Abstract:Measles is a highly contagious and severe disease. Despite mass vaccination, it remains a leading cause of death in children in developing regions, killing 114,900 globally in 2014. In 2006, China committed to eliminating measles by 2012; to this end, the country enhanced its mandatory vaccination programs and achieved vaccination rates reported above 95% by 2008. However, in spite of these efforts, during the last 3 years (2013-2015) China documented 27,695, 52,656, and 42,874 confirmed measles cases. How measles manages to spread in China-the world's largest population-in the mass vaccination era remains poorly understood. To address this conundrum and provide insights for future public health efforts, we analyze the geospatial pattern of measles transmission across China during 2005-2014. We map measles incidence and incidence rates for each of the 344 cities in mainland China, identify the key socioeconomic and demographic features associated with high disease burden, and identify transmission clusters based on the synchrony of outbreak cycles. Using hierarchical cluster analysis, we identify 21 epidemic clusters, of which 12 were cross-regional. The cross-regional clusters included more underdeveloped cities with large numbers of emigrants than would be expected by chance (p = 0.011; bootstrap sampling), indicating that cities in these clusters were likely linked by internal worker migration in response to uneven economic development. In contrast, cities in regional clusters were more likely to have high rates of minorities and high natural growth rates than would be expected by chance (p = 0.074; bootstrap sampling). Our findings suggest that multiple highly connected foci of measles transmission coexist in China and that migrant workers likely facilitate the transmission of measles across regions. This complex connection renders eradication of measles challenging in China despite its high overall vaccination coverage. Future immunization programs should therefore target these transmission foci simultaneously.
Estimation of the relationship between meteorological factors and measles using spatiotemporal Bayesian model in Shandong Province, China

Abstract:Abstract Background Measles-containing vaccine (MCV) has been effective in controlling the spread of measles. Some countries have declared measles elimination. But recently years, the number of cases worldwide has increased, posing a challenge to the global goal of measles eradication. This study estimated the relationship between meteorological factors and measles using spatiotemporal Bayesian model, aiming to provide scientific evidence for public health policy to eliminate measles. Methods Descriptive statistical analysis was performed on monthly data of measles and meteorological variables in 136 counties of Shandong Province from 2009 to 2017. Spatiotemporal Bayesian model was used to estimate the effects of meteorological factors on measles, and to evaluate measles risk areas at county level. Case population was divided into multiple subgroups according to gender, age and occupation. The effects of meteorological factors on measles in subgroups were compared. Results Specific meteorological conditions increased the risk of measles, including lower relative humidity, temperature, and atmospheric pressure; higher wind velocity, sunshine duration, and diurnal temperature variation. Taking lowest value (Q1) as reference, RR (95%CI) for higher temperatures (Q2–Q4) were 0.79 (0.69–0.91), 0.54 (0.44–0.65), and 0.48 (0.38–0.61), respectively; RR (95%CI) for higher relative humidity (Q2–Q4) were 0.76 (0.66–0.88), 0.56 (0.47–0.67), and 0.49 (0.38–0.63), respectively; RR (95%CI) for higher wind velocity (Q2–Q4) were 1.43 (1.25–1.64), 1.85 (1.57–2.18), 2.00 (1.59–2.52), respectively. 22 medium-to-high risk counties were identified, mainly in northwestern, southwestern and central Shandong Province. The trend was basically same in the effects of meteorological factors on measles in subgroups, but the magnitude of the effects was different. Conclusions Meteorological factors have an important impact on measles. It is crucial to integrate these factors into public health policies for measles prevention and control in China.

Analysis on measles epidemiological characteristics of migrant population in Haidian District of Beijing during 2005-2009

Abstract:OBJECTIVE To analyze measles epidemiological characteristics in Haidian during 2005-2009 in order to prevent and control measles.METHODS Total 1 076 cases were investigated for epidemiological information.RESULTS Among all migrant cases,the average annual incidence was 20.7 per 100 000.Cases among floating population accounted for 58.0% of all cases;there were 271 cases aged below 5 years,with incidence of 25.2%;there were 306 cases aged between 20-29 years.Most cases accounted in April-May.27.8% patients had exact immune history,35.3% without immune history,36.9% with unknowing immune history.CONCLUSION Children aged﹤5 and young people aged 20-29 are the main crowds.We should strengthen management immunization and improve measles vaccination coverage for children among migrant people,which are important measures for controlling measles.Measles vaccination should also be covered migrant adults.

Abstract:Objectives Despite the adoption of a new childhood immunization program in China, the incidence of mumps remains high. This study aimed to describe the epidemiological characteristics of mumps in Jilin Province from 2005 to 2019 and to assess the transmissibility of mumps virus among the whole population and different subgroups by regions and age groups. Methods The Non-age-specific and age-specific Susceptible–Exposed–Pre-symptomatic–Infectious–Asymptomatic–Recovered (SEPIAR) models were fitted to actual mumps incidence data. The time-varying reproduction number ( R t ) was used to evaluate and compare the transmissibility. Results From 2005 to 2019, a total of 57,424 cases of mumps were reported in Jilin Province. The incidence of mumps was the highest in people aged 5 to 9 years (77.37 per 100,000). The two SEPIAR models fitted the reported data well ( P < 0.01). The median transmissibility ( R t ) calculated by the two SEPIAR models were 1.096 (range: 1.911 × 10 −5 –2.192) and 1.074 (range: 0.033–2.114) respectively. The age-specific SEPIAR model was more representative of the actual epidemic of mumps in Jilin Province from 2005–2019. Conclusions For mumps control, it is recommended that mumps-containing vaccines (MuCV) coverage be increased nationwide in the 5–9 years age group, either by a mumps vaccine alone or by a combination of vaccines such as measles-mumps-rubella (MMR) vaccine. The coverage of vaccines in Jilin Province should be continuously expanded to establish solid immunity in the population. China needs to redefine the optimal time interval for MuCV immunization.

A Hospital-Associated Measles Outbreak in Health Workers in Beijing: Implications for Measles Elimination in China, 2018.

Abstract:Background: Since the National Action Plan for Measles Elimination 2006-2012 was released in 2006, China has entered into an era of accelerated measles elimination. Despite substantial decline, measles outbreak continued to occur mainly in a non-targeted population, in particular health workers due to occupational exposure. We investigated a measles outbreak in a Chinese medicine hospital, Beijing, in March 2018. Methods: Descriptive analysis was performed on epidemiology of the outbreak, clinical and laboratory evaluations, vaccination status of cases, and public health response. A case-control study was also conducted to identify the associated risk factors for measles. Results: From March 13, 2018, through March 27, 2018, a total of 11 measles cases in health workers were reported in Hospital X, Beijing. The median age of cases was 26 years (range, 21-53) and 4 (36.4%) were male. Of the 11 cases, ten had not been vaccinated or had unknown vaccination status; two occurred in emergency departments. No inpatients became infected, 141 (83.9%) of whom in affected departments were 55 years of age or older. The outbreak was confirmed by laboratory testing; the virus strain was genotype H1. The response activities included isolation of cases, and enhanced health surveillance of health workers and inpatients, particularly in affected departments. A total of 2234 doses of measlescontaining vaccines (MCVs) were administered, covering 93.3% of susceptible health workers, in addition to 973 doses to the susceptible individuals in household and community. Lack of documentation of MCV vaccination before the outbreak was identified as a risk factor for measles (odd ratio: 3.333, 95% confidence interval: 1.295-8.621). Conclusions: High coverage of outbreak response immunization activities contributed largely to limit the spread of measles and might provide an indirect protection for inpatients. 2-Dose measles-containing vaccination of personnel in healthcare settings is warranted to prevent potential nosocomial transmission of measles. Abstract:OBJECTIVES: Varicella (chickenpox) is a highly transmissible childhood disease. Between 2010 and 2015, it displayed two epidemic waves annually among school populations in Shenzhen, China. However, their transmission dynamics remain unclear and there is no school-based vaccination programme in Shenzhen to-date. In this study, we developed a mathematical model to compare a school-based vaccination intervention scenario with a baseline (i.e. no intervention) scenario.METHODS: Data on varicella reported cases were downloaded from the Infectious Disease Reporting Information Management System. We obtained the population size, age structure of children aged 15 or under, the class and school distribution from Shenzhen Education Bureau. We developed an Agent-Based Susceptible-Exposed-Infectious-Recovered (ABM-SEIR) Model that considered within-class, class-to-class and out-of-school transmission modes. The intervention scenario was that school-wide vaccination intervention occurred when an outbreak threshold was reached within a school. We varied this threshold level from five to ten cases. We compared the reduction of disease outbreak size and estimated the key epidemiological parameters under the intervention strategy.RESULTS: Our ABM-SEIR model provided a good model fit to the two annual varicella epidemic waves from 2013 to 2015. The transmission dynamics displayed strong seasonality. Our results suggested that a school-based vaccination strategy could effectively prevent large outbreaks at different thresholds.CONCLUSIONS: There was a considerable increase in reported varicella cases from 2013 to 2015 in Shenzhen. Our modelling study provided important theoretical support for disease control decision making during school outbreaks and the development of a school-based vaccination programme.

Abstract:Due to the Expanded Program on Immunization (EPI) and supplementary immunization activities (SIAs) in China, the incidence of measles in China has decreased extensively. The incidence reached its lowest levels in contemporary history in 2012 and 2017, with incidence rates of 4.6 and 4.3 per million population, respectively. However, more than 147,000 measles cases were reported from 2013 to 2016. Furthermore, the proportions of cases in infants < 8 months and adults have been increasing since 2013, representing a considerable challenge for measles elimination in China. A total of 14,868 measles viruses were isolated from confirmed measles cases from 2011 to 2017, of which 14,631 were identified as the predominant endemic genotype, H1; 87 were identified as genotype A viruses that were vaccine associated strains; and 150 were identified as non-H1 genotype viruses. The non-H1 genotype viruses included 62 D8 viruses, 70 D9 viruses, 3 D11 viruses, 14 B3 viruses, and 1 G3 virus, which were identified as imported or import-related viruses that caused sporadic cases or small outbreaks. Most of the transmission chains detected during the period 2011-2012 were interrupted and were followed by many new transmission chains of unknown origin that spread, causing a large measles resurgence in China during 2013-2016. After 4 years of measles resurgence and continuous implementation of the routine immunization program and SIAs, the population immunity reached a sufficiently high level to interrupt most of the transmission chains; only a few strains survived, which continued to be sporadically detected in China in 2017. In the present study, the results from the combined epidemiological and molecular virological data demonstrated the great progress towards measles elimination in China by the further analysis of circulation dynamics for the endemic H1 genotype measles virus from 2011 to 2017. And this study accumulated critical baseline data on circulating wild-type measles viruses in China and provides comprehensive information to the world. These comprehensive baseline data provide evidence to support measles elimination in the future, not only in China but also in other countries worldwide. In addition, the information will be very useful to other countries for tracing their sources of measles cases and for identifying transmission links, which can help prevent potential measles outbreaks.

Abstract:Objective To understand the epidemiological characteristics of laboratory confirmed measles cases in Haidian district,Beijing,during 2009-2012.Methods A descriptive epidemiological study was conducted to analyze laboratory confirmed cases measles reported to China Diseases Reporting Information System.Results The annual incidence of measles was in decline during this period except in 2010.A total of 333 laboratory confirmed measles cases were reported,accounting for 60.65% of the total(333/549),and 269(80.78%) occurred during February-June.Among 328 cases with data available,the cases in age groups<8 months and 20-39 years accounted for 58.85%,the cases in migrant population accounted for 74.09%.The cases with the immunization history of measles containing vaccine(MCV) accounted for 20.12%,the cases without the immunization history accounted for 42.68% and the cases with the unknown immunization history accounted for 37.20%.The isolation rate of measles virus in tested samples was 26.09%(6/23).All the isolated strains were belonged to genotype H1.Conclusion The measles incidence was at lowest level in Haidian during this period,but it is still necessary to further reduce the incidence of measles by improving timely vaccination of the first dose of MCV for infants aged 8 months,increasing routine vaccination coverage and conducting supplementary immunization activities as well as the adult immunization in migrant population.

Abstract:Routine immunizations and supplementary immunization activities (SIAs) have significantly improved measles control over the past two decades in China. Progress towards eliminating measles currently faces multiple challenges as the infection age increases, and adult-targeted SIA strategies are being considered. This study developed an age-stratified susceptible-exposed-infectious-recovered model using a recently published contact matrix to depict measles transmissions between individuals in seven age groups. Hubei, a high measles-incidence province in central China, was the selected setting. The baseline scenario was calibrated by fitting with the 2012–2015 age-stratified incidence data. SIAs targeting multiple age groups were simulated. Adult-targeted (>29 years) two-year SIA cycles produced the greatest annual incidence rate decrease, reducing incidences by half over a long timespan with 90% coverage levels. Incidences could remain below 10/100,000 until 2030 if SIAs were provided to individuals ≥6 years old with at least 50% coverage. These findings will help officials prioritize supplementary vaccination strategies. Public health officials in China should consider adult-to-adult transmissions and provide adult-targeted SIAs. Although officials have reported approximately 90% SIA coverage in the past, SIAs for the adult population should be provided on shorter intervals, particularly for the aging population with decreased immunity.

Abstract:The China's national measles incidence rate has reduced sharply since 2008 ,but has arisen again since 2012 ,with some cities reporting multiple adult outbreaks .One possible reason for the rise of measles incidence rate may be that the actual vaccine coverage in China is lower than that reported ,due to the insufficient implementation of routine immunization .Also ,some populations remain susceptible to measles as they received neither routine immunization nor the supplementary immunization ,and the accumulation of such populations could also cause the outbreaks in the country .China has achieved significant progress on measles elimination ,but cautions should be taken to prevent the resurgence of the disease .China needs to further improve the routine vaccine coverage and the supplementary vaccination for priority regions and populations .Also ,the better implementation of the measles contingency plans will further help to control and eliminate measles outbreaks in China .

Abstract:Despite the adoption of a national immunization program in China, the incidence of mumps remains high. This study aimed to describe the epidemiological characteristics, including the time, region, occupation, and age, of mumps in Wuhan from 2005 to 2018 and to evaluate its transmissibility. In this study, the susceptible–exposed–infectious–asymptomatic–recovered (SEIAR) model fitted the actual incidence data of mumps. The effective reproduction number ( R t ) was used to evaluate and compare the transmission capacity in different areas. From 2005 to 2018, there were 36,415 cases. The incidence of mumps was highest among people aged 5–10 years (460.02 per 100,000). The SEIAR model fitted the reported mumps data well ( P < 0.01). The median transmissibility ( R t ) was 1.04 (range = 0–2.50). There were two peak spreads every year (from March to May and from October to December). The R t peak always appeared in the first 2 months of the peak incidence rate. The peak time of the epidemic spread of mumps was 1–2 months earlier than the peak incidence rate. The prevention and control measures of vaccination for children aged 5–10 years should be taken before the peak transmission capacity each year, 2 months before the peak of the outbreak, to reduce the spread of mumps.

Is Short-Term Exposure to Ambient Fine Particles Associated with Measles Incidence in China? A Multi-City Study.

Abstract:Background: China's rapid economic development has resulted in severe particulate matter (PM) air pollution and the control and prevention of infectious disease is an ongoing priority. This study examined the relationships between short-term exposure to ambient particles with aerodynamic diameter <= 2.5 mu m (PM2.5) and measles incidence in China.Methods: Data on daily numbers of new measles cases and concentrations of ambient PM2.5 were collected from 21 cities in China during Oct 2013 and Dec 2014. Poisson regression was used to examine city-specific associations of PM2.5 and measles, with a constrained distributed lag model, after adjusting for seasonality, day of the week, and weather conditions. Then, the effects at the national scale were pooled with a random-effect meta-analysis.Results: A 10 mu g/m(3) increase in PM2.5 at lag 1 day, lag 2 day and lag 3 day was significantly associated with increased measles incidence [relative risk (RR) and 95% confidence interval (CI) were 1.010 (1.003, 1.018), 1.010 (1.003, 1.016) and 1.006 (1.000, 1.012), respectively]. The cumulative relative risk of measles associated with PM2.5 at lag 1-3 days was 1.029 (95% CI: 1.010, 1.048). Stratified analyses by meteorological factors showed that the PM2.5 and measles associations were stronger on days with high temperature, low humidity, and high wind speed.Conclusions: We provide new evidence that measles incidence is associated with exposure to ambient PM2.5 in China. Effective policies to reduce air pollution may also reduce measles incidence.

Abstract:BACKGROUND:Measles vaccination over the past 50 years has greatly reduced the incidence of measles. However, measles among migrants and the resulting changes in epidemiological characteristics have brought new challenges to the elimination of measles. We aim to describe the measles epidemic trend over the past 30 years in a central district in Shanghai, China.METHODS:The present study was conducted in the Jing'an District, which is located in the center of Shanghai. Based on historical surveillance data of measles, we calculated the incidence of measles among local residents and migrants separately. Next, we classified all of the cases of the measles among local residents between 1984 and 2015 into 8 age groups and 5 birth cohorts. Finally, we calculated the measles incidence in each time period by the different age groups and birth cohorts, to understand the measles epidemic trend over past 30 years in the Jing'an District.RESULTS:A total of 103 cases of measles were reported from the Jing'an District, Shanghai, from 1984 to 2015. For infants less than 1 year of age and adults over 30 years of age, the incidence of measles continued to rise over the past 30 years. For a specific birth cohort, the incidence of measles after measles vaccination declined initially, and was then followed by a rebound.CONCLUSIONS:The incidence of measles in older adults and infants increased in some developed regions, which slows the process of measles elimination. This suggested that the population immunity against measles after measles vaccination would gradually reduce with time. We recommend supplemental immunization against measles in adults in order to reduce the immunity decline, especially for migrants.

Abstract:The distribution of histamine‐, octopamine‐, gamma‐aminobutyric acid‐ (GABA) and taurine‐like immunoreactivity in the bivalve mollusc Macoma balthica was studied immunocytochemically with antisera produced in rabbits. Histamine levels in the ganglia and whole animals were also measured by high‐performance liquid chromatography using a postcolumn derivatization method. Immunoreactivity for these substances, except for taurine, is found in the central nervous system of this species. The most extensive neuronal system is revealed with the antiserum against histamine. All the main ganglia contain histamine‐immunoreactive cell bodies, and a dense network of nerve fibers is seen in the ganglia and nerve roots. Histamine‐immunoreactive nerve fibers project to the mantle edge, lips and oesophagus. The basal part of the inhalant siphon is rich in histamine‐immunoreactive fibers. Unlike histamine, octopamine‐ and GABA‐like immunoreactivities are restricted to the central nervous system. Taurine‐like immunoreactivity is not found in the nervous system of this species. In the nervous system, histamine‐immunoreactive cell bodies and fibers are more numerous than those that are octopamine‐ and GABA‐immunoreactive. The distribution of these substance in the ganglia is different. GABA‐immunoreactive cells are typically smaller than most of the histamine‐ and octopamine‐immunoreactive cells. Most GABA‐ and octopamine‐immunoreactive cells and fibers are located in the pedal ganglion. Histamine is distributed more evenly in the ganglia and nerve roots. The biochemical measurements of histamine correlate well with the immunohistochemical findings and confirm the predominant location of the amine in the nervous tissue. These results suggest that histamine is more widespread than some other putative transmitters, and support the concept that histamine may have an important role in many physiological processes in molluscs. © 1993 Wiley‐Liss, Inc.
Unveiling measles transmission dynamics: Insights from a stochastic model with nonlinear incidence

Abstract:In this paper, taking into account the inevitable impact of environmental perturbations on disease transmission, we primarily investigate a stochastic model for measles infection with nonlinear incidence. The transmission rate in this model follows a logarithmic normal distribution influenced by an Ornstein–Uhlenbeck (OU) process. To analyze the dynamic properties of the stochastic model, our first step is to establish the existence and uniqueness of a global solution for the stochastic equations. Next, by constructing appropriate Lyapunov functions and utilizing the ergodicity of the OU process, we establish sufficient conditions for the existence of a stationary distribution, indicating the prevalence of the disease. Furthermore, we provide sufficient conditions for disease elimination. These conditions are derived by considering the interplay between the model parameters and the stochastic dynamics. Finally, we validate the theoretical conclusions through numerical simulations, which allow us to assess the practical implications of the established conditions and observe the dynamics of the stochastic model in action. By combining theoretical analysis and numerical simulations, we gain a comprehensive understanding of the stochastic model's behavior, contributing to the broader understanding of measles transmission dynamics and the development of effective control strategies.

Abstract:Measles remains a major threat to human health despite widespread vaccination. While we know that maternal antibodies can impair vaccine-induced immunity, the relative contributions of pre-existing immunity levels, maternal and infant characteristics on vaccine responses remain unclear, hampering evidence-based vaccination policy development. Here we combine serological data from 1,505 individuals (aged 0–12 years) in a mother–infant cohort and in a child cohort with empirical models to reconstruct antibody trajectories from birth. We show that while highly heterogeneous across a population, measles antibody evolution is strongly predictive from birth at the individual level, including following vaccination. Further, we find that caesarean section births were linked with 2.56 (95% confidence interval: 1.06–6.37) increased odds of primary vaccine failure, highlighting the long-term immunological consequences of birth route. Finally, we use our new understanding of antibody evolution to critically assess the population-level consequences of different vaccination schedules, the results of which will allow country-level evaluations of vaccine policy.

Abstract:Objective To analyze the epidemiological characteristics of measles in Shanghai during the year of 2006-2011,so as to provide evidence for measles control.Methods Data were collected through surveillance system of infectious disease and measles.Descriptive epidemiological analysis was conducted on measles surveillance data in Shanghai.Results A total of 5 180 measles cases were reported and the average annual incidence was 4.17/100 000,with the highest incidence in 2008 and lowest incidence in 2011.The incidence of measles in immigrant population was significantly higher than that in resident population.The measles cases were mainly children under 5 years old and young adults aged in 20-35 years.And scattered children,workers,farmers and household service had high incidence of measles.Conclusions The incidence of measles dropped to the lowest level in this decade,suggesting a coming era of measles elimination.Furthermore,it is essential needs to strength measles surveillance and promote vaccination program in children and immigrant workers in Shanghai,so as to improve the level of herd protection in the whole population.

Abstract:Objective To study epidemiological characteristics of measles in Haidian during 2005 ～2009 in order to prevent and control its epidemic in the future. Methods In total,330 cases of measles were investigated and analyzed for understanding their epidemiological characteristics. Results Among these 330 measles cases,61. 2 percent were migrant children and 34. 8 percent were Beijing children with permanent residence. One hundred and sixty-nine cases of measles (51. 2% ) aged one to eight months and another 161 cases (48. 8% ) aged eight months to two years. Peak of measles incidence was April to May of 2005 ～ 2009. Proportion of measles children with history of hospital visits in their incubation and infectivity period was 34. 5 percent and 100 percent,respectively and more than 50 percent of the children ever visited children’s hospitals as their first choice. Conclusions Measles occurred mostly in infants of eight months old or less in Haidian,Beijing during 2005 ～ 2009,with hospital aggregation in those with two years old or less, suggesting potential nosocomial infection.
