Abstract:Ulcerative colitis (UC) is a chronic idiopathic inflammatory bowel disorder of the colon that causes continuous mucosal inflammation extending from the rectum to the more proximal colon, with variable extents. UC is characterized by a relapsing and remitting course. UC was first described by Samuel Wilks in 1859 and it is more common than Crohn's disease worldwide. The overall incidence and prevalence of UC is reported to be 1.2-20.3 and 7.6-245 cases per 100,000 persons/year respectively. UC has a bimodal age distribution with an incidence peak in the 2nd or 3rd decades and followed by second peak between 50 and 80 years of age. The key risk factors for UC include genetics, environmental factors, autoimmunity and gut microbiota. The classic presentation of UC include bloody diarrhea with or without mucus, rectal urgency, tenesmus, and variable degrees of abdominal pain that is often relieved by defecation. UC is diagnosed based on the combination of clinical presentation, endoscopic findings, histology, and the absence of alternative diagnoses. In addition to confirming the diagnosis of UC, it is also important to define the extent and severity of inflammation, which aids in the selection of appropriate treatment and for predicting the patient's prognosis. Ileocolonoscopy with biopsy is the only way to make a definitive diagnosis of UC. A pathognomonic finding of UC is the presence of continuous colonic inflammation characterized by erythema, loss of normal vascular pattern, granularity, erosions, friability, bleeding, and ulcerations, with distinct demarcation between inflamed and non-inflamed bowel. Histopathology is the definitive tool in diagnosing UC, assessing the disease severity and identifying intraepithelial neoplasia (dysplasia) or cancer. The classical histological changes in UC include decreased crypt density, crypt architectural distortion, irregular mucosal surface and heavy diffuse transmucosal inflammation, in the absence of genuine granulomas. Abdominal computed tomographic (CT) scanning is the preferred initial radiographic imaging study in UC patients with acute abdominal symptoms. The hallmark CT finding of UC is mural thickening with a mean wall thickness of 8 mm, as opposed to a 2-3 mm mean wall thickness of the normal colon. The Mayo scoring system is a commonly used index to assess disease severity and monitor patients during therapy. The goals of treatment in UC are three fold-improve quality of life, achieve steroid free remission and minimize the risk of cancer. The choice of treatment depends on disease extent, severity and the course of the disease. For proctitis, topical 5-aminosalicylic acid (5-ASA) drugs are used as the first line agents. UC patients with more extensive or severe disease should be treated with a combination of oral and topical 5-ASA drugs +/- corticosteroids to induce remission. Patients with severe UC need to be hospitalized for treatment. The options in these patients include intravenous steroids and if refractory, calcineurin inhibitors (cyclosporine, tacrolimus) or tumor necrosis factor-α antibodies (infliximab) are utilized. Once remission is induced, patients are then continued on appropriate medications to maintain remission. Indications for emergency surgery include refractory toxic megacolon, colonic perforation, or severe colorectal bleeding.
Natural History of Adult Ulcerative Colitis in Population-based Cohorts: A Systematic Review

Abstract:Background & aims: A comprehensive knowledge of the natural history of ulcerative colitis (UC) helps understand disease evolution, identify poor prognostic markers and impact of treatment strategies, and facilitates shared decision-making. We systematically reviewed the natural history of UC in adult population-based cohort studies with long-term follow-up. Methods: Through a systematic literature review of MEDLINE through March 31, 2016, we identified 60 studies performed in 17 population-based inception cohorts reporting the long-term course and outcomes of adult-onset UC (n = 15,316 UC patients). Results: Left-sided colitis is the most frequent location, and disease extension is observed in 10%-30% of patients. Majority of patients have a mild-moderate course, which is most active at diagnosis and then in varying periods of remission or mild activity; about 10%-15% of patients experience an aggressive course, and the cumulative risk of relapse is 70%-80% at 10 years. Almost 50% of patients require UC-related hospitalization, and 5-year risk of re-hospitalization is ∼50%. The 5-year and 10-year cumulative risk of colectomy is 10%-15%; achieving mucosal healing is associated with lower risk of colectomy. About 50% of patients receive corticosteroids, although this proportion has decreased over time, with a corresponding increase in the use of immunomodulators (20%) and anti-tumor necrosis factor (5%-10%). Although UC is not associated with an increased risk of mortality, it is associated with high morbidity and work disability, comparable to Crohn's disease. Conclusions: UC is a disabling condition over time. Prospective cohorts are needed to evaluate the impact of recent strategies of early use of disease-modifying therapies and treat-to-target approach with immunomodulators and biologics. Long-term studies from low-incidence areas are also needed.
Is Adult-Onset Ulcerative Colitis Just Confined to Colon?

Abstract:To the Editor: Ulcerative colitis (UC) is traditionally known to be confined to the large bowel. The presence of upper gastrointestinal (GI) involvement has long been considered as a sign of Crohn disease (CD) rather than UC. However, involvement of the upper GI and small intestine has previously been reported in UC.[1,2] Importantly, both the symptoms and endoscopic features easily lead to a misdiagnosis of acute erosive or infectious gastroduodenitis. The present case highlights the need to consider upper GI involvement of UC as a differential diagnosis. This case may prompt physician to recognize the possibility of an adult-onset UC subtype involving the upper GI; to consider whether upper GI involvement should be classified as a special and rare type of UC like CD, suggesting that it may be a high-risk factor for active disease or poor prognosis; and to determine whether more aggressive treatment are needed for patients with this kind of condition. A 24-year-old woman was diagnosed with UC with left-sided colonic involvement at the age of 22. Disease remission was achieved with oral mesalamine, which she elected to discontinue after 1 year because she felt well. Eleven months later, she experienced recurrent bloody diarrhea, together with abdominal pain and significant nausea and vomiting. On presentation to the emergency department, she was afebrile and not tachycardic; however, tenderness was noted on palpation of the epigastrium and left lower abdomen. Laboratory tests showed elevated inflammatory marker level (C-reactive protein 86 mg/dL), mild anemia (hemoglobin 10.6 g/dL) and hypoalbuminemia (albumin 29.2 g/L). A non-contrast computed tomography (CT) scan unexpectedly showed diffuse gastroduodenitis, in addition to continuous colitis [Figure 1A]. She had not taken any drugs, including non-steroidal anti-inflammatory drugs in the last 11 months. Infectious causes including Clostridium difficile, Epstein-Barr virus (EBV), and cytomegalovirus (CMV) were ruled out. She was treated with full-dose oral and topical mesalamine, intravenous proton pump inhibitor, and empiric antibiotics.Figure 1: (A) Non-contrast CT showed gastroduodenitis and colitis. Esophagogastroduodenoscopy revealed diffuse gastroduodenitis with multiple small fibrin-covered erosions in the stomach and duodenum [Figure 1B–D], which closely resembled the UC lesion on colonoscopy [Figure 1E–G]. The pathology of colon [Figure 1H–J] and upper GI tract [Figure 1K–M] were diagnosed with colonic UC and UC with upper GI involvement, respectively, as they showed negative staining for Helicobacter pylori, CMV and EBV; no evidence of granulomas which might suggest CD; and no features resembling erosive gastroduodenitis. The diagnosis of UC was further confirmed by serologic tests for perinuclear anti-neutrophil cytoplasmic antibody (p-ANCA) positive and anti-Saccharomyces cerevisiae antibody negative, as well as by capsule enteroscopy, which demonstrated normal ileal and jejunal mucosa. Intravenous methylprednisolone 60 mg once daily was commenced, with complete resolution of all symptoms within 3 days. Abdominal CT was repeated after 5 days of treatment which showed that the gastroduodenal inflammation had considerably improved [Figure 1N]. The patient was discharged on a tapering course of oral corticosteroids and was maintained on mesalamine. Follow-up endoscopy after a course of steroid therapy showed histologic remission in the colon, with improvement of the inflammation in the gastroduodenum, although a chronic diffuse coarse granular appearance remained [Figure 1O–Q]. Pathologic re-examination of gastroduodenum [Figure 1R and 1S] showed gastroduodenal lesion of UC in remission. Remission was still maintained with mesalamine in the 20-month follow-up. Our case is a rare occurrence that challenges the traditional Montreal classification of adult-onset UC, which does not include upper GI or small-bowel involvement. The present case might be a new phenotype of UC (i.e., UC with upper GI involvement) for several reasons: (1) The patient met the diagnostic criteria for colonic UC, with typical clinical manifestations, endoscopic and radiographic features and pathologic evidence (diffuse and continuous colitis), which ruled out CD, inflammatory bowel disease unclassified and indeterminate colitis; (2) the upper GI lesion in this patient was diagnosed with UC with upper GI involvement because of the good response to steroid therapy and its high similarity to colonic lesions in the endoscopic and macroscopic findings of the upper GI tract in the active inflammatory stage and especially in the remissive stage; (3) other common reasons for gastroduodenitis were excluded, including infectious and drug factors. The results of tests for serologic markers were positive for p-ANCA and were negative for ANCA, also suggested UC. This raises the question of whether upper GI involvement should be classified as a special and rare type of UC. Involvement of the upper GI tract and small intestine has been reported in adult-onset UC.[1–3] A previous study has also demonstrated that patients with UC and upper GI involvement may be at a higher risk for both extraintestinal manifestations and post-operative pouchitis.[4] As we know, the classification of adult-onset UC generally does not change to CD over time. However, the Montreal classification of UC in adults lacks of the subtype of UC with upper GI involvement. In contrast, the revised Porto classification of UC in children and adolescents from the European Society for Paediatric Gastroenterology Hepatology and Nutrition defines two categories of UC (typical and atypical), in which the presence of upper GI/small-bowel involvement is classified as atypical UC.[5] Taking insights from categories of UC in children and adolescents, it is worth considering if UC with upper GI involvement in adults should be classified as a separate disease subtype with a different disease course. Abstract:BACKGROUND: Ulcerative colitis (UC) is a chronic relapsing disease necessitating lifelong treatment. Most patients present with mild-to-moderate disease characterized by alternating periods of remission and clinical relapse. Continued disease progression and relapse of UC over time are associated with an increased risk of colorectal cancer (CRC).OBJECTIVE: To discuss the latest treatment options for mild-to-moderate UC, to review the current data involving the economics of UC, and to demonstrate the relationship between treatment adherence, clinical relapse, inflammation severity, CRC risk, and treatment outcomes.SUMMARY: One of the main goals of therapy in UC is to induce and maintain a long-lasting remission of disease to reduce or avoid the high personal and financial costs of relapse. In recent studies, researchers have demonstrated a link between increased colonic inflammation and CRC risk, highlighting the importance of preventing relapse, which can lead to costly surgical procedures and hospital stays and thus increase the cost of treatment 2- to 20-fold. The risk of disease relapse is affected by several factors, of which the most prominent is nonadherence to maintenance therapy. Nonadherence to therapy can be associated with several other factors, including forgetfulness, male sex, complicated dosing regimens, treatment delivery methods (oral vs. rectal), and pill burden. In the treatment of mild-to-moderate UC, 5-aminosalicyclic acid (5-ASA) is the standard first-line therapy and the treatment of choice for maintaining remission of disease. Novel formulations of 5-ASA and newly devised high-dose 5-ASA regimens offer more options for the treatment of UC and thus may lead to improved treatment adherence, longer remission, and improved patient well-being.CONCLUSION: Periods of remission during UC treatment must be aggressively maintained to prevent relapse and decrease the risk of an unfavorable outcome. By controlling the risks and conditions that lead to therapeutic nonadherence and relapse among patients with UC, clinicians can increase the likelihood of long-term remission and ensure favorable long-term outcomes.

Abstract:Ulcerative colitis (UC) is a chronic inflammatory bowel disease of unknown aetiology affecting the colon and rectum. Multiple factors, such as genetic background, environmental and luminal factors, and mucosal immune dysregulation, have been suggested to contribute to UC pathogenesis. UC has evolved into a global burden given its high incidence in developed countries and the substantial increase in incidence in developing countries. An improved understanding of the mechanisms underlying UC has led to the emergence of new treatments. Since the early 2000s, anti-tumour necrosis factor (TNF) treatment has significantly improved treatment outcomes. Advances in medical treatments have enabled a paradigm shift in treatment goals from symptomatic relief to endoscopic and histological healing to achieve better long-term outcomes and, consequently, diagnostic modalities have also been improved to monitor disease activity more tightly. Despite these improvements in patient care, a substantial proportion of patients, for example, those who are refractory to medical treatment or those who develop colitis-associated colorectal dysplasia or cancer, still require restorative proctocolectomy. The development of novel drugs and improvement of the treatment strategy by implementing personalized medicine are warranted to achieve optimal disease control. However, delineating the aetiology of UC is necessary to ultimately achieve disease cure.

[Headache and impacted upper wisdom teeth].

Abstract:The relationships between wisdom tooth and headache have been rarely studied. The aim of this paper was to follow 13 patients, which suffered with idiopathic headache, after extraction of impacted maxillary wisdom tooth. The authors separated in two group the patients on the basis of a different anatomic site of pain. Patients with bilateral headache or monolateral headache without preferable anatomic site did not respond positively to the extractions. On the other hand the group of patients with monolateral headache or bilateral headache with preferable anatomic site obtained an improvement after the extraction of the maxillary homolateral wisdom tooth.
Effect of stomatal opening on the transfer of 131I2 from air to grass.

Abstract:The effect of stomatal opening on the air-to-trass transfer of molecular iodine gas (I2) was studied in a small environmental chamber with an airflow rate of about 1 m/sec above the grass. Measured stomatal densities and stomatal areas were used to determine the per cent of Bromegrass leaf area composed of stomatal openings (As). The transfer parameter Pt(g-1) (analogous to the normalized transfer velocity) was found to be directly dependent upon stomatal opening as measured by As: Pt = 2.21 X 10(-4) + (4.60 X 10(-4) As. The maximum value of Pt observed was 5.43 X 10(-4) g-1 or 2.4 times that observed for As approximately 0. This proportionality to pore area is in agreement with measured diffusion rates through perforated metal screens. It was also found that Bromegrass is far from a "perfect sink" for sorption of 131I2, a fact which must be considered in the development of models of radioiodine transfer. Retention of radioiodine by contaminated foliage is dependent on the radioiodine transfer process and the implications of the current results are indicated.

Abstract:Ulcerative colitis (UC) is a chronic disease characterized by diffuse inflammation of the mucosa of the colon and rectum. The hallmark clinical symptom of UC is bloody diarrhea. The clinical course is marked by exacerbations and remissions, which may occur spontaneously or in response to treatment changes or intercurrent illnesses. UC is most commonly diagnosed in late adolescence or early adulthood, but it can occur at any age. The incidence of UC has increased worldwide over recent decades, especially in developing nations. In contrast, during this period, therapeutic advances have improved the life expectancy of patients, and there has been a decrease in the mortality rate over time. It is important to emphasize that there is considerable variability in the phenotypic presentation of UC. Within this context, certain clinical and demographic characteristics are useful in identifying patients who tend to have more severe evolution of the disease and a poor prognosis. In this group of patients, better clinical surveillance and more intensive therapy may change the natural course of the disease. The aim of this article was to review the epidemiology and demographic characteristics of UC and the factors that may be associated with its clinical prognosis.

We aimed to evaluate the factors associated with a chronic active UC disease course. Patients and Methods: Data of all UC outpatients followed for at least 3 years after diagnosis between 2005 and 2018 were retrospectively collected. The primary aim was to identify risk factors for chronic active disease 3 years after diagnosis. Moreover, the following variables were investigated: proximal disease extension or disease regression, proctocolectomy, early use of biologics (BIO) or immunomodulators (IMM), hospitalization, colorectal cancer, and adherence. We defined adherence as both, taking the prescribed therapy and constancy in scheduled follow-up visits. Results: A total of 345 UC patients followed for a median period of 82 months were included. Patients with extensive colitis at diagnosis had a higher rate of chronic active disease 3 years after diagnosis (p< 0.012) together with a higher rate of surgery (p< 0.001) at maximum follow-up. Patients with pancolitis showed significant disease regression over time (51%) without differences in treatment. The only factor associated with chronic active disease was non-adherence (p < 0.03; OR 0.49, 95% CI: 0.26– 0.95). Adherent patients developed chronic active disease (p< 0.025) less frequently but did receive more frequent IMM (p< 0.045) or BIO (p< 0.009) therapy. Conclusion: Patients diagnosed with pancolitis were more likely to have chronic active disease and to undergo colectomy. The only predictor for developing chronically active UC regardless of disease extension was the lack of adherence to therapy within the first 3 years after diagnosis, underlining the importance of tight control of UC patients and the need to timely identify potential risk factors for non-adherence. The clinical course of UC including the risk for surgery is still difficult to predict being characterized by periods of remission and exacerbation which have variable duration. A discrete percentage of patients will develop chronic active disease leading to very poor quality of life (QoL) and a high rate of surgery. 1,2 In a recent paediatric study, 3 the clusters adopted by Henriksen et al 4 were investigated during the first 5 years after diagnosis showing that more than 30% of the patients will develop a chronic active or intermittent disease course despite appropriate treatment. A timely identification of patients with a more disabling disease course over time would be helpful in order to recognize who to treat with a more aggressive therapy aimed to prevent complications and irreversible tissue damage, while, in those with indolent disease, overtreatment may be avoided, and consequently, potential side effects. Abstract:Ulcerative Colitis (UC) is an idiopathic chronically-remitting inflammatory bowel disorder characterized by a contiguous inflammation of the colonic mucosa affecting the rectum that generally extends proximally in a continuous manner through the entire colon. Patients typically experience intermittent exacerbations, with symptoms characterized by bloody diarrhea associated with urgency and tenesmus. The anatomical extent of mucosal involvement is the most important factor determining disease course and is an important predictor of colectomy. The precise etiology of UC is unknown. However, a combination of genetic predisposition and environmental factors seems to have a key role in the development of the disease. UC usually is mildly active but it can be a life-threatening condition because of colonic and systemic complications, and later in the disease course due to the development of colorectal cancer. Interestingly, even if pathogenetic features detected in patients with sporadic CRC can be also found in UC-related colorectal cancer (UC-CRC), this latter is, usually, driven by an inflammation-driven pathway rising from a non-neoplastic inflammatory epithelium to dysplasia to cancer. Thus, a long-term follow-up with colonoscopy surveillance has been recommended. Approximately 15% of UC patients develop an acute attack of severe colitis, and 30% of these patients require colectomy. The initial treatment strategy in UC typically follows the traditional step-up approach. One third of the patients will not respond to steroid therapy and cyclosporine and infliximab are the most common salvage agents employed in these cases in order to avoid emergent surgery. Unfortunately, although a significant short-term benefit have been observed after infliximab treatment, the colectomy rate have remained stable. Surgery in UC depends on the stage of the disease as well as patient's status and is divided into the following settings: urgent, emergent and elective. Despite many efforts the surgical management of UC remains a significant challenge. A multidisciplinary management of UC is key in order to define the best timing and the best procedure for each patient in an individualized basis.

Abstract:Ulcerative Colitis (UC) is a chronic Inflammatory Bowel Disease (IBD) that affects the inner lining of the colon, imposing a significant burden on healthcare systems worldwide. Symptoms include bloody stools and abdominal pain. The prevalence of UC is increasing, particularly among young adults. The pathogenesis involves immune responses, genetics and environmental factors. Diagnostic delays are common, leading to complications and increased costs. Early diagnosis is crucial for preventing complications. Endoscopy is the primary diagnostic tool. Treatment aims to reduce inflammation and achieve remission. Delayed diagnosis can lead to longterm complications such as Colorectal Cancer (CRC) and bowel stenosis. Prompt recognition and management are essential for minimising the disease burden and optimising care. The present study delves into the complex nature of UC, a chronic IBD, revealing its epidemiology, pathogenesis, diagnostic challenges, treatment strategies, prognosis, complications and the impact of delayed diagnosis on patients’ quality of life. The study highlights the evolving understanding of UC pathogenesis, incorporating immune responses, genetics, environmental factors and biomarkers. It also discusses diagnostic delays due to symptom similarities and the need for increased awareness among healthcare providers and the public. The study also discusses advanced diagnostic methods and therapeutic approaches, including novel drug targets and surgical interventions. It also highlights the negative consequences of delayed diagnosis, including increased morbidity, psychological distress and socioeconomic burden. The study emphasises the need for collaborative efforts to ensure timely and appropriate care for UC patients.
Impact of ulcerative colitis from patients' and physicians' perspectives: Results from the UC: NORMAL survey

Abstract:Background: Two national internet surveys were conducted to understand how patients perceive the impact of ulcerative colitis (UC) relative to gastroenterologists. Methods: In total, 451 patients with UC (20% mild, 63% moderate, 13% severe, 4% unsure [patient self-assessment]) were recruited for one survey and 300 gastroenterologists (not associated with the patients) were recruited for the other survey. Results: Patients reported, on average, 8 (self-defined) flares per year; this was more than the number anticipated by gastroenterologists. Sixty-two percent of patients with UC reported that their disease made it difficult to lead a normal life, compared with gastroenterologists' estimations of 36%. Only 42% of patients believed that being in remission could mean living without symptoms. Both patients and gastroenterologists reported that it is difficult for patients to take medication as prescribed every day (42% and 90%) and that managing UC medication is a struggle for patients (49% and 41%). Forty-six percent of patients admitted nonadherence to their therapy over the previous week, while gastroenterologists believed that 41% of their patients were not adherent. Conclusions: These surveys identified disparities between patients' and gastroenterologists' perceptions of the impact of UC on patients' lives. The results suggest that more patients than gastroenterologists estimated chose to adapt their lives to accommodate UC rather than act to optimize therapy and adherence. Improved communication between patients and gastroenterologists, as well as better management strategies and education are necessary.

Abstract:The number of patients with inflammatory bowel disease (IBD) approaching an older age, together with the number of over-60-year-old patients newly diagnosed with IBD, is steadily increasing, reaching 25% of all patients. The present review focuses on late-onset ulcerative colitis (UC) and its initial disease course in comparison with that observed in younger adults in terms of extension at onset and the risk of proximal disease progression, medical treatment, surgery and hospitalization in the first years after diagnosis. We summarize the clues pointing to a milder disease course in a population which frequently presents major frailty due to comorbidities. With increasing age and thus increasing comorbidities, medical and surgical therapies frequently represent a challenge for treating physicians. The response, persistence, and risks of adverse events of conventional therapies indicated for late onset/older UC patients are examined, emphasizing the risks in this particular population, who are still being treated with prolonged corticosteroid therapy. Finally, we concentrate on data on biotechnological agents for which older patients were mostly excluded from pivotal trials. Real-life data from newer agents such as vedolizumab and ustekinumab show encouraging efficacy and safety profiles in the population of older UC patients.

Abstract:Abstract Background Patients with acute severe ulcerative colitis (ASUC) are at increased risk of colectomy following an episode of acute severe exacerbation. Medical rescue therapy in steroid non-responders has reduced the need for emergency colectomy. However, impact of biologics on longer-term colectomy risk is unclear. Methods A retrospective cohort study of adult patients aged ≥18 years hospitalized with acute exacerbation of UC requiring hospitalization and managed with intravenous corticosteroids between 2010 and 2022 at two regional hospitals in London, Ontario, Canada. Steroid non-response was defined as requirement of medical rescue therapy or colectomy. Results A total of 264 adults hospitalized with ASUC were included in the analysis (male: 51.1% [n=136], mean age at admission: 40.8±17.8 years). The majority had extensive colitis (71.3% [n=169]) at the time of diagnosis. 46.3% (n=118/255) presented within one year of disease onset and 23% (n=61) had ASUC as their first presentation with UC. 25.7% (n=66/257) of patients had prior purine analogues and 23.3% (60/257) had previous biologics prior to ASUC presentation. After admission, 55.7% (n=147) responded to intravenous corticosteroids. Of the steroid non-responders, 37.5% (n=99) patients were managed with infliximab rescue therapy and only one patient received tofacitinib rescue therapy. Median CRP, stool frequency and Lindgren index score were statistically significant between steroid responders and non-responders (Table 1). A higher proportion of steroid non-responders had prior exposure to biologics or tofacitinib compared to steroid responders (31.9% vs 16.3%, p<0.01). On multivariate analysis for various factors predicting steroid non-response, assessment by Oxford criteria on day 3 was the only factor that was statistically significant (odds ratio 4.70 (95% CI 1.06-20.8), p = 0.04). Oxford criteria on day 3 had a sensitivity, specificity of 58.6% and70.8%, respectively for predicting steroid non-response. 8% (n=21) required colectomy during index admission. An additional 13% (n = 32) underwent colectomy within 12 months of discharge, for which there was no difference between steroid responders and non-responders (12.2% vs 14.7%). The short term (3 months following discharge) and long-term (3-12 months following discharge) colectomy rates following discharge were 8% (20/245) and 5.4% (12/223) respectively. Hospitalization with UC exacerbation rate following discharge was 17% (38/223). Conclusion 8% of patients admitted for ASUC require colectomy during the same admission and additional 13% required colectomy within 12 months of discharge. Despite a high initial response to corticosteroids, long term colectomy rates and re-hospitalization rates remain high.

Abstract:The incidence and prevalence of ulcerative colitis (UC) has been reported to be rising in newly industrialised regions, such as Latin America. Here, we review data from published studies reporting demographics and clinical aspects of UC in Latin America to further understand epidemiology and disease burden. The incidence and prevalence of UC in Latin America varied between regions and studies, ranging between 0.04 to 8.00/100,000 and 0.23 to 76.1/100,000, respectively, and generally increased over the period from 1986 to 2015. The majority of patients with UC were female (53.6–72.6%) and urban residents (77.8–97.4%). Extraintestinal manifestations were reported in approximately 26–89.4% of patients. Use of biologic therapies was generally low (0.8–16.2%), with the exception of Mato Grosso do Sul, Brazil, with a greater proportion of patients tending to receive 5-aminosalicylates, immunosuppressants or corticosteroids; colectomy rates varied between studies (1.5–22%). A high proportion of patients had moderate to severe UC (45.9–73.0%) and, in 11 of 19 studies, the greatest proportion of patients had extensive disease (pancolitis). Colorectal cancer (0–1.7%) and mortality rates (0–7.6%) were low. This evaluation of published studies may influence therapeutic approaches and the development of strategies to improve healthcare access and patient outcomes, although further high-quality studies are required in patients with UC in Latin America.

Abstract:Ulcerative colitis (UC) is a chronic inflammatory bowel disease that exhibits nonspecific characteristics, involving inflammation limited to the colon mucosa and submucosa, explicitly impacting the rectum and sigmoid colon. Clinical symptoms include persistent or recurrent abdominal pain, diarrhea, mucus, and bloody stools. It is difficult to treat and has a long duration. Severe cases can lead to local or systemic complications and an increased risk of developing cancer. [ 1 , 2 ] The disease is recognized as a difficult-to-treat inflammatory bowel disease by the World Health Organization. Although the incidence of UC is high in Europe and North America, it is stable, while in Asia and the Middle East, the incidence is low but has been reported to be on the rise in recent years. [ 3 , 4 ] Till now, the exact etiology and pathogenesis of UC are still not comprehensively understood. It is thought to arise from a complex interplay of various factors, including genetic predisposition, environmental influences, infectious agents, psychological factors, and dysfunction in the mucosal immune system. [ 5 ] Current clinical treatment for UC primarily utilizes anti-inflammatory drugs, including 5-aminosalicylic acid, corticosteroids, immunosuppressants, and biological agents. However, these drugs can cause adverse reactions, opportunistic infections, bone marrow suppression and tumor induction. [ 6 , 7 ] Emerging therapies such as bone marrow stem cell transplantation and fecal microbiota transplantation (FMT) are still in the early stages of development. These costly treatments have unpredictable efficacy, significantly burdening individuals, families, and society. [ 8 , 9 ] The rising prevalence of UC and the absence of safe and effective treatments underscore the significance of UC research as a cutting-edge frontier in scientific investigation.

Elderly Patients with Moderate-To-Severe Ulcerative Colitis Are More Likely to Have Treatment Failure and Adverse Outcome

Abstract:Background and Aim: The aim of this study was to compare therapeutic responses and prognosis between elderly and nonelderly ulcerative colitis (UC) patients with moderate-to-severe activity. Methods: 148 UC patients with moderate-to-severe activity hospitalized between 2000 and 2019 were enrolled consecutively, including 74 patients with the age of diagnosis over 60 years and 74 patients diagnosed less than 60 years. Patients were matched by gender, duration (±15%), disease activity, and admission time (±1 year). They were followed up until the latest medical record or December 2019. The primary outcome was UC-related colectomy or death. Results: 148 patients were followed over median 37.5 months. For steroid use, 76.8% of elderly UC patients were responsive, lower than that in adult group (85.7%). A decreased level of clinical activity index (2.0 [−1.5 to 4.00] vs. 6.0 [3.0–8.0], p < 0.001), reduction of C-reactive protein (23.9 [3.5–65.5] vs. 27.8 [9.7–58.1] mg/L), and erythrocyte sedimentation rate (9.0 [−1.3 to 30.5] vs. 15.5 [3.8–36.5] mm/h) at 4 weeks after steroid induction was less obvious in the elderly. More elderly patients manifested steroid dependence and resistance. 28.4% of elderly UC patients took colectomy, remarkably more than adult patients (12.2%), which also occurred earlier (8.0 [0.5–44.75] vs. 39.5 [12–57.38] months, p = 0.001). Aging (hazard ratio [HR] 2.868, 95% confidence interval [CI]: 1.290–6.375, p = 0.01), male, steroid resistance, and occurrence of complications were independently related to colectomy. The rate of serious infections was significantly higher in the elderly (55.4% vs. 35.1%, p = 0.013), mainly including cytomegalovirus infection, bacterial infection, and extraintestinal infection. Aging (odds ratio [OR] 2.774, 95% CI: 1.355–5.675, p = 0.015), extensive colonic involvement, steroid resistance, and biologics usage were independently associated with a high risk of concomitant infections. Conclusion: Elderly patients with moderate-to-severe UC experienced more treatment failure and increased risk of UC-related colectomy, mortality, and severe infections, predicting demand for more strict and individualized management.

Abstract:A concise and efficient method for the synthesis of α-methylene-γ-lactams is developed from multicomponent one-pot reactions of aldehydes or ketones, hydrazides, and ethyl 2-(bromomethyl)acrylate promoted by tin powder. The reaction proceeds smoothly under mild reaction conditions without using any catalyst to give the corresponding products in high yields. α-Methylene-γ-spirolactams can also be prepared from cyclic ketones.

Abstract:The term inflammatory bowel disease (IBD) refers principally to two major categories of chronic relapsing inflammatory intestinal disorders: Crohn's disease (CD) and ulcerative colitis (UC). In the United States, it is currently estimated that about 1.5 million people suffer from IBD, causing considerable suffering, mortality and economic loss every year. Yet the cause of IBD is unknown, and until we understand more, prevention or cure will not be possible. There is a lot of variation in the incidence and prevalence of CD based on geographic region, environment, immigrant population, and ethnic groups. The annual incidence of CD in North America is reported to be 3.1-20.2 per 100,000 with a prevalence of 201 per 100,000 population. Based on the epidemiological, genetic and immunological data, CD is considered to be a heterogeneous disorder with multifactorial etiology in which genetics and environment interact to manifest the disease. Several genes have been studied so for with respect to CD, but thus far the strong and replicated associations have been identified with NOD2, IL23R and ATG16L1 genes. The risk factors implicated with CD include smoking, low fiber- high carbohydrate diet, altered microbiome and medications such as non-steroidal anti-inflammatory drugs. CD is typically characterized by transmural inflammation of the intestine and could affect any part of the gastrointestinal tract from mouth to perianal area. In terms of distribution of the disease 25% of the patients have colitis only, 25% is ileitis only and 50% have ileocolitis. The Montreal classification is based on the age at diagnosis (<16, 17-40, > 40), disease location (Ileal, colonic, Ileocolonic) and the disease behavior (nonstricturing/nonpenetrating, stricturing, penetrating). The key features for diagnosing CD comprises a combination of radiographic, endoscopic and pathological findings demonstrating focal, asymmetric, transmural or granulomatous features. Abdominal Computed tomography (CT) enterography is the most preferred first-line radiologic study used in the assessment of small bowel CD. The diagnostic accuracy of magnetic resonance enterography/enteroclysis is similar to that of CT scans and also prevents exposure to ionizing radiation. Endoscopic scores are considered to be the gold standard tool to measure the activity of CD and they are used more commonly in the clinical trials to measure the efficacy of various drugs on inducing and maintaining mucosal healing. The most common scoring systems used to measure clinical disease activity include Crohn's Disease Activity Index (CDAI), HBI- Harvey-Bradshaw index (HBI), short inflammatory bowel disease questionnaire (SIBDQ) and Lehmann score. Management of Crohn's disease has been seen as an evolving challenge owing to its widely heterogeneous manifestations, overlapping characteristics with other inflammatory disorders, often elusive extraintestinal manifestations and uncertain etiology. Therapeutic interventions are tailored to address symptomatic response and subsequent tolerance of the intervention. Chronology of treatment should favor treatment dose acute disease or "induction therapy", followed by maintenance of adequate response or remission, i.e. "maintenance therapy". The medications which are highly effective in inducing remission include steroids and Tumor Necrosis Factor (TNF) inhibitors. Medications used to maintain remission include 5-aminosalicyclic acid products, immunomodulators (Azathioprine, 6-mercaptopurine, methotrexate) and TNF inhibitors (infliximab, adalimumab, certolizumab and golimumab). Surgical interventions like bowel resection, stricturoplasty or drainage of abscess is required in up to two thirds of CD patients during their lifetime. The most common indications for surgical resection are medically refractory disease, perforation, persisting or recurrent obstruction, abscess not amenable to percutaneous drainage, intractable hemorrhage, dysplasia or cancer. Endoscopic recurrence in postoperative CD patients, as defined by Rutgeers score i2-i4 occur in 30-90% of the patients at the neoterminal ileum within 12 months of surgery and almost universally by 5 years. Treating CD requires a comprehensive care team including the patient, primary care provider, and gastroenterologist. In summary CD is a chronic inflammatory condition with a remitting and relapsing course primarily affecting relatively younger population with significant socioeconomic effects.

Abstract:During an examination period of one year in a well defined population of 50 000 inhabitants 44 patients with colitis ulcerosa were diagnostically clarified and secured histologically. 37 patients (84.1%) could be treated and brought into a phase of remission with salazosulfapyridin (Azulfidine), glucocorticoids as well as a liberal gastroenterologic basic diet with restriction on dairy products and occasionally a diet, developed for astronauts. Only 7 patients (15.9%) had to be hospitalized for therapy because of accompanying illnesses, regression in the course of colitis and complications such as anemia and electrolyte imbalances. With view to the degree of severity, the majority of the patients showed signs of erosive and ulcerative changes as well as expanded pseudopolypic formations in the mucous membrane indicating the chronically continuing and chronically relapsing course of the disease. 54.5% of the affected showed endoscopical and histological changes in the rectosigmoid; an additional 27.2? had inflammatory changes up to the left colon flexure (see figure 2). Only 18.3% of the patients showed inflammatory attack of the entire colon. From the degree of severity of the inflammatory changes, the anatomic distribution and expansion in the large bowel, no difference in outpatient therapy with regard to its effectiveness can be seen.
