Abstract:Objective:To analyze the application value of acupuncture combined with conventional Western medicine in postoperative treatment of patients with critical cerebral hemorrhage.Methods:A total of 92 patients with critical cerebral hemorrhage treated in Luohe Second People’s Hospital from July 2019 to March 2021 were selected and divided into acupuncture group and conventional group by random number table method, with 46 cases in each group. The conventional group was treated with conventional Western medicine, and the acupuncture group was treated by acupuncture based on the treatment of the conventional group. The efficacy, and the National Institutes of Health Stroke Scale (NIHSS), activity of daily living scale (ADL), Fugl Meyer motor function assessment (FMA), acute physiology and chronic health evaluation-II (APACHE II) and Glasgow Coma Scale (GCS) before and after treatment were compared between the two groups. The indexes of cerebral hemodynamics and oxidative stress before and after treatment were compared between the two groups.Results:The total effective rate of acupuncture group (91.30%, 42/46) was higher than that of conventional group (76.09%, 35/46), P<0.05. Before treatment, there was no significant difference in NIHSS, ADL, APACHE II, FMA and GCS scores between the two groups ( P>0.05); after treatment, NIHSS, ADL and APACHE Ⅱ scores were lower, FMA and GCS scores were higher in acupuncture group, compared with conventional group ( P<0.05). Before treatment, there was no significant difference in systolic peak velocity (Vs), mean blood flow velocity (Vm) and oxidative stress between the two groups ( P>0.05); after treatment, Vs and Vm in acupuncture group were higher than those in conventional group ( P<0.05), and the levels of oxidative stress in acupuncture group were lower than those in conventional group ( P<0.05). Conclusions:Early interventional therapy of acupuncture combined with conventional Western medicine is effective in the treatment of critical cerebral hemorrhage. It can improve cerebral hemodynamics, regulate oxidative stress, reduce the severity of the disease, improve neurological function, motor function and coma, and improve the ability of daily living.
Association of occupational burnout and type of personality in Iranian general practitioners.

Abstract:BACKGROUND Physicians experience high level of occupational burnout that exposes them to physical and mental exhaustion as well as job dissatisfaction. OBJECTIVE The association between different types of personality traits and occupational burnout among a group of Iranian general practitioners is assessed. METHODS One hundred general practitioners working in emergency wards of eight hospitals in Iran were studied. Occupational burnout was measured using the Maslach Burnout Inventory and type of personality was assessed using the Holland personality test. RESULTS An average of 15.4% of personnel displayed high frequency in subscale of emotional exhaustion, 14.5% displayed high frequency in subscale of depersonalization, and 10.2% displayed high frequency in subscale of lack of personal accomplishment. Realistic personality was revealed in 2%, social types in 41%, investigative type in 35%, enterprising types in 6.0%, artistic type in 13% and conventional type of personality in 3.0% of them. The subjects with social personality experienced the lowest burnout. There were negative correlation between physician experiences and different subdivisions of burnout. CONCLUSION A notable number of Iranian general practitioners suffered occupational burnout, especially emotional exhaustion. The majority of physicians had social or investigative personalities.

At 6 months, patients receiving standard care were more likely to have died (adjusted hazard ratio, 4.44; 95% confidence interval [CI], 1.24–15.87); for morbidity outcomes, a 6-point difference in the Physical Component Summary score of the 36-item Short Form Questionnaire (95% CI, 4.2–8.7), a 7-point difference for the Mental Component Summary score (95% CI, 4.5–9.5), a 13-point difference in Modified Barthel Index scores (95% CI, 6.8–18.3), and a 6-point difference in Self-Rating Anxiety Scale scores (95% CI, 4.4–8.3) was reported in favor of the intervention groups.Conclusions—For the first time, we have shown that commencing rehabilitation within 48 hours of intracerebral hemorrhage improves survival and functional outcomes at 6 months after stroke in hospitalized patients in China.Clinical Trial Registration—URL:http://www.chictr.org/en. Abstract:Objective: To explore the effect of early rehabilitation training on motor function and neural function of patients with brainstem hemorrhage after stereotactic individualized operation. Methods: A total of 84 patients with brainstem hemorrhage after stereotactic individualized surgery admitted to Nanyang Central Hospital from January 2020 to January 2022 were selected as the study subjects.The patients were randomly divided into observation group (n=42) and control group (n=42) according to random number table method. The control group received conventional Western medicine treatment, and the observation group received early rehabilitation training on the basis of drug treatment in the control group. The motor function assessment [Fugl Meyer Assessment (FMA) scores], neural function [National Institutes of Health Stroke Scale (NIHSS) scores], ability of daily living [Barthel index (BI) scores], cerebral blood flow [mean blood flow (MBF), mean flow velocity (MFV), peripheral vascular resistance (PVR)] and nerve factor [serum neuron specific enolase (NSE), brain derived neurotrophic factor (BDNF), central nervous specific protein(S100β)] levels were compared between the two groups before and after the treatment. In addition, the rehabilitation effect and complications of the two groups were observed. Results: The total effective rate (95.24%) in the observation group was higher than that in the control group (76.19%%) (P<0.05). After the treatment, the FMA scores, BI scores, MBF, MFV and BDNF levels of the two groups were higher than those before the treatment, and the observation group were higher than the control group (P<0.05). NIHSS scores, PVR, NSE and S100β levels in the two groups after the treatment were lower than those before the treatment, and those in the observation group were lower than those in the control group (P<0.05). The incidence of complications in the observation group [7.14% (3/42)]was lower than that in the control group [23.81% (10/42), P<0.05]. Conclusion: For patients with brain stem hemorrhage after stereotactic individualized surgery, early rehabilitation training can improve the motor, neural function and daily living ability, rehabilitation effect, regulate cerebral hemodynamics and nerve factor levels, and reduce the incidence of complications.

Early decompressive surgery improves long-term prognosis in patients with intracerebral hemorrhage

Abstract:Background: Multiple studies have shown that early decompressive surgery in patients with intracerebral hemorrhage can effectively limit hematoma expansion, reduce perihematomal edema, and improve prognosis. However, these studies are limited by small sample sizes and short follow-up times. Objective: To analyze the effect of early decompressive surgery on the long-term prognosis of patients with cerebral hemorrhage and identify the influencing factors for poor prognosis. Methods: A retrospective analysis of 78 patients with cerebral hemorrhage admitted between January 2020 and December 2022 was conducted. Patients were divided into early and delayed surgery groups for comparison of outcomes such as mortality rate, modified Rankin Scale score, and Short Form-36 scores. Additionally, factors influencing long-term prognosis were analyzed through logistic regression based on significant differences observed between groups. Results: The early decompressive surgery group showed superior outcomes with lower mortality rates, modified Rankin Scale (mRS) scores, hematoma expansion rates, and perihematomal edema volumes compared to the delayed surgery group (P< 0.05). Additionally, age, preoperative Glasgow Coma Scale (GCS) score, preoperative hematoma volume, and a history of hypertension or diabetes were identified as independent prognostic factors for patients with cerebral hemorrhage, with odds ratios (ORs) greater than 1. Conclusions: Early decompressive surgery can improve the long-term prognosis and quality of life of patients with cerebral hemorrhage, reduce mortality rates, and decrease hematoma expansion and perihematomal edema. Older patients, those with higher preoperative hematoma volume and GCS score, and those with coexisting hypertension and diabetes should be given special attention to decrease the occurrence of adverse prognosis.
Effect of minimally invasive surgery for intracerebral hemorrhage: a single center retrospective analysis

Abstract:Objective:To evaluated the safety, efficacy and longterm prognosis of different minimally invasive surgery treatments in patient with intracerebral hemorrhage (ICH).Methods:Patients with basal ganglia hemorrhage, admitted to Neurosurgery Department of Second Affiliated Hospital, School of Medicine, Zhejiang University from January 2019 to December 2020, were divided into three groups: small bone window craniotomy (SBWC) group, endoscopic surgery (ES) group and stereotactic hematoma aspiration followed by urokinase (SHAU) group. The baseline characteristics, perioperative information and lone-term outcome data of the three groups were analyzed to evaluate the safety and effectiveness of different minimally invasive treatments in the treatment of ICH.Results:There were 356 patients in this group, including 116 cases in ES group, 135 cases in SHAU group and 105 cases in SBWC group. No significant differences were observed among all three groups the baseline characteristics, including age, sex, hematoma location, GCS score, NIHSS score and hematoma volume (P>0.05). There were significant differences among the three groups with respect to the operative blood loss and operative time (P<0.05). The rebleeding rate in the SHAU groups was significantly higher than in the other two groups (P<0.05). The evacuation rate 24 h after operation in AMWC and ES groups was significantly higher than SHAU group (P<0.05). The evacuation rate 3 d after operation in ES group was better than that in SBWC group and SHAU group (P<0.05), but there was no significant difference between SBWC group and SHAU group (P>0.05). On 7 d after surgery, the NIHSS score in ES and SHAU groups were significantly higher than the AMWC group, the good prognosis rate in ES and SHAU groups were significantly higher than the AMWC group (P<0.05). Six months after surgery, the NIHSS score in ES and SHAU groups were significantly higher than the AMWC group, the good prognosis rate in ES and SHAU groups were significantly higher than the AMWC group (P<0.05).Conclusion:Minimally invasive surgery has a definite effect on the treatment of ICH and can significantly improve the long-term outcome. Among them, endoscopic surgery has high hematoma removal efficiency and fewer complications.

Abstract:Between 1997 and March 2004, the nervous form, or visna, of maedi-visna infection was diagnosed in 71 of 1631 sheep (4·35 per cent) examined in the Castilla y León region of Spain, of which 634 had shown nervous signs. The presence of the virus was confirmed by immunohistochemistry and in some cases by PCR on frozen-thawed or paraffin-embedded tissue samples. The main clinical signs were hindleg ataxia and paresis, but blindness or nystagmus were also observed. Thirty-three of the affected sheep (46·5 per cent) were two years old or younger. The affected sheep showed variable degrees of a non-suppurative meningoencephalitis, and immunohistochemistry identified positive cells in all cases, with no relation to the intensity of the inflammatory lesion.

Abstract:ABSTRACT Introduction: Acute cerebral infarction refers to the deficiency of the blood supply to the brain and the damage of tissue function. Objective: To study the effect of exercise training in early rehabilitation of patients with hemiplegia treated in the neurology department. Methods: Ninety-six patients with acute cerebral infarction hemiplegia were studied. According to the order of admission, 96 patients were divided into a control group and an early recovery group, with 48 patients in each group. Results: Before early rehabilitation, there was no statistical significance in NIHSS and FUGL-Meyer scores between the two groups (P > 0.05). After early rehabilitation,the NIHSS score of early rehabilitation group was lower than both its pre-early-rehab score and the control group score, while the FUGL-Meyer score was higher than both its pre-early-rehab and the control group score (P 0.05). After early rehabilitation, the GQOLI-74 score was higher in the early rehabilitation group than both its pre-early-rehab score and the control group score (P<0.05). Conclusions: The clinical effect of early rehabilitation training in acute cerebral infarction patients with hemiplegia is significant and can further improve the clinical treatment of patients and their quality of life. Abstract:To investigate whether early rehabilitation has a positive impact on the recovery of the activities of daily living and motor function after intracerebral hemorrhagic stroke, 364 patients with hemorrhagic stroke were selected and randomly divided into a rehabilitation group and a control group. The rehabilitation group underwent a standardized, three-stage rehabilitation program. The control group was treated with standard hospital ward, internal medical intervention. The simplified Fugl-Myere assessment scale (FMA) and Modified Barthel Index (MBI) were administered at various time points. The magnitude of improvement was significantly higher in the rehabilitation group than in the control group for both the FMA (p<0.05) and MBI scores (p<0.05). The greatest improvement was observed in the first month post-stroke. Thus, our study shows that early rehabilitation can significantly improve the daily activities and motor functions of patients with stroke.
Stereotactic puncture surgery combined with early hyperbaric oxygen therapy may be better for long-term functional recovery in patients with moderate amounts of thalamic-inner capsule region haemorrhage

Abstract:Background: The prognosis of patients with thalamic hemorrhage is poor, and their long-term neurological impairment is heavy, which seriously affects their work and life.To comparatively analyse the efficacy and prognosis of patients with moderate hemorrhage in the thalamic region who underwent conservative treatment, stereotactic puncture surgery and neuroendoscopic surgery. Method: This study retrospectively analyzed hospitalization data from 139 adult patients with moderate-volume cerebral hemorrhage in the thalamo-endocapsular region. They were categorized into a stereotactic group (39cases), a neuroendoscopic group (36cases), and a conventional conservative group (64cases). Logistic regression analysis was used to assess risk factors for severe neurological deficits in patients. Multivariate regression modeling was used to compare the correlation of severe neurological deficits among the three groups of patients. Results: Patients with thalamic moderate-volume cerebral hemorrhage had statistically significantly higher Assessment of Daily Living (ADL) scores in the stereotactic surgery group than in the conservative treatment group and the neuroendoscopic surgery group after 6 months of treatment (p< 0.001).The amount of residual hematoma was significantly lower in the surgery groups than in the conservative treatment group at 3 days, 7 days, and 2 weeks after the onset of the disease (P< 0.001).In multivariate logistic regression analyses, after adjusting for all covariates, the odds ratios for severe neurologic dysfunction in the stereotactic group and the neuroendoscopy group were, respectively, OR: 0.37 (0.12-0.87), P< 0.001 and 0.42 (0.23-1.13), P=0.361). Conclusion: In patients with moderate volume cerebral hemorrhage in the thalamus-inner capsule region cerebral hemorrhage, patients treated with stereotactic surgery combined with early hyperbaric oxygen therapy may have better long-term neurological recovery compared with conservative and neuroendoscopic surgical treatments.

Abstract:Cerebral hemorrhage is a common cerebrovascular disease affecting middle-aged and older adults, with an increasing annual incidence. Surgical removal of intracranial hematomas can save the lives of patients with cerebral hemorrhage. However, surgery-induced stress injuries can severely affect the perioperative nutritional status of patients, leading to an adverse prognosis. This study aimed to examine the effect of preventive care on patients with cerebral hemorrhage to improve patient safety during treatment. We found that patients in the experimental group who received preventive care had substantially improved postoperative rehabilitation quality, a decreased incidence of postoperative adverse reactions, and a shortened hospital stay than those in the control group who received routine care. Furthermore, preventive care reduced the risk of malnutrition and considerably improved the nutritional status of patients with CH. Preventive care can effectively improve the postoperative safety of patients with CH, shorten their rehabilitation cycle, promote their body function recovery, and enhance their nutritional status, which is essential in clinical practice.

Abstract:BACKGROUND: To compare the long-term therapeutic effects of stereotactic aspiration (SA), endoscopic evacuation (EE), and open craniotomy (OC) in the surgical treatment of spontaneous basal ganglia hemorrhage and explore the appropriate clinical indications for each technique.METHODS: Multiple-treatment inverse probability of treatment weighting (IPTW)-adjusted logistic regression analysis was performed to evaluate the therapeutic effects of these techniques. The primary and secondary outcomes were 6-month modified Rankin Scale (mRS) and mortality rates, respectively.RESULTS: A total of 703 patients were ultimately enrolled. For the entire cohort, the 6-month mortality rate was significantly higher (OR 2.396, 95% CI: 1.865-3.080), and the 6-month functional outcome was significantly worse (OR 1.359, 95% CI: 1.091-1.692) for SA than that of EE. The 6-month mortality rate for OC was significantly higher (OR 1.395, 95% CI: 1.059-1.837) than that of EE. Further subgroup analysis was stratified by initial hematoma volume and Glasgow Coma Scale (GCS) score. The mortality rate for SA was significantly higher for patients with hematoma volume of 20-40 mL (OR 6.226, 95% CI: 3.848-10.075), 40-80 mL (OR 2.121, 95% CI: 1.492-3.016), and ≥80 mL (OR 5.544, 95% CI: 3.315-9.269) than in the same subgroups of EE. The functional outcomes for SA were significantly worse than that of EE for hematoma volume subgroups of 40-80 mL (OR 1.424, 95% CI: 1.039-1.951) and ≥80 mL (OR 4.224, 95% CI: 1.655-10.776). The mortality rate for SA was significantly higher than that of EE for the GCS score subgroups of 6-8 (OR 2.082, 95% CI: 1.410-3.076) and 3-5 (OR 2.985, 95% CI: 1.904-4.678). The mortality rate for OC was significantly higher for the GCS score of 3-5 subgroup (OR 1.718, 95% CI: 1.115-2.648), and a tendency for a higher mortality rate of 6-8 subgroup (OR 1.442, 95% CI: 0.965-2.156) than that of EE.CONCLUSIONS: EE can decrease the 6-month mortality rate and improve the 6-month functional outcomes of spontaneous basal ganglia hemorrhage in patients with a hematoma volume ≥40 mL. EE can decrease the 6-month mortality rate of spontaneous basal ganglia hemorrhage in patients with a GCS score of 3-8.

Safety and Effectiveness of an Enhanced Recovery Protocol in Patients Undergoing Burr Hole Evacuation for Chronic Subdural Hematoma

Abstract:BACKGROUND AND OBJECTIVES: Enhanced recovery programs may be especially useful in patients with chronic subdural hematoma or hygroma (cSDH), who frequently exhibit frailty and multimorbidity. We aim to evaluate the real-world safety and effectiveness of an enhanced recovery protocol in this population. METHODS: From a prospective registry, burr hole evacuations for cSDH carried out under the protocol (including early thromboprophylaxis, no flat bed rest, early mobilization without drain clamping, and early resumption of antithrombotic medication) were extracted, along with those procedures carried out within the past year before protocol change. Propensity score–based matching was carried out. A range of clinical and imaging outcomes were analyzed, including modified Rankin Scale as effectiveness and Clavien–Dindo adverse event grading as safety primary end points. RESULTS: Per group, 91 procedures were analyzed. At discharge, there was no significant difference in the modified Rankin Scale among the standard and enhanced recovery groups (1 [1; 2] vs 1 [1; 3], P = .552), or in Clavien–Dindo adverse event grading classifications of adverse events ( P = .282) or occurrence of any adverse events (15.4% vs 20.9%, P = .442). There were no significant differences in time to drain removal (2.00 [2.00; 2.00] vs 2.00 [1.25; 2.00] days, P = .058), time from procedure to discharge (4.0 [3.0; 6.0] vs 4.0 [3.0; 6.0] days, P = .201), or total hospital length of stay (6.0 [5.0; 9.0] vs 5.0 [4.0; 8.0] days, P = .113). All-cause mortality was similar in both groups (8.8% vs 4.4%, P = .289), as was discharge disposition ( P = .192). Other clinical and imaging outcomes were similar too (all P > .05). CONCLUSION: In a matched cohort study comparing perioperative standard of care with a novel enhanced recovery protocol focusing on evidence-based drainage, mobilization, and thromboprophylaxis regimens as well as changes to the standardized reuptake of oral anticoagulants and antiaggregants, no differences in safety or effectiveness were observed after burr hole evacuation of cSDH.

Abstract:Objective To evaluate the effect of aescine sodium combined with albumin in the treatment of hypertensive intracerebral hemorrhage after intracranial hematoma minimally invasive removal surgery.Methods 50 patients with hypertensive intracerebral hemorrhage after intracranial hematoma minimally invasive removal surgery were randomly divided into the two groups:the treatment group was treated with aescine sodium and albumin,the control group was treated with the conventional medical drugs;The neural function defect scale,brain edema area of postoperative patients after 1d and 15d,clinical effects after 15d were observed.Results 15d after operation,in the treatment group,the brain edema area was (2.40 ± 0.32) cm2,neural function defect scale was (9.44 ± 2.25) points,which were better than (3.40 ±0.85) cm2 (t =4.721,P ＜0.01) and (15.65 ±3.04) points(t =3.625,P ＜0.01).The total effective rate of the treatment group was 88.9％,which was better than 69.6％ of the control group(x2 =13.58,P ＜0.01).Conclusion Aescine sodium combined with albumin can effectively reduce perihematomal brain edema area,improve nerve function defect and clinical effect in the patients with hypertensive cerebral hemorrhage after minimally invasive scavenging surgery.

Abstract:<span>Background: The intracerebral hemorrhagic stroke (ICH) is associated with high mortality and severe disability in survivors, which causing about 42% of the disability-adjusted life years lost and 50% of all stroke patients dead within 1 year. Although early functional training is recommended to facility rehabilitation after the stroke, the benefit and safety are still controversial.Introduction: This systematic review aims to investigate whether early physical rehabilitation could have a beneficial effect for the patients with ICH compared with standard rehabilitation care.Methods and analysis: Pubmed, Embase, and Cochrane library will be searched to include randomized control trials which investigate the rehabilitation effective of the early mobilization for patients with ICH compared with routine nursing or standard care. Rev-Man version 5.3 will be used to perform all calculations related to the meta-analysis. Dichotomous data will be calculated in terms of a fixed or random effect model and expressed by the relative risk (RR) with 95% confidence interval (CI). The Cochrane collaborations tool in the following aspects was used to assess the risk of bias (ROB) in included studies. Abstract:Objective To explore the safety and effectiveness of very early mobilization on acute cerbral infarction patients .Method T his study w as prospective randomized controlled trial ,100 patients w ith acute ischemic stroke ad‐mitted in the neurology department of Nanjing Drum Towel hospital from January 2014 to January 2015 was select‐ed and randomly divided into routine mobilization group and very early mobilization group .Result There were no dead case at the end of follow‐up ,progressive of nervous functional defects ,cranial hemorrhage after infarct between the two groups .There was no significant differences (P> 0 .05) .Compared with the complications and NIHSS at pa‐tients discharged ,the average length of stay of two groups was significantly differences(P< 0 .05);The ADL and quality of life in the two groups at 14 days after stroke onset or discharged and 3 months after stroke onset was sig‐nificant differences (P<0 .05) .Conclusion Very early mobilization applied for acute cerbral infarction patients was safety ,and might reduce severe complication due to long‐term bed after stroke onset ,and improve neurologic impair‐ment ,promote the speed and extent of activities of daily life recovery and enhance quality of life of patients .
Functional Outcome Analysis of Stereotactic Catheter Aspiration for Spontaneous Intracerebral Hemorrhage: Early or Late Hematoma Evacuation?

Abstract:BACKGROUND:Minimally invasive stereotactic catheter aspiration becoming a promising surgical alternative for intracerebral hemorrhage (ICH) patients. Our goal is to determine the risk factors that lead to poor functional outcomes in patients undergoing this procedure. METHODS:Clinical data of 101 patients with stereotactic catheter ICH aspiration were retrospectively reviewed. Univariate and multiple logistic analyses were used to identify risk factors for poor outcomes 3 months and 1 year after discharge. Univariate analysis was used to compare the functional outcome between early (<48 h after ICH onset) and late hematoma evacuation (≥48 h after ICH onset) groups, as well as for the odd ratios assessment in terms of rebleeding. RESULTS:Independent factors for poor 3-month outcome included lobar ICH, ICH score > 2, rebleeding, and delayed hematoma evacuation. Factors for poor 1-year outcome included age > 60, GCS < 13, lobar ICH, and rebleeding. Early hematoma evacuation was linked to a lower likelihood of poor outcome both 3 months and 1 year post-discharge, but with higher risk of postoperative rebleeding. CONCLUSIONS:Lobar ICH and rebleeding independently predicted both poor short- and long-term outcomes in patients with stereotactic catheter ICH evacuation. Early hematoma evacuation with preoperative rebleeding risk evaluation may benefit patients with stereotactic catheter ICH evacuation.
