Abstract:Macrophage activation syndrome (MAS) and widespread brain lesions are rare and severe complications of childhood-onset systemic lupus erythematosus (SLE). We report an 11-year-old girl who presented with recurrent rashes for half a year and fever for 2 weeks. Clinical and laboratory features at admission pointed to the diagnosis of SLE and SLE-associated MAS. Cerebral magnetic resonance imaging taken on day 4 after admission showed abnormal signals. Glucocorticoid therapy was started on day 5. Two days later, the patient appeared weak and ill, then the next day she exhibited dizziness, drowsiness, apathia, and dysarthria. High-dose methylprednisolone, cyclophosphamide, and intravenous immunoglobulin were used to treat the patient, and intrathecal dexamethasone was given. The patient was discharged on day 30 after admission and showed complete clinical resolution and improved magnetic resonance imaging resolution.

Abstract:BackgroundMaffucci syndrome is characterized by the sporadic occurrence of multiple enchondromas together with multiple hemangiomas. Patients with Maffucci syndrome are at increased risk of developing different kinds of malignant tumors.Case presentationWe report on a 39-year-old woman who was diagnosed with Maffucci syndrome together with intrahepatic cholangiocarcinoma (IHCC). Heterozygous somatic mutations in the isocitrate dehydrogenase 1 and 2 (IDH1/IDH2) genes are associated with a number of different tumor types (e.g. IHCC) and also with Maffucci syndrome. For IHCC, mutations in IDH1/IDH2 are associated with higher survival rates. IHCC tissue as well as normal liver tissue and peripheral blood were analyzed for IDH1/IDH2-mutations in our patient. In the tumor sample, we identified a recurrent somatic IDH1-mutation affecting Arg132, while in normal liver tissue and peripheral blood, no variants were detected, as expected.ConclusionThis case report presents the second patient in the literature exhibiting the features of Maffucci syndrome along with cholangiocarcinoma. This supports the hypothesis that IDH1/2-mutations, which can be present in different types of tumor tissue simultaneously, arise during embryonic development in a mosaic pattern; as a result, a more aggressive follow-up is proposed in patients with Maffucci syndrome to exclude neoplasms.

Abstract:Objective To analyze the clinical characteristics and diagnosis of multisystemic smooth muscle dysfunction syndrome (MSMDS).Method Clinical data of a case diagnosed as MSMDS and hospitalized in our hospital in July 2016 was retrospectively analyzed.Literature search was performed at databases of PubMed,Wanfang,China National Knowledge Infrastructure and VIP with the key words "multisystemic smooth muscle dysfunction syndrome ACTA2".The literature retrieval was confined from January 1980 to November 2016.The characteristics of MSMDS were summarized through review of literature.Result A girl aged 1.6 years had recurrent cough and wheeze for more than 1 year,complicated with congenital fixed dilated pupils,patent ductus arteriosus,pulmonary hypertension,chronic lung disease,and cerebrovascular abnormalities.We had done gene analysis for the patient and found ACTA2 c.536C ＞ T (p.R179H) heterozygous mutations,but her parents were normal.Totally 11 reports were retrieved from foreign language literature and no report from Chinese literature could be found;the retrieved articles reported a total of 25 cases of multiple system smooth muscle dysfunction syndrome.The minimum age was 11 months,17 cases were female,8 were male.The clinical common characteristic is congenital fixed dilated pupils,patent ductus arteriosus,cerebrovascular disease,pulmonary hypertension,chronic lung disease,and so on.Conclusion Genetic testing for ACTA2 gene mutations should be considered in infants presenting with congenital fixed dilated pupils and patent ductus arteriosus.
[Münchausen syndrome by proxy in a forensic psychiatric evaluation--the description of a case and ethical controversy]

Abstract:Aim: Presentation of the difficulties in diagnosing Münchausen syndrome by proxy. Method: Comparison of four different conclusions in forensic psychiatric opinions issued in one case, due to the need of answering to the questions of the court, in what periods and why was the deterioration of the child's health, in connection with an allegation that the mother has acted to his detriment. In the first medical-legal opinion the treating physician, while being the head of the ward and the person informing the police of the action against a child by the mother, recognised Münchausen syndrome by proxy. The second opinion was delivered by a psychologist, who stated that the mother distinguished the introspective attitude characterised by excessive, exaggerated accuracy, thoroughness, and did not give a unambiguous answer to the questions of the court. In the third opinion the experts accepted that the functioning of the subject does not create a threat to life and health of the child. The fourth opinion developed by the authors of this paper noted that in the analysed medical records of the childs' hospitalisation, no arguments were found to recognise Münchausen syndrome by proxy. RESULTS. In the conducted extended ambulant study of the subject and the analysis of the medical documentation of the 31 hospitalisations of the child showed that the improvement of the child's health was not associated in exclusion of pushing aside the mother from the child, but was the result of the consistency of the therapeutic treatment team, the continuation and the modification of the treatment. Conclusions: In appointing the expert, the art. 196 of the Code of Criminal Procedure should be considered, which states that the expert should not be the treating doctor, because his opinion loses value impartiality. Diagnosis of Münchausen Syndrome by proxy itself stirs up numerous controversies..

Abstract:Munchausen's syndrome by proxy (MSBP) is a rare form of abuse characterized by the fabrication or induction of symptoms of illness in a child by a close relative, typically a parent, leading to multiple consultations and varying degrees of invasive medical interventions. Various clinical presentations are described in the literature, ranging from organic manifestations to psychiatric expressions. This syndrome remains a challenging diagnosis to make and requires increased awareness among healthcare professionals. Prompt recognition is key to preventing potential long-term comorbidities and even fatalities. Here, we are reporting two cases of MSBP manifested by bleeding, with the perpetrator being the mother.

Abstract:Macrophage activation syndrome (MAS), is a severe and fatal complication of various pediatric inflammatory disorders. Kabuki syndrome (KS), mainly caused by lysine methyltransferase 2D (KMT2D; OMIM 602113) variants, is a rare congenital disorder with multi-organ deficiencies. To date, there have been no reported cases of MAS in patients with KS. This report describes a case of a 22-year-old male with Kabuki syndrome (KS) who developed MAS. This unique case not only deepens the understanding of the involvement of KMT2D in immune regulation and disease, but expands the phenotype of the adult patient to better understand the natural history, disease burden, and management of patients with KS complicated with autoimmune disorders.

The Etiology and Outcome of Area Postrema Syndrome in Childhood: Two Cases and a Literature Review

Abstract:Background: Area postrema syndrome (APS), a rare childhood condition, manifests as intractable nausea and hiccups. APS has high diagnostic significance in neuromyelitis optica syndrome spectrum disorders (NMOSD) and can be the initial presentation of other critical diseases, including brainstem glioma. Methods: We described two representative cases of unrelated Japanese patients with APS. An etiologic evaluation, including a detailed intracranial neuroradiological examination and autoantibodies assessment, was performed. We also reviewed the literature focusing on the prognosis of pediatric APS symptoms. Results: A 14-year-old girl with aquaporin-4 antibody-positive NMOSD showed a good prognosis with immunotherapy, whereas another nine-year-old girl with irresectable medullary low-grade glioma had persistent symptoms for more than 10 years. All reported children aged >12 years were diagnosed with NMOSD, and patients aged <13 years showed heterogeneous etiologies. Conclusions: Distinctive time courses and neuroimaging features were key clinical findings for the diagnostic and therapeutic processes in these patients. This literature review highlights the wide spectrum and prognosis of pediatric-onset APS.

Abstract:BackgroundMaffucci syndrome is a congenital, non-hereditary mesodermal dysplasia characterized by multiple enchondromas and hemangiomas. The presence of visceral vascular lesions in this syndrome is exceedingly rare.Case presentationWe report a 26-year-old female who was diagnosed with Maffucci syndrome along with sclerosing angiomatoid nodular transformation (SANT) of the spleen. The patient underwent a laparoscopic splenectomy. Immunostaining of the excised specimen revealed 3 distinct types of vessels in the angiomatoid nodules: CD34−/CD8−/CD31+ small veins, CD34−/CD8+/CD31+ sinusoids, and CD34+/CD8−/CD31+ capillaries, leading to the diagnosis of SANT of the spleen.ConclusionsThis case reports the first patient in the literature exhibiting the features of Maffucci syndrome along with SANT of the spleen. The spleen is probably a predilection site of visceral vascular lesions in this syndrome with a proportion of 4 out of 14. An abdominal Computed Tomography (CT) scan is recommended for any cases of abdominal discomfort. Surgical excision is usually sufficient because of the relatively benign behavior of SANT, however, a more aggressive follow-up is proposed due to the high risk of malignant transformation of enchondromas and development of other neoplasms associated with this syndrome. Further studies are required to reveal its genetic basis for comprehensive prognosis evaluation and therapeutic guidance.

Abstract:Susac syndrome is a rare and enigmatic complex neurological disorder primarily affecting small blood vessels in the brain, retina, and inner ear. Diagnosing Susac syndrome may be extremely challenging not only due to its rarity, but also due to the variability of its clinical presentation. This paper describes two vastly different cases—one with mild symptoms and good response to therapy, the other with severe, complicated course, relapses and long-term sequelae despite multiple therapeutic interventions. Building upon the available guidelines, we highlight the utility of black blood MRI in this disease and provide a comprehensive review of available clinical experience in clinical presentation, diagnosis and therapy of this disease. Despite its rarity, the awareness of Susac syndrome may be of uttermost importance since it ultimately is a treatable condition. If diagnosed in a timely manner, early intervention can substantially improve the outcomes of our patients.

Abstract:Susac syndrome is an immune-mediated microvascular disease characterized by the clinical triad of acute multiple encephalopathies, branch retinal artery occlusion, and sensorineural deafness. However, the typical clinical triad is not seen in all patients at disease onset. A 29-year-old male was admitted to our hospital due to aggravation of headache accompanied by retarded reaction. After treatment, the patient's retarded reaction and neurological dysfunction were improved. One year later, the patient's condition did not worsen and he discontinued taking prednisone voluntarily five months after discharge. Two years later, the patient was admitted to our hospital again owing to a sudden field defect in the superior nasal quadrant of the left eye, and Susac syndrome was diagnosed. After treatment, the patient's condition became stabilized with no further progress, but the visual field defect did not recover. At the onset of Susac syndrome, the typical clinical triad of Susac syndrome is rare and difficult to identify. The case we report presented the clinical triad two years after the disease onset. We expect that this case report will increase physicians' understanding of this subject.

Abstract:Objective To improve understanding of the clinical manifestations, diagnosis and treatment of childhood Kasabach-Merritt phenomenon (KMP). Methods The clinical data of 13 patients admitted for KMP to XXX from January 2010 to January 2016 was retrospectively analyzed, with a review of relevant literature. Results The patients were 10 males and 3 females. The age of presentation varied from newborn to 5 months. 12 patients had cutaneous manifestations, like petechiae, ecchymosis, jaundice, skin masses, etc, 1 patient had pleural effusion. The location of lesions varied. The laboratory hallmark consists of profound thrombocytopenia and hypofibrinogenemia with elevated D-dimers. The median time from initial presentation to diagnosis was 60 days. After approaches like surgery, corticosteroids, propranolol, interferon, sirolimus, etc, 10 patients got remission while 3 patients died. 6 patients treated with sirolimushad complete response. Conclusions KMP is characterized with vascular tumor, severe thrombocytopenia and consumptive coagulopathy. Clinically, KMP often presents with early-onset and delay in diagnosis. Surgery is an effective approach for KMP. Sirolimus appears to be a promising treatment for KMP.

Abstract:Background. Superior mesenteric artery syndrome (SMAS) is a rare condition caused by the compression of the duodenum, which may occur in the case of fast weight loss. Currently, the relationship between superior mesenteric artery syndrome and anorexia nervosa is still unclear. The aim of this study is to identify the precocious clinical signs and symptoms of SMAS in patients affected by anorexia nervosa so as not to delay the diagnosis. Methods. We present the clinical case of a young female patient with anorexia nervosa complicated by SMAS. We performed a literature review of SMAS in children affected by anorexia nervosa between 1962 and 2023, according to the PRISMA Extension Guide for Scoping Reviews. Results. Reviewing the literature, 11 clinical cases were described for the pediatric age. The median age at diagnosis was 17 years (ranging from 13 to 18 years). The diagnosis of SMAS may be challenging as symptoms overlap those of anorexia, but it should be kept in mind mostly in cases of post-prandial abdominal pain, anxiety or depression, nausea, vomiting, and weight loss. Conclusion. Even specific clinical symptoms may act as flag tags to drive attention to this rare but potentially fatal condition.

Abstract:Klinefelter syndrome (KS), characterized by an additional X-chromosome in males, manifests in a wide range of neuroendocrine and psychiatric symptoms. Individuals with KS often face increased risks of hormonal dysfunction, leading to depression and anxiety, although extended research during pediatric and adolescent age is still limited. This critical phase, decisive for KS children, is influenced by a combination of genetic, environmental and familial factors, which impact brain plasticity. In this report, we reviewed, in a narrative form, the crucial KS psychopathological hallmarks in children. To better describe neuroendocrine and neuropsychiatric outcomes in children with KS, we presented the case of an 11-year-old prepubertal child with mosaic KS who was referred to our Center of Developmental Psychopathology due to a decline in his academic performance, excessive daytime fatigue and increased distractibility over the past few months. Family history revealed psychiatric conditions among first- and second-degree relatives, including recently divorced parents and a 15-year-old sister. Early-onset persistent depressive disorder and anxious traits were diagnosed. Timely identification of susceptible children, with thorough examination of familial psychiatric history, environmental influences and neurocognitive profile, alongside targeted interventions, could potentially mitigate lifelong psychopathology-related disabilities in pediatric and adolescent KS cases, including those with mosaic KS.

Abstract:Morgellons disease is characterized by the persistent delusion of skin infestation, ultimately inflicting wounds and impairing quality of life. There is insufficient and conflicting research pertaining to this condition, imposing challenges on clinicians in understanding, diagnosing, and treating it. In this review, we summarize the available literature on Morgellons disease including its historical evolution, epidemiology, proposed pathophysiology, underlying structural and functional brain pathologies, typical and atypical clinical presentations, diagnosis, and treatment. A comprehensive review of the literature was conducted on PubMed, Embase, and Scopus using specified keywords. Selected articles were screened by two independent reviewers based on set inclusion and exclusion criteria. Conflicts were resolved by a third reviewer as needed. No limit to the date of selected articles was set due to the scarce literature available on the subject. Morgellons disease is an underdiagnosed entity, owing mostly to the lack of an established pathophysiology and treatment guidelines. While many authors classify it as a type of delusional infestation (DI), others correlate MD with an underlying spirochetal infection, namely Lyme disease. Neuroimaging studies have revealed abnormalities in the "fronto-striato-thalamo-parietal network", a finding common to patients with DI, in addition to alterations in structures related to the "Itch Processing Pathway". Patients tend to extract fibers from their skin lesions and place them in a match box hence the term "match box sign". The diagnosis is that of exclusion, requiring extensive work up to rule out secondary causes and differential diagnoses. Treatment is largely based on the use of antipsychotics, with or without cognitive behavioral therapy. Despite being a diagnosis of exclusion, clinicians must be aware of this entity and have a profound understanding of the pathogenesis underlying it. Upon clinical suspicion, secondary Morgellons should always be ruled out through a thorough history taking, physical examination, and laboratory exams. Despite the challenges brought by the heterogeneous presentation of the condition and the paucity of research revolving around it, the great impact that Morgellons disease has on patients' quality of life forms a pressing need for its adequate detection, diagnosis, and treatment.

Abstract:Background Multiple system atrophy (MSA) is a complex and fatal neurodegenerative movement disorder. Understanding the comorbidities and drug therapy is crucial for MSA patients' safety and management. Objectives To investigate the pattern of comorbidities and aspects of drug therapy in MSA patients. Methods Cross-sectional data of MSA patients according to Gilman et al. (2008) diagnostic criteria and control patients without neurodegenerative diseases (non-ND) were collected from German, multicenter cohorts. The prevalence of comorbidities according to WHO ICD-10 classification and drugs administered according to WHO ATC system were analyzed. Potential drug-drug interactions were identified using AiDKlinik®. Results The analysis included 254 MSA and 363 age- and sex-matched non-ND control patients. MSA patients exhibited a significantly higher burden of comorbidities, in particular diseases of the genitourinary system. Also, more medications were prescribed MSA patients, resulting in a higher prevalence of polypharmacy. Importantly, the risk of potential drug-drug interactions, including severe interactions and contraindicated combinations, was elevated in MSA patients. When comparing MSA-P and MSA-C subtypes, MSA-P patients suffered more frequently from diseases of the genitourinary system and diseases of the musculoskeletal system and connective tissue. Conclusions MSA patients face a substantial burden of comorbidities, notably in the genitourinary system. This, coupled with increased polypharmacy and potential drug interactions, highlights the complexity of managing MSA patients. Clinicians should carefully consider these factors when devising treatment strategies for MSA patients.

Abstract:1‐3‐Bis‐(2‐chlorethyl)‐1‐nitrosourea (BCNU) was administered intravenously to 72 patients with advanced gastrointestinal cancer. Total doses for each course of therapy ranged between 250 and 375 mg. per square meter, given in divided doses over periods of from 3 to 6 days. Initial toxic effects were transient nausea and vomiting. Bone‐marrow depression occurred late and involved all formed elements of the blood. Four patients had evidence of clinically significant hepatotoxicity. Courses of therapy could be repeated at 8 week intervals without significant cumulative toxicity. Objective tumor regressions were observed in 12.5 per cent of patients treated, and 19.5 per cent claimed symptomatic improvement. The mean duration of the tumor regressions was 4 months. Because of the low rate of tumor regressions and the short duration of these regressions, BCNU, as administered by the method employed in this study, does rwt seem to have a practical role in the management of advanced gastrointestinal cancer.

Abstract:The field of syndromology spans a vast range of different and exciting research areas. Many famous clinical geneticists have laid the groundwork for today's landscape of syndromology. They described the clinical landscape of rare syndromes and started to unravel the underlying genetic causes. The advent of next generation sequencing technologies has led to the discovery of many novel disease-associated genes. This enabled clinical geneticists to increase the diagnostic yield and enlarge patient cohorts, broaden the phenotypic and genotypic spectrum of rare diseases and dig deeper into the underlying pathogenetic mechanisms. In this issue, we want to give an overview of this impressive development of syndromology by assembling a potpourri of different lines of research. We focus on the evolving phenotypes of rare diseases and their clinical management in adulthood, the booming field of epimutations, and the most commonly used model systems for exploration of pathogenetic mechanisms. Some research groups have started to focus on describing the evolving phenotypes of rare genetic diseases into adulthood. The article by Ariane Schmetz and colleagues gives an overview of these adult phenotypes and emphasizes the need of further studies to improve counselling of families with young children and adults with rare syndromes. They describe the phenotypes of four adult individuals with Costello syndrome illustrating the high variability of neurocognitive outcome and documenting recurrent carcinomata in some individuals. Dorothee Maliszewski-Makowka and Dagmar Wieczorek describe the developments in and missions of the so-called "MZEB" ("Medizinische Zentren für Erwachsene mit Behinderungen"), which were founded to ensure that adults with multiple disabilities receive medical care in an interdisciplinary and multi-professional setting. They summarize the efforts of the MZEB in the LVR clinic in Bedburg-Hau and the Institute of Human Genetics of the Heinrich-Heine University Düsseldorf to employ genetic testing in adults with mental disabilities and developmental and psychiatric disorders. The article written by Bernhard Horsthemke addresses the growing role of epigenetics in rare diseases by illustrating the classification of epimutations. Despite the large number of disease-associated chromatin modifier genes, for which he gives a comprehensive overview, the pathogenetic mechanisms remain to be elucidated. He concludes that the disease-relevant loci affected by the mutated chromatin modifier genes and their pathogenetic mechanisms are unknown and gives a critical outlook on the clinical application of newly developed epigenome editing tools. Different model systems are needed to gain insight into these and other pathogenetic mechanisms leading to syndromic diseases, to validate novel disease genes and variants of unknown significance and to assess potential therapeutic means for these rare diseases in a pre-clinical setting. The article by Anne Gregor and Christiane Zweier compiles the most commonly used models and outlines their application areas, as well as their advantages and limitations.
