Abstract:Pancreatic islet isolation is critical for type 2 diabetes research. Although -omics approaches have shed light on islet molecular profiles, inconsistencies persist; on the other hand, functional studies are essential, but they require reliable and standardized isolation methods. Here, we propose a simplified protocol applied to very small-sized samples collected from partially pancreatectomized living donors. Islet isolation was performed by digesting tissue specimens collected during surgery within a collagenase P solution, followed by a Lympholyte density gradient separation; finally, functional assays and staining with dithizone were carried out. Isolated pancreatic islets exhibited functional responses to glucose and arginine stimulation mirroring donors' metabolic profiles, with insulin secretion significantly decreasing in diabetic islets compared to non-diabetic islets; conversely, proinsulin secretion showed an increasing trend from non-diabetic to diabetic islets. This novel islet isolation method from living patients undergoing partial pancreatectomy offers a valuable opportunity for targeted study of islet physiology, with the primary advantage of being time-effective and successfully preserving islet viability and functionality. It enables the generation of islet preparations that closely reflect donors' clinical profiles, simplifying the isolation process and eliminating the need for a Ricordi chamber. Thus, this method holds promises for advancing our understanding of diabetes and for new personalized pharmacological approaches.

Abstract:Islet β cell secretion deficiency and( or) the decreased insulin sensitivity of target tissue are the important pathophysiological mechanisms of diabetes. So,detection and assessment of isletβcell function in the early stages,could be of great significance for disease severity evaluation,early intervention and prognosis of the disease. At present,the main methods of the testing and assessment ofβcell function includeβcell function evaluating indexes,pulsatile insulin secretion,insulin secretion by glucose or non-glucose secretagogues and func-tion testing by other secretions of isletβcells. Among of them,βcell functional assessment methods by detecting C-peptide( especially aspects such as 90 minutes of C-peptide testing in mixed-meal tolerance test,urinary C-pep-tide creatinine ratio) have experienced some progress in recent years.

Abstract:The ability to consistently and reliably assess the total number and the size distribution of isolated pancreatic islet cells from a small sample is of crucial relevance for the adequate characterization of islet cell preparations used for research or transplantation purposes. Here, data from a large number of isolations were used to establish a continuous probability density function describing the size distribution of human pancreatic islets. This function was then used to generate a polymeric microsphere mixture with a composition resembling those of isolated islets, which, in turn, was used to quantitatively assess the accuracy, reliability, and operator-dependent variability of the currently utilized manual standard procedure of quantification of islet cell preparation. Furthermore, on the basis of the best fit probability density function, which corresponds to a Weibull distribution, a slightly modified scale of islet equivalent number (IEQ) conversion factors is proposed that incorporates the size distribution of islets and accounts for the decreasing probability of finding larger islets within each size group. Compared to the current calculation method, these factors introduce a 4–8% downward correction of the total IEQ estimate, but they reflect a statistically more accurate contribution of differently sized islets.

Abstract:A simple method for the isolation of intact islets from the normal rat pancreas is described. The method is based upon disruption of the acinar parenchyma by injecting Hanks solution into the pancreatic duct system followed by incubation of the pancreas in collagenase. Islets can be separated rapidly from this mixture by sedimentation. The isolated islets release insulin in vitro and appear normal by light and electron microscopy after incubation. A centrifugation method is also described for isolation of large quantities of islets for biochemical studies.

Abstract:Aims/hypothesisEfficient islet isolation is an important prerequisite for successful clinical islet transplantation. Although progressively improved, islet yield and quality are, however, unpredictable and variable and require standardisation.MethodsSince 1989 we have processed 437 pancreases using the automated method. The donor characteristics, pancreas procurement, and digestion and purification procedures including a wide enzyme characterisation of these pancreases were analysed and correlated with islet yield and transplant outcome.ResultsBy univariate analysis, islet yield was significantly associated with donor age (r=0.16; p=0.0009), BMI (r=0.19; p=0.0004), good pancreas condition (p=0.0031) and weight (r=0.15; p=0.0056), total collagenase activity (r=0.22; p=0.0001), adjusted collagenase activity/mg (r=0.18; p=0.0002), collagenase activity/solution volume (r=0.18; p=0.0002) and neutral protease activity/solution volume (r=0.14; p=0.0029). A statistically significant contribution to the variability of islet yield in a multivariate analysis performed on donor variables was found for donor BMI (p=0.0008). In a multivariate analysis performed on pancreas variables a contribution was found for pancreas weight (p=0.0064), and for a multivariate analysis performed on digestion variables we found a contribution for digestion time (p=0.0048) and total collagenase activity (p=0.0001). Twenty-four patients with type 1 diabetes received single islet preparations from single donors. In these patients, multivariate analyses showed that the reduction in insulin requirement was significantly associated with morphological aspects of islets (p=0.0010) and that 1-month C-peptide values were associated with islet purity (p=0.0071).Conclusions/interpretationThese data provide baseline donor, digestion and purification selection criteria for islet isolation using the automated method and indicate that the morphological aspect may be a clinically relevant measure of islets on which the decision for transplant can be based.

Abstract:Objective Evaluation of mitochondrial oxygen consumption and ATP production is important to investigate pancreatic islet pathophysiology. Most studies use cell lines due to difficulties in measuring primary islet respiration, which requires specific equipment and consumables, is expensive and poorly reproducible. Our aim was to establish a practical method to assess primary islet metabolic fluxes using standard commercial consumables. Methods Pancreatic islets were isolated from mice/rats, dispersed with trypsin, and adhered to pre-coated standard Seahorse or Resipher microplates. Oxygen consumption was evaluated using a Seahorse Extracellular Flux Analyzer or a Resipher Real-time Cell Analyzer. Results We provide a detailed protocol with all steps to optimize islet isolation with high yield and functionality. Our method requires a few islets per replicate; both rat and mouse islets present robust basal respiration and proper response to mitochondrial modulators and glucose. The technique was validated by other functional assays, which show these cells present conserved calcium influx and insulin secretion in response to glucose. We also show that our dispersed islets maintain robust basal respiration levels, in addition to maintaining up to 89% viability after five days in dispersed cultures. Furthermore, OCRs can be measured in Seahorse analyzers and in other plate respirometry systems, using standard materials. Conclusions Overall, we established a practical and robust method to assess islet metabolic fluxes and oxidative phosphorylation, a valuable tool to uncover basic β-cell metabolic mechanisms as well as for translational investigations, such as pharmacological candidate discovery and islet transplantation protocols.

Abstract:Objective:To explore the method of obtaining enough mouse islets with high purity in order to set up an animal model for clinical islet transplantation.Methods: The 6~8 weeks male C57BL/6 mice weighing 25~30 g were intraperitoneal anesthetized before the operation of common bile duct ligation.Then we isolated and purified mice islets by adopting collagenⅤ retro perfusion,situ digestion,gradient centrifugation in Ficoll-400 solution and sorted islets using sterile capillary pipettes in the phase contrast microscope.Islets purity was assessed by dithizone staining.Eventually,we cultured islet cells and observed the shape of islets on day 3,8 and 24 separately.Results: From this way,we could obtain 390±20 islets in each mouse.The isolated islets were round or mass,50-150 μm in diameter,complete and bright.The purity of the islets was more than 85%.Conclusions: We improve the method of isolating islets including retro perfusion,situ digestion and gradient centrifugation in Ficoll-400 solution,which is timesaving and effective.The cultured islet cells had high activity and formed single cells in petri dishes within 24 days,which is suitable for further study.

Abstract:Objective: Islet transplantation is an effective treatment for type 1 diabetes. However, transplant success depends on quick islet assessment because islets deteriorate 2-3 days after isolation. A new tool, single-cell Western blot (scWestern), offers results within one day. In this study, we aimed to test the suitability of scWestern to detect protein markers for beta (insulin), alpha (glucagon), and delta (somatostatin) cells, the three major endocrine cell types in islets. Methods: We characterized the antibody specificity, signal intensity, and cell identification on the scWestern platform, and then compared the islet cell composition analysis between scWestern and immunohistochemistry performed by the Integrated Islet Distribution Program (IIDP). Results: Islet cell composition is comparable for alpha and beta cells, but not delta cells. Protein expression levels of insulin, glucagon, and somatostatin in individual islet cells varied greatly, highlighting cell type heterogeneity. Surprisingly, scWestern revealed double-hormonal cells (~1%), co-expressing insulin and somatostatin or insulin and glucagon, in non-diabetic and non-obese adult human islets, which was confirmed by confocal immunofluorescence microscopy. Conclusions: These results demonstrate that each alpha, beta, and delta cells express varying levels of peptide hormones, and a small subpopulation co-expresses double hormones in normal human islets. The scWestern platform will enable timely assessment of beta cell mass in isolated islets before clinical transplantation.

Abstract:Purpose To explore good methods for isolation and purification of rat islets. Methods Islets of langerhans were isolated from male Sprague-Dawley rat pancreata by a collagenase P digestion technique and discontinuous Ficoll density gradient centrifugation.Islets purity was assessed by dithizone staining with islets counted and scored for size.The viability of islets was assessed by fluorescence staining of aridine orange(AO) and propidium iodide(PI).The function of purified islets was determined by glucose stimulated test.The changes of cultured islets were observed by using microscope. Results The total number of purified islets from one donor was(629±102) IEQ.The purity of islets was more than 90%.The viability of islets cells was more than 90%.The secretion amount of insulin was((3.701 5±0.492) 0 mIU/L) and(10.477 1±2.233 4) mIU/L at 1.67 mmol/L and 16.7 mmol/L concentration of glucose respectively,the stimulation index was 2.837 8±0.102 3. Conclusions The digestion of collagenase P for rat has a high yield and high purity only when isolation conditions having been grasped.

Abstract:OBJECTIVE:To establish an semi-automated effective method for large-scale purification of islet cells from human pancreas. METHODS:Human pancreas tissue was digested with collagenase P using a semi-automated pancreas-digestion system followed by purification in a HCA-Ficoll continuous gradient using Cobe2991 cell separator. After isolation, the islet cell yield and purity was evaluated with light microscope with DTZ staining, and the islet function assessed by insulin release assay in vitro. RESULTS:The number of the islets collected from each pancreas averaged 38 6201-/+78 219 islet equivalents (IEQ) before purification, and 231 420-/+28 054 IEQ after the purification with discontinuous gradient centrifugation. From each gram of the pancreatic tissue, 3148-/+317 IEQ were obtained with an average purity of (62.81-/+2.68) %. The purified islets responded well to high-concentration (16.7 mmol/L) glucose stimulation with a 2.53-fold increase of insulin secretion over the basal level (3.3 mmol/l, P<0.001). CONCLUSION:The established semi-automated method can be applicable for large-scale purification of fully functional islet cells from human pancreas.

Abstract:Islet transplantation represents a promising therapeutic approach for diabetes management, yet the isolation and evaluation of pancreatic islets remain challenging. This study focuses on the isolation of islets from rabbit pancreases, followed by a comprehensive assessment of their viability and functionality. We developed a novel method for isolating islet cells from the pancreas of adult rabbits. We successfully isolated viable islets, which were subsequently evaluated through a combination of viability assays, an insulin enzyme-linked immunosorbent assay (ELISA), and fluorescence lifetime imaging microscopy (FLIM). The viability assays indicated a high percentage of intact islets post-isolation, while the insulin ELISA demonstrated robust insulin secretion in response to glucose stimulation. FLIM provided insights into the metabolic state of the islets, revealing distinct fluorescence lifetime signatures correlating with functional viability. Our findings underscore the potential of rabbit islets as a model for studying islet biology and diabetes therapy, highlighting the efficacy of combining traditional assays with advanced imaging techniques for comprehensive functional assessments. This research contributes to the optimization of islet isolation protocols and enhances our understanding of islet functional activity dynamics in preclinical settings.

A Preliminary Study on Isolation and Purification of Human Pancreatic Islets

Abstract:Objective To establish the method for human pancreatic islet isolation and purification.Methods Ten adult human pancreas were digested by the modified Ricordi procedure and islets were purified by continuous density gradient.The islet yield was counted with international standard known as islet equivalent(IEQ).Results Ten adult human pancreas were isolated and purified,and five of them were counted with islet equivalent(IEQ).The islet yield ranged from 6367 to 108725 IEQs/pancreas,with the average of 47678 IEQs/pancreas,and 2055 IEQ/g in average.Conclusions Human islets isolation with modified Ricordi procedure can harvest a large amount of islets,but the technique should be more skillfully operated.
Method of gradient centrifugation for mouse pancreatic islet isolation

Abstract:Objective To establish a stable method for mouse pancreatic islet isolation. Methods Male Balb/c mice were used for islet harvest and isolation. The pancreas were digested by 0.5mg/ml col agenase, and the islets were isolated and puri-fied with different gradient centrifugation strategy, stained with DTZ, and observed by microscopy to determine an optimal proto-col for islet isolation. AO/PI staining was used to detect the activity of isolated islets. Different protocols were compared usingx2 and one- way analysis of variance (ANOVA). Results The optimal conditions used for isolation of pancreatic islets from Balb/c mice were as fol ows:using Ficol - 1.108 (4ml), 1.096 (2ml), 1.069 (2ml), HBSS (2ml), acceleration 1- deceleration 1, 400rpm for 4min transfer to 1800rpm for another 8min. The preliminary yield was (125±39)IEQ, purity was (79.6±10.5)%, then underwent another gradient centrifugation with the same strategy, the purity significantly increased to (96.2±1.4)%, while the yield was stil satisfying. AO/PI staining confirmed good activity of the isolated islets. Conclusion The modified islet isolation method is a better choice to obtain higher purity and considerable numbers of islets.
[Islet Perifusion: a New Method to Evaluate First-Phase Insulin Secretory Function of Beta-Cell in Vitro].

Abstract:OBJECTIVE:To set up islet perifusion system, a new method to evaluate first-phase insulin secretory function of beta-cell in vitro.METHODS:Islet perifusion system was set up, including perifusion framework, waterbath system, infusion pump system, afferent system, islet capsule, and efferent system. Kinetics of insulin release in vitro was studied using the perifusion system. Pancreatic islets were isolated as mentioned above and used freshly after isolation. Size-matched 50 islets were placed in each column. Then the columns were gently closed with the top adaptors, immersed in vertical position and controlled temperature in the water bath at 37 degrees C. The perifusion medium was maintained at 37 degrees C in a water bath. And all columns were perifused in parallel at a flow rate of 0.5 ml/min with KRBB (2.8 mmol/L glucose) at 37 degrees C. After 60 min static incubation with KRBB (2.8 mmol/L glucose), the islets were stimulated in the continuous presence of a high concentration of 16.7 mmol/L glucose. Samples were collected every 20-second until 2 min, every 1 min until 5 min, thereafter every 5 min until 30 min. Samples were immediately stocked at -80 degrees C until further analysis. Insulin concentration was measured with an insulin RIA kit.RESULTS:An Islet perifusion system was established successfully, and kinetic curves of insulin secretion of db/m and db/db mice were traced out.CONCLUSION:An established islet perifusion system, could be widely used to evaluate insulin secretory function of beta-cell in in vitro research of diabetes.

Abstract:Insulin secreted from islets of Langerhans is the main hormone to reduce blood glucose. Examination of insulin secretion patterns at the single islet level reveals functional differences in the timings and patterns of release. This heterogeneous response highlights the importance of developing systems to measure dynamic release from small numbers of islets in parallel. Toward this, we describe fluorescence anisotropy imaging immunoassays as a relatively simple method for increased throughput of islet secretion measurements. In this system, vacuum pressure from a syringe pump pulled perfusate from 12 islet chambers and reagents into 12 parallel mixing channels for a competitive immunoassay. Light from a Xe arc lamp was filtered and polarized prior to focusing on the microfluidic device at the region where the 12 mixing channels converged. Emission was collected and passed through vertical and horizontal emission polarizers housed in an automated filter wheel before being imaged with a sCMOS camera for the determination of anisotropy. This microfluidic system was tested by monitoring insulin release from groups of murine and human islets. Heterogeneity was observed in the islet traces; however, the presence of islets affected the resistance of the islet chambers, hampering insulin quantification. Nonetheless, this microfluidic system is a step towards increasing the throughput of hormone release measurements from islets of Langerhans.

Scaffold-supported Transplantation of Islets in the Epididymal Fat Pad of Diabetic Mice

Abstract:Islet transplantation has been clinically proven to be effective at treating type 1 diabetes. However, the current intrahepatic transplantation strategy may incur acute whole blood reactions and result in poor islet engraftment. Here, we report a robust protocol for the transplantation of islets at the extrahepatic transplantation site-the epididymal fat pad (EFP)-in a diabetic mouse model. A protocol to isolate and purify islets at high yields from C57BL/6J mice is described, as well as a transplantation method performed by seeding islets onto a decellularized scaffold (DCS) and implanting them at the EFP site in syngeneic C57BL/6J mice rendered diabetic by streptozotocin. The DCS graft containing 500 islets reversed the hyperglycemic condition within 10 days, while the free islets without DCS required at least 30 days. The normoglycemia was maintained for up to 3 months until the graft was explanted. In conclusion, DCS enhanced the engraftment of islets into the extrahepatic site of the EFP, which could easily be retrieved and might provide a reproducible and useful platform for investigating the scaffold materials, as well as other transplantation parameters required for a successful islet engraftment.

Abstract:Objective:To optimize the isolation and purification of rat islets. Methods:Pancreas of SD rats were digested by collagenase V and purified by Histopaque-1077. The islets were stained with trypan blue to identify the viability, and stained with dithezone to count the yield. Glucose stimulated insulin secretion(GSIS) of islets was performed in vitro. Allogeneic islets transplantation via portal vein was performed in STZ-induced diabetic SD rats and the blood glucose level was observed. Results:The average yield was 646 ± 67 perrat and the livability rate was above 90%. GSIS showed that SI(stimulating index) was 2.6, 1.7 and 1.4 at the 1 st, 3 rd, 5 th respectively. There was significant difference between low glucose groups and high glucose groups(P 0.05). After transplantation, the blood glucose decreased to 11.1 mmol/L at the 2nd day and kept below 15 mmol/L for 8 days. Conclusion:Collagenase V digestion combined with Histopaque-1077 purification is an effective method for rat islets isolation, and the islet yield is high with good function.

Abstract:Pancreatic β cells secrete insulin in response to elevated levels of glucose. Stem cell derived β (SCβ) cells aim to replicate this glucose-stimulated insulin secretion (GSIS) function, but current preparations cannot provide the same level of insulin as natural β cells. Here, we develop an assay to measure GSIS at the single cell level to investigate the functional heterogeneity of SCβ cells and donor-derived islet cells. Our assay involves randomly depositing single cells and insulin capture microbeads in open-top nanowells (40 × 40 × 55 μm3) fabricated on glass-bottom imaging microwell plates. Insulin secreted from single cells is captured on microbeads and then stained using a detection antibody. The nanowell microstructure limits diffusion of secreted insulin. The glass substrate provides an optically flat surface for quantitative microscopy to measure the concentration of secreted insulin. We used this approach to measure GSIS from SCβ cells and donor-derived islet cells after 15 minutes exposure to 3.3 mM and 16.7 mM glucose. Both cell types exhibited significant GSIS heterogeneity, where elite cells (<20%) produced the majority of the secreted insulin (55-78%). This assay provides an immediate readout of single cell glucose-stimulated insulin secretion in a flexible well plate-based format.
Improved islet isolation from rat pancreas using 35% bovine serum albumin in combination with Dextran gradient separation

Abstract:We have developed a gradient separation technique in which albumin-coated (35% BSA) rat islets are isolated in Dextran (BSA/D). Versus Dextran alone (DA), BSA/D increases mean islet yield from 850 +/- 115 to 1222 +/- 210 islets/donor (mean +/- SD, P=.001). With numbers of donors held constant, the time to euglycemia was significantly shorter in syngenic recipients of BSA/D than DA islets (5.7 +/- 4.8 versus 14.2 +/- 5.6 days, P=.02). However, when the number of islets were held constant, differences in time to cure were not statistically significantly different (P>.19): with 1200 islets, 3.8 +/- 2.8 (DSA/D) versus 9.2 +/- 8.2 (DA) days; with 750 islets, 18.3 +/- 5.8 (BSA/D) versus 18.0 +/- 4.5 (DA) days. In all groups, posttransplant day 1 glucose was lowest with the BSA/D technique. Thus BSA/D isolation gives a higher yield with at least equal viability. We recommend the BSA/D technique for its efficiency and economy.
