Abstract:Kawasaki disease (KD), which is also known as cutaneous mucosal lymph node syndrome, is an acute, self-limiting, necrotizing vasculitis with unclear cause that primarily affects small- and medium-sized blood vessels and most commonly affects children aged 6 months to 5 years. Currently, diagnosis is based primarily on typical clinical symptoms. Approximately 15%-20% of patients are highly suspected of having KD; however, they do not match the diagnostic criteria for typical KD, which is referred to as incomplete Kawasaki disease (IKD), and this has become a major challenge in the diagnosis and treatment of KD. We describe a case of a 7-year-old boy who had a fever and jaundice as his initial symptoms. After a series of clinical laboratory and imaging examinations and marked improvement of symptoms after treatment with intravenous immunoglobulin (IVIG), IKD was considered as the diagnosis. When children present with jaundice and fever, physicians should consider KD as a possible diagnosis to ensure early detection and treatment of the disease.
Clinical Features of Kawasaki Disease in Children with an Atypical Age of Onset

Abstract:To evaluate the frequency of patients younger than six months and older than five years in a large cohort of patients with Kawasaki disease (KD), and to determine the clinical characteristics and outcomes of patients with an age of onset outside the typical age distribution. We performed retrospective chart review of KD patients treated at our hospital during a six-year period. Data of patients were record from discharge database. Patients were divided into three groups according to age of onset of illness. The typical age group (6 months to 5 years) was used as a reference, and the clinical features of KD patients <= 6 months old and >5 years of age were analysed. A total of 2,318 cases were enrolled in this study population consisting of 1,432 boys and 886 girls. Cases <= 6 months of age and >5 years of age accounted for 14.7% (340) and 14.0% (325) of the overall sample. Compared with the typical age group, the younger age group had longer of hospital stay, higher proportion of incomplete KD (iKD) cases, fewer illness days at admission, and fewer number of major criteria. Compared with the typical age group, the older age group had longer illness days at admission, longer total fever duration, and higher rate of delayed diagnosis. There were no significant difference between two groups in term of the rates of iKD, intravenous immunoglobin resistance, and coronary artey lesion. Patients with KD <= 6 months of age tend to manifests incomplete symptoms with more pronounced systemic inflammation, while increased awareness contributed to improvement of diagnoses and outcomes. Patients with KD >5 years of age have relatively low levels of systemic inflammation without worse treatment and outcome, but were prone to delayed diagnoses.

Abstract:In recent years,the understanding of KD has been deepened.Children with complete KD were timely diagnosed,but there was still a challenge for patients with incomplete KD.Full recognition of clinical features of incomplete KD would enable doctors to avoid delayed diagnosis and treatment,and reduce the incidence of coronary artery complications.Patients with 4 or fewer main criteria were considered with incomplete KD,and those with insufficient criteria and other atypical organ involved were considered with atypical KD.The main criteria for KD were in a clinically dynamic evolution,and some patients with incomplete KD present new main criteria and eventually meet the diagnostic criteria for complete KD.Early recognition of clinical features of patients with potential turning into complete KD was useful to improve the prognosis of children with KD.

Abstract:Objective To investigate the clinical characteristics and serum cytokine levels in children with Kawasaki disease shock syndrome (KDSS).Methods Seventeen KDSS children and 43 age-and gender-matched patients with Kawasaki disease (KD) admitted during January 2014 to December 2016 were included in this study.Serum levels of interleukin-2(IL-2),IL-4,IL-6,IL-10,tumor necrosis factor-α (TNF-α),and interferon gamma (IFN-γ) were measured by flow cytometry in two groups.The clinical characteristics,serum cytokine levels and routine laboratory parameters,the liver,kidney and blood coagulation functions and disease outcomes were compared between two groups.Results Compared with KD group,the duration of fever and length of hospital stay in KDSS group were significantly prolonged,the WBC,neutrophil,ESR and serum CRP,creatinine,urea nitrogen,D-dimer,IL-6,IL-10,TNF-α IFN-γ.levels were elevated,the incidence of coronary artery lesions,intravenous immunoglobulin (IVIG) resistance,admission to ICU and other organ damage were significantly increased (all P＜0.05),while blood hemoglobin,albumin,sodium and potassium concentrations were significantly decreased (all P＜0.05).No deaths were found in two groups.Conclusion Increased levels of IL-6,IL-10,TNF-oα,IFN-γ and IVIG resistance leading to persistent inflammation may be involved in the development of KDSS.The duration of fever and hospital stay are prolonged,the conditions are more serious and more susceptible to coronary and other organ damage in patients with KDSS.
[Clinical Features of Recurrent Kawasaki Disease in 20 Children].

Abstract:OBJECTIVE:To investigate the clinical features of recurrent Kawasaki disease (KD) and the relationship of recurrent KD with coronary artery lesions.METHODS:The medical data of 20 children with recurrent KD who were admitted to the Children's Hospital from January 1998 to May 2007 were retrospectively studied. Their clinical features were compared with those of children with initial KD.RESULTS:The incidence of recurrent KD was 1.34% (20/1489). KD relapsed 2 months to 4.6 years (average: 1.2 years) after the first episode in the 20 children. Compared with the initial KD group, the clinical symptoms in the recurrent KD group were incomplete, complicated and less severe. The period of fever, platelet count, C-reactive protein and ESR were remarkably reduced in the recurrent KD group, but the incidence of coronary artery lesions increased significantly compared with the initial KD group (40% vs 25%; P<0.05).CONCLUSIONS:The clinical symptoms of recurrent KD were incomplete in children. Recurrent KD was associated with an increased incidence of coronary artery lesions.

Abstract:OBJECTIVE:The study was designed to investigate the changes in CD(69), CD(25) and HLA-DR expressions in peripheral blood T cell in Kawasaki disease (KD). METHODS:The authors detected CD(69), CD(25) and HLA-DR expressions in peripheral blood T cell by using flow cytometry. The patients who met the diagnostic criteria for KD comprised sixteen boys and fifteen girls (4 - 60 months of age; mean, 26 +/- 18 months). All received intravenous gammaglobulin at a dose of 1 g/(kg.d), for 2 days and oral aspirin at a dose of 30 - 50 mg/(kg.d). In case of persistent fever, a repeated dose of intravenous gammaglobulin or I.V. methylprednisolone at a dose of 20 mg/(kg.d) for three daily doses was attempted. The authors tested blood samples from 17 healthy controls consisting of nine boys and eight girls (3 - 84 months of age; mean, 25 +/- 18 months) and the samples from 31 patients. RESULTS:The percentage of peripheral blood CD(3)(+) T lymphocyte was (54.4 +/- 9.0)% in acute stage of KD and (65.0 +/- 7.0)% in healthy controls. There was a significant difference between the two groups (P < 0.001). The values of CD(69)(+) [(11.2 +/- 12.6)%, vs. (0.6 +/- 0.4)%], CD(25)(+) [(9.2 +/- 3.5)% vs. (3.9 +/- 1.8)%] and HLA-DR(+) [(8.3 +/- 5.0)% vs. (4.3 +/- 2.3)%] in KD patients were markedly increased compared to those of the healthy controls. After intravenous gammaglobulin treatment, the percentage of CD(3)(+)CD(69)(+) and CD(3)(+)CD(25)(+) significantly decreased [CD(3)(+)CD(69)(+): (14.0 +/- 13.0)% vs. (1.6 +/- 1.2)%, P < 0.05; CD(3)(+)CD(25)(+): (7.8 +/- 4.1)% vs. (2.0 +/- 0.6)%, P < 0.01]. However, the CD(3)(+) T lymphocytes increased [(50.8 +/- 5.0)% vs. (64.9 +/- 5.5)%, P < 0.01]. There was no significant difference in expression of CD(3)(+) T lymphocyte cell activating markers between coronary artery disease group and normal coronary artery group in KD cases (P > 0.05). CONCLUSION:CD(3)(+) T cell activation in the early and middle stages is involved in the mechanism responsible for cardiovascular injury.

Abstract:Background Cardiovascular damages poses risks to children with Kawasaki disease (KD). Although hypertriglyceridemia and hypercholesteremia are risk factors of cardiovascular damages, studies on the blood lipid metabolism in KD are still limited. This study aims to analyze the blood lipids and coagulation in KD. Methods Triglyceride (TG) and cholesterol levels in the plasma and serum from 20 children with KD were examined in comparison with 10 healthy children (HC) as well as 10 children with high fever from identified bacterial infections (BT). Using electrospray ionization mass spectrometry, we profiled the lipid species. Blood coagulation was analyzed. Statistics was analyzed by one-way ANOVA using SigmaStat. Results We found that in KD, plasma TG level was significantly increased, but not serum TG. A total of 19 molecular species of TG were identified, and they were all increased in KD and BT patients, and more pronounced in KD. On the other hand, major molecular species of plasma phosphotidylcholine and lyso-phosphotidylcholine were decreased in KD and BT. Pronounced hypercoagulation was found in KD blood. Conclusion Our data indicate hyperlipidemia in KD, especially for TG, which contributes to the hypercoagulation and the potential risk of cardiovascular damages. Evaluation of blood lipid levels in severe KD patients could provide valuable information for treatment and prognosis, thus would be worthy of consideration.
Appendicitis as a Leading Manifestation of Kawasaki Disease in Older Children

Abstract:Kawasaki disease (KD) is a systematic inflammatory disease with multiple organ involvement. Timely diagnosis and prompt management are essential for successful treatment. KD, with an atypical presentation, remains a diagnostic challenge for physicians. We report a five-year-old boy who presented with appendicitis. An appendectomy was performed; however, his fever persisted. The boy was diagnosed with KD and intravenous immunoglobulin was administered. His symptoms resolved, and he had an uneventful recovery. Furthermore, we performed a literature review with 13 cases identified in the literature. Most cases were male, and the average age was older than typical for KD. In conclusion, KD may present with abdominal complaints and appendicitis may be a rare initial presentation of KD. Multidisciplinary cooperation and high awareness are warranted for timely diagnosis, especially in older children experiencing persistent fever after an appendectomy.

Abstract:Background: Kawasaki disease (KD) is a childhood acute febrile vasculitis of unknown aetiology. The diagnosis is based on clinical criteria, including unilateral cervical lymphadenopathy, which is the only presenting symptom associated with fever in 12% of cases. A prompt differential diagnosis distinguishing KD from infective lymphadenitis is therefore necessary to avoid incorrect and delayed diagnosis and the risk of cardiovascular sequelae. Case presentation: We describe the case of a 4 years old boy presenting with febrile right cervical lymphadenopathy, in which the unresponsiveness to broad-spectrum antibiotics, the following onset of other characteristic clinical features and the evidence on the magnetic resonance imaging (MRI) of retropharyngeal inflammation led to the diagnosis of incomplete and atypical KD. On day 8 of hospitalisation (i.e., 13 days after the onset of symptoms), one dose of intravenous immunoglobulins (IVIG; 2 g/kg) was administered with rapid defervescence, and acetylsalicylic acid (4 mg/kg/day) was started and continued at home for a total of 8 weeks. Laboratory examinations revealed a reduction in the white blood cell count and the levels of inflammatory markers, thrombocytosis, and persistently negative echocardiography. Clinically, we observed a gradual reduction of the right-side neck swelling. Fifteen days after discharge, the MRI of the neck showed a regression of the laterocervical lymphadenopathy and a resolution of the infiltration of the parapharyngeal and retropharyngeal spaces. Conclusion: Head and neck manifestations can be early presentations of KD, which is frequently misdiagnosed as suppurative lymphadenitis or retropharyngeal infection. A growing awareness of the several possible presentations of KD is therefore necessary. Computed tomography (CT) or MRI can be utilised to facilitate the diagnosis.
Clinical Features and Trend Analysis of Kawasaki Disease from 2007 to 2016: a Single-Center Study

Abstract:Objective To analyze the clinical features of children with Kawasaki disease(KD) in order to find out the trend of its clinical features and to provide guidance for clinical diagnosis and treatment.Methods Children with KD hospitalized in the Pediatric Ward of Peking University Third Hospital from January 2007 to December 2016 were collected and analyzed for their characteristics of onset,clinical features and treatment.The children were divided into 2 groups according to the time:2007 to 2011 group,2012 to 2016 group.The changes in clinical characteristics between the 2 stages were analyzed.Results A total of 337 children with KD were enrolled in this study,including 212 males and 125 females,with a ratio of 1.69 ∶ 1.00.The age of onset was from 2 months to 12 years old.The median age was 24 months.A total of 325(96.4％) children were under 6 years of age.There were 114(33.8％) cases of KD children from 2007 to 2011,among whom incomplete KD (IKD),intravenous gamma globulin non-response and coronary artery injury cases accounted for 23.7％ (27 cases),4.5％ (5 cases),38.6％ (44 cases),respectively;there were 223 (66.2％) cases of KD children from 2012 to 2016,among whom IKD,intravenous gamma globulin non-response and coronary artery injury cases accounted for 38.6％ (86 cases),11.9％ (26 cases),31.4％ (70 cases),respectively.The group of 2012 to 2016 was compared with the the group of 2007 to 2011 and it was found that the proportion of IKD and intravenous gamma globulin non-response cases increased,and the differences were significant (x2 =7.495,4.654,P =0.006,0.031),but the proportion of coronary artery injury cases decreased with no statistical difference (x2 =1.750,P =0.186).Conclusion With the incidence of KD increasing,the proportion of incomplete KD and intravenous gamma globulin non-response KD also showed an upward trend,which should be paid more attention to by clinicians in order to promptly diagnose and implement more targeted treatment.
An Infant With Kawasaki Disease: A Case Report and Literature Review

Abstract:Kawasaki disease (KD) is the leading cause of acquired heart disease in children in developed countries. Delayed treatment can lead to coronary artery (CA) abnormalities, potentially causing myocardial ischemia, infarction, and death. Younger age is a risk factor for developing bilateral large CA aneurysms in KD patients. A one-and-a-half-month-old infant presented with fever and elevated inflammatory markers. Post-admission ceftriaxone injections were ineffective. Subsequently, the patient experienced recurrent high fevers, accompanied by rashes, erythema, and induration of the palms and soles, erythema, swelling at the Bacillus Calmette-Guerin (BCG) scar site, cracked lips, and conjunctival hyperemia, all of which were indicative of KD. Intravenous immunoglobulin (IVIG) and aspirin were administered on the third day of fever. Follow-ups at one, three, six, and 12 months post discharge revealed normal findings. This case demonstrates that even very young infants can develop complete KD, and early treatment can prevent CA complications.

Abstract:Key clinical message: Kawasaki disease (KD), a self-limiting vasculitis, can present with a broader spectrum of vascular involvements, necessitating early recognition and prompt management. This case exemplifies the importance of involving multiple teams on board in managing complex presentations of KD. It also highlights the importance of close monitoring for the progression of the disease spectrum as well as family education to ensure favorable outcomes. The case also emphasizes the importance of long-term follow-up and further research to understand the prognosis, early screening tools, and possible complications due to multi-organ involvement in KD along with their management strategies. Abstract: Kawasaki disease (KD) is a multisystem vascular inflammatory syndrome, which predominantly affects the small and medium vessels in children within the age group of less than 5 years. The most threatened complication is the development of coronary artery aneurysms (CAAs). We present an extremely rare case of KD in a 2-month, 21-day-old male infant with extensive vascular involvement, expanding the disease spectrum beyond the involvement of coronary arteries. These included aneurysmal dilatations of both internal carotid arteries, the descending aorta, bilateral multilevel intercostal arteries, coeliac artery, superior mesenteric artery, and both renal arteries. Implementing a multidisciplinary approach with early treatment via intravenous immunoglobulin (IVIG) and dexamethasone proved to be most effective in the patient's management. Despite unique challenges such as severe coronary dilation and pseudomonas sepsis during the special care, the patient was stabilized and discharged after 11 days of hospital stay, highlighting the importance of early prompt management and a centered approach to evaluate in a broader spectrum. This case emphasizes the importance of long-term follow-up and further research to understand the prognosis, early screening tools, and possible complications due to multi-organ involvement in KD along with their management strategies.
Kawasaki disease in children: a retrospective cross-sectional study

Abstract:Introduction: Kawasaki disease (KD) is a systemic vasculitis, seen mostly in children. Epidemiology of KD is dependent on geographical location and seasonality. Although many years have passed since the first report of KD, multiple related factors are still unknown. Material and methods: We investigated the clinical, paraclinical, and therapeutic aspects of KD in Kerman, Iran by performing a retrospective, descriptive, cross-sectional study on all children hospitalized due to KD between 2007 and 2020. Results: A total of 340 patients with mean ±SD age of 29.83 ±22.55 months participated in the study. Most of our patients were two to five years old. The male : female ratio was ~ 1.4 : 1. A few of our patients had a family history of KD or vasculitis (0.3%, 1.7%). Typical KD was more common by a large margin (316 patients with typical KD). More than half of our patients had a duration of hospitalization of under ten days. All of our patients were febrile. Hand/foot and lip/mouth changes were the second and third most common clinical findings in more than 60% of our patients. Other manifestations were conjunctivitis in 40%, skin rashes in 34.8%, gastrointestinal manifestations in 33.9%, and lymphadenopathy in 25.3%. Echocardiography revealed abnormalities in 78.6% of the participants; coronary artery aneurysm (CAA) was the most frequent (22.5%) and follow-up echocardiography revealed that all of them regressed within 6 months after treatment. The two laboratory tests with the highest ratio of abnormality were erythrocyte sedimentation rate (95%) and hemoglobin (83.3%). C-reactive protein and liver function tests were also abnormal in most patients. All of our patients received intravenous immunoglobulin and acetylsalicylic acid. Conclusions: Kawasaki disease must be considered in every febrile child, especially those with risk factors, because timely diagnosis and treatment are essential to prevent complications. Health policies should focus on appropriate diagnosis and treatment to prevent the occurrence of sequelae.

Abstract:Purpose: Discussing the clinical features and experiences of diagnosis and treatment of Kawasaki disease with Mycoplasma pneumoniae infection in children;Methods:A retrospective analysis of children suffering from Kawasaki disease associated with Mycoplasma pneumoniae infection;Results: 14 cases had fever symptoms,hyperemia of conjunctiva bulbar in 13 cases;tetter in 8 cases;Alterations of oral mucosa in 14 cases;Foot rigidly edema,means(toe) peeling in 14 cases;Cervical lymph nodes in 12 cases;BCG mark,flushing of anal mucosa in 5 cases.Increased leukocyte,blood CRP hypertension 9 cases,Cardiac ultrasound and coronary artery lesions(CAL) in 2 cases.All of the children were treated by Azithromycin Sequential,early high-dose intravenous immunoglobulin(IVIG) aspirin,and dipyridamole,13 cases were cured,the other 1 case had no IVIG reaction,IVIG again to be recovered.Conclusions: Mycoplasma pneumoniae may be involved in the pathogenesis of Kawasaki disease.

Abstract:Kawasaki disease (KD) is an acute inflammatory disorder that primarily affects children and can lead to coronary artery lesions (CAL) if not diagnosed and treated promptly. The original clinical criteria for diagnosing KD were reported by Dr. Tomisaku Kawasaki in 1967 and have been used for decades. However, research since then has highlighted the limitations of relying solely on these criteria, as they might lead to underdiagnosis or delayed diagnosis, potentially increasing the risk of coronary artery complications. This review appears to discuss several important aspects related to KD diagnosis and management. The current diagnostic methods for KD might need updates, especially considering cases that do not fit the typical clinical criteria. Recognizing diagnostic pitfalls and distinguishing KD from other conditions that might have similar clinical presentations is essential. The differences and similarities between KD and Multisystem Inflammatory Syndrome in Children (MIS-C), another inflammatory condition that has been associated with COVID-19, were also reviewed. The review explores the potential role of eosinophil count, new biomarkers, microRNA panels, and scoring systems in aiding the diagnosis of KD. Overall, the review article provides a comprehensive overview of the evolving landscape of KD diagnosis and management, incorporating new diagnostic methods, biomarkers, and treatment approaches to improve patient outcomes and reduce the risk of complications.

Abstract:Intravenous immunoglobulin(IVIG)therapy together with aspirin is effective in Kawasaki disease(KD).However,some of KD patients have persistent or recurrent fever after IVIG treatment;further more,these patients are at higher risk of developing coronary artery abnormalities.But pathogenesis of intravenous immunoglobulin refractory KD is still unclear.Many risk factors are associated with intravenous immunoglobulin-refractory KD,such as small age of patients,days of illness at initial treatment ≤4,increase of white blood cell,AST and CRP,high percentage of neutrophil and decrease of plasma albumin and platelet.Treatments of intravenous immunoglobulin-refractory KD patients are still in dispute,but additional IVIG is well accepted.Other treatments are also in further research,such as corticosteroids,infli-ximab and plasma exchange.The domestic and foreign research progress about incidence,pathogenesis,risk factors and treatment of intravenous immunoglobulin-refractory KD are summarized in this article.
Typical Kawasaki Disease Presenting With Pancreatitis and Bilateral Parotid Gland Involvement: A Case Report and Literature Review

Abstract:We describe the case of a 3-year child in which pancreatic and parotid gland involvement preceded the development of the classical clinical phenotype of a typical Kawasaki disease (KD). The child was referred to the Emergency Department with a story of 3 days of continuous fever associated with abdominal pain and bilaterally swelling in the parotid regions; laboratory evaluation identified markedly increased levels of total amylase, pancreatic amylase, lipase, and transaminase, and diagnosis of pancreatitis was posed. After 9 days of fever and persistence of the clinical features, the classical signs of KD appeared, and the child was treated with intravenous immunoglobulins (IVIG), showing a dramatic response with complete resolution of the clinical picture. In this work, we reviewed the literature about gastrointestinal (GI) symptoms in KD, focusing on pancreatic and hepatic involvement. This analysis highlighted that, in case of fever associated with pancreatic inflammation, KD must be considered in the spectrum of differential diagnosis, and that GI involvement in KD is frequently associated with an incomplete response to IVIG treatment.

Abstract:ImportanceKawasaki disease shock syndrome (KDSS) is a rare Kawasaki disease (KD) manifestation. The pediatricians are not aware of the full range of clinical characteristics of KDSS. ObjectiveWe aimed to investigate the clinical features, diagnosis and treatment of KDSS in two patients and we included a literature review. MethodsWe collected and analyzed the clinical data for two patients with KDSS. Additionally, using "Kawasaki diseases shock syndrome" as a key phrase, we searched PubMed, Biotechnology Information and Wanfang Data Knowledge Service Platform databases for any similar reports between January 2009 and March 2017. ResultsBoth of our patients diagnosed with KD developed sustained hypotension during the course of intravenous immunoglobulin treatment, as well as hypoalbuminemia, and increased C-reactive protein and brain natriuretic peptide levels during hypotension. Both patients responded well to fluid resuscitation and inotropic support. No aneurysms formed in either patient during follow-up. We reviewed two related studies in Chinese and 11 studies in English. InterpretationKD may present with severe shock, and requires proper diagnosis and rapid treatment. The prognosis for most patients with KDSS is excellent.
An atypical case of Kawasaki disease with severe pneumonia in a neonate

Abstract:Abstract Background Kawasaki disease (KD) is an acute, febrile, systemic vasculitis of unknown etiology that primarily affects the coronary arteries and generally occurs at around 1 year of age. Although the diagnosis of KD is generally not difficult, it is challenging in cases of incomplete KD lacking characteristic clinical manifestations. The incidence of incomplete KD is higher in infants younger than 6 months of age. Pneumonia is an extremely rare complication of KD and can be misinterpreted as atypical pneumonia rather than KD. Herein, we report a neonate with atypical KD and severe pneumonia who required mechanical ventilation. Case presentation Japanese one-month-old infant had only fever and rash on admission (day 1), and he was transferred to the intensive care unit for severe pneumonia on day 2. Although pneumonia improved following intensive care, he was diagnosed with KD on day 14 because of emerging typical clinical manifestations such as fever, bulbar nonexudative conjunctival injection, desquamation of the fingers, and coronary artery aneurysm. KD symptoms improved after three doses of intravenous immunoglobulin plus cyclosporine. However, small coronary aneurysms were present at the time of discharge. In a retrospective analysis, no pathogens were detected by multiplex real-time PCR in samples collected at admission, and the serum cytokine profile demonstrated prominent elevation of IL-6 as well as elevation of neopterin, sTNF-RI, and sTNF-RII, which suggested KD. Conclusions The patient’s entire clinical course, including the severe pneumonia, was caused by KD. As in this case, neonatal KD may exhibit atypical manifestations such as severe pneumonia requiring mechanical ventilation.
