Abstract:Objectives: This article focuses on older immigrant women from Black Africa living in Montreal. Its purpose is to present the meaning that they give to "successful aging" in their hosting country and to highlight the factors they consider essential to experience "a maximum of satisfaction and happiness" during this stage of their life. It is based on the fact that Quebec society is facing an ever increasing aging of its population which is accompanied with a more and more significant ethnocultural diversification due to immigration. Statistically the number of black women over 65 years increases over the years. In Canada, older women in general are victims of various social inequalities. These vulnerabilities are even more flagrant when one comes from a visible ethnocultural community and can have a biopsychosocial impact on the lives of these older women. These situations justify our concern for the quality of life and well-being of these aged women. Method: Our methodology leans on an exploratory qualitative approach conducted with seven women aged from 65 to 77 years who immigrated to Canada when they were more than 50 years old. Five of them had less than 10 years of stay in Quebec while the other two were 18 and 20 years. To gather their views, semi-structured interviews were recorded, transcribed and coded. The content was analyzed with an approach inspired by analysis techniques of data from the grounded theory. Then, an intersectional analytical framework has been favored, firstly to understand the complex nature of various forms of identities and social inequalities experienced by the participants, and secondly to examine the connections between discriminatory phenomena such as ageism, racism or sexism, ethnicity and even the migratory journey. Results: As results, this research allows noting that older immigrant women of Black Africa are facing many difficulties due to the interrelation of their age, their ethnic background, their gender, with other aspects of their identity such as culture, language, and migratory course. Anyway, it is important to emphasize that the majority looked at aging and this time of their life as normal and even as a privilege. Their comments reflected no denial, no bitterness, and no regret. These women who presented themselves as resourceful people, hardworking, determined and combative have enumerated four key elements that they find essential for successful aging: social engagement, intergenerational relationships, financial autonomy and faith. Even though a multitude of obstacles affects their perception of "successful ageing," their stories have demonstrated their ability to cope with difficulties and have shown their inner strengths. Black African older women are working hard to integrate or adjust to an environment in which changes and novelty are not always favorable to their development. Conclusion: If old age is undoubtedly a woman's world, it is also more and more diverse and multi-ethnic. In an ever-changing immigration environment, the results of this study cannot be generalized because of the limited number of participants. However, this research clearly shows that significant efforts are still required to tackle the problems and obstacles faced by older and aging ethnic minorities. Beside limitations identified in this study, several avenues of research could be considered, particularly with regard to their relationship to health services, in conjunction with language and cultural barriers. It is therefore appropriate to interpellate government officials to take into account in their new social policies: gender, ethnicity, economic conditions, social isolation, and poor access to health services of aged immigrant women.
Embodied Aging: Everyday body practices and Later Life Identities among the South Asian Indian Gujarati Diaspora in Canada

Abstract:This study explores how South Asian Indian Gujarati older adults in Canada (Greater Vancouver area) strive to maintain personal continuity, citizenship, and selfhood through everyday body management practices (exercise/yoga, medication/health supplements, skin, and hair care routines) and cultural markers such as food, sartorial choices, and community engagement. This examination, we contend, is noteworthy against the backdrop of contemporary North American academic and popular discourses of a burgeoning consumerist movement around the medicalization of bodies and anti-aging technologies. Drawing on in-depth qualitative interviews of 26 older adults, we discuss how growing old in the diaspora is marked with moral ambivalence between ‘successful aging’ and ‘aging gracefully.’ Based on an inductive thematic analysis, we identify four major themes in how the older diaspora negotiate aging and reorganise their lives through changing social relations and shifting cultural institutions. The first theme is the growing salience of both bodily and social changes in conceptualizing “old age,” and how the experiences of aging vary by gender. Specifically, while most of the female participants visualized old age in terms of a loss of physical functionality, the male participants described agedness in terms of a loss of economic and social worth. The second major theme encapsulates the acceptable coping strategies for dealing with bodily changes and the associated reconfigurations of social roles. While a fit body and functionality were regarded as foundational traits for aging well by all participants, corrective measures or anti-aging products were not espoused as the most culturally appropriate “Indian” way of growing old. The third theme highlights the apprehensions regarding growing old in a foreign country, including a foreboding anxiety of dependence and frailty in the absence of traditional familial care networks. The final theme, explores how for most participants, the notion of home evoked ambivalence in constructing their sense of belonging and identity, often expressed through everyday practices and memory-keeping. Taken together, we ultimately show how age and embodiment are inextricably linked in the experience of growing old in the diaspora.
Social and Cultural Insights into Healthy Aging: A Qualitative Study from the South Indian City of Bengaluru, India

Abstract:Background: Qualitative methods through lived experience narratives provide relevant sociocultural insights into healthy aging. Objectives: The aim of this qualitative study was to explore social and cultural perceptions of healthy aging from older adults (OAs), their next of kin, and those involved in providing services to OAs in Bengaluru, India. Materials and methods: We conducted in-depth interviews with 28 participants, all purposefully selected based on specific inclusion criteria, to get as varied a sample as possible. A pilot-tested, open-ended topic guide was used for every interview which was audio recorded with the permission of the respondent. Verbatim data were transcribed, reviewed for errors, and coded using NVivo 12 software and the framework analysis method of combining deductive and inductive codes. Results: In total, 794 codes covering concepts of healthy aging, enablers and threats to healthy aging, and perspectives for the future were categorized into four themes supporting healthy aging, namely emotional well-being and a sense of purpose, family and social support, financial security, and health-care access. Each of these had a bearing on the OA's physical and mental health. Across socioeconomic groups, a sense of purpose at the level of the self, family, and society emerged as a key emotional sustainer. Social and economic deprivations were key threats to healthy aging and hence required social security and governmental interventions. Conclusion: Sociocultural economic factors are key to healthy physical and mental aging in the context of India. The same factor could be an enabler and in its absence a threat.

Abstract:The COVID pandemic brought to light the pressing issues of social isolation and loneliness for older adults. Immigrant older adults' experience of loneliness is even more exacerbated by factors, such as, language barriers, and the loss of cultural community. Key determinants of loneliness in older immigrants are not clear in the literature. A cross-sectional study was conducted in nine cities across Canada to: describe the experience of emotional, social and overall loneliness; and examine the determinants of loneliness among Punjabi, Mandarin, and Arabic-speaking older immigrants. A total of 647 older immigrants participated in the study. Descriptive statistics were used to describe their experience of loneliness, and multiple regression analysis was performed to examine the determinants of loneliness. Most participants had a post-secondary education, were married, and had been in Canada for about 16 years. On average, the participants reported good physical and mental health, and moderate levels of emotional, social, and overall loneliness. Ethnocultural group, emotional wellbeing, and depression were associated with emotional loneliness. Social loneliness was associated with education, depression, psychological distress, age, and ethnocultural group. Determinants of overall loneliness were age, gender, ethnocultural group, self-rated mental health, emotional wellbeing, depression, and psychological distress. Community based interventions that target these key factors must be designed to address loneliness experienced by older immigrants.
Critical leg ischaemia in New Zealand: economic cost of amputation versus intravenous iloprost.

Abstract:The purpose of this study was to establish the incidence of surgical amputation for critical leg ischaemia in New Zealand, and estimate the hospital, prostheses and indirect costs of this intervention. The cost of amputations was then compared with the cost of treating such patients with iloprost. The study was retrospective. Individual patient records relating to 1991 for both public and private hospitals were analysed. Unit costs relevant to 1991 were applied to the volume data of patients and procedures to derive total costs. Costs were estimated on an incremental basis taking a societal perspective. Conservative estimates were obtained for hospital costs, prostheses and for production loss (loss of output or productivity). Total cost was $NZ15.9 million (hospital and prosthesis cost $NZ13.1 million, production loss $NZ2.8 million). The total quantified cost per amputation was $NZ23 038 (hospital and prosthesis cost $NZ19 020, production loss $NZ4017). 32% of patients requiring amputations were in the working age group. The theoretical avoidance of amputation by treatment with iloprost resulted in net savings of hospital and prosthetic costs of between $NZ6660 and $NZ8720 per patient. Amputation for critical leg ischaemia is costly and has a high mortality, but for iloprost treatment to be cost effective in a New Zealand hospital setting, patients must be targeted and a success rate of at least 55% achieved in avoidance of amputation and reduction of pain while at rest.

Abstract:Introduction: Racialised immigrant older adults (RIOAs) in Canada have poorer self-rated health and are more likely to report chronic conditions, while they concurrently experience well-documented challenges in navigating and accessing the healthcare system. There is strong evidence that patient and caregiver engagement in their healthcare leads to improved management of chronic disease and better health outcomes. International research suggests that engagement has the potential to reduce health disparities and improve quality of care. We aim to (1) describe what role(s) RIOAs are/are not taking in their own healthcare, from the perspectives of participant groups (RIOAs, caregivers and healthcare providers (HCPs)); and (2) develop a codesign process with these participants, creating linguistically aligned and culturally aligned tools, resources or solutions to support patient engagement with RIOAs. Methods and analysis: Using a cross-cultural participatory action research approach, our work will consist of three phases: phase 1, strengthen existing partnerships with RIOAs and appropriate agencies and cultural associations; phase 2, on receipt of informed consent, in-depth interviews with RIOAs and caregivers (n=~45) and HCPs (n=~10), professionally interpreted as needed. Phase 3, work with participants, in multiple interpreted sessions, to codesign culturally sensitive and linguistically sensitive/aligned patient engagement tools. We will conduct this research in the Waterloo-Wellington region of Ontario, in Arabic, Bangla, Cantonese, Hindi, Mandarin, Punjabi, Tamil and Urdu, plus English. Data will be transcribed, cleaned and entered into NVivo V.12, the software that will support team-based analysis. Analysis will include coding, theming and interpreting the data, and, preparing narrative descriptions that summarise each language group and each participant group (older adults, caregivers and HCPs), and illustrate themes. Ethics and dissemination: Ethics clearance was obtained through the University of Waterloo Office of Research Ethics (ORE #43297). Findings will be disseminated through peer-reviewed publications, presentations and translated summary reports for our partners and participants.

Abstract:Drawing on data from face-to-face semi-structured interviews of 31 older Chinese women and men emigrating recently from mainland China to Canada, this paper explores their settlement experience with regard to socio-cultural adaptation, and overall self-perceived quality of life in the new country. Descriptive analysis of data was done using SPSS and the interview data were transcribed and analyzed in combination with demographic profiles for emerging themes guided by theoretical frameworks. Results indicated that needs for grandchildren’s care assistance appeared to be a strong reason for their international migration. Co-residing with their married children resulted in a decline in power, status and decision-making. The participants keenly felt the loss of their social support networks and socialization/physical activities back in China. Although religious participation, attendance at ESL classes and physical activities were the popular means of their new socialization, these were limited by their care-giving and housekeeping roles. Their overall perception of quality of life after immigration was marginalized by losses and gains, and contingent on multifaceted factors including living arrangement, language, intergenerational relationships, economic status, and supportive networks at individual levels. Recommendations for policies, programs and more services to be offered to elderly immigrants are provided.
Meeting the Challenge of Diabetes in Ageing and Diverse Populations: A Review of the Literature from the UK

Abstract:The impact of type 2 diabetes on ageing societies is great and populations across the globe are becoming more diverse. Complications of diabetes unequally affect particular groups in the UK older people, and people with a South Asian background are two population groups with increased risk whose numbers will grow in the future. We explored the evidence about diabetes care for older people with South Asian ethnicity to understand the contexts and mechanisms behind interventions to reduce inequalities. We used a realist approach to review the literature, mapped the main areas where relevant evidence exists, and explored the concepts and mechanisms which underpinned interventions. From this we constructed a theoretical framework for a programme of research and put forward suggestions for what our analysis might mean to providers, researchers, and policy makers. Broad themes of cultural competency; comorbidities and stratification; and access emerged as mid-level mechanisms which have individualised, culturally intelligent, and ethical care at their heart and through which inequalities can be addressed. These provide a theoretical framework for future research to advance knowledge about concordance; culturally meaningful measures of depression and cognitive impairment; and care planning in different contexts which support effective diabetes care for aging and diverse populations.

Abstract:Immigration in late life can be a complex experience. Older adults who have spent a considerable part of their life in one cultural milieu face several challenges in adapting to a new societal framework. Demographically speaking, the numbers of immigrants of Asian Indian origin continue to rise phenomenally in the United States. In this project, the experience of Asian Indian elderly immigrants to the United States was recorded through home visits and personal interviews. Parents of adult immigrants often choose to immigrate late in life primarily for purposes of family reunification. Providing assistance with raising grandchildren was also an important consideration. This article explores various aspects that surfaced from the analysis of interviews; these include personal investment in adult children, language/cultural barriers, use of formal services, acculturative experience, aging in India, intergenerational relationships, and expectations for the future. The findings highlight the need for gerontological research that is culturally attuned to the needs of these elders so service delivery may be optimally provided.

Molecular Biology of the Cell

Abstract:Wee1 inactivates the Cdc2-cyclin B complex during interphase by phosphorylating Cdc2 on Tyr-15. The activity of Wee1 is highly regulated during the cell cycle. In frog egg extracts, it has been established previously that Xenopus Wee1 (Xwee1) is present in a hypophosphorylated, active form during interphase and undergoes down-regulation by extensive phosphorylation at M-phase. We report that Xwee1 is also regulated by association with 14-3-3 proteins. Binding of 14-3-3 to Xwee1 occurs during interphase, but not M-phase, and requires phosphorylation of Xwee1 on Ser-549. A mutant of Xwee1 (S549A) that cannot bind 14-3-3 is substantially less active than wild-type Xwee1 in its ability to phosphorylate Cdc2. This mutation also affects the intranuclear distribution of Xwee1. In cell-free kinase assays, Xchk1 phosphorylates Xwee1 on Ser-549. The results of experiments in which Xwee1, Xchk1, or both were immunodepleted from Xenopus egg extracts suggested that these two enzymes are involved in a common pathway in the DNA replication checkpoint response. Replacement of endogenous Xwee1 with recombinant Xwee1-S549A in egg extracts attenuated the cell cycle delay induced by addition of excess recombinant Xchk1. Taken together, these results suggest that Xchk1 and 14-3-3 proteins act together as positive regulators of Xwee1.
Understanding dementia in minority ethnic communities: The perspectives of key stakeholders interviewed as part of the IDEAL programme

Abstract:Future populations of older adults in the UK, those aged 65+, will demonstrate increased diversity in terms of their ethnic identity resultant from the ageing of the post-war migrants from India, Pakistan, Bangladesh, and the Caribbean. As a consequence, there will be an increase in the numbers of older adults from these communities living with age-related chronic diseases such as dementia. In response to these demographic changes, we need to develop a research, policy and practice agenda that is inclusive and provides evidence for the development of culturally diverse and effective models of service delivery. This requires engagement with three key stakeholder groups: (a) people with dementia; (b) their carers; and (c) the wider community. As part of the IDEAL research programme on living well with dementia, we undertook semi-structured interviews with twelve community leaders, defined as known and trusted individuals active in their respective communities, and six community members (two people living with dementia and four carers). We explored their understandings, experiences, and views of about dementia. Our analysis identified two overarching themes. The migrant lifecourse highlighted issues of not belonging, discrimination and racism. This framed our second theme, the cultural context of dementia, which addressed dementia knowledge and attitudes, service provision and service access, and how being part of a minority ethnic community made a difference to these experiences. Our study highlights how lifecourse experiences of negative hostile social and policy environments and services can be profound and long-lasting and provide a prism through which accessing dementia care is experienced. Our findings argue for the inclusion of diverse views and lifecourse experiences within the context of developing a dementia strategy for research, policy and practice that is appropriate for a multicultural and heterogenous society.

Abstract:Cultural diversities, histories, communities and nations are intricately intertwined with experiencing old age. Individual and/or household culture, norms and expectations precede the decision-making process for health-seeking behaviour that influences elderly people's choice and use of health-care services. This study addresses the understanding of this issue among migrant Bangladeshi elderly women from culturally and linguistically diverse background, residing in North 24 Parganas, West Bengal, India. Accordingly, data were collected through purposive sampling from 104 elderly women, all aged above 65. Processed data highlighted inconvenience – related to (language) pronunciation, socio-economic status, traditional health beliefs, understandings and practices, and reliance on alternative medical amenities.

Abstract:BackgroundIndia’s elderly population is rising at an unprecedented rate, with a majority living in rural areas. Health challenges associated with ageing, changing social networks and limited public health infrastructure are issues faced by the elderly and caregivers. We examined perceptions of health needs of the elderly across local stakeholders in an urbanizing rural area.MethodsThe qualitative study was conducted among participants in the Andhra Pradesh Children and Parents Study (APCAPS) site in Rangareddy district, Telangana. We collected data using focus group discussions and interviews among communities (n = 6), health providers (n = 9) and administrators (n = 6). We assessed stakeholders’ views on the influence of urbanization on health issues faced and interventions for alleviating these challenges. We used a conceptual-analytical model to derive themes and used an inductive approach to organizing emerging codes as per a priori themes. These were organized as per thematic groups and ranked by different authors in order of importance. Bronfebrenner’s theory was used to understand stakeholder perspectives and suggest interventions within four identified spheres of influence - individual, household, community and services.ResultsStakeholders reported frailty, lack of transport and dependence on others as factors impacting health access of the elderly. Existing public health systems were perceived as overburdened and insensitive towards the elderly. Urbanization was viewed positively, but road accidents, crime and loneliness were significant concerns. Interventions suggested by stakeholders included health service outreach, lifestyle counseling, community monitoring of healthcare and engagement activities.ConclusionsWe recommend integrating outreach services and lifestyle counseling within programs for care of the elderly. Community institutions can play an important role in the delivery and monitoring of health and social services for the elderly.

Everyday solitude, affective experiences, and well-being in old age: the role of culture versus immigration

Abstract:Objectives: Being alone is often equated with loneliness. Yet, recent findings suggest that the objective state of being alone (i.e. solitude) can have both positive and negative connotations. The present research aimed to examine (1) affective experience in daily solitude; and (2) the association between everyday affect in solitude and well-being. We examined the distinct roles of culture and immigration in moderating these associations. Method: Using up to 35 daily life assessments of momentary affect, solitude, and emotional well-being in two samples (Canada and China), the study compared older adults who aged in place (local Caucasians in Vancouver , Canada and local Hong Kong Chinese in Hong Kong, China) and older adults of different cultural heritages who immigrated to Canada (immigrated Caucasians and immigrated East Asians). Results: We found that older adults of East Asian heritage experienced more positive and less negative affect when alone than did Caucasians. Reporting positive affect in solitude was more positively associated with well-being in older adults who had immigrated to Canada as compared to those who had aged in place. Conclusions:These findings speak to the unique effects of culture and immigration on the affective correlates of solitude and their associations with well-being in old age.

Abstract:The aging of immigrants is a critical component in the health dynamics of the nation's aging population. To date, few studies have addressed within-group diversity and linked contemporary contexts of global connectedness with the aging experiences of older immigrants. This study aims to conceptually understand the diversity in aging dynamics within a specific immigrant group: Indian immigrants in New York City. The impact of globalization and transnational connection on aging experiences on 2 within groups-Indians who came to the United States at age of 65 or older (LLIs) and those who came at an early age (ELIs) are analyzed. Implications for social work practice, research and policy are discussed.

Abstract:Abstract The role of social capital has gained a lot of attention as one of the determinants of wellbeing of aging adults. It serves the functions of connecting people with vital resources essential to improvement of health, promotion of social cohesion and networks, and enhancement of support received. Social capital could be influenced by a variety of individual and socio-cultural factors among racialized groups. This study aims to examine the variations in social capital in Chinese and South Asian aging adults in Hong Kong. A sample of 1,015 people aged 55 and above, consisting of Chinese (n=800) and South Asian participants (n=215), participated in a survey via telephone (Chinese) and face-to-face (South Asian) interview respectively. A 25-item World Bank’s Social Capital Assessment Tool was used for measuring six variables related to social capital including social participation, social support, social connection, trust, cohesion, and reciprocity, covering the structural and cognitive dimensions of social capital. A generalized linear model was performed to detect differences between the two groups after controlling for demographics. The aging Chinese people exhibited a stronger cohesion level than their South Asian counterparts, whereas the aging South Asians demonstrated higher levels of social participation, social support, social connection, trust, and reciprocity. The identified gaps further illustrate the socio-cultural differences between the groups, highlighting the importance of strategies for enhancing social inclusion and equity for racialized older people in the Chinese context.

Abstract:Objective: To explore the perceptions of elderly people from diverse cultures regarding the factors relevant to aging, identifying similarities and differences, and describing elements that facilitate or hinder this process, with a focus on the impact of care services. Method: Qualitative study with 48 participants over 50 years old, residing in the Alt Empordà region, Catalonia. Seven focus groups were conducted, including one exploratory multicultural group and six stratified by origin: native, Western Europe, Eastern Europe, Latin America, North Africa, and West Africa. Results: All groups valued health and the desire to remain in their homes or communities for as long as possible. African groups emphasized the importance of free health services, while the Latin American group valued personalized end-of-life care. The native group pointed out deficiencies in home care and residential care services. Western European participants mentioned language barriers and challenges with digitalization as the only means of accessing information, and Eastern European participants highlighted the need for empowerment and social participation. Additionally, the importance of new family models, restrictions on non-Catholic funeral practices, and the need for community integration were mentioned. Conclusion: While there are differences in perceptions of aging among the different groups, common factors were identified that either facilitate or hinder this process, regardless of whether there is a migration background.
Cultural Perceptions of Health in Asian Indian Adults

Abstract:Objective: To gain an understanding of the cultural perceptions of health among Asian Indian adults in an urban setting. Design: Qualitative semistructured interviews. Setting: Midwest urban community PARTICIPANTS: Asian Indian adults (n = 20) aged ≥25 years, who self-identified as 100% Asian Indian descent. Phenomenon of interest: Individual interviews were conducted by a trained interviewer to assess cultural perceptions of health. Analysis: Transcript analysis was performed by 2 independent coders using verbatim transcripts. Content analysis was used to identify themes using a grounded theory approach. Results: The salient themes that emerged were a cultural definition of health, acculturation, mental health, and health information. Participants believed good health was associated with the ability to perform daily activities, regular exercise, and eating well. There was a lack of awareness of Asian Indian-specific body mass index categories and that overweight and obesity were an important risk factor for chronic diseases. Conclusion and implications: These data provide a context for health promotion efforts and underscore a gap in awareness of risk factors risk for chronic diseases among the Asian Indian community. Culturally specific interventions targeted at the Asian Indian population, considering their worldview and perceptions of health, will help address this important public health concern.

Urinary Calprotectin Differentiates Between Prerenal and Intrinsic Acute Renal Allograft Failure

Abstract:BackgroundUrinary calprotectin has recently been identified as a promising biomarker for the differentiation between prerenal and intrinsic acute kidney injury (AKI) in the nontransplant population. The present study investigates whether calprotectin is able to differentiate between these 2 entities in transplant recipients as well. MethodsUrinary calprotectin was assessed by enzyme-linked immunosorbent assay in 328 subjects including 125 cases of intrinsic acute allograft failure, 27 prerenal graft failures, 118 patients with stable graft function, and 58 healthy controls. Acute graft failure was defined as AKI stages 1 to 3 (Acute Kidney Injury Network criteria), exclusion criteria were obstructive uropathy, urothelial carcinoma, and metastatic cancer. The clinical differentiation of prerenal and intrinsic graft failure was performed either by biopsy or by a clinical algorithm including response to fluid repletion, history, physical examination, and urine dipstick examination. ResultsReasons for intrinsic graft failure comprised rejection, acute tubular necrosis, urinary tract infection/pyelonephritis, viral nephritis, and interstitial nephritis. Calprotectin concentrations of patients with stable graft function (50.4 ng/mL) were comparable to healthy controls (54.8 ng/mL, P = 0.70) and prerenal graft failure (53.8 ng/mL, P = 0.62). Median urinary calprotectin was 36 times higher in intrinsic AKI (1955 ng/mL) than in prerenal AKI (P < 0.001). Receiver-operating characteristic curve analysis revealed a high accuracy of calprotectin (area under the curve, 0.94) in the differentiation of intrinsic versus prerenal AKI. A cutoff level of 134.5 ng/mL provided a sensitivity of 90.4% and a specificity of 74.1%. Immunohistochemical stainings for calprotectin in renal allograft biopsy specimens confirmed the serological results. ConclusionsUrinary calprotectin is a promising biomarker for the differentiation of prerenal and intrinsic acute renal allograft failure.
