Abstract:Abstract Background To achieve hepatitis C virus (HCV) elimination, treatment programs need to be developed to engage, treat, and cure people who inject drugs (PWID). Methods We present final data from the Accessible Care Trial for curing HCV in PWID. The randomized clinical trial compared on-site, low-threshold HCV treatment with care-coordination at a NYC syringe service program (Accessible Care) with facilitated referral to local providers through a patient navigation program (Usual Care). Eligible participants were HCV RNA+ and had injected drugs in the past 90 days. Participants were randomized 1:1 to the Accessible Care or Usual Care arm. The primary endpoint was achievement of sustained virologic response (SVR12) within 12 months of enrollment. Secondary endpoints examined rates HCV care cascade step completion from referral to clinician, attending clinical visit, baseline lab testing, treatment initiation, and cure. Results Among the 572 participants screened, 167 met eligibility criteria and were enrolled, with two excluded post-randomization (N=165). Demographics were similar with a median age of 41 years; 22% women; 59% Hispanic and 5% non-Hispanic black. At baseline, 84% of participants had injected drugs in the past 30 days with those averaging 22 injections/month, 70% were receiving either methadone or buprenorphine, 57% were recently homeless, 7% were HIV+, and 11% were HCV treatment experienced. In the intention-to-treat analysis, 55/82 (67.1%) of the Accessible Care arm and 19/83 (22.9%) of the Usual Care arm achieved SVR12 (p< 0.001). The SVR12 rates of those starting therapy were 55/64 (85.9%) and 19/22 (86.3%) in the two arms (p=0.96). Loss to follow-up (12.2% and 16.9%, p=0.51) was similar in the two arms. Significantly more participants in the Accessible Care arm achieved all steps in the care cascade with the greatest attrition in the Usual Care arm seen in referral to clinician and attending clinical visit. Conclusion Among HCV RNA+ PWID significantly higher rates of cure were achieved using the Accessible Care model focused on low-threshold, destigmatized, flexible HCV care compared to facilitated referral. To achieve HCV elimination, expansion of treatment programs specifically geared toward engaging PWID is paramount. Abstract:A kinetic model for progression of a population of cutaneous melanomas through categories defined by the range of Breslow thickness, with melanomas in situ (MIS) in category 0, and melanomas with Breslow thickness ≥2 mm in category 3, is described. The model assumes that all melanomas start out in category 0; this category is further subdivided into indolent and progressing melanomas. Steady-state solutions for the distributions of excised melanomas were found. Depending on the proportion of indolent MIS, these solutions predict very different distributions of excised melanomas and melanoma mortality, when either frequency of examinations by physicians or sensitivity to melanoma is changed. Although it is not currently possible to differentiate between indolent and progressing MIS either clinically or histologically, solutions of this kinetic model can be used to determine the proportion of such indolent lesions in a population-based study. The steady-state solutions of the kinetic model can be used to analyze melanoma progression in any stable patient population, in which the total number of melanomas detected per year is either stable or varies slowly. As an example, melanoma progression is analyzed using the American Cancer Society estimates of melanoma incidence and mortality. For a fixed incidence rate, melanoma mortality and melanoma treatment cost in the USA could be significantly reduced by increasing the biopsy sensitivity of physicians to in-situ and thin-invasive melanomas.

Abstract:Background: People who inject drugs are experiencing syndemic conditions with increasing risk of infection with hepatitis C (HCV) and HIV. However, rates of accessing HCV and HIV testing and treatment among people who inject drugs are low for various reasons, including the criminalization of drug use, which leads to a focus on treating drug use rather than caring for drug users. For many people who inject drugs, health care becomes a form of structural violence, resulting in traumatic experiences, fear of police violence, unmet needs, and avoidance of medical care. There is a clear need for novel approaches to health care delivery for people who inject drugs. Objective: This study aimed to analyze the process of a multidisciplinary team—encompassing health care professionals, community representatives, researchers, and people with lived experience using drugs—that was formed to develop a deep understanding of the experiences of people who inject drugs and local ecosystem opportunities and constraints to inform the cocreation of low-barrier, innovative HCV or HIV care in a rural community. Given the need for innovative approaches to redesigning health care, we sought to identify challenges and tensions encountered in this process and strategies for overcoming these challenges. Methods: Analysis was based on an in-depth review of meeting notes from the project year, followed by member-checking with the project team to revise and expand upon the challenges encountered and strategies identified to navigate these challenges. Results: Challenges and tensions included: scoping the project, setting the pace and urgency of the work, adapting to web-based work, navigating ethics and practice of payment, defining success, and situating the project for sustainability. Strategies to navigate these challenges included: dedicated effort to building personal and meaningful connections, fostering mutual respect, identifying common ground to make shared decisions, and redefining successes. Conclusions: While cocreated care presents challenges, the resulting program is strengthened by challenging assumptions and carefully considering various perspectives to think creatively and productively about solutions.

Abstract:Abstract Background Sixty-eight percent of the nearly 3.5 million people living with hepatitis C virus (HCV) in the United States are people who inject drugs (PWID). Despite effective treatments, uptake remains low in PWID. We examined the social determinants of health (SDoH) that affect the HCV care cascade. Methods We conducted a secondary analysis of data from 720 PWID in a cluster-randomized trial. We recruited PWID from 12 drug-affected areas in Baltimore. Inclusion criteria were injection in the prior month or needle sharing in the past 6 months. Intake data consisted of a survey and HCV testing. Focusing on SDoH, we analyzed self-report of (1) awareness of HCV infection (in those with active or previously cured HCV) and (2) prior HCV treatment (in the aware subgroup). We used descriptive statistics and logistic regression for statistical analyses. Results The 342 participants were majority male and Black with a median age of 52 years. Women were more likely to be aware of their status but less likely to be treated. Having a primary care provider and HIV-positive status were associated with increased awareness and treatment. Unhoused people had 51% lower odds of HCV treatment. People who reported that other PWID had shared their HCV status with them had 2.3-fold higher odds of awareness of their own status. Conclusions Further study of gender disparities in HCV treatment access is needed. Increased social support was associated with higher odds of HCV treatment, suggesting an area for future interventions. Strategies to identify and address SDoH are needed to end HCV.

Abstract:Abstract Background This study evaluates a novel multidisciplinary program providing expanded access to Hepatitis C virus (HCV) treatment for rural Appalachian patients in South Carolina. This program identified patients using an opt-out emergency department screening program and aimed to achieve HCV cure using community paramedics (CPs) to link and monitor patients from treatment initiation through 12-week sustained virologic response (SVR). Methods HCV RNA positive patients (18yrs+) were eligible for enrollment if they failed to appear for a scheduled HCV appointment or reported barriers to accessing office-based treatment. CPs provided home visits (initial, 4-, 12-, and 24-weeks) using a mobile WiFi hotspot to support HIPAA compliant telemedicine appointments, perform focused physical assessments, venipuncture, and coordinated home delivery of medications. Descriptive statistics described participant characteristics, prevalence of SVR and patient satisfaction results at 12-weeks post treatment. Results Thirty-four patients were eligible for SVR labs by August 31st 2023, the majority were male (61.7%) and White (64.7%), with an average age of 56 (STD=11.7). Twenty-eight (82.4%) completed treatment and achieved 12-week SVR. Six (17.6%) were lost to follow-up. Two-thirds strongly agreed that they were satisfied with the overall care they received, and half strongly agreed that their overall health had improved. Conclusions This CP-augmented HCV treatment program demonstrated success curing HCV virus for rural patients who lacked access to office-based HCV treatment. Other healthcare systems may consider this novel delivery model to treat hard-to-reach HCV positive individuals.

Abstract:BackgroundProvider perceptions regarding barriers to and facilitators of hepatitis C (HCV) treatment initiation and adherence have not been fully evaluated in the interferon-free treatment era. New treatments have provided opportunities for non-specialists to treat HCV, underscoring the importance of understanding primary care provider (PCP) and specialist perspectives.MethodsBased on qualitative sampling principles, 12 PCPs and 12 hepatology providers (HPs) from the VA Pittsburgh Healthcare System completed audio-recorded semi-structured interviews. Qualitative analysts coded perceived barriers and facilitators from the interviews with 100% double coding. Codes were thematized and analyzed using Atlas.ti.ResultsKey barriers to treatment described by HPs and PCPs included patients’ substance use disorders, mental health, transportation availability, history of non-adherence, and concern about side effects. PCPs also focused on medication cost as a system-based barrier. The main facilitators of treatment initiation and adherence described by both HPs and PCPs were provider education and encouragement. HPs focused almost exclusively on provider-based facilitators, while PCPs noted patient-based facilitators including past adherence, media exposure to information about HCV medications, a desire to clear the virus, and positive feedback regarding treatment response.ConclusionsProviders generally focused on perceived patient-level barriers to HCV treatment initiation and adherence, as well as provider-level facilitators; PCPs additionally noted patient preferences and system-level issues that guide decision making regarding treatment initiation. While HPs focused almost exclusively on provider-level facilitators, PCPs additionally focused on patient-level facilitators of treatment. These data provide novel insights and suggest focusing on patient, provider, and system-level strategies to further improve HCV treatment initiation and adherence.
Implementation of a multidisciplinary, team-based model to treat chronic hepatitis C in the primary care setting: Lessons learned

Abstract:Hepatitis C virus (HCV) is the most common blood-borne virus in the U.S., and its incidence continues to rise. With approval of direct-acting antiviral medications, treatment for Chronic Hepatitis C (CHC) has become highly efficacious with a minimal side effect profile. Primary care physicians are well-positioned to address this increased demand, yet most do not feel comfortable treating CHC. In this case report, we describe implementation of a multidisciplinary team-based approach for treating CHC at multiple primary care sites across a large safety net health system. We focus on the evolving nature of implementation of our model through iterative Plan-Do-Study-Act (PDSA) cycles, highlighting the importance of developing an interdependent, multidisciplinary team, providing training, and ongoing support of Primary Care Hepatitis C Specialists, responding to the evolving nature of CHC treatment and policies, and ensuring high quality treatment. This process allowed us to continually grow and adapt our approach to make it feasible and successful. We share our "lessons" learned for others looking to bring CHC treatment, and potentially other specialty-based treatment, into the primary care setting.

Abstract:While published cure rates for new hepatitis C virus (HCV) treatments exceed 90%, barriers such as cost, medication access, housing or phone service instability, and substance use complicate medication adherence for uninsured, at-risk populations. Lincoln Community Health Center (LCHC) Pharmacy, in collaboration with primary care providers and care coordination professionals, implemented clinical pharmacist services to facilitate treatment of HCV infection in uninsured patients using manufacturers' patient assistance programs (PAP). Eighty-four (84) uninsured patients initiated treatment at LCHC using PAP during the first two years of the program. Nearly all patients (65/67 or 97%) who completed lab monitoring at 12 weeks post-treatment achieved undetectable viral load or a sustained virologic response (SVR-12), considered a proxy for cure. Successfully treating HCV infection in an uninsured, at-risk population may be achieved in a safety-net community health center by incorporating clinical pharmacist services into a team-based model.

Abstract:Abstract This study assessed the programmatic implementation of a multi-disciplinary program to prevent hepatocellular carcinoma through hepatitis C (HCV) testing, secondary prevention among diagnosed individuals (defined as interventions that prevent further progression of liver disease), and treatment within a medically and minority underserved clinic. The Lewis Cancer & Research Pavilion (LCRP) at St. Joseph's/Candler Health System is a Community Cancer Center and regional destination for cancer care in coastal Georgia. LCRP disparities efforts focus on cancer prevention, screening, and facilitating access to cancer care at two clinics for underserved and uninsured, low-income, predominately African American and Hispanic populations. Because the primary cause of hepatocellular carcinoma is cirrhosis from chronic hepatitis infection, in 2014 the LCRP began implementing Centers for Disease Control and Prevention (CDC) recommendations for HCV testing as a cancer prevention activity at the clinics. As HCV patients were identified, it became clear that disease specific protocols needed to be developed in order to improve nurse practitioners' clinical practice. Barriers identified and addressed during program implementation included: 1) perception that expensive HCV treatment is inaccessible for uninsured patients, 2) lack of knowledge of CDC clinical testing guidelines resulting in inaccurate HCV diagnosis 3) lack of knowledge of secondary prevention and treatment leading to inappropriate clinical care for HCV+ patients, and 4) under-utilization of Electronic Medical Record (EMR) and other data tracking tools to enhance clinical care. The LCRP and clinical team identified available community and statewide resources, physician champions, and education resources. A multi-disciplinary team developed protocols for secondary prevention activities while a clinical team implemented clinical testing and treatment algorithms using CDC, American Association for the Study of Liver Diseases (AASLD), and Infectious Diseases Society of American (IDSA) guidelines. These initiatives are consistent with CDC efforts to move HCV management to community-based health-care providers within primary care with supportive training and supervision by HCV care specialists. The clinic conducted a baseline chart audit of patients with HCV documented in their medical record. At baseline, CDC testing algorithms to confirm diagnosis were followed in 14% of patients who received the initial HCV+ antibody test. Six months after implementation of testing algorithms and other clinical and programmatic improvements, 44% of birth cohort patients (patients born between 1945 and 1964) were screened in compliance with CDC guidelines. Average age of HCV+ patients was 51, 58% were African American, 41% Caucasian, 62% female, and 38% male. Secondary prevention activities including hepatitis A & B vaccines, referral for alcohol and/or substance abuse counseling, depression and mental health screening, and identification of prevention and/or treatment barriers were initiated during the first six months of 2014 with 34% of HCV+ patients. Prevention activities conducted concurrently with treatment planning. Nurse practitioners at the clinic obtained co-management HCV treatment support from an internal medicine and primary care physician. This cancer prevention program demonstrated improvement in HCV testing, prevention, and treatment within a primary-care nurse-managed community clinic. The inter-disciplinary approach outlined in this model utilized evidence based guidelines and diagnostic algorithms. Application of secondary prevention activities and effective data tracking increased overall programmatic success, and ultimately contributes to the prevention of hepatocellular carcinoma among disparate populations. Abstract:Progress in treating chronic hepatitis C virus (HCV) infection has been slow amongst people who use drugs (PWUD). This study describes the HCV treatment cascade amongst people accessing a mobile clinic offering integrated low‐threshold buprenorphine and infectious disease services in Baltimore City. From May 1, 2021, to December 31, 2022, 560 people had a rapid HCV antibody test, of whom 201 (36%) had a positive result and amongst those, 117 (58%) had an HCV RNA test performed, 81 (40%) had a documented positive RNA, 45 (22%) were prescribed medication, 42 (21%) started medication, 32 (16%) completed medication, 22 (11%) had blood work to assess for sustained virologic response and 20 (10%) had a documented cure. Challenges including housing instability, insurance barriers and lack of venous access limit progress in this cascade. Providing integrated care models to meet the needs of PWUD in the community is necessary but not sufficient to make progress in improving HCV treatment. Removal of insurance restrictions, availability of point‐of‐care HCV RNA testing, development of rapid HCV treatment guidelines and development of long‐acting injectable HCV treatment are needed to move towards a same‐day, one‐time test and treat model of care.

Abstract:Abstract Background Persons experiencing homelessness have a high prevalence of hepatitis C virus (HCV) infection and multiple barriers to diagnosis and treatment. We implemented and evaluated a universal HCV screening and HCV care coordination and treatment program for Veterans living at a tiny shelter encampment on the West Los Angeles Veterans Affairs campus. Hepatitis C Cascade of Care All patients who completed treatment and obtained end of treatment lab work achieved SVR12, excluding Veterans who were lost to follow up (LTFU) and/or non-adherent (NA) (n=5), on treatment (n=4), or SVR12 pending (n=5). Methods We implemented a low-barrier to entry HCV treatment program for Veterans experiencing homelessness (VEH) that spanned April 2020-Dec 2021 (pre-intervention) and Jan 2022-April 2023 (post-intervention). The intervention involved a novel e-consult direct-to-treatment program facilitated by pharmacists and referral to ID/GI clinics (if ineligible for e-consult) and extensive engagement by encampment providers, including provision of direct-acting antivirals (DAAs) in weekly installments. Prior to the intervention, management was achieved solely through ID/GI clinic referrals. We evaluated HCV prevalence, along with linkage to care, treatment initiation, and sustained virologic response (SVR) of this novel program pre-and post-intervention. Data, including demographics, medical history healthcare visits, and other laboratory values were abstracted from the electronic medical record. Descriptive statistics were used to characterize prevalence of HCV and other covariates. Results Of the 704 Veterans who sheltered at the encampment at some point from April 2020-April 2023, 587 underwent HCV antibody screening (83%). Of those, 20% (120/587) were HCV Ab positive and 44% (53/120) had HCV viremia. Median age of those who were viremic was 64 years, 100% were male, 32% (17/53) were Black, 9% (5/53) Hispanic, and 58% (31/53) White. Pre- and post-intervention, 25 and 28 Veterans with HCV viremia were identified, 11 and 21 Veterans were linked to care (44% and 75%), 4 and 20 initiated DAAs (36% and 95%), and 1 and 6 achieved SVR (25% and 30%). Nine patients are currently on treatment (n=4) or pending SVR12 (n=5). Conclusion After an intervention involving e-consult and encampment provider engagement in VEH, 75% of Veterans with HCV viremia were linked to care, compared to 44% of Veterans prior to the intervention. Abstract:Background: Telemedicine offers the opportunity to provide clinical services remotely, thereby bridging geographic distances for people engaged in the medical system. Following the COVID-19 pandemic, the widespread adoption of telemedicine in clinical practices has persisted, highlighting its continued relevance for post-pandemic healthcare. Little is known about telemedicine use among people from socially marginalized groups. Methods: The No One Waits (NOW) Study is a single-arm clinical trial measuring the acceptability, feasibility, and safety of an urban point-of-diagnosis hepatitis C (HCV) treatment initiation model delivered in a non-clinical community setting. Participants enrolled in the NOW Study are recruited via street outreach targeting people experiencing homelessness and injecting drugs. Throughout the NOW Study, clinical care is delivered through a novel staff-facilitated telemedicine model that not only addresses geographic and transportation barriers, but also technology and medical mistrust, barriers often unique to this population. While clinicians provide high-quality specialty practice-based care via telemedicine, on-site staff provide technical support, aid in communication and rapport, and review the clinicians' instructions and next steps with participants following the visits. Research questionnaires collect information on participants' experience with and perceptions of telemedicine (a) prior to treatment initiation and (b) at treatment completion. Discussion: For people from socially marginalized groups with HCV infection, creative person-centered care approaches are necessary to diagnose, treat, and cure HCV. Although non-clinical, community-based staff-facilitated telemedicine requires additional resources compared to standard-of-care telemedicine, it could expand the reach and offer a valuable entrance into technology-delivered care for socially marginalized groups. Abstract:Introduction: The opioid crisis and the hepatitis C virus epidemic perpetuate and potentiate each other in a syndemic with escalating morbidity. Policy-driven funding can help resolve the syndemic through collaborative solutions that rapidly translate evidence-based interventions into real-world applications. Methods: We report development and programmatic evaluation of Peer-Assisted Telemedicine for Hepatitis C (PATHS), which utilizes State Opioid Response (SOR) funding to scale-up a positive randomized trial of peer-assisted telemedicine HCV treatment. PATHS employs staff within an academic medical center and partners with people with lived experience of drug use, "peers," to recruit rural-dwelling people who use drugs living with HCV. PATHS staff record patient data by abstracting clinical records or directly communicating with patients and peers. Peers are funded by a separate SOR-supported program administered through the state health authority. Peers support patients through HCV screening, treatment initiation via telemedicine, adherence, and cure. Results: Between March 2021 and June 2024, PATHS expanded to 18 of Oregon's 36 counties. In that time, PATHS diagnosed 198 rural PWUD with HCV. One hundred sixty-seven (84.3 %) linked to telemedicine and of these, 145 (86.8 %) initiated treatment. Of those who initiated treatment, 91 (62.8 %) completed treatment, of which 61 (67.0 %) are cured. Conclusions: By rapidly translating a clinical innovation in HCV treatment to achieve highly effective real-world results, PATHS models how policy-driven funding can facilitate collaboration between community partners, academic medical centers, and state health departments to end the opioid-HCV syndemic.

Abstract:Elimination of HCV infection as a public health concern by the end of this decade will require a concerted effort in all target populations, including drug‐users in the inner‐city. Several strategies have been proposed to identify, engage and provide HCV‐infected residents with antiviral therapy and maximise treatment and cure achievement. This study aims to assess the effectiveness of a multidisciplinary approach in delivering HCV treatment to people who inject drugs (PWID) within Vancouver's inner city. We have evaluated a novel approach, the Community Pop‐Up Clinic, for its ability to promote access to care and uptake of HCV therapy, with additional analyses of HCV reinfection and opioid‐related mortality. From January 2021 to August 2023, we evaluated 1968 individuals. 620 (31.5%) were found to carry HCV antibodies and of these, 474 (76.5%) were found to be viremic. Treatment engagement has been secured in 387 (81.6%). 326 (84.2%) have started treatment, 60 in the pre‐treatment phase and 1 died of an overdose in pre‐treatment. Of 326, 302 completed treatments, 18 are currently on treatment and 1 died of an overdose. Of 302 who completed treatment, 286 confirmed as cured (SVR 12), 16 are awaiting SVR 4, 2 had documented virologic relapse and 1 was reinfected. Three patients withdrew from treatment. By mITT, the cure rate is 286/288 (99.3%). We documented 2 overdose deaths over 326 PY. The data presented validates multidisciplinary programs such as ours aimed at treating HCV in inner‐cities and highlights societal benefits that could be achieved including lower overdose death rates.

Abstract:All‐oral, direct‐acting antivirals can cure hepatitis C virus (HCV) in almost all infected individuals; yet, many individuals with chronic HCV are not treated, and the incidence of acute HCV is increasing in some countries, including the United States. Strains on healthcare resources during the COVID‐19 pandemic negatively impacted the progress toward the World Health Organization goal to eliminate HCV by 2030, especially among persons who inject drugs (PWID). Here, we present a holistic conceptual framework termed LOTUS (Leveraging Opportunities for Treatment/User Simplicity), designed to integrate the current HCV practice landscape and invigorate HCV treatment programs in the setting of endemic COVID‐19: (A) treatment as prevention (especially among PWID), (B) recognition that HCV cure may be achieved with variable adherence with evidence supporting some forgiveness for missed doses, (C) treatment of all persons with active HCV infection (viremic), regardless of acuity, (D) minimal monitoring (MinMon) during treatment, and (E) rapid test and treat (TnT). The objective of this article is to review the current literature supporting each LOTUS petal; identify remaining gaps in knowledge or data; define the remaining barriers facing healthcare providers; and review evidence‐based strategies for overcoming key barriers.

Abstract:Background: Point-of-care hepatitis C virus (HCV) testing streamlines testing and treatment pathways. In this study, we established an HCV model of care in a homelessness service by offering antibody and RNA point-of-care testing. Methods: A nurse and peer-led HCV model of care with peer support were implemented between November 2021 and April 2022 at a homelessness service in Adelaide, Australia. All clients of the service were eligible to participate. Clients were offered an initial antibody point-of-care test, and antibody positive clients were immediately offered RNA point-of-care testing. Clients who tested RNA positive were linked to a viral hepatitis nurse for treatment. Results: A total of 230 clients received an HCV antibody point-of-care test, of which 68 (30%) were antibody positive and 11 (5%) were RNA positive. Of these, seven (64%) clients successfully completed treatment and five (45%) received a sustained virological response (SVR) test to confirm cure. Conclusions: We successfully established HCV testing and a treatment pathway at a homelessness service using HCV antibody and RNA point-of-care testing. The high testing uptake underscores the utility of HCV point-of-care testing when establishing HCV testing and treatment pathways. The low RNA positivity suggests that an initial HCV antibody test was cost-effective, and the four clients diagnosed with chronic HCV who were lost to follow-up indicate a need for enhanced treatment support.

Abstract:Background: Telemedicine has the potential to remove geographic and temporal obstacles to health care access. Whether and how telemedicine can increase health care access for underserved populations remains an open question. To address this issue, we integrated facilitated telemedicine encounters for the management of hepatitis C virus (HCV), a highly prevalent condition among people with opioid use disorder (OUD), into opioid treatment programs (OTPs). In New York State, OTPs are methadone-dispensing centers that provide patient-centered, evidence-based treatment for OUD. We investigated the integration and impact of facilitated telemedicine into OTP workflows in these settings. Objective: This study aims to understand OTP staff experiences with integrating facilitated telemedicine for HCV treatment into OTPs, including best practices and lessons learned. Methods: We conducted semistructured interviews with 45 OTP staff members (13 clinical, 12 administrative, 6 physicians, and 14 support staff members) at least one year after the implementation of facilitated telemedicine for HCV management. We used hermeneutic phenomenological analysis to understand OTP staff experiences. Results: We identified 4 overarching themes illustrating the successful integration of facilitated telemedicine for HCV care into OTPs. First, integration requires an understanding of the challenges, goals, and values of the OTP. As OTP staff learned about new, highly effective HCV therapies, they valued an HCV cure as a "win" for their patients and were excited about the potential to eliminate a highly prevalent infectious disease. Second, the integration of facilitated telemedicine into OTPs fosters social support and reinforces relationships between patients and OTP staff. OTP staff appreciated the ability to have "eyes on" patients during telemedicine encounters to assess body language, a necessary component of OUD management. Third, participants described high levels of interprofessional collaboration as a care team that included the blurring of lines between disciplines working toward a common goal of improving patient care. Study case managers were integrated into OTP workflows and established communication channels to improve patient outcomes. Fourth, administrators endorsed the sustained and future expansion of facilitated telemedicine to address comorbidities. Conclusions: OTP staff were highly enthusiastic about facilitated telemedicine for an underserved population. They described high levels of collaboration and integration comparable to relevant integrative frameworks. When situated within OTPs, facilitated telemedicine is a high-value application of telemedicine that provides support for underserved populations necessary for high-quality health care. These experiences support sustaining and scaling facilitated telemedicine in comparable settings and evaluating its ability to address other comorbidities. Abstract:Introduction/Objectives: The incidence of hepatitis C (HCV) infection is rising among people who inject drugs (PWID). Even in the context of known HCV prevention and treatment strategies, some PWID remain unengaged in HCV care. This study aimed to identify and characterize experiences and perceptions of PWID regarding the acceptability and effectiveness of HCV testing and treatment at a local syringe service program (SSP). Methods: A total of 36 PWID participated in semi-structured interviews at an SSP in New York City. Interviews were audio-recorded, transcribed, and coded by three coders, following a constructivist grounded theory approach. Relevant themes were identified as they emerged from the data. Results: Interviews with PWID revealed three themes related to the impact of SSPs on HCV care: (1) non-stigmatizing SSP environments, (2) the role of SSPs in improving HCV knowledge, and (3) acceptability of SSPs as sites for HCV care among PWID. Discussion: This paper contributes to the ongoing understanding that SSPs provide a well-accepted source of HCV services for PWID. Participants believed that SSPs are accessible and effective sites for HCV care, and suggested that stigma among PWID continues to affect receipt of HCV care in traditional settings. Conclusions: Understanding attitudes and beliefs of PWID regarding the effectiveness of SSPs as sites for HCV care is crucial for the development of focused strategies to reduce HCV transmission, and to ultimately achieve HCV elimination. Given this, further research is warranted investigating how best to improve HCV care at harm reduction sites such as SSPs.
