Abstract:Background: The purpose of this study was to develop and validate a positioning method with hand-guiding and contact position feedback of robot based on a human-robot collaborative dental implant system (HRCDIS) for robotic guided dental implant surgery. Methods: An HRCDIS was developed based on a light-weight cooperative robot arm, UR5. A three-dimensional (3D) virtual partially edentulous mandibular bone was reconstructed using the cone bone computed tomography images. After designing the preoperative virtual implant planning using the computer software, a fixation guide worn on teeth for linking and fixing positioning marker was fabricated by 3D printing. The fixation guide with the positioning marker and a resin model mimicking the oral tissues were assembled on a head phantom. The planned implant positions were derived by the coordinate information of the positioning marker. The drilling process using the HRCDIS was conducted after mimicking the experimental set-up and planning the drilling trajectory. Deviations between actual and planned implant positions were measured and analyzed. Results: The head phantom experiments results showed that the error value of the central deviation at hex (refers to the center of the platform level of the implant) was 0.79 +/- 0.17 mm, central deviation at the apex was 1.26 +/- 0.27 mm, horizontal deviation at the hex was 0.61 +/- 0.19 mm, horizontal deviation at the apex was 0.91 +/- 0.55 mm, vertical deviation at the hex was 0.38 +/- 0.17 mm, vertical deviation at the apex was 0.37 +/- 0.20 mm, and angular deviation was 3.77 +/- 1.57 degrees. Conclusions: The results from this study preliminarily validate the feasibility of the accurate navigation method of the HRCDIS.
Multi-orientation trajectory smoothing planning of robot for dental implant

Abstract:A highly automated dental implant surgery robot system was developed based on the optical positioning technology according to the requirements of dental implant surgery. The transformation of system space coordinate system was studied. The smooth orientation trajectory planning has an important impact on the accuracy and efficiency of the dental implant robot. Then a multi-orientation C2 continuous smooth interpolation algorithm was proposed based on the quaternion and rotation vector. The rotation vector was interpolated by smooth splicing of linear interpolation and the B-spline curve in three-dimensional space, and the corresponding quaternion would maintain the continuity in three-dimensional space. The multi-orientation C2 continuous interpolation calculation of unit quaternion was realized. The proposed B-spline multi-orientation smooth interpolation algorithm has C2 continuity through the experimental analysis and comparative verification is superior to the SQUAD algorithm in real-time performance and motion efficiency, and is suitable for multi-orientation trajectory planning of robot end effector.

Abstract:PurposeDental implant surgery is an effective method for remediating the loss of teeth. Robot is expected to increase the accuracy of dental implant surgery. However, most of them are industrial serial robot, with low stiffness and non-unique inverse kinematic solution, which may reduce the success rate and safety of robotic surgery. Compared to serial robot, parallel robot is more stiffness and has unique inverse kinematic. However, its workspace is small, which may not meet surgical requirements. Therefore, a novel hybrid robot dedicated to dental implant is proposed.MethodsThe hybrid robot is composed of three translation joints, two revolute joints, and Stewart parallel manipulator. Stewart is used for performing surgical operation, while the joints are used for enlarging the workspace of Stewart. In order to ensure the safety of robot motion, physical human–robot interaction based on a variable admittance controller is applied in the robotic system. In addition, considering the small workspace of Stewart, an optimal model is proposed to minimize the joint movement of Stewart in adjusting the orientation of drill bit.ResultsPhantom experiments were carried out based on the prototype robot. In the experiments, the optimal model could be solved after 20 iterations, finding an ideal joint configuration. The proposed variable admittance controller could enhance comfort level effectively. The accuracy of robot is evaluated by angle, entry and exit deviation, which are 0.74 ± 0.25°, 0.93 ± 0.28 mm, and 0.96 ± 0.23 mm, respectively.ConclusionThe phantom experiments validate the functionality of the proposed hybrid robot. The satisfactory performance makes it more widely used in the practical dental implant surgery in the future.

Abstract:Several unknown Escherichia coli genes for different species of acid phosphatase were cloned in vivo with the plasmid Mu dII4042. When present in a multicopy state, each gene promoted hydrolysis of p-nitrophenyl-phosphate at acidic pH. Among seven recombinant clones that encoded periplasmic acid phosphatase activities, five different genes could be distinguished by the pH optimum and substrate preference for the enzyme and by the restriction enzyme pattern. A 1.7-kilobase recombinant DNA fragment, common to two clones, was inserted into plasmid pBR322 and shown to contain a new gene, agp, which leads to the overexpression of the periplasmic acid glucose-1-phosphatase, a dimer of a 44-kilodalton polypeptide. Fusions of agp to gene phoA deprived of its own signal sequence conferred an alkaline phosphatase-positive phenotype to bacteria, showing the presence of an export signal on agp. The resulting hybrid proteins were characterized by immunoprecipitation with an antiserum directed against purified acid phosphatase or against alkaline phosphatase, showing that agp is the structural gene of the acid phosphatase. The beginning, the orientation, and the end of gene agp on the cloned DNA fragment were determined by the characteristics of such hybrid proteins.

Abstract:In this paper, we study on the cooperation of the virtual digital surgery design technology, three-dimensional navigation and high-precision robot, aiming to implement the preoperative design content more accurately. In this system, virtual digital surgery design acts as the “brain” to provide position and motion information. Three-dimensional navigation system can carry out tasks such as registering coordinates, offering intraoperative navigation, measuring and verification. Besides, because of the advantages in accuracy and stability, high-precision intelligent manipulator UR5 can assist doctors in improving the operating accuracy and quality. This system can implement the virtual digital operation plan in world space to insure that surgery can be operated more accurately and satisfactory. Preliminary experiments for orthognathic surgery have been carried out to analyze the reliability of this system.

Abstract:Agile maneuvers are essential for robot-enabled complex tasks such as surgical procedures. Prior explorations on surgery autonomy are limited to feasibility study of completing a single task without systematically addressing generic manipulation safety across different tasks. We present an integrated planning and control framework for 6-DoF robotic instruments for pipeline automation of surgical tasks.We leverage the geometry of a robotic instrument and propose the nodal state space (NSS) to represent the robot state in SE(3) space. Each elementary robot motion could be encoded by regulation of the state parameters via a dynamical system. This theoretically ensures that every in-process trajectory is globally feasible and stably reached to an admissible target, and the controller is of closed-form without computing 6-DoF inverse kinematics. Then, to plan the motion steps reliably, we propose an interactive (instant) goal state of the robot that transforms manipulation planning through desired path constraints into a goal-varying manipulation (GVM) problem. We detail how GVM could adaptively and smoothly plan the procedure (could proceed or rewind the process as needed) based on on-the-fly situations under dynamic or disturbed environment. Finally, we extend the above policy to characterize complete pipelines of various surgical tasks. Simulations show that our framework could smoothly solve twisted maneuvers while avoiding collisions. Physical experiments using the da Vinci Research Kit (dVRK) validates the capability of automating individual tasks including tissue debridement, dissection, and wound suturing. The results confirm good task-level consistency and reliability compared to state-of-the-art automation algorithms.

Abstract:Background Minimally invasive robot-assisted techniques are being used increasingly in surgical procedures, and the preoperative preparation for this kind of operation is more complicated than traditional operations. Method Taking the placement requirements of minimally invasive thyroid surgery as the research object, taking the kinematic performance and safety of the manipulator as the optimization goal, a set of indexes and pendulums for evaluating the accuracy and safety of the manipulators are proposed accordingly. Results The optimal preoperative positioning scheme is obtained based on Non dominated sorting genetic algorithm- II multi-objective genetic algorithm. Meanwhile, compared with the optimization results of Gaot genetics and the positional scheme given by the surgeon based on experience, the advantages of the scheme are proved to be obvious. Conclusion The scheme showed good performance in the comparison and proved the feasibility of the positional scheme in preoperative planning.

Abstract:The results of oral and maxillofacial surgery (OMS) significantly depend on the individual capabilities of surgeons. In this article, an autonomous surgical robot is proposed to automatically perform surgery according to a preoperative plan with the assistance of a surgeon. A 3-D computed tomography image of the subject's teeth was used to build an offline shape-based model by creating a large amount of 2-D contour data. A monochrome camera was mounted on a self-developed robot to capture the real-time image of the teeth for comparison with the offline 2-D contour to verify the teeth pose without using common markers. The data of the preoperative plan and the teeth pose were integrated to guide the movement of the robot during operation. Drilling experiments were conducted on the five 3D-printed mandible models, and a statistical analysis revealed highly accurate results. This autonomous robot provides a practical solution for robotically conducted OMS and has the potential to significantly reduce the workload of surgeons.

Porter (1963) by summarizing the very limited knowledge of plant cell ultrastructure that we had prior to that publication, and, by way of our three retrospective accounts, show how this paper stimulated and influenced subsequent research on plant microtubules. Micrographs of historical interest are presented that are either previously unpublished or from primary publications.

Abstract:Statement of problem: The complete arch implant-supported treatment concept with 2 angled implants has been widely used for the prosthetic rehabilitation of edentulous patients. While mechanical analysis plays a pivotal role in minimizing suboptimal outcomes or premature failure, it is notably time-consuming. Consequently, clinical treatment planning relies heavily on dentists' subjective judgment and an optimization process is needed. Purpose: The purpose of this study was to develop an optimization process for providing immediate recommendations to support decision-making in configuring complete arch implant-supported prostheses. Material and methods: This research was carried out in 2 phases. The first consisted of collecting a dataset from a total of 2800 finite element simulations by randomly configuring 10 implant design variables with 4 types of mandibles. The dataset was used to train an artificial neural network to predict the biomechanical performance of a given complete arch implant-supported prosthesis design configuration. In the second phase, the artificial neural network was used as the objective function predictor in a particle swarm optimization process to enable immediate recommendations for the implant placement. The optimization process was evaluated for accuracy, computing performance, and adaptability for unseen mandibles. Results: Within the specified design space, the optimization process was able to find an optimal design based on an imported mandible model in 30 seconds. The optimized designs were found to improve peri-implant stress by 11.08 ±6.43%. When verified through finite element analysis, the prediction error was found to be 10.4 ±8.1%. Furthermore, the prediction of the optimal design was highly accurate when tested on 2 unseen mandibles, yielding an error of less than 1.7%. Conclusions: The suggested approach can quickly provide an optimal implant configuration for each individual and effectively reduce the average peri-implant stress in the mandible.

Abstract:Spinal fusion surgery requires highly accurate implantation of pedicle screw implants, which must be conducted in critical proximity to vital structures with a limited view of anatomy. Robotic surgery systems have been proposed to improve placement accuracy, however, state-of-the-art systems suffer from the limitations of open-loop approaches, as they follow traditional concepts of preoperative planning and intraoperative registration, without real-time recalculation of the surgical plan. In this paper, we propose an intraoperative planning approach for robotic spine surgery that leverages real-time observation for drill path planning based on Safe Deep Reinforcement Learning (DRL). The main contributions of our method are (1) the capability to guarantee safe actions by introducing an uncertainty-aware distance-based safety filter; and (2) the ability to compensate for incomplete intraoperative anatomical information, by encoding a-priori knowledge about anatomical structures with a network pre-trained on high-fidelity anatomical models. Planning quality was assessed by quantitative comparison with the gold standard (GS) drill planning. In experiments with 5 models derived from real magnetic resonance imaging (MRI) data, our approach was capable of achieving 90% bone penetration with respect to the GS while satisfying safety requirements, even under observation and motion uncertainty. To the best of our knowledge, our approach is the first safe DRL approach focusing on orthopedic surgeries.

Abstract:Journal of Mechanics in Medicine and Biology, Ahead of Print. In oral and maxillofacial surgery, computer-assisted navigation technologies have been widely used to achieve intraoperative positioning. The traditional methods mainly rely on the experience of doctors and the difference between the locations of key points in the surgical area and the preoperative planning, which have certain limitations. In this paper, a new intraoperative surgical navigation framework based on mobile laser scanner is proposed, which ensures that surgery is performed accurately according to the preoperative planning. The framework mainly includes two parts. First, the real-time surface reconstruction of the anatomy should be realized during the operation. Second, the acquired image is matched to the planned image in real time. Although the most common method of surface reconstruction is to render the volume directly from raw data or render the surface from the segmented data using computed tomography/magnetic resonance (CT/MR) data, this method is too complicated for performing the real-time operation during surgery. Furthermore, a new surface registration technique is proposed for image-guided oral and maxillofacial surgery based on the point sets. To improve the registration accuracy and robustness, the point sets are modeled by Mixed Student's t-Distribution model. In the experiments, the point sets of CT data are from 10 patients with craniomaxillofacial diseases and the surface point set is from the LRS. The TRE of 10 data was less than 1[math]mm. Compared with the paired-point registration method and Iterative Closest Point algorithm, the results demonstrated better performance of the proposed method, the surgical situation can be displayed in real time during the surgical process, and any differences from the surgical plan can also be reflected.

Abstract:<p>Three-dimensional (3D) computer-assisted corrective osteotomy has become the state-of-the-art for surgical treatment of complex bone deformities. Despite available technologies, the automatic generation of clinically acceptable, ready-to-use preoperative planning solutions is currently not possible for such pathologies. Multiple contradicting and mutually dependent objectives have to be considered, as well as clinical and technical constraints, which generally require iterative manual adjustments. This leads to unnecessary surgeon efforts and unbearable clinical costs, hindering also the quality of patient treatment due to the reduced number of solutions that can be investigated in a clinically acceptable timeframe. In this paper, we propose an optimization framework for the generation of ready-to-use preoperative planning solutions in a fully automatic fashion. An automatic diagnostic assessment using patient-specific 3D models is performed for 3D malunion quantification and definition of the optimization parameters' range. Afterward, clinical objectives are translated into the optimization module, and controlled through tailored fitness functions based on a weighted and multi-staged optimization approach. The optimization is based on a genetic algorithm capable of solving multi-objective optimization problems with non-linear constraints. The framework outputs a complete preoperative planning solution including position and orientation of the osteotomy plane, transformation to achieve the bone reduction, and position and orientation of the fixation plate and screws. A qualitative validation was performed on 36 consecutive cases of radius osteotomy where solutions generated by the optimization algorithm (OA) were compared against the gold standard solutions generated by experienced surgeons (Gold Standard; GS). Solutions were blinded and presented to 6 readers (4 surgeons, 2 planning engineers), who voted OA solutions to be better in 55% of the time. The quantitative evaluation was based on different error measurements, showing average improvements with respect to the GS from 20% for the reduction alignment and up to 106% for the position of the fixation screws. Notably, our algorithm was able to generate feasible clinical solutions which were not possible to obtain with the current state-of-the-art method.</p>

Development and validation of a robotic system for milling individualized jawbone cavities in oral and maxillofacial surgery

Abstract:Objectives: This study aimed to develop and validate a robotic system capable of performing accurate and minimally invasive jawbone milling procedures in oral and maxillofacial surgery. Methods: The robotic hardware system mainly includes a UR5E arm (Universal Robots, Denmark) and the binocular positioning system (FusionTrack 250, Atracsys LLC, Switzerland). The robotic software (Dental Navi 3.0.0, Yakebot Technology Ltd., China) is capable of generating cutting tool paths based on three-dimensional shape description files, typically in the stereolithography format, and selected cutting tool parameters, as well as designing surgical accessories. Fully impacted supernumerary tooth models in the maxilla were fabricated using software and three-dimensional printing. Following the planning of a customized cavity to fully expose the tooth, maxillary bone milling was performed on both the robot and static guide groups (n = 8). After milling, all models underwent scanning for assessment. Results: In the experiment with fully buried supernumerary tooth models in the maxilla, the root mean square, translation error, over-removal rate, and maximum distance were significantly smaller in the robot group compared to the static guide group. Moreover, the overlap ratio and Dice coefficient were significantly greater in the robot group. No statistically significant differences were observed between the two groups in terms of the rotation error (P = 0.80) or under-removal rate (P = 0.92). Conclusions: This study has developed a robotic system for milling individualized jawbone cavities in oral and maxillofacial surgery, and its accuracy has been preliminarily verified to meet clinical requirements. Clinical significance: The robotic system can achieve precise, minimally invasive, individualized jawbone milling in a variety of oral and maxillofacial surgeries, including tooth autotransplantation, surgical reshaping for zygomatic fibrous dysplasia, removal of fully impacted supernumerary or impacted teeth, and endodontic microsurgery, among other relevant clinical applications.

Abstract:In the field of oral rehabilitation, the combined use of 3D imaging technologies and computer-guided approaches allows the development of reliable tools to be used in preoperative assessment of implant placement. In particular, the accurate transfer of the virtual planning into the operative field through surgical guides represents the main challenge of modern dental implantology. Guided implant positioning allows surgical and prosthetic approaches with minimal trauma by reducing treatment time and decreasing patient’s discomfort. This paper aims at defining a CAD/CAM framework for the accurate planning of flapless dental implant surgery. The system embraces three major applications: (1) freeform modelling, including 3D tissue reconstruction and 2D/3D anatomy visualization, (2) computer-aided surgical planning and customised template modelling, (3) additive manufacturing of guided surgery template. The tissue modelling approach is based on the integration of two maxillofacial imaging techniques: tomographic scanning and surface optical scanning. A 3D virtual maxillofacial model is created by matching radiographic data, captured by a CBCT scanner, and surface anatomical data, acquired by a structured light scanner. The pre-surgical planning process is carried out and controlled within the CAD application by referring to the integrated anatomical model. A surgical guide is then created by solid modelling and manufactured by additive techniques. Two different clinical cases have been approached by inserting 11 different implants. CAD-based planned fixture placements have been transferred into the clinical field by customised surgical guides, made of a biocompatible resin and equipped with drilling sleeves.

Abstract:Background: We developed an image-guided robot system to provide mechanical assistance for skull base drilling, which is performed to gain access for some neurosurgical interventions, such as tumour resection. The motivation for introducing this robot was to improve safety by preventing the surgeon from accidentally damaging critical neurovascular structures during the drilling procedure. Methods: We integrated a Stealthstation navigation system, a NeuroMate robotic arm with a six-degree-of-freedom force sensor, and the 3D Slicer visualization software to allow the robotic arm to be used in a navigated, cooperatively-controlled fashion by the surgeon. We employed virtual fixtures to constrain the motion of the robot-held cutting tool, so that it remained in the safe zone that was defined on a preoperative CT scan. Results: We performed experiments on both foam skull and cadaver heads. The results for foam blocks cut using different registrations yielded an average placement error of 0.6 mm and an average dimensional error of 0.6 mm. We drilled the posterior porus acusticus in three cadaver heads and concluded that the robot-assisted procedure is clinically feasible and provides some ergonomic benefits, such as stabilizing the drill. We obtained postoperative CT scans of the cadaver heads to assess the accuracy and found that some bone outside the virtual fixture boundary was cut. The typical overcut was 1-2 mm, with a maximum overcut of about 3 mm. Conclusions: The image-guided cooperatively-controlled robot system can improve the safety and ergonomics of skull base drilling by stabilizing the drill and enforcing virtual fixtures to protect critical neurovascular structures. The next step is to improve the accuracy so that the overcut can be reduced to a more clinically acceptable value of about 1 mm.
