Long working hours and preventive oral health behaviors: a nationwide study in Korea (2007-2021)

Abstract:Background: This study explored the association between working hours and preventive oral health behaviors. Methods: In total, 48,599 workers (22,992 females) were included from the Korea National Health and Nutrition Examination Survey (2007-2021). Weekly working hours were self-reported. The following three preventive oral health behaviors were set as outcomes: participation in annual dental check-ups; adherence to the recommended toothbrushing frequency (≥twice a day); and use of interdental cleaning devices. We estimated odds ratios (ORs) and 95% confidence intervals (CIs) using logistic regression. Results: In male workers, the adjusted OR (95% CI) of the association between working ≥55 h/week and each outcome was 0.84 (0.77-0.92) for dental check-ups, 0.82 (0.72-0.94) for toothbrushing ≥twice a day, and 0.83 (0.76-0.92) for utilization of interdental cleaning device when compared to 35-40 h/week. In female workers, the adjusted OR (95% CI) of the association between working ≥55 h/week and each outcome was 0.79 (0.70-0.89) for dental check-ups, 0.88 (0.70-1.11) for toothbrushing ≥twice a day, and 0.80 (0.71-0.90) for utilization of interdental cleaning device when compared to 35-40 h/week. Additionally, low socio-economic status, such as low educational attainment, low income level, and blue-collar occupations, were major risk factors associated with non-adherence to preventive oral health behaviors in both male and female workers. Conclusions: Our study suggests that individuals who work long hours are more likely to exhibit undesirable oral health behaviors.

Abstract:Objectives Long working hours and overwork are growing public health concerns in the Western-Pacific region. We explored the relationship between working hours and smoking behaviors of Korean workers. Methods This study included 284 782 observations (50 508 workers) from four nationwide cohort studies in Korea. Using generalized estimating equations, we estimated the associations of working hours with current smoking status, smoking initiation, and smoking cessation within each cohort. Cohort-specific estimates were combined through random-effect meta-analysis. Effect sizes were presented as odds ratios (OR) and 95 confidence intervals (CI). Results The overall smoking prevalence was 26.8% within the cohorts. The adjusted OR (95% CI) of the association between working hours and current smoking were 1.01 (0.94–1.08) for <35 hours/week, 1.04 (1.01–1.09) for 41–48 hours/week, 1.06 (1.01–1.10) for 49–54 hours/week, and 1.07 (1.04–1.10) for ≥55 hours/week compared with 35–40 hours/week. The adjusted OR (95% CI) of the association between working hours and smoking cessation in the follow-up were 0.93 (0.85–1.02) for <35 hours/week, 0.89 (0.83–0.96) for 41–48 hours/week, 0.87 (0.81–0.95) for 48–54 hours/week, and 0.91 (0.85–0.98) for ≥55 hours/week compared with 35–40 hours/week. No clear associations were observed between working hours and smoking initiation. Conclusion Long working hours are associated with high current smoking risk and reduced likelihood of smoking cessation among Korean workers. Abstract:OBJECTIVES: We aimed to explore the association between long working hours and health-related productivity loss (HRPL), due to either sickness, absenteeism or presenteeism, stratified by household income level.METHODS: From January 2020 to February 2020, data were collected using a web-based questionnaire. A total of 4197 participants were randomly selected using the convenience sampling method. The nonparametric association between weekly working hours and HRPL was determined. Subsequently, a stratified analysis was conducted according to household income (1st, 2nd, and 3rd tertiles). Finally, the differences in HRPL of the different working hour groups (<40, 40, 40-51, and ≥52 hours) were investigated using a multivariate linear regression model.RESULTS: Long working hours were more significantly associated with HRPL, as compared to the 'standard' working hours (40 hours/week). A larger proportion of productivity loss was associated with the presenteeism of workers, rather than absenteeism. The relationship between HRPL and weekly working hours was more prominent in the lower household income group.CONCLUSIONS: The results of our study indicate that HRPL is associated with long working hours, especially in the lower household income group. Reducing the workload for the individual employee to a manageable level and restructuring sick leave policies to effectively counteract absenteeism and presenteeism may be a feasible option for better labor productivity and employee health.

Effect of long working hours on psychological distress among young workers in different types of occupation

Abstract:Background: We investigated how the association between long working hours and psychological distress varies across different employment and occupation types in young workers. Methods: Examining a nationally representative sample of 7246 Korean workers (3621 women) aged 15 to 40, we analyzed 23,492 observations spanning from 2016 to 2020. Psychological distress was measured using the Brief Encounter Psychosocial Instrument. We employed a generalized estimating equation to estimate odds ratios (ORs) and 95% confidence intervals (CIs). Results: Of the total observations, 5.2% worked <35 h/week, 52.9% worked 35-40 h/week, 23.5% worked 41-48 h/week, 10.3% worked 49-54 h/week, and 8.2% worked ≥55 h/week. The OR (95% CI) of the association between long working hours and psychological distress was 1.38 (1.11-1.72) for <35 h/week, 1.47 (1.32-1.65) for 41-48 h/week, 1.74 (1.49-2.04) for 49-54 h/week, and 2.11 (1.75-2.55) for ≥55 h/week compared to 35-40 h/week. The OR (95% CI) of the association between working ≥55 h/week and psychological distress was significantly higher among wage workers (OR [95% CI]: 2.37 [1.94-2.89]) compared to self-employed workers (OR [95% CI]: 0.84 [0.52-1.36]). Additionally, the OR (95% CI) of the association between working ≥55 h/week and psychological distress was significantly higher among white-collar workers (OR [95% CI]: 3.24 [2.54-4.13]) compared to service/sales workers (OR [95% CI]: 1.22 [0.86-1.72]) or blue-collar workers (OR [95% CI]: 1.71 [1.10-2.67]). No clear gender differences were observed. Conclusion: Psychological distress caused by long working hours can be pronounced among white-collar and wage workers.

Abstract:Objectives Long working hour is a known risk factor for metabolic diseases. We explored the association between working hours and metabolic dysfunction–associated steatotic liver disease (MASLD). Study design Data on working hours among 22,818 workers (11,999 females) from the Korea National Health and Nutrition Examination Survey (2013–2021) were used for this study. Methods MASLD was defined as a combination of hepatic steatosis combined with one or more of cardiometabolic risk factors (overweight/obesity, prediabetes/diabetes, raised blood pressure, hypertriglyceridemia, low high-density lipoprotein cholesterol). Hepatic steatosis was assessed using the hepatic steatosis index. Logistic regression was used to estimate the odds ratios (ORs) and 95% confidence intervals (CIs). Results The overall prevalence of MASLD was 30.4% in men and 18.1% in women. Among male workers, 20.2% worked ≥55 h/week, whereas among female workers, 10.1% worked ≥55 h/week. Compared with working 35–40 h/week, working ≥55 h/week was positively associated with overweight/obesity (OR: 1.21; 95% CI: 1.05–1.40), pre–diabetes mellitus (pre-DM)/DM (OR: 1.20; 95% CI: 1.04–1.38), raised blood pressure (OR: 1.17; 95% CI: 1.02–1.35), and presence of any cardiometabolic risk factors (OR: 1.56; 95% CI: 1.21–2.02). The adjusted OR (95% CI) of the association between working hours and MASLD was 1.27 (1.09–1.47) for ≥55 h/week compared with working 35–40 h/week in male workers. In female workers, long working hours were not clearly associated with cardiometabolic risk factors and MASLD. Conclusion Long working hours are positively associated with MASLD among Korean male workers. Policy interventions are needed to mitigate the adverse metabolic effects of prolonged working hours.

Abstract:Ornithobacterium rhinotracheale was found to cause growth retardation in both turkeys and chickens after experimental intra-air sac administration and to cause growth retardation together with airsacculitis and pneumonia after aerosol administration. Both turkey and chicken isolates of O. rhinotracheale were able to induce the same kind of respiratory inflammations and weight-gain losses in chickens as well as turkeys. Turkey rhinotracheitis virus was found to have a triggering effect on the O. rhinotracheale infection in turkeys, and Newcastle disease virus and to a lesser extent infectious bronchitis virus showed triggering effects on the O. rhinotracheale infection in chickens. Ornithobacterium rhinotracheale could be reisolated from affected organs of experimentally infected birds.
Factors Influencing the Unmet Healthcare Needs of Elderly with Chronic Diseases : Focusing on the 2017 Korean National Survey on Elderly

Abstract:This study endeavors to provide basic data for healthcare policy development by identifying factors that influence the unmet healthcare needs of the elderly with chronic diseases. The study utilized data from the 2017 Korean National Survey on the Elderly, and analyzed 9,117 elders afflicted with more than 1 chronic disease. We determined that 851 (9.3%) elders experienced unmet healthcare needs from hospital & clinical treatment, and 1,469 (16.1%) elders experienced unmet dental treatment. 'Economic difficulty' was the most common reason expressed by 425 (49.9%) elders for unmet healthcare needs from hospital and clinical treatment, and by 1,066 (72.6%) elders for dental treatment. Furthermore, unmet healthcare needs in hospital and clinical treatment were affected by various factors, including spouse, education, self-rated health status and exercise, whereas unmet healthcare needs in dental treatment were affected by factors such as spouse, education, medical aids, self-rated health status, smoking, and exercise. Based on the research results achieved, we propose a necessity to establish healthcare policies that consider the influencing factors, and help resolve the unmet healthcare needs of the elderly with chronic diseases.

Systematic review and meta-analysis of the association between long working hours and hypertension risk

Abstract:Background Hypertension, a major public health problem worldwide, has been linked to lifestyle factors and work conditions, with conflicting evidence on the association between long work hours and risk of hypertension. Methods We conducted a systematic review and meta-analysis of observational studies to investigate the relationship between working hours and hypertension or blood pressure, assessed the risk of bias and performed subgroup analyses. The protocol was registered with the International Prospective Register of Systematic Reviews. Results The pooled OR for the association between long working hours and risk of hypertension was 1.09 (95% CI: 0.88 to 1.35) in the 15 studies that used hypertension as the outcome. In the three studies that used blood pressure as the outcome, diastolic blood pressure was higher among those with long working hours compared with those with non-long working hours (1.24 mm Hg, 95% CI: 0.19 to 2.29). In subgroup analysis, the pooled OR for the association between long working hours and risk of hypertension was 1.28 (95% CI: 1.14 to 1.44) and 1.00 (95% CI: 0.64 to 1.56) in women and men, respectively. Conclusions Although this study could not clearly confirm the relationship between long working hours and hypertension, the subgroup analysis suggests that long working hours may be associated with hypertension, particularly among women. More reliable research is needed to establish causality. PROSPERO registration number CRD42023406961.

Abstract:Abstract Background “Unmet healthcare needs” refers to the situation in which patients or citizens cannot fulfill their medical needs, likely due to socioeconomic reasons. The purpose of this study was to analyze factors related to unmet healthcare needs among South Korean adults. Methods We used a retrospective cross-sectional study design. This nationwide-based study included the data of 26,598 participants aged 19 years and older, which were obtained from the 2013–2017 Korea National Health and Nutrition Examination Surveys. Using multiple logistic regression models, we analyzed the associations between factors that influence unmet healthcare needs and participants’ subgroups. Results Despite South Korea’s universal health insurance system, in 2017, 9.5% of South Koreans experienced unmet healthcare needs. In both the male and female groups, younger people (age 19–39) had a higher odds ratio (OR) of experiencing unmet healthcare needs compared to older people (reference: age ≥ 60) (men: OR 1.83, 95% confidence interval [CI] = 1.35–2.48; women: OR 1.42, 95% CI 1.12–1.81). In particular, unlike men, women’s unmet healthcare needs increased as their incomes decreased (1 quartile OR 1.55, 2 quartiles OR 1.29, 3 quartiles OR 1.26). Men and women showed a tendency to have more unmet healthcare needs with less exercise, worse subjective health state, worse pain, and a higher degree of depression. Conclusions The contributing factors of unmet healthcare needs included having a low socioeconomic status, high stress, severe pain, and severe depression. Considering our findings, we suggest improving healthcare access for those with low socioeconomic status.

Abstract:Abstract Background Working hour regulation in Korea is being revised to allow increasing variability in number of working hours. We sought to investigate the association between variability in the number of daily or weekly working hours with or without long working hours (> 52 h/w) and mental health among South Korean workers. Methods We used data from 28,345 full-time, non-shift employed workers working more than 30 h per week participating in the Korean Working Condition Survey in 2017. We defined six groups according to variability in daily or weekly working hours (same number vs different number) and weekly working hours (31–40, 41–52, > 52 h per week). Odds ratios (ORs) and confidence intervals (CIs) for self-reported depressive symptoms and anxiety were calculated using workers with same number of working hours/31–40 h per week as the reference. Results Variability in number of work hours every day or week combined with > 52 working hours per week showed the highest risk of depressive symptoms (OR = 5.13, 95% CI 3.25–8.11) and anxiety (OR = 3.75, 95% CI 2.39–5.88) compared to the reference group, controlling for age, sex, education, occupation, industry, salary, workers’ choice of working hours and overtime payment. Workers working ≤52 h/w were adversely impacted by variable working hours as well. Conclusions Variable daily or weekly working hours were associated with poorer self-reported depressive and anxiety symptoms in Korea, among full-time and non-shift workers. Reform of the Korean Labor Standards Act warrants consideration.

Abstract:Purpose: This analysis aims to evaluate the association between the time since and reason for a patient's last dental appointment across clinical oral health outcomes. Materials and methods: We used data from the 2017-2020 National Health and Nutrition Examination Survey (NHANES), a cross-sectional, nationally-representative survey of noninstitutionalized US adults. The predictors were the time since and the reason for the last dental appointment (routine vs. urgent). We examined the presence and number of missing teeth and teeth with untreated coronal and root caries. Multivariable regression models with interaction were used to assess the association between the time since the last dental appointment and clinical oral health outcomes among routine and urgent users separately. Results: Two-thirds of the US population had a dental appointment within a year, while nearly 44 million individuals did not visit a dentist for the last three years. The odds of having teeth with untreated coronal or root caries increased with the length of time since the last appointment, and urgent users had worse dental outcomes compared to routine users. Compared to those who had a dental appointment within a year, individuals who had their last dental appointment more than 3 years ago had 2.94 times the average number of teeth with untreated caries among routine users (95%CI=2.39, 3.62) and 1.60 times the average among urgent users (95%CI=1.05, 2.43). Conclusions: Recent, routine dental appointments are associated with improved oral health outcomes. The outcomes reiterate how social determinants of health impact access to oral health care and oral health outcomes.

Abstract:There has been no subsequent meta-analysis examining the effects of long working hours on health or occupational health since 1997. Therefore, this paper aims to conduct a meta-analysis covering studies after 1997 for a comparison. A total of 243 published records were extracted from electronic databases. The effects were measured by five conditions, namely, physiological health (PH), mental health (MH), health behaviours (HB), related health (RH), and nonspecified health (NH). The overall odds ratio between long working hours and occupational health was 1.245 (95% confidence interval (CI): 1.195–1.298). The condition of related health constituted the highest odds ratio value (1.465, 95% CI: 1.332–1.611). The potential moderators were study method, cut-point for long weekly working hours, and country of origin. Long working hours were shown to adversely affect the occupational health of workers. The management on safeguarding the occupational health of workers working long hours should be reinforced.

Abstract:Background: Many studies have been conducted to investigate the harmful effect of shift work on physical and mental health. Although, by definition, "working evening shift" is included in the scope of shift work, most related studies conducted thus far have focused on working night shifts, overtime work, or different types of shift work, with little research effort dedicated to "working evening shifts." Therefore, to fill this research gap, we investigated the effect of working evening shifts on workers' mental health.Methods: The participants of this study were 16,692 employees of the 50,205 that participated in the 5th wave of the Korean Working Conditions Survey. We performed χ2 test and logistic regression analysis to analyze the effects of independent variables on health problems and calculated odds ratios and 95% confidence intervals (CIs).Results: In the logistic regression analysis adjusted for sociodemographic characteristics, health-related factors, and work-related characteristics, employees who worked evening shifts showed higher levels of depression and anxiety compared to those that did not. In particular, the adjusted odds ratios of the group working evening shifts between one and nine times a month were the highest with 2.723 (95% CI: 2.014-3.682) for depression, 3.294 (95% CI: 2.547-4.259) for anxiety.Conclusions: The results of our study suggest that working evening shifts has a negative effect on employees' mental health. This trend decreased with an increase in the monthly frequency of evening work.
Association of rotating night shift work with tooth loss and severe periodontitis among permanent employees in Japan: a cross-sectional study

Abstract:Background The modern 24/7 society demands night shift work, which is a possible risk factor for chronic diseases. This study aimed to examine the associations of rotating night shift work duration with tooth loss and severe periodontitis. Methods This cross-sectional study used data from a self-administered questionnaire survey conducted among 3,044 permanent employees aged 20–64 years through a Japanese web research company in 2023. The duration of rotating night shift work was assessed using a question from the Nurses’ Health Study. Tooth loss was assessed based on self-reported remaining natural teeth count. Severe periodontitis was assessed using a validated screening questionnaire comprising four questions related to gum disease, loose tooth, bone loss, and bleeding gums. We employed linear regression models for tooth loss and Poisson regression models for severe periodontitis, adjusting for demographic, health and work-related variables and socioeconomic status. Results Among participants included, 10.9% worked in rotating night shifts for 1–5 years, while 11.0% worked in such shifts for ≥6 years. In fully adjusted models, rotating night shift work duration of 1–5 years was associated with tooth loss (beta −0.74, 95% confidence interval (CI) [−1.55 to 0.08]) and severe periodontitis (prevalence ratio 1.80, 95% CI [1.33–2.43]); however, the association with tooth loss was not statistically significant. Conclusions This study supports that employees who work short-term rotating night shifts may experience poor oral conditions. Further research is needed to determine whether long-term rotating night shift work is associated with deteriorated oral health.

Abstract:This study investigated the prevalence of unmet needs for health care among Korean adults and related factors. Using data from the 2012 Korea Health Panel, 1,896 adults aged 65 and over and two age groups(Young-Old(66-74) and Old-Old()) were analyzed to identify these factors. Logistic regression analysis was used to examine the main factors associated with unmet medical needs. According to the results of this study, the factors influencing unmet medical needs with regard to economic factors were Subjective Health Status(young-old), Disability(young-old), Activity Limitation(young-old), Education Level(old) and Economic Activity(old). The factors influencing unmet medical needs were factors of attitude, information, and disease. factors were Disability(young-old), Activity Limitation(young-old), Education Level(old) and Economic Activity(old). Therefore, further research that investigates unmet needs depending on age group in the elderly would suggest helpful policy implications.

Background:Workers' reporting of work-related injuries or illnesses is important for treatment and prevention, yet research often focuses on reporting barriers. This study aimed to identify factors related to work-related musculoskeletal disorder (WRMSD) reporting attitudes and their connection to reporting intention and behavior.Methods:We analyzed data from 377 direct care workers employed in 19 long-term care facilities in South Korea. A self-administered questionnaire collected demographics, job characteristics, physical and psychosocial factors, musculoskeletal symptoms, reporting attitudes, and WRMSD reporting intentions and behavior between May and August 2022. We used a generalized linear mixed model with a random intercept by employers to identify factors influencing reporting attitudes. To explore the relationship between reporting attitude and reporting intention and behavior, simple logistic regression was also conducted.Results:We achieved an 86% response rate. The majority of the study participants were female (87.2%), married (95.9%), and non-immigrant (72.8%). Of the study participants, 48.9% had no intention to report WRMSDs, and 44.3% held negative reporting attitudes. Among 200 workers with WRMSDs, 86.5% did not report them. Attitudes were associated with work duration, safety training, management safety priority, WRMSD experience, and symptom severity and frequency. Management safety priority did not moderate this relationship. Significant links existed between attitudes and reporting intention and behavior.Conclusions/Applications to Practice:This study highlights the vital influence of workers' attitudes on reporting work-related injuries and illnesses. Occupational health providers should employ strategies, such as tailored safety training and management commitment, with a focus on addressing the unique needs of long-tenured and musculoskeletal-exposed workers. Fostering a safety culture that promotes open and timely reporting is crucial, and implementing these strategies can significantly enhance workplace safety and health.

Abstract:Objectives We aimed to find evidence of the inflammation‐mediated mechanism by which long working hours contribute to cardiovascular disease (CVD). Methods This cohort study was performed in 56 953 Korean adults free of CVD who underwent a comprehensive screening examination and were followed for up to 7 years. An increase in blood high‐sensitivity C‐reactive protein (hsCRP) of 1 mg/L or more at the follow‐up visit was defined as an incidence. The average weekly working hours in the past year were categorized as ≤40, 41–52, 53–60, and ≥60 h per week. Odds ratios (ORs) and 95% confidence intervals (CIs) were estimated using generalized estimating equations to calculate the risk of an incidental increase of hsCRP. Results Participants with longer working hours had a higher incidence of hsCRP. Multivariable‐adjusted ORs (95% CIs) of incident cases for ≥61 h compared with ≤40 h was 1.69 (1.04–2.75). In subgroup analyses according to sex and the presence of hypertension and diabetes, the risk of hsCRP incidence were highest in the group working more than 61 h in all subgroups, but none of them were statistically significant. Conclusions Working hours are independently associated with increased risk of elevated hsCRP in a dose–response relationship. Excessive long‐time work is a risk factor for CVD, and it was found that an increase in hsCRP was associated with the pathogenesis.
