Abstract:We investigated the overall situation of LT for BA in mainland China and analyzed their survival outcomes based on data from CLTR. Between January 1996 and December 2013, 509 liver transplants for BA were performed in mainland China and were included in this study. Patients' median age was 9.6 months (range: 4.8-175.2 months). KP was previously performed in 194 cases (38.1%). Grafts from living donors were used in 380 cases (74.7%). Era I (1996.1-2010.12) and era II (2011.1-2013.12) comprised 151 cases (29.7%) and 358 cases (70.3%), respectively. Twenty-five centers had performed at least one liver transplant for children with BA. Centers from Shanghai (197 cases), Tianjin (143 cases) and Beijing (81 cases) involved 82.7% of the 509 cases. One-and five-yr graft survival rates were 84.7% and 72.6%, respectively. Split grafts, center volume <20, GRWR >= 4.0%, and steroid-free immunosuppression regimen were independent risk factors for graft survival. In conclusion, the dramatic expansion of pediatric liver transplant programs in mainland China has enabled improved survival for those children affected by this devastating disease. However, screening of BA in neonates should be emphasized throughout the country to enhance early referrals for KP.
Evaluation of Perioperative Complications in the Management of Biliary Atresia.

Abstract:Purpose: To analyze the influence of perioperative complications in the management of biliary atresia (BA).Methods: A retrospective study was performed using a total of 422 BA patients who underwent Kasai portoenterostomy (KPE) in a single institution between February 2016 and May 2017. Data on patients' clinical characteristics, laboratory examinations, perioperative complications, and outcomes were collected. Unpaired two-tailed t-test and χ2 test were employed for the comparison between BA patients with and without perioperative complications. Cox regression analysis was used to screen the risk factors for 2-years NLS in BA, and their influence on the 2-years NLS was analyzed using Kaplan–Meier survival analysis as well as the log-rank test.Results: The incidence of perioperative complications, 6-months jaundice clearance (JC) and 2-years native liver survival (NLS) rate were 60.4, 59.5, and 56.6%, respectively. Patients with perioperative complications had lower serum albumin (ALB) level, but higher aspartate aminotransferase-to-platelet ratio index (APRI) and international normalized ratio (INR) levels when compared with those without perioperative complications (ALB, P < 0.05; APRI, P < 0.01; INR, P < 0.05). Moreover, perioperative complications were correlated with glucocorticoid administration (P = 0.002). Univariate Cox regression analysis showed no relationship between perioperative complications and 2-years NLS (P > 0.05). However, multivariate Cox regression analysis indicated 6-months JC was an independent protective factor for 2-years NLS [P < 0.0001, hazard ratio (HR) = 0.074, 95% confidence interval = 0.05–0.11], and concordance index of this prediction model including age, weight, APRI, glucocorticoid, and 6-months JC was 0.811.Conclusion: Although perioperative complication is common during and after KPE, it had no influence on the prognosis of BA. However, assessment of the serum level of total bilirubin after KPE may serve as an important predictor for the outcome in BA.

Abstract:Objective:To explore the prognostic factors of biliary atresia (BA) with multicenter data.Methods:Retrospective analysis was performed for clinical data of 569 BA children undergoing Kasai procedure from June 2015 to June 2017 at 11 children's medical centers in China. Clinical data was retrieved and postoperative follow-up period was 2 years after operation. Gender, operative age, preoperative liver function, cytomegalovirus (CMV) infection (CMV-IgM in sera), postoperative cholangitis occurrence duration and frequency, postoperative liver function, bilirubin and native liver survival (NLS) were recorded. Chisquare or two independent sample t-test was employed for univariate analysis and Logistic regression for multivariate analysis.Results:Among them, there were 257 boys and 312 girls with an average operative age of (70.1±22.9)(9-180) days. There were 89 cases with bilirubin completely cleared(total bilirubin <34.2 μmol/L) within 1 month and 248 cases within 3 months postoperatively. And 216 cases (38.0%) had cholangitis postoperatively, including early cholangitis (n=84) and recurrent cholangitis (n=61). The rates of NLS at 1/3/6-month and 1/2-year postoperatively were 97.4%, 77.5%, 58.7%, 46.4% and 38.5% respectively. No significant difference existed in preoperative liver function parameters (ALT, AST, GGT, TBIL & DBIL) between children with NLS and those without NLS (all P>0.05). According to operative age, they were divided into ≤45 d, 46-60 d, 61-75 d, 76-90 d, 91-105 d, 106-120 d and >120 d groups. In each age group, boy vs girl group, cholangitis vs non-cholangitis group, recurrent cholangitis vs and non-recurrent cholangitis group, negative vs positive group of cytomegalovirus infection, 2-year NLS rates showed no difference of statistical significance (all P>0.05). The 2-year NLS rate postoperatively was significantly lower in early cholangitis group than that in non-early cholangitis group (28.6% vs. 40.2%)( χ2=4.094, P=0.043). There was statistical significance. The 2-year NLS rate at Month 3 post-operation was significantly higher in jaundice clearance group than that in jaundice non-clearance group [65.3%(162/248) vs 29.4%(57/194)] and the difference was statistically significant ( χ2=56.248, P<0.001). Multivariate logistic regression analysis indicated that early cholangitis ( OR=2.094, 95% CI: 1.172-3.739, P=0.013) and clearance of jaundice within 3 months postoperatively ( OR=0.214, 95% CI: 0.142-0.322, P<0.001) were independent influencing factors for 2-year NLS rate of BA. Conclusions:Early cholangitis and clearance of jaundice within 3 months after Kasai procedure are the independent influencing factors of BA. Early clearance of jaundice after Kasai procedure for BA and active prevention of early cholangitis may improve the prognosis.

Abstract:Objective: Biliary atresia (BA) is a rare disease and reported outcomes of surgical management, typically a Kasai portoenterostomy (KPE), vary considerably across the world. Centralization has been proposed to improve this. Summary Background Data: A national centralization programme was started in Jan. 1999, involving 3 English units with co-located liver transplant facilities. As the program has now reached the 20-year point, the main aim was to update outcome statistics and identify trends. Methods: Prospective registry and database. The main measures of outcome were (i) time to KPE, (ii) Clearance of Jaundice (CoJ), defined as reaching a bilirubin value of <20μmol/L (≈1.5 mg/dL), and (iii) actuarial native liver survival (NLS) and overall survival (OS). Data are quoted as median (IQR) and non-parametric statistical comparison used with P <0.05 regarded as significant. Results: 867 infants were born with BA and managed between January 1999 and December 2019. Death occurred without intervention (n=10, 1.1%) or were subject to primary transplant (n=26, 3.0%); leaving 831 (95.9%) infants who underwent KPE at median age of 51 (IQR 39-64) days. Age at KPE reduced over the period ( P =0.0001) becoming 48(35-57) days in the last 5-year era. CoJ was achieved in 505/831 (60.6%), also increasing over the period ( P =0.002). 42 (5.0%) died post-KPE and 384 were transplanted, leaving 405 alive with their native livers at last follow-up. Of the 412 children transplanted, there were 23 (5.6%) deaths, leaving 387 alive. 5-year and 10-year native liver survival were 51.3% (95% CI 54.8-47.8) and 46.5% (95% CI 50.1 - 42.9) and overall survival were 91.5% (95% CI 93.2 – 89.4) and 90.5% (95% CI 92.3 – 88.2%) respectively. Conclusions: There have been continued improvements in efficiency over the period of centralization with a significant reduction in time to KPE and improved CoJ following KPE. Overall survival in this disease remains >90%.

Abstract:Background: In patients with biliary atresia (BA), severe portal hypertension (HTN) develops even with successful bile flow restoration, suggesting an intrinsic factor driving portal HTN independent from bile obstruction. We hypothesize that patients with BA have abnormal portal vein (PV) development, leading to PV hypoplasia. Methods: In this observational cohort study, we enrolled patients who were referred to a tertiary center from 2017 to 2021 to rule out BA. Newborns who underwent computed tomography (CT) angiogram as a clinical routine before intraoperative cholangiogram, and laparoscopic Kasai hepatoportoenterostomy. The diameter of the PV and hepatic artery (HA) were compared to the degree of liver fibrosis in the wedge biopsies. The jaundice clearance, native liver survival, and clinical portal hypertensive events, including ascites development and intestinal bleeding, were assessed. Results: 47 newborns with cholestasis were included in the cohort; 35 were diagnosed with BA. The patients with BA had a smaller median PV diameter (4.3 vs. 5.1 mm; p < 0.001) and larger median HA diameter (1.4 vs. 1.2 mm; p < 0.05) compared to the patients with other forms of cholestasis. The median PV and HA diameter did not correlate with the degree of liver fibrosis. Among 35 patients with BA, 29 patients (82.9%) achieved jaundice clearance, and 23 patients (65.7%) were alive with their native liver at two years of age. Seven patients (20%) developed intestinal bleeding, and seven patients (20%) developed ascites, with one overlapping patient. Conclusion: PV hypoplasia is present in patients with BA independent of liver fibrosis at the time of diagnosis.
Long-term outcomes of liver transplantation for biliary atresia and results of policy changes: over 20 years of follow-up experience

Abstract:Objective: Biliary atresia (BA) patients develop chronic liver disease after the Kasai operation and are eventually indicated for liver transplantation (LT). The purposes of this study were to analyze long-term outcomes after LT and risk factors that affect complications to reduce graft failure. Study design: Overall, 145 pediatric patients who underwent LT between June 1996 and June 2020 after a diagnosis of BA were included. We performed a retrospective analysis of medical records and evaluated patient and graft survival, cumulative incidence of complications, risk factors, and the results of policy changes. Results: Patient and graft survival rates in over 20 years were 95.8% and 91.0%, respectively. Post-transplantation lymphoproliferative disease was frequently observed in the early period of immunosuppression within the first 1–2 years after LT. The incidence of cholangitis and rejection steadily increased over time. Weight-to-portal vein size was evaluated as a risk factor for cholangitis and bile duct strictures (OR = 12.82, p = 0.006 and OR = 16.54, p = 0.015, respectively). When evaluated using 2013 as a reference point, the split graft indication was expanded and the group that received LT after 2013 had a significantly lower survival over time compared with that of the group that received LT before 2013 ( p = 0.006). Conclusion: This study revealed time differences in prevalence of complications. The evaluation of weight-to-duct or vessel size is a more important factor in considering complications than the graft-to-recipient weight ratio. Survival outcomes may have been altered by a policy change that affects the donor type ratio in transplantation.

Abstract:Purpose Hepatocyte mitochondrial morphology and gene expression were compared between biliary atresia (BA), infantile cholestasis (IC), and normal liver (NL) as prognostic indicators. Methods Specimens of liver at portoenterostomy (PE) for BA, from intrahepatic bile duct paucity patients for IC, and from choledochal cyst or hepatoblastoma patients for NL were collected prospectively (P) beginning in 2021 (P-BA = 11, P-IC = 9, P-NL = 7) and retrospectively (R) from paraffin-embedded tissue going back to 1981 (R-BA = 25, R-IC = 9, R-NL = 4). The P-cohort had transmission electron microscopy (TEM) to image mitochondria, immunoblotting for heat shock protein 60 (HSP60), and quantitative PCR (qPCR) for HSP60 and mitochondrial functional genes. Both cohorts had immunofluorescence for HSP60 quantified as a ratio to albumin-positive hepatocytes (ALB) with HSP60/ALB<1.0 as a cutoff limit using ImageJ. Results HSP60 was significantly lower in BA/IC than NL on qPCR (BA: p < 0.01, IC: p < 0.05) and lower in BA than IC/NL on immunoblotting (p < 0.05). HSP60/ALB was significantly lower in BA than NL/IC (p < 0.001). Despite BA subjects being matched for types of BA and ages at PE, HSP60/ALB did not correlate with jaundice clearance (JC; T-Bil<1.2 mg/dL) but was significantly higher in native liver survivors (NLS) after PE compared with liver transplant (LTx) cases (p < 0.05) and significantly lower in LTx cases achieving JC than NLS achieving JC (p < 0.05). TEM showed BA had significantly more mitochondrial inclusion bodies (p < 0.05) and significantly larger cristae (p < 0.01) than IC/NL. qPCR in BA showed significant repression of mitochondrial functional genes for mRNA stabilization and energy facilitation. Conclusion HSP60/ALB correlates with NLS after PE for BA. Level of evidence II

Abstract:Biliary atresia (BA) is a rare neonatal cholestatic disorder caused by obstruction of extra- and intra-hepatic bile ducts. If untreated, progressive liver cirrhosis will lead to death within 2 years. Early diagnosis and operation improve the outcome significantly. Infants with neonatal hepatitis syndrome (NHS) present similar symptoms, confounding the early diagnosis of BA. The lack of noninvasive diagnostic methods to differentiate BA from NHS greatly delays the surgery of BA infants, thus deteriorating the outcome. Here we performed a metabolomics study in plasma of BA, NHS, and healthy infants using gas chromatography time-of-flight mass spectrometry. Scores plots of orthogonal partial least-squares discriminant analysis clearly separated BA from NHS and healthy infants. Eighteen metabolites were found to be differentially expressed between BA and NHS, among which seven (L-glutamic acid, L-ornithine, L-isoleucine, L-lysine, L-valine, L-tryptophan, and L-serine) were amino acids. The altered amino acids were quantitatively verified using ultraperformance liquid chromatography tandem mass spectrometry. Ingenuity pathway analysis revealed the network of "Cellular Function and Maintenance, Hepatic System Development and Function, Neurological Disease" was altered most significantly. This study suggests that plasma metabolic profiling has great potential in differentiating BA from NHS, and amino acid metabolism is significantly different between the two diseases.
Surgical outcome and etiologic heterogeneity of infants with biliary atresia who received Kasai operation less than 60 days after birth: A retrospective study.

Abstract:This study aimed to analyze the impact of etiologic heterogeneity and operation age on prognosis of infants with biliary atresia (BA) who received Kasai operation prior to 60 days of age.From 2004 to 2010, 158 infants received Kasai operation before turning 60 days old. According to Davenport 2012 classifications, 4 groups of BA were defined: cystic BA, syndrome BA, and associated malformation, cytomegalovirus (CMV)-associated BA, and isolated BA. Native (autologous) liver survival rates and incidence of cholangitis 2 years after operation, as well as jaundice clearance rates 3 months after operation, were recorded.Although infants who received the operation between 51 and 60 days of age had a better jaundice clearance 3 months after operation and lower incidence of cholangitis as compared with those under 40 or between 41 and 50 days of age, there was no significant difference in survival rates. Among types of BA, infants with cystic BA had the best prognosis. In the syndrome BA and associated malformations group, as well as CMV-associated group, infants who received the operation early (<40 days of age) had a worse outcome as compared with those who received the operation between 41 and 50 days or 51 and 60 days of age.Both clinical etiologic heterogeneity and operation age may influence BA prognosis.
Effects of different admission pathways on the diagnosis and treatment of biliary atresia

Abstract:Objective To explore the effects of different admission pathways of internal medicine or sur-gery on the diagnosis and treatment of biliary atresia (BA). Methods From March 201 5 to March 201 6,273 obstructive jaundice patients with suspected BA and laparoscopic bile duct exploration were recruited.They were assigned into 2 groups by different admission pathways:237 cases admitted directly into surgical group while 36 cases internal medicine initially and then transferred into surgical group.Retrospective analyses were performed by operative age and preoperative hospitalization,accuracy of BA diagnosis,jaundice clearance (to-tal bilirubin <20 μmol/L)at 3 months after Kasai procedures and total hospitalization stay and expenditure. Results Among them,202 patients of surgical group and 26 patients of medical group were diagnosed as BA by laparoscopic bile duct exploration.The average operative age of surgical group was younger than that of medical group (66.50 ±20.52 d vs 72.58 ±27.01 d).While there was no statistical significance (P=0.31 9 6 >0.05 ).However,as compared with medical group,surgical group had shorter preoperative hospitalization (2.21 ±1 .1 2 d vs 6.85 ±3.52 d,P=0.000 0)and higher diagnostic accuracy of BA (85.23%vs 72.22%, P=0.050).Jaundice clearance at 3 months after Kasai procedure had no inter-group statistical difference (P>0.05).In surgical group,total hospitalization stay was much shorter (1 8.07 ±5.62 d vs 21.63 ±3.92 d,P<0.000 1 )and the expenditure was lower (34 1 54.66 ±7 457.43 yuan vs 39 657.52 ±6 1 63.36 yuan,P<0.000 1 )than medical group. Conclusions Different admission pathways of internal medicine or surgery have shown not much effect on the operative age of BA patients.But admission into surgery department can help shorten hospitalization stay and reduce hospitalization experditure.
A morphometric study of hepatic artery branches in biliary atresia

Abstract:Objective To evaluate the wall thickness and inner diameter of arterial branches at portal area of biliary atresia (BA) and seek its clinical correlation.Methods From June 2012 to March 2014,49 BA children undergoing Kasai procedure at our hospital were included.And the characteristics of arterial branches were morphometrically evaluated.And the controls were patients with intrahepatic cholestasis (n =10).The follow-up period was at least 6 months.They were divided into excellent,favorable and unfavorable groups.According to the age at Kasai procedure,they were divided into ≤60 days,60-90 (includ 90) days and ＞90 days groups.The relevance of age during Kasai procedure,short-term prognosis and hepatic arterial branch morphology was analyzed statistically.Results The authors evaluated 182 arterial branches (152 in BA,30 in controls).The arterial wall thickness of portal area was (83.58 ± 28.30) and (72.13 ± 16.59) μm respectively.And the inner diameter of arterial branches was (136.32 ± 52.64) and (113.94 ± 41.94) μm respectively.The arterial wall thickness of excellent,favorable and unfavorable groups was (83.03 ± 29.42),(78.16 ± 25.06) and (87.00 ± 28.61) μm respectively.The inner diameters of arterial branches were (138.92± 52.96),(125.81 ± 45.04) and (138.88 ± 56.00) μm respectively.The arterial wall thickness of ≤60 days,60-90 (includ 90) days and ＞90 days groups was (87.02 ± 23.66),(81.07 ± 29.94) and (83.08 ± 31.60)μm respectively.The inner diameter of arterial branches was (148.27 ± 54.11),(126.13 ± 47.97) and (137.16± 56.07) μm respectively.The arterial wall of BA cases was thicker than that of infants with intrahepatic cholestasis (P =0.038).The branches of hepatic artery were dilated in BA cases (P =0.026).There was a significant positive linear correlation between arterial wall thickness and inner diameter of arterial branches at portal area.Short-term prognosis was not associated with morphological changes of hepatic arterial branches.No statistical difference existed between age during Kasai procedure and changes of arterial branches at portal area.Conclusions In BA,thickening of arterial wall and dilation of arteries may be found in periportal area.And a significant positive linear correlation exists between arterial wall thickness and inner diameter of arterial branches at portal area of BA.

Abstract:Background Kasai portoenterostomy (KPE) remains the primary intervention for biliary atresia (BA), but its outcomes are highly variable. Reliable prognostic biomarkers remain elusive, complicating the management and prediction of postoperative progression. Method Liver biopsies from BA patients taken at and after KPE (post-KPE) were used to generate organoids for RNA-sequencing analysis. Control organoids were derived from non-BA livers. Differential gene expression and enrichment analyses were performed to assess post-KPE transcriptomic changes between native liver survivors (NLS) and patients who eventually became liver transplant recipients (LTR). Results Organoid datasets: 70 from liver biopsies at KPE (10 patients), 112 from post-KPE livers (13 livers; 12 patients), and 47 from control livers (9 patients). At KPE, BA organoids displayed mainly hepatocyte expression, a trait notably reduced in control organoids. Similarly, post-KPE organoids from NLS revealed a significant decrease in hepatocyte expression features and an overall increase in cholangiocyte expression features. A similar hepatocyte-to-cholangiocyte expression transition was evidenced in paired liver organoids (at- and post-KPE) generated from an NLS. In contrast, post-KPE organoids from LTR maintained a high level of hepatocyte expression features. Conclusion Our study demonstrated that an elevated expression of hepatocyte features in KPE organoids may indicate aberrant cholangiocyte development in BA livers. In contrast, a post-KPE hepatocyte-to-cholangiocyte expression transition in NLS may imply effective biliary recovery. The lack of this transition in LTR organoids indicates ongoing disease progression, highlighting the potential for organoid-based transcriptomic profiling to inform KPE success and guide BA management. Abstract:Objective: There are subclinical cardiac abnormalities (SCA) in children with biliary atresia (BA). However, data on the consequences of these cardiac changes after liver transplantation (LT) remain controversial in the pediatric field. We aimed to determine the relationship between outcomes and the subclinical cardiac abnormalities in pediatric patients with BA based on two-dimensional echocardiography (2DE) parameters. Methods: A total of 205 children with BA were enrolled in this study. The relationship between 2DE parameters and outcomes, including death and serious adverse events (SAE) after LT, was analyzed by regression analysis. Using receiver operator characteristic (ROC) curves to determine the optimal cut-off values of 2DE parameters for outcomes. Differences in the AUCs were compared using DeLong's test. The Kaplan -Meier method with log-rank testing was used to evaluate survival outcomes between groups. Results: Left ventricular mass index (LVMI) and relative wall thickness (RWT) were found to be independently associated with SAE (OR: 1.112, 95% CI: 1.061 − 1.165, P < 0.001 and OR: 1.193, 95% CI: 1.078 − 1.320, P = 0.001, respectively). The cutoff value of LVMI for predicting the SAE was 68 g/m2.7 (AUC = 0.833, 95% CI 0.727-0.940, P < 0.001), and the cutoff value of RWT for predicting the SAE was 0.41 (AUC = 0.732, 95% CI 0.641-0.823, P 68 g/m2.7, and/or RWT > 0.41) was associated with lower patient survival (1-year, 90.5% vs 100.0%; 3-year, 89.7% vs 100.0, log-rank P = 0.001). and higher incidence of SAE events. Conclusions: Subclinical cardiac abnormalities were correlated with post-LT mortality and morbidity in children with BA. LVMI can predict the occurrence of death and serious adverse events after liver transplantation.

Abstract:Biliary atresia (BA) is a fibroinflammatory disease of the intrahepatic and extrahepatic biliary tree. Surgical hepatic portoenterostomy (HPE) may restore bile drainage, but progression of the intrahepatic disease results in complications of portal hypertension and advanced cirrhosis in most children. Recognizing that further progress in the field is unlikely without a better understanding of the underlying cause(s) and pathogenesis of the disease, the National Institutes of Diabetes and Digestive and Kidney Diseases (NIDDK) sponsored a research workshop focused on innovative and promising approaches and on identifying future areas of research. Investigators discussed recent advances using gestational ultrasound and results of newborn BA screening with serum direct (conjugated) bilirubin that support a prenatal onset of biliary injury. Experimental and human studies implicate the toxic properties of environmental toxins (e.g., biliatresone) and of viruses (e.g., cytomegalovirus) to the biliary system. Among host factors, sequence variants in genes related to biliary development and ciliopathies, a notable lack of a cholangiocyte glycocalyx and of submucosal collagen bundles in the neonatal extrahepatic bile ducts, and an innate proinflammatory bias of the neonatal immune system contribute to an increased susceptibility to damage and obstruction following epithelial injury. These advances form the foundation for a future research agenda focused on identifying the environmental and host factor(s) that cause BA, the potential use of population screening, studies of the mechanisms of prominent fibrosis in young infants, determinations of clinical surrogates of disease progression, and the design of clinical trials that target subgroups of patients with initial drainage following HPE. Abstract:This study aimed to analyze the impact of age at Kasai operation on the short-term outcome of type III biliary atresia (BA) and to discuss if pathological scoring can be the prediction of effect in the Kasai procedure for the older (≥ 90 days) infant.During the period 2004 through 2010, 452 infants with type III BA were reviewed. The relationship between ages at Kasai operation and jaundice clearance rates and two year native liver survival rates were assessed, retrospectively. Pathological slides were analyzed with a histological scoring system.All of the patients were divided into 3 groups according to their ages at operation (group A: aged 60 days or less (n=146), group B: age between 60 and 90 days (n=222) , and group C: age on or over 90 days (n=84)). The worst outcome of clearance of jaundice was found in group A but not in group C 2 weeks after the operation (P<0.05). Jaundice clearance rates showed no difference among the three groups either at 3-months or 6-months after operation. Moreover, in group C patients, the pathological scores showed no difference between the jaundice clearance group and jaundice persistence group 6 months after surgery. In group C, two year survival rate of patients with native livers was 36.1%.Some patients over 90 days of age at surgery can still benefit from a Kasai procedure. The pathological scoring system does not play a role in predicting jaundice clearance in patients over 90 days of age at surgery.

Abstract:14540 Background: Expression of β-catenin and UDP-glucose dehydrogenase (UGDH) is markedly altered in prostate adenocarcinomas. The objective of this work was to evaluate expression of these proteins in normal appearing acini (NAA) of cancer-bearing prostate glands, as potential biomarkers of PC field disease. METHODS Archived core biopsies of 42 pairs of PC cases and controls were labeled for β-Catenin, and a tissue microarray of 10 prostatectomy specimens of cancer cases and controls were labeled for UGDH. A fluorophore-conjugated secondary antibody was incorporated into the staining protocol. Fluorescence in prostatic acini was measured with an automated Leica microscope and Image Pro Plus software and expressed as mean pixel intensity (MPI) per acinus and average MPI (AMPI) per case or control. RESULTS Mean AMPI of β-catenin in NAA of 33 cases (545 ± 93) was lower (p < 0.01) than that of normal acini (NA) in age-matched controls (684 ± 90), whereas mean AMPI of NAA in the remaining 9 cases (712 ± 111) was higher (p = 0.003) than that of the matched controls (634 ± 95). Twelve cases under-expressing β-catenin in NAA had AMPIs below the lowest AMPI (506 ± 83) of all controls. Mean AMPI of UGDH in NAA of 7 cases (372 ± 169) was lower (p < 0.05) in comparison to both NA (558 ± 119) and cancerous acini (CA) (645 ± 88). The difference between CA and NA did not achieve statistical significance. CONCLUSIONS Reduced expression of β-catenin is a cancer-associated event in NAA. A cut-off AMPI (506) for β-catenin detected 29% of PC cases with 100% specificity. Altered expression of UGDH in NAA is a potential biomarker of PC. Funded by DoD grant DAMD17-02-1-0121. A Nomogram Predicting the Prognosis of Children With Biliary Atresia After Hepatoportoenterostomy

Abstract:Background: Although Kasai portoenterostomy (KPE) is performed timely for most children with biliary atresia (BA), the native liver survival (NLS) is still poor due to the progressive liver fibrosis. Many children have to receive liver transplantation (LT) within 2 years after KPE. Early prediction of the prognosis permits the implementation of prophylactic treatments for BA children. However, studies about the prediction are limited. Objective: The purpose of this study is to establish a nomogram to predict the prognosis of BA children within 2 years after KPE. Methods: The follow-up data of 151 BA children were retrospectively reviewed, and were randomly divided into a training cohort for constructing a nomogram ( n = 103) and a validation cohort ( n = 48). In the training cohort, patients were divided into Group A and Group B according to whether death or LT were observed within 2 years post-KPE. Multivariate Cox regression based on the baseline characteristics, liver function indicators and LSM (liver stiffness measurement) values at KPE and 3 months after KPE was utilized for the establishment of the nomogram in predicting the prognosis of BA within 2 years after KPE. The discrimination and calibration of the nomogram were internally and externally validated. Results: Fifty-six BA children were included in Group A and 47 were included in group B. Age at KPE, METAVIR score F4, LSM at 3 months, first onset of cholangitis within 3 months, and jaundice clearance time were the independent predictors for the prognosis of BA children within 2 years after KPE (all P < 0.05). The developed nomogram based on these independent predictors showed good discrimination and calibration by the internal and external validation. Its performance was better than each predictor in predicting the prognosis (all P < 0.05). Conclusions: The established nomogram based on the indicators from the first 3 months after KPE may be useful for predicting the prognosis of BA children within 2 years post-KPE and helpful for the consideration of LT.

Abstract:Purpose: Cholangitis after Kasai procedure has been previously shown to be related to poor prognosis in Biliary Atresia (BA). To investigate the risk factors and clinical outcomes of cholangitis, we did a retrospective study in post-Kasai BA patients. Methods: Two-year follow-up data of 180 type-III BA patients after Kasai procedure in 2016 in our hospital were analyzed, including 119 cholangitis patients (66.11%). Among the cholangitis group, patients were further divided into early vs late cholangitis and single vs recurrent cholangitis groups. Liver pathology, liver function, cholangitis occurrence and frequency, jaundice clearance, and survival rates were examined. Results: Higher gamma-glutamyl transferase level before Kasai is a risk factor for cholangitis (p = 0.0393). Older age and higher liver fibrosis score at Kasai are risk factors for recurrent cholangitis (p < 0.05). Shorter prophylactic intravenous antibiotics usagemay contribute to early cholangitis, which may lead to higher cholangitis frequency (p < 0.0001). Recurrent cholangitis is associated with earlier cholangitis onsets (p < 0.0001). Cholangitis patients have a relatively delayed jaundice clearance, while early and recurrent cholangitis may contribute to lower overall survival. Conclusions: Personalized treatment considering risk factors in individual BA patients is needed to prevent cholangitis, especially early onsets, and to improve postoperative outcomes.
