Abstract:Introduction: Epiploic appendagitis (EA) is an essential cause of abdominal pain that can be confused with more typical causes such as acute diverticulitis and appendicitis. Epiploic appendagitis accounts for 1% of all cases of abdominal pain in adults. The scarcity of information has limited its recognition as an essential nonsurgical cause of acute abdominal pain.Methods: We performed a systematic review of all EA cases published. We searched Scopus, Medline, Web of Science, and Google Scholar to retrieve all available studies from January 2000 to November 2023.Results: 196 case reports and case series were analyzed, with 371 patients with EA included. The mean age at the time of diagnosis was 39 years. Most patients were male (59%). The primary presenting symptoms were pain (100%), tenderness (59.5%), and rebound tenderness (27.4%). The left abdomen was the most common localization of pain (53%). The most frequently identified differential diagnoses were acute appendicitis (26.4%) and acute diverticulitis (16.1%). Most patients (53%) were treated conservatively, and 98 (26.4%) underwent surgical treatment. A significant difference in the choice of treatment was found for signs and symptoms such as rebound tenderness, nausea, anorexia, and diarrhea.Conclusions: Acute EA is an essential clinical condition of rare occurrence that might present a diagnostic challenge, as it can masquerade as another acute abdominal pain etiology. The optimal management of EA is conservative, so a higher recognition by surgeons and emergency physicians is essential to avoid unnecessary surgical interventions and their associated consequences.

Abstract:Epiploic appendagitis is a rare cause of acute abdomen that often manifests with acute onset of pain in the left or right lower quadrant. Its symptoms can mimic and be mistaken for acute diverticulitis, appendicitis, or omental infarction. In this case report, we discuss a 65-year-old woman who presented with sharp right upper and lower quadrant abdominal pain, for which she had an emergent abdominal computed tomography (CT) scan. On CT images, epiploic appendagitis will appear as oval lesions with a central area of fat attenuation, accompanied by surrounding inflammation. Ultrasound and magnetic resonance imaging are more often used to evaluate acute abdominal pain in the pediatric and obstetric populations, so the respective findings of acute epiploic appendagitis must be recognized in those examinations as well. Despite the rarity of the condition and its common omission from differential diagnoses, the ability to recognize and diagnose epiploic appendagitis from its imaging is important for radiologists, especially considering its potential complications. If not diagnosed correctly, epiploic appendagitis can result in unnecessary hospital admission and patient workup, antibiotic use, dietary restrictions, and perhaps even unnecessary surgery. In this case, the diagnosis of epiploic appendagitis using CT allowed the patient to avoid surgery and other invasive treatment, and the patient was eventually discharged on conservative medical management.

Abstract:Epiploic appendagitis is an unusual and very commonly overlooked source of acute abdominal pain. Its incidence is highest in middle-aged obese males. It presents clinically as a focal lower quadrant abdominal pain, usually in the absence of pyrexia, nausea, vomiting or change in bowel habit, and unremarkable laboratory markers. Due to its vague presentation, epiploic appendagitis may be mistaken for other more severe causes of acute abdominal pain like diverticulitis and appendicitis, thereby causing patients to undergo unwarranted management interventions and hospital stay. Epiploic appendagitis is usually diagnosed through imaging, most commonly computed tomography (CT). This condition is largely self-resolving and can be managed conservatively with nonsteroidal anti-inflammatory drugs (NSAIDs). Operative intervention is usually employed when symptoms persist or when complications arise. We present a case of epiploic appendagitis in a patient who presented with right lower quadrant pain initially misdiagnosed as acute appendicitis.

Abstract:Epiploic appendagitis (EA) is a rare clinical entity caused by an inflammatory/ischemic process involving the serosal outpouchings of the colon. Its clinical presentation of acute, localised, lower abdominal pain often mimics more common conditions like diverticulitis or appendicitis. The diagnosis of EA is challenging due to the lack of pathognomic clinical features. The definitive diagnosis primarily relies on cross-sectional imaging modalities like abdominal ultrasound or computed tomography (CT). Being a benign and self-limiting condition, it can be managed conservatively with analgesic and anti-inflammatory drugs. We present two cases to highlight EA as an important differential diagnosis for cases of acute lower abdominal pain, crucial to prevent unnecessary antibiotic therapy and surgical interventions.

Abstract:Epiploic appendagitis is a rare entity of abdominal pain, often being confused with common acute abdominal pathologies, such as appendicitis and diverticulitis. Its pathophysiology is based on a torsion process of the vascular pedicle of the epiploic appendix. The correct diagnosis is essential, in order to avoid unnecessary therapeutic interventions, with computerized axial tomography being the diagnostic method of choice. In most cases, therapy is symptomatic, with spontaneous remission occurring within a few days using medication to control symptoms and sometimes antibiotics. The authors present the report of a clinical case of a 37-year-old female patient who came to the consultation for abdominal pain in the lower quadrants, with epiploic appendagitis being published. Conservative management has been demonstrated. Anatomy, pathophysiology, clinical presentation, radiological evaluation, and emergency management of epiploic appendagitis are reviewed in this article.

Abstract:Epiploic appendagitis is a benign and self-limiting condition that has garnered more recognition, particularly in preoperative settings, thanks to advancements in imaging techniques, notably computed tomography (CT). Its distinct radiologic features facilitate accurate diagnosis prior to surgery. Despite its unique characteristics, the clinical presentation of epiploic appendagitis often resembles that of various other intra-abdominal medical and surgical conditions, leading to potential confusion. Here, we present the case of a 37-year-old otherwise healthy woman who was diagnosed with primary epiploic appendagitis based on CT scan findings conducted initially for suspected appendicitis. The patient received successful conservative treatment. This paper seeks to highlight the importance of recognizing this clinical condition and its characteristic imaging features, aiming to prevent unnecessary hospitalizations, antibiotic treatments, and the associated morbidity and mortality linked with surgical interventions.

Abstract:Epiploic appendagitis (EA) is a rare condition caused by the infarction of the epiploic appendages, small outpouches of adipose tissue found on the outer surface of the bowel wall. EA results in inflammation and is often mistaken for other gastrointestinal disorders, such as diverticulitis or appendicitis. Diagnosis is primarily made through computed tomography scans, with ultrasound and magnetic resonance imaging used less often. Treatment initially involves analgesia with or without anti-inflammatory medication. However, surgery in the form of laparoscopic appendage removal may be required if symptoms persist or worsen. In total, 2 cases of EA are presented, one mimicking appendicitis and the other sigmoid diverticulitis. The purpose of the presentation is to increase awareness of EA as a cause of abdominal pain and to avoid unnecessary surgery.

Abstract:Background: Appendices Epiploicae, also referred as Epiploic appendages, are 50-100 fat filled finger like projection from the serosal surface of large intestine. Epiploic Appendagitis is a self-limiting, benign disease process which results from the inflammation of these Appendices Epiplocae or thrombosis of the draining vein of Appendices Epiplocae. Case Report: We report a case of 49 year old female, generally well, presented to the A & E with complaints of a painful lump in her left iliac fossa growing in size for last four weeks. She presented to us because of acute increase in size and pain, resulting in significant discomfort. Owing to her history of CIN-1 we arranged a CT abdomen and pelvis with contrast fearing some sinister underlying ongoing pathology causing her symptoms. But to our surprise Ct reported a underrated cause of her abdominal pain, Epiploic Appendagitis. We were surprised of her presentation as a hard abdominal lump which was quite unusual for EA to present as. We assume it was secondary to an extensive underlying local inflammatory reaction. Patient was reassured and treated with NSAID, antibiotics and follow up with surgery ambulatory care. Conclusion: We authors are reporting this case of primary epiploic appendagitis because we think every emergency and primary care physicians should be aware of this very rare condition which might present to emergency as an acute abdominal presentation mimicking other common presentations like acute diverticulitis and acute appendicitis. Being aware of this condition is utmost important to diagnose it early and avoid more invasive surgical managements and unnecessary antibiotic usage.

Abstract:Background Epiploic appendagitis (EPA) is an uncommon emergency surgical condition that causes acute abdominal pain, rendering a list of differential diagnoses. Therefore, careful examination and imaging tools are required. EPA is a self‐limiting condition that can be resolved in 1–2 weeks and rarely needs surgical intervention. Its low incidence makes EPA less well‐known among the public and some medical professionals, and it is frequently under‐diagnosed. We aimed to explore the incidence, clinical presentation, modalities of imaging to diagnose and options for treating EPA. Methods An observational retrospective analysis was conducted between 2016 and 2022 at a tertiary hospital in an Arab Middle Eastern country. Results There were 156 EPA cases diagnosed over six years, with a mean age of 33 years. Males represented 82% of the cohort. The entire cohort was treated non‐operatively except for eight patients who had surgical intervention using open or laparoscopic surgery. The diagnosis was made by a computerized tomographic scan (CT). However, plain X‐ray, abdominal ultrasound, and magnetic resonance imaging (MRI) were performed initially in a few selected cases to rule out other conditions. No specific blood test indicated EPA; however, a histopathology examination was diagnostic. No mortality was reported in the study cohort. Conclusion This is the most extensive study analyzing EPA patients from the Middle East. EPA is a rare and mostly self‐limiting acute abdominal disorder; however, early ultrasound and CT scan can pick it up quickly after a high index of suspicion.

Abstract:Epiploic appendagitis is a relatively rare disease characterized by an inflammation of fat-filled serosal outpouchings of the large intestine, called epiploic appendices. Diagnosis of epiploic appendagitis is made challenging by the lack of pathognomonic clinical features and should therefore be considered as a potential diagnosis by exclusion first of all with appendicitis or diverticulitis which are the most important causes of lower abdominal pain. Currently, with the increasing use of ultrasound and computed tomography in the evaluation of acute abdominal pain, epiploic appendagitis can be diagnosed by characteristic diagnostic imaging features. We present a case of epiploic appendagitis with objective of increasing knowledge of this disease and its diagnostic imaging findings, in order to reduce harmful and unnecessary surgical interventions.

Abstract:A versatile synthesis of unsymmetrically N,N'-substituted saturated carbenes is described. The novel racemic imidazolidin-2-ylidenes rac-5 have been synthesized by reductive desulfurization of the corresponding imidazolidin-2-thiones rac-4. The thiones were prepared in two reaction steps from aldimines and secondary amines. Three different substituents at N1, N3 and C4 of the five-membered N-heterocyclic ring can be introduced by choice of suitable aldimines and secondary amines. The dimerization behaviour (diaminocarbene/enetetramine equilibrium) for the unsymmetrically N,N'-substituted imidazolidin-2-ylidenes has been investigated by NMR spectroscopy. Unsymmetrically N-iPr and N-iBu substituted N-heterocyclic carbenes undergo a slow dimerization, whereas N-tBu substituted derivatives are stable as monomeric carbenes indefinitely. The carbene ligand rac-5d has been coordinated to rhodium(I) to give the square-planar rhodium carbene complex [Cl(cod)Rh(rac-5d)]rac-6d which has been characterized by an X-ray diffraction analysis.

Abstract:Epiploic appendagitis is a rare cause of acute lower abdominal pain. Epiploic appendices are fat-filled serosal outpouchings of the cecum and sigmoid colon. Primary epiploic appendagitis (PEA) is characterized by epiploic inflammation caused by torsion of the appendage leading to ischemia or thrombosis of the appendage draining vein. Secondary epiploic appendagitis occurs in association with other inflammatory conditions of the abdomen or pelvis, most commonly diverticulitis. PEA is an important clinical mimicker of more severe causes of acute abdominal pain, such as diverticulitis, appendicitis, or gynaecological causes. The ease of access to computed tomography (CT), the diagnostic test of choice, has resulted in increased recognition of PEA. The classic CT findings of PEA are an ovoid mass measuring between 1.5 and 3.5 cm surrounded by a hyperattenuating/hyperdense ring with a centrally located hyperdense area. It is important to diagnose PEA as it is self-limiting and the correct diagnosis can prevent unnecessary hospital admission, antibiotic use, or even surgical intervention. We present a case of a 65-year-old male with a history of diverticulitis, presenting with left lower quadrant abdominal pain who was diagnosed with PEA based on CT and successfully managed with conservative treatment.
Primary Epiploic Appendagitis: A Case Report

Abstract:Primary epiploic appendagitis (PEA) is a rather uncommon and self-limiting cause of acute abdomen managed conservatively. Overlapping clinical features with other common causes of acute abdomen usually requiring surgical intervention, and rare occurrences have led to misdiagnosis of the condition and unnecessary surgical intervention. However, with identification of definite characteristic features on imaging (computed tomography [CT] scan) has led to easier diagnosis and avoidance of exploratory laparotomy. Here we present a case of PEA in a 34-year-old otherwise healthy Caucasian male with a chief complaint of acute left-sided abdominal, flank and inguinal pain with diarrhea. Laboratory investigation reports were more or less within normal limits; CT scan confirmed the diagnosis of PEA. The patient was managed successfully with an oral antibiotic and a non-steroidal anti-inflammatory drug. CT scan should be done in cases of acute abdomen (if not absolutely contraindicated) for confirmation of diagnosis, as in our case CT scan helped in confirmation of diagnosis of PEA and thus avoided unnecessary surgical intervention. However, with the current advances in radiological tools, correct diagnosis of acute abdomen has become a lot easier, leading to timely surgical intervention and also at the same time avoidance of unnecessary exploratory laparotomy. Again, with documentation of specific characteristic radiological features of PEA, diagnosis of PEA has become much easier. After careful correlation among clinical, radiological, and laboratory findings, diagnosis of PEA was confirmed. The patient was managed conservatively at home with the advice of plenty of fluid intake and bed rest. Furthermore, he was prescribed an oral antibiotic (ciprofloxacin) and a non-steroidal anti-inflammatory drug (ibuprofen) empirically for seven days to prevent further complications like adhesions, bowel obstruction, intussusception, peritonitis, and local abscess formation. The patient recovered completely (the symptoms and signs resolved clinically) after one week. To conclude, it can be said, although rare in occurrence and lacking in specific presenting features, diagnosis of PEA has become easier with imaging techniques like CT scan and magnetic resonance imaging (MRI); thus, with prior awareness regarding this disease among physicians, unnecessary surgical interventions can be avoided.

Abstract:Appendagitis has always been considered as a diagnosis of exclusion. Most of the times it is diagnosed incidentally. The majority of patients present flank pain, right and/or left iliac fossa pain with few other symptoms. These findings lead usually the physician toward a surgical disease. Imaging is very important to confirm the diagnosis of appendagitis and to avoid unnecessary hospitalizations and surgeries. In this article, we present five cases of acute appendagitis that have had nearly similar symptoms at presentation and while we were suspecting more severe diseases on their admission at emergency department (ER). The five patients received only medical treatment for their conditions and were discharged home the same day to be reviewed* few weeks later and showed complete resolution of the appendagitis on follow up imaging.

Abstract:Background Acute epiploic appendagitis is a rare differential diagnosis of unclear or acute abdomen. Aim To describe – by means of a scientific case report – the extraordinary diagnosis of acute epiploic appendagitis along with contained diverticulitis and incarcerated herniation of the greater omentum into the hernial sac of a former trocar site (medical history, significant for laparoscopic ovarian cyst removal) in a 29-year-old female based on experiences obtained in the successful clinical case management, and on selective references from medical scientific literature. Case summary Medical history: A 29-year-old female was admitted with abdominal pain in the lower left quadrant. She reported a laparoscopic ovarian cyst removal 3 years prior. Physical examination of the abdomen revealed tenderness in the lower left quadrant without a palpable mass. Leading diagnoses were found using transabdominal ultrasound and confirmed by an abdominal CT scan; namely, incarcerated trocar hernia, diverticulitis of sigmoid colon, and acute epiploic appendagitis. Therapeutic approach was comprised of an explorative laparoscopy (because of the incarcerated hernia), adhesiolysis, removal of a tip of the greater omentum out of the hernial sac, closure of the hernial orifice, and removal of an unclear, inflamed, and bloody fatty tissue from the wall of the descending colon ( histopathological investigation confirmed acute epiploic appendagitis). This was flanked by conservative treatment of diverticulitis of the sigmoid colon. Further clinical course was uneventful, with discharge on the 3 rd postoperative day with favorable long-term outcome, characterized by no further complaints for 15 months. Discussion Acute epiploic appendagitis is an inflammatory, usually self-limiting condition of the epiploic appendages of the colon. It typically manifests with abdominal pain in the lower left quadrant. Imaging is an important diagnostic tool to determine whether the patient has, in fact, acute epiploic appendagitis, so recognizing the characteristic oval lesions with the surrounding inflammation and central fat attenuation on CT, as well as the hyperechoic oval lesions with a hypoechoic peripheral band on ultrasound images is crucial. Nevertheless, it is often overlooked in patients and confused with other differential diagnoses, such as appendicitis or diverticulitis. Although the condition appears infrequently, it is essential to be proficient in the diagnostic evaluation, as a misdiagnosis may lead to unnecessary treatment and even surgical intervention. In conclusion , the patient was initially diagnosed with an incarcerated abdominal hernia, and therefore subsequently underwent surgery. The inflamed epiploic appendage was discovered in laparoscopic exploration, removed, and confirmed through the histopathology report. This is an approach to be performed with great caution so as not to 1. misinterpret an inflamed diverticula or covered perforation of it as well as not 2. overlooking a peritoneal tumor lesion. Abstract:PURPOSE: Acute epiploic appendagitis (EA) is a relatively rare, benign and local inflammatory disease involving the epiploic appendices. Unlike its mimics, EA is generally a self-limiting inflammatory disease and can be treated conservatively.CASE PRESENTATION: A 33-year-old Caucasian man presented to our emergency department with a sever and sharp left iliac fossa pain. He underwent abdominal X-ray, ultrasound (US) and computed tomography (CT) evaluations.CONCLUSION: We illustrate US and CT findings to increase the radiologists' awareness of this condition and to avoid diagnostic delay and unnecessary use of antibiotics, hospitalization and surgery.

Abstract:Acute epiploic appendicitis (AEA) is a rare disease characterized by infection or ischemia of fat-filled sacs around the colon. It is difficult to diagnose and can be confused with other acute abdominal pain requiring surgery. Computed tomography (CT) is the gold standard technique for acute epiploic appendicitis. Our study aims to evaluate the CT findings of acute epiploic appendicitis and the diagnostic value of neutrophil-lymphocyte ratios (NLR) and neutrophil-platelet ratios (NPR). We included one hundred twenty patients, 40 of whom were diagnosed with acute epiploic appendicitis, and 80 had similar complaints of abdominal pain and needed urgent surgery for treatment. First of all, power analysis was performed to determine the number of patients in the control group. Control group patients were determined by a lottery program. Pathologies detected in the control group were acute appendicitis, peptic ulcer perforation, colon perforation, mesenteric ischemia, ureteral stone, ovarian torsion, ovarian cyst rupture, and ectopic pregnancy rupture. The mean age in the AEA group was 41.6 years, and the group was predominantly male (70%). The most common complaint of the AEA group was abdominal pain (80.1%).
