Abstract:Juvenile primary Sjögren syndrome (pSS) with renal involvement is extremely rare, reported approximately in 50 children, predominantly girls. Here, we present the first reported case of a male child with juvenile pSS with ocular surface disease (previously keratoconjunctivitis sicca), submandibular salivary gland involvement, and tubulointerstitial nephritis. First, two symptoms were clinically apparent at presentation. We illustrate here that kidney involvement in pSS should be actively looked for, as juvenile pSS may be associated with asymptomatic renal involvement. Immunophenotyping of peripheral blood cells using multicolor flow cytometry revealed at the time of diagnosis changes in both adaptive (T memory cells and B memory cells), and innate immunity (an increased activation of natural killer cells, as well as monocytes and neutrophils, and an increased representation of intermediate monocytes). Our case report points to the importance of kidney examination, early diagnosis and therapy in juvenile pSS, as well as highlights international collaboration to obtain more data for this rare disease.

Abstract:Objective: Sjögren's syndrome (SS) is a rare disease with unclear diagnostic criteria among the children and adolescents. The purpose of this study is to describe the clinical features of pediatric Sjögren's syndrome and validate with Japanese diagnostic guidelines criteria of 2018. Methods: We conducted a retrospective analysis of the clinical data of a cohort of 54 patients with pediatric Sjögren's syndrome admitted to our hospital over a total of 10 years from September 2013 to September 2022. Results: The ratio of females to males was 49:5 among the 54 children (34 cases of primary SS and 20 cases of secondary SS), the average age of onset of symptoms for the first time was 9.9 years, and the average age at diagnosis was 10.2 years. In terms of subjective symptoms, 7 cases (13.0%) presented with dry mouth and 5 cases (9.3%) reported dry eyes. The positive rates were 9.3% for Schirmer I test, 70.4% for salivary gland function test, and 55.6% for salivary gland ultrasonography. The positive rates were 94.4% for Anti-Ro/SSA antibodies, 66.7% for Anti-La/SSB antibodies, 88.9% for ANA, 59.3% for RF, and the elevation rate of IgG was 63.0%. Among the EULAR Sjögren's syndrome disease activity index (ESSDAI) domains, the biological, constitutional, glandular, cutaneous, and lymphadenopathy domains were most involved. Treatment consisted of glucocorticoids in 88.9% of the patients in our study and hydroxychloroquine in 92.6%. As per the Japanese version of the clinical practice guidance for Sjögren's Syndrome in pediatric patients (2018), 5 cases were identified as Definite SS, 35 cases as Probable SS, and 14 cases as Possible SS. With respect to primary and secondary SS, there was essentially no significant difference between the groups in any of the above aspects. Conclusions: Patients with pediatric SS presented with a wide spectrum of clinical features, a low prevalence of reported symptoms of dry mouth and dry eyes, and various clinical manifestations with multi-system involvement. These are similar to other pediatric study cohorts in terms of epidemiology, auxiliary investigation results, disease activity scores, and treatment. The coincidence between our study and the Japanese version of the clinical practice guidance for Sjögren's Syndrome in pediatric patients (2018) is good for the diagnosis of pediatric SS.

Abstract:AbstractObjectives To characterize clinical features and outcome of childhood-onset primary Sjögren’s syndrome (pSS). Methods Patients less than 18 years-old, fulfilling the 2002 the American-European Consensus Group (ACEG) criteria or 2016 the American College of Rheumatology/European League Against Rheumatism (ACR/EULAR) criteria for pSS, or 1999 proposed Juvenile pSS criteria, were included. Electronic medical records of pSS from 2013 to 2020 were collected and analyzed. Results 39 patients were included. Of them, 27 (69.2%), 38 (97.4%) and 35 (89.7%) patients fulfilled the AECG criteria, ACR/EULAR criteria and proposal juvenile pSS criteria. The female: male ratio was 3.9:1. Median age at first signs or symptoms and at diagnosis were 9.2 (4.7, 14.5) years and 10.9 (6.3, 15.0) years. The main clinical manifestations were rash or purpura (20, 51.3%), followed by fever (12, 30.8%), glandular enlargement/recurrent parotitis (10, 25.6%), and dry mouth and/or dry eyes (9, 23.1%). 28 (56.4%) patients existed systemic damage, the most common was hematological involvement (14, 35.9%), followed by hepatic (13, 33.3%) and renal involvement (8, 20.5%). 38 (97.4%) patients preformed labial minor salivary gland biopsy, all exhibited focal lymphocytic sialadenitis. All patients had global ESSDAI score ≥ 1 at diagnosis, the median total score at diagnosis was 7 (2, 27). 36 (92.3%) patients followed up at a median time of 23.6 (7.9, 79.5) months, three patients developed systemic lupus erythematosus (SLE) at follow-up time of 13.3, 38.8 and 63.8 months, separately. Conclusions The presentation of childhood-onset pSS is atypical, extraglandular manifestations and systemic involvements were more common. Labial salivary gland biopsy is vital for probable patients. Some patients may develop SLE over time.

Abstract:Objective:Renal involvement is an important clinical concern in patients with primary Sjgren's syndrome(pSS).In this study,we investigate the clinical features of the renal involvement and the pathological characteristics of renal lesion in pSS by retrospective analysis of 14 pSS patients who underwent renal biopsy in our clinic. Methodology:Fourteen pSS patients,all female,aged between 23 and 56,were included in this retrospective analysis.Renal biopsy was performed in all of the 14 patients (4～120) months after the onset of the disease.Clinical data with respect to tubulointerstitial function and autoimmunity,and renal pathological data with respect to grades of glomerular and tubulointertitial lesion were analyzed,and the co relationships among clinical and pathological parameters were investigated. Results:①Clinically,all the patients which were middle age women,and renal manifestation included hyposthenuria (14/14,100%),renal tubular acidosis(13/14,92 9%),mild proteinuria (8/14,57 5%) and microscopic hematuria (3/14,21 4%).In the 13 patients with renal tubular acidosis,8 were classified as Type Ⅰ,2 as Type Ⅱ,2 as mixed type and 1 as incomplete distal RTA.②Immunological study revealed that 13 of the 14 had multiple positivities(+) for autoantibodies.Increased serum IgG was found in 8 of 11 patients,elevated serum cryoglobulin in 6/12,hyperglobulinemia in 10/14,hypergamalobulinemia in 7/7.③Renal histology confirmed that the prominent lesion located in the tubulointerstital area,with immunodeposits neither in glomeruli nor along tubular basement membrane,nor on any vessel wall.Correlation analysis indicated that renal tubular lesion correlated significantly with the disease course,hyperglobinemia,and cryoglobinemia. Conclusion:Renal involvement in pSS patients is characterized clinically by prominent tubular acidosis and decreased urinary osmolarity,and pathologically by the chronic tubularinterstitial lesion as increased intertubular area and cellular deeper intersitial infiltration plus tubular cell degradation.

Abstract:Objectives Paediatric Sjögren's syndrome (pSS) is a rare chronic autoimmune disorder, characterised by inflammation of exocrine glands. B cell hyperactivation plays a central role in adult-onset Sjogren. This study was designed to analyse B cell and T cell phenotype, levels of BAFF, and selection of autoreactive B cells in patients with pSS. Methods A total of 17 patients diagnosed with pSS and 13 healthy donors (controls) comparable for age were enrolled in the study. B cell and T cell subsets and frequency of autoreactive B cells in peripheral blood were analysed by flow cytometry. Levels of BAFF were analysed by ELISA. Results The relative frequency of total B cells, transitional, naïve and switched memory B cells was similar between pSS patients and controls. In patients with pSS, we observed a reduction in the frequency of unswitched memory B cells, an increased frequency of atypical memory B cells and an expansion of PD1 hi CXCR5 – T peripheral helper cells. Levels of BAFF were higher in patients with pSS compared with controls and correlated with serum levels of total IgG and titres of anti-Ro antibodies. The frequency of autoreactive B cells in the transitional, unswitched memory and plasmablast compartment was significantly higher in pSS patients than in controls. Conclusions Our results point to a hyperactivation of B cells in pSS. Current therapies do not seem to affect B cell abnormalities, suggesting that novel therapies targeting specifically B cell hyperactivation need to be implemented for paediatric patients.

Clinicopathologic Characteristics and Prognosis in Primary Sj(o)gren's Syndrome with Renal Involvement

Abstract:Objective:To analyze clinical manifestations,immunological characteristics,pathological changes and prognosis in primary Sjgren's syndrome patients with renal involvement. Methodology:Sixty-two primary Sjgren's syndrome patients with renal involment,female/male 60/2 cases and average age of(36.8±10.7)years old,were enrolled in this retrospective study.Their clinical manifestations,renal pathological features and prognosis were analyzed. Results:88.1% cases showed a decrease in urine osmosis,49 cases(79%) renal tubular acidosis(RTA),including type Ⅰ 44 cases(89.8%),type Ⅱ 2 cases(4.1%) and type Ⅲ 3 cases(6.1%).47 cases(75.8%) presented with hypokalemia.9 cases(14.5%) with nephrogenic diabetes insipidus and one case(1.6%) with Fanconi syndrome.43 cases(69.4%) had urinary protein excretion from 0.42～7.51 g/d,of which 41(95.3%) had proteinuria less than 2 g/d,and 2(4.7%) more than 3.5 g/d.4 cases(6.5%) had microscopic hematuria,9(14.5%) chronic renal insufficiency,and 45(72.6%)had hyperglobulinaemia.The positive rate of serum autoantibody levels were as follows: anti-SSA(86.0%),ANA(77.4%),anti-SSB(52.6%),RF(39.5%),anticardiolipin antibodies(21.6%),and anti-RNP antibody(15.3%).In histological examinations,56 cases(90.3%) presented with interstitial nephritis,6 combined with mesangial proliferation and IgA deposition,2 combined with membranous nephropathy,and 6(9.7%) presented with normal.56 cases were followed up(37.5±38.0) months,all of 5 and 10 years renal and patient survival were 100%. Conclusion:Urine concentration ability decline and RTA with hypokalemia were most common in primary Sjgren's syndrome patients with renal involvement.The interstitial nephritis was the major pathological changes and could be combined with mesangial proliferation and IgA deposition,membranous nephropathy.Their long-term prognosis was good.
Juvenile systemic sclerosis - observations of one clinical centre

Abstract:Objectives: The systemic form of scleroderma (SSc) in children is a very rare disease; therefore, it is recognized relatively late, which increases the risk of complications. The aim of the study was to assess the clinical symptoms of juvenile systemic sclerosis (JSSc) in our cohort patients. Material and methods: A group of (N = 22) scleroderma patients aged between 2 and 16 years were observed. Demographic data and all clinical results obtained during 16 years of observation in the clinic of rheumatic diseases of developmental age were collected and analysed. Results: In all observed children the major JSSc criterion was found, i.e. skin thickening proximal to the metacarpal phalangeal and/or metatarsophalangeal joints. Other symptoms are presented as follows: nailfold capillary abnormalities - 100%, Raynaud's phenomenon - 90.9%, sclerodactyly - 27.3%, digital tip ulcers - 27.3%, dysphagia - 18.2%, gastroesophageal reflux - 27.3% (assessed in only 10 children), arrhythmias - 22.7%, heart failure - 9.1%, new-onset arterial hypertension - 9.1%, pulmonary fibrosis - 72.7%, pulmonary arterial hypertension - 9.1%, neuropathy - 13.6%, carpal tunnel syndrome - 4.5%, tendon friction rubs - 4.5%, arthritis - 22.7%, and myositis - 13.6%. There were no cases of renal crisis. Decreased diffusing capacity of oxygen was confirmed in 12 patients (58.3%). The presence of antinuclear antibodies was noticed in 86.7% of patients, and among SSc selective autoantibodies: anticentromere - 31.8%, anti-topoisomerase I - 18.2%, anti-PM-Scl 100 or 75 - 45.5%, anti-RP11, Th/To, PCNA in total in 27.3% were presented. In 4.5% of cases, apart from the presence of anti-PM-Scl autoantibodies, positive lupus band test, reduced concentration of complement, and antiphospholipid antibodies were also found. In 59% of studied children, the body mass index was below the 25th percentile. Conclusions: The presented retrospective analysis shows that the occurrence of Raynaud's phenomenon with changes in nailfold capillaroscopy is the best screening toll for the assessment of risk of JSSc. All patients of developmental age with Raynaud's phenomenon, especially in the case of the appearance of antinuclear antibodies, should be monitored with capillaroscopy regardless of other laboratory or imaging tests.

Abstract:Aims/Purpose: Juvenile Systemic Lupus Erythematosus (jSLE) is a chronic multisystem inflammatory disease that manifests before the age of 16 years, following a remitting ‐ relapsing course. The range of clinical presentations of juvenile Systemic Lupus Erythematosus (jSLE) in children is multifaceted. Ocular manifestations are uncommon for children with jSLE but can have a significant effect on their vision if not diagnosed early. Any part of the visual pathway can be affected by jSLE. The aim of this report is to systematically review the ocular manifestations of jSLE. Methods: A systematic review according to Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA) 2020 Guidelines was conducted searching MEDLINE and Scopus, including reference lists with appropriate terms for jSLE ocular manifestations. Epidemiological studies, preferably cross‐sectional studies, that describe the prevalence of ocular manifestations in children with a diagnosis or upon diagnosis of jSLE were assessed. Results: Our search yielded 131 results, screened by two independent reviewers. 33 studies were finally included in the review after preliminary assessment. Keratoconjunctivitis sicca, dry eye syndrome, retinopathy and optic neuritis are among the conditions that have been reported. Ocular manifestations in jSLE may be the result of three different mechanisms: the disease itself, infections related to immunosuppression and side effects of drug therapy. Retinal involvement is one of the most important manifestations, implying high possibility of visual loss. It is usually caused by immune complexes and autoantibodies deposition to the retinal vessels or hypertension resulting from kidney disease. Conclusions: Ocular manifestations can occur early in the course of jSLE, sometimes even before other systemic symptoms and can serve as markers of disease activity. Diagnosing ocular manifestations in children with jSLE is important for early intervention, monitoring disease activity, preventing vision loss and providing comprehensive care. By addressing ocular manifestations alongside systemic lupus management, healthcare providers can optimize outcomes and improve the overall well‐being of children with jSLE.

Abstract:Abstract Systemic lupus erythematosus is a rare lifelong multi-systemic autoimmune condition. Juvenile-onset SLE (JSLE) is recognized to have a more active disease course when compared with adult-onset disease and patients have a worse long-term survival. Kidney involvement occurs in over 50% of children and treatment decisions are guided by the histological classification. Several international groups have produced treatment protocols that rely on an intense period of immunosuppression to halt the acute kidney inflammatory process, followed by maintenance therapy with close observation for disease improvement and prompt evaluation of disease flares. A reduced glomerular filtration rate at presentation is predictive of later stage chronic kidney disease (CKD) in multivariate analysis. Kidney remission remains suboptimal with only 40–60% of patients achieving complete remission. Kidney flares are seen in over a third of patients. The rate of CKD 5 is reported to be up to 15% and the presence of lupus nephritis (LN) has an established link with an associated increase in mortality. In established kidney failure, transplantation seems to be the optimal kidney replacement modality for this group of patients, ideally after a period of disease quiescence. Modified outcome measures in clinical trials have demonstrated that biologic agents can be effective in this disease. Current biologic agents under investigation include obinutuzimab, belimumab, atacicept, anifrolumab, tocilizumab, eculizumab, dapirolizumab, and abatacept. Future research should focus on discovering early disease biomarkers, including surrogates for later cardiovascular disease, and evaluating biological agents as adjuncts to improve the rates of complete remission and subsequently influence the kidney outcome. The aim of this review article is to summarize the current kidney outcomes for this disease with a view to identifying key areas that may help to reduce the risk of long-term CKD.

Spectrum of renal disease in scleroderma other than scleroderma renal crisis: A review of the literature

Abstract:Background: Systemic sclerosis (SSc) is a multi-system rheumatic disease characterized by vascular and fibrotic manifestations that can affect practically every organ. Scleroderma renal crisis (SRC) is the most common renal manifestation of SSc. However, with the use of angiotensin-converting enzyme inhibitors (ACEi), the morbidity and mortality associated with SRC has significantly reduced. Renal manifestations in SSc other than SRC have been generally under-recognized and can be left untreated, which can lead to grave consequences in this patient population. In this article, we will describe the spectrum of renal disease in SSc besides SRC. Materials and methods: A literature search was conducted on PubMed and Cochrane from inception to December 2022 using medical subject headings (MeSH) terms for "scleroderma", "systemic sclerosis" combined with "renal injury", and "renal dysfunction". We included case reports, case series, observational studies, and literature reviews. Results: The initial search revealed 393 articles. After the exclusion of duplicates and non-relevant articles, data was included from 30 articles and 45 patients. The mean age was 55.2 years, 9 males (20%) and 36 females (80%). The most reported renal manifestations included: ANCA-associated vasculitis (n = 22), penicillamine-induced renal injury (n = 8), oxalate nephropathy (n = 5), Goodpasture syndrome (n = 4), nephrotic range proteinuria (n = 2), renal artery stenosis (n = 2), membranous glomerulonephritis (n = 1), and Evans syndrome (n = 1). Conclusion: The spectrum of kidney involvement in SSc can range from asymptomatic reduction of the glomerular filtration rate to life-threatening scleroderma renal crisis. Therefore, it is essential that physicians closely monitor renal function in these patients for any emerging renal dysfunction.

Abstract:OBJECTIVE:To observe the clinical features of primary Sjogren syndrome (pSS) in young people (< or =34 years).METHODS:The data of 232 patients with pSS who were in hospital from 2004 to 2008 were analysed in a prospective study.RESULTS:A total of 23.7%(55/232) patients' symptoms came on in youth (< or =34 years), who were all females, mean age was (27.6+/-6.2) years. The other 177 patients' symptoms came on when they were more than 34 years old. In young group, 32.73%(18/55) patients' first symptoms were xerostomia and/or keratoconjunctivitis sicca, the other 67.27%(37/55)came on without xerostomia and keratoconjunctivitis sicca, while in the middle-aged and aged group, 59.32%(105/177) patients' first symptoms were xerostomia and/or keratoconjunctivitis sicca, 40.67% (72/177)without xerostomia and keratoconjunctivitis sicca. There was significant difference(P=0.001). The duration from first symptom to first diagnosis of pSS in the young group was (93+/-107) months, which was significantly longer than those of the middle-aged and aged group [(45+/-59) months, P=0.001]. The systemic damage of the young group was 61.8%(34/55), which was significantly greater than 41.24%(73/177)in the middle-aged and aged group(P=0.026).CONCLUSION:Primary Sjogren syndrome in young people is not rare, and about 2/3 of them come on without xerostomia and/or keratoconjunctivitis sicca. Not only the duration from first symptom to first diagnosis of pSS in the young group is longer, but also the systemic damage of the young group is greater than those of the middle-aged and aged group. These findings should alert the clinician to make the possible diagnosis of pSS in young patients.

Abstract:Objective To understand the impact of primary Sjogren's syndrome(PSS)on kidney by following up 113 patients with PSS in 1996 to 2006.Methods One hundred and thirteen patients of PSS with renal impairment were analyzed by routine,clinical features,tubular function and immunoassay.Results Among 113 PSS,89 patients were diagnosed as renal tubular acidosis(RTA),of whom 76 cases presented with Myastheina,69 with polydipsia and polyuria,57 with ostealgia,9 with diabetesinsipidus,and 21 hypokalenic paralysis.Of 113 patients,24 patients presented with glomerulonephritis,36 patients initially presented with renal impairment,significant elevation of IgG was noticed in 82.3%,87 patients responded to the combination of steroid and immunosuppressors,and 28 had renal function completely recovered.Conclusion PSS patients with renal involvement developed varied syndrome,mainly tubular acidosis and glomerulonephritis.The combination of steroid and immunosuppressors can significantly improve the renal function of patients with PSS.

Abstract:Juvenile systemic sclerosis (jSSc), the pediatric counterpart of systemic sclerosis (SSc), is a rare autoimmune disorder characterized by vasculopathy and fibrotic disorders. It ranks among the rheumatologic diseases with the highest rates of morbidity and mortality, predominantly impacting females. Although a universally accepted classification for jSSc remains elusive, a provisional classification proposed in 2007 integrates major and minor criteria, reflecting the involvement of diverse organs and tissues. Pulmonary manifestations are relatively common in jSSc, occurring in 36% to 55% of cases. Particularly lung complications include children s interstitial lung disease (chILD), pulmonary arterial hypertension (PAH) and nodules. The aim of this paper is to describe the main pulmonary manifestations of patients with jSSc in relation to SSc, highlighting fundamental pathophysiological, and clinical features based on the latest literature data.

Abstract:Sjögren’s syndrome (SS) is a chronic autoimmune inflammatory disease that typically affects the salivary and lacrimal glands. Renal involvement is relatively uncommon and may precede other complaints. Tubulointestitial nephritis (TIN) is the most common renal involvement in SS. Osteomalacia occurring as the first manifestation of renal tubular disorder due to SS is very rare. We report a 39-year-old male who presented with polydipsia, polyuria, and multiple bone pain. Bone density test showed severe osteoporosis, and laboratory findings suggested hypokalemia, hypophosphatemia, and vitamin D deficiency, which supported the diagnosis of hypophosphatemic osteomalacia. He had nephrogenic loss of phosphate and potassium, tubular acidification, and concentration dysfunction. And, the diagnosis of chronic TIN was subsequently confirmed by renal biopsy. The patient reported dry mouth and physical examination showed multiple dental caries. Xerophthalmia, abnormal morphology, and function of the salivary glands by sonography and scintigraphy, together with positive anti-SSA and anti-SSB, confirmed the diagnosis of SS. The TIN indicated SS as the underlying cause of osteomalacia. After taking supplements of potassium, phosphate, vitamin D, and sodium bicarbonate for 1 month, bone pain was alleviated and serological potassium and phosphorus were also back to normal. In conclusion, renal involvement in SS may be latent and precede the typical sicca symptoms. Osteomalacia can be the first manifestation of renal disorder due to SS. Therefore, autoantibody investigations as well as the lacrimal and salivary gland examinations for SS should be considered and performed for suspected patients.
