Abstract:ObjectiveTo analyze a case of neglected cervicovesical fistula with intrauterine adhesions caused by cesarean section.MethodsA 36-year-old female patient with a history of two previous cesarean sections complained of the absence of menstruation for the last 18 months. The diagnosis of the cervicovesical fistula was made through hysteroscopy and cystoscopy. The reconstruction of the uterus and bladder was achieved by a laparoscopic repair technique.ResultsThe patient resumed normal menstruation postoperatively without complaining of any complications. Uterine continuity and cavity had been restored to normal at the second look of hysteroscopy.ConclusionsCervicovesical fistula with intrauterine adhesions is very rare in our clinical work. Hysteroscopy might play an essential role in diagnosing cervicovesical fistula and IUA. In our literature review, a surgical approach was the mainstay and definitive management of the cervicovesical fistula following a cesarean section.
Early Surgical Repair of Iatrogenic Ureterovaginal Fistula Secondary to Gynecologic Surgery

Abstract:Objective: To describe the early surgical repair of iatrogenic ureterovaginal fistula (UVF) secondary to gynecologic surgery, and evaluate the efficacy and feasibility of early surgical intervention. Methods: Data were retrospectively reviewed for 52 patients with iatrogenic UVF who underwent early surgical repair between January 1996 and January 2011 at the Sir Run Run Shaw Hospital, Hangzhou, China. Preoperative patient characteristics and postoperative endpoints, including type of index procedure, repair technique, operative time, perioperative complications, and postoperative follow-up data were assessed. Results: All early surgical repairs were uneventful. The mean time from diagnosis to surgery was 15.7 days (range, 14-21 days). The mean operative time was 73.4 min (range, 51-110 min) for ureteroneocystostomy, and 104 min (range, 91.5-153 min) for Boari flap procedure. The mean estimated blood loss was 57.8 mL (range, 35-80 mL). No major intraoperative complications occurred, although 5 patients had postoperative surgical site infections. Forty-three patients were followed for a mean of 18.4 months (range, 10.5-24.0 months). Ureteral stricture with asymptomatic hydronephrosis occurred in 2 (4.7%) patients. Ipsilateral renal function was preserved in all patients. Conclusion: Early surgical repair of UVF secondary to gynecologic surgery was safe and effective, and preserved ipsilateral renal function. Usage of acellular dermal matrix allograft as a novel technikue in the treatment of vesicovaginal fistula

Abstract:Objective To describe an effective and simple method for repairing vesicovaginal fistula(VVF) using an acellular dermal matrix allograft(ADMA).MethodsWe reviewed retrospectively 8 women suffering from VVF after surgical trauma or prolonged obstructed labour who underwent transabdominal,extraperitoneal repair of their complicated VVF using an ADMA technique between December 2005 and August 2006.The preoperative and postoperative mdeical records were reviewed.ResultsAll patients noted an immediate result with no evidence of leakage,and remain dry at follow-up,for 6-12 months.No intraoperative or postoperative complications occurred.ConclusionThe free ADMA technique is a simple,effective,and mini-invasive method for complex VVF repair,obviating the need for rotational flaps.

Abstract:The occurrence of rectovaginal fistula (RVF) due to injury and infection during surgery has become an increasing trend. Currently the incidence is 0.9-9.9% and the vast majority of cases must be corrected surgically. The authors present a case of a 48-year-old patient who underwent late-onset rectovaginal fistula after laparoscopically assisted vaginal hysterectomy (LAVH). The RVF healed spontaneously with nutritional support and avoidance of infection.

Abstract:AIM To study the active constituents of Swertia davidi Franch. METHODS Column chromatographies on silica gel, Sephadex LH-20 and Diaion-201 et al. were used to isolate and purify the chemical components. Their structures were identified by UV, IR, MS, NMR and 2D-NMR. RESULTS These compounds were elucidated as 8-O-beta-D-glucopyranosyl-1, 3, 5-trihydroxyxanthone (I), 5-O-beta-D-glucopyranosyl-1, 8-dihydroxy-3-methoxyxanthone (II), 5-O-beta-D-glucopyranosyl-1, 3, 8-trihydroxyxanthone (III) and swertamarin (IV). CONCLUSION Compound III is a new xanthone glucoside. The other compounds were isolated from this plant for the first time.
Subband Architecture for HFB-Based A/D Converters

Abstract:The classic structure of hybrid filter bank (HFB) A/D converters is very sensitive to realization errors. The relation between input and output is time-variant in the original HFB because of decimation. It causes the blind techniques not to be applicable for compensating the realization errors. In this paper, a new structure is proposed for HFB-based A/D converters which is called subband HFB architecture. The analog input is assumed to be decomposed into M narrowband signals and decimated for an M-channel HFB structure. They are considered as the new virtual inputs of the subband HFB. Subband HFB-based A/D converters estimate these virtual inputs and provide M output signals. The distortion and inter-channel interference (ICI) functions are defined for this new architecture. The synthesis part of subband HFB structure is designed using a matrix of FIR filters. The realization errors of subband HFB may be compensated using the techniques such as blind deconvolution. The new architecture is simulated for an eight-channel structure considering simply realizable RC and RLC circuits as the analysis filter bank. A good performance is reported in terms of distortion and ICI in the case where some parts of the spectrum are allocated to guard bands. The new subband HFB structure exhibits a better performance in comparison with the original one

Abstract:Objective To investigate long‐term and patient‐reported outcomes, including sexual function, in women undergoing urogenital fistula (UGF) repair, addressing the lack of such data in Western countries, where fistulas often result from iatrogenic causes. Patients and Methods We conducted a retrospective analysis at a tertiary referral centre (2010–2023), classifying fistulas based on World Health Organisation criteria and evaluating surgical approaches, aetiology, and characteristics. Both objective (fistula closure, reintervention rates) and subjective outcomes (validated questionnaires) were assessed. A scoping review of patient‐reported outcome measures in UGF repair was also performed. Results The study included 50 patients: 17 (34%) underwent transvaginal and 33 (66%) transabdominal surgery. History of hysterectomy was present in 36 patients (72%). The median (interquartile range [IQR]) operating time was 130 (88–148) min. Fistula closure was achieved in 94% of cases at a median (IQR) follow‐up of 50 (16–91) months and reached 100% after three redo fistula repairs. Seven patients (14%) underwent reinterventions for stress urinary incontinence after transvaginal repair (autologous fascial slings). Patient‐reported outcomes showed median (IQR) scores on the International Consultation on Incontinence Questionnaire Female Lower Urinary Tract Symptoms Modules (ICIQ‐FLUTS) of 5 (3–7) for filling symptoms, 1 (0–2) for voiding symptoms and 4.5 (1–9) for incontinence symptoms. The median (IQR) score on the ICIQ Female Sexual Matters Associated with Lower Urinary Tract Symptoms Module (ICIQ‐FLUTSsex) was 3 (1–5). The median (IQR) ICIQ Satisfaction (ICIQ‐S) outcome score and overall satisfaction with surgery item score was 22 (18.5–23.5) and 10 (8.5–10), respectively. Higher scores indicate higher symptom burden and treatment satisfaction, respectively. Our scoping review included 1784 women, revealing mixed aetiology and methodological and aetiological heterogeneity, thus complicating cross‐study comparisons. Conclusions Urogenital fistula repair at a specialised centre leads to excellent outcomes and high satisfaction. Patients with urethrovaginal fistulas are at increased risk of stress urinary incontinence, possibly due to the original trauma site of the fistula.

Burden of Colovesical Fistula and Changing Treatment Pathways: A Systematic Literature Review

Abstract:Purpose: Colovesical fistula (CVF) is a rare complication of sigmoid diverticulitis causing significant morbidity and quality of life impairment. Aim of this study was to analyze contemporary literature data to appraise the current standard of care and changes of treatment algorithms over time. Materials and methods: A systematic review of the literature on surgical management of CVF was conducted through PUBMED, EMBASE, and COCHRANE databases, according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses statement guidelines. Results: Fourteen papers published between 2014 and 2020 and including 1061 patients were analyzed. One-stage colonic resection with primary anastomosis, with or without loop ileostomy, was the most common surgical procedure. A laparoscopic or robotic approach was attempted in 39.5% of patients, and conversion rate to open surgery was 7.8%. Clavien-Dindo grade ≥3 complication rate, 30-day mortality, and recurrence rate were 7.4%, 1.5%, and 0.5%, respectively. Conclusions: Minimally invasive sigmoidectomy with primary anastomosis is safe and should be the first-choice approach for CVF. Bladder repair is not necessary after a negative intraoperative leak test. A standardized perioperative care can improve clinical outcomes and reduce the length of hospital stay and the duration of Foley catheterization.

Abstract:Fistulas in the upper gastrointestinal (GI) tract are complex conditions associated with elevated morbidity and mortality. They may arise as a result of inflammatory or malignant processes or following medical procedures, including endoscopic and surgical interventions. The management of upper GI is often challenging and requires a multidisciplinary approach. Accurate diagnosis, including endoscopic and radiological evaluations, is crucial to build a proper and personalized therapeutic plan, that should take into account patient's clinical conditions, time of onset, size, and anatomical characteristics of the defect. In recent years, several endoscopic techniques have been introduced for the minimally invasive management of upper GI fistulas, including through-the-scope and over-the-scope clips, stents, endoscopic suturing, endoluminal vacuum therapy (EVT), tissue adhesives, endoscopic internal drainage. This review aims to discuss and detail the current available endoscopic techniques for the treatment of upper GI fistulas.

Abstract:A wide variety of fistulae occur in the female pelvis, most of which cause significant morbidity. Diagnosis, characterization, and treatment planning may be difficult using traditional imaging modalities such as fluoroscopy and computed tomography. To date, there is no comprehensive literature review of the radiologic findings associated with various types of female pelvic fistulae, and furthermore, none dedicated to magnetic resonance imaging (MRI). In this article, we seek to provide a broad overview of the MRI characteristics of female pelvic fistulizing disease in combination with epidemiologic and clinical characteristics. MRI is often considered the imaging modality of choice for evaluation of fistulae owing to its superior soft-tissue contrast and ability to provide surgeons with the highest quality information derived from just one study, including anatomic location of fistulae and associated pelvic pathology. In other instances, MRI can be complementary to the more traditional imaging techniques. This review will describe the etiology, anatomy, MRI findings, and treatment pearls for several of the more common pelvic fistulae found in female patients, including anovaginal, rectovaginal, colovaginal, vesicovaginal, colovesical, and other complex fistulae. Fine structure of the atrial cardiocytes in the house musk shrew (Suncus murinus).

Abstract:The atrial and ventricular cardiocytes of the house musk shrew were examined by immunohistochemistry, and the right atrium containing the auricle was studied by transmission electron microscopy. The atrial natriuretic peptide (ANP)-granules of the cardiocytes in the auricle and the rest of the atrium were also analyzed by ultrastructural morphometry. On immunohistochemistry, ANP immunoreactivity was detected in the atria, with the most intensely reacting cardiocytes being localized in the right auricular part of the atrium. ANP immunoreactivity was not detected in the ventricular muscles. On ultrastructure, in most of the atrial cardiocytes including the auricle, ANP-granules, well-developed Golgi apparatus and rough endoplasmic reticulum were observed, and the nuclei were characteristically situated in the periphery of the cardiocytes, being different from many other mammalian hearts. The ANP-granules were classified into two types (A and B), with most of these granules being located in the paranuclear region in association with the Golgi apparatus, and a few ANP granules being observed throughout the sarcoplasmic layers intervening between the myofibrilar bundles. On ultrastructural morphometry, the total number of granules in the right auricular cardiocytes was significantly greater than those in the atrial cardiocytes, and the diameter of the A-granules was significantly greater than that of the B-granules in both the atrial and auricular cardiocytes.

A review of the last 10-year articles focused on the various novel aspects of anal fistula was performed using papers obtained from electronic databases, including PubMed, Embase, and Scopus. Abstract:Since the mid-1970s a number of investigators have developed measures of the extent to which oral disorders compromise functional, social and psychological well-being. They have also examined the associations between clinical indicators of oral health status and these subjective indicators. In general, these associations have been inconsistent and weak. One reason for this might be that the subjective indicators employed were rudimentary and insensitive to the health outcomes of oral disorders. The development of the Oral Health Impact Profile, a more sophisticated measure of the health outcomes of oral disorders, provided a method to examine this hypothesis. Using data from an oral health survey of older adults, we examined the associations between OHIP scores and a variety of clinical indicators of tooth loss, caries and periodontal disease. Even with this measure the associations were predominantly weak, the strongest of the correlations being 0.53. We also examined the influence of personal and sociodemographic characteristics on the relationship between tooth loss and its psychosocial outcomes. Five variables reflecting expectations and resources explained as much variance in OHIP scores as did the number of missing teeth. This analysis illustrates the essential distinction between disease and health and the way in which measures of oral health can be used to pursue fundamental issues in behavioural science and health services research.
Treatment outcomes of obstetrical fistula surgical repair in low‐ and middle‐income countries: A scoping review

Abstract:Background Obstetrical fistula is an abnormal opening between the reproductive tract and lower urinary and/or gastrointestinal tract resulting from obstetrical complications, affecting nearly two million women worldwide. It imposes physical, economic, social, and mental consequences on the affected women. Treatment outcomes vary and, mostly, surgical treatment results in improved quality of life and successful subsequent pregnancy for survivors. Objectives The review aimed to chart and examine the treatment outcomes of obstetrical fistula surgical repair in low‐ and middle‐income countries. Search Strategy This is a scoping review study to identify treatment outcomes of obstetrical fistula surgical repair in low‐ and middle‐income countries. The search was conducted from databases (PUBMED, Embase, CINAHL, Scopus, and Web of Science), and gray literature (Google Scholar, Google, and conference proceedings). Selection Criteria The eligibility criteria were constructed using a participant, concept, and context framework and included study types of primary research, reviews, and reports. Studies without full text and in languages other than English were excluded. Data Collection and Analysis The relevant characteristics of the included studies were extracted on an Excel spreadsheet and analyzed to chart treatment outcomes. Main Findings The review examined the full text of 57 studies on the treatment outcomes of obstetrical fistula. The findings were grouped into two themes: early and late outcomes. The early outcomes included incontinence, surgical‐site infection, urine retention, hemorrhage, and retained catheter. The late outcomes included fistula recurrence, residual incontinence, quality of life, reproductive issues, mental health, family and social support, and financial status. Conclusion The treatment outcomes of obstetrical fistula can be grouped into short‐term and long‐term outcomes. Although this review found adequate studies for the analysis, most study designs were poor. Stronger studies are recommended in the future to guide policy and decision‐making. We would like to suggest that researchers conduct systematic reviews and meta‐analyses independently for short‐term and long‐term outcomes.

Clinical analysis of different surgical repair procedures for vesicovaginal fistula after gynecologic surgery

Abstract:Objectives:To compare the outcome of different surgical repair procedures for vesicovaginal fistula after gynecologic surgery.Methods:The clinical data of 12 patients with vesicovaginal fistula after gynecologic surgery from April 2009 to November 2015 in Peking University Third Hospital were analyzed retrospectively.All the 12 cases had a history of gynecologic operations:10 cases underwent hysterectomy,1 case underwent pelvic mass resection and 1 case underwent transurethral resection of the bladder.The duration from gynecologic operation to vaginal leakage was 5 to 12 months,with an average of 8.4 ± 2.9 months.The diameter of the fistula was 0.3 ～2cm,with an average of 1.1 ±0.6 cm.All the 12 cases were single fistula.Results:Six cases were repaired by a vaginal approach.The average operation time was 105.0 ± 32.0 min;average intraoperative bleeding was 4.5 ± 1.2 ml;average postoperative hospital stay was 3.0 ± 1.5 d.No recurrence was found in 6 months after the repair operation.Three cases underwent laparoscopic transabdominal repair.The average operation time was 345.3-± 192.3 min;average intraoperative bleeding was 83.3 ± 28.9 ml;average postoperative hospital stay was 10.3 ± 3.2 d.No recurrence was found in 6 months after the repair operation within 2 patients.One patient experienced two times of surgical repair treatment because of fistula recurrence.One case underwent open transabdominal repair.The operation time was 234 min;operation bleeding was 200 ml,and postoperative hospital stay was 40 d.Fat liquefaction occurred in wound after operation,cured by conservative treatment.No recurrence was found during follow-up.Studer neobladder vagina fistula occurred in 1 patient,who underwent operation in a combined abdominal and vaginal approach.The operation time was 224 min;operation bleeding was 100 ml,and postoperative hospital stay was 20 d.No recurrence was found.One case underwent electric coagulation due to the smaller diameter of fistula.The operation time was 10 min;operation bleeding was 2 ml,and postoperative hospital stay was 2 d.The patient experienced second transvaginal repair treatment because of recurrence.Condusion:The vesicovaginal fistula caused by gynecologic surgical procedures can be cured by surgical repair operation.Transvaginal repair is a feasible and effective procedure with less blood loss and shorter recovery time.As far as vagina is able to be revealed,separated and sutured,transvaginal repairs can be used as a minimal invasive procedure and provide favorable outcomes.
Influence of Platelet-Rich Plasma on Recurrent Vesicovaginal Fistula—A Histological and Immunohistochemical Study

Abstract:Vesicovaginal fistula is a cause of deterioration in the quality of life. It is a communication between the bladder and vagina resulting in the uncontrollable leakage of urine through the vagina. Recently, regenerative methods have been used more frequently, and platelet-rich plasma is one of these methods. The functional properties of platelet-rich plasma are based on the synthesis and secretion of multiple growth factors released after platelet activation. The aim of this study was to investigate how platelet-rich plasma influences the condition of the tissue and the healing ability of the urothelium, vaginal epithelium and tissues surrounding the fistulous canal. The study included eight patients who had undergone the Latzko procedure aimed at closing the vesicovaginal fistula. Samples were collected during primary surgery without platelet-rich plasma and after failed surgery, during a second attempt. The specimens were subjected to histological and immunohistochemical evaluation. The histology demonstrated that in platelet-rich plasma patients, greater vascularization and wider subepithelial mucosa layering was observed. Inflammatory infiltration in the subepithelial layer was increased in platelet-rich plasma patients. No localization differences in growth factor proteins were found in either group, but in platelet-rich plasma-patients, the reactions were stronger. It can be concluded that the use of platelet-rich plasma improves the morphological structure of the injected tissues.

Uretero-uterine fistula following Manual Vacuum Aspiration for Incomplete abortion: A rare case report

Abstract:Genitourinary fistulas are a well-recognized complication of various gynecological, obstetrical, and endourological interventions. The incidence of uretero-uterine fistula is very rare compared to other genitourinary fistulas. Few cases are reported in literature regarding the uretero- uterine fistula following manual vacuum aspiration of retained product of placenta. We report a case of 28 year multi-parous women who had presented with complain of involuntary passage of urine following manual vacuum aspiration for retained product of conception. Common tests for suspicion of uretero-uterine fistula include cystoscopy, triple swab test, and CT urography. Our patient was diagnosed in CT urography and was managed by exploratory laparotomy with end-to-end anastomosis of the right ureter, DJ stenting, and repair of a uterine perforation. Due to relatively rare incidence of this condition, there has not been a specific guideline for management. A multimodality and multidisciplinary approach have been proposed for the management of uretero-uterine fistulas.

Abstract:BACKGROUND: Several techniques have been described for the management of fistula-in-ano, but all carry their own risks of recurrence and incontinence. Technology has evolved over the past 2 decades that may enable surgeons to deal with this troublesome issue with greater success. This review summarizes the history of fistula-in-ano management, the current techniques available, and describes new technologies.METHODS: Medline searches were performed using the PubMed, Ovid, Embase, Cochrane, and Google Scholar databases to identify articles reporting on fistula-in-ano management using surgery alone, fibrin glues, and fistula plugs. Forty-one articles reporting on the history of fistula-in-ano management and the use of new technologies were included.RESULTS: Conventional fistula surgery techniques have their place, but new technologies such as fibrin glues and the anal fistula plugs offer an alternative approach, with initial studies reporting good success rates.CONCLUSIONS: New technologies provide promising alternatives to traditional methods of management. There is, however, a real need for high-quality randomized control trials.

Abstract:A recent review of the literature shows an increasing number of studies on this subject, most of which are retrospective and presented solely from a clinical point of view. Some of them are no more than case-reports. There is a lack of valid epidemiologic data about the incidence of rectourethral fistulas. According to the literature, these fistulas result from many different causes. Rectourethral fistulas may have a congenital or acquired origin. Acquired cases include inflammatory, neoplastic, or traumatic etiologies. The diagnostic algorithm is clear and very classical. The new diagnostic approach represents an MRI investigation of the male urethra. It is important to recognize this diversity of etiology because each type requires a different surgical strategy. The aim of the surgical approach is the closure of all types of fistulas. Spontaneous closure of the recto-urethral fistulas after double diversion or by means of a one-stage procedure is possible only in a few cases. In most cases, the treatment proceeds in three stages (double diversion-urinary and bowel, closure technique, undiversion). An endoscopic approach using biological sealants seems to be promising. Few urologists and general surgeons have attained wide experience in the management of rectourethral fistulas. No single procedure has been proven most effective or even universally applicable.
Vaginal peritoneum fistula through pouch of Douglas during peritoneal dialysis: Case report and literature review

Abstract:Introduction: Peritoneal dialysis is the preferred approach for kidney replacement therapy. A peritoneal-vaginal fistula is a rare complication associated with peritoneal dialysis. Case presentation: A 69-year-old woman with a history of type two diabetes and systemic arterial hypertension for twenty years is scheduled to undergo the surgical placement of a Tenckoff catheter to begin renal replacement therapy. After having thirty dialysis sessions, she was discharged to continue therapy at home. Five days later, she observed a notable rise in vaginal discharge after peritoneal dialysis. This case report investigates the etiology, diagnosis, and management of peritoneal vaginal fistula and analyzes current medical literature. Discussion: Factors associated with the formation of peritoneum-vaginal fistula include increased intra-abdominal pressure due to dialysis, anatomical predisposition, peritonitis, and malnutrition. Conclusions: Peritoneal vaginal fistula is an uncommon consequence of peritoneal dialysis. Diagnosis entails demonstrating the movement of dialysis fluid from the peritoneum to the vagina. Treatment should be customized according to the etiology of the fistula and the individual needs of each patient.
