Bond Strength and Surface Roughness of Two Ceramics After Metal Bracket Debonding

Abstract:Objective: The aims of this study were to compare the bond strength between metallic brackets and two different glass ceramics and to evaluate the ceramic surface roughness after different finishing protocols. Methods: The surface roughness of lithium disilicate and resin matrix ceramic samples was measured (initial). All samples were treated with hydrofluoric acid and silane and bonded to metallic brackets with orthodontic cement adhesive. Shear bond strength tests were performed using a universal testing machine (n=12). The surface roughness was measured again (intermediate, n=6) after removing the remaining cement adhesive from the ceramic surfaces with a diamond or 24-blade bur after polishing the ceramic surfaces (final, n=6). Results: The resin matrix ceramic had the highest bond strength. The rotatory instrument used for the removal of cement adhesive did not affect the surface roughness of the resin matrix ceramic or lithium disilicate (p=0.985 and p=0.504, respectively), but did affect the evaluation time (p<0.001) for both restorative materials. The intermediate roughness was the highest. For the resin matrix ceramic, polishing promoted a final surface roughness similar to the initial condition; however, changes in the surface shape of this ceramic could be visibly observed when using a 24-blade bur. Conclusion: The bond strength of metallic brackets bonded on resin-matrix ceramics is higher than bonding on lithium disilicate. The use of diamond burs for the removal of the remaining adhesive from the resin matrix ceramics is highly recommended.
Ceramic and Composite Polishing Systems for Milled Lithium Disilicate Restorative Materials: A 2D and 3D Comparative In Vitro Study

Abstract:Purpose: This study aims to evaluate the effectiveness of two ceramic and two composite polishing systems for a novel chairside computer-aided design/computer-aided manufacturing (CAD/CAM) lithium disilicate ceramic with three-dimensional and two-dimensional microscopy images. This ceramic material can be used for implant-supported or tooth-borne single-unit prostheses. Materials and Methods: Sixty flat samples of novel chairside CAD/CAM reinforced lithium disilicate ceramic (Amber Mill, Hass Bio) were divided into five groups (n = 15/group) and treated as follows: Group 1 (NoP), no polished treatment; group 2 (CeDi), polished with ceramic Dialite LD (Brasseler USA); group 3, (CeOp) polished with ceramic OptraFine (Ivoclar Vivadent); group 4, (CoDi) polished with composite DiaComp (Brasseler USA), and group 5 (CoAs), polished with composite Astropol (Ivoclar Vivadent). The polished ceramic surface topography was observed and measured with three-dimensional and two-dimensional images. Results: All polishing systems significantly reduced the surface roughness compared with the non-polished control group (Sa 1.15 μm). Group 2 (CeDi) provided the smoothest surface arithmetical mean eight with 0.32 μm, followed by group 3 (CeOp) with 0.34 μm. Group 5 (CoAs) with 0.52 μm provided the smoothest surface among the composite polishing kits. Group 4 (CoDi) with 0.66 μm provided the least smooth surface among all polishing systems tested. Conclusions: Despite the effectiveness of ceramic polishing systems being superior to composite polishing systems of the CAD/CAM lithium disilicate restorative material, both polishing systems significantly improved the smoothness.

Abstract:Background: Developing efficient bonding techniques for orthodontic brackets and all-ceramic materials continues to pose a clinical difficulty. This study aimed to evaluate the shear bond strengths (SBS) of metal and ceramic brackets to various all-ceramic CAD-CAM materials, such as lithium disilicate CAD (LDS-CAD), polymer-infiltrated ceramic (PIC), zirconia-reinforced lithium silicate glass ceramic (ZLS), and 5YTZP zirconia after different surface treatments and thermal cycling. Materials and methods: The samples were divided into two groups to be bonded with ceramic and metal lower incisor brackets. Each group was subdivided into a control group devoid of any surface treatment, 10% HF acid (HFA) etching, ceramic etch & prime (MEP), Al2O3 air abrasion, and medium grit diamond bur roughening. After surface treatment, brackets were bonded with composite resin cement, thermal cycled, and tested for shear bond strength. The failed surfaces were evaluated with a digital microscope to analyse the type of failure. The data were statistically analysed using a one-way ANOVA and Tukey HSD tests at p < 0.05. Results: The highest mean bond strengths were found with HFA etching in LDS-CAD (13.17 ± 0.26 MPa) and ZLS (12.85 0.52 MPa). Diamond bur recorded the lowest mean bond strength roughening across all the ceramic groups. There were significant differences in mean shear bond values per surface treatment (p < 0.001) and ceramic materials. Conclusion: Among the surface treatment protocols evaluated, HFA etching and MEP surface treatment resulted in enhanced bond strength of both ceramic and metal brackets to CAD-CAM all ceramic materials.
Comparison of Various Surface Treatment Procedures on the Roughness and Susceptibility to Staining of Provisional Prosthodontic Materials

Abstract:Esthetically pleasing temporary prostheses are often necessary for extended periods in a variety of clinical scenarios. Adjustments to the occlusion or margins are commonly needed before cementing the temporary prosthesis. Therefore, it is clinically necessary to repolish the rough surface to avoid biological and esthetic issues associated with rough surfaces. The purpose of this in vitro study was to assess and compare the impact of various polishing protocols on the surface roughness and color stability of three resin materials used for provisional crowns. A total of 150 specimens were fabricated from auto-polymerizing polymethyl methacrylate, bis-acryl composite, and Methyl methacrylate-LC resin using a stainless steel mold. Each material group was divided into five groups (n = 10) based on the applied surface treatment: positive control group (G1): no roughening or surface treatment, Negative control group (G2): acrylic bur-roughened surface without any polishing, the different surface treatment groups of silicon carbide and aluminum oxide stone polishing (G3), diamond-coated rubber twist (G4), and Surface Glaze (G5). An optical profilometer was used to assess the surface roughness of all samples. After undergoing 6000 cycles of thermocycling followed by immersion in a coffee solution for 15 days at 37 °C, color parameters were measured using a spectrophotometer both before and after a storage period to evaluate color differences. A two-way ANOVA test with α = 0.05 significance level was carried out to determine the impacts of both the materials utilized and the polishing protocol. Among the three types of resin examined, the bisacryl group exhibited superior surface quality in positive control groups, while PMMA resin demonstrated higher polishability. The diamond-coated rubber twits resulted in lower Ra values of 0.36 (0.01) µm, 0.52 (0.11) µm, and 0.28 (0.05) µm for PMMA, BAMA, and MMLC resins, respectively. The application of photo-polymerized surface glaze led to a plaque accumulation threshold of 0.2 µm across all resin groups. The greatest mean color change occurred in the negative control group, indicating a propensity for more staining on rougher surfaces. The Bisacryl resin exhibited higher ΔE values, whereas PMMA showed better color stability. The lowest ΔE values were found when the surface glaze was applied to all of the provisional crown resins. Untreated Bisacryl resin exhibited the lowest Ra values, while PMMA resins demonstrated superior surface morphology after polishing. PMMA provisional crown resins showed increased resistance to staining. The use of surface glaze enhanced both smoothness and color stability on the surfaces.

Abstract:Background: The purpose of this study was to evaluate orthodontic debonding methods by comparing the surface roughness and enamel morphology of teeth after applying two different debonding methods and three different polishing techniques. Methods: Forty eight human maxillary premolars, extracted for orthodontic reasons, were randomly divided into three groups. Brackets were bonded to teeth with RMGIC (Fuji Ortho LC, GC, Tokyo, Japan) (two groups, n = 18 each) after acid etching (30s), light cured for 40 s, exposed to thermocycling, then underwent 2 different bracket debonding methods: debonding pliers (Shinye, Hangzhou, China) or enamel chisel (Jinzhong, Shanghai, China); the third group (n = 12) comprised of untreated controls, with normal enamel surface roughness. In each debonded group, three cleanup techniques (n = 6 each) were tested, including (I) diamond bur (TC11EF, MANI, Tochigi, Japan) and One-Gloss (Midi, Shofu, Kyoto, Japan), (II) a Super-Snap disk (Shofu, Kyoto, Japan), and (III) One-Gloss polisher. The debonding methods were compared using the modified adhesive remnant index (ARI, 1-5). Cleanup efficiencies were assessed by recording operating times. Enamel surfaces were qualitatively and quantitatively evaluated with scanning electron microscopy (SEM) and surface roughness tester, respectively. Two surface roughness variables were evaluated: Ra (average roughness) and Rz (10-point height of irregularities). Results: The ARI scores of debonded teeth were similar with debonding pliers and enamel chisel (Chi-square = 2.19, P > 0.05). There were significant differences between mean operating time in each group (F = 52.615, P < 0.01). The diamond bur + One-Gloss took the shortest operating time (37.92 ± 3.82 s), followed by the Super-Snap disk (56.67 ± 7.52 s), and the One-Gloss polisher (63.50 ± 6.99 s). SEM appearance provided by the One-Gloss polisher was the closest to the intact enamel surface, and surface roughness (Ra: 0.082 ± 0.046 μm; Rz: 0.499 ± 0.200 μm) was closest to the original enamel (Ra: 0.073 ± 0.048 μm; Rz: 0.438 ± 0.213 μm); the next best was the Super-Snap disk (Ra: 0.141 ± 0.073 μm; Rz: 1.156 ± 0.755 μm); then, the diamond bur + One-Gloss (Ra: 0.443 ± 0.172 μm; Rz: 2.202 ± 0.791 μm). Conclusions: Debonding pliers were safer than enamel chisels for removing brackets. Cleanup with One-Gloss polisher provided enamel surfaces closest to the intact enamel, but took more time, and Super-Snap disks provided acceptable enamel surfaces and efficiencies. The diamond bur was not suitable for removing adhesive remnant.

Abstract:Introduction: Emergence of superficial enamel roughness following orthodontic bracket debonding leads to the accumulation of microbial plaque, resulting in the development of dental caries and compromising the aesthetics and health of the tooth enamel. The objective of this current study is to investigate the Enamel Surface Roughness after Orthodontic Brackets Debonding and Surface Polishing with Restorative and Orthodontic Composites. Materials and Methods: In the present study, three types of composites were used for bonding metal orthodontic brackets. The samples were randomly divided into three cure groups: XT, GC, and SHOFU. Subsequently, the brackets were debonded, and the target surfaces were treated using a carbide bur for one minute by an operator to remove adhesive remnants and be prepared for bonding. The superficial enamel roughness was then examined. Furthermore, the samples were evaluated for the Adhesive Remnant Index before and after adhesive removal and enamel surface preparation. The data were analyzed using ANOVA test and SPSS software version 22. Results: The results indicated that there was no significant difference in surface roughness among the tested groups. However, a significant difference was observed between the surface roughness of the experimental groups and the healthy enamel surface (P 0.05). Conclusion: The results of the present study indicate a desirable and acceptable performance of the composite material. It is suggested that future studies focus on a comparative investigation of several cleaning and surface preparation systems.

Abstract:The purpose of this in vitro study was to evaluate the enamel surface structure subjected to various techniques of debonding orthodontic attachments and to develop a technique for residual adhesive removal that restores the enamel surface as closely as possible to its pretreatment condition without introducing iatrogenic damage. Enamel surface structure was examined with a scanning electron microscope before bonding of twin metal brackets to 60 previously extracted premolars with two heavily filled composite resins. Two groups, each consisting of 30 teeth, were equally subdivided into 10 subgroups. The first three subgroups were used to compare the efficacy of three bracket removing instruments. Since there were no differences in the debonding properties between the two resins, the two groups were combined. In this way, each subgroup (n = 6) could be used for a more meaningful comparison. On the basis of the results of this comparison, the bracket removing instrument that produced the most consistent separation at the bracket-adhesive interface was used in the remainder of the study. After appliance removal, the teeth were again examined microscopically and photographed, and seven different procedures for residual resin removal were compared. After resin removal, the final polished enamel surface was followed by microscopic evaluation. Results of this study show the bracket removing plier produced the most consistent separation at the bracket-adhesive interface, leaving the enamel surface intact. Carbide burs at high speed and air coolant proved to be efficient in residual resin removal, but when used alone, failed to produce a satisfactory enamel surface. After the removal of residual resin, graded medium, fine, and superfine Sof-Lex finishing disks (Unitek Corp., Monrovia, Calif.) produced surfaces that could be readily restored satisfactorily after receiving a final polish with a rubber cup and Zircate paste.
Performance of two laser motion modes versus conventional orthodontic ceramic brackets debonding technique on enamel surface topography

Abstract:The risk of enamel deterioration that frequently coexists with debonding of orthodontic teeth brackets elevates the mandate for finding an optimum approach for debonding them without harmful effects. This in-vitro study is intended to compare the effects of two different laser modes (scanning and circular) and a conventional method on the enamel surface after debonding orthodontic brackets. 66 extracted premolars were assigned into 3 groups. After that, light-cure composite resin was used to attach the ceramic brackets to the teeth. Amongst the test groups, Group I: specimens that were debonded using conventional debonding using pliers; Group 2: specimens that were debonded using Er, Cr: YSGG laser applications using the circular motion method; and Group 3: specimens that were debonded using Er, Cr: YSGG laser applications using the scanning motion method. Adhesive Remnant Index (ARI) assessment, intra-pulpal temperature increase, enamel surface roughness after polishing, and assessment of the microstructure of enamel were carried out with scanning electron microscopy. The gathered information was examined statistically. The conventional debonding method had a significantly higher proportion of adhesive remnant index (ARI) scores of 2 and 3 in comparison to the circular ( p < .004) and scanning laser groups ( p < .001). There was no significant difference in ARI scores between the circular and scanning laser groups ( p > .05). Moreover, the circular and scanning laser debonding methods resulted in a significantly higher proportion of Enamel Surface Roughness (ESR) scores of 0 and a lower proportion of ESR scores of 3 compared to the conventional technique group ( p < .001). However, there was no significant difference in ESR scores between the circular and scanning laser methods ( p = .945). Lastly, the average intra-pulpal temperature was significantly higher in the circular laser group (1.9 ± 0.5 ) compared to the scanning laser group (0.9 ± 0.2) with p < .001. Er, Cr: YSGG laser irradiation is a tool that shows promise for debonding ceramic brackets with minimal harm to the enamel surface. The scanning laser technique is more desirable due to the lower intra-pulpal temperature increase.

Effect of different polishing methods on roughness and color stability of air-abraded restorative materials after artificial accelerated aging

Abstract:Purpose: To evaluate the effect of air abrasion and polishing procedures on roughness and color stability of ceramic and composite materials after artificial accelerated aging. Methods: In this study, six restorative materials were tested: feldspathic ceramic (CEREC Blocks), glass ceramic (IPS e.max CAD), resin-based hybrid ceramic (Cerasmart), microhybrid composite (Charisma Classic), nanohybrid composite (Charisma Diamond) and nanoceramic composite (CeramXOne). Forty square-shaped composite specimens were fabricated from each composite and CAD-CAM ceramic material. Initial surface roughness measurements were performed using a profilometer and color measurements of each specimen with a spectrophotometer. Ten control specimens for each group did not receive air abrasion. The other specimens were treated by an air abrasion device and then were randomly divided into three subgroups of 10 specimens (n= 10). After air abrasion, 10 specimens of each group did not receive polishing (Air abrasion group) and others were repolished with Sof-Lex kit (Sof-Lex group) or a rubber kit (Rubber group). Surface roughness and color measurements were repeated before and after 300 hours of artificial accelerated aging (AAA). The univariate test and then three-way ANOVA and two-way ANOVA were performed for comparison of groups (α= 0.05). Results: The univariate statistical analysis revealed that the restorative materials were differently affected after air abrasion, polishing methods and AAA (P< 0.001). Three-way ANOVA showed that the surface roughness of the restorative materials increased after air abrasion and AAA (P< 0.001). Two-way ANOVA showed statistically significant differences between color changes of ceramic (CEREC and IPS e.max CAD) and composite based restorative materials (P< 0.001). Clinical significance: Clinicians should be aware that air abrasion at a specified power and time significantly changes the surface roughness of the materials except for CEREC. Additionally, polishing procedures (Sof-Lex, Rubber) did not significantly reduce the surface roughness of the ceramic groups. After air abrasion, depending on the material type used clinically, restorations should be repolished to reduce roughness and ensure color stability.
Surface preparation for orthodontic bonding to porcelain

Abstract:This study evaluated the effect of various porcelain surface treatments on the tensile strength of orthodontic brackets bonded to a feldspathic metal ceramic porcelain. The porcelain was fused to flat gold alloy tabs and divided into six groups that were subjected to sandblasting, silane application, intermediate resin, or etchants (9.6% hydrofluoric acid or 4% APF gels). Two brackets were bonded onto each porcelain/metal tab (n=60) with Bis-GMA resin (Concise, 3M Corp., St. Paul, Minn.) or 4-META resin (MCP-bond, Sun Medical Co. Ltd., Tokyo, Japan). The samples were stored in 37 degrees C water, thermocycled 1000 times from 5 degrees C to 55 degrees C and tested in tension. Alignment and uniform loading during testing were secured by engaging a hook in a circular ring soldered onto the bracket slot before bonding. Similar control brackets (n=12) were bonded with Concise to extracted caries-free mandibular incisors. Bond failure sites were classified according to a modified Adhesive Remnant Index (ARI) system. Silane application to the sandblasted porcelain surface significantly increased the bond strengths according to analysis of variance and Duncan's multiple range test. The quality of the bonds was further enhanced by the addition of the intermediate resin. Etching the porcelain with 9.6% hydrofluoric acid provided similar bond strengths, but the 4% APF gel was less effective. The MCP-bond was not significantly better than Concise in bond strength to sandblasted porcelain. Several difficulties associated with the clinical interpretation of laboratory data on bonding to dental porcelains are discussed, and clinical trials are necessary for final evidence of efficacy.
Effect of surface finish and resin cement on the bond strength to CAD-CAM ceramics for interim resin-bonded prostheses

Abstract:Statement of problem: Resin-bonded prostheses, including interim resin-bonded prostheses, are effective in preserving tooth structure compared with other types of fixed dental prostheses for the replacement of missing teeth. However, loss of retention remains a notable concern with these types of prostheses. Purpose: The purpose of this in vitro study was to investigate the influence of glass-ceramic type, resin type, and surface finish on the shear bond strength (SBS) to the CAD-CAM ceramics used to fabricate interim resin-bonded prostheses. Material and methods: Eighty 10×2-mm glass-ceramic disks were fabricated by using a diamond saw (IsoMet 1000), 40 from feldspathic porcelain blocks (Vita Mark II) and 40 from lithium disilicate blocks (IPS e.max CAD). Half of the specimens in each group were left with a dull or matte surface finish after cutting, while the other half were glazed with an add-on glaze (VitaAkzento Plus Glaze Spray and IPS e.max CAD Glaze Spray, respectively). The disks were mounted in acrylic resin, and each group was subdivided into 2, with 1 receiving a photopolymerized resin cement (RelyX Veneer) and the other receiving a flowable composite resin (Filtek Supreme Ultimate Flow) to form 2.38×2-mm cylinders. SBS was determined using a universal testing machine (Instron 4411) in accordance with the International Organization for Standardization (ISO) 29022:2013 standard, and failure modes were analyzed by using a stereomicroscope with ×40 magnification. The data were analyzed with a 3-way analysis of variance and Tukey post hoc analysis. The chi-squared test was used to analyze the failure mode (α=.05 for all tests). Results: Ceramic type, resin type, and surface finish significantly impacted SBS (P<.001, P=.003, P<.001, respectively). Lithium disilicate showed higher SBS than feldspathic porcelain, and flowable composite resin exhibited higher SBS than resin cement. Glazed surfaces displayed lower SBS compared with the dull or matte surfaces. The combinations among the 3 materials also impacted SBS (P=.03). In addition, the combinations between ceramic type and surface finish affected SBS (P<.001), regardless of resin cement type. No other combinations affected the SBS (P>.05). The mode of failure was different among the groups (P<.001). In comparison with all other groups, cohesive failures were most prevalent in feldspathic porcelain with a dull or matte surface finish, regardless of the resin type used. Conclusions: The SBS to glass-ceramics was influenced by ceramic material, resin cement type, and surface finish. Flowable composite resin showed higher SBS than resin cement. A dull or matte surface finish exhibited greater bond strength than a glazed surface. Lithium disilicate had higher SBS than feldspathic porcelain.

Abstract:ObjectivesTo investigate the surface roughness of CAD/CAM materials immediately after milling and after different chairside and labside polishing procedures. A two-body wear test was performed to compare the different wear characteristics of the materials and the corresponding antagonists.Materials and methodsSpecimens (n = 12 per series) from different CAD/CAM materials (three composites: Lava Ultimate, Cerasmart, BRILLIANT Crios; one hybrid ceramic: VITA Enamic; three ceramics: Celtra Duo, VITA Suprinity, IPS Emax.CAD) were polished according to the manufacturer’s instructions. The effect of different polishing procedures was investigated by comparing surface roughness (Ra, Rmax) after labside polishing and after chairside polishing. Wear behavior (mean, volume, and maximum wear) of specimens and antagonists as well as changes in surface roughness were determined in a pin-on-block wear test. Statistical analysis was performed with a one-way analysis of variance (ANOVA)/Bonferroni multiple-comparison post hoc test and a multifactorial ANOVA/Tukey’s significant difference post hoc test (α = 0.05). SEM micrographs were used for the qualitative evaluation of surfaces and wear traces.ResultsAfter chairside high-gloss polishing, ceramics and composites exhibited Ra values between 0.08 and 0.10 μm and between 0.11 and 0.13 μm, respectively. After labside high-gloss polishing, values varied between 0.02 and 0.09 μm for ceramics and between 0.06 and 0.16 μm for resin composites. No significant differences were found between labside and chairside pre- and high-gloss polishing. For the ceramics, lower mean wear depths (between − 132.2 ± 19.9 and − 137.0 ± 19.0 μm) were identified compared to the resin composites (which exhibited wear depths between − 159.1 ± 19.4 and − 176.3 ± 23.9 μm). For maximum wear depth and volume, a different ranking of the materials was found. Antagonistic wear varied between 12.0 ± 6.4% and 30.6 ± 9.9% and was higher for the ceramic materials and Lava Ultimate. For all materials, a smoothing between 0.20 and 2.70 μm (Ra) was identified after wear simulation.ConclusionsChairside polishing is as effective as labside polishing, although surfaces were directly adjusted (roughened) only before the chairside polishing. Wear was lowest for ceramics, followed by the resin-infiltrated material and the resin composites.Clinical relevancePolishing after milling or adjustment is essential to guaranteeing optimal clinical performance. Chairside polishing after adjustment leads to comparably smooth surfaces as labside polishing after milling and grinding. Ceramics are expected to exhibit lower wear than resin composites under clinical conditions.

Abstract:Objectives. To evaluate and compare the impacts, bond strength, residual adhesive, and time invested on the enamel surface after debonding of recently introduced ceramic buccal molar tubes with different systems. Materials and Methods. Ceramic molar tubes were bonded to fifty-four maxillary molar teeth, and a shear bond strength (SBS) test was performed. The adhesive remnant index (ARI) scores were recorded, and the samples were divided into two groups for adhesive removal with low-speed instruments: tungsten carbide bur or diamond-coated micropolisher point. The time to clean the enamel surfaces was also noted down for each tooth. The enamel surfaces were investigated with scanning electron microscope (SEM) after adhesives were cleaned. Shapiro-Wilk's, Kolmogorov-Smirnov's, and Student's independent tests were used for statistical analysis. Results. The mean SBS value of the tested ceramic molar tubes was ‚ÄČMPa, and the majority of the samples were scored as ARI 1 and ARI 2. No statistically significant difference between PoGo micropolisher and TCB was found in terms of time values for surface cleaning. The enamel surface characteristics of TCB for adhesive remnant removal resulted in a better enamel surface than the single-step diamond polisher when the samples were investigated by using SEM. Conclusions. Ceramic molar tubes may be an enamel-safe product for patients seeking for fully aesthetic orthodontic treatment, if used in carefully handled clinical conditions. One-step polishing systems utilised with low-speed instruments could be used confidentially for cleaning the resin remnants on enamel after orthodontic treatment.

Abstract:Objective This article evaluates the effect of multipurpose polishing kit on surface roughness and hardness of three computer-aided design/computer-aided manufacturing (CAD/CAM) ceramic materials at different polishing durations. Weight changes of the polishing bur were also determined. Material and Methods Three CAD/CAM ceramic materials were lithium disilicate glass ceramic (IPS e.max CAD), translucent zirconia (VITA YZ), and zirconia-reinforced lithium disilicate ceramic (Celtra Duo). Ceramics were ground with a diamond bur, and polished with the multipurpose polishing kit (Eve Diacera HP), which comprises coarse and fine polishing burs. Surface roughness value (Ra) was measured using a noncontact optical profilometer ( n = 10 per group) after grinding and every 15 seconds of coarse and fine polishing until 60 seconds. The complete polishing Ra was compared with the lab as-received specimens and human enamel. Surface morphology was examined using a scanning electron microscope after 60-second coarse and fine polishing and compared with the lab as-received specimens. Hardness was measured using a Vickers hardness tester on the lab as-received specimens and after the final polishing process ( n = 4 per group). Changes in surface roughness and polishing bur weight of each material were analyzed using one-way repeated-measures analysis of variance (ANOVA) and dependent t -test. One-way ANOVA was used to detect differences in surface roughness, Vickers hardness, and bur weight among materials within the same polishing duration ( α = 0.05). Results From grinding to complete polishing, the greatest Ra reduction was found in VITA YZ, followed by Celtra Duo and IPS e.max CAD. Final Ra values of all ceramics after 60-second fine polishing were not significantly different, and were similar to that of enamel and lab as-received specimens. Vickers hardness of ceramic materials did not change after grinding and polishing. Coarse polishing bur demonstrated the highest weight loss after polishing VITA YZ, followed by Celtra Duo and IPS e.max CAD. Conclusion The multipurpose polishing kit reduced surface roughness of CAD/CAM ceramic materials to the similar level of the lab as-received specimen and enamel regardless of material's hardness. The reductions of surface roughness and a coarse polishing bur weight were highest in VITA YZ, followed by Celtra Duo and IPS e.max CAD. Noninvasive Evaluation of Breathing Pattern and Thoraco—Abdominal Motion Following the Infusion of Ketamine or Droperidol in Humans

Abstract:The authors compared the respiratory effects of an intravenous infusion of ketamine (1 mg · kg-1) with droperidol (0.1 mg · kg-1), or placebo on three different occasions in a double-blind, randomized fashion in eight healthy volunteers. Breathing pattern, thoraco–abdominal motion, end-expiratory positions of the rib cage and abdomen, arterial hemoglobin oxygen saturation (SaO2), and end-tidal carbon dioxide concentration (FECO2) were continuously measured with noninvasive techniques. During the 1-h monitoring period following drug injection, droperidol produced occasionally significant but clinically unimportant differences in respiratory variables when compared with placebo. In contrast, ketamine induced a significant (P < 0.001) and persistent increase in minute ventilation (+75%) from 5 to 20 min after start of infusion by increasing both the driving (i.e., tidal volume/inspiratory time [VT/T]) and the timing (i.e., in-spiratory time/total respiratory cycle time [T/T]) components of ventilation (Milic-Emili J, Grunstein MM: Chest 70 (Suppl): 131–133, 1976). This was obtained without any significant change in end-expiratory positions or change in relative rib cage contribution to tidal volume. Despite multiple apneic episodes observed with ketamine, the subjects maintained a stable SaO2 and FECO2, indicating no resting respiratory depression. This study, performed with a noninvasive respiratory monitoring technique, confirms that droperidol infused over 5 min at a clinically used dosage does not cause respiratory depression in healthy subjects, whereas ketamine produces an important ventilatory stimulation.

Abstract:The case of acute poisoning reported here raises the question of the harmlessness of preparations destined for cutaneous application in infants. After describing the method perfected for the identification and estimation of camphor, menthol and thymol in biological material, the authors present the toxicological, clinical and biological arguments in favour of the notion that the cutaneous resorption of these substances was responsible for this acute intoxication in an infant. Particular attention should be paid to poisoning which may result, in the newborn and infant, from the cutaneous application of active substances.

Abstract:Objective: To assess the color stability and translucency of full cubic stabilized zirconia (FSZ) following orthodontic bonding with different surface treatments and coffee thermocycling (CTC).Methods: This in vitro study was conducted on 120 disc-shaped specimens of FSZ. Thirty specimens were selected as the control group and remained intact. The remaining specimens were randomly divided into three groups based on the type of surface treatment (n = 30): airborne particle abrasion (APA), silica-coating (CoJet), and carbon dioxide (CO2) laser. After metal bracket bonding in the test groups, debonding and polishing were performed. Subsequently, all specimens underwent CTC (10,000 cycles). Color parameters, color difference (ΔE00), and translucency parameter (TP) were measured three times at baseline (t0), after debonding and polishing (t1), and after CTC (t2). Data were statistically analyzed (α = 0.05).Results: Significant difference existed among the groups regarding ΔE00t0t2 (p < 0.001). The APA group showed minimum (ΔE00 = 1.15 ± 0.53) and the control group showed maximum (ΔE00 = 0.19 ± 0.02) color stability, with no significant difference between the laser and CoJet groups (p = 0.511). The four groups were significantly different regarding ΔTPt0t2 (p < 0.001). Maximal increases in TP were noted in the CoJet (1.00 ± 0.18) and APA (1.04 ± 0.38) groups while minimal increase was recorded in the control group (0.1 ± 0.02).Conclusions: Orthodontic treatment makes zirconia restorations susceptible to discoloration and increased translucency. Nonetheless, the recorded ΔE00 and ΔTP did not exceed the acceptability threshold.

Abstract:Abstract Objectives Due to the high demand for all-ceramic restorations, monolithic zirconia restorations are nowadays frequently used. With the demand for adult orthodontic treatments, orthodontists need to be mindful of the quality of their brackets bonding to this type of material, as it requires special conditioning. This study aimed to compare different surface treatments of zirconia when bonding metal or ceramic orthodontic brackets. The objectives are to compare the shear bond strength; the amount of adhesive remaining on the surface of the material; the incidence of adhesive, cohesive, and mixed failures; and the occurrence of zirconia fractures. Materials and Methods Forty monolithic blocks of zirconia of a diameter of 10 mm and a length of 10 mm were prepared and randomly divided into two groups (n = 20): metallic or ceramic brackets. Each group was subsequently divided into two subgroups (n = 10) depending on the surface preparation (laser treatment or airborne particle abrasion): SMB (airborne particle abrasion, metal bracket), SCB (airborne particle abrasion, ceramic bracket), LMB (laser; metal bracket), and LCB (laser, ceramic bracket). The samples were tested for shear bond strength using a universal testing machine. The adhesive remnant index and the occurrence of zirconia fractures and different types of failures were assessed by optical and electron microscopy. Statistical Analysis Results were analyzed using analysis of variance. Results The differences were significant between the metallic (SMB, LMB) and ceramic (SCB, LCB) bracket groups with regard to shear bond strength, with respectively 23.29 ± 5.34 MPa, 21.59 ± 4.03 MPa, 20.06 ± 4.05 MPa, and 17.55 ± 3.88 MPa. In terms of surface treatment, no statistical differences were found between the different groups. Conclusion Metal brackets have a greater bond strength than ceramic brackets when cemented to zirconia. The surface treatment of zirconia surface has no influence on the shear bond strength.

Abstract:The surface roughness (Ra) of indirect computer-aided design/computer-aided manufacturing (CAD/CAM)-fabricated dental restorations is crucial for their long-term durability. This study intended to evaluate the Ra of five different types of contemporary indirect CAD/CAM restorative materials with varying compositions that were glazed and finished/polished. A total of 75 specimens, disc-shaped (10 mm × 2 mm), were obtained from five materials (n = 15) (Tetric CAD, IPS e.max CAD, IPS e.max ZirCAD, CELTRA Duo, and Vita Enamic) and fabricated by CAD/CAM. One of the two surfaces for each specimen was subjected to glazing, while the other surface was subjected to finishing/polishing. The Ra of the two surfaces in micrometers (μm) was evaluated using a Profilometer, while the surface topography was examined using a scanning electron microscope. Using SPSS, the Kruskal–Wallis, post hoc Conover, and Mann–Whitney tests were used to statistically evaluate the data. A comparison of the Ra for the finished/polished surfaces of the five test materials showed significant differences (p < 0.0001). Among the finished/polished surfaces, the mean rank values of Vita Enamic were significantly higher than the other four test materials (p < 0.0001). A comparison of the Ra of glazed surfaces among the five study materials revealed significant differences (p < 0.0001). The Ra for the IPS e.max ZirCAD material was significantly higher than the rest of the four materials (p < 0.001). A comparison of the Ra for two types of surface conditioning within each of the five test materials showed a significant difference (p < 0.05). Only for IPS e.max ZirCAD was the Ra of the glazed surface significantly higher than the finished/polished surface (p < 0.0001). Significant variations in the surface roughness (Ra) were exhibited between the finished/polished and glazed surfaces of the five test materials. Hybrid ceramics showed the highest Ra values for the finished/polished surfaces, and zirconia exhibited the highest Ra values among the glazed surfaces among the tested materials. The Ra values of either finished/polished or glazed surfaces of the test materials were within the clinically acceptable range (0.2–0.5 μm), except for the glazed surface of the zirconia ceramics (0.84 μm).
