Abstract:Abstract Background: Even after successful treatment, tuberculosis (TB) survivors experience elevated morbidity and mortality. However, there is limited evidence on how these risks vary over time and according to individual characteristics. Methods: We conducted a retrospective cohort study of individuals diagnosed with TB in Brazil, using national TB notifications and linked mortality records for 2007-2016. For this study population we estimated mortality rate ratios (MRRs) and cumulative mortality by year since TB diagnosis, as compared to general population mortality rates matched on age, sex, year, and state. We identified clinical and sociodemographic factors associated with elevated post-TB mortality, and compared the distribution of causes of death to the general population. Findings: The study sample included 834,594 individuals, with 4.1 million person-years of follow-up. The TB cohort had elevated mortality compared to the general population, particularly in the first year post-diagnosis (MRR 11.28, 95%CI: 11.18-11.37). Post-TB MRRs declined from 3.59 (3.53-3.64) in year 2 to 1.46 (1.34-1.59) in year 10. Cumulative excess mortality was 6.12% (95%CI: 6.07-6.17) after 1 year and 9.90% (9.58-10.24) after 10 years. MRRs were highest for individuals 30-44 years-old at diagnosis. Relapse, loss to follow-up, and co-prevalent conditions like HIV and alcoholism were strongly associated with higher MRRs. Over time, causes of death shifted from TB and HIV to chronic conditions like cardiovascular disease and cancer. Interpretation: Individuals developing TB disease face elevated mortality up to 10 years after diagnosis. These excess risks vary across demographic and clinical characteristics. Funding: NIH.
Long-term survival and cause-specific mortality of patients newly diagnosed with tuberculosis in São Paulo state, Brazil, 2010-15: a population-based, longitudinal study

Abstract:Background: Long-term survival and cause-specific mortality of patients who start tuberculosis treatment is rarely described. We aimed to assess the long-term survival of these patients and evaluate the association between vulnerable conditions (social, health behaviours, and comorbidities) and cause-specific mortality in a country with a high burden of tuberculosis. Methods: In this population-based, longitudinal study in São Paulo state, Brazil, we described the 5-year survival of patients who were newly diagnosed with tuberculosis in 2010. We included patients with newly-diagnosed tuberculosis, aged 15 years or older, and notified to the São Paulo State Tuberculosis Program in 2010. We excluded patients whose diagnosis had changed during follow-up (ie, they did not have tuberculosis) and patients who had multidrug-resistant (MDR) tuberculosis. We selected our population with tuberculosis from the dedicated electronic system TBweb. Our primary objective was to estimate the excess mortality over 5 years and within the group who survived the first year, compared with the general São Paulo state population. We also estimated the association between social vulnerability (imprisonment and homelessness), health behaviours (alcohol and drug use), and comorbidities (diabetes and mental disorders) with all-cause and cause-specific mortality. We used the competing risk analysis framework, estimating cause-specific hazard ratios (HRs) adjusted for potential confounding factors. Findings: In 2010, there were 19 252 notifications of tuberculosis cases. We excluded 550 cases as patients were younger than 15 years, 556 cases that were not tuberculosis, 2597 retreatments, and 48 cases of MDR tuberculosis, resulting in a final cohort of 15 501 patients with tuberculosis. Over a period of 5 years from tuberculosis diagnosis, 2660 (17%) of 15 501 patients died. Compared with the source population, matched by age, sex, and calendar year, the standardised mortality ratio was 6·47 (95% CI 6·22-6·73) over 5 years and 3·93 (3·71-4·17) among those who survived the first year. 1197 (45%) of 2660 deaths were due to infection. Homelessness and alcohol and drug use were associated with death from infection (adjusted cause-specific HR 1·60, 95% CI 1·39-1·85), cardiovascular (1·43, 1·06-1·95), and external or ill-defined causes of death (1·80, 1·37-2·36). Diabetes was associated with deaths from cardiovascular causes (1·70, 1·23-2·35). Interpretation: Patients newly diagnosed with tuberculosis were at a higher risk of death than were the source population, even after tuberculosis treatment. Post-tuberculosis sequelae and vulnerability are associated with excess mortality and must be addressed to mitigate the tuberculosis burden worldwide. Funding: Wellcome Trust.
Social and biological factors associated with unsuccessful TB treatment outcome in Ireland 2011-2021

Abstract:Abstract Background Tuberculosis (TB) treatment success rates are crucial for TB control programme evaluations. The World Health Organisation sets targets of > 90% but Ireland remains below 70%. We analysed notifications from the Irish National TB Surveillance System from 2011-2021 to explore factors associated with successful treatment outcome in Ireland. Methods We selected potential predictor variables based on clinical relevance and literature review. We examined each predictor's unadjusted association with treatment success using univariable logistic regression. We used backwards elimination to remove predictors that failed to remain significant in multivariable logistic regression. Treatment success was defined as treatment completed or cured, while unsuccessful treatment was defined as treatment failed, treatment interrupted, transferred out and died from any cause. We used Stata for analysis. Results Between 2011-2021, 52% of cases were successfully treated, 10.7% were unsuccessfully treated and treatment outcome was not reported for 37%. Treatment outcome completeness declined significantly between 2019-2021. Univariable analysis showed successfully treated cases had higher odds of being a migrant (OR:1.9; 1.5-2.5), being linked to outbreaks (OR:1.9; 1.2-3.0), being a contact of a case (OR: 1.0-2.5;), having a normal X-ray (OR:1.7;1.1-2.6) and being an outpatient (OR:3.0;2.2-4.2). Significant predictors of unsuccessful treatment in the final model were age >70 years, unemployment, immunesuppression and any anti-TB drug resistance. Conclusions Surveillance data shows that Ireland fails to meet the WHO target of > 90% treatment success with high levels of missing data. Data quality improvement initiatives are needed to reverse the decline in completeness associated with the Covid-19 pandemic period so control programmes can robustly evaluate performance. Additional supports should be considered for patients who are older, unemployed, immunesuppressed or with drug resistant TB. Key messages • Additional patient supports should be considered for patients who are unemployed, age 70 years and older, immunesuppressed or with drug resistant forms of TB. • Ireland fails to meet the WHO TB treatment success target with high levels of missing data. Treatment outcome completeness needs to be improved so control programmes can robustly evaluate performance.

Abstract:Objective: To investigate the survival time of multidrug-resistant tuberculosis (MDR-TB) patients and the influential factors in Wuhan. Methods: The relevant information were collected from TB management information system, cause of death reporting system and medical records by trained doctors. The univariate and multivariate Cox proportional hazards model were applied to analyze the factors affecting survival time of patients. Results: A total of 552 patients with MDR-TB were included in the analysis. After the diagnosis of MDR-TB, the cumulative survival rates from the first year to the third year were 0.94, 0.88, and 0.80, respectively. The mortality density of MDR-TB patients was 6.52/100 person-years, and the median survival time was (89.52±1.85) months. Kaplan-Meier analysis showed that the cumulative survival rate of the standardized treatment group was significantly higher than that of the non-standardized treatment group (Log rank=101.070, P<0.001). Compared with the patients aged <30 years, the HR of the patients aged 30-years and ≥60 years were 2.987 (95%CI: 1.268-7.036), 4.957 (95%CI: 1.942-12.653). Compared with the patients with the education level of high school and above, the HR of the patients with education level of junior high school/primary school and below were 1.908 (95%CI: 1.152-3.160), 1.681(95%CI: 1.033-2.735). Compared with the patients without diabetes, the HR of the patients with diabetes was 1.961(95%CI: 1.347-2.854). Compared with the patients without other serious diseases, the HR of the patients with other serious diseases was 2.597 (95%CI: 1.820-3.706). Compared with the patients who had been treated less than one time, the HR of the patients having previous treatment with more than 2 times was 1.611 (95%CI: 1.077-2.409). Compared with patients receiving standard MDR regimen treatment, the HR of the patients receiving no standard MDR regimen treatment was 3.155 (95%CI: 2.132-4.670). Conclusions: The cumulative survival rate of MDR-TB patients without standard treatment was significantly lower than that of patients with standard treatment. Older age, low educational level, diabetes mellitus, other serious diseases, more than two times treatment in the past, and receiving no multi-drug resistance regimen treatment were the risk factors affecting the survival of MDR-TB patients.

Abstract:Abstract Background Tuberculosis (TB) and Acquired Immune Deficiency Syndrome (AIDS) are leading causes of death globally. However, little is known about the long-term mortality risk and the timeline of death in those co-infected with human immunodeficiency virus (HIV) and Mycobacterium tuberculosis (MTB). This study sought to understand the long-term mortality risk, factors, and the timeline of death in those with HIV- Mycobacterium tuberculosis (MTB) coinfection, particularly in those with insufficient TB treatment. Methods TB-cause specific deaths were classified using a modified ‘Coding of Cause of Death in HIV’ protocol. A longitudinal cross-registration-system checking approach was used to confirm HIV/MTB co-infection between two observational cohorts. Mortality from the end of TB treatment (6 months) to post-treatment year (PTY) 5 (60 months) was investigated by different TB treatment outcomes. General linear models were used to estimate the mean mortality at each time-point and change between time-points. Cox’s proportional hazard regressions measured the mortality hazard risk (HR) at each time-point. The Mantel-Haenszel stratification was used to identify mortality risk factors. Mortality density was calculated by person year of follow-up. Results At the end point, mortality among patients with HIV/MTB coinfection was 34.7%. From the end of TB treatment to PTY5, mortality and loss of person years among individuals with TB treatment failure, missing, and adverse events (TBFMA) were significantly higher than those who had TB cure (TBC) and TB complete regimen (TBCR). Compared to individuals with TBC and with TBCR, individuals with TBFMA tended to die earlier and their mortality was significantly higher (HR TBFMA-TBC = 3.0, 95% confidence interval: 2.5–3.6, HR TBFMA-TBCR = 2.9, 95% CI: 2.5–3.4, P < 0.0001). Those who were naïve to antiretroviral therapy, were farmers, had lower CD4 counts (≤200 cells/μL) and were ≥ 50 years of age were at the highest risk of mortality. Mortality risk for participants with TBFMA was significantly higher across all stratifications except those with a CD4 count of ≤200 cells/μL. Conclusions Earlier and long-term mortality among those with HIV/MTB co-infection is a significant problem when TB treatment fails or is inadequate.

Abstract:Objective To determine the incidence of mortality and its predictors among pulmonary TB (PTB) survivors treated at a rural Ugandan tertiary hospital. Methods We conducted a retrospective chart review of data between 2013 and 2023. We included all people that met the WHO's definition of tuberculosis cure and traced them or their next of kin to determine vital status (alive/deceased). We estimated the cumulative incidence of mortality per 1,000 population, crude all-cause mortality rate per 1,000 person-years, and median years of potential life lost (YPLL) for deceased individuals. Using Cox proportional hazard models, we investigated predictors of mortality. Results Of 334 PTB survivors enrolled, 38 (11.4%) had died. The cumulative incidence of all-cause mortality was 113.7 per 1,000 population, and the crude all-cause mortality rate was 28.5 per 1,000 person-years. The median YPLL for deceased individuals was 23.8 years (IQR: 9.6-32.8). Hospitalization (aHR: 4.3, 95% CI: 1.1-16.6) and unemployment (aHR: 7.04, 95% CI: 1.5-31.6) at TB treatment initiation predicted mortality. Conclusion PTB survivors experience post high mortality rates after TB cure. Survivors who were hospitalized and unemployed at treatment initiation were more likely to die after cure. Social protection measures and long-term follow-up of previously hospitalized patients could improve the long-term survival of TB survivors.

Abstract:SETTING: Pulmonary tuberculosis (PTB) causes a considerable number of deaths in China; however, the factors related to mortality are not well known.OBJECTIVE: To determine mortality among PTB patients and to explore its risk factors in Shanghai, China.DESIGN: This was a retrospective population-based study. A cohort of PTB patients who initiated treatment in a district of Shanghai from 2004 to 2015 was evaluated. Mortality in PTB patients was studied using the standardised mortality ratio (SMR) and Cox's proportional hazards model.RESULTS: Of 2741 PTB patients recruited in our study, 394 (14.4%) died during the 12-year follow-up. The summarised SMR was 2.8, and death was most likely to occur during the first months of anti-tuberculosis treatment. Age >60 years (adjusted hazard ratio aHR] 4.039, P < 0.001), male sex (aHR 1.603, P < 0.001), sputum smear test positivity (aHR 1.945, P < 0.001), multidrug-resistant TB (MDR-TB; aHR 3.502, P = 0.001), diabetes mellitus (aHR 1.422, P = 0.012), chronic obstructive pulmonary disease (aHR 2.505, P < 0.001) and having cancer (aHR 4.319, P < 0.001) were risk factors for PTB mortality.CONCLUSION: The overall mortality in PTB patients was higher than that in the general population. MDRTB and comorbidity were the two leading risk factors for mortality in PTB patients. Early, accurate diagnosis, together with comprehensive management and treatment, can reduce the mortality rate in PTB patients.

Abstract:Background: Pulmonary tuberculosis (PTB) is a severe infection, and its factors that influence survival have not been explored in some developed regions. Therefore, this study aimed to identify the factors that influence TB survival in China. Methods: We acquired data of PTB deaths through matching two manage systems of PTB and mortality surveillance. The Kaplan-Meier method and Cox regression model were used to identify the significant factors under various survival times. Results: A total of 283 deaths were caused by PTB, in which 53.4% occurred during the first year. In the Cox regression model, regular sputum smear test could reduce the risk of PTB death from the result of one-, three-, and five-year survival; and history of PTB treatment could decline the risk of PTB death in the first year. Additionally, age of "75-90 years" and positive result of sputum smear test significantly increased the risk of PTB death for the findings of three- and five-year survival, while age of "60-75 years" could increase the probability of PTB death for five-year survival. Conclusion: To reduce PTB deaths, more attention should be paid to the older group especially in the first year. Regular sputum smear test could effectively reduce the mortality of PTB cases. More evidence-based interventions should be implemented to enhance the survival of patients with PTB.

Abstract:BACKGROUND: Persistent racial and ethnic disparities in tuberculosis incidence exist in the USA, however, less is known about disparities along the tuberculosis continuum of care. This study aimed to describe how race and ethnicity are associated with tuberculosis diagnosis and treatment outcomes.METHODS: In this analysis of national surveillance data, we extracted data from the US National Tuberculosis Surveillance System on US-born patients with tuberculosis during 2003-19. To estimate the association between race and ethnicity and tuberculosis diagnosis (diagnosis after death, cavitation, and sputum smear positivity) and treatment outcomes (treatment for more than 12 months, treatment discontinuation, and death during treatment), we fitted log-binomial regression models adjusting for calendar year, sex, age category, and regional division. Race and ethnicity were defined based on US Census Bureau classification as White, Black, Hispanic, Asian, American Indian or Alaska Native, Native Hawaiian or Pacific Islander, and people of other ethnicities. We quantified racial and ethnic disparities as adjusted relative risks (aRRs) using non-Hispanic White people as the reference group. We also calculated the Index of Disparity as a summary measure that quantifies the dispersion in a given outcome across all racial and ethnic groups, relative to the population mean. We estimated time trends in each outcome to evaluate whether disparities were closing or widening.FINDINGS: From 2003 to 2019, there were 72 809 US-born individuals diagnosed with tuberculosis disease of whom 72 369 (35·7% women and 64·3% men) could be included in analyses. We observed an overall higher risk of any adverse outcome (defined as diagnosis after death, treatment discontinuation, or death during treatment) for non-Hispanic Black people (aRR 1·27, 95% CI 1·22-1·32), Hispanic people (1·20, 1·14-1·27), and American Indian or Alaska Native people (1·24, 1·12-1·37), relative to non-Hispanic White people. The Index of Disparity for this summary outcome remained unchanged over the study period.INTERPRETATION: This study, based on national surveillance data, indicates racial and ethnic disparaties among US-born tuberculosis patients along the tuberculosis continuum of care. Initiatives are needed to reduce diagnostic delays and improve treatment outcomes for US-born racially marginalised people in the USA.FUNDING: US Centers for Disease Control and Prevention.

Bayesian Spatial Survival Analysis of Duration to Cure among New Smear-Positive Pulmonary Tuberculosis (PTB) Patients in Iran, during 2011–2018

Abstract:Mycobacterium tuberculosis is the causative agent of tuberculosis (TB), and pulmonary TB is the most prevalent form of the disease worldwide. One of the most concrete actions to ensure an effective TB control program is monitoring TB treatment outcomes, particularly duration to cure; but, there is no strong evidence in this respect. Thus, the primary aim of this study was to examine the possible spatial variations of duration to cure and its associated factors in Iran using the Bayesian spatial survival model. All new smear-positive PTB patients have diagnosed from March 2011 to March 2018 were included in the study. Out of 34,744 patients, 27,752 (79.90%) patients cured and 6992 (20.10%) cases were censored. For inferential purposes, the Markov chain Monte Carlo algorithms are applied in a Bayesian framework. According to the Bayesian estimates of the regression parameters in the proposed model, a Bayesian spatial log-logistic model, the variables gender (male vs. female, TR = 1.09), altitude (>750 m vs. ≤750 m, TR = 1.05), bacilli density in initial smear (3+ and 2+ vs. 1–9 Basil & 1+, TR = 1.09 and TR = 1.02, respectively), delayed diagnosis (>3 months vs. <1 month, TR = 1.02), nationality (Iranian vs. other, TR = 1.02), and location (urban vs. rural, TR = 1.02) had a significant influence on prolonging the duration to cure. Indeed, pretreatment weight (TR = 0.99) was substantially associated with shorter duration to cure. In summary, the spatial log-logistic model with convolution prior represented a better performance to analyze the duration to cure of PTB patients. Also, our results provide valuable information on critical determinants of duration to cure. Prolonged duration to cure was observed in provinces with low TB incidence and high average altitude as well. Accordingly, it is essential to pay a special attention to such provinces and monitor them carefully to reduce the duration to cure while maintaining a focus on high-risk provinces in terms of TB prevalence.

Effects of the COVID-19 pandemic on TB outcomes in the United States: a Bayesian analysis

Abstract:Background Tuberculosis (TB) cases and deaths in the United States fluctuated substantially during the COVID-19 pandemic. We analyzed multiple data sources to understand the factors contributing to these changes and estimated future TB trends. Methods We identified four mechanisms potentially contributing to observed TB trends during 2020-2023: immigration, respiratory contact rates, rates of accurate diagnosis and treatment initiation, and mortality rates for persons with TB disease. We employed a Bayesian approach to synthesize evidence on how these mechanisms changed during the pandemic and how they might have combined to produce observed 2020-2023 TB data, using a transmission-dynamic model to link mechanisms to TB outcomes. We also simulated a no-pandemic counterfactual scenario that assumed mechanisms followed pre-pandemic trends. We estimated TB outcomes associated with the pandemic until 2035 to capture lagged effects. We evaluated additional scenarios to estimate the individual effect of each mechanism. Results Over the 2020-2035 study period, we estimate an additional 2,784 (95% uncertainty interval: 2,164-3,461) TB cases and 1,138 (1,076-1,201) TB deaths in the United States associated with changes occurring during the COVID-19 pandemic. The four mechanisms had offsetting effects - decreases in TB diagnosis rates and increases in TB mortality rates led to more TB deaths, while reductions in contact rates reduced TB deaths. Immigration changes initially reduced TB deaths, but increased deaths over time. Discussion While the direct impacts of the COVID-19 pandemic occurred between 2020-2023, these changes may continue to influence TB incidence and mortality in future years.
Deaths of Tuberculosis Patients in Urban China: a Retrospective Cohort Study

Abstract:BACKGROUNDThe case-fatality rate for tuberculosis (TB) remains high in China, whereas risk factors for deaths among TB cases are still unclear.DESIGNThis was a retrospective cohort study of all pulmonary TB patients registered in four districts of Shanghai from 2004 to 2008. Data were derived from the China National TB Surveillance System. A total of 4271 patients were followed up in communities. Data were analysed using Cox regression.RESULTSThe percentage of all-cause deaths in the study population was 15% 2-6 years after the most recent TB diagnosis. TB was responsible for only 17% of all deaths; male sex was significantly associated with all-cause deaths. After adjustment for sex, age and treatment history, four factors were significantly and independently associated with increased risk of death: psychopathy, chronic bronchitis, cancer and the presence of multiple diseases. TB was responsible for 7.2 potential years of life lost (PYLL); PYLL was higher in females (8.2) than males (5.3).CONCLUSIONSTB remains a significant problem in urban China. Implementation of improved clinical management, prevention strategies and other public health programmes should target TB patients with chronic conditions, particularly those with multiple diseases.
Mortality and lost to follow-up among Tuberculosis patients on treatment in Meru County, Kenya: a retrospective cohort study.

Abstract:Tuberculosis (TB) remains a leading cause of death globally. Patients who get lost-to-follow-up (LTFU) during TB treatment have high risk of relapse, mortality, treatment failure and developing Multidrug resistant TB. Empirical data to monitor long-term TB treatment outcomes in low-and-middle income countries (LMICs) are sparse. We determined proportion of TB patients who die or are LTFU during six months of treatment and identified factors independently associated with mortality or LTFU. A retrospective cohort using data from routine Meru County TB surveillance system. We included 38020 records of TB patients aged ≥15 years on treatment 2012 to 2022. TB treatment outcomes of interest were LTFU or death within six months of treatment. Survival analyses accounting for competing events were performed. Among the 38020 patients included, 27608 (73%) were male and the median (IQR) age was 32 [25-42] years. 26599 (70%) had bacteriologically confirmed TB while 11421 (30%) were clinically diagnosed. During 16531 person-years of follow-up, 2385 (6.3%, 95%CI 6.0-6.5) and 1942 (5.1%, 95%CI 4.9-5.3) patients were LTFU and died respectively. In the multivariable model, patients on re-treatment after LTFU or after failure and those previously treated compared to new TB patients were positively associated with LTFU. Patients coinfected with HIV and those with unknown HIV status were positively associated with LTFU. In contrast, females, clinically diagnosed, extra-pulmonary TB and older patients were negatively associated with LTFU. Patients HIV infected on ARVs, not on ARVs and unknown HIV status versus not infected were positively associated with mortality. Other comorbidities (not HIV), clinically diagnosed, undernourished and older patients were positively associated with mortality. More than 10% of TB patients either die or are LTFU before completing treatment. TB patients on re-treatment, HIV infected, clinically diagnosed, undernourished and the elderly needs more targeted interventions to improve survival and successfully treatment.

Sex disparities of the effect of the COVID-19 pandemic on mortality among patients living with tuberculosis in the United States

Abstract:Background: We aimed to determine the trend of TB-related deaths during the COVID-19 pandemic. Methods: TB-related mortality data of decedents aged ≥25 years from 2006 to 2021 were analyzed. Excess deaths were estimated by determining the difference between observed and projected mortality rates during the pandemic. Results: A total of 18,628 TB-related deaths were documented from 2006 to 2021. TB-related age-standardized mortality rates (ASMRs) were 0.51 in 2020 and 0.52 in 2021, corresponding to an excess mortality of 10.22 and 9.19%, respectively. Female patients with TB demonstrated a higher relative increase in mortality (26.33 vs. 2.17% in 2020; 21.48 vs. 3.23% in 2021) when compared to male. Female aged 45–64 years old showed a surge in mortality, with an annual percent change (APC) of −2.2% pre-pandemic to 22.8% (95% CI: −1.7 to 68.7%) during the pandemic, corresponding to excess mortalities of 62.165 and 99.16% in 2020 and 2021, respectively; these excess mortality rates were higher than those observed in the overall female population ages 45–64 years in 2020 (17.53%) and 2021 (33.79%). Conclusion: The steady decline in TB-related mortality in the United States has been reversed by COVID-19. Female with TB were disproportionately affected by the pandemic.

A decline in tuberculosis diagnosis, treatment initiation and success during the COVID-19 pandemic, using routine health data in Cape Town, South Africa

Abstract:Background: Coronavirus disease (COVID-19) negatively impacted tuberculosis (TB) programs which were already struggling to meet End-TB targets globally. We aimed to quantify and compare diagnosis, treatment initiation, treatment success, and losses along this TB care cascade for drug-susceptible TB in Cape Town, South Africa, prior to and during COVID-19. Methods: This observational study used routine TB data within two predefined cohorts: pre-COVID-19 (1 October 2018-30 September 2019) and during-COVID-19 (1 April 2020-31 March 2021). The numbers of people diagnosed, treated for TB and successfully treated were received from the Western Cape Provincial Health Data Centre. Pre and post treatment loss to follow up and cascade success rates (proportion of individuals diagnosed with an outcome of treatment success) were calculated and compared across cohorts, disaggregated by sex, age, HIV status, TB treatment history and mode of diagnosis. Results: There were 27,481 and 19,800 individuals diagnosed with drug-susceptible TB in the pre- and during-COVID-19 cohorts respectively, a relative reduction of 28% (95% CI [27.4% - 28.5%]). Initial loss to follow up increased from 13.4% to 15.2% (p<0.001), while post treatment loss increased from 25.2% to 26.1% (p < 0.033). The overall cascade success rate dropped by 2.1%, from 64.8% to 62.7% (p< 0.001). Pre- and during-COVID-19 cascade success rates were negatively associated with living with HIV and having recurrent TB. Conclusions: An already poorly performing TB program in Cape Town was negatively impacted by the COVID-19 pandemic. There was a substantial reduction in the number of individuals diagnosed with drug-susceptible. Increases in pre-and post-treatment losses resulted in a decline in TB cascade success rates. Strengthened implementation of TB recovery plans is vital, as health services now face an even greater gap between achievements and targets and will need to become more resilient to possible future public health disruptions.
Contribution of post-TB sequelae to life-years and quality-adjusted life-years lost due to TB disease in the United States, 2015-2019

Abstract:Background: Individuals surviving TB disease may experience chronic sequelae that reduce survival and quality-of-life. These post-TB sequalae are not generally considered in estimates of the health impact of TB disease. We estimated reductions in life expectancy and quality-adjusted life expectancy for individuals developing TB disease in the United States, including post-TB sequelae. Methods: We extracted national surveillance data on individuals diagnosed with TB during 2015-2019, including demographics, vital status at diagnosis, treatment duration, treatment outcome, and co-prevalent conditions. Using a mathematical model we simulated life expectancy and quality-adjusted life-years (QALYs) for the TB cohort, as compared to a no-TB counterfactual. We disaggregated results to report the proportion due to post-TB sequelae, and stratified outcomes by age, sex, and race. Findings: Estimated life expectancy after TB diagnosis was 30.3 (95% uncertainty interval: 29.9, 30.7) years for the TB cohort versus 32.3 (31.9, 32.7) without TB, a difference of 2.03 (1.84, 2.21) years and 1.93 (1.69, 2.18) QALYs. Life-years lost were greatest for 65-74-year-olds versus other age groups, for men versus women, and for American Indian or Alaska Native individuals versus persons from other race/ethnicities. Overall, 41% (35, 46) of life-years and 48% (42, 54) of QALYs lost were estimated to result from post-TB sequelae. Interpretation: In the United States, a substantial fraction of the life-years and QALYs lost from TB are attributable to post-TB sequelae. Evidence is needed on approaches to prevent and repair post-TB lung damage, in the context of frequent co-prevalent health conditions. Funding: CDC.

Abstract:Background: Drug resistant TB in China has been rapidly growing and becoming a cause for critical public health concern. It was reported that the acquired drug-resistant TB mainly results from incompliance to treatment, uncompleted treatment, irregular treatment and unqualified health services. Methods & Materials: Case-cohort was established in four counties in eastern rural China. All registered active TB patients from April 2013 to March 2014 were investigated using structure questionnaire to collect demographic, geographic,socioeconomic information and disease profile. Enzyme-linked immunosorbent assay (ELISA) was used to assess the level of serum cytokines (IFN-γ, IL - 4) before anti-TB treatment. All the participants were followed up 6 or 8 months until treatment was completed. Results: In total, 1404 patients were recruited, in which 1270 (94.5%) were newly diagnosed TB patients, 447 (33.8%) were smear-positive (SS+). The average age was 49.5±18.8, and 76.2% was male. The serum IFN-γ and IL-4 before anti-TB treatment was 7.936 and 12.292 pg/ml respectively. 80.9% of newly diagnosed TB patients received standard anti-TB treatment, while 87.7% retreatment TB patients received standard anti-TB treatment. The successfully treated rate was 92.5% and 78.6% of new TB patients and retreatment TB patients respectively. The sputum conversion rate of SS+ TB patients after 2 month anti-TB treatment was 96.5%, while the cure rate and successful rate was 80.5% and 90.0% respectively. 92.6% smear-negative patients completed anti-TB treatment. The successful rate in Jiangsu (97.3%), the more developed area, was higher than in Jiangxi (87.6%) (OR = 6.276, 95% CI: 2.528 15.579). Patients with BMI<18.5 may be more risk to fail the treatment compared with the health BMI (18.5-23.9) patients (OR = 3.196, 95% CI: 1.592 6.418). There was no relationship between serum cytokines level and treatment outcomes. The cure rate of TB comorbidity with diabetes was same as the one without diabetes. Conclusion: The sputum conversion rate, cure rate and successful rate of TB patients were high in eastern rural China. Improving patients’ nutrition, keeping a healthy BMI could be the key point of increasing successfully treated rate of tuberculosis. More research should be done in future to evaluate the impact of the serum cytokines during anti-TB treatment.

Objective: This study aimed to investigate the survival rate and predictors of mortality among TB-HIV co-infected patients during TB treatment at public health facilities in Bahir Dar, Northwest Ethiopia. Methods: A retrospective follow-up study was conducted among 401 TB-HIV co-infected patients who were treated for tuberculosis between July 2018 and June 2022 at public health facilities in Bahir Dar city, Ethiopia. Data were collected using a structured checklist from patient charts. Data entry and analysis were done using EpiData 3.1 and Stata version 15, respectively. A Cox proportional Hazard regression model was used to identify predictors of mortality. Predictors with P < 0.05 in the multivariable regression were considered statistically significant. Results: Among the 401 TB-HIV co-infected patients, 59 (14.7%) died during the follow-up period. Predictors like lower BMI (AHR = 3.00, 95% CI = 1.44, 6.28), extrapulmonary TB infection (AHR = 3.30, 95% CI = 1.50, 7.29), presence of opportunistic infection (AHR = 5.07, 95% CI = 2.55, 10.08), functional status (bedridden: AHR = 4.49, 95% CI = 1.63, 12.33), and adherence to TB treatment (fair = AHR = 2.74, 95% CI = 1.41, 7.20, and poor = AHR = 3.75, 95% CI = 1.52, 9.23) were associated with mortality. Conclusion: Mortality among TB and HIV coinfected people was high at public health facilities in Bahir Dar city. This result suggested that in order to increase patient survival, it would be necessary to enhance nutritional status, increase adherence to TB treatment, and prevent opportunistic infections. Tuberculosis is one of the most common illnesses and causes of death among people living with HIV (PLWHIV). HIV positive individuals have a 20–30 times greater risk of contracting TB than HIV negative individuals due to lowered immunity. Reports also indicated that PLWHIV are more likely to develop active TB than HIV negative. 1,2 The global TB report 2021 stated that out of ten million people infected with TB, 8% were PLHIV. The proportion of TB episodes coinfected with HIV was highest in the African region, exceeding 50% in parts of Southern Africa. 2 TB and HIV form a lethal combination, each speeding the other's progress. As a result, individuals with co-infections of TB and HIV have a significant risk of increased mortality. Without proper treatment, 60% of HIV-negative people with TB on average and nearly all HIV-positive people with TB will die. 3 Globally, 1.6 million people died from TB and 1 million from HIV in 2017 and accounting for one in every 300,000 deaths from both diseases. 1 Most recently, about one-third of all AIDS-related deaths worldwide are caused by TB, which is the leading cause of death among HIV-positive individuals. An estimated 167,000 people died from TB-HIV coinfection in 2022. 3 The syndromic interaction between TB and HIV has played a ruinous role, with Africans bearing a disproportionate share of the burden. In 2019, people in Africa accounted for 73% of TB-HIV co-infection cases and 81% of all TB-HIV deaths.
