Abstract:Background: Post-traumatic stress disorder (PTSD) is a common psychological problem following natural disasters. Although pre-disaster risk factors are important for early detection and proactive support, the examination of such has been limited to sociodemographic factors, which were largely unaffected by the disasters. We examined the association between pre-disaster physical functioning and lifestyle and PTSD symptoms five months after the earthquake in the Great East Japan Earthquake survivors who were participating in a pre-existing cohort study.Methods: We designed a retrospective cohort study of a cooperative association in Sendai from August 2010 to August 2011. In 2010, lifestyle, physical condition, and sociodemographic factors were examined by self-reported questionnaires completed by 522 employees of this organization. We also measured the leg extension power of all the participants. PTSD symptoms were evaluated by the Japanese version of the Impact of Event Scale-Revised (IES-R-J) following the earthquake of 2011.Results: In multivariate linear regression analysis, leg extension power (beta = -0.128, P = 0.025), daily drinking (beta = 0.203, P = 0.006), and depressive symptoms (beta = 0.139, P = 0.008) were associated with total score of the IES-R-J among men. Moreover, for the IES-R-J subscale, leg extension power was also negatively associated with Intrusion (beta = -0.114, P = 0.045) and Hyperarousal (beta = -0.163, P = 0.004) after adjusting for all other significant variables. For women, hypertension (b = 0.226, P = 0.032) and depressive symptoms (beta = 0.205, P = 0.046) were associated with the total score of the IES-R-J.Conclusions: Leg extension power is a potentially modifiable pre-disaster risk factor among men for attenuating the severity of PTSD symptoms associated with great disasters such as the Great East Japan Earthquake among men.
Factor Structure of the Children's Revised Impact of Event Scale among Children and Adolescents Who Survived the 2008 Sichuan Earthquake in China.

Abstract:To evaluate the psychometric properties of the Chinese version of Children's Revised Impact of Event Scale (CRIES) and its applicability among Chinese children and adolescents, a study was conducted on two samples, the first, 1 month after the 2008 Sichuan earthquake, the second, 7 months after the earthquake. High levels of posttraumatic stress symptoms were found among both groups of children. The results also showed a decline of intrusion and arousal symptoms in accordance with the different periods of time elapsed since the earthquake; however, no difference was found in the avoidance symptoms between the two samples. Both the subscales and the CRIES total showed moderate to good reliability. Confirmatory factor analysis (CFA) of CRIES supported the presumed three inter-correlated factors model. However, the results of the second sample (with more than 6 months elapsed subsequent to the earthquake) are more likely to support this model than those of the first sample. This study generally justifies the use of CRIES as a screening instrument for probable PTSD victims among children and adolescents exposed to horrible natural disasters in China. Limitations of the present study and directions for future research are also discussed.

Abstract:Since the introduction of the Posttraumatic Growth Inventory (PTGI), the factor structure of PTG has been a hot topic of debate. Nevertheless, no consensus has been achieved to date. Previous researchers have focused mainly on adults, such that the applicability of this measure to traumatized children remains rather unclear. Although several studies have suggested that time since trauma might influence PTG factor structure, no study has examined the stability of the factor structure of PTG over time. Thus, the present study aimed to assess the factor structure of PTG among traumatized children following the Ya'an earthquake, and to specifically examine PTG optimal factor structure over time. Self-report scales were administered to 303 children, 6 months after the earthquake. A five-factor structure of PTG that includes relating to others, new possibilities, personal strength, spiritual changes, and appreciation of life had the best fit indices among all five structural models of PTG examined. The five factor structure of PTG satisfied the factor loading invariance criterion but did not satisfy the item intercept and factor variance invariance criteria. This study extends the extant literature on the five factor structure of PTG from adults to a child sample and confirms that the five factor structure is stable across time, but that PTG scores are indeed moderated by time since trauma. Implications for clinical practice and future research are discussed along with study limitations.
Unveiling temporal dynamics of PTSD and its functional impairments: A longitudinal study in UK healthcare workers

Abstract:PTSD has been associated with negative long-term consequences, including social and occupational impairments. Yet, a nuanced understanding of the interplay between PTSD symptoms and distinct domains of impairments on a short-term basis (weeks/ months) at the within-person level remains underexplored. In a large sample (nwave 1 = 1096, nwave 7 = 304) of UK healthcare workers assessed across seven assessment waves during the COVID-19 pandemic (spaced 6 weeks apart), we employed exploratory graphical vector autoregression models (GVAR) models to discern within-person temporal (across time) and contemporaneous (within same time window) dynamics between PTSD symptoms and functional impairment domains. The contemporaneous network highlighted strong co-occurrences between different symptoms and impairments. The temporal network revealed a mutually reinforcing cycle between intrusion and avoidance symptoms. Intrusion symptoms showed the highest out-strength (i.e., most predictive symptom), predicting avoidance symptoms, elevated sense of current threat, and various functional impairments. Avoidance symptoms, elevated after increased levels of intrusions, predicted work impairments that in turn were associated with difficulties in fulfilling other obligations. Our findings underscore the dynamics between perceived threat and intrusions, and the role intrusions may play in predicting a cascade of adverse effects. Targeted interventions aimed at mitigating intrusions may disrupt this negative cycle.

Abstract:Background: High comorbidity between posttraumatic stress disorder (PTSD) and depression among adolescents often follows severe traumatic events. Models on the pathway to comorbidity dispute greatly and how PTSD and depression get comorbidity, remain unclear.Methods: A follow-up investigation was conducted of 424 adolescent survivors of the Jiuzhaigou earthquake at 12 months (T1) and 27 months (T2). Results: Contemporaneous network analysis and cross-lagged panel network analysis showed that PTSD and depression are two separate disorders with strong associations via links between dysphoric symptoms of PTSD and somatic or non-somatic symptoms of depression. However, the association weakened from T1 to T2, and internal connections between symptoms within each disorder became stronger.Limitation: We only measured the comorbidity of PTSD and depression at two time points following the earth-quake, which may limit the long-term applicability of our findings following trauma. Conclusions: The findings also showed that the centrality in contemporaneous networks may indicate node connectivity rather than the influence or potential causality among nodes. These results help to elucidate the relationship between PTSD and depression and could contribute to the development of appropriate therapies.
Network Analysis of PTSD in College Students Across Different Areas after the COVID-19 Epidemic

Abstract:Background: Various studies have examined the psychological 'typhoon eye' and 'ripple' effects in mental disorders following COVID-19. However, these studies only considered the disorders as entities and assessed severity, and overlooked the differences in specific symptoms of disorders. Objectives: This aim of the study is to assess the psychological typhoon eye and ripple effects at the symptom-level in posttraumatic stress disorder (PTSD), which is considered as a common psychopathology following the COVID-19 epidemic. Method: In total, 1150 undergraduates, including 271 students from the Hubei province (e.g. epidemic centre) and 879 students from other provinces, completed the self-report questionnaire. The networks were estimated and compared using the R packages. Results: Although the PTSD networks of Hubei and non-Hubei undergraduates were similarly connected and shared some symptoms with high centrality (e.g. flashbacks, irritability and anger), there were differences across the networks. Distorted cognition and no positive emotion only exhibited high centrality in the Hubei network. Physiological responses and exaggerated startle only exhibited high centrality in the non-Hubei network. Conclusion: These findings suggested that the psychological typhoon eye and ripple effects may co-exist at the symptom level. Targeted and distinct psychological services for college students in Hubei and non-Hubei provinces should be emphasized following COVID-19.
Longitudinal Linkages Between Posttraumatic Stress Disorder Symptoms and Violent Behaviors among Chinese Adolescents Following the Wenchuan Earthquake

Abstract:This study was conducted to examine the longitudinal relations between posttraumatic stress disorder (PTSD) symptoms and violent behaviors. In this study, 415 adolescent survivors of the Wenchuan earthquake in China (May 12, 2008) were assessed by using self-report questionnaires 1 year (T1), 1.5 years (T2), and 2 years (T3) after the earthquake. The findings suggested that from 1 to 1.5 years after the earthquake, only intrusive symptoms of PTSD were a risk factor for violent behaviors, whereas violent behaviors were a risk factor for all 3 PTSD symptom clusters. Furthermore, 1.5 to 2 years after the earthquake, avoidance symptoms of PTSD were a risk factor for violent behaviors and hyperarousal symptoms of PTSD had no significant relation with violent behaviors.
Big data networks: Dynamic Time Warping as a statistical tool for network analysis using Ecological Momentary Assessment data

Abstract:Introduction In recent research, psychological disorders have been increasingly defined as complex dynamic systems in which symptoms are interconnected and influence each other, thereby forming symptom networks. This paradigm shift calls for the analysis and interpretation of relationships between symptoms that are complex, potentially non-linear, and dynamic. Dynamic Time Warping (DTW) is used to measure similarity in temporal sequences, and has recently been found effective in modelling psychopathology symptom networks. Objectives We aim to demonstrate that DTW could also be used to model the network structure in Ecological Momentary Assessment (EMA) data. Methods 355 participants of the Netherlands Study of Depression and Anxiety (NESDA), of which 100 with and 255 without current disorder, completed EMA assessments of 20 symptoms (e.g., feeling sad, tired, satisfied) five times a day for two weeks. DTW analysis was performed on the group level, comparing participants suffering from mood disorders to healthy controls. DTW distances were visualized as an undirected symptom network, in which we adjusted for the average symptom severity per item per person. Results DTW analysis of close to half a million symptom scores yielded six symptom dimensions based on their aggregated similarity of changes over time within the participants. Surprisingly, negative affect symptom networks were found to be less strongly connected in those currently suffering from mood disorders than in controls, whereas the network density of (reverse-coded) positive affect symptoms was more closely connected in this group. This is contrary to the results of previous studies, where negative affect-related symptom networks of those with mood disorders were found to be more strongly interconnected. Conclusions DTW is a promising new technique for analyzing EMA data and modeling dynamic symptom networks at both the individual and group levels. Using EMA data, symptom networks and dimensions can be modeled with great structural and temporal detail. The Temporal Propagation of Intrinsic Brain Activity Associate With the Occurrence of PTSD.

Abstract:The abnormal brain activity is a pivotal condition for the occurrence of posttraumatic stress disorder. However, the dynamic time features of intrinsic brain activities still remain unclearly in PTSD patients. Our study aims to perform the resting-state lag analysis (RS-LA) method to explore potential propagated patterns of intrinsic brain activities in PTSD patients. We recruited 27 drug-naive patients with PTSD, 33 trauma-exposed controls (TEC), and 30 demographically matched healthy controls (HC) in the final data statistics. Both RS-LA and conventional voxel-wise functional connectivity strength (FCS) methods were employed on the same dataset. Then, Spearman correlation analysis was conducted on time latency values of those abnormal brain regions with the clinical assessments. Compared with HC group, the time latency patterns of PTSD patients significantly shifted toward later in posterior cingulate cortex/precuneus, middle prefrontal cortex, right angular, and left pre- and post-central cortex. The TEC group tended to have similar time latency in right angular. Additionally, significant time latency in right STG was found in PTSD group relative to TEC group. Spearman correlation analysis revealed that the time latency value of mPFC negatively correlated to the PTSD checklist-civilian version scores (PCL_C) in PTSD group (r = 0.578, P < 0.05). Furthermore, group differences map of FCS exhibited parts of overlapping areas with that of RS-LA, however, less specificity in detecting PTSD patients. In conclusion, apparent alterations of time latency were observed in DMN and primary sensorimotor areas of PTSD patients. These findings provide us with new evidence to explain the neural pathophysiology contributing to PTSD.
Assessing the Latent Structure of DSM-5 PTSD among Chinese Adolescents after the Ya'an Earthquake.

Abstract:To examine the underlying substructure of DSM-5 PTSD in an adolescent sample, this study used a confirmatory factor analysis alternative model approach to assess 813 adolescents two and a half years after the Ya'an earthquake. Participants completed the PTSD Checklist for DSM-5, the Center for Epidemiologic Studies Depression Scale for Children, and the Screen for Child Anxiety Related Emotional Disorders. The results found that the seven-factor hybrid PTSD model entailing intrusion, avoidance, negative affect, anhedonia, externalizing behaviors, anxious arousal, and dysphoric arousal had significantly better fit indices than other alternative models. Depression and anxiety displayed high correlations with the seven-factor model. The findings suggested that the seven-factor model was more applicable to adolescents following the earthquake, and may carry important implications for further clinical practice and research on posttraumatic stress symptomatology.

Abstract:The aim of this study is to examine the longitudinal relationships between posttraumatic stress disorder (PTSD) and posttraumatic growth (PTG) among adolescent survivors of the 2008 Wenchuan earthquake in China. The participants in our study included 245 adolescent survivors who were randomly selected from several primary and secondary schools in the counties of Wenchuan, which are the areas most severely affected by the Wenchuan earthquake. Participants completed the Revised Child PTSD Symptom Scale and the Posttraumatic Growth Inventory (PTGI) at 3.5 years after the earthquake (T1), 4.5 years after the earthquake (T2), and 5.5 years after the earthquake (T3). The results found that PTSD reported in T1 and T2 predicted subsequent PTG reported at T2 and T3 and that PTG did not predict PTSD from T1 to T3. In addition, the cross-sectional correlation between PTSD and PTG weakened from T1 to T3. These results indicate that PTSD and PTG can coexist in individuals after a traumatic experience, and they further suggest that the reduction in PTSD does not indicate the appearance of PTG.

Abstract:Background: This study examined the trajectories of posttraumatic stress disorder (PTSD) among adolescents in the area worst-hit by the Wenchuan earthquake, and assessed the role of trauma exposure in differentiating distinct PTSD trajectories. Methods: In total, 391 adolescents were selected from the Wenchuan and Maoxian counties in Sichuan province (the area worst-hit by the earthquake). Participants completed a trauma exposure questionnaire at 1 year, and the Child PTSD Symptom Scale at 1 year, 1.5 years, 2 years, and 2.5 years after the earthquake. Results: Three latent PTSD trajectories were found in adolescents: moderate-stable (81.6%), decreasing (8.7%), and increasing trajectories (9.7%). T1 trauma exposure was more likely to be related to the decreasing trajectory group. Limitations: All variables were measured by self-report scales. There was high non-random dropout. Conclusion: The course of PTSD was heterogeneous among adolescents following the same natural disaster.

Abstract:Background: Analysis of dynamic (temporal) networks allows an identification of important targets of treatment.Objective: This study examined the dynamic network of symptoms in patients diagnosed with post-traumatic stress disorder (PTSD) during exposure therapy.Method: Patients (n = 65) were randomized to either standard prolonged exposure, which includes imaginal exposure to the traumatic memory, or modified prolonged exposure, where imagery re-scripting of the memory replaced imaginal exposure, in a 10-week residential program. They completed a measure of DSM-IV PTSD symptoms weekly. The multilevel vector autoregressive (mlVAR) model was used to analyse the data, producing a temporal (dynamic), contemporaneous, and between-person network.Results: Physiological reactivity to reminders in a given week was positively related to distress reactivity and to flashbacks the subsequent week. Hypervigilance one week was positively related to startle response and external avoidance the subsequent week. In addition, sleep problems were positively predicted by previous week internal avoidance and negatively predicted by previous week flashbacks. Hypervigilance and physiological reactivation had the highest out-strength, indicating that they were the most predictive of other symptoms.Conclusions: The present within-person results make a preliminaryrect basis for the recommendation to monitor and facilitate change in physiological reactivation and hypervigilance in the treatment of PTSD. Future studies should examine the replicability of our temporal PTSD network and also include causal variables beyond symptoms.

Intrinsic Network Brain Dysfunction Correlates With Temporal Complexity in Post-traumatic Stress Disorder

Abstract:BackgroundResearch has been looking into neural pathophysiology of post-traumatic stress disorder (PTSD) and dynamic functioning connectivity (dFC) applying resting state functional magnetic resonance imaging (rs-fMRI). Previous studies showed that PTSD related impairments are associated with alterations distributed across different brain regions and disorganized functional connectivity, especially in Default Mode Network and the cerebellar area. In this study, we specifically looked into dFC on a whole brain level, and we focused on critical regions such as DMN and cerebellum. MethodsTo explore the characteristics of dFC among patients with PTSD, we collected rs-fMRI data from 27 PTSD patients and 30 healthy controls. The study also added a control group of 33 trauma-exposed individuals to further look into trauma impact. Utilizing group spatial independent component analysis (ICA), the dynamic properties on whole brain level were detected with sliding time window approach, and k-means clustering. ResultsTwo reoccurring FC “States” were identified, with connections being more concentrated on a within-network level in one state and more strongly inter-connected in the other state. Abnormalities in dFC were found within DMN, between DMN and cerebellum, and between DMN and visual network. ConclusionsThe findings were in accordance with the study hypothesis that PTSD patients demonstrated deficits in emotional modulation and dysfunctional self-referential thoughts, and the deficits could be indicated in dFC alterations. Abnormalities in dFC among PTSD patients could serve as appropriate indicators of PTSD symptoms including depression and anxiety, hypervigilance, as well as impaired cognitive functioning and self-referential information processing.
Posttraumatic Stress Disorder and Growth: Examination of Joint Trajectories in Children and Adolescents

Abstract:Positive health endpoints are not the opposite of negative endpoints. Previous studies examining posttraumatic stress disorders (PTSD) and posttraumatic growth (PTG) trajectories have overlooked the co-existence of PTSD and PTG, making it difficult to accurately distinguish individuals with various posttraumatic presentations, causing the effects of targeted interventions to be discounted. To fill this gap, the current study sought to examine joint PTSD and PTG trajectories in children and adolescents. Eight hundred and seventy-six Chinese children and adolescents were recruited to complete self-report questionnaires 6, 12, and 18 months after the Ya'an earthquake. Multiple-process growth mixture modeling analysis was used to test the study proposal. Five distinct joint PTSD and PTG trajectory types were found: recovery, growth, struggling, resistant, and delayed symptoms. Female students and students who felt trapped or fearful were more likely to be in the struggling group, and students who experienced injury to themselves or family members were more likely to belong to the delayed symptom group. These findings suggest that postdisaster psychological services should be provided to relieve delayed symptoms in individuals who experience injury to themselves or their family members, and individuals in the struggling group should be supported to achieve growth.
Temporal Transitions in Patterns of Posttraumatic Stress Disorder and Depression among Adolescents Following the Wenchuan Earthquake

Abstract:Posttramatic stress disorder (PTSD) and depression are persistent disorders with heterogeneous comorbidity. Cross-sectional design limitations have prevented previous studies from examining symptom pattern transitions, which limits the understanding on the change of mental health over time since trauma. This study examined transition patterns of PTSD and depression comorbidity and assessed the role of personality. PTSD, depression, and personality scales were used to assess 619 adolescents 1 year after the Wenchuan earthquake and then to longitudinally assess 332 adolescents 2 years post-earthquake. Data were analyzed using latent transition analysis and logistic regression. Four PTSD and depression comorbidity patterns were identified at both times: moderate comorbidity, high comorbidity, no symptoms, and depression. Patterns of PTSD and depression changed in 23.4% of adolescents: 4.4% and 7.1% transitioned from no symptoms to depression and from depression to moderate comorbidity, respectively; 7.5% transitioned from moderate comorbidity to depression. Extraversion and conscientiousness were more likely and openness was less likely to be associated with moderate comorbidity symptoms transitioned to depression symptoms. These findings indicated that patterns of PTSD and depression in adolescents are heterogeneous and show temporal change.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Revealing the dynamic interplay between posttraumatic stress disorder (PTSD) symptom clusters has always been an important topic in traumatic stress studies. Based on longitudinal studies, different hypotheses have been proposed to explain PTSD symptom dynamics. But currently, no study have been conducted to test these hypotheses in children and adolescents.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Data were derived from a longitudinal study of child and adolescent traumatic event survivors of an explosion accident (N = 659). <em>DSM-5</em> PTSD symptoms was measured by the PTSD Checklist-5 (PCL-5) at 4, 8, and 13 months after the disaster. Latent difference score (LDS) modeling was used to evaluate the dynamic interplay between clusters.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The results of LDS model indicated that intrusion level positively predicted subsequent rate of increase for hyperarousal (<em>p</em> = .008) and negative changes in cognitions and mood symptoms (<em>p</em> = .036). Also, intrusion level trended to positively predict subsequent increase rate of avoidance symptoms (<em>p</em> = .059).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>This study expands previous knowledge of dynamic relations between symptom clusters during the maintenance and fluctuation of child and adolescent PTSD symptoms. By using new methodology, this study provided novel evidence for the hypothesis that intrusion symptom plays an important role in maintaining other PTSD symptoms.</p>

Abstract:The neural substrates of posttraumatic stress disorder (PTSD) are still not fully elucidated. Hence, this study is to explore topological alterations of the default mode network (DMN) in victims with PTSD after a magnitude of 8.0 earthquake using resting-state functional magnetic resonance imaging (rs-fMRI).This study was approved by the local ethical review board, and all participants signed written informed consent. Sixty-two PTSD victims from the 2008 Sichuan earthquake and 62 matched exposed controls underwent rs-fMRI. PTSD was diagnosed by Clinician-Administered PTSD Scale, and underwent PTSD Checklist-Civilian Version for symptom scoring. The DMN was analyzed by using graph theoretical approaches. Further, Pearson correlation analysis was performed to correlate neuroimaging metrics to neuropsychological scores in victims with PTSD.Victims with PTSD showed decreased DMN functional connectivity strength between the right superior frontal gyrus and left inferior parietal lobule (IPL), and showed increased functional connectivity between the right IPL and precuneus or left posterior cingulate cortex. It was also found that victims with PTSD exhibited decreased nodal efficiency in right superior frontal gyrus and precuneus, and increased nodal efficiency in right hippocampus/parahippocampus. Apart from that, PTSD showed higher nodal degree in bilateral hippocampus/parahippocampus. In addition, the functional connectivity strength between the right IPL and precuneus correlated negatively to the avoid scores (r = -0.26, P = .04).This study implicates alteration of topological features on the DMN in PTSD victims after major earthquake, and provides new insights into DMN malfunction in PTSD based on graph theory.
Dynamic symptom associations in posttraumatic stress disorder: a network approach

Abstract:Background and objective: The current study aimed to investigate the within-day symptom dynamics in PTSD patients, specifically focusing on symptoms that most predict changes in other symptoms. The study included a baseline diagnostic assessment, followed by an assessment using the experience sampling method (ESM) via a smartphone. Method: Participants answered questions related to their PTSD symptoms four times per day for 15 consecutive days (compliance rate 75%). The clinical sample consisted of 48 treatment-seeking individuals: 44 with PTSD as a primary diagnosis, and four patients with subsyndromal PTSD, all of whom had not yet begun trauma-focused treatment. The ESM assessment included the 20 items from the PTSD Checklist for DSM-5, five items from the International Trauma Questionnaire (ITQ) assessing disturbances in relationships and functional impairment, and two items from the Clinician-Administered PTSD Scale for DSM-5 assessing symptoms of depersonalization and derealization. Results: Temporal networks (prospective associations between symptoms) showed that changes in hypervigilance predicted changes in the greatest number of symptoms at the next time point. Furthermore, hypervigilance showed temporal connections with at least one additional symptom from each of the DSM-5 PTSD symptom clusters. Conclusions: Results show that the contemporaneous network (representing the relationship between given symptoms within the same assessment occasion) and the temporal network (representing prospective associations between symptoms) differ and that it is important to estimate both. Some findings from earlier research are replicated, but heterogeneity across studies remains. Future studies should include potential moderators. Antecedentes y objetivo: El presente estudio buscó investigar las dinámicas de los síntomas dentro de un día en pacientes con TEPT, centrándose específicamente en los síntomas que más predicen los cambios en otros síntomas. El estudio incluyo una evaluación diagnostica inicial, seguida de una evaluación mediante el método de muestreo de experiencias (ESM en su sigla en inglés) a través de un teléfono inteligente. Método: Los participantes contestaron preguntas relacionadas con sus síntomas del TEPT cuatro veces al día por 15 días consecutivos (tasa de cumplimiento 75%). La muestra clínica consistió en 48 individuos que buscaban tratamiento: 44 con TEPT como diagnóstico principal, y cuatro pacientes con TEPT subclínico, todos los cuales aún no habían empezado un tratamiento centrado en el trauma. La evaluación del ESM incluyó los 20 ítems de la Lista de Chequeo de TEPT para el DSM-5, cinco ítems del Cuestionario Internacional de Trauma (ITQ en sus siglas en ingles) que evalúa las alteraciones en las relaciones y el deterioro funcional, y dos ítems de la Escala de TEPT administrada por el clínico según el DSM-5 que evalúa los síntomas de despersonalización y desrealización. Resultados: Las redes temporales (asociaciones prospectivas entre síntomas) mostraron que los cambios en hipervigilancia predijeron los cambios en la mayor cantidad de síntomas en el siguiente momento. Además, la hipervigilancia mostró conexiones temporales con al menos un síntoma adicional de cada grupo de síntomas de TEPT del DSM-5. Conclusiones: Los resultados muestran que las redes contemporáneas (que representa la relación entre los síntomas dados dentro de la misma ocasión de evaluación) y la red temporal (que representa las asociaciones prospectivas entre síntomas) difirieren y que es importante estimar ambas. Se replican algunos hallazgos de investigaciones previas, pero persiste la heterogeneidad entre los estudios. Futuras investigaciones deberían incluir potenciales moderadores.

Abstract:Objective: Children and adolescents are found to be more vulnerable to developing PTSD than adults over time after major disasters. This study aims to investigate the network structures of PTSD and the directions of relationships between symptoms among adolescent survivors in the year after the Yancheng Tornado in China. Method: A total of 395 youth survivors completed the Child PTSD Symptom Scale (Foa et al., 2001) at 3 months and 12 months following the tornado. Network analysis was used to compare networks of PTSD symptoms and changes over time. Results: Different centrality symptoms existed at different time points. Anger, startle responses, and physiological reactivity were important to the maintenance of PTSD symptoms arising from the tornado at 3 months, while dreams/nightmares and distancing/avoidance were important to maintaining PTSD symptoms at 12 months. Analysis suggested that sleep difficulty and intrusive thoughts were the key PTSD symptoms to be treated at 3 months; sleep remained to be the key symptoms to be treated at 12 months. Conclusions: Our findings suggest that sleep difficulty could be a main cause of other symptoms and trigger the entire symptom system into undesirable psychopathological development among adolescent survivors in the year following major disasters.
