Abstract:Objective: To analyze the trends of overweight and obesity prevalence in Chinese children, aged from 6 to 15 years old among 4 provinces and cities from 2009 to 2019. Methods: Reviewed the national multi-center epidemiological survey data of children from the National Key Technology R&D Program of China during the Eleventh Five-Year Plan (2009 to 2010) and the National Key Research and Development Program of China during the Thirteenth Five-Year Plan (2017 to 2019). The participants' data were selected from four provinces,municipalities and autonomous region,including Beijing, Tianjin (Northern region), Zhejiang (Eastern region), and Guangxi (Southern region). Totally 14 597 pairs of 6-15 year-old children were surveyed. According to the body mass index (BMI) and standard deviation score (SDS) of children among different genders, ages, and regions, t test or chi-square test was used to evaluate the changes in overweight and obesity over a 10-year span. Results: Totally 7 721 pairs of boys and 6 876 pairs of girls were collectted in this study, whose mean age was (10.7±2.5) years. In the past 10 years, the overall BMISDS were 0.39±1.24 and 0.36±1.31 and the overall obesity rate were 11.8% (n=1 773) anel 12.5% (n=1 813) of children in the 4 administrative regions did not have statistically significant differences (all P>0.05). However, the overall overweight rate rose from 17.1% (n=2 496) to 19.1% (n=2 781) (χ²=18.657, P<0.01), and the average annual growth rate was 0.20%. The BMISDS in the Eastern region increased from 0.10±1.07 to 0.19±1.22 (t=-4.095, P<0.01), and the overweight rate and obesity rate increased by 3.8% (n=202) and 3.1% (n=169) respectively (both P<0.01); the BMISDS in the Northern region and the obesity rate did not have statistically significant differences(all P>0.05), but the overweight rate rose from 20.5% (n=1 233) to 22.8% (n=1 365) significantly (χ²=7.431, P<0.01); BMISDS in the Southern region was significantly decreased from 0.30±1.19 to 0.09±1.25 (t=1.426, P<0.01), and the rate of obesity decreased from 9.8% (n=315) to7.9% (n=256) (χ²=6.46, P<0.05), the overweight rate was not stafistically significant (P=0.10), respectively. The obesity rate of boys had risen from 16.4% (n=1 265) to 18.2% (1 407) (χ²=8.997, P<0.01) in the past 10 years, and the overweight rate had risen from 18.0% (n=1 393) to 20.5% (n=1 579) (χ²=14.26, P<0.01). The overweight+obesity rate rose from 34.4% (n=2 658) to 38.7% (n=2 986) (χ²=29.859, P<0.01), and the weight problem in the age group of 8 to 11 years was particularly severe (all P<0.01). The obesity rate of girls dropped from 6.8% (n=468) to 5.9% (n=406) (χ²=4.546, P<0.05), the overweight rate rose from 16.0% (n=1 103) to 17.5% (n=1 202) (χ²=5.006, P<0.05), and the overall overweight+obesity rate rose from 22.8% (n=1 571) to 23.4% (n=1 608) (χ²=0.53, P>0.05). Conclusions: The growth rate of obesity among children in China had slowed down from 2009 to 2019, but the overweight rate was still on the rise. The overall base of overweight and obesity population continued to expand. The weight problem of peri-adolescent boys was particularly prominent. The current status of obesity epidemics in different regions, ages, and genders are significantly different and had their own characteristics. It is necessary to establish a personalized prevention and control strategy.

Abstract:Background: Geographical profiles of overweight and obesity in China are limited. We aimed to compare region-specific prevalences of overweight and obesity for Chinese children with a full range of ages and to describe the distribution of potential risk factors.Methods: We analyzed data from Prevalence and Risk factors for Obesity and Diabetes in Youth (PRODY) study, which was a cross-sectional survey of children aged 3-18 years from 11 provinces, autonomous regions, and municipalities that are geographically representative of western, southern, eastern, northern and central China. All participants underwent physical examinations, and their parents or caregivers completed online, self-administered questionnaires. We used multilevel logistic regression models to estimate prevalences of overweight and obesity and to assess related risk factors across sexes, age groups and regions. Findings: Between January 2017 and December 2019, 200,937 children in the PRODY study were included in our analysis. We found that children aged 8-13 years in northern China and children aged 3-5 years in western China were heavily burdened with overweight and obesity. In northern China the obesity prevalences peaked at 31·0% for boys and at 18·8% for girls, while in western China the prevalences of overweight and obesity peaked at 39·0% and 26·0%, respectively. Boys were vulnerable to overweight and obesity throughout all ages and regions (except western China). In addition, risk factors for overweight and obesity varied across ages, sexes and regions. Early-life factors (breast-feeding duration) and lifestyle factors (daily screen time, sleep time and exercise frequency) were the most important, modifiable factors for obesity control. Parental factors (parent’s overweight, socioeconomic characteristics and education status) had noteworthy impacts on their offspring’s overweight and obesity. Interpretation: The age- and sex-specific prevalences of overweight and obesity in Chinese children vary geographically; therefore, modifications of the major risk factors for the prevention and treatment of overweight and obesity should take age, sex and regional characteristics into consideration. Special attention should be paid to the most vulnerable children, and targeted interventions are urgently needed to mitigate the obesity burden in such a vast country. Abstract:BACKGROUND:Prevalence of overweight and obesity in adolescents has been increasing during recent decades. The present study aimed to assess the prevalence of obesity in 14-18 years old Anhui Secondary school children, and provide more comprehensive guidelines for intervention programs.METHODS:This cross-sectional study was conducted among 4788 children in secondary school, and date from the routine health survey was analyzed. Trained health workers took anthropometric measures, height and weight, at the school. Definition of children overweight and obesity children was based on the international age-and sex-specific body mass index reference standards proposed by the International Obesity Task Force and National Center for Health Statistics.RESULTS:According to the IOTF standard the prevalence of overweight (including obesity) and obesity revealed decreasing trend with age in male (overweight, from 17.3% at 14-y to 5.3% at 18-y; obesity, from 3.8% at 14-y to 2.3% at 18-y). Depending on the NCHS references used, the overall prevalence of overweight (including obesity) and obesity in male aged 14 to 18 decreases from 16.4% to 11.7% and from 4.6% to 2.8%, respectively.CONCLUSION:The overweight (including obesity) and obesity from 14 to 18 years old have a decreasing trend, and more attention should be taken to male students in China. These findings suggest secondary school and government need to design appropriate guidelines to keep healthy for adolescents in China.

Abstract:BACKGROUND:Overweight and obesity are epidemic worldwide. The purpose of this research is to estimate whether the prevalence of obesity among primary school children is high, and to identify its potential determinants to optimize the methods of prevention to combat further increases in childhood overweight.METHODS:A cross-sectional study was designed to collect the routine health screening data for primary school children in Wannan area, China. Overweight and obesity status were determined using the International Obesity Task Force standard (IOTF) BMI cut-off points.RESULTS:A total of 67956 subjects (36664 male and 31292 female) aged 5-14 years were recruited in this study. Depending on the references used (IOTF), the overall prevalence of overweight, including obesity of the subjects was 17.85% , the prevalence of overweight, including obesity was 22.9% in male subjects and 11.9% in female subjects, respectively. The overall prevalence of obesity was3.7%, the prevalence of obesity was 5.2 % in male subjects and 1.8% in female subjects, respectively. An interesting observation made was that the prevalence of overweight was high in male subjects.CONCLUSIONS:Overweight is prevalence among primary school children, especially in male children. The relate department of school and government should take some measure to reduce the prevalence of overweight and obesity.

Abstract:Objective This study aimed to analyze a comprehensive set of potential risk factors for obesity and overweight among Chinese children with a full range of ages and with wide geographical coverage. Methods In the Prevalence and Risk Factors for Obesity and Diabetes in Youth (PRODY) study (2017-2019), the authors analyzed 193,997 children aged 3 to 18 years from 11 provinces, autonomous regions, and municipalities that are geographically representative of China. All participants underwent physical examinations, and their caregivers completed questionnaires including dietary, lifestyle, familial, and perinatal information of participants. A multilevel multinomial logistic regression model was used to evaluate the potential risk factors. Results Among the actionable risk factors that were measured, higher consumption frequencies of animal offal (odds ratios [OR] for an additional time/day = 0.91, 95% CI: 0.88-0.95, same unit for OR below unless specified otherwise), dairy products (0.91, 95% CI: 0.88-0.94), freshwater products (0.94, 95% CI: 0.91-0.96), staple foods (0.94, 95% CI: 0.92-0.96), and coarse food grain (OR for every day vs. rarely = 0.92, 95% CI: 0.86-0.98) were associated with lower relative risk of obesity. However, higher restaurant-eating frequency (OR for >4 times/month vs. rarely = 1.21, 95% CI: 1.15-1.29) and longer screen-viewing duration (OR for >2 hours vs. <30 minutes = 1.16, 95% CI: 1.10-1.22) were associated with higher relative risk of obesity. Increased exercise frequency was associated with the lowest relative risk of obesity (OR for every day vs. rarely = 0.72, 95% CI: 0.68-0.77). Conclusions Changes in lifestyle and diet of Chinese children may help relieve their obesity burden.

Abstract:Objective To explore the nutritional status of young children in a city in Zhejiang from 2002 to 2015 and analyze potential influencing factors. Methods We used electronic medical recorder system and extracted maternal and offspring's medical information from the decoded database. Z-scores were used to elevate nutrition status. Logistic regression models were used to explore the influencing factors of the nutritional status. Results Among 48 827 children,the prevalence rate of underweight, stunting, thinness, overweight and obesity were 1.43％,1.31％, 1.90％,2.62％,and 0.34％,respectively. The prevalence of obesity had significant differences between genders(χ2 =5.164 =0.023). The prevalence of underweight was increased, and the prevalence of overweight and obesity were decreased. Furthermore, mother with higher educational level had lower risk of child's malnutrition, and mother with higher pre-pregnancy body mass index (BMI) had higher risk of child's overweight( OR =1.063, 95％ 1.057-1.069,P＜ 0.001) and obesity(OR=1.074, 95％ CI: 1.060-1.088, P＜ 0.001); Maternal high-risk pregnancy was the risk factor of underweight (OR =2.214, 95％ C/:l.928-2.542 <0.001), stunting ( OR=2.543,95％ CI:2.193-2.949,P＜0.001) and thinness(OR=:1.211, 95％ CI:l.062-1.380,P=0.004) in children. Conclusions Overall, children's physical development in this city was in good condition. However, problems are still existing with malnutrition and overnutrition. Maternal high pre-pregnancy BMI, low educational level and high-risk pregnancy were independently related to child's malnutrition and overnutrition.

Abstract:Background: Despite perceiving their child as being above a healthy weight, many parents do not intervene. Little is known about the factors influencing parental action. We assessed parental perception of child's weight status, the prevalence of mitigating parental action, and the underlying factors. Methods: We studied 20,242 children and adolescents from 6 centers across China. Anthropometry was measured by research nurses. Parents answered questionnaires, including their perception of their child's weight status, and any subsequent weight treatment. Results: A total of 3254 children had obesity (16.1%), with 63.0% correctly perceived as overweight by their parents. These children were more likely to be older (>= 8 years; p < 0.0001), have severe obesity [adjusted relative risk (aRR) 1.41; p < 0.0001], and have mothers with overweight/obesity (aRR 1.15; p < 0.0001). In particular, parents of children aged <8 years were over five times more likely to perceive their child with overweight/obesity as "thin" than parents of teenagers. Conversely, girls, older children/adolescents, and urban youth were more likely to be wrongly perceived by parents as having an overweight issue. Only one in four children (27.8%) with available information received treatment for their perceived weight problem. Children with severe obesity were more likely to be treated (aRR 1.34; p < 0.0001), as were children of mothers with overweight/obesity (aRR 1.18; p = 0.002). Conclusions: Only one in four Chinese children perceived as overweight by their parents received treatment for their weight problem. Given that overweight/obesity in childhood tracks into adulthood and many parents did not intervene despite perceiving an overweight problem in their child, interventions for childhood obesity need to extend beyond parental perception of children's weight status.
Incidence of Overweight and Obesity of 10～17 Years Old Teenagers and the Influencing Factors in Shanghai.

Abstract:【Objective】 To analyze the incidence of overweight and obesity of 10~17 years old teenagers in Shanghai in 2011 and identify the influencing factors.【Methods】 With the samples of 7 737 youth physical assessment data in Shanghai horizontal research part of the Metabolic Syndrome Project supported by national The Eleventh Five Year Plan,adopt China(WGOC) and the International Obesity Task Force(IOTF) childhood BMI standard used to calculated the incidence of overweight and obesity,compared the difference of this two standard,and analyzed the influencing factors of childhood overweight and obesity.【Results】 Among 7 737 teenagers of 10~17 years old investigated,the incidence of overweight and obesity was 11.9%,6.2%(WGOC);and 13.6%,3.6%(IOTF).Boys' incidence of overweight and obesity was 17.5%,7.7%;being 6.0%,and 4.6% in girls.The results of Chi-square test suggested that gentle,age,birth weight,family revenue,father childbearing age and mother childbearing age,father's degree,father smoking,mother smoking,mother drinking,father overweight,mother overweight,patriarchal history of hypertension,diabetes,hyperlipaemia,coronary disease,fatty liver and matrilineal history of fatty liver were associated with teenagers' overweight and obesity.The results of Logistic regression analysis suggested that gentle(P0.001,OR=3.376),age(P0.001,OR=1.736),family revenue(P=0.001,OR=1.467),mother childbearing age(P=0.023,OR=0.704),father overweight(P0.001,OR=1.545),mother overweight(P0.001,OR=3.14),patriarchal history of hyperlipaemia(P=0.018,OR=1.293)were associated with overweight and obesity of teenagers.【Conclusions】 Nearly 20% of the teenagers in Shanghai has overweight and obesity,being a high level in China.The major risk factors for teenagers' overweight and obesity are gender,age,family history,etc.

Abstract:China is confronted with the challenge of increasing childhood obesity. Although interventions for childhood obesity have been developed, their effectiveness and implementation can vary considerably across nations. In this paper, we review and consolidate the evidence on childhood obesity intervention strategies aimed at multiple levels and components across life, in China. School-based interventions, primarily delivered with group-based dietary and physical education, have shown potential efficacy and can be enhanced by integrating food environment reform and family support. However, most interventional studies were done in eastern and urban areas of China, which highlights the need for attention in regions with insufficient health resources where policies and initiatives are less accessible. In addition, the sparse reporting on implementation design and delivery might hinder the scalability of potentially effective strategies. Amid rapid social and economic development over the past few decades, China faces unique challenges on urbanisation, physical inactivity, and nutritional transitions, necessitating an updated policy agenda for primary prevention of childhood obesity. Structural longitudinal evaluations of public initiatives led by the government and organisations along with inclusive and equitable interventions targeting children of different ages, urban–rural regions, and different ethnic and socioeconomic groups are warranted. Tackling childhood obesity in China needs a cohesive approach that integrates social, economic, cultural, and environmental strategies, combining school-based, family-based, and individual-based approaches, and concerted efforts from multiple sectors and entities within China.
"Eat Less, Exercise More": A Qualitative Study of Perspectives of Overweight Children and Their Parents on Weight Management in China

Abstract:With the rapid economic and social change accelerating the process of nutrition transition in China, the prevalence of overweight and obesity among Chinese children has increased dramatically. This study was to explore the perspectives of overweight children and their parents on weight management. Semi-structured interviews were conducted with 21 parents and their overweight children (16 boys and five girls aged 7-16 with a BMI ranging from 20.95-44.53 kg/m2) admitted to a tertiary pediatric hospital in Hangzhou, China. Interviews were transcribed and thematically analyzed. Themes emerged for parents as dissatisfactions with obesity care services, inadequate knowledge about childhood obesity, false beliefs with Chinese cultural values, barriers (parents and grandparents disagreeing, limited resources outside the hospital, etc.) and concerns about children’s academic performance and mental wellbeing. For children, themes emerged as awareness of being overweight, insufficient health education, facilitators (gift rewards, supportive home environment), barriers (lazy parenting, priority of studies, etc.) as well as social impact. In order to improve the efficiency of childhood obesity treatment, children’s weight management should be integrated into community health services in China. Future interventions on childhood obesity would achieve greater effectiveness by considering all facilitators and barriers especially social cultural factors perceived by both parents and children.
Trends and Prevalence of Overweight and Obesity among Children Aged 2–7 Years from 2011 to 2017 in Xiamen, China

Abstract:Background: Childhood overweight and obesity have become some of the most serious public health problems in the 21st century and have a significant impact on affected children as they grow into adulthood. Objective: The purpose of this study was to evaluate the overweight and obesity status and their main influencing factors in preschool children aged 2–7 years in urban areas of China between 2011 and 2017. Methods: A stratified cluster random sampling method was used to conduct a sample survey of children aged 2–7 years in Xiamen, one of China’s five special economic zones. The 56,738 participants (29,444 boys, 27,294 girls) were examined at three time points (15,757 in 2011, 19,098 in 2014, and 21,883 in 2017), and were analyzed for factors influencing obesity. Results: Between 2011 and 2017, the prevalence of overweight and obesity in preschool children between 2 and 7 years old was 10.91 and 5.66%, respectively. The overweight and obesity rates were higher in boys (11.85 and 7.11%) than in girls (9.90 and 4.09%), and the difference was statistically significant (p < 0.01). In the past 7 years, the overweight and obesity rates showed a downward trend in both boys and girls (p < 0.01). The peak ages for overweight were 6 years in boys and 2 years in girls, while the obesity rate peaked at 6 years. In those children with obesity, the proportion of those with moderate to severe obesity decreased from 40.70% in 2011 to 32.80% in 2017. Multiple stepwise regression analysis showed that children who were averse to sports activities preferred greasy foods and had earlier introduction of solid foods as infants, as well as those who were born at a high birth weight, ate fast, and those with parents with obesity were more likely to have obesity themselves (p < 0.05). Conclusions: Although preventative and control measures for childhood obesity have achieved initial results, Chinese preschool children remain to have high levels of overweight and obesity. It is therefore necessary to strengthen monitoring of overweight and obesity in preschool-aged children and implement appropriate interventions when necessary.

Abstract:OBJECTIVE:In order to develop strategies on prevention and cure for obesity, a study regarding dynamic analyses on the prevalence of obesity and overweight of Chinese children and adolescents was carried out.METHODS:Data of more than 980 thousands students of Han nationality, aged 7 through 18 years old were collected from National Surveys on Chinese Students' Constitution and Health which were carried out in 1985, 1991, 1995 and 2000. Samples were divided into four groups as "metropolis", "medium and small sized cities", "prosperous village" and "below-averaged village". Study targets were screened using the new criterion-the "Reference Norm for Screening Overweight and Obesity in Chinese Children and Adolescents" which was set up by the Working Group on Obesity in China. The prevalence of overweight and obesity in various groups in different years were compared and analyzed.RESULTS:The prevalence rates of obesity in 1985 including from the metropolis area, were only 0.2% and 0.1% for boys and girls, and the prevalence of overweight was between 1% and 2% indicating that was no trend of obesity epidemic then. However, a rapid increase of overweight prevalence has been noticed since the early 1990s, and the increments were more seen in the urban than in the rural areas and more in boys than in girls. In the most developed cities including Beijing, the prevalence rates of obesity appeared to be 4.7%, 3.8% and 3.2%, among three groups with higher risk: the 7 - 9, 10 - 12 year-old boys and 7 - 9 year-old girls, respectively. Both high prevalence rates of malnutrition and overweight were also found in that period. Around 1995, a large scale of increments of overweight were found both in the urban and rural groups. In the most developed metropolis, the prevalence of overweight was two to three folds more than that of 10 years ago. The prevalence of obesity were 6% - 8% for boys and 4% - 6% for girls, respectively. Since 2000, most of the Chinese metropolis have started the so called 'overall increment period' of obesity. The prevalence rates of obesity plus overweight had reached 25.4%, 25.5%, 17.0% and 14.3% for boys aged 7 - 9 years and 10 - 12 years, and girls aged 7 - 9 years and 10 - 12 years, respectively. Among them, the prevalence rates of obesity were 12.9% and 9.1% for boys aged 7 - 9 and 10 - 12 years, which had already reached the average level seen in the medium-developed countries in the world. Although the increments of overweight were high, the prevalence of obesity was still low in most of the other groups: 4.8% for boys and 2.6% for girls in the "medium sized" group, 1.5% for boys and 1.7% for girls in the "prosperous village" group, and 0.9% for boys and 1.2% for girls in the "below-averaged village" group, respectively.CONCLUSIONS:The prevalence of obesity in Chinese children and adolescents was considered to be still relatively low. However, the rapid increasing of both obesity and overweight, in both urban and rural areas would arouse special attention. Comprehensive strategies of intervention should include periodical monitoring, education on pattern of nutrition, oxygen-consuming physical exercises and healthy dietary behavior. Two areas on obesity prevention to protect the child from becoming overweight in early ages, and to control the tendency of obesity from overweight, should be emphasized in order to improve the health status and life quality of the Chinese children and adolescents.

Abstract:IMPORTANCE Obesity is a public health challenge in China, but the geographical profiles of overweight and obesity among Chinese children are limited. OBJECTIVE To examine regional disparities in the prevalence of obesity among the heterogeneous population of Chinese children and adolescents to provide a more accurate profile of obesity among children in China. DESIGN, SETTING, AND PARTICIPANTS The Prevalence and Risk Factors for Obesity and Diabetes in Youth (PRODY) study was a cross-sectional survey study conducted from January 1, 2017, to December 31, 2019, among 201 098 children aged 3 to 18 years from 11 provinces, autonomous regions, and municipalities that produced a sample of Chinese children with a full range of ages and wide geographical coverage using a multistage, stratified, cluster-sampling design. EXPOSURES Five regions geographically representative of China (northern, eastern, southern, western, and central). MAIN OUTCOMES AND MEASURES The body weights and heights of all participants were measured. Multilevel, multinomial logistic regression models were used to estimate the prevalence of overweight and obesity. RESULTS Among 201 098 healthy children (105 875 boys [52.6%]; mean [SD] age, 9.8 [3.8] years) from eastern, southern, northern, central, and western China, the highest obesity prevalence was estimated for children aged 8 to 13 years in northern China (from 18.8% [95% CI, 16.2%-21.7%] to 23.6%[95% CI, 20.5%-26.9%]) and for boys aged 3 to 6 years in western China (from 18.1%[95% CI, 10.4%-29.4%] to 28.6%[95% CI, 14.3%-49.0%]). Boys had a higher prevalence than girls of obesity only in eastern and northern China, with a mean difference in prevalence of 4.6%(95% CI, 3.8%-5.4%) and 7.6%(95% CI, 6.5%-8.6%), respectively. CONCLUSIONS AND RELEVANCE In this survey study, substantial geographic disparities in the prevalence of obesity and overweight were found among the heterogeneous population of Chinese children. The results suggest that special attention should be paid to vulnerable children and that regionally adapted interventions are needed to efficiently mitigate obesity in children.

Abstract:Over the past four decades, China has witnessed an important nutritional transition, characterised by a heightened overnutrition burden among children. The country now has the largest population of children with obesity globally. In this paper, we review the epidemiology of childhood obesity in China, its determinants, and risk factors, with a particular focus on school-aged children. Evidence unveils substantial variations across age, gender, and region. We describe multilevel obesogenic determinants, including macro-level social, cultural, and environmental factors; meso-level factors related to schools and communities; and micro-level factors tied to families and individuals from the perinatal-infant stage to childhood and adolescence. The primary drivers of childhood obesity appear to be rooted in the broader macro-level social, economic, and technological environment; obesogenic factors, which have affected school, community, and family environments; and accelerated unhealthy behaviour uptake. Identifying and characterising the catalysts behind the rise in childhood obesity in China is imperative for the development of scalable, effective, and tailored prevention, control, and intervention strategies.

Abstract:Objective To study the body mass index(BMI) distribution and the prevalence of overweight and obesity among children and adolescents in Hangzhou.Methods Based on the BMI reference criteria for screening overweight and obesity in Chinese children and adolescents which was set up by the Working Group On Obesity in Chinese(WGOC),totally 21536 children and adolescents aged 7～18 yeas were sampled,which from 17 primary schools and 12 high schools.Results The P85 and P95 of BMI in male students were higher than WGOC criteria for overweight and obesity.The P85 of BMI in female students aged 7～10 years was higher than WGOC criteria for overweight,while those in older aged groups were lower than the WGOC criteria;the P95 of BMI in all age groups were lower than WGOC criteria for obesity.The prevalence rates of overweight and obesity were 23.5% and 10.0% respectively for boys and 14.0% and 4.9% respectively for girls.Conclusions The prevalence of overweight and obesity among children and adolescents in Hangzhou are high,especially in boys.The emphasis of school health work should be put on prevention and control of them,such as carrying out health education,leading students and their parents to set up right life style and healthy concept.

Abstract:Prevention and control of childhood obesity in China is complex. Despite numerous existing policy endeavours, particularly Healthy China 2030, accompanied by multiministerial initiatives, childhood obesity persists and even exacerbates. In this paper, we review current national policies, assess progress of the existing system managing childhood weight, and identify implementation challenges. Leveraging insights from existing literature and guided by a PEDALS (ie, problem, evidence-based practice, determinants, actions, long term, and scalability) framework, we propose a strategy to refine and integrate current policies, interventions, and research into a policy framework, embedding evidence-based practices into practical solutions for childhood obesity prevention and control in China.

Abstract:Background: Family-based child obesity prevention and control strategy has not yet established in many countries or regions, including China, thus what it needs to do now is to continuously develop and improve the strategies. The purpose of this study were to describe a wider spectrum of risk factors of obesity among preschool children and add to the mounting evidence for further improving suggested intervention measures in future family-based programs. Methods: Data was collected as part of a series of national epidemiological surveys in childhood conducted in 9 Chinese cities. A population-based, 1:1 matched case-control design was employed to examine risk factors of obesity by means of conditional logistic regression. Obesity was defined as the International Obesity Task Force (IOTF) BMI-for-age cut offs. Eligible subjects were 1234 boys and 610 girls aged 3-7 years in 1996 and 2290 boys and 1008 girls in 2006, including obese and non-obese. Results: High birth weight, gestational hypertension and parents' BMI were closely associated with childhood obesity. Breast feeding in the first 4 months was a protective factor in univariate model in 2006 (OR = 0.834, P = 0.0234), but the association was not seen in multivariate. Appetite, eating speed, daily time and intensity for outdoor activities, night sleep time, and time for TV viewing were identified statistically by multivariate model. Those children brought up in extended family or mainly raised by their grandparents or lived in high income or low education families might have an increased risk of becoming obese. Parents' attitudes on weight control of their children significantly differed between obese and non-obese groups. Discussion: A wider spectrum of risk factors and an empirical aggregation of family-related risk factors are discussed to further improve future family-based child obesity prevention and control strategies. Most of the risk factors identified by this study presented ranked or quantitative characteristics which might be transformed from unhealthy threshold to healthy range by behavior modification. Some variables are likely to interact each other, such as appetite and eating speed, or outdoor activity and TV viewing, or BMI and income, but which needs to be further explored in future surveys. Conclusions: The family-related risk factors were summarized from our identified risk factors of obesity among preschool children which strongly supported the further development of family-based programs in preschool period.

Abstract:【Objective】 To examine the epidemic of children's obesity in some areas of Shanghai and the relationship between demographic characteristics and children's obesity so as to provide the clues for preventing the obesity. 【Methods】 1 350 children from ten kindergartens were selected randomly from Hongkou district,Putuo and Pudong in Shanghai.A self-designed questionnaire was used to investigate the children's weight,height,and parenting characteristics,and so on.Children's height and weight came from the data of physical check in June,2009.The relationships between demographic characteristics and children's obesity were analyzed with χ2 test and multivariable logistic regression. 【Results】 The data showed that the rates of obesity and over-weight were 10.5% and 14.0% respectively.The place of registered residence,father's BMI,mother's BMI,children from only child family,and family structure were associated with obesity when other variables were not controlled.The father's BMI,mother's BMI,and family structure were associated with obesity when other variables were considered. 【Conclusions】 The rate of obesity and overweight in Shanghai is similar to that in developed countries.The main influence factor of child obesity in Shanghai is highly parents' BMI and nuclear family.
Explorations on risk profiles for overweight and obesity in 9501 preschool-aged children

Abstract:BACKGROUND: Childhood obesity places a major burden on global public health. We aimed to identify and characterize potential factors, both individually and jointly, in association with overweight and obesity in Chinese preschool-aged children.METHODS: We cross-sectionally recruited 9501 preschool-aged children from 30 kindergartens in Beijing and Tangshan. Overweight and obesity are defined according to the World Health Organization (WHO), International Obesity Task Force (IOTF), and China criteria.RESULTS: After multivariable adjustment, eating speed, sleep duration, birthweight, and paternal body mass index (BMI) were consistently and significantly associated with childhood overweight and obesity under three growth criteria at a significance level of 5%. Additional fast food intake frequency, maternal BMI, gestational weight gain (GWG) and maternal pre-pregnancy BMI were significant factors for overweight (WHO criteria) and obesity (both IOTF and China criteria). Importantly, there were significant interactions between parental obesity and eating speed for childhood obesity. Finally, for practical reasons, risk nomogram models were constructed for childhood overweight and obesity based on significant factors under each criterion, with good prediction accuracy.CONCLUSION: Our findings indicated a synergistic association of lifestyle, fetal and neonatal, and family-related factors with the risk of experiencing overweight and obesity among preschool-aged children.

Abstract:ObjectiveTo examine the trends in the prevalence of overweight and obesity among preschool children from 2006 to 2014.MethodsA total of 145,078 children aged 3-6 years from 46 kindergartens finished the annual health examination in Tianjin, China. Height, weight and other information were obtained using standardized methods. Z-scores for weight, height, and BMI were calculated based on the standards for the World Health Organization (WHO) child growth standards.ResultsFrom 2006 to 2014, mean values of height z-scores significantly increased from 0.34 to 0.54, mean values of weight z-scores kept constant, and mean values of BMI z-scores significantly decreased from 0.40 to 0.23. Mean values of height z-scores, weight z-scores, and BMI z-scores slightly decreased among children from 3 to 4 years old, and then increased among children from 4 to 6 years old. Between 2006 and 2014, there were no significant changes in prevalence of overweight (BMI z-scores >2 SD) and obesity (BMI z-scores >3 SD) among 3-4 years children. However, prevalence of obesity (BMI z-scores >2 SD) increased from 8.8% in 2006 to 10.1% in 2010, and then kept stable until 2014 among 5-6 years children. Boys had higher prevalence of obesity than girls.ConclusionsMean values of BMI z-scores decreased from 2006 to 2014 among Chinese children aged 3-6 years old due to the significant increase of height z-scores. Prevalence of obesity increased from 2006 to 2010, and then kept stable until 2014 among children aged 5-6 years. The prevalence of obesity was higher in boys than in girls.
