Abstract:Our strategy entails investigating the influence of varied concentrations (0, 10, 100 and 1000ng/ml) of human recombinant bone morphogenetic protein-2 (rhBMP-2) on the osteogenic expression of canine osteoblasts, seeded onto poly-caprolactone 20% tricalcium phosphate (PCL-TCP) scaffolds in vitro. Biochemical assay revealed that groups with rhBMP-2 displayed an initial burst in cell growth that was not dose-dependent. However, after 13 days, cell growth declined to a value similar to control. Significantly less cell growth was observed for construct with 1000ng/ml of rhBMP-2 from 20 days onwards. Confocal microscopy confirmed viability of osteoblasts and at day 20, groups seeded with rhBMP-2 displayed heightened cell death as compared to control. Phase contrast and scanning electron microscopy revealed that osteoblasts heavily colonized surfaces, rods and pores of the PCL-TCP scaffolds. This was consistent for all groups. Finally, Von Kossa and osteocalcin assays demonstrated that cells from all groups maintained their osteogenic phenotype throughout the experiment. Calcification was observed as early as four days after stimulation for groups seeded with rhBMP-2. In conclusion, rhBMP-2 seems to enhance the differentiated function of canine osteoblasts in a non-dose dependent manner. This resulted in accelerated mineralization, followed by death of osteoblasts as they underwent terminal differentiation. Notably, PCL-TCP scaffolds seeded only with canine osteoblasts could sustain excellent osteogenic expression in vitro. Hence, the synergy of PCL with bioactive TCP and rhBMP-2 in a novel composite scaffold, could offer an exciting approach for bone regeneration.

Abstract:Adipose-derived stromal cells (ASCs) have the potential to differentiate into a variety of cell lineages both in vitro and in vivo. A novel biodegradable biphasic calcium phosphate nanocomposite (NanoBCP) comprising beta-tricalcium phosphate matrix and hydroxyl apatite nanofibers is favorable for bone tissue engineering. In this study, ASCs were harvested from Sprague-Dawley (SD) rats and induced to osteogenesis before seeded into porous NanoBCP scaffold. To determine ectopic in vivo osteogenic differentiation, these constructs were implanted in nude mice subcutaneously. Meanwhile, the ability of engineered constructs to stimulate in situ bone repair was assessed in rat critical-size cranial defects. The defects were filled with NanoBCP containing osteogenic ASCs in experimental group; with cell-free NanoBCP in negative controls; and with nothing in blank controls. The retrieved specimens were analyzed with morphological, histological, and molecular methods. Histological analysis of the retrieved specimens from nude mice in experimental group showed obvious ectopic bone formation. There were positive expression of osteopontin (OPN) and osteocalcin (OCN) at RNA and protein level. As for the cranial defects, there was complete repair in experimental group, but only partial repair in negative controls. The radiographs, H&E staining, and Masson's trichrome method showed better bone regeneration at experimental sites. Combining osteogenic ASCs with NanoBCP can lead to formation of ectopic new bone. Histological Evaluation of Osteogenesis of 3D-Printed Poly-Lactic-Co-Glycolic Acid (plga) Scaffolds in A Rabbit Model

Abstract:Utilizing a suitable combination of lactide and glycolide in a copolymer would optimize the degradation rate of a scaffold upon implantation in situ. Moreover, 3D printing technology enables customizing the shape of the scaffold to biometric data from CT and MRI scans. A previous in vitro study has shown that novel 3D-printed poly-lactic-co-glycolic acid (PLGA) scaffolds had good biocompatibility and mechanical properties comparable with human cancellous bone, while they could support proliferation and osteogenic differentiation of osteoblasts. Based on the previous study, this study evaluated PLGA scaffolds for bone regeneration within a rabbit model. The scaffolds were implanted at two sites on the same animal, within the periosteum and within bi-cortical bone defects on the iliac crest. Subsequently, the efficacy of bone regeneration within the implanted scaffolds was evaluated at 4, 12 and 24 weeks post-surgery through histological analysis. In both the intra-periosteum and iliac bone defect models, the implanted scaffolds facilitated new bone tissue formation and maturation over the time course of 24 weeks, even though there was initially observed to be little tissue ingrowth within the scaffolds at 4 weeks post-surgery. Hence, the 3D-printed porous PLGA scaffolds investigated in this study displayed good biocompatibility and are osteoconductive in both the intra-periosteum and iliac bone defect models.
Three-dimensional-printed Titanium Alloy Porous Scaffold Combined with Trans-Cinnamaldehyde for Repairing Osteonecrosis of the Femoral Head in a Dog Model.

Abstract:Osteonecrosis of the femoral head (ONFH) is a common disorder that may be idiopathic, caused by trauma, or associated with alcohol or glucocorticoid use. The goals of early treatment include delaying or avoiding hip replacement, but there are no effective treatments for early-stage disease. The aim of the present study was to evaluate the effects of treatment with 3D-printed porous titanium alloy scaffold combined with daily intraperitoneal trans-cinnamaldehyde (TCA) in a dog model of ONFH. Four weeks after creation of the ONFH model, MRI examination of the femoral head showed the characteristic "double line sign" of ONFH, verifying the validity of our model. After another 12 weeks, femoral head specimens were harvested and examined by gross inspection; micro-computed tomography; histologic staining (hematoxylin and eosin; Masson); immunohistochemical analysis and quantitative real-time polymerase chain reaction analysis. Gross inspection of the femoral head in untreated ONFH animals at 16 weeks after model creation showed pale, exfoliating articular cartilage and disordered trabecular bone. Treatment with 3D-printed titanium alloy porous scaffold combined with TCA ameliorated the pathologic ONFH changes and significantly reduced inmature bone tissue as well as imature collagen in the femoral head, as shown by Masson staining. This treatment also increased VEGF, BMP2, β-catenin, b-FGF, and RUNX2 expression and decreased PPARγ expression, compared with untreated ONFH. In conclusion, 3D-printed titanium alloy porous scaffold combined with TCA can effectively improve ONFH, which may be related to local repair. This provides the theoretical basis for a new treatment strategy for ONFH.
Biomedical Engineering in Surgical Repair of Osteonecrosis: the Role of Imaging Technology

Abstract:We fabricated biomimetic biphasic calcium phosphate (BCP) for tissue engineering in repair of avascular necrosis (AVN) or osteonecrosis (ON) and observed its outcome of reconstructiion in ON femoral head of dogs. Firstly, BCP ceramic scaffolds were fabricated with three-dimensional (3D) gel-lamination technique according to the two-dimensional (2D) images of trabecular structure of dog femoral head. Then, the 3D structure of the scaffolds was scanned and reconstructed using micro-CT for characterization of 3D patterns of its material structure related to trabecular parameters, including bone volume fraction (BVF-BV/TV), bone surface/bone volume (BS/BV), trabecular thickness (Tb.Th), trabecular number (Tb.N), trabecular spacing (Tb.Sp), structure model index (SMI), and trabecular pattern factor (Tb.Pf). After coating marrow stromal cells (MSCs) as seed cells onto biomimetic porous BCP constructed in vitro, the tissue-engineered bones were implanted into the bony defects of dog femoral head through trap-door procedure for 30 weeks. The femoral heads in control group were compacted with autograft bone chips for comparison. Results showed similar micro-CT data between control samples and experimental group implanted with BCP scaffolds, except that a more plate-like "trabecular" was pattern found in BCP scaffolds. Thirty weeks after operation, the contour and integrity of the femoral heads were basically maintained and the BCP scaffold showed trabecular bone formation and integration. On the other hand, the femoral heads in control group collapsed and presented osteoarthritic changes. These findings suggested that the biomimetic porous BCP scaffolds developed for this study coated with MSCs might be an effective approach in repair of the bony defect in ON of the femoral head.
A novel animal model of osteonecrosis of the femoral head based on 3D printing technology

Abstract:Background: Osteonecrosis of the femoral head (ONFH) is a prevalent orthopedic condition characterized by the disruption of blood supply to the femoral head, leading to ischemia of internal tissues, subchondral bone fractures, necrosis, and eventual collapse of the weight-bearing portion of the femoral head. This condition results in severe functional impairment, pain, and even disability of the hip joint. Existing animal models of ONFH have limitations in replicating the natural disease progression accurately. Thus, there is a critical need to develop a novel animal model capable of better simulating localized pressure on the human femoral head to facilitate ONFH-related research. Methods: In this study, we present a novel approach for modeling ONFH, which involves integrating stress factors into the modeling process through the utilization of 3D printing technology and principles of biomechanics. A total of 36 animals were randomly assigned to six groups, where they received either the novel modeling technique or the traditional hormone induction method. Subsequently, an 8-week treatment period was implemented, followed by conducting micro-CT scans and histological evaluations to assess tissue outcomes. Results: The study evaluated the cytotoxicity of the material used in the new model, and it was observed that the material did not exhibit any cytotoxic effects on cells. Additionally, the novel model successfully replicated the pathological features of ONFH, including femoral head collapse, along with a substantial presence of empty bone lacunae, cartilage defects, and subchondral bone fractures in the subchondral bone region. Conclusion: In conclusion, our study provides evidence that the new model shows the ability to simulate the progression of the disease, making it a valuable tool for research in this field and can contribute to the development of better treatment strategies for this debilitating condition. It holds great promise for advancing our understanding of the pathogenesis of ONFH and the potential therapeutic interventions for this challenging clinical problem.
Micro-CT-based bone ceramic scaffolding and its performance after seeding with mesenchymal stem cells for repair of load-bearing bone defect in canine femoral head.

Abstract:Osteonecrosis of the femoral head is a debilitating and painful orthopedic condition characterized by joint collapse. Salvage of the femoral head is highly desirable to preserve the contour and mechanical properties and prevent joint collapse. This study aimed to develop a new tissue-engineering approach for treatment of large bone defect in femoral head, that is, after osteonecrosis. The biphasic calcium phosphate (BCP) ceramic scaffolds were fabricated by a 3D gel-lamination technique based on micro-computed tomography (micro-CT) images of the cancellous bone microarchitecture of femoral heads. After seeding with autologous bone marrow-derived mesenchymal stem cells (BMSCs) in vitro, the cell-scaffold composite was implanted into a bone defect surgically induced in canine femoral head via trapdoor procedure, which was a common procedure for treatment of osteonecrosis. A total of 24 adult dogs were randomly divided into three groups (n = 8 each) for implantation of the BCP scaffold with or without with BMSCs, and also the autologous bone chips for comparisons. All animals were sacrificed at 30 weeks postoperatively and processed for radiological and histological evaluations. The contour of the femoral head was well preserved with implantation of BCP scaffolds with or without BMSCs, whereas joint collapse was found after treatment with autologous bone chips. The osteointegration and new bone formation was significantly greater with BCP scaffold implantation with than without BMSC seeding and showed greater strength and compressive modulus in the repair Site. Micro-CT-based bone ceramic scaffolds seeding with BMSC might be a promising way to repair bone defects in the femoral head. (C) 2010 Wiley Periodicals, Inc. Reconstruct the Bone Defects of Femoral Head with Structural Oriented Biphasic Calcium Phosphate Scaffolds Based Tissue Engineered Bone

Abstract:Objective To observe the effect of based biomimetic biphasic calcium phosphate(BCP)tissue engineered bone masses to repair the bone defects and prevent the collapse of femoral head in dogs.Methods Biomimetic BCP ceramic scaffolds was fabricated using three-dimensional(3D)gel-lamination technology according the micro-computed tomography(Micro-CT)information of cancellous bone of dogs' femoral heads.Tissue engineered bone masses were constructed by the combination of biomimetic BCP scaffolds and induced osteogenic bone marrow stromal cells(BMSCs)in vitro.In 10 dogs,tissue engineered bone was implanted into the bone defect on the weight-bearing area of femoral head.In the control group of another 10 dogs,autogenous cancellous bone chips were impressed into the bone defect on the weight-bearing area of femoral head.The gross appearance and regeneration of new bone were observed in 30 weeks postoperatively.Results The fabricated biomimetic BCP scaffolds had better 3D structure.The trabecular system of the scaffolds had some orientation along the weight-bearing line of femoral head and represented plate-like model.BMSCs grow flourishing on the surface of the scaffolds.After 30 weeks operatively,in the tissue engineered bone implanted dogs,the gross appearances of the femoral head were integrity and a great lot of new bone tissue wraps the trabecula of the scaffolds under microscope.In the control group,the mostly femoral heads were collapse gross appearance.Conclusion Tissue engineered biomimetic BCP had better biocompatibility and could prevent the collapse of femoral head after it is implanted into the bone defects of femoral head.
A new 3D printed titanium metal trabecular bone reconstruction system for early osteonecrosis of the femoral head

Abstract:Presently, biomechanical support therapy for the femoral head has become an important approach in the treatment of early osteonecrosis of the femoral head (ONFH). Previous studies have reported that the titanium metal trabecular bone reconstruction systems (TMTBRS) achieved satisfactory clinical results for the treatment of early femoral head necrosis. Electron beam melting technology (EBMT) is an important branch of 3D printing technology, which enables the construction of an interface that is required for support of bone in-growth. However, the effect of TMTBRS created using EBMT for clinical applications for early ONFH is still unknown. At present, there are no reports on this topic worldwide. The purpose of this study was to assess the safety of a new 3D printed TMTBRS implant and to evaluate its clinical efficacy in early ONFH.Thirty patients who underwent surgery for ONFH were selected. The stages of ONFH were classified according to the Association Research Circulation Osseus (ARCO) classification. They were followed-up and radiological examination was performed at 6, 12, and 24 months post-surgery to assess TMTBRS stability and bone growth in the bone trabecular holder portion surface. To evaluate hip function, postoperative Harris and Visual Analogue Scale (VAS) scores were used.The postoperative Harris score increased significantly and VAS score decreased significantly at the 12-month follow-up compared to the 24-month follow-up, wherein the Harris score declined slightly and the VAS score was slightly elevated with the aggravation of ONFH. With the passage of time, postoperative improvement rates were 100% for IIA, 70% for IIB, and 0% for IIC. Hip-preserving rates were 100% for IIA, 100% for IIB, and 50% for IIC.The effect of TMTBRS treatment for early ONFH in ARCO IIA and ARCO IIB is satisfactory. However, it is not recommended for a relatively large area of necrosis such as in ARCO IIC.

Abstract:The purpose of this study was to evaluate the ability of a quantitative prediction method using a taste sensor to determine the bitterness of clarithromycin powder suspensions of various concentrations and of a commercial clarithromycin dry syrup product (Clarith dry syrup, Taisho Pharmaceutical Co., Ltd., Tokyo) containing aminoalkyl methacrylate polymer as a taste-masker. The bitterness of the clarithromycin dry syrup product dissolved in various beverages was also evaluated in gustatory sensation tests and using the taste sensor. In the sensor measurements, three variables were used to predict bitterness in single and multiple regression analysis: relative sensor output (R), the change of membrane potential caused by adsorption (CPA), and CPA/R ratio. The CPA values for channel 3 of the sensor predicted well the bitterness of clarithromycin powder suspensions and their filtered solutions. For Clarith dry syrup, the sensor output was small, suggesting that aminoalkyl methacrylate polymer was successful in almost complete masking of the bitter taste of the dry syrup product. When the bitterness intensities of mixtures of 1 g of Clarith dry syrup with 25 ml of water, coffee, tea, green tea, cocoa, milk, and a sports drink were examined, a good correlation was obtained between the results from human taste tests and the predicted values calculated on the basis of multiple regression analysis using CPA data from channel 4, and the CPA/R ratio from channel 3 of the taste sensor (r(2)=0.963, p<0.005). Co-administration of 1 g of Clarith dry syrup with an acidic sports drink was found to be the most bitter using either method.
Regenerative repair of bone defects with osteoinductive hydroxyapatite fabricated to match the defect and implanted with combined use of computer-aided design, computer-aided manufacturing, and computer-assisted surgery systems: a feasibility study in a canine model

Abstract:BACKGROUND: Currently, regenerative repair of large bone defects that result from bone tumor resection or severe trauma is a challenging issue because of the limited regenerative potential of bone and treatment modalities. The aim of this study was to achieve repair of large bone defects to the original three-dimensional (3D) anatomical state by combining computer-aided technologies and local delivery of bone morphogenetic protein (BMP) in a canine model.METHODS: Computed tomography (CT) images of the pelvic bone of each dog were obtained, and an imaginary spherical malignant bone tumor of 15-mm diameter was placed in the left ilium of a canine on the 3D CT image. Resection of the whole tumor with a 10-mm margin of healthy bone was planned preoperatively by using computer-aided design (CAD) software. In addition, an image of the implant to be used to fill the resulting bone defect was constructed on the computer image. A porous hydroxyapatite (HA) implant identical to the imaged bone defect was made by shaving a tetragonal porous apatite block (40 × 20 × 10 mm) with a computer-aided manufacturing system operated by using the CT-image data of the bone defect obtained from the CAD system. To resect the iliac bone as planned preoperatively on the 3D CT image, computer-aided surgery was performed using the CT data. The defect was filled with the HA implant fabricated as described and coated with a putty carrier either with BMP-2 (BMP group, n = 6) or without BMP-2 (control group, n = 6).RESULTS: In the BMP group, new bone formation was noted around each implant on CT images at 3 weeks after surgery and was remodeled to restore the original anatomy of the ilium on serial CT images. At 12 weeks, the implant was enclosed within new bone, and histological analysis revealed bone formation on and within the implant. Little bone formation was noted in the control group.CONCLUSIONS: This new method may enable efficacious and precise regenerative repair of large bone defects without bone grafting.

Treatment of Osteonecrosis of the Femoral Head with VEGF165 Transgenic Bone Marrow Mesenchymal Stem Cells in Mongrel Dogs

Abstract:We evaluated the efficacy of vascular endothelial growth factor 165 (VEGF165) transgenic bone marrow mesenchymal stem cells (BMSCs) for the repair of early-stage osteonecrosis of the femoral head (ONFH) in mature mongrel dogs. This animal model was surgically established by femoral neck osteotomy and subsequent repinning. Twenty-seven dogs (54 hips) were divided into 3 equal-sized groups: a pCI-neo-VEGF165 BMSC group, a pCI-neo BMSC group and a core decompression-alone group. The lipofectamine was used to introduce the VEGF165 gene into the BMSCs. After core decompression, transgenic and non-transgenic autologous BMSCs were implanted. Therapeutic efficacy, including new bone formation and neovascularization in the femoral head, was examined by computed radiography, single-photon emission computed tomography, histological and histomorphometric analysis and immunofluorescent staining for von Willebrand factor in pathological sections. The femoral osteotomy site healed completely by the 4th week after the osteotomy surgery and regions of histologically evident osteonecrosis were found 12 weeks later. A regular arrangement of trabeculae and obvious bone regeneration were observed in the animals receiving implanted VEGF-transgenic BMSCs. The quantity of newly generated capillaries was significantly increased in the pCI-neo-VEGF165 BMSC group, but there was no significant difference between the pCI-neo BMSC group and the core decompression-alone group. These results demonstrated that VEGF165 transgenic autologous BMSCs enhanced bone reconstruction and blood vessel regeneration in the ONFH model. Compared with non-transgenic BMSCs, this approach could provide advanced benefits in the treatment of ONFH.
[Minimally invasive treatment for osteonecrosis of the femoral head in ARCO stage Ⅱ and Ⅲ with bioceramic system]

Abstract:Objective: To perfect the theory system of minimally invasive treatment for osteonecrosis of the femoral head (ONFH) with β tricalcium phosphate (β-TCP) bioceramic system and evaluate the effectiveness. Methods: Eighteen New Zealand white rabbits aged 7-8 months were used to establish an animal model to verify the vascularization of porous β-TCP bioceramic rods. Micro-CT based three-dimensional reconstruction and fluorescence imaging were used to display the new blood vessels at 4, 8, and 12 weeks after operation. The inserting depth, number and diameter of vessels in the encapsulated area were analyzed. Nine pig femoral specimens were randomly divided into 3 groups ( n=3): group A was normal femur; group B had cavity (core decompression channel+spherical bone defect in femoral head); in group C, mixed bioceramic granules were implanted to fill the defect in femoral head, and porous β-TCP bioceramic rod was implanted into decompression channel. The stiffness and yield load of specimens were analyzed by biomechanical test. A multicenter retrospective study was conducted to analyze 200 patients (232 hips) with femoral head necrosis treated with bioceramic system in 7 hospitals in China between January 2012 and July 2018. There were 145 males and 55 females, with an average age of 42 years (range, 17-76 years). According to the Association Research Circulation Osseous (ARCO) stage, 150 hips were in stage Ⅱ and 82 hips in stage Ⅲ. Postoperative imaging assessment was carried out regularly, and hip function was evaluated by Harris score. The effectiveness of ARCO stage Ⅱ and Ⅲ was also compared. Results: Animal experiments showed that blood vessels could grow into the encapsulated area and penetrate it at 12 weeks. The inserting depth, number and diameter of blood vessels in the encapsulated area gradually increased, and there was significant difference between different time points ( P<0.05). Biomechanical tests showed that the stiffness and yield load of specimens in groups B and C were significantly lower than those in group A, while the yield load in group B were significantly lower than that in group C ( P<0.05). The stiffness in group C was restored to 41.52%±3.96% in group A, and the yield load was restored to 46.14%±7.85%. Clinical study showed that 200 patients were followed up 6-73 months, with an average of 22.7 months. At last follow-up, 12 patients (16 hips) underwent total hip arthroplasty, and the hip survival rate was 93.10%. According to the imaging evaluation, 184 hips (79.31%) were stable and 48 (20.69%) were worse. Harris score (79.3±17.3) was significantly higher than that before operation (57.3±12.0) ( t=18.600, P=0.000). The excellent rate of hip function was 64.22% (149/232). The survival rate of hip joint, imaging score and Harris score of patients in ARCO stage Ⅱ were better than those in ARCO stage Ⅲ ( P<0.05). Conclusion: β-TCP bioceramic system can guide the abundant blood supply of greater trochanter and femoral neck to the femoral head to promote repair; it can partly restore the mechanical properties of the femoral head and neck in the early stage, providing a new minimally invasive hip-preserving method for patients with ONFH, especially for those in early stage.
Clinical value of 3D printing guide plate in core decompression plus porous bioceramics rod placement for the treatment of early osteonecrosis of the femoral head

Abstract:Background: The conventional method of core decompression combined with porous bioceramics rod is usually performed under C-arm fluoroscopy for the treatment of early osteonecrosis of the femoral head (ONFH). This study was to evaluate the clinical value and efficacy of three-dimensional (3D) printing guide plate in the process of core decompression plus porous bioceramics rod for the treatment of early ONFH. Methods: Forty patients were enrolled, including 20 patients undergoing the surgery with 3D printing guide plate in the experiment group and 20 controls with C-arm fluoroscopy. The following parameters such as surgery time, blood loss, fluoroscopy times, and the accuracy of core decompression for necrosis area, function outcome according to Harris Hip Score (HHS), and any possible complications were recorded and compared between the two groups. All the patients were followed up at 6, 12, and 18 months postoperatively. Results: The surgery time, fluoroscopy time, and intraoperative blood loss in the experiment group was significantly less (P < 0.05) than those in the control group. There was no statistical significance in the accuracy of core decompression and porous bioceramics rod placement between the two groups (P > 0.05). All patients were followed up for 18 months. There was a significant difference between the preoperative and final follow-up HSS scores in both groups (P < 0.05). In addition, there was also a significant difference between the groups in the last follow-up HSS scores (P < 0.05). Conclusions: Compared with the traditional method, 3D printing guide plate could shorten the surgery time and fluoroscopy times and decrease intraoperative blood loss. It seems to be an effective method in the combined core decompression with porous bioceramics rod placement for early ONFH.

Abstract:The removal of necrotic bone and implantation of bone repair materials is an effective treatment for osteonecrosis of the femoral head (ONFH)-type bone defects, but there are currently no clinically applicable bone repair materials. In this study, a biotin-doped bone repair scaffold was created using low-temperature deposition (LTD) three-dimensional (3D) printing technology, and its ability to repair bone defects in ONFH was evaluated. The scaffold was characterized in vitro, and its cytotoxicity and osteogenic capacity were assessed by co-culturing the scaffold with rat bone marrow mesenchymal stem cells. The scaffolds were implanted in an animal model of ONFH-type bone defects, and the effect of scaffolds on promoting bone repair was evaluated by means of radiology and histopathology. LTD 3D-printed biotin-doped scaffolds showed cancellous bone-like structures without inducing cytotoxicity, whereas high-biotin β-TCP scaffolds (HBPT; containing 2% biotin) promoted osteogenic differentiation more effectively. Experiments on animals revealed that the effect of HBPT on bone repair was significantly superior to that of other groups. The in vivo biocompatibility of HBPT was confirmed by blood analysis and hematoxylin and eosin staining of the main organs. In conclusion, biotin-doped scaffolds can be used to treat ONFH-type bone defects by virtue of their ability in promoting bone regeneration.

[CLINICAL APPLICATION OF THREE DIMENSIONAL PRINTED NAVIGATION TEMPLATES FOR TREATMENT OF OSTEONECROSIS OF FEMORAL HEAD WITH PEDICLED ILIAC BONE GRAFT]

Abstract:Objective: To investigate the feasibility and early effectiveness to treat osteonecrosis of the femoral head (ONFH) with pedicled iliac bone graft assisted by individual digital design and three dimensional (3D) printed navigation templates. Methods: Between February and June 2014, 15 patients (24 hips) with ONFH underwent pedicled iliac bone graft assisted by individual digital design and 3D printed navigation templates. There were 11 males (17 hips) and 4 females (7 hips) with a mean age of 38 years (range, 18-56 years) and a mean disease duration of 7.5 months (range, 1-24 months); the left hip was involved in 2 cases, the right hip in 4 cases, and both hips in 9 cases. There were 7 cases (12 hips) of steroid-induced ONFH, 5 cases (8 hips) of alcohol-induced ONFH, 1 case (1 hip) of traumatic ONFH, and 2 cases (3 hips) of idiopathic ONFH. The preoperative Harris score was 56.60 ± 6.97. According to Association Research Circulation Osseous (ARCO) staging system, 5 hips were classified as stage IIB, 8 hips as stage IIC, 6 hips as stage IIIB, and 5 hips as stage IIIC. The navigation templates were designed and printed to assist accurate location and debridement of necrosis area according to preoperative CT scanning at the beginning of pedicled iliac bone grafting procedure. Results: The mean operation time was 135 minutes (range, 120-160 minutes), mean amount of bleeding was 255 mL (range, 200-300 mL). All the wounds healed primarily, no complication of deep vein thrombosis or infection was observed. All patients were followed up 12-16 months (mean, 14 months). The location of necrosis area was in accordance with preoperative design, which was removed completely without penetration of joint surface, pedicled iliac bone graft was performed at the right site according to postoperative imaging examination. Radiographically, graft fusion was achieved at 2.7 months (range, 2-3 months) in all patients. All the hips had no collapse during follow-up. Hip pain was relieved, and range of motion was improved. The Harris score was significantly improved to 89.53 ± 5.83 at last follow-up (t = 14.3 19, P = 0.000). The results were excellent in 12 hips, good in 10 hips, and fair in 2 hips according to Harris score standard. Conclusion: Pedicled iliac bone graft assisted by individual digital design and 3D printed navigation templates for treatment of adult ONFH has the advantages of accurate location and complete debridement of necrosis area, so satisfactory results can be obtained.

Abstract:Background: The repair of bone defect in osteonecrosis of femoral head is difficult, and the ultrastructure of the osteogenic process porous bioceramic β-tricalcium phosphate (β-TCP) combined with bone marrow mesenchymal stem cells (BMSCs) in vivo still remains unclear. Objective: To observe the ultrastructure of the repair of porous bioceramic β-TCP combined with BMSCs in osteonecrosis of femoral head model, to test the osteogenesis during the repairing process, and to verify the feasibility that porous bioceramic β-TCP is an excellent scaffold of tissue engineering. Design, time and setting: A randomized controlled research on animals was performed in Bone Circulation and Osteonecrosis Laboratory of China-Japan Friendship Hospital from February to August in 2008. Materials: Rabbit BMSCs were cultured in vitro, and then combined with the porous bioceramic β-TCP for 2 weeks. Methods: Thirty New Zealand white rabbits were divided into three groups. Subsequent to the establishment of osteonecrosis of femoral head model, blank control group received no padding, β-TCP group was transplanted with β-TCP, and β-TCP + BMSCs group was transplanted with the compound of the two. Main outcome measures: The compound of β-TCP with BMSCs was observed under environmental scanning electron microscope; The specimens were collected postoperative 6 and 12 weeks, and the ultrastructure in the repair of β-TCP combined with BMSCs was observed by electron microscope and the substitution of osteogenesis and material degradation was also investigated. Results: The BMSCs could adhere, grow, proliferate and differentiate in porous bioceramic β-TCP. Both 6 and 12 weeks postoperatively, fibrous tissue was still found in the defect area of blank control group whereas no bone trabecula was observed. In the β-TCP group, the material began to degrade and not clear with matrix 6 weeks postoperatively, the material was identical with periphery area 12 weeks postoperatively and porous frame structure began to disappear. In the β-TCP + BMSCs group, new bone formed at 6 postoperative weeks, the parallel trabecularism formed and the degradation was obvious. A lot of osteoblasts were found in the periphery area. Compared with blank control group, the repairing and osteogenesis effect was better in other two groups, especially in the β-TCP + BMSCs group. Conclusion: Porous bioceramic β-TCP is a fine scaffold of tissue engineering, easy to combine with BMSCs, and suitable for repairing of bone defect in osteonecrosis of femoral head.

Abstract:Objective To evaluate the efficacy of 3D‐printed scaffolds that were osteoinductively functionalized with a bone morphogenetic protein 2 (BMP‐2)‐incorporated biomimetic calcium phosphate particles (BMP‐2‐inc. BpNcCaP)/hyaluronic acid (HA) composite gel in vertical bone augmentation in beagle dogs. Materials and Methods Four Beagle dogs were used in this study. Three months after the extraction of 1st, 2nd, 3rd, and 4th premolars at both sides of the lower jaws of Beagle dogs, one or two critical‐size vertical bone defects (4 mm vertical bone defect without buccal and lingual bone) on each side were surgically created. The defects were randomly subjected to the following groups: (1) Control (without bone‐defect‐filling materials); (2) 3D scaffold; (3) BMP2‐inc. BpNcCaP/HA‐functionalized 3D scaffold. Six weeks post‐surgery, samples were harvested and subjected to micro‐CT and histomorphometric analyses. Results The struts of the BMP2‐inc. BpNcCaP/HA‐func. 3D scaffold were covered by a thick layer of cemented irregular particles with an average pore size at 327 ± 27 μm. The BpNcCaP/HA‐func. 3D scaffold group bore significantly higher bone volume, bone volume fraction, trabecular number, trabecular thickness, bone mineral density, connectivity density, and bone volumes in three directions (mesiodistal, buccolingual, and apicocoronal) when compared with the groups of Control and 3D scaffold. Moreover, the BMP2‐inc. BpNcCaP/HA‐func. 3D scaffold group bore significantly lower trabecular separation and exhibited significantly higher bone‐to‐scaffold contact percentage and newly formed bone area percentage within pores in comparison with 3D scaffold. Conclusions BMP2‐inc. BpNcCaP/HA‐func. 3D scaffold dramatically enhanced vertical alveolar bone augmentation, which suggests a promising application potential of BMP2‐inc. BpNcCaP/HA‐func. 3D scaffold in dental clinic.

Abstract:Objective To evaluate the effect of antigen-extracted cancellous bone in treatment of bone defect of dog femoral head.Methods Five hybrid adult dogs were selected to construct the bone defect model behind the femoral head lateral and near the head and neck junction.The trimmed AECB were filled into one side of the defected region and there was no material filled on the other side.After 12 weeks,the material was select to observe effects after dyeing.Results On AECB side,the mature bone tissue can be seen at the junction area,the woven bone has formed surrounding,and the form of medullary cavity can be seen.On the control side,the defected region and surrounding tissue was seen with clear boundary,bone formation was seen obviously around the defected region,the fiber granulation tissue was seen in the defected region.Much cartilage formation was seen around or interior of the material region with more bone tissue calcification.But only a small amount of cartilage formation was seen on the control side of the defected region with no cartilage or bone formation.Much collagenⅠ and less collagen Ⅲ were seen around or in the material,but there was no collagenⅠor Ⅲ in the control side of the defected region.Conclusion AECB has good histocompatibility,and may be an ideal material for treatment of osteonecrosis of the femoral head.
A 3D printed porous titanium alloy rod with biogenic lamellar configuration for treatment of the early-stage femoral head osteonecrosis in sheep

Abstract:There is no ideal implant for mechanical strut on early-stage osteonecrosis of the femoral head (ONFH) after core decompression. In this study, a biogenic trabecular porous titanium rod with lamellar configuration was designed and fabricated using selective laser melting technique. Early-stage ONFH of sheep induced by cryo-insult were dealt with core decompression combined with rod insertion (Rod group) and core decompression alone (CD group) after X-ray evaluation was used to assess the necrotic region one months after cryo-intervention. Bone integration and ingrowth of the two groups were investigated and compared. Early-stage ONFH intervened with the rod gained better bone ingrowth than CD 3 and 6 months after the intervention, as evidenced by radiographic, micro-CT and histological evaluation. X-ray images showed compact integration between rods and peripheral bone, evidenced by no radiolucent lines encircling the rods at 3 and 6 months. Micro-CT and histological images showed that the new bone had grown into the centre of rods along the metal at 3 months, whereas the new bone grew mainly at the periphery of the decompressive channel. Micro-CT analysis show that the ratios of bone volume to total volume (BV/TV) of volume of interest (VOI) in Rod group was 890.0% and 438.1% higher than CD group at 3 (0.198 ± 0.0094 VS 0.020 ± 0.0058, p < 0.05, n = 3) and 6 (0.226 ± 0.0166 VS 0.042 ± 0.0061, p < 0.05, n = 3) months respectively. Histological analysis showed that the BV/TV of VOI in Rod group was 881.0% and 413.3% higher than CD group at 3 (0.206 ± 0.0102 VS 0.021 ± 0.0061, p < 0.05, n = 3) and 6 (0.231 ± 0.0156 VS 0.045 ± 0.0059, p < 0.05, n = 3) months respectively. The mechanical tests revealed that the maximum load of Rod group was 57.6% larger than CD group at 6 months (4505.25 ± 443.86 N VS 2858.25 ± 512.91 N, p < 0.05, n = 3). These favourable short-term results can provide insight on treatment of early-stage ONFH.
