Abstract:As absorbable hemostatic dressings, chitosan with a deacetylation degree of 40% (CS-40) and 73% (CS-73) have been fabricated into sponges via a modified method. The hemostatic, biocompatible and biodegradable properties were evaluated through in vivo assays. In a hepatic hemorrhage model, the chitosan sponges, with excellent blood compatibility, achieved less blood loss than the gelation sponge (GS). In addition, CS-40 showed better hemostatic capability and biodegradability than CS-73. After implantation, a histological analysis indicated that CS-40 exhibited the best biodegradability, tissue regeneration and least tissue adhesion. By contrasting CS-40 and CS-73, the deacetylation degree is confirmed to be a key factor for the hemostatic effect, biodegradability, biocompatibility and tissue regeneration. Our overall results demonstrated the potential application of CS-40 for use in absorbable hemostatic dressings.
A Chitosan Hydrogel Sealant with Self-Contractile Characteristic: from Rapid and Long-Term Hemorrhage Control to Wound Closure and Repair.

Abstract:Emergent and long-term hemorrhage control is requisite and beneficial for reducing global mortality and postoperative complications (e.g., second bleeding and adverse tissue adhesion). Despite recent advance in injectable hydrogels for hemostasis, achieving rapid gelation, strong tissue-adhesive property and stable mechanical strength under fluid physiological environment is still challenging. Herein, we developed a novel chitosan hydrogel (CCS@gel) via dynamic Schiff base reaction and mussel-inspired catechol chemistry. The hydrogel possessed high gelation rate (<10 s), strong wet adhesiveness, excellent self-healing performance and biocompatibility. More importantly, the CCS@gel exhibited saline-induced contractile performance and mechanical enhancement, promoting its mechanical property in moist internal conditions. In vivo studies demonstrated its superior hemostatic efficacy for diverse anticoagulated visceral and carotid bleeding scenarios, compared to commercialized fibrin glue. The hydrogel-treated rats survived for 8 weeks with minimal inflammation and postoperative adhesion. These results revealed that the promising CCS@gel would be a facile, efficient and safe sealant for clinical hemorrhage control.

Abstract:Excessive blood loss caused by natural accidents or major surgical procedures can be life-threatening and requires rapid hemorrhage management. Developing hemostatic materials with high water absorption capacity and high strength properties is still challenging. Super hydrophilic/high-strength chitosan hemostatic sponges (SHHS-CHS) were prepared by freeze-drying and alkali treatment. The freeze-drying technology formed a porous three-dimensional structure of sponges, and alkali treatment increased the hydrophilicity of sponges and the rigidity of the chitosan molecular chain, improving the hydrophilicity and mechanical properties of chitosan sponges. Characterization experiments revealed that SHHS-CHS had high water absorption capacity, rich pore structure, and excellent mechanical properties. The cytotoxicity and hemolysis experiments showed that SHHS-CHS had good biocompatibility. Compared to the commercial hemostatic gelatin sponges and chitosan hemostatic material, SHHS-CHS had better blood coagulation and red blood cell and platelet adhesion ability. Moreover, SHHS-CHS showed better hemostatic ability than gelatin sponges in rat-liver injury and rat leg vein rupture models, significantly reducing the hemostasis time and bleeding volume. In conclusion, SHHS-CHS has broad prospects for clinical application as a hemostatic material.

Abstract:Procoagulant, antibacterial and analgesic hemostatic hydrogel dressing with high wet tissue adhesion, ultra-high burst pressure, and easy preparation shows huge promising for rapid hemostasis in emergencies, yet it remains a challenge. Herein, we propose hemostatic microsheets based on quaternized chitosan-g-gallic acid (QCS-GA) and oxidized hyaluronic acid (OHA), which merge the benefits of sponges, hydrogels, and powders for rapid hemostasis and efficient wound healing. Specifically, they exhibit a large specific surface area and excellent hydrophilicity, rapidly absorbing blood and self-gelling through electrostatic interaction and Schiff base crosslinking. And this results in dense, porous hydrogel adhesives with superior mechanical properties, adhesion strength, and ultra-high burst pressure. Furthermore, the microsheets are biocompatible, biodegradable, and possess procoagulant, antibacterial, and antioxidant properties. In mouse and rat liver hemorrhage models, the optimized formulation (QCS-GA + OHA4) demonstrated superior hemostatic effects compared to Celox. In particular, QCS-GA + OHA4 microsheets could stop bleeding quickly from rat femoral artery transection and deliver lidocaine to provide analgesia during emergency treatment. Additionally, they promoted wound healing in mouse full-thickness skin defect wound. These easy-to-manufacture hemostatic microsheets are adaptable to irregular wounds, providing a novel solution for rapid hemostasis and wound healing.

Silk Fibroin-Based Tough Hydrogels with Strong Underwater Adhesion for Fast Hemostasis and Wound Sealing

Abstract:Rapid and strong adhesion of hydrogel adhesives is required for instant wound closure and hemostasis. However, in situ hydrogel formation and sufficient adhesion at target tissue sites in biological environments are severely compromised by the presence of blood and body fluids. In this work, an underwater adhesive hydrogel (named SHCa) is fabricated with rapid in situ gelation, enhanced mechanical toughness, and robust underwater adhesion. The SHCa can undergo rapid UV irradiation-induced gelation under water within 5 s and adhere firmly to underwater surfaces for 6 months. The synergistic effects of crystalline β-sheet structures and dynamic energy-dissipating mechanisms enhance the mechanical toughness and cohesion, supporting the balance between adhesion and cohesion in wet environments. Importantly, the SHCa can achieve rapid in situ gelation and robust underwater adhesion at various tissue surfaces in highly dynamic fluid environments, substantially outperforming the commercially available tissue adhesives. The lap shear adhesion strength and wound closure strength of SHCa on blood-covered substrates are 7.24 and 12.68 times higher than those of cyanoacrylate glue, respectively. Its fast hemostasis and wound sealing performance are further demonstrated in in vivo animal models. The proposed hydrogel with strong underwater adhesion provides an effective tool for fast wound closure and hemostasis.
A Novel Chitosan-Based Sponge Coated with Self-Assembled Thrombin/tannic Acid Multilayer Films As a Hemostatic Dressing

Abstract:In order to prepare a novel hemostatic dressing for uncontrolled hemorrhage, a porous chitosan sponge was coated with self-assembled (thrombin/tannic acid) n films, which were based on hydrogen bonding interactions between thrombin and tannic acid at physiologic pH. According to the whole blood clotting test, the coated chitosan sponges showed a significantly high rate of blood clotting due to the addition of thrombin. On the other hand, the storable half-life of immobilized thrombin is extended to 66.9 days at room temperature, which is 8.5 times longer than unfixed thrombin. It is because of the immobilization effect of, not only the porous structure of chitosan sponge but also the interactions between thrombin and tannic acid. In addition, the tannic acid has similar antibacterial effect to chitosan. Therefore, it is an excellent combination of chitosan, thrombin and tannic acid. Besides, all of materials in this research have been approved by the United States Food and Drug Administration (FDA). So the chitosan-based sponge is a promising candidate dressing for uncontrolled hemorrhage due to its storable, bio-safe and highly effective hemostatic properties.
Injectable hemostatic hydrogel adhesive with antioxidant, antibacterial and procoagulant properties for hemorrhage wound management

Abstract:In emergencies, uncontrolled severe bleeding can result in undesired complications and even death of the injured. Designing advanced hemostatic agents is a potential solution for emergency hemostasis, yet it remains challenging to realize the persistent adhesion in a wet wound environment. In this study, based on dynamic reversible Schiff base bond and photo-initiated double-bond polymerization, a novel injectable hemostatic hydrogel (L-COC) consisting of methacrylated carboxymethyl chitosan (CMCSMA), oxidized konjac glucomannan (OKGM) and (+)-catechin hydrate (CH) was synthesized for emergency hemostasis. To our delight, the incorporated CH imparted enhanced blood procoagulantion to the L-COC hydrogel by intensifying the hydrogel-red blood cell interactions. As a result, the hemostatic effect of the engineered L-COC hydrogel was significantly superior to that of fluid gelatin SurgifloTM for liver bleeding wounds in rats (Blood loss: 0.62 ± 0.11 g (L-COC), 0.90 ± 0.08 g (SurgifloTM); hemostasis time: 69.0 ± 2.9 s (L-COC), 84.0 ± 2.2 s (SurgifloTM)). With the favorable antioxidant and antibacterial activities, as well as multifunctional properties, the bio-adhesive L-COC hydrogel and the underlying design principles may facilitate further development of practical hemostatic hydrogels.

Abstract:Hydrogels with injectability, self-healing ability and adhesiveness have great potential for hemostasis and full-thickness skin wound repair, which are usually fabricated by multistep chemical synthesis and the use of organic solvents and catalyst. Herein, we report an injectable and self-healing hydrogel facilely prepared through one-pot heating of chitosan and graphene oxide mixture solution, without any pollutant and waste generated. The dynamic reversible breakage and recombination of noncovalent bonds between chitosan and graphene oxide endows the hydrogel injectability and self-healing ability. In addition, the mechanical and rheological properties of the hydrogels can be controlled by varying the dosage of graphene oxide. Meanwhile, hydrogels exhibited good adhesiveness and hemocompatibility. Finally, in vivo experiments in a rat liver bleeding model and full-thickness skin defect model verified the outstanding hemostatic and wound healing capability of the hydrogels, indicating the promising future for use as wound dressing.
Corn stalk modified chitin composite sponge for effective hemostasis and promoting wound healing

Abstract:The hemorrhage in daily life was a great challenge for the life health. Before hospitalization and infection, stopping traumatic bleeding timely is an important measure to decrease the death threat. The high crystallinity and low porous structure of chitin (CH) make texture of sole CH sponge not soft enough, which limit its hemostatic properties. In this work, loose corn stalk (CS) was used to modify the structures and properties of sole CH sponge. The novel hemostatic composite sponge of CH/CS4 was prepared by cross-linking and freeze-drying process of chitin and corn stalk suspension. The composite sponge obtained best physical and hemostatic properties at the 1:1 volume ratio of chitin and corn stalk. Thanks to the porous structures, CH/CS4 possessed high water/blood absorption ability (34 ± 2 g/g and 32.7 ± 2 g/g), rapid hemostatic time (31 s) and low blood loss (0.31 g), allowing it to be delivered into the wound bleeding sites to reduce the wound bleeding by robust physical barrier and pressure effect. Furthermore, CH/CS4 displayed excellent hemostatic performance than sole CH and commercial polyvinyl fluoride sponge (PVF). Moreover, CH/CS4 displayed superior wound healing ability and cytocompatibility. Therefore, the CH/CS4 has high potential application in medical hemostatic field.

Abstract:Releasing the biomedical application potential of chitosan has become an emerging research hotspot for effective hemostatic agents. However, designing a flexible chitosan-based hemostatic agent with self-adhesion feature in humid environments and rapid hemostasis performance is still a persistent challenge. Herein, a self-adhesive hydrogel (DCS-PEGSH gel) with a typical multi-level pore structure cross-linked by 3-(3,4-Dihydroxyphenyl) propionic acid modified chitosan (DCS) and sebacic acid-terminated polyethylene glycol-modified by p-hydroxybenzaldehyde (PEGSH) was successfully synthesized. The bio-hydrogel displays favorable cytocompatibility, suitable stretchability (~780%), and water absorbability (1300% ± 50%), which can be precisely regulated by the proportion of PEGSH polymer in the gel network. The strong adhesion (68.5 kPa) allows long-term stability of the assembled hydrogel on pigskin in both static and dynamic humid environments, as well as firm adherence to the bleeding wound without shedding. The Rat hemorrhage model was established to certificate that the hydrogel has lower blood clotting time (50 s) and blood clotting index (BCI, 41) compared to a commercial chitosan sponge (288 s, BCI 65). Notably, the amount of blood loss from liver in rats is reduced almost 90% compared with the control group. This study provides a novel prospect including both molecular design and structural regulation for developing chitosan-based hydrogel with self-adhesive, self-healing, stretchability, biocompatibility, antibacterial ability, and antioxidant properties for further biomedical application regarding emergency hemostasis, particularly in joints and extremities.

Abstract:Benefiting from the biocompatibility, adhesiveness, and natural extracellular matrix-mimicking ability, hydrogels have received increasing research in recent years. In this study, a hydrogel system composed of dopamine, quaternized ammoniated chitosan (QCS), and polyvinylpyrrolidone was reported to exhibit fast hemostatic properties in Sprague-Dawley rat tail amputation and liver bleeding models. The results showed that this hydrogel had good hemostatic properties. The designed hydrogel showed high swelling ratios in H 2 O, PBS, and 0.9% NaCl solution, indicating its capability to absorb tissue residual exudate and form a stable hydrogel. Compared with the control group, the blood loss in Sprague-Dawley rat tail amputation and liver bleeding were reduced by nearly 78% and 76%, respectively. Interestingly, dopamine endowed the hydrogel with antioxidant properties, thus holding a great application promise in inflammatory wounds. Furthermore, the designed hydrogel demonstrated good and reversible adhesion properties (12.23 ± 0.22 kPa–24.31 ± 0.55 kPa), ensuring its firm attachment to bleeding wounds of pig skin in wet environments. This research points out a novel path for designing chitosan-based hydrogels for biomedical applications.

Blood-responsive mussel-inspired hydrogels for hemostasis, antibacterial action, and wound healing

Abstract:Rapid hemostasis, potent antimicrobial activity, and efficient wound management are critical factors in enhancing the survival of trauma patients. Chitosan, as a green and sustainable biomaterial with low cost, degradability and biocompatibility, is widely used in the biomedical field. However, chitosan dissolves in an acidic environment, which is not conducive to wound healing. In this study, chitosan was chemically modified to address this limitation. A mussel-inspired hydrogel composed of caffeic acid-grafted chitosan, gallic acid-grafted chitosan, and oxidized microcrystalline cellulose (CHI-C/CSG/OMCC) was designed. This hydrogel exhibits blood-responsive gelation behavior and offers a synergistic combination of tissue adhesion, antimicrobial properties, and tissue repair capabilities. The carboxyl, hydroxyl, phenolic hydroxyl and aldehyde groups within the hydrogel system endowed the hydrogel with excellent adhesion properties (53.1 kPa adhesion strength to porcine skin-adherent tissues), biocompatibility, and excellent antimicrobial properties. Surprisingly, this hydrogel not only achieved rapid and effective hemostasis, but also effectively promoted wound healing in a mouse skin injury model. In addition, its remarkable efficacy in stopping bleeding within approximately 2 min without rebleeding was demonstrated in a porcine model of acute gastrointestinal hemorrhage in the esophagus, stomach, and intestines. This blood-responsive ternary hydrogel offers a promising alternative to wound management materials due to its excellent overall performance and superior efficacy in all phases of wound healing.
Fabrication of Hydroxypropyl Chitosan/Soy Protein Isolate Hydrogel for Effective Hemorrhage Control

Abstract:Hemostatic materials are increasingly important in civilian and military clinics. In this work, a hydrogel was fabricated from hydroxypropyl chitosan (HPCS) and soy protein isolate (SPI) through the crosslinking of epichlorohydrin. Effects of SPI content on the structure, and physical and biological properties of the prepared hydrogels were characterized using Fourier-transform infrared spectroscopy, X-ray diffractometry, scanning electron microscopy, water uptake testing, mechanical properties testing, MTT assay, hemolysis ratio testing, and routine blood coagulation test. The results indicated that the hydrogels showed high water uptake ability and compressive strength. The in vitro biocompatibility evaluation revealed that the hydrogel contains 30% SPI content (HCSH-30), could promote blood coagulation and cell proliferation. Furthermore, the hemostatic model of liver in New Zealand rabbit was applied to assess the hemostatic efficacy of the hydrogels. The results demonstrated that HCSH-30 stopped bleeding in 75 +/- 1.63 s and improved hemostasis as compared with medical gauze. Thus, the HPCS/SPI hydrogel is expected to be a potential candidate for effective hemorrhage control. Impact statement Stoppage of bleeding is of paramount clinical significance in prophylactic, surgical, and emergency scenarios. Abstract:Uncontrolled non-compressible hemorrhage, which is often accompanied by coagulopathy, is a major cause of mortality following traumatic injuries in civilian and military populations. In this study, coagulopathy-independent injectable catechol-modified chitosan (CS-HCA) hemostatic materials featuring rapid shape recovery were fabricated by combining controlled sodium tripolyphosphate-crosslinking with hydrocaffeic acid (HCA) grafting. CS-HCA exhibited robust mechanical strength and rapid blood-triggered shape recovery. Furthermore, CS-HCA demonstrated superior blood-clotting ability, enhanced blood cell adhesion and activation, and greater protein adsorption than commercial hemostatic gauze and Celox. CS-HCA showed enhanced procoagulant and hemostatic capacities in a lethal liver-perforation wound model in rabbits, particularly in heparinized rabbits. CS-HCA is suitable for mass manufacturing and shows promise as a clinically translatable hemostat.
Facile and green approach towards biomass-derived hydrogel powders with hierarchical micro-nanostructures for ultrafast hemostasis.

Abstract:Although a series of biomass-derived hemostats has been developed, the desire for green-prepared hemostatic materials with biosafety has not decreased. Herein, we constructed porous carboxymethyl chitosan/sodium alginate/Ca(OH)2 powders (PCSCPs) with suitable adaptability for instant control of irregular hemorrhage via a facile and green approach. By one-pot chemical crosslinking of carboxymethyl chitosan and sodium alginate, hydrogels were formed and immediately ionically cross-linked along with the generation of Ca(OH)2 to prepare PCSCPs. As hydrogel powders, PCSCPs with abundant hydrophilic carboxymethyl groups and porous hierarchically micro-nanostructures displayed a high water absorption ratio of over 1600%. The PCSCPs were confirmed with favorable hemocompatibility, non-cytotoxic effects and excellent degradability. Hemostasis assays in vitro showed that PCSCPs possessed an outstanding property of platelet activation and red blood cell aggregation. The PCSCPs effectively shortened the hemostatic time and blood loss to ca. 50% in rodent bleeding models compared with medical gauze and commercial chitosan-based hemostats. Furthermore, a mouse subcutaneous implantation model demonstrated an ignorable inflammation response and potential tissue repair capability of PCSCPs. It's believed that green-prepared and biomass-derived PCSCPs are feasible biomedical hemostatic materials in view of engineering and provide a promising platform to design hemostats in prehospital management and clinical settings.

Abstract:Chitosan, as a natural polysaccharide, is widely used in the field of hemostasis. However, its inherent limitations such as poor mechanical strength and difficulty in dissolving under alkaline conditions restrict the broader application. This study designed a glycidyl methacrylate-modified carboxymethyl chitosan (M-CMCS) photo-crosslinked hydrogel, incorporating metal-polyphenol network (MPN) microcapsules. The integration of these microcapsules functionalized the hydrogel, enhancing its hemostatic and mechanical properties while imparting anti-inflammatory and antioxidant capabilities. The hydrogel demonstrated excellent biocompatibility and hemostatic efficacy, promoting the formation of stable blood clots, and had significant potential as a clinically relevant hemostatic material.

Abstract:Hydrogel is a kind of hemostatic agent with good application prospect. However, the water molecules on the wound made the hydrogel less adhesive to wet wound tissue. Herein, the carboxymethyl chitosan (CMCS)/oxidized dextran (OD)/γ-polyglutamic acid (γ-PGA) hydrogel was prepared using a double-barreled syringe for hemostasis of diffuse and incompressible wound bleeding. The hydrogel formation was based on the intramolecular lactam bonds, intermolecular amide bonds, and Schiff base bonds. In the hydrogel, the super hydrophilic γ-PGA could drain the surface moisture of the wound and create a local dry environment for enhanced surface adhesion. In vivo study showed that the CMCS/ODex/γ-PGA hydrogel possessed a good biosafety and biodegradability. Interestingly, the CMCS/ODex/γ-PGA hydrogel possesses excellent hemostatic function in dynamic humid environment and resists a high blood pressure of 238 mmHg, which exceeds the threshold systolic blood pressure of healthy adults ( i.e. , 120 mmHg). Together with the antibacterial and reactive nitrogen species scavenging activity, this study is expected to provide a new method to design the wet-surface adhesives for the efficient hemostatic application.

Abstract:Stanching severe traumatic bleeding requires hemostatic materials that can provide rapid and efficient hemostasis. However, most currently available hemostatic agents exhibit low efficiency and poor biocompatibility. In this study, a novel carboxymethyl chitosan modified with methacrylic anhydride (double-bonded carboxymethyl chitosan)/cysteamine-modified chondroitin sulfate (Me-CMC/CSS) rapid hemostatic dressing was developed using thiol-ene click chemistry. The freeze-dried Me-CMC/CSS dressings absorbed water rapidly and transformed into soft hydrogels that could seal wounds, achieving good hemostasis. The relative blood clotting index of the optimal Me-CMC/CSS dressing was decreased by 53% (p < 0.001) and 29% compared with a pure Me-CMC dressing and a commercial hemostatic gelatin sponge, respectively. In a rat liver hemorrhage model, the MeCMC/CSS dressing stopped bleeding rapidly, and the total blood loss was significantly decreased by 92% (p < 0.0001) compared with the untreated group and was lower than that with gelatin sponge treatment. The MeCMC/CSS dressing also exhibited a higher ability to adhere to platelets than the gelatin sponge. Furthermore, the Me-CMC/CSS dressing exhibited good hemocompatibility and cytocompatibility. The results indicate that the composite dressings can be potentially applied as rapid hemostatic materials.

Abstract:Hydrophilic hemostatic sponge plays an important role in trauma bleeding control because of its robust coagulant functions. However, its strong tissue adhesion can easily result in wound tear and rebleeding during removing the sponge. Herein, the design of a hydrophilic anti-adhesive chitosan/graphene oxide composite sponge (CSAG) that possesses stable mechanical strength, rapid liquid absorption and strong intrinsic/extrinsic coagulation stimulations, is reported. For one thing, CSAG exhibits outstanding hemostatic performance, which significantly outperforms two commercial hemostats in two in vivo serious bleeding models. For another, CSAG shows low tissue adhesion; its peeling force is approximately 79.3 % lower than the commercial gauze. Moreover, in the peeling process, CSAG triggers partial detachment of the blood scab, because of the exist of bubbles or cavities at the interface, allowing the CSAG to be easily and safely peeled off from the wound without rebleeding. This study opens new avenues in constructing anti-adhesive trauma hemostatic materials.

Abstract:Abstract Bone bleeding and bone defects arising from trauma or bone tumor resection pose a great threat to patients and they are challenging problems to orthopedic surgeons. Traditional hemostatic materials are not suitable for bone fractures where compression cannot be applied, neither are they effective during surgeries where large amounts of body fluids prevent them from adhering to the large and irregular bone wound sites. This research introduces a catechol‐conjugated chitosan (CHI‐C) multi‐functional hydrogel with adhesion, self‐healing, cytocompatibility, hemocompatibility, and blood cell coagulation capacity. The hydrogel can be injected into internal and irregular bleeding sites and bone defective areas, and then rapidly self‐heals (within 2 min) to an integrated hydrogel that fully fills the defective sites and strongly sticks to bleeding areas in the presence of body fluids during surgery. In vivo experiments using a rabbit ilium bone defect model demonstrate quick hemostasis after the hydrogel is applied and the blood loss is only ¼ compared to the untreated injuries. In addition, the bone regeneration is not interfered by the hydrogel and the bone defect is no longer visible with disappearance of the hydrogel after 4 weeks. This multi‐functional hydrogel is potentially valuable for clinical applications towards tissue adhesion, hemostasis, and bone regeneration.
