Abstract:Background and aims: Endoscopy is a pivotal method for detecting early gastric cancer (EGC). However, skill among endoscopists varies greatly. Here, we proposed a deep learning-based system named ENDOANGEL-ME to diagnose EGC in magnifying image-enhanced endoscopy (M-IEE). Methods: M-IEE images were retrospectively obtained from 6 hospitals in China, including 4667 images for training and validation, 1324 images for internal tests, and 4702 images for external tests. One hundred eighty-seven stored videos from 2 hospitals were used to evaluate the performance of ENDOANGEL-ME and endoscopists and to assess the effect of ENDOANGEL-ME on improving the performance of endoscopists. Prospective consecutive patients undergoing M-IEE were enrolled from August 17, 2020 to August 2, 2021 in Renmin Hospital of Wuhan University to assess the applicability of ENDOANGEL-ME in clinical practice. Results: A total of 3099 patients undergoing M-IEE were enrolled in this study. The diagnostic accuracy of ENDOANGEL-ME for diagnosing EGC was 88.44% and 90.49% in internal and external images, respectively. In 93 internal videos, ENDOANGEL-ME achieved an accuracy of 90.32% for diagnosing EGC, significantly superior to that of senior endoscopists (70.16% ± 8.78%). In 94 external videos, with the assistance of ENDOANGEL-ME, endoscopists showed improved accuracy and sensitivity (85.64% vs 80.32% and 82.03% vs 67.19%, respectively). In 194 prospective consecutive patients with 251 lesions, ENDOANGEL-ME achieved a sensitivity of 92.59% (25/27) and an accuracy of 83.67% (210/251) in real clinical practice. Conclusions: This multicenter diagnostic study showed that ENDOANGEL-ME can be well applied in the clinical setting. (Clinical trial registration number: ChiCTR2000035116.).

Abstract:Background: Esophagogastroduodenoscopy (EGD) is a prerequisite for detecting upper gastrointestinal lesions especially early gastric cancer (EGC). An artificial intelligence system has been shown to monitor blind spots during EGD. In this study, we updated the system (ENDOANGEL), verified its effectiveness in improving endoscopy quality, and pretested its performance in detecting EGC in a multicenter randomized controlled trial. Methods: ENDOANGEL was developed using deep convolutional neural networks and deep reinforcement learning. Patients undergoing EGD in five hospitals were randomly assigned to the ENDOANGEL-assisted group or to a control group without use of ENDOANGEL. The primary outcome was the number of blind spots. Secondary outcomes included performance of ENDOANGEL in predicting EGC in a clinical setting. Results: 1050 patients were randomized, and 498 and 504 patients in the ENDOANGEL and control groups, respectively, were analyzed. Compared with the control group, the ENDOANGEL group had fewer blind spots (mean 5.38 [standard deviation (SD) 4.32] vs. 9.82 [SD 4.98]; P < 0.001) and longer inspection time (5.40 [SD 3.82] vs. 4.38 [SD 3.91] minutes; P < 0.001). In the ENDOANGEL group, 196 gastric lesions with pathological results were identified. ENDOANGEL correctly predicted all three EGCs (one mucosal carcinoma and two high grade neoplasias) and two advanced gastric cancers, with a per-lesion accuracy of 84.7 %, sensitivity of 100 %, and specificity of 84.3 % for detecting gastric cancer. Conclusions: In this multicenter study, ENDOANGEL was an effective and robust system to improve the quality of EGD and has the potential to detect EGC in real time.

Abstract:Background: With the development of new technologies such as magnifying endoscopy with narrow band imaging, endoscopists achieved better accuracy for diagnosis of gastric cancer (GC) in various aspects. However, to master such skill takes substantial effort and could be difficult for inexperienced doctors. Therefore, a novel diagnostic method based on artificial intelligence (AI) was developed and its effectiveness was confirmed in many studies. AI system using convolutional neural network has showed marvelous results in the ongoing trials of computer-aided detection of colorectal polyps. Summary: With AI’s efficient computational power and learning capacities, endoscopists could improve their diagnostic accuracy and avoid the overlooking or over-diagnosis of gastric neoplasm. Several systems have been reported to achieved decent accuracy. Thus, AI-assisted endoscopy showed great potential on more accurate and sensitive ways for early detection, differentiation, and invasion depth prediction of gastric lesions. However, the feasibility, effectiveness, and safety in daily practice remain to be tested. Key messages: This review summarizes the current status of different AI applications in early GC diagnosis. More randomized controlled trails will be needed before AI could be widely put into clinical practice.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background and Aims</h3><p>Gastric precancerous conditions, including gastric atrophy (GA) and intestinal metaplasia (IM), play an important role in the development of gastric cancer. Image-enhanced endoscopy (IEE) shows great potential in diagnosing gastric precancerous conditions and adenocarcinoma. In this study, a deep convolutional neural network (DCNN) system, named ENDOANGEL, was constructed to detect gastric precancerous conditions by IEE.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Endoscopic images were retrospectively obtained from 5 hospitals in China for the development, validation, internal and external test of the system. Prospective consecutive patients receiving IEE were enrolled from January 13, 2020 to October 29, 2020 in Renmin Hospital of Wuhan University to assess in real time the applicability of the proposed computer-aided detection (CADe) system in clinical practice, and the performance of CADe was compared with that of endoscopists.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Six thousand two hundred fifty endoscopy images from 760 patients and 98 video clips from 77 individuals undergoing IEE were enrolled in this study. The diagnostic accuracy of GA was 0.901 (95% CI, 0.883-0.917) in the internal test set, 0.864 (95% CI, 0.842-0.884) in the multicenter external test set, and 0.878 (95% CI, 0.796-0.935) in the prospective video test set. The diagnostic accuracy of IM was 0.908 (95% CI, 0.889-0.924) in the internal test set, 0.859 (95% CI, 0.837-0.880) in the multicenter external test set, and 0.898 (95% CI, 0.820-0.950) in the prospective video test set. CADe achieved similar diagnostic accuracy to that of the experts for detecting GA (0.869 [95% CI, 0.790-0.927] vs 0.846 [95% CI, 0.808-0.879]; p=0.396) and IM (0.888 [95% CI, 0.812-0.941] vs 0.820 [95% CI, 0.780-0.855]; p=0.117), and it was superior to that of nonexperts for GA (0.750 [95% CI, 0.711-0.786]; p=0.008) and IM (0.736 [95% CI, 0.697-0.773]; p=0.028).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>CADe achieved high diagnostic accuracy in gastric precancerous conditions, which was similar to that of experts and superior to that of nonexperts. Thus, CADe has a wide application prospect in assisting to diagnose gastric precancerous conditions.</p>

Abstract:Background and aims: White-light endoscopy (WLE) is the most pivotal tool to detect gastric cancer in an early stage. However, the skill among endoscopists varies greatly. Here, we aim to develop a deep learning-based system named ENDOANGEL-LD (lesion detection) to assist in detecting all focal gastric lesions and predicting neoplasms by WLE. Methods: Endoscopic images were retrospectively obtained from Renmin Hospital of Wuhan University (RHWU) for the development, validation, and internal test of the system. Additional external tests were conducted in 5 other hospitals to evaluate the robustness. Stored videos from RHWU were used for assessing and comparing the performance of ENDOANGEL-LD with that of experts. Prospective consecutive patients undergoing upper endoscopy were enrolled from May 6, 2021 to August 2, 2021 in RHWU to assess clinical practice applicability. Results: Over 10,000 patients undergoing upper endoscopy were enrolled in this study. The sensitivities were 96.9% and 95.6% for detecting gastric lesions and 92.9% and 91.7% for diagnosing neoplasms in internal and external patients, respectively. In 100 videos, ENDOANGEL-LD achieved superior sensitivity and negative predictive value for detecting gastric neoplasms from that of experts (100% vs 85.5% ± 3.4% [P = .003] and 100% vs 86.4% ± 2.8% [P = .002], respectively). In 2010 prospective consecutive patients, ENDOANGEL-LD achieved a sensitivity of 92.8% for detecting gastric lesions with 3.04 ± 3.04 false positives per gastroscopy and a sensitivity of 91.8% and specificity of 92.4% for diagnosing neoplasms. Conclusions: Our results show that ENDOANGEL-LD has great potential for assisting endoscopists in screening gastric lesions and suspicious neoplasms in clinical work. Abstract:BackgroundDiagnosis of early gastric cancer (EGC) under narrow band imaging endoscopy (NBI) is dependent on expertise and skills. We aimed to elucidate whether artificial intelligence (AI) could diagnose EGC under NBI and evaluate the diagnostic assistance of the AI system.MethodsIn this retrospective diagnostic study, 21,785 NBI images and 20 videos from five centers were divided into a training dataset (13,151 images, 810 patients), an internal validation dataset (7057 images, 283 patients), four external validation datasets (1577 images, 147 patients), and a video validation dataset (20 videos, 20 patients). All the images were labeled manually and used to train an AI system using You look only once v3 (YOLOv3). Next, the diagnostic performance of the AI system and endoscopists were compared and the diagnostic assistance of the AI system was assessed. The accuracy, sensitivity, specificity, and AUC were primary outcomes.ResultsThe AI system diagnosed EGCs on validation datasets with AUCs of 0.888–0.951 and diagnosed all the EGCs (100.0%) in video dataset. The AI system achieved better diagnostic performance (accuracy, 93.2%, 95% CI, 90.0–94.9%) than senior (85.9%, 95% CI, 84.2–87.4%) and junior (79.5%, 95% CI, 77.8–81.0%) endoscopists. The AI system significantly enhanced the performance of endoscopists in senior (89.4%, 95% CI, 87.9–90.7%) and junior (84.9%, 95% CI, 83.4–86.3%) endoscopists.ConclusionThe NBI AI system outperformed the endoscopists and exerted potential assistant impact in EGC identification. Prospective validations are needed to evaluate the clinical reinforce of the system in real clinical practice.Graphical abstract

Abstract:White light endoscopy is the most pivotal tool for detecting early gastric neoplasms. Previous artificial intelligence (AI) systems were primarily unexplainable, affecting their clinical credibility and acceptability. We aimed to develop an explainable AI named ENDOANGEL-ED (explainable diagnosis) to solve this problem. A total of 4482 images and 296 videos with focal lesions from 3279 patients from eight hospitals were used for training, validating, and testing ENDOANGEL-ED. A traditional sole deep learning (DL) model was trained using the same dataset. The performance of ENDOANGEL-ED and sole DL was evaluated on six levels: internal and external images, internal and external videos, consecutive videos, and man–machine comparison with 77 endoscopists in videos. Furthermore, a multi-reader, multi-case study was conducted to evaluate the ENDOANGEL-ED's effectiveness. A scale was used to compare the overall acceptance of endoscopists to traditional and explainable AI systems. The ENDOANGEL-ED showed high performance in the image and video tests. In man–machine comparison, the accuracy of ENDOANGEL-ED was significantly higher than that of all endoscopists in internal (81.10% vs. 70.61%, p < 0.001) and external videos (88.24% vs. 78.49%, p < 0.001). With ENDOANGEL-ED's assistance, the accuracy of endoscopists significantly improved (70.61% vs. 79.63%, p < 0.001). Compared with the traditional AI, the explainable AI increased the endoscopists' trust and acceptance (4.42 vs. 3.74, p < 0.001; 4.52 vs. 4.00, p < 0.001). In conclusion, we developed a real-time explainable AI that showed high performance, higher clinical credibility, and acceptance than traditional DL models and greatly improved the diagnostic ability of endoscopists.

Abstract:Background: Prompt diagnosis of early gastric cancer (EGC) is crucial for improving patient survival. However, most previous computer-aided-diagnosis (CAD) systems did not concretize or explain diagnostic theories. We aimed to develop a logical anthropomorphic artificial intelligence (AI) diagnostic system named ENDOANGEL-LA (logical anthropomorphic) for EGCs under magnifying image enhanced endoscopy (M-IEE). Methods: We retrospectively collected data for 692 patients and 1897 images from Renmin Hospital of Wuhan University, Wuhan, China between Nov 15, 2016 and May 7, 2019. The images were randomly assigned to the training set and test set by patient with a ratio of about 4:1. ENDOANGEL-LA was developed based on feature extraction combining quantitative analysis, deep learning (DL), and machine learning (ML). 11 diagnostic feature indexes were integrated into seven ML models, and an optimal model was selected. The performance of ENDOANGEL-LA was evaluated and compared with endoscopists and sole DL models. The satisfaction of endoscopists on ENDOANGEL-LA and sole DL model was also compared. Findings: Random forest showed the best performance, and demarcation line and microstructures density were the most important feature indexes. The accuracy of ENDOANGEL-LA in images (88.76%) was significantly higher than that of sole DL model (82.77%, p = 0.034) and the novices (71.63%, p<0.001), and comparable to that of the experts (88.95%). The accuracy of ENDOANGEL-LA in videos (87.00%) was significantly higher than that of the sole DL model (68.00%, p<0.001), and comparable to that of the endoscopists (89.00%). The accuracy (87.45%, p<0.001) of novices with the assistance of ENDOANGEL-LA was significantly improved. The satisfaction of endoscopists on ENDOANGEL-LA was significantly higher than that of sole DL model. Interpretation: We established a logical anthropomorphic system (ENDOANGEL-LA) that can diagnose EGC under M-IEE with diagnostic theory concretization, high accuracy, and good explainability. It has the potential to increase interactivity between endoscopists and CADs, and improve trust and acceptability of endoscopists for CADs. Abstract:The landscape of gastrointestinal endoscopy continues to evolve as new technologies and techniques become available. The advent of image-enhanced and magnifying endoscopies has highlighted the step toward perfecting endoscopic screening and diagnosis of gastric lesions. Simultaneously, with the development of convolutional neural network, artificial intelligence (AI) has made unprecedented breakthroughs in medical imaging, including the ongoing trials of computer-aided detection of colorectal polyps and gastrointestinal bleeding. In the past demi-decade, applications of AI systems in gastric cancer have also emerged. With AI's efficient computational power and learning capacities, endoscopists can improve their diagnostic accuracies and avoid the missing or mischaracterization of gastric neoplastic changes. So far, several AI systems that incorporated both traditional and novel endoscopy technologies have been developed for various purposes, with most systems achieving an accuracy of more than 80%. However, their feasibility, effectiveness, and safety in clinical practice remain to be seen as there have been no clinical trials yet. Nonetheless, AI-assisted endoscopies shed light on more accurate and sensitive ways for early detection, treatment guidance and prognosis prediction of gastric lesions. This review summarizes the current status of various AI applications in gastric cancer and pinpoints directions for future research and clinical practice implementation from a clinical perspective.

Abstract:Background: Artificial intelligence (AI) for gastric cancer diagnosis has been discussed in recent years. The role of AI in early gastric cancer is more important than in advanced gastric cancer since early gastric cancer is not easily identified in clinical practice. However, to our knowledge, past syntheses appear to have limited focus on the populations with early gastric cancer. Objective: The purpose of this study is to evaluate the diagnostic accuracy of AI in the diagnosis of early gastric cancer from endoscopic images. Methods: We conducted a systematic review from database inception to June 2020 of all studies assessing the performance of AI in the endoscopic diagnosis of early gastric cancer. Studies not concerning early gastric cancer were excluded. The outcome of interest was the diagnostic accuracy (comprising sensitivity, specificity, and accuracy) of AI systems. Study quality was assessed on the basis of the revised Quality Assessment of Diagnostic Accuracy Studies. Meta-analysis was primarily based on a bivariate mixed-effects model. A summary receiver operating curve and a hierarchical summary receiver operating curve were constructed, and the area under the curve was computed. Results: We analyzed 12 retrospective case control studies (n=11,685) in which AI identified early gastric cancer from endoscopic images. The pooled sensitivity and specificity of AI for early gastric cancer diagnosis were 0.86 (95% CI 0.75-0.92) and 0.90 (95% CI 0.84-0.93), respectively. The area under the curve was 0.94. Sensitivity analysis of studies using support vector machines and narrow-band imaging demonstrated more consistent results. Conclusions: For early gastric cancer, to our knowledge, this was the first synthesis study on the use of endoscopic images in AI in diagnosis. AI may support the diagnosis of early gastric cancer. However, the collocation of imaging techniques and optimal algorithms remain unclear. Competing models of AI for the diagnosis of early gastric cancer are worthy of future investigation. Abstract:Abstract Gastric cancer (GC) is one of the most common malignant tumors with high mortality. Accurate diagnosis and treatment decisions for GC rely heavily on human experts' careful judgments on medical images. However, the improvement of the accuracy is hindered by imaging conditions, limited experience, objective criteria, and inter-observer discrepancies. Recently, the developments of machine learning, especially deep-learning algorithms, have been facilitating computers to extract more information from data automatically. Researchers are exploring the far-reaching applications of artificial intelligence (AI) in various clinical practices, including GC. Herein, we aim to provide a broad framework to summarize current research on AI in GC. In the screening of GC, AI can identify precancerous diseases and assist in early cancer detection with endoscopic examination and pathological confirmation. In the diagnosis of GC, AI can support tumor-node-metastasis (TNM) staging and subtype classification. For treatment decisions, AI can help with surgical margin determination and prognosis prediction. Meanwhile, current approaches are challenged by data scarcity and poor interpretability. To tackle these problems, more regulated data, unified processing procedures, and advanced algorithms are urgently needed to build more accurate and robust AI models for GC.

Abstract:Although the blood supply has become safer with regard to transmission of infectious agents, attention should continue to focus on understanding and eliminating the other serious risks associated with transfusion. Transfusion-related acute lung injury (TRALI) is one such risk, only recently becoming recognized as an important and potentially preventable clinical syndrome. Strategies for prevention of TRALI, however, must rely on knowledge regarding its etiology and diagnosis, and significant gaps in our understanding of the syndrome currently exist. This review summarizes what is known and unknown about the incidence, severity, etiology, diagnosis, and prevention of TRALI and the potential consequences of these knowledge gaps.
Development and validation of a real-time artificial intelligence-assisted system for detecting early gastric cancer: A multicentre retrospective diagnostic study

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>We aimed to develop and validate a real-time deep convolutional neural networks (DCNNs) system for detecting early gastric cancer (EGC).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>All 45,240 endoscopic images from 1364 patients were divided into a training dataset (35823 images from 1085 patients) and a validation dataset (9417 images from 279 patients). Another 1514 images from three other hospitals were used as external validation. We compared the diagnostic performance of the DCNN system with endoscopists, and then evaluated the performance of endoscopists with or without referring to the system. Thereafter, we evaluated the diagnostic ability of the DCNN system in video streams. The accuracy, sensitivity, specificity, positive predictive value, negative predictive value and Cohen's kappa coefficient were measured to assess the detection performance.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Finding</h3><p>The DCNN system showed good performance in EGC detection in validation datasets, with accuracy (85.1%–91.2%), sensitivity (85.9%–95.5%), specificity (81.7%–90.3%), and AUC (0.887–0.940). The DCNN system showed better diagnostic performance than endoscopists and improved the performance of endoscopists. The DCNN system was able to process oesophagogastroduodenoscopy (OGD) video streams to detect EGC lesions in real time.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Interpretation</h3><p>We developed a real-time DCNN system for EGC detection with high accuracy and stability. Abstract:Background and Aims The clinical application of gastrointestinal endoscopy for the diagnosis of multiple diseases using artificial intelligence (AI) has been limited by its high false positive rates. There is an unmet need to develop a Gastrointestinal Endoscopy AI-assisted Diagnosis System (GEADS) to improve diagnostic accuracy and clinical utility. Methods In this retrospective, multicenter study, a convolutional neural network was trained to assess upper gastrointestinal diseases based on 26,228 endoscopic images from Dazhou Central Hospital that were randomly assigned (3:1:1) to the training dataset, validation dataset, and test dataset, respectively. To validate the model, 6 external independent datasets comprising 51,372 images of upper gastrointestinal diseases were collected. Besides, one prospective dataset comprising 27,975 images were collected. The performance of GEADS was compared with endoscopists with two professional degrees of expertise: expert, novice. 8 endoscopists in the expert group with more than 5 years of experience, 3 endoscopists in the novice group with 1 to 5 years of experience. Results The GEADS model achieved an accuracy of 0.918 (95% confidence interval [CI]: 0.914-0.922), with an F1 score of 0.884 (95% CI: 0.879-0.889), recall of 0.873 (95% CI: 0.868-0.878) and a precision of 0.890 (95% CI: 0.885-0.895) in the internal validation dataset. In the external validation datasets and one prospective validation dataset, the diagnostic accuracy of GEADS ranged from 0.841 (95% CI: 0.834-0.848) to 0.949 (95% CI: 0.935-0.963). With the help of GEADS, the diagnosing accuracies of novice and expert endoscopists were significantly improved ( P < 0.001). Conclusions The AI system can assist endoscopists in improving the accuracy of diagnosing upper gastrointestinal diseases.

Abstract:Background Artificial intelligence-assisted gastroscopy (AIAG) based on deep learning has been validated in various scenarios, but there is a lack of studies regarding diagnosing neoplasms under white light endoscopy. This study explored the potential role of AIAG systems in enhancing the ability of endoscopists to diagnose gastric tumor lesions under white light. Methods A total of 251 patients with complete pathological information regarding electronic gastroscopy, biopsy, or ESD surgery in Xi’an Gaoxin Hospital were retrospectively collected and comprised 64 patients with neoplasm lesions (excluding advanced cancer) and 187 patients with non-neoplasm lesions. The diagnosis competence of endoscopists with intermediate experience and experts was compared for gastric neoplasms with or without the assistance of AIAG, which was developed based on ResNet-50. Results For the 251 patients with difficult clinical diagnoses included in the study, compared with endoscopists with intermediate experience, AIAG’s diagnostic competence was much higher, with a sensitivity of 79.69% (79.69% vs. 72.50%, p = 0.012) and a specificity of 73.26% (73.26% vs. 52.62%, p < 0.001). With the help of AIAG, the endoscopists with intermediate experience (<8 years) demonstrated a relatively higher specificity (59.79% vs. 52.62%, p < 0.001). Experts (≥8 years) had similar results with or without AI assistance (with AI vs. without AI; sensitivities, 70.31% vs. 67.81%, p = 0.358; specificities, 83.85% vs. 85.88%, p = 0.116). Conclusion With the assistance of artificial intelligence (AI) systems, the ability of endoscopists with intermediate experience to diagnose gastric neoplasms is significantly improved, but AI systems have little effect on experts.

Abstract:Objective: The aim of this study was to assess the diagnostic ability of artificial intelligence (AI) in the detection of early upper gastrointestinal cancer (EUGIC) using endoscopic images. Methods: Databases were searched for studies on AI-assisted diagnosis of EUGIC using endoscopic images. The pooled area under the curve (AUC), sensitivity, specificity, positive likelihood ratio (PLR), negative likelihood ratio (NLR), and diagnostic odds ratio (DOR) with 95% confidence interval (CI) were calculated. Results: Overall, 34 studies were included in our final analysis. Among the 17 image-based studies investigating early esophageal cancer (EEC) detection, the pooled AUC, sensitivity, specificity, PLR, NLR, and DOR were 0.98, 0.95 (95% CI, 0.95–0.96), 0.95 (95% CI, 0.94–0.95), 10.76 (95% CI, 7.33–15.79), 0.07 (95% CI, 0.04–0.11), and 173.93 (95% CI, 81.79–369.83), respectively. Among the seven patient-based studies investigating EEC detection, the pooled AUC, sensitivity, specificity, PLR, NLR, and DOR were 0.98, 0.94 (95% CI, 0.91–0.96), 0.90 (95% CI, 0.88–0.92), 6.14 (95% CI, 2.06–18.30), 0.07 (95% CI, 0.04–0.11), and 69.13 (95% CI, 14.73–324.45), respectively. Among the 15 image-based studies investigating early gastric cancer (EGC) detection, the pooled AUC, sensitivity, specificity, PLR, NLR, and DOR were 0.94, 0.87 (95% CI, 0.87–0.88), 0.88 (95% CI, 0.87–0.88), 7.20 (95% CI, 4.32–12.00), 0.14 (95% CI, 0.09–0.23), and 48.77 (95% CI, 24.98–95.19), respectively. Conclusions: On the basis of our meta-analysis, AI exhibited high accuracy in diagnosis of EUGIC. Abstract:Convolutional neural networks in the field of artificial intelligence show great potential in image recognition. It assisted endoscopy to improve the detection rate of early gastric cancer. The 5-year survival rate for advanced gastric cancer is less than 30%, while the 5-year survival rate for early gastric cancer is more than 90%. Therefore, earlier screening for gastric cancer can lead to a better prognosis. However, the detection rate of early gastric cancer in China has been extremely low due to many factors, such as the presence of gastric cancer without obvious symptoms, difficulty identifying lesions by the naked eye, and a lack of experience among endoscopists. The introduction of artificial intelligence can help mitigate these shortcomings and greatly improve the accuracy of screening. According to relevant reports, the sensitivity and accuracy of artificial intelligence trained on deep cirrocumulus neural networks are better than those of endoscopists, and evaluations also take less time, which can greatly reduce the burden on endoscopists. In addition, artificial intelligence can also perform real-time detection and feedback on the inspection process of the endoscopist to standardize the operation of the endoscopist. AI has also shown great potential in training novice endoscopists. With the maturity of AI technology, AI has the ability to improve the detection rate of early gastric cancer in China and reduce the death rate of gastric cancer related diseases in China.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background and Aims</h3><p>Narrow-band imaging with magnifying endoscopy (ME-NBI) has shown advantages in the diagnosis of early gastric cancer (EGC). However, proficiency in diagnostic algorithms requires substantial expertise and experience. In this study, we aimed to develop a computer-aided diagnostic model, EGCM, to analyze and assist in the diagnosis of EGC under ME-NBI.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>A total of 1777 ME-NBI images from 295 cases were collected from 3 centers. These cases were randomly divided into a training cohort (TC, n=170), an internal test cohort (ITC, n=73), and an external test cohort (ETC, n=52). EGCM based on VGG-19 with a single fully connected 2-classification layer was developed via fine-tuning and validated on all of the cohorts. Furthermore, we compared the model with 8 endoscopists with varying experience. Primary comparison measures included accuracy (ACC), the area under the receiver operating characteristic curve (AUC), sensitivity (Sn), specificity (Sp), positive predictive value (PPV), and negative predictive value (NPV).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>EGCM acquired AUCs of 0.808 in the ITC and 0.813 in the ETC. Moreover, EGCM achieved similar predictive performance to the senior endoscopists (ACC: 0.770 vs 0.755, <em>p</em>=0.355; Sn: 0.792 vs 0.767, <em>p</em>=0.183; Sp: 0.745 vs 0.742, <em>p</em>=0.931), but better than the junior endoscopists (ACC: 0.770 vs 0.728, <em>p</em><0.05). After referring to the results of EGCM, the average diagnostic ability of the endoscopists was significantly improved in terms of accuracy, sensitivity, PPV, and NPV (<em>p</em><0.05).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>EGCM exhibited comparable performance to senior endoscopists in the diagnosis of EGC and showed the potential value in aiding and improving the diagnosis of EGC by endoscopists.</p>
