Abstract:Background and Objectives: Aortic arch pathologies represent a surgical challenge. The challenge is partly due to the necessity of complex cerebral, visceral, and myocardial protection measures. Aortic arch surgery generally requires a significant duration of circulatory arrest, which includes deep hypothermia levels with the associated sequelae. This retrospective observational study shows the feasibility of a strategy that reduces circulatory arrest duration and eliminates the need for deep hypothermia during the procedure. Materials and Methods: Between January 2022 and January 2023, 15 patients (n = 15) with type A aortic dissection underwent total arch replacement with a frozen elephant trunk. Cardiopulmonary bypass and organ perfusion were established via arterial lines in the right axillary artery and one of the femoral arteries. In the latter vessels, a y-branched arterial cannula was used (ThruPortTM), allowing for endo-clamping of the stent part of the frozen elephant trunk with a balloon and subsequent perfusion of the lower body. Results: Applying this modified perfusion technique, circulatory arrest time could be reduced to a mean of 8.1 ± 4.2 min, and surgery was performed at a mean lowest body temperature of 28.9 ± 2.3 °C. The mean ICU and hospital stays were 18.3 ± 13.7 days and 23.8 ± 11.7 days, respectively. The rate for 30-day survival was 100%. Conclusions: By applying our modified perfusion technique, the circulatory arrest duration was below ten minutes. As a consequence, deep hypothermia could be avoided, and surgery could be performed at moderate hypothermia. Future studies will have to show whether these changes may be translated into a clinical benefit for our patients.
It Is Advisable to Control the Duration of Hypothermia Circulatory Arrest During Aortic Dissection Surgery: Single-Center Experience

Abstract:Objective: The duration of hypothermic circulatory arrest (HCA) is one of the important factors affecting the prognosis of arch surgery, which is still controversial. The purpose of this study was to investigate the effect of HCA duration on early prognosis in type A aortic dissection (TAAD) patients who underwent arch surgery in our center. Methods: All consecutive patients who underwent surgical treatment for TAAD in Fuwai Hospital from January 2013 to December 2018 were included in this study and divided into four quartile groups based on HCA time. Baseline characteristics, perioperative indicators, and early mortality were statistically analyzed by propensity score matching (PSM) and restricted cubic spline (RCS) method. Perioperative adverse events were confirmed according to the American STS database and Penn classification. Results: About 1,018 consecutive patients (mean age 49.11 ± 1.4 years, male 74.7%) with TAAD treated surgically were eventually included in this study. After PSM, with the prolongation of HCA time, the surgical mortality rates of group [2,15], (15,18], (18,22], and (22,73] were 4.1, 6.6, 7.8, and 10.9% with p = 0.041, respectively. As shown in RCS, the mortality rate increased sharply after the HCA time exceeded 22 min. And from the subgroup analysis, the HCA time of 22 min or less was associated with better clinical outcomes (OR 2.09, 95%CI 1.25–3.45, p = 0.004). Conclusions: The early mortality increases significantly with the duration of HCA time when arch surgery was performed. And multiple systems throughout the body can be adversely affected.

Abstract:Background Moderate hypothermic circulatory arrest (MHCA) has been widely used in aortic arch surgery. However, the renal function after MHCA remains controversial. We performed a systematic review and meta‐analysis direct comparison of the postoperative renal function of MHCA versus deep hypothermic circulatory arrest (DHCA) in aortic arch surgery. Methods and Results We searched PubMed, Embase, and the Cochrane Library for postoperative renal function after aortic arch surgery with using MHCA and DHCA, published from inception to January 31, 2020. The primary outcome was renal failure. Secondary outcomes were the need for renal therapy and other major postoperative outcomes. The random‐effects model was used for all comparisons to pool the estimates. A total of 14 observational studies with 4142 patients were included. Compared with DHCA, MHCA significantly reduced the incidence of renal failure (odds ratio [OR], 0.76; 95% CI, 0.61–0.94; P =0.011; I 2 =0.0%) and the need of renal replacement (OR, 0.68; 95% CI, 0.48–0.97; P =0.034; I 2 =0.0%). Subgroup analysis showed that when the hypothermic circulatory arrest time was <30 minutes, the incidence of renal failure in MHCA group was significantly lower than that in DHCA group (OR, 0.73; 95% CI, 0.54–0.99; P =0.040; I 2 =1.1%), whereas an insignificant difference between 2 groups when hypothermic circulatory arrest time was >30 minutes (OR, 0.76; 95% CI, 0.51–1.13; P =0.169; I 2 =17.3%). Conclusions MHCA compared with DHCA reduces the incidence of renal failure and the need for renal replacement. Abstract:Objectives: The optimal core temperature for hypothermic circulatory arrest during aortic arch surgery remains contentious. This study aims to evaluate patient outcomes under various temperatures within a large single-centre cohort. Methods: Between 2010 and 2018, patients diagnosed with type A aortic dissection underwent total arch replacement at Fuwai Hospital were enrolled. They were categorized into 4 groups: deep hypothermia group, low-moderate hypothermia group, high-moderate hypothermia group and mild hypothermia group. Clinical data were analysed to ascertain differences between the groups. Results: A total of 1310 patients were included in this cohort. Operative mortality stood at 6.9% (90/1310), with a higher incidence observed in the deep hypothermia group [29 (12.9%); 35 (6.9%); 21 (4.8%); 5 (3.4%); all adjusted P < 0.05]. Overall 10-year survival was 80.3%. Long-term outcomes did not significantly differ among the groups. Multivariable logistic analysis revealed a protective effect of higher core temperature on operative mortality (odds ratio 0.848, 95% confidence interval 0.766-0.939; P = 0.001). High-moderate hypothermia emerged as an independent protective factor for operative mortality (odds ratio 0.303, 95% confidence interval 0.126-0.727; P = 0.007). Multivariable Cox analysis did not detect an effect of hypothermic circulatory arrest on long-term survival (all P > 0.05). Conclusions: High-moderate hypothermia (24.1-28°C) offers the most effective protection against surgical mortality and is therefore recommended. Different hypothermic circulatory arrest temperatures do not influence long-term survival or quality of life.

Abstract:Objective:To analyze the prognosis of patients receiving aortic arch surgery requiring deep hypothermic circulatory arrest (DHCA).Method:The clinical data of patients after aortic arch surgery-DHCA in our hospital from January 2006 to December 2007 were studied retrospectively.Result:There were 58 patients after aortic arch surgery-DHCA during the study period. The overall mortality rate was 10.34%, the risk factors of prognosis after aortic arch surgery -DHCA were duration of cardiopulmonary bypass, duration of aortic cross-clamping, postperative serum creatinine, postoperative neurological dysfunction and continuous renal replacement therapy (CRRT).Conclusion:The main organs dysfunction after aortic arch surgery -DHCA was the predictor of poor prognosis, the risk factors merit further focused research to the perioperative organ protection.

Abstract:Shifts from deep to moderate hypothermic circulatory arrest (HCA) in aortic arch surgery necessitate an examination of their differential impacts on neurocognitive functions, especially structured verbal memory, given its significance for patient recovery and quality of life. This study evaluates and synthesizes evidence on the effects of deep (≤20.0 °C), low-moderate (20.1-24.0 °C), and high-moderate (24.1-28.0 °C) hypothermic temperatures on structured verbal memory preservation and overall cognitive health in patients undergoing aortic arch surgery. We evaluated the latest literature from major medical databases such as PubMed and Scopus, focusing on research from 2020 to 2024, to gather comprehensive insights into the current landscape of temperature management during HCA. This comparative analysis highlights the viability of moderate hypothermia (20.1-28.0 °C), supported by recent trials and observational studies, as a method to achieve comparable neuroprotection with fewer complications than traditional deep hypothermia. Notably, low-moderate and high-moderate temperatures have been shown to support substantial survival rates, with impacts on structured verbal memory preservation that necessitate careful selection based on individual surgical risks and patient profiles. The findings advocate for a nuanced approach to selecting hypothermic protocols in aortic arch surgeries, emphasizing the importance of tailoring temperature management to optimize neurocognitive outcomes and patient recovery. This study fills a critical gap in the literature by providing evidence-based recommendations for temperature ranges during HCA, calling for ongoing updates to clinical guidelines and further research to refine these recommendations. The implications of temperature on survival rates, complications, and success rates underpin the necessity for evolving cardiopulmonary bypass techniques and cerebral perfusion strategies to enhance patient outcomes in complex cardiovascular procedures.

Abstract:The definitive management of combined aortic arch and descending aortic pathologies such as aneurysms and dissections is either a single or staged operation associated with high morbidity and mortality. Stroke, kidney dysfunction, coagulopathy and high blood transfusion requirements are all affiliated with hypothermic circulatory arrest and prolonged cardiopulmonary bypass times. Considering the perilous nature of these operations, the authors describe a step-by-step zone 2 arch replacement as a staged frozen elephant trunk procedure, which provides an adequate landing zone for a later-placed endovascular stent yet maintains a short cardiopulmonary bypass time and no circulatory arrest.
Association between Duration of Deep Hypothermic Circulatory Arrest and Surgical Outcome in Patients with Acute Type A Aortic Dissection: A Large Retrospective Cohort Study

Abstract:(1) Background: Deep hypothermic circulatory arrest (DHCA) with selective antegrade cerebral perfusion (ACP) is an established cerebral protection technique for the conduction of complex surgical procedures involving the aortic arch. It is controversial whether the duration of DHCA is associated with adverse outcome in patients with acute type A aortic dissection (AAAD). Our goal was to investigate whether DHCA time was associated with surgical outcome in patients undergoing a surgical treatment of AAAD. (2) Methods: A total of 410 patients were divided into two groups based on the DHCA time less than 60 min and equal to or longer than 60 min. (3) Results: Patients with longer DHCA times were significantly younger (p = 0.001). Intraoperatively, complex procedures with aortic arch surgery were more common in patients with longer DHCA times (p < 0.001). Accordingly, cardiopulmonary bypass (p < 0.001), cross-clamping (p < 0.001) and DHCA times (p < 0.001) were significantly longer in this group. Postoperatively, only the duration of mechanical ventilation (p < 0.001) and the rate of tracheotomy were significantly higher in these patients. Thirty-day mortality was satisfactory for both groups (p = 0.746). (4) Conclusions: Our results showed that improvements in perioperative management including ACP allow for the successful performance of surgical treatment of AAAD under DHCA with a duration of even longer than 60 min.

Abstract:OBJECTIVE To discuss the better method of brain protection for deep hypothermic circulatory arrest. METHODS We selected 30 patients which had disease on the aorta. When their nasopharyngeal temperature were decreased to 12~13℃ and the rectal temperature were decreased to 17~18℃, we stopped the circulation. When the circulation was recovery, we increased the temperature step by step. And when the nasopharyngeal temperature achieved to 37℃, the the rectal temperature achieved to 35℃, we end the CPB. 10 cases were antegrade perfused to axillary artery, and 1 case was retrograde perfused to superior vena cava. RESULTS The time of circulatory arrest: 8-86min(average47.6±25.8min).The rate of heart automatic recovery is 63%. None of patients dead for the operation ,but 7 patients had obvious complication. CONCLUSION The methods of brain protection for deep hypothermic circulatory arrest are various, and they all have advantages and disadvantages. So we need to study and research the better method.

Abstract:BACKGROUND: Due to assumptions of excessive risk, hypothermic circulatory arrest (HCA) has been considered prohibitive in elderly patients. However, as more elderly patients are referred for assessment of difficult aortic valve, ascending aorta, and aortic arch pathology, the risk of HCA in these patients needs to be addressed. We hypothesized that the use of HCA would not increase mortality or complications in elderly patients compared to younger counterparts.METHODS: We retrospectively reviewed the charts of adult patients who underwent elective HCA between January 1995 and June 2007. Of 147 procedures, 45 patients were >or=75 years old. These patients were compared to their younger counterparts in terms of comorbidities, operations, and complications.RESULTS: Comparing patients >or=75 years old to their younger counterparts revealed no significant differences in outcomes including nearly identical rates of confusion (>or=75 15% vs <75 9%, p > 0.5) and stroke (>or=75 11% vs <75 7%, p > 0.2). There was also no difference in 30-day mortality (>or=75 7% vs <75 7%, p = 0.9). Lengths of hospital stays and intensive care unit stays were longer in the older patients, but this was not statistically significant.CONCLUSION: In this study, elderly patients faired well with HCA compared to younger patients. These data suggest that the use of HCA is safe in selected elderly patients. Elderly patients should be considered for indicated procedures of the aortic valve, ascending aorta, and aortic arch regardless of age.

Abstract:BACKGROUND:High mortality and morbidity rates are associated with the conventional surgery for aortic arch aneurysm because of cardiopulmonary bypass and deep hypothermic circulatory arrest. In this report, we describe a "hybrid" treatment for aortic arch aneurysm that combines the surgical debranching procedure and the stenting technique.METHODS:A surgical bypass graft is created from the ascending aorta to every main branch of the aortic arch. Subsequently, an endovascular stent graft is deployed retrogradely through the femoral artery with the aid of digital subtraction angiography.RESULT:The patient was discharged on postoperative day 7. A computed tomography scan was routinely performed at 1 week postoperatively to confirm the good patency of all grafts and the stent.CONCLUSION:The feasibility and the benefit of the hybrid treatment for aortic arch aneurysm are confirmed.
Deep hypothermia and circulatory arrest as an elective technique in the treatment of type B dissecting aneurysm of the aorta

Abstract:The combination of deep hypothermia and circulatory arrest has been used in a variety of cardiovascular surgical techniques and is presented in this article as an elective method in the treatment of type B dissecting aneurysms that may or may not involve the distal aortic arch. Out of 190 patients operated on with acute aortic dissection, 10 patients with type B underwent surgical procedures, between January 1985 and December 1987, four with acute dissection (less than 14 days evolution), and six with chronic dissection (more than 14 days evolution). The approach was by left posterolateral thoracotomy with cardiopulmonary bypass using femoro-femoral cannulation, deep hypothermia, and circulatory arrest. The duration of circulatory arrest was between 27 and 58 minutes, mean 37 minutes. Extracorporeal circulation (ECC) lasted between 68 and 142 minutes, with a mean perfusion time of 83.7 minutes, and temperature fell to 14 degrees C and 16 degrees C. Intrahospital mortality was 20% (two patients). No long-term mortality has been recorded. Eighty percent of the patients (eight patients) are alive and showed good evolution between 10 and 44 months following surgery, with a long-term mean survival of 24 months and 23 days. The only neurological complication was a single case of right unilateral blindness followed by complete visual recuperation. Myocardial and spinal cord protection are excellent, without any incidence of postoperative infarct or paraplegia. Postsurgical blood loss ranged from 200 to 650 mL with a mean of 385 mL. Acute renal insufficiency was not detected. We believe that the combined techniques of deep hypothermia and circulatory arrest used electively, and not just out of occasional necessity, is a viable choice that allows easier surgical manipulation of type B dissection aneurysms and complete resolution of those involving retrograde dissection to the aortic arch. In addition. this combined technique does not increase postoperative mortality in this critically dangerous disease.
Risk Factors of Delayed Awakening after Aortic Arch Surgery under Deep Hypothermic Circulatory Arrest with Selective Antegrade Cerebral Perfusion

Abstract:Background: To determine the risk factors of delayed awakening following aortic arch surgery under deep hypothermic circulatory arrest (DHCA) in combination with selective antegrade cerebral perfusion (SACP). Methods: We retrospectively analyzed the clinical data of all patients who underwent aortic arch surgery under DHCA + SACP between September 2015 and September 2017 in our hospital. Delayed awakening was defined as recovery of consciousness later than 24 hours after the surgery. Risk factors of delayed awakening were evaluated using multivariate logistic regression analysis. Results: A total of 168 subjects were included. In-hospital mortality of the overall sample was 19.05% (n=32). Delayed awakening occurred in 76 (45.23%) subjects. Subjects with delayed awakening had older age, hypertension, higher rate of emergency surgery and blood transfusion, and longer cardiopulmonary bypass (CPB) time and myocardial blocking time. Multivariate regression analysis showed emergency surgery (P=0.005) and CPB time >240 min (P<0.001) as risk factors for delayed awakening, even after adjusting potential confounders, including age, hypertension, aortic cross-clamp time and blood transfusion. Conclusions: In patients undergoing aortic arch surgery under DHCA + SACP, emergency surgery and CPB time >240 min are risk factors for delayed awakening.

Abstract:Abstract Background Aortic aneurysms involving the proximal aortic arch, which require hemiarch-type repair, typically require circulatory arrest with antegrade cerebral perfusion. Left carotid antegrade cerebral perfusion (LCP) via distal arch cannulation without circulatory arrest was used in this study’s patient population. The goal was to assess the operative efficiency and clinical outcomes of using a distal arch cannulation technique that would not require any hypothermic circulatory arrest (HCA) time compared with more traditional brachiocephalic artery cannulation with right-sided unilateral antegrade cerebral perfusion (RCP) and HCA. Methods A single-center retrospective review of patients with replacement of the distal ascending aorta involving the proximal arch was performed. Patients with an intramural hematoma or dissection were excluded. Between January 2015 and December 2019, 68 adult patients had undergone a hemiarch repair because of aneurysmal disease. Analysis of baseline demographics, operative data, and clinical outcomes was performed. Results Comparing the 68 patients: 21 patients were treated with RCP (via brachiocephalic artery graft with HCA), and 47 patients were treated with LCP (via distal aortic arch cannulation with cross-clamp between the brachiocephalic and left common carotid arteries without HCA). Baseline characteristics and outcomes were evaluated for both groups. The LCP group was younger (LCP median [IQR] age, 60 [53-65] years vs RCP median [IQR] age, 67 [59-71] years]. Sex, race, body mass index, comorbidities, and ejection fraction were similar between the groups. Cardiopulmonary bypass time (LCP, 123 minutes vs RCP, 149 minutes) and unilateral cerebral perfusion time (LCP, 17 minutes vs RCP, 22 minutes) were longer in the RCP group. Bleeding, prolonged ventilatory support, kidney failure, and length of stay were similar. In-hospital mortality was 2% in the LCP group vs 0% in the RCP group. Stroke occurred in 2 patients (4.2%) in the LCP group and in 0% of the RCP group. Mortality at 6 months in the LCP and RCP groups was 3% and 10%, respectively. Conclusion Distal arch cannulation with LCP without HCA is a reasonable and safe alternative strategy for patients requiring hemiarch replacement for aneurysmal disease. This technique may provide additional benefits by avoiding circulatory arrest in these complex cases.

Abstract:Objective: To summarize the anesthetic management experiences of patients with Stanford A aortic dissection( AD) undergoing surgical treatment under cardiopulmonary bypass( CPB). Methods: From January2012 to January 2013,a total of 55 patients with Stanford A AD underwent replacement of the ascending aorta and aortic arch,implantation of descending aorta stent under CPB with deep hypothermic circulatory arrest( DHCA) and selective cerebral perfusion. All the patients underwent general anesthesia and endotracheal intubation. In the operation,we closely monitored the hemodynamic parameters,electrocardiogram and oxygen saturation. We maintained stable breathing,circulation and internal environment. Results: CPB time was 162-493( 255. 31 ±42. 36) min and aortic cross-clamp time was 86-282( 170. 61 ± 43. 58) min. The time of DHCA was 13-55( 36. 23 ± 18. 14) min. All the patients were sent to intensive care unit. Eight patients died. Cause of death after the operation was respectively bleeding and heart failure,heart failure,infection and multiple organ failure,cerebral infarction and liver and kidney failure. Conclusion: Strengthening the vital organs and blood protection is the key to reduce the postoperative complications and mortality.

Abstract:One treatment for acute type A aortic dissection is to replace the ascending aorta and aortic arch with a graft during circulatory arrest of the lower body, but this is associated with high mortality and morbidity. Maintaining the balance between oxygen supply and demand during circulatory arrest is the key to reducing morbidity and is the primary challenge during body perfusion. The aim of this review is to summarize current knowledge of body perfusion techniques and to predict future development of this field. We present three perfusion techniques based on deep hypothermic circulatory arrest (DHCA): DHCA alone, DHCA with selective cerebral perfusion, and DHCA with total body perfusion. DHCA was first developed to provide a clear surgical field, but it may contribute to stroke in 4%–15% of patients. Antegrade or retrograde cerebral perfusion can provide blood flow for the brain during circulatory arrest, and it is associated with much lower stroke incidence of 3%–9%. Antegrade cerebral perfusion may be better than retrograde perfusion during longer arrest. In theory, blood flow can be provided to all vital organs through total body perfusion, which can be implemented via either arterial or venous systems, or by combining retrograde inferior vena caval perfusion with antegrade cerebral perfusion. However, whether total body perfusion is better than other techniques require further investigation in large, multicenter studies. Current techniques for perfusion during circulatory arrest remain imperfect, and a technique that effectively perfuses the upper and lower body effectively during circulatory arrest is missing. Total body perfusion should be systematically compared against selective cerebral perfusion for improving outcomes after circulatory arrest.

Abstract:Articular cartilage protects opposing bone extremities in diarthrodial joints in the form of a highly resilient, shock-absorbing cushion lined with a well-lubricated surface. Built during development, it undergoes limited turnover in adulthood and responds poorly to the various environmental and genetic adverse conditions that it can be subjected to in a lifetime. Consequently, damage to the tissue is generally irreversible and becomes gradually worse, leading to common joint degenerative diseases collectively referred to as osteoarthritis (1). These diseases can cause severe joint pain, deformation, and incapacitation. Intense research efforts have therefore been deployed over the last decades to decipher mechanisms that underlie articular cartilage development, adult maintenance and degeneration, and to discover ways to prevent osteoarthritis and regenerate healthy joints (2–4). Recent findings have further stimulated these efforts by eliciting hope that articular stem cells (ASCs) may be present or inducible in adult joints and could thus be used to design efficient cell-based therapies (5–8) Some studies have pinpointed ASCs in the superficial layers of articular cartilage by exploiting cell quiescence (very slow cycling), a defining property of stem cells, and others have proposed the existence of ASCs in articular cartilage, synovium and intrapatellar fad pad surfaces based on the ability of cells to express mesenchymal markers in vivo and to undergo chondrocyte differentiation in vitro. Lineage-tracing experiments in the mouse using a Gdf5Cre transgene along with a Rosa26floxlacZ CRE recombinase reporter have demonstrated that articular chondrocytes and other joint cell types arise during development from a common pool of mesenchymal progenitors and that these progenitors are distinct from those for growth plate chondrocytes and endochondral bone osteoblasts (9,10). It is thus believed that some of these embryonic joint progenitors may persist as ASCs in adult articular tissues. However, since this Gdf5Cre/Rosa26floxlacZ genetic strategy activates lacZ expression in all embryonic joint precursor cells and transmits this property to all daughter cells for life, it has not allowed addressing a number of important questions, such as the following: do joint surface-lubricating cells and articular chondrocytes arise as independent lineages from joint progenitors or do joint surface-lubricating cells give rise to articular chondrocytes? An elegant study published in this issue of Arthritis and Rheumatology by the Harvard University groups of Andrew Lassar and Matthew Warman describes a novel lineage-tracing strategy that will greatly help answer such questions and that already starts tackling some of them (11).

Abstract:OBJECTIVE To assess the effect of various antirheumatic drugs on cytokine, cytokine inhibitor, and prostaglandin E (PGE) production by normal blood mononuclear cells (MNC) and rheumatoid arthritis (RA) synovial fibroblasts in vitro. METHODS MNC from healthy donors and RA synovial fibroblasts were preincubated with or without prostaglandin E2 (PGE2), indomethacin, dexamethasone, gold sodium thiomalate (GSTM), methotrexate (MTX), and cyclosporin A (CyA), and then cultured in the absence or presence of interleukin-1 beta (IL-1 beta) or tumor necrosis factor-alpha (TNF-alpha) for 48 h. We characterized cytokines such as IL-1 beta, IL-8, monocyte chemoattractant protein-1 (MCP-1), and cytokine inhibitors such as IL-1 receptor antagonist (IL-1ra) and soluble TNF receptors (sTNFR p55 + p75) as well as PGE in the cell-free culture supernatants. RESULTS In MNC and synovial fibroblast cultures dexamethasone, GSTM, and PGE2 most markedly downregulated spontaneous and/or cytokine stimulated production of IL-1 beta, IL-14a, IL-8, and MCP-1, whereas sTNFR shedding was not affected. In contrast, MTX and CyA had only marginal or no effects on mediator release, whereas indomethacin inhibited only PGE production. CONCLUSION Among several antirheumatic drugs examined, dexamethasone and GSTM exhibited the most potent inhibitory effects on inflammatory cytokine and cytokine inhibitor production by blood mononuclear cells and synovial fibroblasts. These drugs may exert their antiinflammatory actions by unspecific suppression of monocyte and fibroblast secretory function.
POINCARE-CHETAYEV EQUATIONS AND FLEXIBLE MULTI-BODY SYSTEMS

Abstract:This article is devoted to the dynamics of flexible multi-body systems and to their links with a fundamental set of equations discovered by H. Poincare one hundred years ago [1]. These equations, called "Poincare-Chetayev equations", are today known to be the foundation of the Lagrangian reduction theory. Starting with the extension of these equations to a Cosserat medium, we show that the two basic sets of equations used in flexible multi-body dynamics. The generalized Newton-Euler model of flexible multi-body systems in the floating frame approach and the partial differential equations of the nonlinear geometrically exact theory in the Galilean approach, are Poincare-Chetayev equations.
