Abstract:The atypical congenital pathway of the internal carotid artery (ICA) is an uncommon anatomical variation with a very low prevalence. The medialization of the internal carotid artery is regarded as an infrequent manifestation. The internal carotid artery may be displaced at the level of the pharyngeal wall, leading to the enlargement of connective tissue in the lateral pharynx and retropharyngeal areas. A 76-year-old male patient with a history of weakness on the left side of his body, difficulty swallowing, and speech difficulties was sent to the otorhinolaryngology department because of pain in his throat. He underwent several unsuccessful attempts at the insertion of a nasogastric tube (NGT), which was eventually done with considerable difficulty. Upon evaluation, the individual displayed regular speech and a strength rating of 4/5 in both of his left limbs. Upon examination of the throat, a significant non-pulsating edema was observed in the right retropharyngeal area, pushing the right tonsil anteromedially. A posterior pharyngeal mass was observed during fiberoptic laryngoscopy. The Doppler examination of the carotid arteries yielded definitive results. Computed tomography angiography (CTA) showed the common carotid arteries via a retropharyngeal route. It is clinically significant to identify variations in the course of the internal carotid artery, particularly those located near the submucosal area of the pharynx. This is because there is a higher risk of injury during procedures involving manipulation of the pharynx, such as intubation, insertion of a nasogastric tube, or surgeries in the internal carotid artery region.

Abstract:Background: Extracranial internal carotid artery (ICA) pseudoaneurysm is a rare condition that can be caused either by penetrating or blunt trauma, including dog bites, which is an uncommon occurrence. Together with the possibility of no symptoms or nonspecific ones such as cervical pain, hematoma, swelling, or mass, considering ICA pseudoaneurysm following a dog attack is of paramount importance to avoid life-threatening complications. Case presentation: We present a rare case of a 17-year-old male with a history of dog bites three months prior, who presented to the emergency department with left-sided neck pain, dizziness, and several episodes of blurred vision and diplopia. On physical examination, a palpable mass measuring approximately 20 × 30 millimeters was identified in the left neck region and multiple superficial lacerations were observed in this area. Laboratory tests yielded normal results. Doppler ultrasound revealed a pseudoaneurysm in the left internal carotid artery. Because the great saphenous veins were insufficient, the patient was successfully treated with synthetic graft patch arterioplasty, and no complications were seen in his one-year follow-up with computed tomography (CT) angiography. Conclusions: This report emphasizes the significance of thorough initial evaluation and imaging in cases of dog attacks, even without apparent significant trauma, to rule out hidden arterial injuries.

Abstract:Background: Anterior inferior cerebellar artery (AICA) aneurysms in the internal auditory canal (IAC) are rare. We have reported a case of dissecting AICA aneurysm in the IAC presenting initially with the eighth nerve palsy followed by the seventh nerve palsy without hemorrhage. Case description: A 68-year-old woman presented with a sudden onset of vertigo accompanied by deafness and tinnitus on the right side that was preceded by intermittent right retroauricular pain 2 weeks before. Audiogram showed severe sensorineural hearing loss. Computed tomography and magnetic resonance imaging (MRI) indicated absence of prior subarachnoid hemorrhage. Magnetic resonance angiogram (MRA) suggested a tiny aneurysm at the fundus of the IAC accompanied with thinning of the lateral pontine segment of the AICA. Conservative treatment led to moderate improvement of the symptoms. However, the patient developed the right retroauricular pain again, followed by the right facial paralysis 5 months later but still without signs of hemorrhage on MRI. Digital subtraction angiogram showed dissecting aneurysm in the IAC. The patient was managed with oral steroids and direct intervention was avoided due to a risk of ischemia supposed by large area irrigated by the AICA. Follow-up MRA 18 months after the first presentation showed improvement in the narrowing of the AICA proximal to the aneurysm. The patient was functionally independent despite right-sided hearing loss and slight facial paresis. Conclusion: This report warns physicians that a dissecting AICA aneurysm without subarachnoid hemorrhage may cause eighth and seventh nerve palsy.

Abstract:Rationale: Intracavernous infectious aneurysm (ICIA), represents a rare entity that is always described in the form of case reports in the literature. The coexistence of ICIA and cavernous sinus thrombosis (CST) is extremely rare and poorly understood. Patient concerns: A 53-year-old female patient presented to our hospital with headache, nausea and fatigue for 3 weeks. She complained of blurry vision and drooping eyelids before admission. Neurological examination revealed bilateral decreased visual acuity, limitation of extraocular movements and decreased sensation of forehead. Brain magnetic resonance imaging (MRI) showed mixed signal intensities in both cavernous sinuses and expansion of right superior ophthalmic vein, suggesting the formation of CST. One month later, computed tomography angiography (CTA) confirmed a large aneurysm was attached to the left intracavernous carotid artery (ICCA). Diagnoese: This patient was diagnosed with ICIA and CST. Interventions: She was administered with intravenous meropenem and vancomycin and subcutaneous injection of low molecular heparin for 4 weeks. Outcomes: One month later, her extraocular movement had significantly improved, without ptosis and conjunctival congestion. At 1-year follow-up, her ophthalmoplegia fully recovered. Fortunately, such large aneurysm did not rupture in spite of slight broadening. Lessons: The coexistence of ICIA and CST is extremely rare. Contiguous infection from adjacent tissues is the foremost cause of ICIA. A repeated angiographic examination is recommended under enough anti-infective treatment due to the characteristics of rapid emergence and fast growth of infectious aneurysms.

Abstract:Agenesis or hypoplasia of the internal carotid artery (ICA) may easily be confused with dissection or occlusion. We report a case of a 24-year-old female with complaint of acute left-hand hypoesthesia and a history of occasional intermittent numbness of her right hand with myoclonic jerking. Because previous imaging studies over 2 years were interpreted as occlusion of the left ICA secondary to carotid dissection, the treating physician had prescribed anticoagulant therapy. During transcranial Doppler (TCD) examination, the spectral waveform was unexpectedly normal, prompting a repeat review of all imaging due to the TCD results. Magnetic resonance angiography (MRA) revealed the same "flame-like" appearance of the ICA origin. Late-phase digital subtraction angiography showed a small caliber cervical ICA (occluded at the skull base). Computed tomography demonstrated absence of the carotid canal, confirming an absent intracranial portion of the ICA and establishing a correct diagnosis of left internal carotid hypoplasia. Vascular ultrasound and TCD examinations are noninvasive and inexpensive tools that can improve the interpretation and understanding of the clinical significance of other "static" radiographic tests (MRA, digital subtraction angiography ). An accurate diagnosis is essential to avoid risky, aggressive treatment, such as anticoagulation for an "absent" dissection.
Incidental neck mass in a young adult: A case report

Abstract:Introduction: While true extracranial internal carotid aneurysms (tECAAs) are extremely rare and often asymptomatic, they can lead to serious complications including rupture, thrombosis or embolization. Surgery has been the standard, even in asymptomatic patients, due to safety and long-term outcomes. However, advancements in technology have introduced endovascular interventions and observation as options. Presentation of case: A 32-year-old obese man presented with sudden right-sided facial numbness lasting 3 h. Personal, family, social and trauma history were unremarkable. Cardiac and neurological examinations were normal. Imaging revealed a non-flow-limiting stenosis and a left internal carotid saccular artery aneurysm (15 × 16 mm). After considering the pros and cons of each treatment and thorough shared decision-making, left carotid stenting (6 × 25 mm ViabahnGORE) was performed. The postoperative course was uneventful. Patient was discharged on the postoperative day 2 with Clopidogrel. Discussion: The choice of treatment approach for tECAAs depends on various factors, including patient characteristics, surgeon experience, and equipment availability. In general, symptomatic aneurysms (regardless of diameter), expanding aneurysms, and larger aneurysms (>20 mm), and those with identified thrombus are typically indications for repair. Conclusion: Endovascular techniques are gaining traction due to their potential advantages, including avoidance of cranial nerve injury and general anesthesia, enhanced access and control of lesions that may be difficult to navigate surgically, and lower rates of perioperative morbidity and mortality.

Abstract:BACKGROUND With the popularization of various cerebrovascular imaging methods and increased attention to the field, more cerebrovascular diseases are being detected in asymptomatic patients. Different cerebrovascular diseases are typically isolated but occasionally occur simultaneously, causing difficulties in diagnosis and treatment. Morphological changes in the collateral circulation of blood vessels in chronic cerebral artery occlusion patients are slow and dynamic, intercepting morphological development at a specific moment. Excessive reliance on single imaging tests such as digital subtraction cerebral angiography (DSA) can lead to misdiagnosis. CASE SUMMARY We report a 52-year-old male who was admitted to our department for treatment of an unruptured aneurysm during a follow-up examination for brain trauma after 1 mo. Computed tomography (CT) scan was negative, but CT angiography (CTA) revealed a sac-like bulge at the bifurcation of the left middle cerebral artery. DSA revealed an unruptured aneurysm with unique scapular morphology. The stump of a middle cerebral artery occlusion was observed during exposure during aneurysm clipping surgery, and the diagnosis of chronic cerebral artery occlusion was confirmed intraoperatively. This case was confusing because of the peculiar morphology of the arterial stump and compensatory angiogenesis due to multiple cerebral artery stenoses observed on preoperative CTA and DSA. The surgery did not cause secondary damage to the patient, and medical treatment for risk factors was continued postoperatively. CONCLUSION Multiple cerebral arterial stenoses can occur in conjunction with aneurysms or arteriovenous malformations, and their unique morphology can lead to misdiagnosis.

Abstract:Internal carotid artery (ICA) dissection is a dangerous condition that results from disruption of the intimal part of the wall of the internal carotid artery. It is a rare disease that may occur spontaneously or as a result of a trauma. Spontaneous dissections of the carotid artery are rare but important causes of ischemic stroke because they usually affect young and middle-aged patients. Up to date, only a few cases were described in the literature about ICA dissection causing isolated cranial nerve palsies, with the Hypoglossal nerve being the most affected. Here, we report a case of a 56-year-old man presenting with progressive dysarthria, dysphagia to liquid diet, and difficult mastication. He was diagnosed as a case of cervical internal carotid dissection with pseudoaneurysm formation causing mass effect resulting in a compressive ipsilateral Hypoglossal nerve palsy based on magnetic resonance imaging (MRI) findings. Angiography confirmed the presence of dissecting pseudoaneurysm which was eventually managed by stenting. This case was reported to highlight and emphasize the importance of radiology, whether diagnostic or interventional, in managing rare and challenging cases such as ICA dissection.

Abstract:This report presents an incidental finding of a potentially life-threatening large fusiform aneurysm of the internal carotid artery at the level of the posterior communicating artery in a patient who reported with complaints relating to the presence of a radiolucent lesion in the mandible that was diagnosed as odontogenic keratocyst. This case highlights the need to study a case in detail in preparation for prescribing the appropriate advanced imaging modality examination, and also to carefully evaluate the entire imaged volume for incidental pathology and not just the region of interest. This report assumes significance in the light of widespread use of cone beam CT by dental clinicians for routine diagnostic tasks without a formal interpretation being carried out on all such studies.
Giant Internal Carotid Artery Aneurysm of the Cavernous Segment

Abstract:A 70-year-old woman with a history of hypertension developed acute onset of severe tightness headache accompanied by vomiting. Patient reported having right diplopia and occasional and temporary right peripheral facial paralysis for two weeks. Neurological examination revealed only a right squint. Cerebral multi-detector computed tomographic angiography (MD-CTA) and Magnetic Resonance Imaging (MRI) revealed a giant right internal carotid aneurysm (2.7 cm) at the cavenous segment (Figure 1A, 1B). No abnormalities were noted in subarachnoid spaces and white - grey matter. A selective digital subtraction angiography (DSA) of the right internal carotid artery confirmed the giant aneurysm at the cavenous segment (Figure 2A, 2B). Subsequently endovascular flow-diverting stent was deployed at the cavernous segment of the right internal carotid artery to treat over time the giant aneurysm. Her clinical course has no neurological symptoms and she was discharged after two days with established over time clinical and imaging follow-up. Giant intracranial aneurysms and vascular anomalies of the internal carotid are rare, and are defined "giant" those greater than 2.5 cm in diameter (1-4). Conservative treatment of giant intracranial aneurysms have a mortality rates of 65-100% in 2-5 years (4). Clinical presentation of internal carotid aneurysms at the petrous segment can manifest from no-symptom to headache, cranial nerve disturbs and epistaxis (1- 4). MD-CTA and MRI are the two gold standard methods for diagnosis in patients with suspected internal carotid artery and intracranial artery aneurysm (1,5). Internal carotid aneurysms at the cavenous segment are located in a surgically difficult accessible area, therefore endovascular percutaneous techniques are considered the first lines treatments (6).

Abstract:Introduction: It is well known that whereas bilateral occlusion is less common, unilateral internal carotid artery blockage happens regularly. Asymptomatic to fatal ischemic stroke can be the clinical presentation, depending on whether there is adequate collateral blood flow. Internal carotid artery occlusion is often associated with significant neurologic events, both at the time of initial occlusion and during follow-up. Case presentation: The authors describe a patient's experience of dizziness followed by a fall. The hyperdense position of the basilar artery near its bifurcation was identified following a computed tomography (CT) scan. Furthermore, the pons, midbrain, and medial parts of the right temporal lobe showed numerous small hypodensities suggestive of an ischemic injury. The patient had medical treatment without surgery after it was determined that they had bilateral internal carotid arteries and a blocked right vertebral artery. After taking dual anti-platelet therapy for 5 days, the patient recovered without incident and was discharged from the hospital. Discussion: In a young patient with bilateral internal carotid arteries occlusion (BICAO), the authors highlighted the significance of prompt diagnosis of stroke-like symptoms, diagnostic possibilities, and treatment options. Options for diagnosis include brain MRI and CT head to check for ischemia and CT angiography (CTA) head and neck to assess for artery obstruction. Options for treatment include severe medical and surgical treatments, such as carotid endarterectomy, stent implantation, or balloon angioplasty, or medical management alone, such as dual anti-platelet medication and thrombolysis. Conclusion: BICAO is associated with a grave prognosis and significant cerebrovascular complications. High-quality studies are needed to establish the best treatment strategy, considering the complex and individualized nature of the condition.

Abstract:Purpose: To describe a case of multiple extremely rare cervical arterial variations. Methods: A 55-year-old man with a tentative diagnosis of right internal carotid artery (ICA) stenosis was examined using computed tomography (CT) angiography for the evaluation of vascular lesions in the neck and head region. A 64-slice CT machine was used. Results: On CT angiography, there was laterally located and narrowed petrous segment of the right ICA, indicative of aberrant course of the petrous ICA. Right vertebral artery (VA) was small in caliber and a relatively large anomalous artery arose from the proximal right ICA. This anomalous artery entered the posterior fossa via the foramen magnum, indicative of a type 1 proatlantal artery. Right occipital artery (OA) arose from the proximal ICA. The left OA also arose from the proximal ICA. Conclusion: An aberrant course of the petrous ICA is an extremely rare arterial variation which is formed by segmental agenesis of the cervical ICA, and the collateral channel passes through the middle ear cavity. It can be dangerous during middle ear surgery. The type 1 proatlantal artery is also an extremely rare arterial variation formed by the persistence of the proatlantal intersegmental artery. It is clinically significant because of its unique blood flow from the carotid system to the vertebrobasilar system. The OA rarely arises from the proximal ICA. Identification of these cervical arterial variations before surgery and vascular intervention are important to avoid complications during the procedure.

Abstract:Vascular, Ahead of Print. BackgroundExtracranial internal carotid artery aneurysms are rare entities that arise within the carotid triangle. Although rare, they are clinically significant due to the risk of neurologic thromboembolic events, compression of cranial nerves and vasculature, rupture, and ischemia.ObjectiveSurgery is the gold-standard treatment for symptomatic patients of all ages with extracranial internal carotid artery aneurysm.MethodA 26-year-old female patient was admitted to our institution with complaints of left auricular pain, hoarseness, and uncontrollable hypertension. She had a pulsatile mass located at the left cervical region. After diagnosing the extracranial internal carotid artery aneurysm, it was decided that surgery was necessary to relieve the patient's discomfort and prevent possible complications from the aneurysm in the left carotid artery.ResultIn the operation, the aneurysmatic segment was resected and sent for histopathological evaluation. Saphenous vein graft was interposed as end to end anastomosis to the proximal and distal healthy segments of the internal carotid artery.ConclusionThis case report highlights the surgical management of a 26-year-old female patient who presented to our institution with complaints of left auricular pain, hoarseness, uncontrolled hypertension, and a pulsatile mass localized at the left cervical region and diagnosed extracranial internal carotid artery aneurysm.

Abstract:OBJECTIVES/HYPOTHESIS: Internal carotid artery (ICA) injury during endoscopic endonasal surgery (EES) is a known and feared complication of paranasal sinus and skull base procedures. These ICA injuries can result in stroke, cranial nerve palsies, and death. This review examines the setting of injury along with the treatment approaches, and patient outcomes.STUDY DESIGN: Systematic review using PubMed/MEDLINE and EMBASE.METHODS: The databases were searched for articles reporting cases of ICA injury during EES. Variables analyzed included patient demographics, operative approach, preoperative diagnosis, setting of injury, repair method, imaging studies, patient outcomes, and follow-up.RESULTS: Twenty-five articles with 50 cases were included in this review. The EES approach was used for skull base procedures in 34 cases and for inflammatory disease in 16 cases. The most commonly injured ICA segment was the cavernous (34 cases), followed by the ophthalmic (three cases). Injuries occurred more commonly on the left (1.3:1). Injury occurred in the setting of various steps during EES with instruments. Stereotactic image guidance was reported in two cases. Initial hemostasis was achieved with packing in 35 cases, endoscopic clip sacrifice in four cases, bipolar coagulation with the intent to seal defect in three cases, and bipolar coagulation with the intent to sacrifice the ICA in one case. Intraoperative or immediate postoperative angiography was reported in 27 cases.CONCLUSIONS: The incidence of reported cases of ICA injury during EES remains low. Abstract:Introduction Several guidelines have addressed symptomatic or asymptomatic carotid artery stenosis treatment with intensive medical management and surgical management based on the severity of stenosis [1]. In acute occlusions, endovascular treatment (EVT) and carotid endarterectomy (CEA) with or without prior thrombolytics provide the best recanalization and functional outcomes when compared to intravenous or intra‐arterial thrombolysis alone. There is, however, less robust evidence on management of symptomatic chronic internal carotid artery (ICA) occlusion occurring in tandem with significant contralateral ICA stenosis [2]. In chronic ICA occlusion (ICAO), EVT and carotid endarterectomy (CEA) are technically unfeasible, but indirect procedures have been arising as safe and effective alternatives, notably CEA or stenting of contralateral stenotic ICA [3] and CEA or stenting of ipsilateral external carotid artery (ECA) [4]. We hereby report a case of symptomatic chronic ICAO that was managed with staggered angioplasty and stenting of bilateral common carotid artery (CCA) bifurcations. Methods We hereby present a case report. Results A 62‐year‐old male with atrial fibrillation and hypertension woke up with sudden dysarthria and right upper extremity and facial weakness. NIHSS at presentation was 4. Last seen normal 9 hours prior. Non‐contrast head computerized tomography (CT) did not show acute intracranial abnormality, CT angiogram of head and neck showed severe right ICA stenosis and complete occlusion of the left ICA at cervical, petrous and cavernous segments, with distal reconstitution at the supraclinoid segment with left proximal middle cerebral artery (MCA) thrombus. CT perfusion showed a core infarct volume of 8cc in left MCA territory, mismatch volume of 78cc and ratio of 10.8. Risks of acute neuro‐intervention were deemed to outweigh benefits. Given favorable perfusion imaging, and in concordance with the EXTEND trial [5], tPA was administered. The patient remained stable without hemorrhagic complications with residual deficits of mild right upper extremity weakness and mild expressive aphasia. MRI confirmed left ICA territory ischemic infarcts. Patient underwent stenting of the asymptomatic right ICA at the CCA bifurcation first, followed by left CCA stent after one month with impressive improvement in subsequent perfusion imaging. Conclusions Our case encourages consideration of well‐timed angioplasty and stenting in cases of chronic bilateral carotid artery disease especially when the asymptomatic side is also significantly stenotic

Abstract:PURPOSE:The anatomic relationship around the horizontal segment of petrous internal carotid artery (ICA) is complicated. Relative researches on human patients using radiographic technique are scarce. It is essential to get adequate anatomic knowledge about this region for performing some bypass procedures.METHODS:Using the independent software Mimics, we prospectively studied the 0.45-mm-thick computed tomography angiography (CTA) images of 29 patients (58 sides). All patients' middle cranial fossa and related critical anatomic structures were reconstructed in 3D. Some measurements were established on the multiplanar reconstructed images, including the distance from foramen ovale and foramen spinosum to the petrous ICA; the distance from the lumen of cochlea to the edge of trigeminal impression and petrous ICA; and the length of the horizontal segment of petrous ICA.RESULTS:The horizontal segment of petrous ICA was surrounded by the trigeminal impression, the cochlea and the auditory tube spatially. On the multiplanar reconstructed CTA images, the average distances from foramen ovale and foramen spinosum to the petrous ICA were 4.1 mm and 5.8 mm, respectively; the average distance from the lumen of cochlea to the edge of trigeminal impression and petrous ICA were 8.2 mm and 2.0 mm, respectively; and the mean length of the horizontal segment of petrous ICA was 15.8 mm.CONCLUSIONS:Our results indicate adequate and reliable anatomic information can be obtained using reconstructed CTA on an individualized basis.
