Abstract:BackgroundAnkle function impairment is a critical factor impairing normal walking in survivors of stroke. The soft robotic exoskeleton (SRE) is a novel, portable, lightweight assistive device with promising therapeutic potential for gait recovery during post-stroke rehabilitation. However, whether long-term SRE-assisted walking training influences walking function and gait quality in patients following subacute stroke is unknown. Therefore, the primary objective of this study was to assess the therapeutic effects of SRE-assisted walking training on clinical and biomechanical gait outcomes in the rehabilitation of patients with subacute stroke.MethodsA group patients who had experienced subacute stroke received conventional rehabilitation (CR) training combined with 10-session SRE-assisted overground walking training (30 min per session, 5 sessions/week, 2 weeks) (SRE group, n = 15) compared with the control group that received CR training only (CR group, n = 15). Clinical assessments and biomechanical gait quality measures were performed pre-and post-10-session intervention, with the 10-Minute Walk Test (10MWT) and 6-Minute Walk Test (6MWT) used to define the primary clinical outcome measures and the Functional Ambulation Category, Fugl-Meyer Assessment for Lower Extremity (FMA-LE) subscale, and Berg Balance Scale defined the secondary outcome measures. The gait quality outcome measures included spatiotemporal and symmetrical parameters during walking.ResultsAfter the 10-session intervention, the SRE and CR groups exhibited significant within-group improvements in all clinical outcome measures (p < 0.05). Between-comparison using covariance analyses demonstrated that the SRE group showed greater improvement in walking speed during the 10MWT (p < 0.01), distance walked during the 6MWT (p < 0.05), and FMA-LE scores (p < 0.05). Gait analyses showed that the SRE group exhibited significantly improved spatiotemporal symmetry (p < 0.001) after 10-session training, with no significant changes observed in the CR group.ConclusionCompared with CR training, SRE-assisted walking training led to greater improvements in walking speed, endurance, and motor recovery. Our findings provide preliminary evidence that SRE may be considered for inclusion in intensive gait training clinical rehabilitation programs to further improve walking function in patients who have experienced stroke.

Abstract:Abstract Background Individuals requiring greater physical assistance to practice walking complete fewer steps in physical therapy during subacute stroke rehabilitation. Powered exoskeletons have been developed to allow repetitious overground gait training for individuals with lower limb weakness. The objective of this study was to determine the efficacy of exoskeleton-based physical therapy training during subacute rehabilitation for walking recovery in non-ambulatory patients with stroke. Methods An assessor-blinded randomized controlled trial was conducted at 3 inpatient rehabilitation hospitals. Patients with subacute stroke (< 3 months) who were unable to walk without substantial assistance (Functional Ambulation Category rating of 0 or 1) were randomly assigned to receive exoskeleton-based or standard physical therapy during rehabilitation, until discharge or a maximum of 8 weeks. The experimental protocol replaced 75% of standard physical therapy sessions with individualized exoskeleton-based sessions to increase standing and stepping repetition, with the possibility of weaning off the device. The primary outcome was walking ability, measured using the Functional Ambulation Category. Secondary outcomes were gait speed, distance walked on the 6-Minute Walk Test, days to achieve unassisted gait, lower extremity motor function (Fugl-Meyer Assessment), Berg Balance Scale, Patient Health Questionnaire, Montreal Cognitive Assessment, and 36-Item Short Form Survey, measured post-intervention and after 6 months. Results Thirty-six patients with stroke (mean 39 days post-stroke) were randomized (Exoskeleton = 19, Usual Care = 17). On intention-to-treat analysis, no significant between-group differences were found in the primary or secondary outcomes at post-intervention or after 6 months. Five participants randomized to the Exoskeleton group did not receive the protocol as planned and thus exploratory as-treated and per-protocol analyses were undertaken. The as-treated analysis found that those adhering to exoskeleton-based physical therapy regained independent walking earlier (p = 0.03) and had greater gait speed (p = 0.04) and 6MWT (p = 0.03) at 6 months; however, these differences were not significant in the per-protocol analysis. No serious adverse events were reported. Conclusions This study found that exoskeleton-based physical therapy does not result in greater improvements in walking independence than standard care but can be safely administered at no detriment to patient outcomes. Abstract:Background: Advanced technologies are increasingly used to address impaired mobility after neurological insults, with growing evidence of their benefits for various populations. However, certain robotic devices have not been extensively investigated in specific conditions, limiting knowledge about optimal application for healthcare. Objective: To compare effectiveness of conventional gait training with exoskeleton-assisted walking for non-traumatic brain injury during early stage rehabilitation. Methods: Clinical evaluation data at admission and discharge were obtained in a retrospective case-control design. Patients received standard of care physical therapy either using Ekso GT or not. Within- or between-group statistical tests were performed to determine change over time and interventional differences. Results: This study analyzed forty-nine individuals (33% female), 20 controls and 29 Ekso participants who were equivalent at baseline. Both groups improved in Functional Independence Measure scores and ambulation ability (p < .00001 and p < .001, respectively). Control subjects demonstrated significantly different distance walked and assistance level values at discharge from those who were treated with the exoskeleton (p < .01). Conclusion: Robotic locomotion is non-inferior for subacute functional recovery after non-traumatic brain injury. Conventional therapy produced larger gait performance gains during hospitalization. Further research is needed to understand specific factors influencing efficacy and the long-term implications after rehabilitation.

Graham Little–Piccardi–Lassueur syndrome in a patient with androgen insensitivity syndrome

Abstract:Graham Little–Piccardi–Lassueur syndrome (GLPLS) is characterized by the triad of cicatricial scalp alopecia, keratosis pilaris-like eruption on the trunk and extremities, and non-scarring hair loss of the pubis and axillae. GLPLS was first reported in 1914 and since then, approximately 40 reports have appeared in the literature. The syndrome is currently classified by the North American Hair Research Society as a subtype of lichen planopilaris. A single previous reported case in 2004 highlighted the possible association between GLPLS and androgen insensitivity syndrome (testicular feminization). Herein, a second case of GLPLS associated with androgen insensitivity is reported. An approximately 40-year-old woman previously diagnosed complete androgen insensitivity syndrome was referred for the evaluation of hair loss. Keratosis pilaris began on the arms and legs in the patient’s teen years and was followed by scalp hair loss beginning on the crown in the early 20s. The scalp was pruritic in the areas of hair loss. There was no history of atopic dermatitis. Current medications included an ethinyl estradiol-levonorgestrel containing oral contraceptive. The patient had previously been using conjugated estrogens and medroxyprogesterone acetate following gonadectomy. Examination revealed a tall and phenotypically female patient. The patient had an approximately 25 cm 9 25 cm area of nearly complete scarring alopecia affecting the vertex and mid-scalp (Fig. 1). At the periphery of the scarred area, there were hair follicles with perifollicular erythema and scale. The pull test in this area was positive for anagen hairs. Facial hair was not present, and axillary and pubic hairs were sparse. A reduction in eyebrow hair density was noted. A keratosis pilaris-like eruption was present on the arms (Fig. 2), legs, and scattered on the trunk. A 4 mm punch biopsy from the scalp revealed extensive scarring. Perifollicular fibrosis and a perifollicular inflammatory infiltrate of lymphocytes were present in a manner consistent with the diagnosis of lichen planopilaris. There were no superficial or deep inflammatory infiltrates to suggest discoid lupus. The basement membrane was not thickened, and there were not epidermal interface changes. Neutrophils were not seen. Blood work revealed that the hemoglobin level was normal (141 g/l) as was serum ferritin (157 lg/l) and thyroid-stimulating hormone (1.84 mIU/l). The follicle-stimulating hormone level was elevated at 57 IU/l, and the estradiol level was low at 47 pmol/l. Testosterone was consistently low following previous gonadectomy at 0.7 nmol/l. Treatment was initiated with hydroxychloroquine 200 mg orally twice daily, intralesional triamcinolone acetonide (5 mg/ml up to 4 ml every 2–4 months), and topical clobetasol propionate 0.05% lotion but provided only minimal symptom improvement over a 6-month period. Adapting to the Mechanical Properties and Active Force of an Exoskeleton by Altering Muscle Synergies in Chronic Stroke Survivors

Abstract:Chronic stroke survivors often suffer from gait impairment resistant to intervention. Recent rehabilitation strategies based on gait training with powered exoskeletons appear promising, but whether chronic survivors may benefit from them remains controversial. We evaluated the potential of exoskeletal gait training in restoring normal motor outputs in chronic survivors (N = 10) by recording electromyographic signals (EMGs, 28 muscles both legs) as they adapted to exoskeletal perturbations, and examined whether any EMG alterations after adaptation were underpinned by closer-to-normal muscle synergies. A unilateral ankle-foot orthosis that produced dorsiflexor torque on the paretic leg during swing was tested. Over a single session, subjects walked overground without exoskeleton (FREE), then with the unpowered exoskeleton (OFF), and finally with the powered exoskeleton (ON). Muscle synergies were identified from EMGs using non-negative matrix factorization. During adaptation to OFF, some paretic-side synergies became more dissimilar to their nonparetic-side counterparts. During adaptation to ON, in half of the subjects some paretic-side synergies became closer to their nonparetic references relative to their similarity at FREE as these paretic-side synergies became sparser in muscle components. Across subjects, level of inter-side similarity increase correlated negatively with the degree of gait temporal asymmetry at FREE. Our results demonstrate the possibility that for some survivors, exoskeletal training may promote closer-to-normal muscle synergies. But to fully achieve this, the active force must trigger adaptive processes that offset any undesired synergy changes arising from adaptation to the device's mechanical properties while also fostering the reemergence of the normal synergies.

Abstract:BackgroundStroke significantly affects thousands of individuals annually, leading to considerable physical impairment and functional disability. Gait is one of the most important activities of daily living affected in stroke survivors. Recent technological developments in powered robotics exoskeletons can create powerful adjunctive tools for rehabilitation and potentially accelerate functional recovery. Here, we present the development and evaluation of a novel lower limb robotic exoskeleton, namely H2 (Technaid S.L., Spain), for gait rehabilitation in stroke survivors.MethodsH2 has six actuated joints and is designed to allow intensive overground gait training. An assistive gait control algorithm was developed to create a force field along a desired trajectory, only applying torque when patients deviate from the prescribed movement pattern. The device was evaluated in 3 hemiparetic stroke patients across 4 weeks of training per individual (approximately 12 sessions). The study was approved by the Institutional Review Board at the University of Houston. The main objective of this initial pre-clinical study was to evaluate the safety and usability of the exoskeleton. A Likert scale was used to measure patient’s perception about the easy of use of the device.ResultsThree stroke patients completed the study. The training was well tolerated and no adverse events occurred. Early findings demonstrate that H2 appears to be safe and easy to use in the participants of this study. The overground training environment employed as a means to enhance active patient engagement proved to be challenging and exciting for patients. These results are promising and encourage future rehabilitation training with a larger cohort of patients.ConclusionsThe developed exoskeleton enables longitudinal overground training of walking in hemiparetic patients after stroke. The system is robust and safe when applied to assist a stroke patient performing an overground walking task. Such device opens the opportunity to study means to optimize a rehabilitation treatment that can be customized for individuals.Trial registration: This study was registered at ClinicalTrials.gov (https://clinicaltrials.gov/show/NCT02114450).

Acquisition of Multidrug-Resistant Gram-Negative Bacteria: Incidence and Risk Factors within a Long-Term Care Population

Abstract:Background and Objective. An improved understanding of the transmission dynamics of multidrug-resistant (MDR) gram-negative bacteria and the mechanism of acquisition in long-term care facilities (LTCFs) could aid in the development of prevention strategies specific to LTCFs. We thus investigated the incidence of acquisition of these pathogens among an LTCF population. Design. Prospective cohort study. Setting. Three separate wards at a 600-bed LTCF in metropolitan Boston, Massachusetts, during the period October 31, 2006, through October 22, 2007. Participants. One hundred seventy-two LTCF residents. Methods. A series of rectal samples were cultured to determine acquisition of MDR gram-negative bacteria, defined as absence of MDR gram-negative bacterial colonization at baseline and de novo recovery of MDR gram-negative bacteria from a follow-up culture. Molecular typing was performed to identify genetically linked strains. A nested matched case-control study was performed to identify risk factors associated with acquisition. Results. Among 135 residents for whom at least 1 follow-up culture was performed, 52 (39%) acquired at least 1 MDR gram-negative organism during the study period. Thirty-two residents (62%) had not been colonized at baseline and had acquired at least 1 MDR gram-negative species at follow-up culture, and 20 residents (38%) were colonized at baseline and had acquired at least I MDR gram-negative species at follow-up culture. The most common coresistance pattern was resistance to extended-spectrum penicillins, ciprofloxacin, and gentamicin (57 isolates [42.5%]). Genetically related strains of MDR gram-negative bacteria were identified among multiple residents and between roommates. On conditional logistic regression analysis, antibiotic exposure during the study period was significantly associated with acquisition of MDR gram-negative bacteria (odds ratio, 5.6 [95% confidence interval, 1.1-28.7]; P = .04). Conclusions. Acquisition of MDR gram-negative bacteria occurred frequently through resident-to-resident transmission. Existing infection control interventions need to be reevaluated.

Abstract:Spinal cord injuries frequently impair the ability to walk. Powered lower limb exoskeletons offer a promising solution to restore walking ability. However, they are currently restricted to even ground. We hypothesized that compliant exoskeleton knees could decrease required effort to maneuver on uneven terrain, and increase gait velocity and stability. We describe a case study of a motor-complete spinal cord injury user (AIS A, Th12) walking with a powered exoskeleton on even and uneven ground over multiple sessions after extensive training. Measurements with compliant or rigid exoskeleton knee joints were performed on three different days for each configuration. Body motion and crutch ground interaction forces were recorded to assess gait performance. We observed higher walking speeds with a compliant exoskeleton knee configuration (mean: 0.116 m/s on uneven and 0.145 m/s on even ground) compared to a rigid configuration (mean: 0.083 m/s and 0.100 m/s). Crutch force impulse was significantly reduced in the compliant configuration. Lastly, gait was more symmetric when the knee joints were compliant. In conclusion, compliant exoskeleton knee joints can help maneuver uneven ground faster and with less user effort than rigid joints. Based on our findings, exoskeleton designers should consider introducing compliance into their design to increase gait robustness and performance, and render exoskeletons more suitable for daily life use.
Heart rate and oxygen demand of powered exoskeleton-assisted walking in persons with paraplegia

Abstract:Historically, persons with paralysis have limited options for overground ambulation. Recently, powered exoskeletons have become available, which are systems that translate the user's body movements to activate motors to move the lower limbs through a predetermined gait pattern. As part of an ongoing clinical study (NCT01454570), eight nonambulatory persons with paraplegia were trained to ambulate with a powered exoskeleton. Measurements of oxygen uptake (VO2) and heart rate (HR) were recorded for 6 min each during each maneuver while sitting, standing, and walking. The average value of VO2 during walking (11.2 +/- 1.7 mL/kg/min) was significantly higher than those for sitting and standing (3.5 +/- 0.4 and 4.3 +/- 0.9 mL/kg/min, respectively; p < 0.001). The HR response during walking was significantly greater than that of either sitting or standing (118 +/- 21vs 70 +/- 10 and 81 +/- 12 beats per minute, respectively: p < 0.001). Abstract:Objective. To compare the efficacy of restoration of gait function in patients in the early recovery period of ischemic stroke (IS) using a lower limb exoskeleton and an active-passive pedal bicycle trainer. Materials and methods. An open randomized study of 47 patients in the early recovery period of IS was undertaken. Patients in group 1 received a single rehabilitation course including exoskeleton exercises using an ExoAtlet exoskeleton; those of group 2 used a pedal trainer for active-passive training (five days per week for two weeks). Treatment results were assessed using a series of tests, including the Hauser Ambulation Index, the 10-m walking test, the Berg balance test, stabilometry, and gait biomechanics. Training courses were completed by 20 patients in group 1 and 21 in group 2. Results. Completion of training courses was followed by statistically significant increases in strength in the paretic muscles, postural stability, functional level, and walking speed in patients of both groups, though recovery was statistically significantly more marked in patients of group 1 (p < 0.05). Patients of group 1 showed a statistically significant decrease in the level of disability and an increase in the activities of daily living, changes being greater than those in group 2. Analysis of the main indicators of the statokinesiogram showed that improvements in patients of group 1 were greater, though statistically significant differences were seen only in the dynamics of the length and area of the curve in the test with the eyes open. Studies of gait biomechanics showed that gait function was altered: there was a statistically significant 2.2-fold reduction in movement speed, a 1.6-fold decrease in double stride length, an a 1.3-fold reduction in tempo compared with normal values. After completion of exercises, patients of group 1 displayed statistically significant increases in double stride length and walking speed and rate and a contraction in the locomotor cycle period. Conclusions. The study identified improvements to locomotion using instrumented rehabilitation using both an exoskeleton and an active-passive pedal trainer, though use of the exoskeleton had advantages, in that it produced statistically significantly greater recovery of strength, stability, and walking speed and symmetry over an analogous period using the trainer. A significant increase in postural stability was obtained with retention of the vertical posture.
Sensory-Motor Neurostimulation to Enhance Exosuit Performance

Abstract:Exosuits typically provide limited mechanical support and rely on a user's residual functional ability. However, people with neurological impairments often suffer from both motor and sensory deficits that limit the assistance an exosuit can provide. To overcome these limitations, we developed the REINFORCE system, that complements the mechanical assistance provided by an exosuit, the Myosuit, with (1) functional electrical stimulation to enhance the activities of leg muscles, and (2) transcutaneous electrical nerve stimulation to restore somatosensory information. It consists of a fully portable and highly modular system that can be easily adapted to the level of impairment and specific need of each participant. Technical verification with three healthy participants showed reliable synchronization between all modules of the systems in all phases of walking. Additionally, we tested the system's effectiveness in one participant with multiple sclerosis who walked overground with and without functional electrical stimulation. Results showed a slight increase in self-selected walking speed (approx. 18%) and in the peak hip flexion at late swing (approx. 12%) as well as reduced step-to-step variability of step length and step time when electrical stimulation was provided. Our findings push towards a clinical trial involving more patients to validate the effectiveness of the REINFORCE system on participants' mobility.

Abstract:Abstract Walking rehabilitation using exoskeletons is of high importance to maximize independence and improve the general well-being of spinal cord injured subjects. We present the design and control of a lightweight and modular robotic exoskeleton to assist walking in spinal cord injured subjects who can control hip flexion, but lack control of knee and ankle muscles. The developed prototype consists of two robotic orthoses, which are powered by a motor-harmonic drive actuation system that controls knee flexion–extension. This actuation module is assembled on standard passive orthoses. Regarding the control, the stance-to-swing transition is detected using two inertial measurement units mounted on the tibial supports, and then the corresponding motor performs a predefined flexion–extension cycle that is personalized to the specific patient’s motor function. The system is portable by means of a backpack that contains an embedded computer board, the motor drivers, and the battery. A preliminary biomechanical evaluation of the gait-assistive device used by a female patient with incomplete spinal cord injury at T11 is presented. Results show an increase of gait speed (+24.11%), stride length (+7.41%), and cadence (+15.56%) when wearing the robotic orthoses compared with the case with passive orthoses. Conversely, a decrease of lateral displacement of the center of mass (−19.31%) and step width (−13.37% right step, −8.81% left step) are also observed, indicating gain of balance. The biomechanical assessment also reports an overall increase of gait symmetry when wearing the developed assistive device.

Abstract:Objective: To evaluate the safety, walking efficiency, physiological cost, don and doff time cost, and user satisfaction of Ai-robot. Design: Prospective, multi-center, and cross-over trial. Subjects: Paraplegic subjects ( n = 40) with T6–L2 level spinal cord injury. Methods: Subjects who could walk independently using Aiwalker, Ailegs, and hip knee ankle foot orthosis (HKAFO) for 6 min within 30 days of training underwent 10 sets of tests. In each set, they completed three 6-min walk test (6MWT) sessions using the three aids in random order. Results: Skin lesions, pressure sores, and fractures, were the main adverse events, likely due to a lack of experience in using exoskeleton systems. The average 6MWT distances of the Aiwalker, Ailegs, and HKAFO groups were 134.20 ± 18.74, 79.71 ± 18.06, and 48.31 ± 19.87 m, respectively. The average heart rate increases in the Aiwalker (4.21 ± 8.20%) and Ailegs (41.81 ± 23.47%) groups were both significantly lower than that in the HKAFO group (62.33 ± 28.32%) (both p < 0.001). The average donning/doffing time costs for Ailegs and Aiwalker were significantly shorter than that of HKAFO (both p < 0.001). Satisfaction was higher in the Ailegs and Aiwalker groups (both p < 0.001). Conclusion: Subjects with paraplegia below T6 level were able to ambulate safely and efficiently with Ai-robot. The use of Ai-robot should be learned under the guidance of experienced medical personnel.

Abstract:Several HLA-A*0201-restricted peptide epitopes that can be used as targets for active immunotherapy have been identified within melanocyte differentiation proteins. However, uncertainty exists as to the most effective way to elicit CD8+ T cells with these epitopes in vivo. We report the use of transgenic mice expressing a derivative of HLA-A*0201, and dendritic cells, to enhance the activation of CD8+ T cells that recognize peptide epitopes derived from human tyrosinase and glycoprotein 100. We find that by altering the cell surface density of the immunizing peptide on the dendritic cells, either by pulsing with higher concentrations of peptide, or by changing the MHC-peptide-binding affinity by generating variants of the parent peptides, the size of the activated CD8+ T cell populations can be modulated in vivo. Significantly, the density of peptide that produced the largest response was less than the maximum density achievable through short-term peptide pulsing. We have also found, however, that while some variant peptides are effective at eliciting both primary and recall CD8+ T cell responses that can recognize the parental epitope, other variant epitopes lead to the outgrowth of CD8+ T cells that only recognize the variant. HLA-A*0201 transgenic mice provide an important model to define which peptide variants are most likely to stimulate CD8+ T cell populations that recognize the parental, melanoma-specific peptide.
A Propulsion Neuroprosthesis Improves Overground Walking in Community-Dwelling Individuals After Stroke

Abstract:Functional electrical stimulation (FES) is a common neuromotor intervention whereby electrically evoked dorsiflexor muscle contractions assist foot clearance during walking. Plantarflexor neurostimulation has recently emerged to assist and retrain gait propulsion; however, safe and effective coordination of dorsiflexor and plantarflexor neurostimulation during overground walking has been elusive, restricting propulsion neuroprostheses to harnessed treadmill walking. We present an overground propulsion neuroprosthesis that adaptively coordinates, on a step-by-step basis, neurostimulation to the dorsiflexors and plantarflexors. In 10 individuals post-stroke, we evaluate the immediate effects of plantarflexor neurostimulation delivered with different onset timings, and retention to unassisted walking (NCT06459401). Preferred onset timing differed across individuals. Individualized tuning resulted in a significant 10% increase in paretic propulsion peak (Δ: 1.41 ± 1.52%BW) and an 8% increase in paretic plantarflexor power (Δ: 0.27 ± 0.23 W/kg), compared to unassisted walking. Post-session unassisted walking speed, paretic propulsion peak, and propulsion symmetry all significantly improved by 9% (0.14 ± 0.09 m/s), 28% (2.24 ± 3.00%BW), and 12% (4.5 ± 6.0%), respectively, compared to pre-session measurements. Here we show that an overground propulsion neuroprosthesis can improve overground walking speed and propulsion symmetry in the chronic phase of stroke recovery. Future studies should include a control group to examine the efficacy of gait training augmented by the propulsion neuroprosthesis compared to gait training alone.

Abstract:Stroke disease leads to a partial or complete disability affecting muscle strength and functional mobility. Early rehabilitation sessions might induce neuroplasticity and restore the affected function or structure of the patients. Robotic rehabilitation minimizes the burden on therapists by providing repetitive and regularly monitored therapies. Commercial exoskeletons have been found to assist hip and knee motion. For instance, unilateral exoskeletons have the potential to become an effective training system for patients with hemiparesis. However, these robotic devices leave the ankle joint unassisted, essential in gait for body propulsion and weight-bearing. This article evaluates the effects of the robotic ankle orthosis T-FLEX during cooperative assistance with the AGoRA unilateral lower-limb exoskeleton (hip and knee actuation). This study involves nine subjects, measuring muscle activity and gait parameters such as stance and swing times. The results showed a reduction in muscle activity in the Biceps Femoris of 50%, Lateral Gastrocnemius of 59% and Tibialis Anterior of 35% when adding T-FLEX to the AGoRA unilateral lower-limb exoskeleton. No differences were found in gait parameters. Nevertheless, stability is preserved when comparing the two legs. Future works should focus on evaluating the devices in ground tests in healthy subjects and pathological patients.

Abstract:Abstract Background Wearable ankle robotics could potentially facilitate intensive repetitive task-specific gait training on stair environment for stroke rehabilitation. A lightweight (0.5 kg) and portable exoskeleton ankle robot was designed to facilitate over-ground and stair training either providing active assistance to move paretic ankle augmenting residual motor function (power-assisted ankle robot, PAAR), or passively support dropped foot by lock/release ankle joint for foot clearance in swing phase (swing-controlled ankle robot, SCAR). In this two-center randomized controlled trial, we hypothesized that conventional training integrated with robot-assisted gait training using either PAAR or SCAR in stair environment are more effective to enhance gait recovery and promote independency in early stroke, than conventional training alone. Methods Sub-acute stroke survivors (within 2 months after stroke onset) received conventional training integrated with 20-session robot-assisted training (at least twice weekly, 30-min per session) on over-ground and stair environments, wearing PAAR (n = 14) or SCAR (n = 16), as compared to control group receiving conventional training only (CT, n = 17). Clinical assessments were performed before and after the 20-session intervention, including functional ambulatory category as primary outcome measure, along with Berg balance scale and timed 10-m walk test. Results After the 20-session interventions, all three groups showed statistically significant and clinically meaningful within-group functional improvement in all outcome measures (p < 0.005). Between-group comparison showed SCAR had greater improvement in functional ambulatory category (mean difference + 0.6, medium effect size 0.610) with more than 56% independent walkers after training, as compared to only 29% for CT. Analysis of covariance results showed PAAR had greater improvement in walking speed than SCAR (mean difference + 0.15 m/s, large effect size 0.752), which was in line with the higher cadence and speed when wearing the robot during the 20-session robot-assisted training over-ground and on stairs. Conclusions Robot-assisted stair training would lead to greater functional improvement in gait independency and walking speed than conventional training in usual care. The active powered ankle assistance might facilitate users to walk more and faster with their paretic leg during stair and over-ground walking. Trial registration: ClinicalTrials.gov NCT03184259. Abstract:Background Even though robotic therapy is becoming more commonly used in research protocols for lower limb stroke rehabilitation, there still is a significant gap between research evidence and its use in clinical practice. Therefore, the present study was designed assuming that the wearable mobile gait device training for chronic stroke patients might have different effects on functional independence when compared to training with a stationary gait device. The present study aims to examine the effects of gait training with ExoAthlet exoskeleton and Lokomat Free-D on functional independence, functional capacity, and quality of life in chronic stroke patients. Methods The present study included 32 chronic stroke patients. Participants were randomly divided into two groups. Functional independence of patients was evaluated by using Functional Independence Measure (FIM), physical function was assessed by using the 30-second chair stand test (30-CST), functional capacity was measured by using the 6-Minute Walk Test (6MWT), and quality of life was assessed by using Short Form 36 (SF36). All participants underwent a conventional physiotherapy program for eight weeks, three sessions per week, and each session lasted 60 min. After the physiotherapy program, one group received gait training by using ExoAthlet exoskeleton (ExoAtlet 1 model/2019, Russia), while the other group received training by using Lokomat Free-D (Hocoma, Lokomat Pro Free-D model/2015, Switzerland). Participants were assessed at baseline and post-intervention. Results Results achieved in this study revealed that there was a statistically significant difference between FIM, 30-CST, 6MWT, and SF36 scores before and after the treatment in both groups (p 0.05). However, there was a statistically significant difference between Exoskeleton ExoAthlet and Lokomat Free-D groups in terms of SF-36 sub-parameters "vitality", "mental health", "bodily pain", and "general health perception" (p < 0.05). Conclusions This study demonstrated that the use of ExoAthlet exoskeleton and Lokomat Free-D in addition to conventional physiotherapy, was effective in improving functional independence, physical function, functional capacity, and quality of life among chronic stroke patients. Incorporation of robotic gait aids into rehabilitation for chronic stroke patients might offer significant advantages.
