MSE) and mean absolute error (MAE), the above five metrics were used to evaluate the accuracy of age estimation. Results Our experimental results showed that the prediction accuracy of dental age was affected by ML algorithms. MD, MAD, MSE, RMSE of the dental age predicted by ML were significantly decreased. Among all the methods, the KNN model based on the Cameriere method had the highest accuracy (ME = 0.015, MAE = 0.473, MSE = 0.340, RMSE = 0.583, R 2 = 0.94). Conclusion The results show that the prediction accuracy of dental age is influenced by ML algorithms and preprocessing method. The KNN model based on the Cameriere method was able to infer dental age more accurately in a clinical setting.

An Explainable Machine Learning Approach for Age and Gender Estimation in Living Individuals Using Dental Biometrics

Abstract:Objectives: Age and gender estimation is crucial for various applications, including forensic investigations and anthropological studies. This research aims to develop a predictive system for age and gender estimation in living individuals, leveraging dental measurements such as Coronal Height (CH), Coronal Pulp Cavity Height (CPCH), and Tooth Coronal Index (TCI). Methods: Machine learning models were employed in our study, including Cat Boost Classifier (Catboost), Gradient Boosting Machine (GBM), Ada Boost Classifier (AdaBoost), Random Forest (RF), eXtreme Gradient Boosting (XGB), Light Gradient Boosting Machine (LGB), and Extra Trees Classifier (ETC), to analyze dental data from 862 living individuals (459 males and 403 females). Specifically, periapical radiographs from six teeth per individual were utilized, including premolars and molars from both maxillary and mandibular. A novel ensemble learning technique was developed, which uses multiple models each tailored to distinct dental metrics, to estimate age and gender accurately. Furthermore, an explainable AI model has been created utilizing SHAP, enabling dental experts to make judicious decisions based on comprehensible insight. Results: The RF and XGB models were particularly effective, yielding the highest F1 score for age and gender estimation. Notably, the XGB model showed a slightly better performance in age estimation, achieving an F1 score of 73.26%. A similar trend for the RF model was also observed in gender estimation, achieving a F1 score of 77.53%. Conclusions: This study marks a significant advancement in dental forensic methods, showcasing the potential of machine learning to automate age and gender estimation processes with improved accuracy.

Abstract:Third molar development is used for dental age estimation when all the other teeth are fully mature. In most medicolegal facilities, dental age estimation is an operator-dependent procedure. During the examination of unaccompanied and undocumented minors, this procedure may lead to binary decisions around age thresholds of legal interest, namely the ages of 14, 16 and 18 years. This study aimed to test the performance of artificial intelligence to classify individuals below and above the legal age thresholds of 14, 16 and 18 years using third molar development. The sample consisted of 11,640 panoramic radiographs (9680 used for training and 1960 used for validation) of males (n = 5400) and females (n = 6240) between 6 and 22.9 years. Computer-based image annotation was performed with V7 software (V7labs, London, UK). The region of interest was the mandibular left third molar (T38) outlined with a semi-automated contour. DenseNet121 was the Convolutional Neural Network (CNN) of choice and was used with Transfer Learning. After Receiver-operating characteristic curves, the area under the curve (AUC) was 0.87 and 0.86 to classify males and females below and above the age of 14, respectively. For the age threshold of 16, the AUC values were 0.88 (males) and 0.83 (females), while for the age of 18, AUC were 0.94 (males) and 0.83 (females). Specificity rates were always between 0.80 and 0.92. Artificial intelligence was able to classify male and females below and above the legal age thresholds of 14, 16 and 18 years with high accuracy.

Abstract:In vivo microperfusion techniques were used to investigate the role of Na+‐H+ exchange in the efferent ducts of the rat. Individual efferent ducts were perfused with a Krebs‐Ringer bicarbonate solution (KRB) containing 0, 1, 3, 5 or 7·5 mM amiloride. Concentrations of 1‐5 mM amiloride inhibited fluid reabsorption from the efferent ducts in a linear dose‐dependent manner with an apparent Km of 3 mM. Inhibition was maximal at 5 mM with reabsorption reduced by about 70%. The effects of amiloride were completely reversible and there was little effect of amiloride on luminal osmolality and concentrations of Na+, Cl− or K+. It is concluded that Na+‐H+ exchange is one of the principal mechanisms responsible for fluid and electrolyte reabsorption in the efferent ducts and offers a means by which the efferent ducts are able to achieve flow‐dependent, autoregulated fluid reabsorption.

Abstract:Abstract Background Biological age (BA) has been recognized as a more accurate indicator of aging than chronological age (CA). However, the current limitations include: insufficient attention to the incompleteness of medical data for constructing BA; Lack of machine learning-based BA (ML-BA) on the Chinese population; Neglect of the influence of model overfitting degree on the stability of the association results. Methods and results Based on the medical examination data of the Chinese population (45–90 years), we first evaluated the most suitable missing interpolation method, then constructed 14 ML-BAs based on biomarkers, and finally explored the associations between ML-BAs and health statuses (healthy risk indicators and disease). We found that round-robin linear regression interpolation performed best, while AutoEncoder showed the highest interpolation stability. We further illustrated the potential overfitting problem in ML-BAs, which affected the stability of ML-Bas’ associations with health statuses. We then proposed a composite ML-BA based on the Stacking method with a simple meta-model (STK-BA), which overcame the overfitting problem, and associated more strongly with CA (r = 0.66, P < 0.001), healthy risk indicators, disease counts, and six types of disease. Conclusion We provided an improved aging measurement method for middle-aged and elderly groups in China, which can more stably capture aging characteristics other than CA, supporting the emerging application potential of machine learning in aging research.

Abstract:Age estimation based on the mineralized morphology of teeth is one of the important elements of forensic anthropology. To explore the most suitable age estimation protocol for adolescents in the South China population, 1477 panoramic radiograph images of people aged 2–18 years in the South were collected and staged by the Demirjian mineralization staging method. The dental ages were estimated using the parameters of the Demirjian and Willems. Mathematical optimization and machine learning optimization were also performed in the data processing process in an attempt to obtain a more accurate model. The results show that the Willems method was more accurate in the dental age estimation of the southern China population and the model can be further optimized by reassigning the model through a nonintercept regression method. The machine learning model presented excellent results in terms of the efficacy comparison results with the traditional mathematical model, and the machine learning model under the boosting framework, such as gradient boosting decision tree (GBDT), significantly reduced the error in dental age estimation compared to the traditional mathematical method. This machine learning processing method based on traditional estimation data can effectively reduce the error of dental age estimation while saving arithmetic power. This study demonstrates the effectiveness of the GBDT algorithm in optimizing forensic age estimation models and provides a reference for other regions to use this parameter for age estimation model establishment, and the lightweight nature of machine learning offers the possibility of widespread forensic anthropological age estimation.

Abstract:In forensic practice, medicolegal physicians are often tasked with estimating age using dental evidence. This calls for an uncomplicated, reliable, and reproducible method for dental age estimation, enabling physicians to proceed without specific odontological expertise. Among various dental methods, third molar eruption analyses are less complicated and easier to perform. In our study, we explored the effectiveness of Gambier et al.'s scoring system, which examines the eruption of all third molars. We retrospectively analysed 1032 orthopantomograms (528 males and 504 females) of individuals aged between 15 and 24 years. The mean chronological age increased with the progression of stages (1 to 3) and phases (A to D) of the third molar eruption for both sexes. In terms of stages, none showed significant discrimination between minors ( 18 years), especially for males. However, Gambier's phase D displayed a relatively high likelihood of being 18 years or older, with an overall 85.9 % of males and 95.7 % of females having all third molars in stage 3 being 18 years or older. While the tested method could be helpful in indicating the completion of the 18th year of life, caution is advised (due to a high percentage of false positives), and it should be used alongside other age assessment methods by experts.

A probability model for assessing age relative to the 18-year old threshold based on magnetic resonance imaging of the knee combined with radiography of third molars in the lower jaw

Abstract:Objective: This study aims to generate a statistical model based on magnetic resonance imaging of the knee and radiography of third molars in the lower jaw, for assessing age relative to the 18-year old threshold. Methods: In total, 58 studies correlating knee or tooth development to age were assessed, 5 studies for knee and 7 studies for tooth were included in the statistical model. The relation between the development of the anatomical site, based on a binary system, and age were estimated using logistic regression. Separate meta-populations for knee and tooth were generated from the individual based data for men and women. A weighted estimate of probabilities was made by combining the probability densities for knee and tooth. Margin of errors for males and females in different age groups and knee and tooth maturity were calculated within the larger framework of transition analysis using a logit model as a base. Evidentiary values for combinations of knee and tooth maturity were evaluated with likelihood ratios. Results: For males, the sensitivity for the method was calculated to 0.78 (probability of correctly classifying adults), the specificity 0.90 (probability of correctly classifying minors), the negative predictive value 0.80 (proportion identified minors are minors) and the positive predictive value 0.89 (proportion identified adults are adults) indicating a model better at identifying minors than adults. The point at which half the female population has reached closed knee lies before the 18-year threshold, adding the knee as an indicator lowers specificity and increases sensitivity. The sensitivity when using tooth as an indicator for females is 0.24 and specificity 0.97, signifying few minors misclassified as adults but also a low probability of identifying adults. The negative predictive value for women when using tooth as the sole indicator is 0.56 and positive predictive value 0.88. Probabilities were calculated for males and females assuming a uniform age distribution between 15 and 21years. The calculated margin of error of minors classified as adults in a population between 15 and 21 years with the model was 11% for males and 12% for females. Further, the evidentiary value as well as margin of error vary for different combinations of knee and tooth maturity. Conclusion: The statistical model based on the combination of MRI knee and radiography of mandibular third molars is a valid method to assess age relative to the 18-year old threshold when applied on males and of limited value in females.

Abstract:The present study aims to evaluate the relation between chronological age and the ratio of pulp volume (PV) to enamel volume (EV) of impacted mandibular third molars (IMTMs) by using cone-beam computed tomography (CBCT) images and an improved 3D image segmentation technique. A sample of CBCT images of IMTM was collected from 414 northern Chinese subjects (214 male and 200 female clinical patients) ranging in age from 20 to 65 years. The GrowCut effect image segmentation (GCEIS) module algorithm was used to calculate the PV and EV from CBCT images. The total sample was divided into a training group and validation group in a ratio of 7 to 3. The PV/EV ratio (PEr) in the training sample was used to develop a mathematical formula for age estimation as follows: age = − 5.817–21.726 × Ln PEr (p < 0.0001) (Ln, natural logarithm). The mean absolute error (MAE) and root mean square error (RMSE) were used to determine the precision and accuracy of the mathematical formula in the validation group and all samples. The MAEs in the male, female, and pooled gender samples were 9.223, 7.722, and 8.41, respectively, and the RMSEs in the male, female, and pooled gender samples were 10.76, 9.58, and 9.986, respectively. The precise and accurate results indicate that the PEr of IMTM in CBCT images is a potential index for dental age estimation and is possible to be used in forensic medicine.

Age estimation based on 3D pulp chamber segmentation of first molars from cone-beam-computed tomography by integrated deep learning and level set

Abstract:Objectives: To develop an automatic segmentation method to segment the pulp chamber of first molars from 3D cone-beam-computed tomography (CBCT) images, and to estimate ages by calculated pulp volumes. Materials and methods: Patients with CBCT scans were retrospectively identified. The age estimation was formulated as CBCT image segmentation using a coarse-to-fine strategy by integrated deep learning (DL) and level set (LS), followed by establishing a linear regression model. On the training data, DL model was trained for coarse segmentation. The validation set was to determine the optimal DL model, and a LS method established on it was to refine the coarse segmentation. On the testing data, the integrated DL and LS method was applied for pulp chamber segmentation, followed by volume calculation and age estimation. Statistical analysis was performed by Wilcoxon rank sum test to demonstrate gender difference in pulp chamber volume, and volume difference between maxillary and mandibular molars. Wilcoxon signed-rank test was adopted to compare true and estimated ages. Results: A total of 180 CBCT studies were randomly divided into 37/10/133 patients for training, validation, and testing data, respectively. In the training and validation sets, the results showed high spatial overlaps between manual and automatic segmentation (dice = 87.8%). For the testing set, the estimated human ages were not significantly different with true human age (p = 0.57), with a correlation coefficient r = 0.74. Conclusions: An integrated DL and LS method was able to segment pulp chamber of first molars from 3D CBCT images, and the derived pulp chamber volumes could effectively estimate the human ages.
Third Molar Eruption in Dental Panoramic Radiographs as a Feature for Forensic Age Assessment-Presentation of a New Non-Staging Method Based on Measurements

Abstract:The evaluation of third molar eruption in dental panoramic radiographs (DPRs) constitutes an evidence-based approach for forensic age assessment in living individuals. Existing methodologies involve staging morphological radiographic findings and comparing them to reference populations. Conversely, the existing literature presents an alternative method where the distance between third molars and the occlusal plane is measured on dental plaster models. The aim of this study was to adapt this measurement principle for DPRs and to determine correlation between eruption and chronological age. A total of 423 DPRs, encompassing 220 females and 203 males aged 15 to 25 years, were examined, including teeth 38 [FDI] and 48. Two independent examiners conducted the measurements, with one examiner providing dual assessments. Ultimately, a quotient was derived by comparing orthogonal distances from the mesial cementoenamel junctions of the second and third molars to a simplified radiological occlusal plane. This quotient was subsequently correlated with the individual's age. We estimated correlations between age and quotients, as well as inter- and intra-rater reliability. Correlation coefficients (Spearman's rho) between measurements and individuals' ages ranged from 0.555 to 0.597, conditional on sex and tooth. Intra-rater agreement (Krippendorf's alpha) ranged from 0.932 to 0.991, varying according to the tooth and sex. Inter-rater agreement ranged from 0.984 to 0.992, with distinctions drawn for different teeth and sex. Notably, all observer agreement values fell within the "very good" range. In summary, assessing the distance of third molars from a simplified occlusal plane in DPRs emerges as a new and promising method for evaluating eruption status in forensic age assessment. Subsequent reference studies should validate these findings.

Abstract:Three-dimensional surface area analyses of developing root apices for age estimation in children and young adults have shown promising results. The current study aimed to apply this three-dimensional method to develop a regression model for estimating age in Malaysian children aged 7 to 14 using developing maxillary second premolars. A training sample of 155 cone-beam computed tomography scans (83 Malays and 72 Chinese) was analysed, and the formula was subsequently validated on an independent sample of 92 cone-beam computed tomography scans (45 Malays and 47 Chinese). The results showed a strong correlation (r= 94%) between the chronological age as a dependent variable and the predictor variables, including root surface area of the apex, sex, ethnicity, and root development status (open/closed apices). For this model, the predictor variables accounted for 88.4% of the variation in age except sex and ethnicity. A mean absolute error value of 0.42 indicated that this model can be reliably used for Malaysian children. In conclusion, this study recognises the method of three-dimensional surface area analyses as a valuable tool for age estimation in forensic and clinical practice. Further studies are highly recommended to assess its effectiveness across different demographic groups.

Abstract:Objective: The aim of this study was to estimate the chronological age (CA) of a growing individual using a new machine learning approach on Cone Beam Computed Tomography (CBCT). Materials and methods: The dataset included 48 CBCT and hand-wrist radiographs of growing individuals. 12 landmarks related to trigeminal trajectories were plotted on each CBCT and principal component analysis was applied for dimensionality reduction. The estimated CA was obtained using a decision tree. Finally, a genetic algorithm was implemented to select the best set of landmarks that would optimize the estimation. The age was also assessed following Greulich and Pyle's (GP) method on hand-wrist radiographs. The results (GP and Machine Learning) were then compared to the true CA. Results: Among the 12 landmarks, the genetic algorithm selected 7 optimal features, and 12 principal components out of 36. The best results for age prediction were obtained by a combination of genetic algorithm, principal component analysis, and regression tree where the Mean Squared Error (MSE) and Mean Absolute Error (MAE) were respectively 1.29 and 0.92. These outcomes showed improved accuracy compared to those of the hand-wrist method (MSE = 2.038 and MAE = 1.775). Conclusions: A numerical application on a dataset of CBCT showed that the proposed machine learning method achieved an improved accuracy compared to conventional methods and had satisfying performance in assessing age for forensic purposes. Validation of the presented method on a larger and more diverse sample would pave the way for future applications in forensic science as a tool for age prediction.

Abstract:Abstract Age assessment of the living is a fundamental procedure in the process of human identification, in order to guarantee fair treatment of individuals, which has ethical, civil, legal, and medical repercussions. The careful selection of the appropriate methods requires evaluation of several parameters: accuracy, precision of the method, as well as its reproducibility. The approach proposed by Mincer et al. (1993), adapted from Demirjian et al. (1973), exploring third molar mineralization, is one of the most frequently considered for age estimation of the living. Thus, this work aims to assess potential bias in the data collection when applying the classification stages for dental mineralization adapted by Mincer and colleagues (1993). A total of 102 orthopantomographs (OPGs), of clinical origin, belonging to individuals aged between 12 and 25 years (x ̅=20.12 years, SD = 3.49 years; n = 65 females, 37 males, all of Portuguese nationality) were included and a retrospective analysis performed by five observers with different levels of experience (high, average, and basic). The performance and agreement between five observers were evaluated using Weighted Cohen’s Kappa and the Intraclass Correlation Coefficient. To access the influence of impaction on third molar classification variables were tested using ordinal logistic regression GENLIN. It was observed variations in the number of teeth identified among the observers but agreement levels ranged from moderate to substantial (0.4 to 0.8). Upon closer examination of the results, it was observed that although there were discernible differences between highly experienced observers and those with less experience, the gap was not as significant as initially hypothesized, and a greater disparity between the classifications of the upper (0.24 to 0.49) and lower third molars (>0.55) was observed. When bone superimposition is present, the classification process is not significantly influenced, however, variation in teeth angulation affects the assessment. The results suggest that with an efficient preparation, the level of experience as a factor can be overcome. Mincer and colleague’s classification system can be replicated with ease and consistency, even though the classification of upper and lower third molars presents distinct challenges.

A Machine Learning Based Aging Measure among Middle-Aged and Older Chinese Adults: the China Health and Retirement Longitudinal Study

Abstract:Objective: Biological age (BA) has been accepted as a more accurate proxy of aging than chronological age (CA). This study aimed to use machine learning (ML) algorithms to estimate BA in the Chinese population.Materials and methods: We used data from 9,771 middle-aged and older Chinese adults (≥45 years) in the 2011/2012 wave of the China Health and Retirement Longitudinal Study and followed until 2018. We used several ML algorithms (e.g., Gradient Boosting Regressor, Random Forest, CatBoost Regressor, and Support Vector Machine) to develop new measures of biological aging (ML-BAs) based on physiological biomarkers. R-squared value and mean absolute error (MAE) were used to determine the optimal performance of these ML-BAs. We used logistic regression models to examine the associations of the best ML-BA and a conventional aging measure—Klemera and Doubal method-BA (KDM-BA) we previously developed—with physical disability and mortality, respectively.Results: The Gradient Boosting Regression model performed the best, resulting in an ML-BA with an R-squared value of 0.270 and an MAE of 6.519. This ML-BA was significantly associated with disability in basic activities of daily living, instrumental activities of daily living, lower extremity mobility, and upper extremity mobility, and mortality, with odds ratios ranging from 1 to 7% (per 1-year increment in ML-BA, all P < 0.001), independent of CA. These associations were generally comparable to that of KDM-BA.Conclusion: This study provides a valid ML-based measure of biological aging for middle-aged and older Chinese adults. These findings support the application of ML in geroscience research and may help facilitate preventive and geroprotector intervention studies.
Dental Age Assessment Based on Developmental Stages and Maturity Index of Third Molars for Medico-Legal Purposes

Abstract:As results may vary depending on the method of examination, this paper analyzes methods of age estimation based on the maturation index of third molars (I3M) and Kohler's developmental stages in living individuals. A total of 1475 orthopantomograms were analyzed. The results showed that the development of third molars tended to be more advanced in males than in females. Regression equations that included the value of the developmental stage of the left third molar most accurately predicted chronologic age in males and females. Using the I3M method, there were no statistically significant bilateral differences between the mandibular right and left third molars. Overall, 82.92% of males and only 72.82% of females were correctly classified with the cut-off value (0.08) for the left mandibular third molar index. In addition, 81.97% of individuals were correctly classified as minors and adults using the Kohler method, while only 78.62% of individuals were correctly classified as minors and adults using the I3M method. Based on the misclassification of minors as adults, both methods should be used with caution and overestimation of age should be considered, especially for those close to 18 years of age.
