Abstract:Background: Paraplegia is a common and an important cause of disablement in all age group and in both sexes. In the days of ancient medicine it was most problematic for the physicians and surgeons to treat the cases and they considered it as “Hopeless”( Edwin Smith Papyrus, Breasted, 1930) written 4000 years ago. Paraplegia is caused by variety of diseases of different etiology from cerebral cortex to muscles. But measures should be taken to diagnose every possible cause and to offer best modality of treatment to eliminate the sufferings of patients. Materials And Methods: 50 patients of non-traumatic paraplegia were studied at Assam Medical College and Hospital from July 2005 to June 2006. Patients underwent a detailed clinical evaluation followed by laboratory investigations and neuroimaging studies. In our study the maximum incidence was in Results: the 4th decade (32%), the next being in the third decade (26%). Out of 50 cases there were 38 males (76%) and 12 females (24%). The occupation of the patients were cultivators (24%), housewife (24%), laborer (18%), student (10%), clerk (8%), teacher (4%), driver (4%), businessman ( 2%), engineer (2%), peon ( 2%). Acute onset was observed in 18 cases (36%), subacute onset in 4 cases (8%) and insidious in 28 cases (56%). The etiologies were found to be Transverse myelitis (20%), L.G.B.S (20%), Pott's paraplegia (16%), Lumbar disc prolapse (16%), Spinal tumor ( 8%), dorsal disc prolapse (4%), spinal arachnoiditis (4%), radiation myelitis (4%). We thus Conclusion: conclude that the present study shows acute transverse myelitis to be the commonest cause and spinal tuberculosis the second most common etiology of non-traumatic paraplegia. The patterns of clinical presentation and incidences of paraplegia due to various etiologies in this study bear close resemblance to the ones described by other workers.

Abstract:Comprehensive management and care of spinal-cord-lesioned patients start at the site of injury or at the moment of the onset of paralysis, depend with its consequences on the treatment in the acute phase, pass to the reactivation and rehabilitation phase and continue throughout all life of such patients. Considering that even a complete transverse lesion is only an instable, temporarily balanced situation, every small complication can destroy this balance and may lead from total independence to total dependence. Orthopedic, neurological, neurosurgical, urological, physio- or ergotherapeutic, socio-professional or other aspects in the field of comprehensive care and rehabilitation can never be analyzed or managed singularly, but have to be seen always in the context of the whole final outcome. The coordinating role of the paraplegiologist and the importance of adequate spinal cord injury units is stressed. The changing pattern of aetiopathogenesis or epidemiology of spinal cord lesions (increase of medical causes such as vascular lesions, tumors, infection disease) and the definition for rehabilitability due to the higher amount of elderly people will bring a new challenge for all careers in the field of rehabilitation.

Abstract:Paralysis of the gluteus medius after poliomyelitis often gives rise to secondary spinal deformity and inclination of the pelvis; the patient presents with a typical waddling gait. The paralysis is usually associated also with varying degrees of involvement and deformity of the extremities. Current treatment for paralysis of the gluteus medius has not been totally satisfactory. With the development of micro-neurovascular procedures, transfer of free muscle grafts for paralyzed or functionless muscles can be performed with functional recovery. The authors present a series of 12 cases, of which 10 were followed-up from 12 to 35 months. All of them had good cosmetic and functional results, with increased hip stability and stable gait. The operative technique, as well as the surgical anatomy, are described.

[Pott's disease paraplegia in children. Mechanics and therapeutic strategies. Six cases]

Abstract:Purpose: We report a series of 6 Pott's disease paraplegias treated between 1982 and 1996. Materials and method: Out of 15 children suffering from Pott's disease, 6 had paraplegia. Treatment consisted of anterior medullar decompression and anterior spine fusion with bone grafting. Two or three weeks later, posterior spine fusion was achieved systematically using a CD fixation device in 3 cases. Results: Neurological signs completely disappeared in 5 children. Vertebral fusion was correct in all patients and kyphosis was less than 50 degrees. Discussion: The posterior approach to the spine must be proscribed as a first step, except for spine dislocation. The anterior approach allowed us to drain the abscess, to correct the kyphosis, and to perform an anterior spine fusion. The posterior spine fusion was performed a few weeks later in order to avoid kyphosis aggravation. Conclusion: Prognosis of Pott's disease is good but at the present time, paraplegia remains too frequent. Adapted treatment must be performed without delay.

Abstract:Shoulder pain or paresis should be assessed carefully, as there are many possible causes, which can be osteoarticular, degenerative, inflammatory, or neurological. Weakness or pain can be related to cervicobrachialgia, plexitis, or focal mononeuropathy. The clinical picture should identify any muscular or mechanical origin of paresis responsible for pseudo-paretic functional limitation. Neurogenic scapulalgia with functional deficit implies the compression or entrapment of a nerve trunk including the axillary, long thoracic, accessory, suprascapular, or dorsal scapular nerves. Nerve conduction study and myography together with medical imaging help to identify the relevant etiology. Treatment mostly includes pain relief and physiotherapy, but surgery is rarely necessary.

Abstract:ABSTRACT Introduction: The inability of the spinal cord to propagate sensory and motor stimuli as a result of the disruption of the nerve tracts is called spinal cord injury. Objective: This study analyzes clinically and radiologically the hands and wrists of spinal cord injured patients, evaluating their motor and sensitive functionality, in order to determine if these patients are more likely to develop degenerative alterations. Methods: 14 patients (8 paraplegics and 6 tetraplegics) were evaluated, undergoing anamnesis and clinical examination - a scale of muscular strength (MRC - Medical Research Council) and the amplitude measurement of the movement with a manual goniometer (ROM), were used for objective evaluation - and x-ray exams. The results were compared with pre-existing data from other studies. Results: When asked, only one of the 14 observed patients complained about constant wrist pain, described as level 3 (weak to moderate), based on the visual analog scale (VAS). The motor evaluation, MRC and ROM divided the group of patients into two subgroups: paraplegic and tetraplegic patients. The x-ray analysis showed, based on Kellgren and Lawrence classification, that all exam images fit grades 1 or 2 of osteoarthritis and osteoarthrosis. Conclusion: In conclusion, spinal cord injured patients showed none or minimal clinical and radiological signs of osteoarthritis on hands or wrists. Overall, the hands and wrists of spinal cord-injured patients behave similarly to noninjured patients. Level of Evidence III; Retrospective Comparative Study.

Abstract:Abstract Introduction Paired associative stimulation (PAS) is a combination of transcranial magnetic stimulation (TMS) and peripheral nerve stimulation (PNS) and induces plastic changes in the human corticospinal tract. We have previously shown that PAS consisting of TMS pulses given at 100% of stimulator output and high-frequency PNS is beneficial for motor rehabilitation of patients with a chronic incomplete spinal cord injury (SCI). The therapeutic possibilities of this PAS variant for walking rehabilitation of paraplegic patients are unexplored. Case presentation A 47-year old man with traumatic incomplete paraplegia (AIS D, neurological level T7) received PAS to his left leg for 3 months at 12 months post injury (PAS1) and for an additional 3 months at 24 months post injury (PAS2). The right leg had normal AIS scores and was not stimulated. Before PAS, the patient was nonambulatory, could not stand without weight support, and was consequently not eligible for conventional walking rehabilitation. After PAS1, the patient could stand for 1.5 min and take 13 steps (24 steps in follow up) on parallel bars without weight support and was enrolled into conventional walking rehabilitation. He achieved independent walking ability with a rollator. During PAS2, walking distance increased 2.4 times faster than during the preceding year. The left leg AIS score and spinal cord independence measure mobility subscore increased. No adverse effects were detected. Discussion This is the first report of PAS with a high-frequency peripheral component that enabled and promoted walking rehabilitation. Together with previous reports on this technique, this result encourages further research into its therapeutic potential and mechanism.

Abstract:Cerebral palsy (CP) is defined as motor impairment that limits activity, and is attributed to non-progressive disturbances during brain development in fetuses or infants. The motor disorders of CP are frequently accompanied by impaired cognition, communication, and sensory perception, behavioural abnormalities, seizure disorders, or a combination of these features. CP is thought to affect three to four individuals per 1000 of the general population. The incidence, prevalence, and most common causes of CP have varied over time because of changes in prenatal and paediatric care. Medical management of children and adults involves care from primary-care physicians with input from specialists in neurology, orthopaedics, and rehabilitation medicine. Physicians should also work in conjunction with rehabilitation therapists, educators, nurses, social care providers, and schoolteachers. The focus of rehabilitation treatment has recently shifted to neurological rehabilitation in response to increasing evidence for neuroplasticity. This approach aims to improve development and function by capitalising on the innate capacity of the brain to change and adapt throughout the patient's life. As the life expectancy of individuals with CP approaches that of the general population, therapies must be developed that address the needs of adults ageing with disability.

Abstract:In 2002, MD was 55 years old when he experienced a complete left-sided BPA due to a road traffic accident. MRI confirmed the absence of any brain or spinal cord lesions but revealed a left plexus brachial injury from C5 to T1, including complete avulsion of C6 and C7 roots. Abstract:Objective To investigate the potential pathogenesis of acute paraplegia resulting from intrathecal chemotherapy and to analyze the factors influencing prognosis.Methods Six cases presented with acute paraplegia after intrathecal chemotherapy for leukemia from January 1 st,2015 to June 1st,2017 in our hospital.The patients' clinical manifestation,data of imaging and cerebrospinal fluid were analyzed.Result Six patients with leukemia were with an average age of 27.3 (7 to 55) years old.Four cases were intrathecally injected with cytarabine,and two cases were intrathecally injected with cytarabine and methotrexate.Paraplegia occurred in 5 patients during or immediately after the injection;pain occurred in 1 patient immediately after the injection followed with paraplegia 2 h later.Four patients underwent MRI scan of the spinal cord but failed to show responsible lesions.The youngest patient who received the most frequent intrathecal injections,the highest dosage of cytarabine,combination with methotrexate set up the most serious symptoms and the poorest prognosis.Conclusions Cytarabine has close relationship with the occurrence of paraplegia.The possible risk factors of poor prognosis might be younger age,more frequent injections,higher dosage,combination of cytarabine and methotrexate.

Abstract:Neuropathic (Charcot) arthropathy of the spine is a relatively rare problem that, nonetheless, must be considered in the differential diagnosis of any patient with degeneration of one or more levels of the spine associated with diminished or absent protective sensation. This study presents 15 patients in whom Charcot arthropathy of the spine developed after traumatic paraplegia. Eight were successfully treated with surgical fusion, and the remaining seven were treated nonoperatively. By the use of combined anterior and posterior fusion with extensive debridement, autogenous grafting, and posterior instrumentation, successful fusion can be achieved in patients with Charcot arthropathy of the spine. However, the surgical technique is demanding, the rehabilitation must be carefully supervised, and the postoperative complication rate remains high. The possibility of developing secondary levels of arthropathy below a previously successful fusion must be considered.

Thoraco-lumbar Spinal Fracture Complicated with Subacute Progressive Ascending Paraplegia

Abstract:Objective:To report the special cases of thoraco-lumbar spinal fractures complicated with suba-cute progressive ascending paraplegia,and to explore its mechanism.Method:Retrospectively summarizing and analyzing the clinical course,features,outcome,and surgical findings of9patients who sustained subacute pro-gressive ascending paraplegia posttrauma of thoraco-lumbar spinal fractures.Result:The paraplegic level began to ascend posttraumaticly24～72hours in all the9patients,and continued to go up for1～2weeks.And then stayed at a certain level.One of the nine patients showed the paraplegia level at C5,one at C7,two at T2,one at C4,two at T7-8,one at C6-7,and one at T10.All the patients showed complete paraplegia although they showed incomplete paraplegia initially.The second exploring operations were performed,and the following macro-pathological changes were found:(1)There was no hemorrhage in epidural space or within spinal cord.There was no obstruction in the epidural space.(2)Cerebrospinal fluid greatly diminished or distinguished.(3)A long segment of spinal cord above fracture level showed dark purple color,and no pulse of spinal cord was seen.(4)The spinal cord shrank and thinned.The follow-up time was5months to3years.6cases died from various complications posttraumaticly1～2.5years.3cases survived3years without improvement in their para-plegia.Conclusion:Thoraco-lumbar spinal fracture complicated with subacute progressive ascending paraplegia is a special type spine fracture,its occurrence rate is about2.3%in our series and no effective method to prevent its progression results in a catastrophic prognosis.The direct cause is ascending ischemia necrosis of spinal cord.

Abstract:The authors present a case-report of a 25-year-old patient who developed symptoms of transversal injury of the spine. The patient was followed-up for 2 years and had a stable course of Chiari malformation I and syringomyelia. Motor disorders and deep sensory disorders developed without external cause and continued to increase during 2.5 weeks with the further development of pelvic disorders. After the urgent cranial/vertebral decompression with the plastics of the dura mater by a fragment of a wide fascia of the thigh, there was an increase in muscle power (with the complete recovery in hands), regress of deep sensory disorders in hands and pelvic disorders. A literature review on case-reports of acute neurological symptoms in Chiari malformation I and syringomyelia in presented.

Abstract:Tuberculosis is a rare disease in economically developed countries. Paraplegia is one of its severe complications, occurring in cases of spondylodiscitis with compressive epiduritis and/or pathological fracture of vertebral bodies with subacute kyphosis. Six patients aged 15-75 years were treated in our department from May 2005 to April 2006 by the same operator (L.N.) for paraplegia complicating Pott's disease. Patients' neurologic function at admission and their outcomes three and 12 months after surgery were graded using the Frankel classification. MRI showed a single lesion in five cases and skip lesions in one case. Standard antituberculosis chemotherapy was started immediately within the first week of admission in five patients following a 12-month regimen. Principles of surgical treatment were ventral spinal cord decompression (with or without dorsal stage procedure), immediate grafting, correction of subacute spinal deformity and instrumentation in all cases. The diagnosis of Mycobacterium tuberculosis was confirmed by laboratory investigations in four cases and by histopathological examination in all cases. One patient who had undergone prior isolated laminectomy performed by an on call team presented neurological deterioration and progressive kyphosis. Neurological outcome improved in five patients. Four of them were able to walk unaided within the first three months after surgery. No perioperative complication occurred. In paraplegia complicating vertebral tuberculosis, overall neurological outcome appeared to be good in case of delayed emergency surgical management combined with antituberculosis chemotherapy.

Abstract:Lower lumbar paraspinal muscles constitute a compartment as they are surrounded by distinct fascial and bony boundaries. Lumbar paraspinal compartment syndrome is a rare entity, often caused by intense exercise, but also can be a postoperative complication. We present a 60-year-old man with low back pain, numbness in the left lower back and radicular pain in the left lower extremity, which started after a surgery that involved prolonged positioning on the left side 7 years before, and persisted to the day of evaluation. There was an immediate transient rise in the creatine kinase after surgery. Electromyography showed a left lower lumbar–sacral plexopathy and a lumbar spine MRI revealed fatty infiltration of the lower lumbar–sacral paraspinal muscles. The emergence of radicular lower limb pain was likely due to the compression of the proximal portion of lumbar–sacral plexus during the acute stage of rhabdomyolysis.

Abstract:Rapidly progressive non-traumatic paraplegia in a child is uncommonly encountered in clinical practice, but is an important presentation to consider given the potential for significant morbidity. We present the case of an 11-year-old girl who was found to have hyperacute paraplegia due to spinal cord infarction. We discuss the appropriate workup, differential diagnosis in children and how this relates to adults; and describe the prognosis and current state of management options for spinal cord infarction.
