Abstract:BACKGROUND: Adverse Childhood Experiences (ACEs) have long-term consequences. The ACEs model has been extensively used in high-income countries. However, its effectiveness has yet to be demonstrated in low and middle-income countries and marginalized social groups. In indigenous populations, research has found inconsistent results in the prevalence and the relationship between ACEs and mental health outcomes. Therefore, the objectives of this study were 1) to determine the ACEs frequency among indigenous and non-indigenous Mexican adolescents; 2) to test the association between ACE scores and symptoms of Mental Health Problems (SMHP) in both groups; and 3) to review the ACEs model adequacy for the indigenous adolescents.PARTICIPANTS AND SETTINGS: Data from an online survey in 20 of the 32 states of Mexico were analyzed. We selected an 87 adolescent sample from indigenous families and 174 non-indigenous matched for age, sex, and state of residence, with a mean age of 16 years; 60 % were women.METHODS: ACE-IQ questionnaire and Youth Self Report instrument (eight SMHP) were used to collect data. ACEs' frequency was analyzed for each group, and Pearson correlations were used to identify the relationship between ACEs and SMHP.RESULTS: There was no statistical significance between Indigenous and non-Indigenous adolescents in the ACEs report. ACE scores were associated with eight SMHP. Correlations between ACE score and SMHP were higher in non-indigenous adolescents.CONCLUSIONS: ACEs were experienced similarly by both groups and were associated with mental health in all participants. Further research is needed to improve ACEs measurement in indigenous populations.

Abstract:Suicide in young people is a public health problem. Typically, protective factors for suicide are not studied; research tends to focus on measuring risk factors. However, knowing the risk factors does not mean that we also know the opposing factors that influence a group's health problems. For this reason, we examined the relationship between developmental assets in Mexican youth aged 18 to 25 years who are not at risk for suicide, exhibit self-injurious behavior, and whose last suicide attempt had low or high lethality. A cross-sectional study of 478 young people (73% female and 27% male) from Mexico City was conducted using an online survey and correlations were tested with dummy variables (groups) and multinomial logistic regression. The no-risk group showed associations with all developmental assets, the self-injurious group had an association with the house rules variable, the low lethality group was correlated with twelve assets and the high lethality group with four assets. Four internal developmental strengths were significant in the regression model: avoidance of risk behaviors, school expectations, resistance to pressure, and expression of anger. These results suggest that PYD is a useful framework for examining suicide risk and promotes skill development in young college students.
The Impact of Adverse Childhood Experiences on Mental Health and Suicidal Behaviors: A Study from Portuguese Language Countries

Abstract:Background : Research on adverse childhood experiences (ACEs) demonstrates that they can be associated with physical and mental health problems throughout the lifecourse. However, few studies have examined this topic in the Community of Portuguese Language Countries (CPLC). Objective : This study aims to assess the impact of ACEs on mental health and suicidal behaviors in a sample of participants from the CPLC. Participants and Setting : The sample consists of 1006 participants aged between 18 and 80 years (mean = 41.76; SD = 14.19). Methods : This study used an online survey that included a sociodemographic questionnaire, the Brief Symptom Inventory-18 (BSI-18) to assess somatization, depression, and anxiety symptoms, and overall mental functioning, the Suicidal Behaviors Questionnaire-Revised (SBQ-R) to assess suicidal behaviors, and the Family Adverse Childhood Experiences Questionnaire to assess ACEs. Results : Emotional abuse was the most reported ACE (32.7%). Participants from Brazil had higher levels of somatization, depression, anxiety, and suicide ideation and attempt, while participants from Portugal had a higher probability of suicide in the future. ACEs were strong and significant predictors of psychological symptoms and the likelihood of suicide in the future, with emotional abuse and emotional neglect being the domains with the greatest contribution, respectively. Conclusions : ACEs are a prevalent and general phenomenon across several countries. It is urgent to alert policymakers and mental health professionals of the need to intervene with children and families to ensure their harmonious and adjusted development, thus promoting quality of life and well-being of populations.
The role of adverse childhood experiences in suicide among sexual minority undergraduate students

Abstract:This study explores the rates and odds of adverse childhood experiences (ACEs) and their relationship to suicide attempts in a sample of undergraduate students ( n = 924, 71.6% women), comparing lesbian, gay and bisexual (LGB) youth to their heterosexual counterparts. Using propensity score matching, we matched 231 sexual minority and 603 heterosexual participants at a ratio of 1:3, based on gender, age, socioeconomic status, and religious beliefs. Sexual minority participants reported a significantly higher ACE score ( M = 2.70 vs. 1.85; t = 4.93; p <.001; d = .391) and higher rates of all but one type of ACEs than their heterosexual counterparts. They also reported a higher prevalence and risk of suicide attempts (33.3% vs. 11.8% respectively, odds ratio = 3.73; p < .001). In logistic regression analysis, sexual minority status, emotional abuse and neglect, bias attack, having a household member with mental health problems, bullying and cyberbullying were significantly associated with suicide attempts.

Abstract:The main goal of this study was to determine the prevalence of adverse childhood experiences (ACEs) among Latino adolescents from an agricultural community and to examine how it may impact their neuropsychiatric functioning. This research particularly assessed the association between ACEs and depression, as well as ACEs and psychosocial problems. The study sample consisted of 852 adolescents treated at a rural primary care clinic with a comprehensive ACE screening protocol that assesses for ACEs, depressed mood, and psychosocial functioning during every annual Well-Child Visit. Study results showed that ACEs were relatively common among participants with 64 % endorsing having experienced at least one ACE. Approximately 23 % of participants screened positive for depressed mood and 11 % for psychosocial problems. ACEs were found to have significant associations with both depression symptoms and with psychosocial problems. Males were found to have less depression symptoms than females among subjects with exposure to most ACE types, and older age was associated with lower psychosocial impairment. Study participants live in an agricultural community and are likely exposed to both chemical and non-chemical stressors. The exposure to ACEs and chemical environmental stressors may interact with pathological synergy to alter their biobehavioral development. Further research is needed to understand the "rules" for which stressors at what dose and at what stage of development place youth at greatest risk.

Abstract:Introduction: Suicide is the second leading cause of death among adolescents worldwide. Suicidal ideation (SI) in adolescents may be influenced by factors such as resilience, anxiety, and depressive symptoms. The COVID-19 pandemic served as a significant stressor with documented impacts, yet further research is needed to clarify its effects. While stressful events are known to be associated with increased SI, direct evidence linking COVID-19 specifically to elevated SI remains limited. Objective: The aim of this study was to identify the risk and protective factors associated with SI in Mexican adolescents during the COVID-19 pandemic. Method: A cross-sectional, correlational descriptive study was conducted between May and June 2022 with a sample of 2,194 high school students, with a mean age of 16.6 years (SD = 1.2). Among the participants, 58.9% were female and 41.1% were male. They completed inventories to assess stress, anxiety, depressive symptoms, and resilience. Results: Of 2194 adolescents, 15.5% presented SI, with females showing a higher prevalence of SI, anxiety, and depressive symptoms compared to males. In contrast, resilience was lower in females. Furthermore, depressive symptoms were identified as a risk factor for SI (OR 1.212; 95% CI 1.186, 1.240), whereas social competence was highlighted as a protective factor (OR 1.046 95% CI 1.044, 1.078). Conclusions: The findings underscore the importance of preventing SI in adolescents by addressing anxiety and depressive symptoms, which were identified as risk factors, and by promoting social competence as a protective factor. Therefore, an integrated approach that addresses both individual mental health and the social context must be considered for preventing SI in adolescents.

Abstract:Depression, suicidal behavior, excessive alcohol intake, and tobacco use are the main mental health problems in adolescents. To address these problems, it is necessary to understand the many factors associated with them, including parental factors. The aim of this study was to assess the associations between parental behavior and mental health problems in adolescents in Mexico. Data from the National Health and Nutrition Survey (ENSANUT) 2018–2019, representative for Mexico, were used. Households in which a parent–adolescent child pairing was identified (regardless of family type) were selected; n = 8758 households. The four outcomes of interest that were measured in the adolescents were: excessive alcohol intake, tobacco use, suicidal behavior, and depressive symptomatology. Logistic regression models using the adjusted odds ratio (AOR) and 95% confidence interval (95% CI) were estimated. Adolescents whose parents used alcohol or tobacco and reported depressive symptoms and suicidal behavior were more likely to present these behaviors themselves (AOR = 1.47, 95% CI: 1.17–1.85; AOR = 2.26, 95% CI: 1.51–3.39; AOR = 2.61, 95% CI: 1.88–3.61; AOR = 1.74, 95% CI: 1.16–2.61, respectively). Child sexual abuse was also strongly associated with the four outcomes of interest in adolescents (AOR = 1.89, 95% CI: 1.06–3.36 for excessive alcohol intake; AOR = 2.97, 95% CI: 1.49–5.91 for tobacco use; AOR = 5.15, 95% CI: 3.27–8.09 for depressive symptoms; AOR = 6.71, 95% CI: 4.25–10.59 for suicidal behavior). The family constitutes the central nucleus of care for children and adolescents; therefore, any effort to promote adolescent mental health must necessarily involve their parents and family.

Abstract:BACKGROUND: Suicide is a leading cause of death among adolescents and young adults. The current study extends the research linking adverse childhood experiences (ACEs) to suicidal behaviors by testing these associations using a nationally representative sample, assessing for suicide ideation and attempts in adulthood, controlling for established risk factors for suicidality, and measuring a broad array of ACEs.METHODS: The sample included 9,421 participants from the National Longitudinal Study of Adolescent Health who participated in four waves of assessments spanning 13 years. We examined longitudinal associations between eight different ACEs (physical, sexual, and emotional abuse, neglect, parental death, incarceration, alcoholism, and family suicidality) with suicidal ideation and suicide attempts in adulthood, while controlling for depression, problem alcohol use, drug use, delinquency, impulsivity, gender, race, age, and urbanicity. We also tested for cumulative associations of ACEs with suicide ideation and attempts.RESULTS: Logistic regression analyses indicated that physical, sexual, and emotional abuse, parental incarceration, and family history of suicidality each increased the risk by 1.4 to 2.7 times for suicidal ideation and suicide attempts in adulthood. The accumulation of ACEs increased the odds of suicide ideation and attempts. Compared with those with no ACEs, the odds of seriously considering suicide or attempting suicide in adulthood increased more than threefold among those with three or more ACEs.CONCLUSIONS: Intervention strategies need to prevent ACEs from occurring and, if they do occur, should take into account the impact of cumulative ACEs on suicide risk. Future research should focus on identifying mediating mechanisms for the ACEs-suicidality association using longitudinal research designs and determine which ACEs are most important to include in a cumulative ACE measure.

Abstract:Concerns regarding college students' mental health are growing worldwide. Adverse childhood experiences (ACEs) are associated with depression and anxiety symptoms. However, resilience buffers their negative impact on mental health. This study assessed the prevalence and association of anxiety, depression, resilience, with the types of ACEs in psychology students. A cross-sectional design was employed with a sample of 337 Mexican psychology students. Of the students, 70% had four or more ACEs, and the most frequent ACEs were emotional neglect, family violence, emotional abuse, and physical abuse. Most students did not report depression or clinical anxiety symptoms. Two probabilistic models were obtained; 1) physical abuse increased the probability of depression (odds ratio, OR 14.1; 95% confidence interval, CI 1.7, 113.9), and resilience in competence (OR .914; 95% CI .851, .981) and acceptance (OR .776; 95% CI .668, .902) dimensions decreased the likelihood of depression; and 2) resilience in acceptance (OR .774; 95% CI .630, .855) dimension decreased the probability of clinical anxiety. ACEs are highly prevalent among psychology students. Physical abuse increases the probability of depression, whereas resilience decreases the probability of depression and/or anxiety symptoms. Impact of resilience on the negative outcomes of ACEs should be evaluated in future.

Abstract:Background Adverse childhood experiences (ACEs) represent a key risk factor for suicidal thoughts and behaviors among adolescents. However, the intervening mechanisms linking ACEs and suicidality, and whether such processes vary by gender, remain underexplored. Objective The present study examines whether the relationships between ACEs and the likelihood of experiencing suicidal thoughts and attempting suicide are indirect through depressive symptoms and low self-control. This study also investigates whether these direct and indirect paths might be moderated by gender. Participants and setting The analyses make use of data on a statewide representative sample of students enrolled in public high schools in Florida ( N = 23,078) from the 2022 Florida Youth Substance Abuse Survey (FYSAS). Methods Generalized structural equation modeling (GSEM) was used to estimate the direct and indirect effects of ACEs on the likelihood of suicidal thoughts and attempting suicide. The indirect effects were assessed using bootstrapping, and between-gender differences in the coefficients were tested. Results ACEs has direct associations with depressive symptoms, low self-control, and both suicidality outcomes. The effects of ACEs on suicidal thoughts are indirect through both depressive symptoms and low self-control, and the effects of ACEs on suicide attempts are indirect through depressive symptoms. Limited gender differences in these pathways emerge. Conclusions For male and female youth, ACEs are associated with heightened depression symptoms and reduced self-control, and both of these factors partially explain the previously established relationship between ACEs and suicidality.

Abstract:Suicide in youth exacts significant personal and community costs. Thus, it is important to understand predisposing risk factors. Experiencing adverse childhood experiences (ACEs), such as child maltreatment (CM-ACE), and the presence of post-traumatic stress disorder has been identified as a risk factor of suicidal behaviors among adults. Theoretical models of suicide suggest that the presence of painful experiences such as CM-ACEs increase the risk of suicidal behaviors. The relation between child maltreatment, post-traumatic stress symptom clusters (PTSS) and suicidal behaviors has not been explicitly examined among youth. The present study examined the relations between CM-ACEs, PTSS clusters, and suicidal behaviors in a clinical population of children. Children, male, ages 6 to 14, enrolled in a residential treatment program completed self-report measures to evaluate variables of interest. Path analyses revealed statistically significant direct effects of CM-ACEs and PTSS clusters on suicidal behaviors. Significant total indirect effects and marginally significant individual indirect effects of intrusion and avoidance symptoms were observed for the relation between CM-ACEs and suicidal behavior. Findings suggest that symptoms associated with specific PTSS clusters might help explain the relation between CM-ACEs and suicidal behavior, and therefore, present important implications for clinical practice and future research.

Abstract:In Mexico, suicide has become an important public health problem, representing the third leading cause of death in the adolescent population. Suicidal behavior in adolescents is associated with the interaction of complex relationships between personal, interpersonal, and sociocultural factors. Through a quantitative, descriptive, and correlational cross-sectional study, the present study aimed to analyze the prevalence and risk factors associated with suicidal ideation among adolescents from different high schools of the high school system (SEMS) of the University of Guadalajara, in response to the psychosocial impact of the COVID-19 pandemic. A descriptive statistical analysis was carried out on the data obtained from the 3583 students surveyed, followed by a principal component analysis (PCA) to identify closely related social, emotional, and behavioral variables. The PCA yielded eight principal components, which together represent 75.42% of the variance across psychometric tests. A multiple linear regression analysis was used, with a regression value (R2) of 0.4811, indicating that the explanatory model can predict 48.1% of the variability in suicidal ideation, with a statistical significance level of 0.05. According to the studies conducted, 19% (688 students) showed indicators of high suicide risk and 26.8% (960 students) showed moderate risk. Depression, mental health, health-related quality of life, physical and psychological well-being, and mood and emotions are the most influential factors in suicidal ideation.

Abstract:Abstract Adverse life events have been shown to increase the chances of harmful thoughts, or behavior against oneself. This study aims to fill this gap by identifying how adverse life events (witnessing a friend’s or family member’s self-injury, eating behavior problems, concern about sexual orientation, physical or sexual abuse) experienced by adolescents and young adults are associated with different indicators of suicidality and non-suicidal self-injury behaviors, according to gender and age. Participants were 625 young people aged between 12 and 25 years (Mean = 15.91, SD = 2.44), of whom 61.7% were girls. Of total participants, 53.44% reported adverse life events. Physical or sexual abuse was more associated with suicidality and non-suicidal self-injury compared to being witness to a friend’s self-injury. Girls who experienced physical abuse had more suicidal ideation than boys, while boys who experienced sexual abuse had more suicidal ideation than girls. Young adults who had experienced sexual abuse and those who witnessed a friend’s self-injury reported more non-suicidal self-injury attempts than adolescents in the same situations. For eating problems, adolescents showed more non-suicidal self-injury attempts than young adults. The findings underline the importance of considering adverse life events in order to prevent suicidality and non-suicidal self-injury behavior.

Abstract:Background: Childhood adversity can have lasting negative effects on physical and mental health. This study contributes to the existing literature by describing the prevalence rates and mental health outcomes related to adverse childhood experiences (ACEs) among adolescents registered for mental health care. Methods: Participants in this cross-sectional study were youths (aged 12–18 years) who were referred to outpatient psychiatric departments in the Netherlands. Demographic information was collected from the medical records. The Child Trauma Screening Questionnaire (CTSQ) was used to examine the presence of ACEs and posttraumatic stress symptoms (PTSS). To assess mental health problems, we used the Dutch translation of the Youth Self Report. Descriptive statistics and frequencies were used to calculate prevalence rates across the various ACEs domains. ANOVA and chi-square tests were used to explore the relationship between ACEs and mental health. Results: Of the 1373 participants, 69.1% reported having experienced at least one ACE and 17.1% indicated exposure to four or more ACEs in their lives. Although there was substantial overlap among all ACE categories, the most frequently reported were bullying (49.2%), emotional abuse (17.8%), physical abuse (12.2%), and sexual abuse (10.1%). Female adolescents (72.7%) reported significantly more ACEs than their male counterparts (27.0%). Furthermore, a higher number of ACEs was associated with significantly more self-reported general mental health problems, an elevated prevalence of both mood and post-traumatic stress disorders, and a greater presence of two or more co-existing psychiatric diagnoses (comorbid psychiatric classification). Conclusions: This cross-sectional study on childhood adversity and its association with mental health showed that ACEs are highly prevalent in youth registered for mental health care. This study provides support for a graded and cumulative relationship between childhood adversity and mental health problems. Antecedentes: La adversidad infantil puede tener efectos negativos a largo plazo en la salud física y mental. Este estudio contribuye a la literatura existente mediante la descripción de las las tasas de prevalencia y los resultados de salud mental asociadas a experiencias infantiles adversas (ACEs por sus siglas en inglés) entre adolescentes registrados por atención en salud mental. Método: Los participantes de este estudio transversal fueron jóvenes (Edad 12–18 años), que fueron remitidos a departamentos psiquiátricos ambulatorios de los Países Bajos. La información demográfica fue recolectada desde su historial médico. El Cuestionario de Detección de Trauma Infantil (CTSQ por sus siglas en inglés) fue utilizado para evaluar la presencia de ACEs y de síntomas de estrés postraumático (PTSS, por sus siglas en inglés). Para evaluar los problemas de salud mental, utilizamos la traducción holandesa del Youth Self Report. Se utilizaron estadísticas descriptivas y de frecuencias para calcular las tasas de prevalencia entre los distintos dominios de ACEs. Se utilizaron test de ANOVA y Chi-cuadrado para explorar la relación entre ACEs y la salud mental. Resultados: De los 1.373 pacientes, 69,1% reportaron haber experimentado al menos una ACEs y un 17,1% señaló haber estado expuesto a cuatro o más ACEs en sus vidas. Aunque existió una sustancial sobreposición entre las categorías de ACEs, aquellas reportadas con mayor frecuencia fueron el bullying (49.2%), abuso emocional (17.8%), abuso físico (12.2%), y abuso sexual (10.1%). Las Adolescentes mujeres (72.7%) reportaron significativamente más ACEs que sus contrapartes masculinas (27.0%). Además, un mayor número de ACEs se asoció con un número significativamente mayor de problemas de salud mental auto informados, con una alta prevalencia de trastornos del ánimo y de trastorno de estrés postraumático, y con una mayor presencia de dos o más diagnósticos psiquiátricos coexistentes (clasificación psiquiátrica de comorbilidad). Conclusiones: Este estudio de corte transversal sobre la adversidad infantil y su asociación con la salud mental, demostró que las ACEs son altamente prevalentes en los jóvenes registrados para el cuidado de la salud mental. Este estudio respalda una relación gradual y acumulativa entre la adversidad infantil y los problemas de salud mental.

Predictive Factors for Suicidal Behaviors among University Students in the North-Central Region of Mexico

Abstract:Introduction. Suicide is a public health problem with multifactorial etiology affecting all age groups. In Mexico, the suicide rate was 6.5 suicides per 100,000 population in 2021. Objective. To determine the predictive factors for suicidal behaviors (SB) in college students in the north-central region of Mexico. Method. A descriptive, comparative, analytical study was conducted of 1,147 randomly selected college students. Sociodemographic data, alcohol consumption (AC), perceived stress (PS), social support (SS) and suicidal behavior (SB), suicidal ideation (SI), suicide attempt (SA) and suicide risk (SR) were measured. Data analysis in SPSS included descriptive and inferential statistics. Results. The mean age of study participants was 20.2 years, with a female predominance (82.2%). Prevalence rates for SB were 62% for SI, 14.9% for SA, and 18.3% for SR. In regard to predictive factors for SB, risky, harmful, or dependent AC increased SR 1.7-fold [1.071-2.926]. At the same time, not having SS increased the risk of SI, SA, and SR by a factor of 2.4 [1.843-3.246], 2.7 [1.890-4.123], and 3.6 respectively. Higher PS increased the risk of SI, SA, and SR by a factor of 5.6 [4.209-7.538], 3.1[2.103-4.663] and 6.4 [4.184-9.826] respectively. PS and lack of SS predict SB in both sexes and across all states. Discussion and Conclusion. The results show the importance of mental health care, in both epidemiologically risky situations and everyday life, together with the early identification, and timely, effective treatment of suicide risk factors in university students.

Identifying pathways from childhood adversity to suicidal thoughts and behaviors among sexual minority adults: An exploratory mediation analysis

Abstract:Background: The current study uses a nationally representative longitudinal dataset of sexual minority adults in the US to investigate the pathways from adverse childhood experiences (ACEs) to adulthood suicidal thoughts and behaviors. Methods: ACEs were measured at year one, potential mediators at year two, and suicidal thoughts and behaviors (suicidal ideation, intent, plan, and attempt) at year three. We conducted an exploratory mediation analysis to identify potential mediating factors linking ACEs to suicidal thoughts and behaviors. Ten candidate mediators were examined: social well-being, felt stigma, experiences of everyday discrimination, social support, psychological distress, alcohol and drug use, importance of sexual identity, community connection, and internalized homophobia. Results: Participants were 1518 adults who identified as lesbian or gay (n = 833; 55 %), bisexual (n = 493; 33 %), or with other sexual minority identities (n = 181; 12 %) and were on average 36.48 years (SD = 14.7) of age. Psychological distress served as a common mediator between ACEs and suicidal ideation, intent, plan, and attempt. Additionally, experiences of everyday discrimination emerged as a specific mediator leading to suicidal intent, whereas social support uniquely mediated the relation between ACEs and suicide plan. Limitations: Potential recall bias due to retrospective reporting of ACEs may be a limitation. Future studies should broaden the measurement scope of ACEs and implement intersectional methods. Conclusion: The current findings underscore the urgent need for targeted interventions that address the specific mental health needs of sexual minority individuals, particularly focusing on mitigating psychological distress, combating systemic discrimination, and enhancing social support.

Abstract:Background Rates of suicidality are high among young adults and minoritized groups are disproportionately affected. Despite evidence that adverse childhood experiences (ACEs) may increase suicide-related risk, the underlying mechanisms through which ACEs may impact suicidal ideation (SI) remain poorly understood, especially among marginalized young adults with serious mental illnesses (SMI) such as schizophrenia-spectrum, major depressive, and bipolar disorders. This study examines associations between ACEs and SI, and whether perceived stress, dispositional hope, and mental health hope may mediate the relationship. Methods The sample consisted of 114 young adults of color with SMI who participated in a larger randomized trial. Data were analyzed using a structural equation modeling (SEM) approach. Results Young adults with more ACEs reported higher perceived stress, which in turn, was associated with more SI. Higher perceived stress was associated with lower dispositional and mental health hope. Although dispositional hope was negatively associated with SI on a bivariate level, neither dispositional nor mental health hope was significantly associated with SI in the multivariate SEM. This suggests that over and above perceived stress, hope does not appear to be associated with SI. Limitations Key limitations include a cross-sectional design, a modest sample size, and an assessment of select ACEs. Conclusion Addressing stress-related processes may be one promising target for suicide prevention efforts in the context of ACEs. Policy interventions focused on the allocation of resources and changing environments that are stress and trauma-inducing are needed to reduce the occurrence of ACEs and their negative sequelae.
