Abstract:Epilepsy, a chronic neuropsychiatric brain disorder characterized with recurrent seizures, is closely associated with abnormal neural communications within the brain. Despite that the phase-amplitude coupling (PAC) has been suggested to offer a new way to observe neural interactions during epilepsy, however, few studies pay attention to alterations of the epileptic functional brain network based on PAC, especially on the β-γ PAC. Therefore, we use scalp electroencephalography (EEG) data of epileptic patients and the β-γ PAC modulation index (MI) to construct functional brain networks to examine variations of neural interactions during different epileptic phases. Statistically, the findings show that between-channel MI values in the post-ictal period significantly increase compared to that in the pre-ictal period, and the between-channel MI value has a close association with the information of phase and amplitude provided by the channels. Importantly, in both the phase-amplitude and amplitude-phase functional brain networks, the average node degree is remarkably higher in the post-ictal period than that in the pre-ictal period, whereas the characteristic path length in the ictal and post-ictal periods is significantly lower than that in the pre-ictal period. Besides, the average betweenness centrality in the post-ictal period is remarkably higher than that in the ictal period. Interestingly, the positive correlations between within-channel MI values and between-channel MI values can be observed during the pre-ictal, ictal and post-ictal periods. These findings suggest that the β-γ PAC-based functional brain network may provide a novel perspective to understanding alterations of neural interactions during the epileptic evolution, and may contribute to effectively controlling the spread of epileptic seizures.

Abstract:Background: Recent studies have demonstrated that changes in brain information processing during anesthetic-nduced loss of consciousness (LOC) might be influenced by phase-amplitude coupling (PAC) in electroencephalogram (EEG). However, most anesthesia research on PAC typically focuses on delta and alpha oscillations. Studies of spatial-frequency characteristics by PAC for EEG may yield additional insights into understanding the impaired information processing under anesthesia unconsciousness and provide potential improvements in anesthesia monitoring. Objective: Considering different frequency bands of EEG represent neural activities on different spatial scales, we hypothesized that functional coupling simultaneously appears in multiple frequency bands and specific brain regions during anesthesia unconsciousness. In this paper, PAC analysis on whole-brain EEG besides delta and alpha oscillations was investigated to understand the influence of multiple cross-frequency coordination coupling on information processing during the loss and recovery of consciousness. Method: EEG data from fifteen patients without cognitive diseases (7 males/8 females, aged 43.8 +/- 13.4 years, weighing 63.3 +/- 14.9 kilograms) undergoing lower limb surgery and sevoflurane anesthesia was recorded. To investigate the spatial-frequency characteristics of EEG source signals during loss and recovery of consciousness, the time-resolved PAC (tPAC) was calculated to reflect cross-frequency coordination in different frequency bands (delta, theta, alpha, beta, gamma) and different functional regions (Visual, Limbic, Dorsal attention, Ventral attention, Default, Somatomotor, Control, Salience networks). Furthermore, different patterns (peak-max and trough-max) of PAC were examined by constructing phase-amplitude histograms using phase bins to investigate the different information processing during LOC. The multivariate analysis of variance (MANOVA) and trend analysis were used for statistical analysis. Results: Theta-alpha and alpha-beta PAC were observed during sevoflurane-induced LOC, which significantly changed during loss and recovery of consciousness (E-4,E-70 = 16.553, p < 0.001 for theta-alpha PAC and F-4,F-70 = 12.446, p < 0.001 for alpha-beta PAC, MANOVA test). Simultaneously, PAC was distributed in specific functional regions, i.e., Visual, Limbic, Default, Somatomotor, etc. Furthermore, peak-max patterns of theta-alpha PAC were observed while alpha-beta PAC showed trough-max patterns and vice versa. Conclusion: Theta-alpha and alpha-beta PAC observed in specific brain regions represent information processing on multiple spatial scales, and the opposite patterns of PAC indicate opposite information processing on multiple spatial scales during LOC. Our study demonstrates the regulation of local-global information processing during sevoflurane-induced LOC. It suggests the utility of evaluating the balance of functional integration and segregation in monitoring anesthetized states.
Phase–Amplitude Coupling and Epileptogenic Zone Localization of Frontal Epilepsy Based on Intracranial EEG

Abstract:Objective: This study aimed to explore the characteristics of phase-amplitude coupling in patients with frontal epilepsy based on their electrocorticography data, in order to identify the localization of epileptic regions and further guide clinical resection surgery. Methods: We adopted the modulation index based on the Kullback-Leibler distance, phase-amplitude coupling co-modulogram, and time-varying phase-amplitude modulogram to explore the temporal-spatial patterns and characterization of PAC strength during the period from inter- seizure to post-seizure. Taking the resected area as the gold standard, the epileptogenic zone was located based on MI values of 7 different seizure periods, and the accuracy of localization was measured by the area under the receiver operating curve. Results: (1) The PAC in the inter- and pre-seizure periods was weak and paroxysmal, but strong PAC channels were confined more to the seizure-onset zone and resection region. PAC during the seizure period was intense and persistent, but gradually deviated from the seizure-onset zone. (2) The characteristics of coupling strength of the inter- and pre-seizure EEG can be used to accurately locate the epileptogenic zone, which is better than that in periods after the beginning of a seizure. (3) In an epileptic seizure, the preferred phases of coupling were usually in the rising branches at the pre- and early-seizure stages, while those in the middle- and terminal-seizure were usually in the falling branch. We thus speculate that the coupling occurred in the rising branch can promote the recruitment of abnormal discharge, while the coupling occurred in the falling branch can inhibit the abnormal discharge. Conclusion: The findings suggest that the phase-amplitude coupling during inter- and pre-seizure is a promising marker of epileptic focus location. The preferred phase of coupling changed regularly with the time of epileptic seizure, suggesting that the surge and suppression of abnormal discharges are related to different phases.

Abstract:Abstract Objective. Diagnosing epilepsy still requires visual interpretation of electroencephalography (EEG) and magnetoencephalography (MEG) by specialists, which prevents quantification and standardization of diagnosis. Previous studies proposed automated diagnosis by combining various features from EEG and MEG, such as relative power (Power) and functional connectivity (FC). However, the usefulness of interictal phase–amplitude coupling (PAC) in diagnosing epilepsy is still unknown. We hypothesized that resting-state PAC would be different for patients with epilepsy in the interictal state and for healthy participants such that it would improve discrimination between the groups. Approach. We obtained resting-state MEG and magnetic resonance imaging (MRI) in 90 patients with epilepsy during their preoperative evaluation and in 90 healthy participants. We used the cortical currents estimated from MEG and MRI to calculate Power in the δ (1–3 Hz), θ (4–7 Hz), α (8–13 Hz), β (13–30 Hz), low γ (35–55 Hz), and high γ (65–90 Hz) bands and FC in the θ band. PAC was evaluated using the synchronization index (SI) for eight frequency band pairs: the phases of δ, θ, α , and β and the amplitudes of low and high γ . First, we compared the mean SI values for the patients with epilepsy and the healthy participants. Then, using features such as PAC, Power, FC, and features extracted by deep learning (DL) individually or combined, we tested whether PAC improves discrimination accuracy for the two groups. Main results. The mean SI values were significantly different for the patients with epilepsy and the healthy participants. The SI value difference was highest for θ /low γ in the temporal lobe. Discrimination accuracy was the highest, at 90%, using the combination of PAC and DL. Significance. Abnormal PAC characterized the patients with epilepsy in the interictal state compared with the healthy participants, potentially improving the discrimination of epilepsy.

Abstract:Amplitude and functional connectivity are two fundamental parameters for describing the spontaneous brain fluctuations. These two parameters present close coupling in physiological state, and present different alteration patterns in epilepsy revealed by functional MRI (fMRI). We hypothesized that the alteration of coupling between these two imaging parameters may be underpinned by specific pathological factors of epilepsy, and can be employed to improve the capability for epileptic focus detection. Forty-seven patients (26 left- and 21 right-sided) with mesial temporal lobe epilepsy (mTLE) and 32 healthy controls underwent resting-state fMRI scans. All patients were detected to have interictal epileptic discharges on simultaneous electroencephalograph (EEG) recordings. Amplitude-connectivity coupling was calculated by correlating amplitude and functional connectivity density of low-frequency brain fluctuations. We observed reduced amplitude-connectivity coupling associated with epileptic discharges in the mesial temporal regions in both groups of patients, and increased coupling associated with epilepsy durations in the posterior regions of the default-mode network in the right-sided patients. Moreover, we proposed a new index of amplitude subtracting connectivity, which elevated imaging contrast for differentiating the patients from the controls. The findings indicated that epileptic discharges and chronic damaging effect of epilepsy might both contribute to alterations of amplitude-connectivity coupling in different pivotal regions in mTLE. Abstract:Epilepsy is a neurological disorder in the brain that is characterized by unprovoked seizures. Epileptic seizures are attributed to abnormal synchronous neuronal activity in the brain. To detect the seizure as early as possible, the identification of specific electroencephalogram (EEG) dynamics is of great importance in investigating the transition of brain activity as the epileptic seizure approaches. In this study, we investigated the transition of brain activity from interictal to preictal states preceding a seizure by combining EEG network and clustering analyses together in different frequency bands. The findings of this study demonstrated the best clustering performance of k-medoids in the beta band; in addition, compared to the interictal state, the preictal state experienced increased synchronization of EEG network connectivity, characterized by relatively higher network properties. These findings can provide helpful insight into the mechanism of epilepsy, which can also be used in the prediction of epileptic seizures and subsequent intervention.
Temporal and spatial dynamic propagation of electroencephalogram by combining power spectral and synchronization in childhood absence epilepsy

Abstract:Objective: During the transition from normal to seizure and then to termination, electroencephalography (EEG) signals have complex changes in time-frequency-spatial characteristics. The quantitative analysis of EEG characteristics and the exploration of their dynamic propagation in this paper would help to provide new biomarkers for distinguishing between pre-ictal and inter-ictal states and to better understand the seizure mechanisms. Methods: Thirty-three children with absence epilepsy were investigated with EEG signals. Power spectral and synchronization were combined to provide the time-frequency-spatial characteristics of EEG and analyze the spatial distribution and propagation of EEG in the brain with topographic maps. To understand the mechanism of spatial-temporal evolution, we compared inter-ictal, pre-ictal, and ictal states in EEG power spectral and synchronization network and its rhythms in each frequency band. Results: Power, frequency, and spatial synchronization are all enhanced during the absence seizures to jointly dominate the epilepsy process. We confirmed that a rapid diffusion at the onset accompanied by the frontal region predominance exists. The EEG power rapidly bursts in 2-4 Hz through the whole brain within a few seconds after the onset. This spatiotemporal evolution is associated with spatial diffusion and brain regions interaction, with a similar pattern, increasing first and then decreasing, in both the diffusion of the EEG power and the connectivity of the brain network during the childhood absence epilepsy (CAE) seizures. Compared with the inter-ictal group, we observed increases in power of delta and theta rhythms in the pre-ictal group (P < 0.05). Meanwhile, the synchronization of delta rhythm decreased while that of alpha rhythm enhanced. Conclusion: The initiation and propagation of CAE seizures are related to the abnormal discharge diffusion and the synchronization network. During the seizures, brain activity is completely changed with the main component delta rhythm. Furthermore, this article demonstrated for the first time that alpha inhibition, which is consistent with the brain's feedback regulation mechanism, is caused by the enhancement of the network connection. Temporal and spatial evolution of EEG is of great significance for the transmission mechanism, clinical diagnosis and automatic detection of absence epilepsy seizures.
Usefulness of low amplitude spikes with continuous focal periodic discharges on interictal scalp electroencephalogram and their patterns of cortical electroencephalogram

Abstract:Objective To reveal the influential factors on scalp electroencephalogram (EEG)recording and provide valuable information for localization of the epileptic focus by analyzing the characteristics of spikes with continuous focal periodic discharges on scalp and cortical EEG. Methods Five patients with refractory epilepsy who had low amplitude spikes with continuous focal periodic discharges on interictal scalp EEG were studied. Intracranial EEG recording was also performed in patients. The amplitudes of spikes and cortical areas of spike-wave foci were measured by DaVinci system. Patterns of continuous periodic activity were determined by autocorrelograms,power spectral density and coherence analysis using Matlab and Spike2 software.T-test was employed to compare the mean amplitudes of spikes on the scalp and cortical EEG.Results The amplitudes of spikes recorded on scalp EEG of the 5 patients were:(22.2±4.8),(30.4±7.1),(20.7±3.2),(58.4±10.1),(23.4±3.9) μV.The amplitudes of spikes recorded on cortical EEG of the 5 patients were:(1253.8 ± 199.3),(806.5 ± 161.4),( 1585.7 ±305.7),(922.5 ± 140.6),(736.8 ±70.9) μV.The amplitudes of spikes on scalp EEG were significantly higher than those on cortical EEG ( t =6.394,P ＜ 0.05 ).The cortical areas of spike-wave foci of the 5 patients were:4.0,6.0,3.5,5.5,6.5 cm2.Power spectral density and autocorrelugrams showed 1-3 Hz oscillations on the cortical of spike-wave foci. Cross-correlation and coherence analysis showed synchronization of electrical activity in two contacts of intracranial electrodes. Conclusion The low amplitude spikes with continuous focal periodic discharges on interictal scalp EEG provide valuable information for localization of the epileptic focus.
Pro-ictal, rather than pre-ictal, brain state marked by global critical slowing and local gamma power increase

Abstract:The clinical workup during the pre-surgical evaluation for epilepsy relies on the electrophysiological recording of spontaneous seizures. The interval until first seizure occurrence is characterized by an increase in seizure likelihood caused by progressive drug dose decreases, during which the epileptic brain transitions from a state of low to a state of high seizure likelihood, so-called pro-ictal state. This study aimed to identify the dynamic brain changes characteristic of this transition from 386 ten-minute segments of intracranial EEG recordings of 29 patients with drug-refractory temporal lobe epilepsy, explored by stereoelectroencephalography, irregularly sampled between electrode implantation and first seizure. As measures of brain dynamics we studied mean phase coherence and relative power in the gamma frequency band, and autocorrelation function width. We further investigate the interaction of those brain dynamics with various susceptibility factors, such as the rate of interictal spikes and high frequency oscillations, circadian and multi-day cycles, and clinical outcomes. We observed a significant increase in relative gamma power in the epileptogenic zone, and an increase in critical slowing in both the epileptogenic zone as well as in presumably healthy cortex. These brain dynamic changes were linked with increases in spike and high frequency oscillations rate. While brain dynamic changes occurred on the slow time scale - from the beginning to the end of the multi-day interval - they did not change in the short-term during the pre-ictal interval. We thus highlight gamma power and critical slowing indices as markers of pro-ictal (as opposed to pre-ictal) brain states, as well as their potential to track the seizure-related brain mechanisms during the presurgical evaluation of epilepsy patients.

Abstract:Epilepsy is a common neurological disorder characterized by hyper-synchronous abnormalities of neurons. Resting state brain activity measured by fMRI might evaluate the synchronization of the disorder. To investigate the alteration of the haemodynamic synchronization in epilepsy, resting-state fMRI (RS-fMRI) was performed on 25 patients with primarily generalized tonic-clonic seizures (GTCS), along with 25 age- and sex-matched healthy subjects. Regional homogeneity (ReHo), a measurement of the synchronization of spontaneous RS-fMRI signal oscillations within spatially neighboring voxels, was examined. Compared with the healthy controls, the patients with GTCS showed bilaterally and symmetrically altered ReHo in the cortical and subcortical structures. In addition, a correlation analysis of the ReHo measurement versus the epilepsy duration was performed, and highly negative correlations were observed in thalamus, insula and the regions followed the pattern of 'default' state of brain function. The current findings demonstrate that altered regional synchronization of brain activity exists in the patients with GTCS during interictal period, and there is potential in utilizing the ReHo method in RS-fMRI analyses of epilepsy.

Altered Functional-Structural Coupling of Large-Scale Brain Networks in Idiopathic Generalized Epilepsy

Abstract:The human brain is a large-scale integrated network in the functional and structural domain. Graph theoretical analysis provides a novel framework for analysing such complex networks. While previous neuroimaging studies have uncovered abnormalities in several specific brain networks in patients with idiopathic generalized epilepsy characterized by tonic-clonic seizures, little is known about changes in whole-brain functional and structural connectivity networks. Regarding functional and structural connectivity, networks are intimately related and share common small-world topological features. We predict that patients with idiopathic generalized epilepsy would exhibit a decoupling between functional and structural networks. In this study, 26 patients with idiopathic generalized epilepsy characterized by tonic-clonic seizures and 26 age- and sex-matched healthy controls were recruited. Resting-state functional magnetic resonance imaging signal correlations and diffusion tensor image tractography were used to generate functional and structural connectivity networks. Graph theoretical analysis revealed that the patients lost optimal topological organization in both functional and structural connectivity networks. Moreover, the patients showed significant increases in nodal topological characteristics in several cortical and subcortical regions, including mesial frontal cortex, putamen, thalamus and amygdala relative to controls, supporting the hypothesis that regions playing important roles in the pathogenesis of epilepsy may display abnormal hub properties in network analysis. Relative to controls, patients showed further decreases in nodal topological characteristics in areas of the default mode network, such as the posterior cingulate gyrus and inferior temporal gyrus. Most importantly, the degree of coupling between functional and structural connectivity networks was decreased, and exhibited a negative correlation with epilepsy duration in patients. Our findings suggest that the decoupling of functional and structural connectivity may reflect the progress of long-term impairment in idiopathic generalized epilepsy, and may be used as a potential biomarker to detect subtle brain abnormalities in epilepsy. Overall, our results demonstrate for the first time that idiopathic generalized epilepsy is reflected in a disrupted topological organization in large-scale brain functional and structural networks, thus providing valuable information for better understanding the pathophysiological mechanisms of generalized tonic-clonic seizures.

Abstract:Phase-amplitude coupling (PAC) describes the interaction of two separate frequencies in which the lower frequency phase acts as a carrier frequency of the higher frequency amplitude. It is a means of carrying integrated streams of information between micro- and macroscale systems in the brain, allowing for coordinated activity of separate brain regions. A beta–gamma PAC increase over the sensorimotor cortex has been observed consistently in people with Parkinson's disease (PD). Its cause is attributed to neural entrainment in the basal ganglia, caused by pathological degeneration characteristic of PD. Disruptions in this phenomenon in PD patients have been observed in the resting state as well as during movement recordings and have reliably distinguished patients from healthy participants. The changes can be detected non-invasively with the electroencephalogram (EEG). They correspond to the severity of the motor symptoms and the medication status of people with PD. Furthermore, a medication-induced decrease in PAC in PD correlates with the alleviation of motor symptoms measured by assessment scales. A beta–gamma PAC increase has, therefore, been explored as a possible means of quantifying motor pathology in PD. The application of this parameter to closed-loop deep brain stimulation could serve as a self-adaptation measure of such treatment, responding to fluctuations of motor symptom severity in PD. Furthermore, phase-dependent stimulation provides a new precise method for modulating PAC increases in the cortex. This review offers a comprehensive synthesis of the current EEG-based evidence on PAC fluctuations in PD, explores the potential practical utility of this biomarker, and provides recommendations for future research.

Abstract:Brain neuron activity is closely related to cerebral blood flow (CBF) changes. Alterations in the regional homogeneity (ReHo) and CBF occur in patients with magnetic resonance imaging negative focal epilepsy (FEP-MRI<sup>-</sup>). However, the coupling alterations of ReHo and CBF in FEP-MRI<sup>-</sup> remain unclear. The study aims to explore neurovascular coupling alterations and their clinical implication in FEP-MRI<sup>-</sup>. We collected resting-state magnetic resonance imaging (MRI) data from 31 healthy controls (HCs) and 48 patients with FEP-MRI<sup>-</sup>,including three-dimensional (3D) T1-weighted imaging, 3D arterial spin labeling (ASL) imaging,and resting-state functional MRI (rs-fMRI). The CBF and ReHo values were calculated from the ASL and rs-fMRI data, respectively. The CBF/ReHo ratio per voxel and whole-brain CBF-ReHo coupling were compared between the two groups. Correlation analysis involved the CBF/ReHo ratio and clinical indicators in FEP-MRI<sup>-</sup>. Patients with FEP-MRI<sup>-</sup> showed significantly increased cross-subject CBF-ReHo and global cross-voxel CBF-ReHo coupling. The CBF/ReHo ratio was higher in the bilateral orbitofrontal gyrus, right parietal lobe, and right middle frontal gyrus of patients with FEP-MRI<sup>-</sup>. Nevertheless, this ratio was lower in the bilateral supplementary motor areas, the left middle and posterior cingulate gyrus, and the right central sulcus cover. The CBF/ReHo ratio was markedly correlated with cognitive function, memory, intelligence, and epilepsy duration in the above abnormal brain regions. CBF/ReHo ratio may be useful as an indicator of neuropathological mechanisms. These results support the hypothesis that CBF/ReHo ratio relates to the neuropathological mechanisms of FEP-MRI<sup>-</sup>. Furthermore, it offers new perspectives for studying the mechanisms of MRI-negative epilepsy.

Abstract:A thinner cortex has higher potential for binding GABA receptor A which is associated with larger amplitudes of intrinsic brain activity (iBA). However, the relationship between cortical thickness and iBA is unknown in intact and epileptic brains. To this end, we investigated the relationship between cortical thickness measured by high-resolution MRI and surface-based iBA derived from resting-state functional MRI in normal controls (n = 82) and patients with generalized tonic–clonic seizures (GTCS) only (n = 82). We demonstrated that the spatial distribution of cortical thickness negatively correlated with surface-based iBA amplitude at both whole-brain and within independent brain functional networks. In GTCS patients, spatial coupling between thickness and iBA amplitude decreased in the default mode, dorsal attention, and somatomotor networks. In addition, the vertex-wise across-subject thickness–iBA amplitude correspondence altered in the frontal and temporal lobes as well as in the precuneus in GTCS patients. The relationship between these two modalities can serve as a brain-based marker for detecting epileptogenic changes.

Abstract:Objective Electroencephalography (EEG) microstate analysis seeks to cluster the scalp's electric field into semistable topographical EEG activity maps at different time points. Our study aimed to investigate the features of EEG microstates in subjects with focal epilepsy and psychogenic nonepileptic seizures (PNES). Methods We included 62 adult subjects with focal epilepsy or PNES who received video‐EEG monitoring at the epilepsy monitoring unit. The subjects (mean age = 42.8 ± 21.2 years) were distributed equally between epilepsy and PNES groups. We extracted microstates from a 4.4 ± 1.0‐min, 21‐channel resting‐state EEG. We excluded subjects with interictal epileptiform discharges during resting‐state EEGs. After preprocessing, we derived five main EEG microstates—MS1 to MS5—for the full frequency band (1–30 Hz) and frequency subbands (delta, 1–4 Hz; theta, 4–8 Hz; alpha, 8–12 Hz; beta, 12–30 Hz), using the MATLAB‐based EEGLAB toolkit. Statistical features of microstates (duration, occurrence, contribution, global field power [GFP]) were compared between the groups, using logistic regression corrected for age and sex. Results We detected no differences in microstate parameters in the full frequency band. We found a longer duration (delta: B = −7.680, p = .046; theta: B = −16.200, p = .043) and a higher contribution (delta: B = −7.414, p = .035; theta: B = −7.509, p = .031) of MS4 in lower frequency bands in the epilepsy group. The PNES group showed a higher occurrence of MS5 in the delta subband (B = 3.283, p = .032). In the theta subband, a higher GFP of MS1 was associated with the PNES group (B = 5.674, p = .025), whereas a higher GFP of MS2 was associated with the epilepsy group (B = −6.579, p = .026). Significance Microstate features show differences between patients with focal epilepsy and PNES. EEG microstates could be a promising parameter, helping to understand changes in brain dynamics in subjects with epilepsy, and should be explored as a potential biomarker.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>Altered functional activities and hypometabolism have been found in medial temporal lobe epilepsy patients with hippocampal sclerosis (mTLE‐HS). Hybrid PET/MR scanners provide opportunities to explore the relationship between resting‐state energy consumption and functional activities, but whether repeated seizures disturb the bioenergetic coupling and its relationship with seizure outcomes remain unknown.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p><sup>18</sup>F‐FDG PET and resting‐state functional MRI (rs‐fMRI) scans were performed with hybrid PET/MR in 26 patients with mTLE‐HS and in healthy controls. Energy consumption was quantified by <sup>18</sup>F‐FDG standardized uptake value ratio(SUVR) relative to cerebellum. Spontaneous neural activities were estimated using regional homogeneity (ReHo), fractional amplitude of low frequency fluctuations (fALFF) from rs‐fMRI. Between‐group differences in SUVR and rs‐fMRI derived metrics were evaluated by two‐sample t test. Voxel‐wise spatial correlations were explored between SUVR and ReHo, fALFF across gray matter and compared between groups. Furthermore, the relationships between altered fALFF/SUVR and ReHo/SUVR coupling and surgical outcomes were evaluated. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Both the patients and healthy controls showed significant positive correlations between SUVR and rs‐fMRI metrics. Spatial correlations between SUVR and fMRI‐derived metrics across gray matter were significantly higher in patients with mTLE‐HS compared with healthy controls (fALFF/SUVR, <i>P</i> < 0.001; ReHo/SUVR, <i>P</i> = 0.022). Higher fALFF/SUVR couplings were found in patients who had Engel class IA after surgery than all other (<i>P</i> = 0.025), while altered ReHo/SUVR couplings (<i>P</i> = 0.097) were not. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>These findings demonstrated altered bioenergetic coupling across gray matter and its relationship with seizure outcomes, which may provide novel insights into pathogenesis of mTLE‐HS and potential biomarkers for epilepsy surgery planning.</p></section>

Abstract:Introduction. Epilepsy is a disease characterized by abnormal paroxysmal bioelectrical activity in the brain cortex and subcortical structures. Seizures per se change brain metabolism in epileptic focus and in distal parts of the brain. However, interictal phenomena can also affect functional connectivity (FC) and brain metabolism in other parts of the brain. Aim of study. We hypothesised that epilepsy affects functional connectivity not only among cortical, but also between subcortical, structures of the brain in a resting state condition. Clinical rationale for study. Investigating functional connectivity in patients with epilepsy could provide insights into the underlying pathophysiological mechanisms. Better understanding may lead to more effective treatment strategies. Material and methods. Functional connectivity was analysed in 35 patients with epilepsy and in 28 healthy volunteers. The group of patients was divided into generalised and focal epilepsy (temporal and extratemporal subgroups). Each patient and healthy volunteer underwent an fMRI resting-state session. During the study, EEG signals were simultaneously recorded with fMRI to facilitate the subsequent detection of potential interictal epileptiform discharges (IEDs). Their potential impact on BOLD signals was mitigated through linear regression. The data was processed and correlation coefficients (FC values) between the BOLD signal from selected structures of the central nervous system were determined and compared between study groups. The results were presented as significant differences in correlation coefficients between brain/subcortical structures in the epilepsy and control groups. Results. Lower FC values for the epilepsy group compared to the control group were shown for connections related to the MPFC, hippocampus, thalamus, amygdala, and the parahippocampal gyrus. Conclusions. Epilepsy alters the functional connectivity of resting state subcortical networks. Patterns of pathological changes of FC differ between epilepsy subtypes, with predominant lower FC between the hippocampus, parahippocampal gyrus, amygdala and thalamus in patients with epilepsy. Clinical implications. This study suggests that epilepsy affects subcortical structures. Identifying distinct patterns of altered FC in epilepsy subtypes may help to tailor treatment strategies. Changes in FC detected by fMRI may precede clinical symptoms, aiding in the early diagnosis of cognitive and emotional disorders in focal epilepsy.

Abstract:Objective We explored whether quantifiable differences between clinical seizures (CSs) and subclinical seizures (SCSs) occur in the pre-ictal state. Methods We analyzed pre-ictal stereo-electroencephalography (SEEG) retrospectively across mesial temporal lobe epilepsy patients with recorded CSs and SCSs. Power spectral density and functional connectivity (FC) were quantified within and between the seizure onset zone (SOZ) and the early propagation zone (PZ), respectively. To evaluate the fluctuation of neural connectivity, FC variability was computed. Measures were further verified by a logistic regression model to evaluate their classification potentiality through the area under the receiver-operating-characteristics curve (AUC). Results Fifty-four pre-ictal SEEG epochs (27 CSs and 27 SCSs) were selected among 14 patients. Within the SOZ, pre-ictal FC variability of CSs was larger than SCSs in 1–45 Hz during 30 seconds before seizure onset. Pre-ictal FC variability between the SOZ and PZ was larger in SCSs than CSs in 55–80 Hz within 1 minute before onset. Using these two variables, the logistic regression model achieved an AUC of 0.79 when classifying CSs and SCSs. Conclusions Pre-ictal FC variability within/between epileptic zones, not signal power or FC value, distinguished SCSs from CSs. Significance Pre-ictal epileptic network stability possibly marks seizure phenotypes, contributing insights into ictogenesis and potentially helping seizure prediction.

Abstract:Abstract Intracranial studies provide solid evidence that high-frequency brain signals are a new biomarker for epilepsy. Unfortunately, epileptic (pathological) high-frequency signals can be intermingled with physiological high-frequency signals making these signals difficult to differentiate. Recent success in non-invasive detection of high-frequency brain signals opens a new avenue for distinguishing pathological from physiological high-frequency signals. The objective of the present study is to characterize pathological and physiological high-frequency signals at source levels by using kurtosis and skewness analyses. Twenty-three children with medically intractable epilepsy and age-/gender-matched healthy controls were studied using magnetoencephalography. Magnetoencephalographic data in three frequency bands, which included 2–80 Hz (the conventional low-frequency signals), 80–250 Hz (ripples) and 250–600 Hz (fast ripples), were analysed. The kurtosis and skewness of virtual electrode signals in eight brain regions, which included left/right frontal, temporal, parietal and occipital cortices, were calculated and analysed. Differences between epilepsy and controls were quantitatively compared for each cerebral lobe in each frequency band in terms of kurtosis and skewness measurements. Virtual electrode signals from clinical epileptogenic zones and brain areas outside of the epileptogenic zones were also compared with kurtosis and skewness analyses. Compared to controls, patients with epilepsy showed significant elevation in kurtosis and skewness of virtual electrode signals. The spatial and frequency patterns of the kurtosis and skewness of virtual electrode signals among the eight cerebral lobes in three frequency bands were also significantly different from that of the controls (2–80 Hz, P < 0.001; 80–250 Hz, P < 0.00001; 250–600 Hz, P < 0.0001). Compared to signals from non-epileptogenic zones, virtual electrode signals from epileptogenic zones showed significantly altered kurtosis and skewness (P < 0.001). Compared to normative data from the control group, aberrant virtual electrode signals were, for each patient, more pronounced in the epileptogenic lobes than in other lobes(kurtosis analysis of virtual electrode signals in 250–600 Hz; odds ratio = 27.9; P < 0.0001). The kurtosis values of virtual electrode signals in 80–250 and 250–600 Hz showed the highest sensitivity (88.23%) and specificity (89.09%) for revealing epileptogenic lobe, respectively. The combination of virtual electrode and kurtosis/skewness measurements provides a new quantitative approach to distinguishing pathological from physiological high-frequency signals for paediatric epilepsy. Non-invasive identification of pathological high-frequency signals may provide novel important information to guide clinical invasive recordings and direct surgical treatment of epilepsy.
